An 8-week feeding trial to investigate the inclusion of ensiled cassava root-leaf blends (ECRLB) fortified with no additive (ECRLB1), Allzyme® SSF (ECRLB2), amino acids (ECRLB3) and Allzyme + amino acids (ECRLB4) at 30% replacement value for maize in diets of gilts was conducted. Maize based diet (MBD) served as control. Twenty gilts (3-4 months old) were randomly allotted to dietary treatments in a completely randomized design with 4 pigs per treatment. Growth performance, haematological indices and serum chemistry were measured. Improved (P<0.05) weight gain was recorded for gilts fed ECRLB3 (433g d
-1 ) and ECRLB4 (419g d-1 ) than others. Feed to gain ratio of fortified ECRLB group were similar to MBD but better (P<0.05) than ECRLB1. The feed costs (₦155.34- ₦162.17) kg-1 weight gain of gilts fed fortified ECRLB were lower (P<0.05) than MBD (₦176.74). Haematological indices and serum chemistry values for gilts fed MBD and fortified ECRLB fell within the recommended range. It was concluded that fortified ECRLB could be used to replace maize in the diets of gilts for optimal performance. [ABSTRACT FROM AUTHOR]- Published

13. A Comparison of Three Different Regimens of Infectious Bursal Disease Vaccination in Chickens.

- Abstract
Five days old progeny chicks from breeders which have received primary and booster doses of live infectious bursal disease vaccine, demonstrated precipitating antibodies unlike progenies from single dose vaccinated breeders. All chicks from the two different groups of breeders were however seronegative at 22 days of age, despite vaccination at 7 or 14 days of age. Post vaccination seroconversion was first noticeable at 32 days in the group of chicks vaccinated at 7 days. Post challenge mortalities was significantly lower (P < 0.05) and organ lesions very significantly minimized (P < 0.01) in 7 day old vaccinated group than in 14 or 28 days old vaccinated chicks. These results showed that early (7 days) IBD Vaccination was superior to vaccination at 14 or 28 days, in terms of antibody response and protectivity against mortalities and bursal lesions. Zusammenfassung Eine vergleichende Untersuchung zur Wirksamkeit von drei verschiedenen Vakzinationsverfahren gegen Gumborokrankheit bei Hühnern Im Gegensatz zu Küken, die eine einzelne Dosis lebender ansteckender Gumborokrankheit-Vakzine erhalten haben, wiesen Küken im Alter von 5 Tagen, die zweimal geimpft wurden, prezipitierende Antikörper auf. 22 Tage nach der Geburt waren jedoch alle Küken von den zwei Zuchtgruppen seronegativ, trotz einer Impfung im Alter von 7 oder 14 Tagen. 32 Tage nach der Geburt konnte bei der Gruppe, die im Alter von 7 Tagen geimpft wurde, eine nach der Impfung stattfindende Serkonversion festgestellt werden. Im Vergleich zu den Gruppen, die 14 bzw. 28 Tage nach der Geburt geimpft wurden, war die Anzahl von Todesfällen (P < 0,05) und Organläsionen (P < 0,01) bei dieser Gruppe erheblich verringert. Diese Ergebnisse zeigen, daß eine rechtzeitige Impfung gegen infektiöse ansteckende Gumborokrankheit (IBD), d. h. 7 Tage nach der Geburt, einen wirksameren Schutz gegen Tod und bursale Läsionen sowie eine bessere Antikörperantwort erzielt als eine Impfung im Alter von 14 bzw. 28 Tagen. 14. The Status of Guinea Fowls ( Numida meleagris) in the Epidemiology of Infectious Bursal Disease (IBD) of Poultry in Nigeria.

- Abstract
A serological survey for IBD in market guinea fowls gave a total prevalence rate of 44.3 % from five different locations in Nigeria. Guinea fowl keets were susceptible to experimental IBD infection and transmitted it to in-contact sentinel chickens. The infected guinea fowl keets showed the typical clinico-pathology and seroconversion for IBD. These results suggest that guinea fowls could play an active part in the epidemiology of IBD. Zusammenfassung Die Rolle von Perlhühnern ( Numida meleagris) in der Epidemiologie der Infektiösen Bursitis (IBD) des Geflügels in Nigeria Ein Überblick über die Serologie von IBD bei Perlhühnern zeigt insgesamt eine Verbreitung von 44,3 % in 5 verschiedenen Gebieten in Nigeria. Perlhuhnküken waren empfänglich für experimentelle Infektion mit IBD und übertrugen diese durch Kontakt auf Testküken. Die infizierten Perlhuhnküken zeigten für IBD typischen klinisch-pathologischen Veränderungen und Serokonversionen. Diese Ergebnisse lassen vermuten, daß Perlhühner eine aktive Rolle in der Epidemiologie von IBD spielen. [ABSTRACT FROM AUTHOR]

- Abstract
Newcastle disease and egg drop syndrome '76 were diagnosed in a flock of grey breasted helmet guinea fowls in Ibadan. The Newcastle disease was characterised by anorexia, drooping wings pneumonia and mortalities among the affected keets. Egg drop syndrome '76 was characterised by sudden drop in egg production and laying of shell-less eggs. Serum samples obtained from affected guinea fowls had high levels of antibodies to egg drop syndrome '76 virus. Zusammenfassung Die atypische Geflügelpest (Newcastle Disease) und das 'Egg drop'-Syndrom (EDS '76) wurden in einer Herde Haubenperlhühner in Ibadan (Nigeria) diagnostiziert. Die atypische Geflügelpest war durch Appetitverlust, hängende Flügel, Pneumonie und Todesfälle unter den betroffenen Vögeln gekennzeichnet. Das 'Egg Drop'-Syndrom '76 war charakterisiert durch einen plötzlichen Abfall der Legeleistung und durch das Legen von schalenlosen Eiern. Serumproben von betroffenen Perlhühnern wiesen hohe Titer an Antikörpern gegen EDS '76-Virus auf. [ABSTRACT FROM AUTHOR]

Funding: Bill & Melinda Gates Foundation. Background: The global burden of lower respiratory infections (LRIs) and corresponding risk factors in children older than 5 years and adults has not been studied as comprehensively as it has been in children younger than 5 years. We assessed the burden and trends of LRIs and risk factors across all age groups by sex, for 204 countries and territories. Methods: In this analysis of data for the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, we used clinician-diagnosed pneumonia or bronchiolitis as our case definition for LRIs. We included International Classification of Diseases 9th edition codes 079.6, 466–469, 470.0, 480–482.8, 483.0–483.9, 484.1–484.2, 484.6–484.7, and 487–489 and International Classification of Diseases 10th edition codes A48.1, A70, B97.4–B97.6, J09–J15.8, J16–J16.9, J20–J21.9, J91.0, P23.0–P23.4, and U04–U04.9. We used the Cause of Death Ensemble modelling strategy to analyse 23 109 site-years of vital registration data, 825 site-years of sample vital registration data, 1766 site-years of verbal autopsy data, and 681 site-years of mortality surveillance data. We used DisMod-MR 2.1, a Bayesian meta-regression tool, to analyse age–sex-specific incidence and prevalence data identified via systematic reviews of the literature, population-based survey data, and claims and inpatient data. Additionally, we estimated age–sex-specific LRI mortality that is attributable to the independent effects of 14 risk factors. Findings: Globally, in 2019, we estimated that there were 257 million (95% uncertainty interval [UI] 240–275) LRI incident episodes in males and 232 million (217–248) in females. In the same year, LRIs accounted for 1·30 million (95% UI 1·18–1·42) male deaths and 1·20 million (1·07–1·33) female deaths. Age-standardised incidence and mortality rates were 1·17 times (95% UI 1·16–1·18) and 1·31 times (95% UI 1·23–1·41) greater in males than in females in 2019. Between 1990 and 2019, LRI incidence and mortality rates declined at different rates across age groups and an increase in LRI episodes and deaths was estimated among all adult age groups, with males aged 70 years and older having the highest increase in LRI episodes (126·0% [95% UI 121·4–131·1]) and deaths (100·0% [83·4–115·9]). During the same period, LRI episodes and deaths in children younger than 15 years were estimated to have decreased, and the greatest decline was observed for LRI deaths in males younger than 5 years (–70·7% [–77·2 to –61·8]). The leading risk factors for LRI mortality varied across age groups and sex. More than half of global LRI deaths in children younger than 5 years were attributable to child wasting (population attributable fraction [PAF] 53·0% [95% UI 37·7–61·8] in males and 56·4% [40·7–65·1] in females), and more than a quarter of LRI deaths among those aged 5–14 years were attributable to household air pollution (PAF 26·0% [95% UI 16·6–35·5] for males and PAF 25·8% [16·3–35·4] for females). PAFs of male LRI deaths attributed to smoking were 20·4% (95% UI 15·4–25·2) in those aged 15–49 years, 30·5% (24·1–36·9) in those aged 50–69 years, and 21·9% (16·8–27·3) in those aged 70 years and older. PAFs of female LRI deaths attributed to household air pollution were 21·1% (95% UI 14·5–27·9) in those aged 15–49 years and 18·2% (12·5–24·5) in those aged 50–69 years. For females aged 70 years and older, the leading risk factor, ambient particulate matter, was responsible for 11·7% (95% UI 8·2–15·8) of LRI deaths. Interpretation: The patterns and progress in reducing the burden of LRIs and key risk factors for mortality varied across age groups and sexes. The progress seen in children younger than 5 years was clearly a result of targeted interventions, such as vaccination and reduction of exposure to risk factors. Similar interventions for other age groups could contribute to the achievement of multiple Sustainable Development Goals targets, including promoting wellbeing at all ages and reducing health inequalities. Interventions, including addressing risk factors such as child wasting, smoking, ambient particulate matter pollution, and household air pollution, would prevent deaths and reduce health disparities. Background: Understanding the magnitude of cancer burden attributable to potentially modifiable risk factors is crucial for development of effective prevention and mitigation strategies. We analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019 to inform cancer control planning efforts globally. Methods: The GBD 2019 comparative risk assessment framework was used to estimate cancer burden attributable to behavioural, environmental and occupational, and metabolic risk factors. A total of 82 risk-outcome pairs were included on the basis of the World Cancer Research Fund criteria. Estimated cancer deaths and disability-adjusted life-years (DALYs) in 2019 and change in these measures between 2010 and 2019 are presented. Findings: Globally, in 2019, the risk factors included in this analysis accounted for 4·45 million (95% uncertainty interval 4·01-4·94) deaths and 105 million (95·0-116) DALYs for both sexes combined, representing 44·4% (41·3-48·4) of all cancer deaths and 42·0% (39·1-45·6) of all DALYs. There were 2·88 million (2·60-3·18) risk-attributable cancer deaths in males (50·6% [47·8-54·1] of all male cancer deaths) and 1·58 million (1·36-1·84) risk-attributable cancer deaths in females (36·3% [32·5-41·3] of all female cancer deaths). The leading risk factors at the most detailed level globally for risk-attributable cancer deaths and DALYs in 2019 for both sexes combined were smoking, followed by alcohol use and high BMI. Risk-attributable cancer burden varied by world region and Socio-demographic Index (SDI), with smoking, unsafe sex, and alcohol use being the three leading risk factors for risk-attributable cancer DALYs in low SDI locations in 2019, whereas DALYs in high SDI locations mirrored the top three global risk factor rankings. From 2010 to 2019, global risk-attributable cancer deaths increased by 20·4% (12·6-28·4) and DALYs by 16·8% (8·8-25·0), with the greatest percentage increase in metabolic risks (34·7% [27·9-42·8] and 33·3% [25·8-42·0]). Interpretation: The leading risk factors contributing to global cancer burden in 2019 were behavioural, whereas metabolic risk factors saw the largest increases between 2010 and 2019. Reducing exposure to these modifiable risk factors would decrease cancer mortality and DALY rates worldwide, and policies should be tailored appropriately to local cancer risk factor burden. - Abstract
Background: Paucity of data on state-wide maternal mortality in Nigeria hampers planning, monitoring and evaluation of the impact of interventions. The Confidential Enquiry into Maternal Deaths in Ondo State was initiated to overcome this problem. This study aimed to compare trends of maternal mortality ratios, causes of deaths, geographical distribution and other associated factors in 12-monthly reports of the Confidential Enquiry into Maternal Deaths in Ondo State., Methods: Notification forms were distributed throughout the State to focal persons and medical records officers at community and facility levels, respectively. Maternal deaths, as defined in the International Classification of Diseases 10th version, were recorded prospectively over 3 years from 1st June 2012 to 30th May, 2015. Forms were submitted, collated and data analysed by a multidisciplinary review committee., Results: Reported numbers of maternal deaths (and maternal mortality ratios) were 114 (253 per 100,000 births), 89 (192) and 81 (170), respectively per year, indicating a 33% reduction in maternal mortality ratio over the course of the study period. Assuming that the confidential enquiry process was the only intervention at the time aimed at reducing maternal mortality, simple linear regression with a correlation coefficient of 0.9314, showed a relationship though the difference in the values were not statistically significant (95% CI = - 184.55 to 101.55, p = 0.169). Postpartum haemorrhage and eclampsia were the leading causes of deaths., Conclusion: There was a trend of reduction in maternal mortality ratio during the period of study with postpartum haemorrhage as the major cause of death. The positive association between the confidential enquiry reports and maternal mortality ratios make us recommend that our model be adopted in other states and at the federal level.

- Abstract
Objectives: Outpatient parenteral antimicrobial therapy (OPAT) is increasingly used to treat a wide range of infections. However, there is risk of hospital readmissions. The study aim was to develop a prediction model for the risk of 30-day unplanned hospitalization in patients receiving OPAT., Methods: Using a retrospective cohort design, we retrieved data on 1073 patients who received OPAT over 2 years (January 2015 to January 2017) at a large teaching hospital in Sheffield, UK. We developed a multivariable logistic regression model for 30-day unplanned hospitalization, assessed its discrimination and calibration abilities, and internally them validated using bootstrap resampling., Results: The 30-day unplanned hospitalization rate was 11% (123/1073). The main indication for hospitalization was worsening or nonresponse of infection (52/123, 42%). The final regression model consisted of age (adjusted odds ratio (aOR), 1.18 per decade; 95% confidence interval (CI), 1.04-1.34), Charlson comorbidity score (aOR, 1.11 per unit increase; 95% CI, 1.00-1.23), prior hospitalizations in past 12 months (aOR, 1.30 per admission; 95% CI, 1.17-1.45), concurrent intravenous antimicrobial therapy (aOR, 1.89; 95% CI, 1.03-3.47) and endovascular infection (aOR, 3.51; 95% CI, 1.49-8.28). Mode of OPAT treatment was retained in the model as a confounder. The model had adequate concordance (c-statistic 0.72; 95% CI 0.67-0.77) and calibration (Hosmer-Lemeshow p 0.546; calibration slope 0.99; 95% CI 0.78-1.21), and low degree of optimism (bootstrap optimism corrected c-statistic, 0.70)., Conclusions: We identified a set of six important predictors of unplanned hospitalization based on readily available data. - Abstract
Screening for meticillin-resistant Staphylococcus aureus (MRSA) carriage in critical care units remains a controversial issue. This cross-sectional study reviewed the MRSA screening policies in adult critical care units in Welsh hospitals. Data were collected by structured questionnaires. The study showed a degree of variability in practice from universal screening to no screening at all. It is recommended that local MRSA policies should be reviewed, bearing in mind the local prevalence of colonization and infection; and compliance with basic infection control strategies should not only be directed at MRSA infection, but also at other healthcare-associated infections., (Copyright © 2012 The Healthcare Infection Society. - Abstract
Extra-intestinal manifestations of Clostridium difficile infection are uncommon. Most cases are associated with gastrointestinal disease and often occur as a mixed infection with other gut flora. We report a case of breast abscess following monomicrobial C. difficile bacteraemia in a female with background chronic hepatitis C infection and alcoholic liver disease. No evidence of colitis was found. Our case shows that C. difficile is indeed capable of spreading from the gastrointestinal tract.

- Abstract
Thirty-five 6-week-old guinea fowl keets, seronegative for maternal antibodies to Newcastle disease virus, were infected with Herts strain (33/56) and Kumarov strain of Newcastle disease virus intramucularly (IM) or intranasally (IN). Clinical signs were first noticed four days post infection (PI) in the group infected IM but five days PI in the group infected IN with Herts strain of Newcastle disease virus. These clinical signs were similar in both groups and included anorexia, droopiness, huddling together, greenish diarrhoea and marked cachexia. Prominent nervous signs, including spasms of the head and neck, were observed in groups infected with Herts strain. The major gross lesions observed were emaciation with prominent keel bone, empty intestinal tract and distended gall bladder in most keets. The histological lesions were characterised by meningoencephalitis, necrosis and loss of lymphocytes from splenic and lymphoid aggregates. There was muscular degeneration and necrosis in the gizzard and mild pulmonary congestion and oedema in some keets. Neither gross or microscopic lesions were observed in keets that had received the Kumarov strain.

Serum samples obtained from 107 Polo horses showing clinical signs of viral respiratory disease were tested for precipitating antibodies to adenovirus by agar gel precipitation test and counter-immunoelectrophoresis method. The results obtained demonstrate serological evidence of adenovirus infection in Polo horses in Nigeria. The counter-immunoelectrophoresis method was observed to be about 3 times more sensitive than the agar gel precipitation test with 19.3 vs 64.5%. It could thus be used to screen a large number of serum samples within a short period.

A sero-epidemiological survey of EDS '76 antibodies was carried out in chickens, guinea fowls, ducks and pigeons in various locations in Nigeria with the haemagglutination-inhibition test. Infection rates of 67.43, 79.68, 73.91 and 86.66% were obtained for chickens, guinea fowls, ducks and pigeons respectively. The high prevalence rates observed in these non-chicken species suggest that they should be considered as important in the epidemiology of EDS '76 in poultry in Nigeria. An extensive survey is recommended together with vaccination.

Faecal samples from 96 piglets with acute gastroenteritis and 41 samples from non-diarrhoeic piglets were investigated for porcine rotavirus (PRV) antigens by enzyme-linked immunosorbent assay (ELISA). PRV was detected in 43 of 96 (44.8%) in the diarrhoeic group whilst none of the samples in the non-diarrhoeic group was positive for PRV (P less than 0.01). Age distribution of infection showed a higher infection rate of 30.2% in piglets aged 1-3 weeks, which was significantly higher (P less than 0.05) than in piglets aged 4-6 weeks, where the infection rate was 14.6 per cent. No PRV antigen was detected in diarrhoeic piglets above 6 weeks of age. Titration of 10 randomly selected ELISA-positive samples showed titres ranging from 512 to greater than or equal to 4.096.

- Abstract
The occurrence of infectious bursal disease (IBD) in industrial poultry flocks in Cameroon was investigated by serological and virological techniques. Antibody to IBD virus was detected in 118 (33.9%) out of 348 serum samples collected in seven randomly selected areas. On the other hand, virological examination was performed on bursa of Fabricius samples obtained post-mortem from flocks in which there was a high mortality among young birds. This virological examination revealed the presence of IBD virus antigen. These observations confirm the occurrence of infectious bursal disease in Cameronian industrial poultry flocks.

- Abstract
A serological survey for IBD in market guinea fowls gave a total prevalence rate of 44.3% from five different locations in Nigeria. Guinea fowl keets were susceptible to experimental IBD infection and transmitted it to in-contact sentinel chickens. The infected guinea fowl keets showed the typical clinical-pathology and seroconversion for IBD. These results suggest that guinea fowls could play an active part in the epidemiology of IBD.

The changes in plasma volume, blood volume, erythrocyte indices, leucocyte indices and serum biochemical values during experimental pestes des petits ruminants (PPR) virus infection of West African dwarf goats (Fouta djallon) were investigated. There was a progressive decline of the plasma volume and serum sodium and potassium values in all the infected goats as from day 5 post-infection and this became quite marked between days 10 and 23 post-infection. The fall was severest in those animals which had diarrhoea. There were no significant changes in the chloride, bicarbonate, phosphate, urea, total protein and albumin values of the goats during the course of the diseases. Haemoconcentration and apparent polycythaemia in the early and terminal stages of the disease were associated with severe dehydration which characterized the infection. There was initial neutrophilic leucocytosis during the phase of fever followed by marked lymphopaenic leucopaenia which progressed terminally in most of the infected goats. The destructive effect of the virus on lymphocytes is suggested as a cause of lymphopaenia.

Five days old progeny chicks from breeders which have received primary and booster doses of live infectious bursal disease vaccine, demonstrated precipitating antibodies unlike progenies from single dose vaccinated breeders. All chicks from the two different groups of breeders were however seronegative at 22 days of age, despite vaccination at 7 or 14 days of age. Post vaccination seroconversion was first noticeable at 32 days in the group of chicks vaccinated at 7 days. Post challenge mortalities was significantly lower (P less than 0.05) and organ lesions very significantly minimized (P less than 0.01) in 7 day old vaccinated group than in 14 or 28 days old vaccinated chicks. These results showed that early (7 days) IBD Vaccination was superior to vaccination at 14 or 28 days, in terms of antibody response and protectivity against mortalities and bursal lesions.
