- Abstract
The assessment of phenolic compounds in food samples, environmental samples, and medical applications has gained importance recently. Here, we present research on novel conjugated polymer nanoparticles (P-PimBzBt NPs) and their composites with two-dimensional titanium disulfide nanosheets (2D-TiS2) for electrochemical tyrosinase (TYR)-based catechol detection. P-PimBzBt NPs are decorated with 2D-TiS2to enhance the electrochemical performance for biosensing. In addition, the interaction of P-PimBzBt NPs with TiS2was investigated at the molecular level by employing van der Waals (vdW) dispersion-corrected density functional theory (DFT) calculations and classical all-atom molecular dynamics simulations. According to the theoretical studies, the presence of the TiS2layer increases the interfacial interaction with the conjugated polymer via electrostatic interactions. Using cyclic voltammetry (CV), electrochemical impedance spectroscopy (EIS), field emission scanning electron microscopy (FE-SEM), and energy-dispersive X-ray spectroscopy (EDS) analyses, the production of SPE/TiS2@P-PimBzBt NPs/TYR nanobiosensors was examined. With a detection range of 3.0–27.5 μM, 0.33 μM LOD, and 3.89 μA/μM·cm2sensitivity values, the sensing layer based on the TiS2@P-PimBzBt NP composites has a targeting ability toward catechol. Its selectivity was investigated using commonly used interfering ions and compounds such as citric acid, urea, glucose, uric acid, KCl, and NaCl. Application of nanobiosensors to actual samples (tap water and black tea) was carried out with high accuracy. The fabricated biosensing platform demonstrates that P-PimBzBt NPs with 2D-TiS2nanomaterial functionalization are appropriate as electrode materials and could be used to create an inexpensive, fast-response, and highly selective electrochemical biosensor for the detection of catechol in actual samples.

6. Relationship between ownership structure and creditor monitoring: evidence from a bank-based civil law emerging country

In civil law countries where investor protection is regarded as low, ownership and debt maturity structures act as substitute corporate governance mechanisms. This study examines the relationship between ownership structure and creditor monitoring in a bank-based civil law emerging country, where creditor monitoring is considered a significant corporate governance mechanism. The generalised method of moments model is employed to analyse the sample consisting of small and medium enterprises, which have unique ownership characteristics, from the Borsa Istanbul BIST SME Industrials Index for the period 2007-2019. The empirical findings provide evidence that when multiple blockholders exist, the frequent monitoring role of creditors is not sought by the shareholders as a corporate governance mechanism. However, the analysis yields an insignificant relationship between family ownership and creditor monitoring.

The pricing and performance of IPOs is one of those issues that never are laid to rest in the finance literature. As a result of the long-standing debate on the reasons of abnormal initial returns from IPOs and long-term under-performance of IPOs, various hypotheses have been formulated by finance researchers. The empirical finance literature contains both supporting and conflicting evidence for these hypotheses. Consequently, the present study aimed to investigate the IPO returns by the data generated by an emerging market, namely ISE, for the period from 1990 to 1997, in two stages. In the first stage, the relationship between the returns is analyzed by comparing mean returns and by univariate regression analysis. In the second stage, the determinants of returns are examined by cross sectional analysis and multivariate regression analysis. The findings of the present study provide evidence to the fads hypothesis and the Winner's Curse hypothesis. Moreover, the factors that decrease the uncertainty associated with the IPOs are found to lead to lower returns. It must also be emphasized that the findings of the study does not provide evidence of long-term underperformance.

The cave paintings in the Akyapi (AlaKapi) cave, near the town of Giilnar (Mersin) have been discovered in 2012 and they are regarded as unique for the Middle Taurus region. In these paintings which are excursively scattered on the cave walls, the same figure was presented several times. The human figures have their arms raised upwards with elbows knicked in an angle of 90° and with knees also knicked in the same angle as the arms. The figures have big phalloi (?). The fact that the standard figure in question was depicted several times and that there is no other subject in the cave walls indicate the importance of both the cave and the depicted character. Because of its huge phal-los, it is suggested that the figure in question is a sacred being, and that the cave was probably a sacred place. It is assumed that the cave paintings of Akyapi in the Middle Taurus region where the number of prehistoric researches are so scanty may be dated to Neolithic or Chalcolithic period.

WOS: 000421963600007, PubMed: 25782122, Objectives: Atrial fibrillation (AF) is one of the most common causes of preventable ischemic stroke and is related to increased cardiovascular morbidity and mortality. There is a lack of data in Turkey on the use of new oral anticoagulants (NOACs), and time in therapeutic INR range (TTR) in vitamin K antagonist users and AF management modality. In this multi-center trial, we aimed to analyze, follow and evaluate the epidemiological data in non-valvular AF patients. Study design: Four thousand one hundred consecutive adult patients from 42 centers with at least one AF attack identified on electrocardiography will be included in the study. Patients with rheumatic mitral valve stenosis and prosthetic valve disease will be excluded from the study. At the end of one year, the patients will be evaluated in terms of major cardiac end points (death, transient ischemic attack, stroke, systemic thromboembolism, major bleeding and hospitalization). Results: First results are expected in June 2015. Data about major cardiovascular end-points will be available in January 2016. Conclusion: The rates and kind of oral anticoagulant use, TTR in vitamin K antagonist users and main management modality applied in non-valvular AF patients will be determined by AFTER-2 study. In addition, the rate of major adverse events (MACEs) and the independent predictors of these MACEs will be detected (AFTER-2 Study ClinicalTrials. gov number, NCT02354456.).

The aim of the present stud, is to analyze the relationship between the stock returns of the firms listed on İstanbul Stock Exchange (ISE) and fundamental factors such as; the beta coefficient, Market Value/Book Value (MV/BV), Debt/Equity (D/E), Market Value of Equity (MVE), Price/Earning (P/E) and Sales/Price (S/P). In addition, the study analyzes the influence of the Jenuary effect on the relationship between fundamental factors and stock returns. The findings support the strengthening effect of jenuary on the relationship betweenstock returns and fundamental factors. Th indings of the study suggest that the beta coefficient, MVE, P/E and S/P ratios have higher explanatory power in explaining stock returns than the MV/BV and D/E ratios.

Günümüzde yaşanan tüm değişimler, muhasebecinin muhasebe bilgisinin yanında, farklı niteliklere de sahip olması gereğini doğurmuştur. Muhasebecinin kimliğinin oluşumunda yaşamsal öneme sahip olan, meslek öncesi ve meslek içi eğitimi bir bütün olarak niteleyen muhasebe eğitimi de, bu değişen muhasebeci kimliğine uygun olarak, değişmek zorundadır. Bu çalışmada bu kimliğe uygun muhasebe eğitiminin niteliklerinin belirlenmesi amaçlanmaktadır. Çalışmada, öncelikle, günümüzde muhasebecinin sahip olması gereken nitelikler ve bunlara etki eden faktörler saptanacaktır. Bu saptamalar çerçevesinde muhasebe eğitimi programlarının standartlarının belirlenmesinde muhasebe eğitiminin sahip olması gerekli olan özellikler ele alınacaktır. Çalışmanın son bölümünde ise, muhasebecinin kimliği ve bu kimliğe uygun muhasebe eğitimi, bir bütün olarak değerlendirilecektir.

Elaiussa Sebaste’nin mimari açıdan en zayıf olduğu Severuslar döneminde, sadece dört km. batısındaki Korykos’un (diğer pek çok Kilikia kenti gibi) en zengin dönemini yaşıyor olması oldukça düşündürücü bir noktadır. Septimius Severus döneminde iki asırlık dev bir metropol olan ve o tarihe kadar bölgenin başkenti durumundaki Elaiussa Sebaste, M.S. 2. yy. sonlarından itibaren küçülmeye başlamış; mütevazı bir yerleşim olan Korykos ise aynı dönemde büyük bir kent olmuş ve tüm bölgenin ekonomik merkezi haline gelmiştir. Farklı arkeolojik kanıtlarla desteklenen bu tespitin en önemli dayanaklarından biri, Elaiussa Sebaste’de tapınak mezar geleneğinin Antoninuslar dönemi sonlarında ortadan kalkmış olmasıdır. Korykos’ta ise bu ve benzeri mezar anıtlarına en fazla Severuslar döneminde rastlanmaktadır. Yine Severuslar dönemine tarihlenebilen mimari buluntu sayısı Elaiussa Sebaste’de yok denecek kadar azken, Korykos ve Kilikia’nın diğer kentleri bu evrede en zengin dönemlerini yaşamıştır. Elaiussa Sebaste ve Korykos’taki “ters orantılı” bu dönüşüm süreci Septimius Severus döneminde gerçekleşmiş ve devam etmiştir. Birbirine komşu olan iki kentin tarihinde yaşanan bu dramatik dönüşümün önemli bir gerekçesi olmalıdır. Septimius Severus’un ilk yıllarının oldukça sıkıntılı geçtiği bilinmektedir. Nitekim bu imparator, Niger ve Albinus gibi iki önemli rakip ile Roma tahtı için iç savaşlara girmiştir. Bu iç savaşlar sürecinde, imparatorluğun çeşitli noktalarında yer alan pek çok kent, birbirine rakip olan üç generalden birini desteklemiştir. Niger ile Albinus’u destekleyen kentlerin ve kişilerin, savaş bittikten sonra Septimius Severus tarafından cezalandırıldıkları bilinmektedir. Elaiussa Sebaste’nin de, rakip bir generalin iddialarını desteklediğine dair bazı ipuçları bulunmaktadır. Dolayısıyla bu kentin yaşadığı duraklama ve gerileme sürecinin sebepleri, bu iç savaşlarla ilişkili gibi görünmektedir. Tarihi kaynaklardan edinilen bilgilere göre Severus, cezalandırdığı bazı kentleri bir süre sonra affetmiş ve tekrar iyi ilişkiler kurmuştur. Ancak Elaiussa Sebaste’nin bu süreçteki durumu hakkında bu tarihi kaynaklar suskun kalmaktadır. Konuyu aydınlatabilecek herhangi bir epigrafik buluntu da söz konusu değildir. Bu nedenle Elaiussa Sebaste’nin içinde bulunduğu atmosferi aydınlatabilecek tek başvuru kaynağı arkeolojik buluntulardır. Elaiussa Sebaste ve Korykos’ta yapılan arkeolojik araştırmalar, Septimius Severus döneminde bu iki kentin cazibe merkezi olma konusunda yer değiştirdiği şeklinde bir sonuç ortaya çıkarmaktadır. Bir başka ifade ile bölgenin en cazip kenti olma özelliği Elaiussa Sebaste’den Korykos’a geçmiştir. M.S. 2. yy. sonları ile 3. yy.’ın başları arasına tarihlenen buluntular, çok net biçimde bu tespiti desteklemektedir. Korykos’ta, Elaiuusa Sebaste’nin tam tersine, çok hızlı bir gelişme evresi yaşanmış, ekonomik büyümenin işareti olarak değerlendirilebilecek imar çalışmaları zirveye ulaşmıştır. Da keine Naturkatastrophe in der betreffenden Region bekannt ist, ist es wahrscheinlich, dass der Grund für den Rückgang einer großen, reichen und führenden Stadt wie Elaiussa Sebaste mit einem politischen Grund zusammenhängt. Die Entwicklungen in den ersten Verwaltungsjahren des Septimius Severus beinhalten recht befriedigende Hinweise zum Verständnis des problematischen Prozesses in der Geschichte von Elaiussa Sebaste. Elaiussa Sebaste’nin mimari açıdan en zayıf olduğu Severuslar döneminde, sadece dört km. batısındaki Korykos’un (diğer pek çok Kilikia kenti gibi) en zengin dönemini yaşıyor olması oldukça düşündürücü bir noktadır. Septimius Severus döneminde iki asırlık dev bir metropol olan ve o tarihe kadar bölgenin başkenti durumundaki Elaiussa Sebaste, M.S. 2. yy. sonlarından itibaren küçülmeye başlamış; mütevazı bir yerleşim olan Korykos ise aynı dönemde büyük bir kent olmuş ve tüm bölgenin ekonomik merkezi haline gelmiştir. Farklı arkeolojik kanıtlarla desteklenen bu tespitin en önemli dayanaklarından biri, Elaiussa Sebaste’de tapınak mezar geleneğinin Antoninuslar dönemi sonlarında ortadan kalkmış olmasıdır. Korykos’ta ise bu ve benzeri mezar anıtlarına en fazla Severuslar döneminde rastlanmaktadır. Yine Severuslar dönemine tarihlenebilen mimari buluntu sayısı Elaiussa Sebaste’de yok denecek kadar azken, Korykos ve Kilikia’nın diğer kentleri bu evrede en zengin dönemlerini yaşamıştır. Elaiussa Sebaste ve Korykos’taki “ters orantılı” bu dönüşüm süreci Septimius Severus döneminde gerçekleşmiş ve devam etmiştir. Birbirine komşu olan iki kentin tarihinde yaşanan bu dramatik dönüşümün önemli bir gerekçesi olmalıdır. Septimius Severus’un ilk yıllarının oldukça sıkıntılı geçtiği bilinmektedir. Nitekim bu imparator, Niger ve Albinus gibi iki önemli rakip ile Roma tahtı için iç savaşlara girmiştir. Bu iç savaşlar sürecinde, imparatorluğun çeşitli noktalarında yer alan pek çok kent, birbirine rakip olan üç generalden birini desteklemiştir. Niger ile Albinus’u destekleyen kentlerin ve kişilerin, savaş bittikten sonra Septimius Severus tarafından cezalandırıldıkları bilinmektedir. Elaiussa Sebaste’nin de, rakip bir generalin iddialarını desteklediğine dair bazı ipuçları bulunmaktadır. Dolayısıyla bu kentin yaşadığı duraklama ve gerileme sürecinin sebepleri, bu iç savaşlarla ilişkili gibi görünmektedir. Tarihi kaynaklardan edinilen bilgilere göre Severus, cezalandırdığı bazı kentleri bir süre sonra affetmiş ve tekrar iyi ilişkiler kurmuştur. Ancak Elaiussa Sebaste’nin bu süreçteki durumu hakkında bu tarihi kaynaklar suskun kalmaktadır. Konuyu aydınlatabilecek herhangi bir epigrafik buluntu da söz konusu değildir. Bu nedenle Elaiussa Sebaste’nin içinde bulunduğu atmosferi aydınlatabilecek tek başvuru kaynağı arkeolojik buluntulardır. Elaiussa Sebaste ve Korykos’ta yapılan arkeolojik araştırmalar, Septimius Severus döneminde bu iki kentin cazibe merkezi olma konusunda yer değiştirdiği şeklinde bir sonuç ortaya çıkarmaktadır. İMKB'de işlem gören fınansal olmayan 192 şirketin websitelerinde sundukları fınansal bilgiler incelenerek, şirketin faaliyette bulunduğusektörün, şirket büyüklüğünün ve performansının bu bilgilerin ne detaydaverildiğine etkilerinin Kruskal-Wallis ve Ki-kare testleri ile smanmıştır.Sınamalar sonucu elde edilen ampirik bulgular, sektörün ve performans ölçütüolarak net karın firmaların web sitesinin varlığını ve sunulan fınansal bilginindetay düzeyini belirleyici faktörler olmadığını ortaya koymuştur. Ancak, şirketbüyüklüğü satışlar, piyasa değeri ve çalışan sayıss bazında artıkça sunulan bilgidaha detaylı olmaktadır.Abstract: The aim of the study is to investigate the level of financialinformation voluntarily disclosed on the company web sites and to analyze thefactors affecting the level of disclosure. The study examined the web sites of192 non-financial companies listed on fstanbul Stoek Exchange. Chi-Square andKruskal-Wallis tests that are applied to determine the effects of industry type,size and performance variables on the details of financial information disclosureshowed that industry type and net profit, as a performance indicator, have nostatistically significant effeet on the existence of company web site and the levelof financial information disclosure. However, the findings suggest that as thefirm size, measured in terms of sales, market value and number of employees,increases, the firm discloses more detailed financial information on thecompany web site.

- Abstract
The present study aims to investigate the influence of ownership structure on initial public offering (IPO) underpricing within the context of the Istanbul Stock Exchange (ISE) in Turkey, an emerging market. The present study provides evidence from an emerging market (the ISE) and a civil law country (Turkey) which possess different characteristics than the developed markets and common law countries that have been extensively investigated by the majority of the existing studies in the IPO underpricing literature. The findings of the study, as a whole, can be interpreted to conclude that the relationship between ownership structure and underpricing, if there is one, is weak. The results may be interpreted as a reflection of Turkey being a civil law country where the focus on the stock market and the corporate control provided by the stock market is quite negligible. As a result, the IPO does not act as a means of change in the control mechanisms of firms, hence control and monitoring concerns do not influence the level of underpricing. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Immune checkpoint inhibitors (ICIs) have transformed the treatment landscape for solid tumors, offering substantial survival benefits. Despite this progress, many patients do not achieve durable responses, highlighting the need for novel prognostic biomarkers. This study investigates the association between serum sodium levels and survival outcomes in patients treated with ICIs., Research Design and Methods: We conducted a retrospective cohort study involving 509 patients with metastatic solid tumors treated with ICIs. We assessed overall survival (OS), progression-free survival (PFS), and response rates using Kaplan-Meier survival analysis and multivariate cox regression analysis., Results: The median age was 62 years (interquartile range (IQR): 54-69), and 76.6% of the patients were male. Multivariate analysis revealed that serum sodium levels between 135-140 mmol/L were an independent predictor of improved OS (HR: 0.58; 95% CI: 0.44-0.77) and PFS (HR: 0.76; 95%CI: 0.58-0.99) and those with levels > 140 mmol/L had an even lower HR of 0.43 (95% CI:0.31-0.62) for OS and HR of 0.62 (95% CI:0.45-0.86) for PFS., Conclusion: This study highlights that ICI-treated patients with higher sodium levels had significantly better OS, PFS, and anti-tumor responses. Baseline serum sodium levels could be cost-effective and valuable predictive biomarker for ICIs across diverse tumor types and ICI agents.

- Abstract
This study aimed to determine microbial colonization in indwelling urinary catheters (UCs) and identify patient-specific risk factors associated with this colonization. This cross-sectional study involved 61 hospitalized intensive care unit patients with indwelling UCs. Bacterial colonization and susceptibility were assessed in the indwelling UCs from the second day onwards following urinary catheterization. The average duration of catheterization was 13.62 ± 13.72 days. Colonization of 10⁵ CFU/mL and above was determined in all indwelling UCs from the second day of catheterization onwards. The catheter was colonized by the following microorganism species: Pseudomonas aeruginosa, Acinetobacter baumannii, Proteus mirabilis, and Staphylococcus aureus. 47.9% of clinical isolates showed multi-drug resistance (MDR). Clinical isolates did not show significant differences based on patient variables such as age, Body Mass Index, and duration of urinary catheterization (p > 0.05). There was a weak correlation (rs:≤ 0.206; p > 0.05) between the species of clinical isolates and patient laboratory variables. Colonization was determined in all indwelling UCs, with nearly half of isolates exhibiting MDR. These findings highlight the urgent need for improved strategies to manage and prevent catheter-associated infections, particularly in high-risk patient populations., (© 2025 The Author(s). - Abstract
Background: The hemoglobin, albumin, lymphocyte, and platelet (HALP) score could be a prognostic biomarker in patients with cancer as a reflector of nutritional and inflammatory status, although the data is limited in patients treated with immune-checkpoint inhibitors (ICIs). Therefore, we sought to investigate the relationship between HALP score and survival in ICI-treated patients., Methods: We included adult patients with advanced cancer treated with ICIs between June 2016 and January 2024. Receiver operating characteristic (ROC) curve analysis was employed to identify the optimal HALP score cutoff point for survival prediction. The Kaplan-Meier method was utilized to create survival curves, and Cox regression was employed for multivariate analysis., Results: A total of 456 patients were included. The median age was 62 years, and 64.7% were male. The optimal HALP cutoff value for survival prediction was 22.8 in ROC analyses (AUC: 0.624, 95% CI: 0.570-0.679, p < 0.001). Multivariate analysis revealed that patients with low HALP scores had significantly shorter OS (HR: 1.394, 95% CI: 1.077-1.805, p = 0.012) and PFS (HR: 1.388, 95% CI: 1.129-1.706, p = 0.002)., Conclusions: Our study results pointed out the possible use of the HALP score as a prognostic marker in ICI-treated patients. If validated in prospective cohorts, the HALP score could enhance prognosis prediction in ICI-treated patients.

- Abstract
The assessment of phenolic compounds in food samples, environmental samples, and medical applications has gained importance recently. Here, we present research on novel conjugated polymer nanoparticles (P-PimBzBt NPs) and their composites with two-dimensional titanium disulfide nanosheets (2D-TiS
2 ) for electrochemical tyrosinase (TYR)-based catechol detection. P-PimBzBt NPs are decorated with 2D-TiS2 to enhance the electrochemical performance for biosensing. In addition, the interaction of P-PimBzBt NPs with TiS2 was investigated at the molecular level by employing van der Waals (vdW) dispersion-corrected density functional theory (DFT) calculations and classical all-atom molecular dynamics simulations. According to the theoretical studies, the presence of the TiS2 layer increases the interfacial interaction with the conjugated polymer via electrostatic interactions. Using cyclic voltammetry (CV), electrochemical impedance spectroscopy (EIS), field emission scanning electron microscopy (FE-SEM), and energy-dispersive X-ray spectroscopy (EDS) analyses, the production of SPE/TiS2 @P-PimBzBt NPs/TYR nanobiosensors was examined. With a detection range of 3.0-27.5 μM, 0.33 μM LOD, and 3.89 μA/μM·cm2 sensitivity values, the sensing layer based on the TiS2 @P-PimBzBt NP composites has a targeting ability toward catechol. Its selectivity was investigated using commonly used interfering ions and compounds such as citric acid, urea, glucose, uric acid, KCl, and NaCl. Application of nanobiosensors to actual samples (tap water and black tea) was carried out with high accuracy. The fabricated biosensing platform demonstrates that P-PimBzBt NPs with 2D-TiS2 nanomaterial functionalization are appropriate as electrode materials and could be used to create an inexpensive, fast-response, and highly selective electrochemical biosensor for the detection of catechol in actual samples.- Published

Objective: Ocular hemorrhages (OHs) may cause visual disturbances and incidence vary from 18 to 39% in newborns. Precipitated/instrumental delivery and perinatal asphyxia were predefined risk factors. Acidosis can interfere with coagulation and disrupt the pressure of ocular capillaries and put infants with moderate acidosis with or without hypoxic-ischemic encephalopathy at risk for OH. We aimed to evaluate the OH in neonates with fetal acidosis., Study Design: Neonates >34 weeks are included if pH < 7.10 and BE < -12 mmol/L within the first hour. Ophthalmologic examinations for retinal (RH), vitreous, and anterior chamber (hyphema) hemorrhage were done within the third day. RH was staged according to Egge's classification. Follow-up of the patients was continued until the age of 2. Clinical characteristics of newborns were analyzed., Results: Sixty-two neonates (38 ± 2.3 weeks, 2,971 ± 612 g) were included. pH = 6.91 ± 0.16, BE = - 17.2 ± 5.3 mmol/L. OH was found in 22 (36.7%) neonates (hyphema n = 2, vitreous n = 2, RH n = 21). Thirty-eight eyes with RH were staged (Stage 3: n = 15 [39.5%]; Stage 2: n =11 [28.9%]; Stage 1: n = 12 [31.6%]). Vaginal delivery (OR: 4.9, 95% CI [1.4-17.8]) and advanced resuscitation at the delivery room (OR: 8.8; 95% CI [1.9-41.7]) were found to increase the risk of RH., Conclusion: Approximately one-third of neonates with moderate to severe acidosis exhibited RH when examined on the third day. Contrary to previous studies that reported mild RH in otherwise healthy neonates, our findings revealed that neonates with moderate to severe acidosis predominantly presented with Stage 3 RH. While the higher incidence of RH in vaginally delivered infants is consistent with previous studies, the identification of advanced resuscitation as a risk factor is a new addition to the literature. Introduction: Infants with perinatal asphyxia are at risk for organ failure aside from the brain, regardless of the severity of the asphyxial insult. We aimed to evaluate the presence of organ dysfunction other than the brain in newborns with moderate to severe acidosis at birth, in the absence of moderate to severe hypoxic ischemic encephalopathy., Materials and Methods: Data of 2 years were retrospectively recorded. Late preterm and term infants admitted to the intensive care unit with ph < 7.10 and BE < -12 mmol/l in the first hour were included in the absence of moderate to severe hypoxic ischemic encephalopathy. Respiratory dysfunction, hepatic dysfunction, renal dysfunction, myocardial depression, gastrointestinal problems, hematologic system dysfunction, and circulatory failure were evaluated., Results: Sixty-five infants were included [39 (37-40) weeks, 3040 (2655-3380) grams]. Fifty-six (86 %) infants had one or more dysfunction in any system [respiratory: 76.9 %, hepatic: 20.0 %, coagulation: 18.5 %, renal: 9.2 %, hematologic: 7.7 %, gastrointestinal: 3.0 %, and cardiac: 3.0 %]. Twenty infants had at least two affected systems. The incidence of coagulation dysfunctions was higher in the infants with severe acidosis (n = 25, ph < 7.00) than the infants with moderate acidosis (n = 40: pH = 7.00-7.10); 32 % vs 10 %; p = 0.03., Conclusions: Moderate to severe fetal acidosis is associated with the development of extra-cranial organ dysfunctions in infants who do not require therapeutic hypothermia. A monitoring protocol is needed for infants with mild asphyxia in order to identify and manage potential complications. Objective: Temporal patterns for suicide over a 24-hour period have shown mixed results among prior studies. However, analyses of 24-hour temporal patterns for wakeful actions including suicidal behavior should adjust for expected sleep requirements that inherently skew such activities to conventional wakeful times. This study analyzed the time-of-day for suicide cases from the Australian population for the year 2017, adjusting for expected sleep patterns. Identification of time-of-day trends using this methodology may reveal risk factors for suicide and potentially modifiable contributors., Methods: The Australian National Coronial Information System database was accessed, and data for completed suicide were extracted for the most recent completed year (2017). Time of suicide was allocated to one of four 6-hourly time bins across 24 hours, determined from time last seen alive and time found subsequently. Prevalence of suicide for each time bin was adjusted for the likelihood of being awake for each bin according to sleep-wake norms published from a large Australian community survey. Observed prevalence of suicide was compared to expected values predicted from likelihood of being awake across each time bin calculated as a standardized incidence ratio (SIR)., Results: For the year 2017, there were 2,808 suicides, of which 1,417 were able to be allocated into one of four 6-hourly time bins. When compared to expected values, suicides were significantly more likely to occur in the overnight bin (2301-0500; SIR = 3.93, P < .001)., Conclusions: Higher-than-expected rates of suicide overnight associated with nocturnal wakefulness may represent a modifiable risk factor for triggering suicide events., (© Copyright 2022 Physicians Postgraduate Press, Inc.)

- Abstract
Background: The use of warfarin and aspirin in combination is restricted to limited patients under relevant guidelines., Aims: To evaluate the prevalence of the inappropriate combination of aspirin and warfarin therapy in daily practice and its risks., Study Design: Cross-sectional study., Methods: The awareness, efficacy, safety, and time in the therapeutic range of warfarin in the Turkish population study is a multi-center observational study that includes 4987 patients using warfarin for any reason between January 1, 2014, and December 31, 2014. To determine the prevalence of inappropriate combination use in daily practice, all patients who had a history of atherosclerotic disease (ischemic heart disease, peripheral artery disease) or cerebrovascular disease (n=1498) were excluded. The data of 3489 patients were analyzed. We defined inappropriate combination as all patients who received aspirin and warfarin regardless of the indication for warfarin use, under the direction of the European Society of Cardiology guideline recommendation., Results: The mean age of patients was 59.2±13.8 years (41.8% male). The prevalence of the inappropriate use of warfarin and aspirin combination was 20.0%. The prevalence of combination therapy in patients with a primary indication for mechanical heart valve, non-valvular atrial fibrillation, and other reasons was 20.5%, 18.7%, and 21.0%, respectively. Multivariate logistic regression analysis revealed that age (odds ratio, 1.009; 95% confidence interval, 1.002-1.015; p=0.010), heart failure (odds ratio, 1.765; 95% confidence interval, 1.448-2.151; p<0.001), smoking (odds ratio, 1.762; 95% confidence interval, 1.441-1.153; p<0.010), chronic kidney disease (odds ratio, 2.057; 95% confidence interval, 1.494-2.833; p<0.001), and deep vein thrombosis (odds ratio, 0.463; 95% confidence interval, 0.229-0.718; p=0.001) were independent predictors of combination therapy (r2=0.66). The mean time in therapeutic range of patients receiving combination therapy was significantly lower than in those on warfarin monotherapy (51.6±27.05 vs. 54.7±23.93; p=0.006). Overall, 19.4% (n=677) of patients had a bleeding event (major bleeding 13.0%, n=88) within a year. Percentages of patients with combination therapy were significantly higher in patients with major bleeding than in patients without major bleeding (29.5% vs. 19.7%; p=0.023)., Conclusion: Our study demonstrated that 20.0% of patients taking warfarin use concomitant aspirin inappropriately in daily practice. Patients receiving aspirin with warfarin were demonstrated to have more comorbidities, lower time in therapeutic range levels, and higher bleeding rates.

- Abstract
Objective: Thyroid disease is a common endocrine disease with important effects on the cardiovascular system. As an adaptive response to myocardial ischemia, coronary collateral circulation (CCC) plays an important role in obstructive coronary artery disease (CAD). The association between serum thyroid hormone levels and development of CCC was investigated in the present study., Methods: In total, 430 consecutive patients who underwent coronary angiography procedure and had documented total occlusion in at least 1 major coronary artery were investigated retrospectively. Degree of CCC was classified according to Cohen-Rentrop method. Serum free triiodothyronine (FT3), free thyroxine (FT4) and thyroid-stimulating hormone (TSH) were assessed by the chemiluminescence immunoassay technique., Results: In spite of diabetes mellitus (p=0.019), smoking (p<0.001), and TSH (p<0.001), FT3 (p<0.001), FT4 (p=0.015), and subclinical hypothyroidism (SCH) (p<0.001) ratios were significantly different between groups. In regression analysis, SCH (p=0.024), DM (p=0.021), smoking (p<0.001), and heart failure (p=0.029) were independent predictors of poor CCC development in multivariate model 1. When regression analyses were performed based on multivariate model 2, TSH (p<0.001), FT3 (p<0.001), heart failure (p=0.022), smoking (p<0.001), and hyperlipidemia (HPL) (p=0.046) were independent predictors of poor CCC development., Conclusion: In addition to traditional risk factors, SCH, higher serum TSH, and lower FT3 levels were associated with development of poor CCC in patients with obstructive CA.

- Abstract
Objectives: Atrial fibrillation (AF) is one of the most common causes of preventable ischemic stroke and is related to increased cardiovascular morbidity and mortality. There is a lack of data in Turkey on the use of new oral anticoagulants (NOACs), and time in therapeutic INR range (TTR) in vitamin K antagonist users and AF management modality. In this multi-center trial, we aimed to analyze, follow and evaluate the epidemiological data in non-valvular AF patients., Study Design: Four thousand one hundred consecutive adult patients from 42 centers with at least one AF attack identified on electrocardiography will be included in the study. Patients with rheumatic mitral valve stenosis and prosthetic valve disease will be excluded from the study. At the end of one year, the patients will be evaluated in terms of major cardiac end points (death, transient ischemic attack, stroke, systemic thromboembolism, major bleeding and hospitalization)., Results: First results are expected in June 2015. Data about major cardiovascular end-points will be available in January 2016., Conclusion: The rates and kind of oral anticoagulant use, TTR in vitamin K antagonist users and main management modality applied in non-valvular AF patients will be determined by AFTER-2 study. In addition, the rate of major adverse events (MACEs) and the independent predictors of these MACEs will be detected (AFTER-2 Study ClinicalTrials.gov number, NCT02354456.).

- Abstract
Background: Despite the advances in medical and interventional treatment modalities, some patients develop epicardial coronary artery reperfusion but not myocardial reperfusion after primary percutaneous coronary intervention (PCI), known as no-reflow. The goal of this study was to evaluate the safety and efficacy of intracoronary epinephrine in reversing refractory no-reflow during primary PCI., Methods: A total of 248 consecutive STEMI patients who had undergone primary PCI were retrospectively evaluated. Among those, 12 patients which received intracoronary epinephrine to treat a refractory no-reflow phenomenon were evaluated. Refractory no-reflow was defined as persistent TIMI flow grade (TFG) ≤ 2 despite intracoronary administration of at least one other pharmacologic intervention. TFG, TIMI frame count (TFC), and TIMI myocardial perfusion grade (TMPG) were recorded before and after intracoronary epinephrine administration., Results: A mean of 333 ± 123 mcg of intracoronary epinephrine was administered. No-reflow was successfully reversed with complete restoration of TIMI 3 flow in 9 of 12 patients (75%). TFG improved from 1.33 ± 0.49 prior to epinephrine to 2.66 ± 0.65 after the treatment (p < 0.001). There was an improvement in coronary flow of at least one TFG in 11 (93%) patients, two TFG in 5 (42%) cases. TFC decreased from 56 ± 10 at the time of no-reflow to 19 ± 11 (p < 0.001). A reduction of TMPG from 0.83 ± 0.71 to 2.58 ± 0.66 was detected after epinephrine bolus (p < 0.001). Epinephrine administration was well tolerated without serious adverse hemodynamic or chronotropic effects. Intracoronary epinephrine resulted in significant but tolerable increase in heart rate (68 ± 13 to 95 ± 16 beats/min; p < 0.001) and systolic blood pressure (94 ± 18 to 140 ± 20; p < 0.001). Hypotension associated with no-reflow developed in 5 (42%) patients. During the procedure, intra-aortic balloon pump counterpulsation was required in two (17%) patients, transvenous pacing in 2 (17%) cases, and both intra-aortic balloon counterpulsation and transvenous pacing in one (8%) patients. One patient (8%) died despite all therapeutic measures., Conclusion: Intracoronary epinephrine may become an effective alternative in patients suffering refractory no-reflow following primary PCI.
