У статті зазначається, що законодавство гарантує кожному право на охорону здоров’я та медичну допомогу. Повномасштабне вторгнення викликає необхідність опрацювання та оновлення нормативно-правових актів щодо забезпечення права особи на медичну допомогу, зокрема на надання реабілітаційних послуг. Зазначено, що реабілітаційна допомога спрямована на низку основних напрямків, по-перше, на підтримку функцій організму пацієнта в процесі загострення хронічних захворювань. По-друге – на попередження, ранню діагностику порушень роботи органів та систем організму пацієнта. Третій напрямок направлений на повне чи часткове відновлення порушених органів чи систем та компенсацію втрачених функцій уражених органів чи систем. Четвертий напрямок пов’язаний із попередженням та зниженням ступеня інвалідизації. Автор наголошує, що відсутність грамотної та ефективної медичної реабілітації в Україні – багатофакторна проблема: є недостатня кількість стаціонарних та амбулаторних відділень реабілітації; нестача медичного персоналу – реабілітологів, психологів; не вистачає реабілітаційного обладнання; існує потреба в належних освітніх програмах для лікарів. У статті автор наголошує, що одним з завдань держави є сприяння реалізації особами з інвалідністю їх конституційних прав; проведення державної політики у сфері реабілітації осіб з інвалідністю, підтримка їх інтеграції у суспільство, а повномасштабна війна на території України зумовлює все більшу актуальність та потребу реабілітації осіб, які постраждали внаслідок бойових дій. Такі чинники здоров’я наcелення, як зростання захворюваності, травматизм, тимчасова втрата працездатності, передчасна смертність населення вимагають розвитку ринку реабілітаційної медицини України та його правового регулювання. У статті зазначається, що позитивними аспектами є розробка нових технологій медичної реабілітації, у тому числі на основі телемедицини та IT-технологій, підготовка нових професійних стандартів для медичної реабілітації на сучасному рівні відповідно до міжнародних рекомендацій. Все це вимагає ефективного функціонування системи, що насамперед потребує удосконалення та розвитку нормативно-правового забезпечення у сфері реабілітації.

Summary: Background: Longitudinal changes in vaccination-induced immune response remain inadequately characterized in adolescents. We present long-term safety, immunogenicity, and COVID-19 incidence following a 2-dose mRNA-1273 100-μg primary series, and immunogenicity following a single dose of mRNA-1273 50 μg in vaccine-naïve adolescents. Methods: TeenCOVE (NCT04649151) Part 1 randomized adolescents (12–17 years) to 2-dose mRNA-1273 100 μg (n = 2490) or placebo (n = 1243) 28 days apart. Subsequently, placebo recipients (n = 91) could receive open-label mRNA-1273. Primary objectives included prespecified adverse events through 12 months; secondary objectives were COVID-19 incidence and neutralizing and spike-binding antibodies (nAbs/bAbs) against SARS-CoV-2 (ancestral/variants) through 12 months (study period: December 2020–January 2022). In Part 2, vaccine-naïve adolescents (n = 52) received up to 2 doses of mRNA-1273 50 μg; interim analysis included Day 28 (D28) nAbs post-injection 1 in SARS-CoV-2-baseline-positive participants (serologic/virologic evidence of prior infection). Findings: In SARS-CoV-2-baseline-negative adolescents (N = 369), mRNA-1273 induced robust nAb responses versus baseline (geometric mean concentration [GMC] = 11; 95% CI, 11–12) at D28 (1868 [1759–1985]), 6 months (625 [583–670]) and 12 months (550 [490–618]) post-injection 2. Similar bAb responses were observed to alpha/beta/delta/gamma variants; nAb/bAb responses were similar in SARS-CoV-2-baseline-positive adolescents. The 2-dose mRNA-1273 100-μg primary series was generally well-tolerated; one case of nonserious, moderate, probable acute myocarditis resolved by 8 days from symptom onset. A single dose of mRNA-1273 50 μg in SARS-CoV-2-baseline-positive adolescents induced higher D28 nAb GMCs against ancestral SARS-CoV-2 than 2-dose mRNA-1273 100 μg in young adults (geometric mean ratio = 4.322 [3.274-5.707]). Interpretation: The overall risk–benefit profile of mRNA-1273 remains favorable in adolescents, with durable 12-month immune responses against SARS-CoV-2 (ancestral/variants). A single mRNA-1273 50-μg injection in vaccine-naïve adolescents elicited robust immune responses against SARS-CoV-2. This article presents the data on the effectiveness of the treatment complex we developed on the activity of bone remodelling markers in the oral fluid during the treatment of generalized periodontitis, depending on the blood group. The use of our proposed drugs of general and local action in patients with generalized periodontitis of different blood groups allowed to reduce the average content of TRACP in the oral fluid by 18.25%, while increasing the level of BSALP by 27.64% in comparison with the corresponding data before treatment, p, р1

Despite decreases in mortality rate, the treatment of cognitive deficits following aneurysmal subarachnoid hemorrhage (aSAH) remains a serious challenge for clinicians and survivors alike. Deficits in executive function, language, and memory prevent more than half of survivors from returning to their previous level of work and put a tremendous amount of stress on the individual and their family. New therapies are needed for survivors of aSAH in order to improve cognitive outcomes and quality of life. The aim of this review is to discuss the prevalence and contributing factors of cognitive deficits following aSAH, as well as areas for therapeutic intervention. Due to the limited research on cognitive rehabilitative strategies for aSAH, a literature search of traumatic brain injury (TBI) was used to explore therapies with the potential to improve cognitive outcomes in aSAH. Across cognitive domains, existing rehabilitative and pharmacotherapeutic strategies for TBI show promise to be useful for survivors of aSAH. However, further study of these therapies in addition to consistent assessment of cognitive deficits are required to determine their efficacy in survivors of aSAH.

У статті проаналізовано особливості правовідносин, що виникають у зв’язку із кріоконсервацією біологічного матеріалу людини – ембріонів та репродуктивних клітин, а саме: визначення походження народженої таким чином дитини. Зазначено, що народження дітей є складовою демографічної політики держави, а застосування допоміжних репродуктивних технологій здатно змінювати демографічну ситуацію в суспільстві на краще. Кріоконсервація актуальна не тільки у випадках, коли у пари, яка бажає мати дітей є складнощі із зачаттям, але і коли таких проблем ще немає, але вони можуть виникнути з великою ймовірністю – це стосується таких категорій громадян, які проходять лікування онкологічних захворювань із ризиком втрати репродуктивного здоров’я, а також громадян, які за родом діяльності постійно ризикують своїм життям та здоров’ям: військовослужбовці, поліцейські або працівники на виробництвах із шкідливими умовами праці. Автор зазначає, що медичні біотехнології стають все різноманітнішими та ефективнішими, наукові досягнення дедалі глибше проникають у природні процеси відтворення людини, тому виникає необхідність вирішення правових питань пов’язаних з кріоконсервацією. А реалізація права на кріоконсервацію вимагає правового регулювання так званого відкладеного батьківства. З точки зору сімейного права ці відносини становлять інтерес у частині виникнення нових сімейно-правових відносин. Автор розглядає кілька моделей визначення походження дитини, народженої з застосуванням допоміжних репродуктивних технологій з використанням кріоконсервованих репродуктивних клітин або ембріонів. Розглядаються окремі питання постмортальної репродукції. Автор робить висновок про необхідність законодавчого регулювання прогалин у сфері кріоконсервації репродуктивних клітин та ембріонів, що дозволить забезпечити належне закріплення гарантії права на батьківство/материнство та ефективного механізму їх реалізації у випадках обмеження життєдіяльності чи смерті особи.

Early-life seizures (ELSs) can cause permanent cognitive deficits and network hyperexcitability, but it is unclear whether ELSs induce persistent changes in specific neuronal populations and whether these changes can be targeted to mitigate network dysfunction. We used the targeted recombination of activated populations (TRAP) approach to genetically label neurons activated by kainate-induced ELSs in immature mice. The ELS-TRAPed neurons were mainly found in hippocampal CA1, remained uniquely susceptible to reactivation by later-life seizures, and displayed sustained enhancement in [alpha]-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid (AMPA) receptor-mediated (AMPAR-mediated) excitatory synaptic transmission and inward rectification. ELS-TRAPed neurons, but not non-TRAPed surrounding neurons, exhibited enduring decreases in Gria2 mRNA, responsible for encoding the GluA2 subunit of the AMPARs. This was paralleled by decreased synaptic GluA2 protein expression and heightened phosphorylated GluA2 at Ser880 in dendrites, indicative of GluA2 internalization. Consistent with increased GluA2-lacking AMPARs, ELS-TRAPed neurons showed premature silent synapse depletion, impaired long-term potentiation, and impaired long-term depression. In vivo postseizure treatment with IEM-1460, an inhibitor of GluA2-lacking AMPARs, markedly mitigated ELS-induced changes in TRAPed neurons. The article presents the results of treatment of 157 patients with generalized periodontitis depending on blood type. In the blood of patients with generalized periodontitis was determined: the number of erythrocytes, the number of leukocytes, platelets, neutrophils, leukocyte intoxication index, fibrinogen. After treatment, analysis of blood cell composition showed that 1 year after treatment of generalized periodontitis, carriers of 0 (I) and A (II) blood type increased the number of erythrocytes in the blood – 1.2 times. At the same time, a decrease in the number of leukocytes on average was determined by 1.4 times, and leukocyte intoxication index values – by 2.0 times in representatives of 0 (I) and B (III) blood type; neutrophils – 1.4 times in people with A (II) blood type; platelets – 1.3 times in carriers with 0 (I) and AB (IV) blood type, according to data before treatment. The improvement of hematological and hemostasiological indicators of blood in patients with generalized periodontitis indicates a positive effect on the organs and systems the medicines we have prescribed, the positive effect of which is reflected, in particular, on the complex of tooth-retaining tissues.

Early childhood education plays a pivotal role in shaping an individual's personality and serves as a preparatory stage for primary education. The curiosity sparked during this phase of life influences one's lifelong learning journey, character formation, worldview, adoption of universal values and acquisition of important life skills. This study aims to explore the significance of interactive teaching methods in preparing preschoolers for school while harnessing the advantages offered by digital infrastructure to enhance learning outcomes. A diagnostic evaluation conducted as part of an educational experiment revealed that students who were exposed to interactive teaching techniques demonstrated superior readiness levels compared to their peers. The use of digital tools such as virtual and augmented reality, educational applications and games positively impacted knowledge acquisition, skill development and overall cognitive abilities among the learners. Future research can focus on developing teacher training programs geared towards implementing interactive teaching methodologies using modern software platforms and digital tools in early childhood education settings. A new approach to defining the effective fracture toughness for heterogeneous materials is proposed. This temporal averaging approach is process dependent, incorporating the crack velocity and material toughness. The effectiveness of the new technique is investigated in the context of hydraulic fracture through heterogeneous rock with a periodic material toughness. The plane strain model is considered without fluid leak-off, to more easily investigate different regimes (toughness/viscosity). Numerical simulations are used to examine the effectiveness of the new homogenisation strategy, with comparison against the recently proposed maximum toughness strategy. The parable of the Prodigal Son in Luke 15 elicits profound responses and emotions in various times, places, and cultures. Why has it stood the test of time as one of Jesus’ most famous parables? One possible answer is that the story carries enduring appeal because of the underlying structure of the parable, a recurring pattern in literature called the monomyth. Peeling back the layers of the parable, one may uncover the foundational archetypes of the parable that make it timeless. Hidden significance of the parable may be illuminated by comparing its narrative to the hero quest of Joseph Campbell and the monomyth archetypes of Northrop Frye and Leland Ryken, both of which emphasize a cyclical movement that unifies all of literature. Also important are the specific archetypes within the general monomyth archetype, such as father and mother, bread and water. The parable also contains the four elements (mythoi) of the circular monomyth: romance, tragedy, anti-romance, and comedy. Using archetypal and myth criticism, this article demonstrates that the parable has enduring attraction because its underlying archetypes appeal to a deep layer of the human psyche and to what is elemental to the human experience.

У статті підкреслюється, що на сьогоднішньому етапі розвитку українського суспільства надзвичайно актуальною є проблема правового регулювання відносин у сфері охорони здоров’я. Пацієнт є основним учасником медичних правовідносин, тому важлива увага повинна бути присвячена дослідженню його особистих немайнових прав та їхньому захисту. Автори виділяють особливості та основні ознаки пацієнта, його особисті немайнові права та обов'язки, способи та форми захисту особистих немайнових прав пацієнта. Робиться висновок, що пацієнт - це фізична особа, яка отримує медичну допомогу з приводу будь-якого захворювання, патологічного стану або іншого порушення здоров’я та життєдіяльності, а також користується медичними послугами незалежно від наявності у нього захворювання. Наводиться юридична конструкція механізму захисту прав пацієнта, яка включає об'єкт, суб'єктів, форми та гарантії захисту. У статті наголошується, що інститут захисту особистих немайнових прав пацієнта є недостатньо врегульованим у законодавстві. Особлива увага приділена праву пацієнта звернутися за захистом до Європейського Суду з прав людини. Автори зазначають, що на сьогоднішній день залишається потреба вдосконалити відповідні чинні нормативні акти з метою забезпечення умов для охорони здоров'я громадян та реалізації права на охорону здоров'я. Робиться висновок, що існує потреба в прийнятті окремого нормативного акту – Закону України «Про захист прав пацієнтів», який би відповідав сучасним реаліям та відобразив основні принципи захисту прав пацієнта, який включив би основні питання правового регулювання відносин між медичними закладами, лікарями та пацієнтами щодо забезпечення медичної допомоги, механізм правового захисту, як пацієнтів і лікарів.

The paper analyses the forms and methods of protection of family rights and interests. It is noted that judicial protection is the most effective and universal; it guarantees the correct application of the law, the establishment of real rights and responsibilities of individuals. The right of participants in family relations to judicial protection is considered, noted on the procedural features of family cases in civil proceedings in Ukraine. The concepts of “private life” and “family life” in the case law of the European Court of Human Rights are analysed. Particular attention is paid to the protection of children’s rights in court, especially in the European Court of Human Rights. The author concludes that there is a need for further comprehensive systematic analysis of the problem of legal regulation of family rights which will provide an opportunity to ensure these rights at the national level, and further guarantee them at the European level.

Objective. To establish the impact of intraintestinal pressure on the intestinal anastomosis integrity and to propose a prophylactic measures of its insufficiency in surgical treatment of the colon diseases. Materials and methods. Intraintestinal pressure was measured in 30 patients, using Waldman method during 72 h postoperatively - before and after the transanal lavage (every 4 h). Primary operation - resection of sigmoid colon with formation of a one-barrel colostomy. The restoration intervention was performed in 3 mo with formation of a large-bowel anastomosis in accordance to the method elaborated (n=17), hand-made two-layered large-bowel anastomosis (n=3), and the apparatus large-bowel anastomosis (n=10). Results. There was noted the intraintestinal pressure raising during first 24 - 36 h up to (5.3 ± 0.1) mm Hg with its further lowering during 12 - 20 h down to (2.1 ± 0.3) mm Hg. In 20 min after the transanal drain lavage the intraintestinal pressure have lowered by (3.0 ± 1.3) mm Hg, and in 3 - 5 h - gradually raised. While application of a hand-made two-layered large-bowel anastomosis its insufficiency was observed (n=1) and anastomositis (n=1); and in the apparatus large-bowel anastomosis formation - anastomositis (n=1). While disorder of the intestinal suture healing the intraintestinal pressure have raised in 16 - 60 h up to (11.4 ± 0.2) mm Hg with peak at (16.3 - 17.1) mm Hg, which persisted 12 -18 h. Prophylactic measures were directed towards lowering of intraintestinal pressure and elimination of unfavorable local and general factors. Conclusion. While persisting noncorrected, the intraintestinal pressure in the 16.3 - 17.1 mm Hg borders during 12 - 18 h leads to complicated course of postoperative period. Direct and nondirect methods of decompression of the interintestinal anastomoses constitutes a basic prophylactic principle for it.

Numerous stages of organismal development rely on the cellular interpretation of gradients of secreted morphogens including members of the Bone Morphogenetic Protein (BMP) family through transmembrane receptors. Early gradients of BMPs drive dorsal/ventral patterning throughout the animal kingdom in both vertebrates and invertebrates. Growing evidence in Drosophila, zebrafish, murine and other systems suggests that BMP ligand heterodimers are the primary BMP signaling ligand, even in systems in which mixtures of BMP homodimers and heterodimers are present. Signaling by heterodimers occurs through a hetero-tetrameric receptor complex comprising of two distinct type one BMP receptors and two type II receptors. To understand the system dynamics and determine whether kinetic assembly of heterodimer-heterotetramer BMP complexes is favored, as compared to other plausible BMP ligand-receptor configurations, we developed a kinetic model for BMP tetramer formation based on current measurements for binding rates and affinities. We find that contrary to a common hypothesis, heterodimer-heterotetramer formation is not kinetically favored over the formation of homodimer-tetramer complexes under physiological conditions of receptor and ligand concentrations and therefore other mechanisms, potentially including differential kinase activities of the formed heterotetramer complexes, must be the cause of heterodimer-heterotetramer signaling primacy. Further, although BMP complex assembly favors homodimer and homomeric complex formation over a wide range of parameters, ignoring these signals and instead relying on the heterodimer improves the range of morphogen interpretation in a broad set of conditions, suggesting a performance advantage for heterodimer signaling in patterning multiple cell types in a gradient.

Abstract Background Primary spontaneous pneumothorax is a common disorder occurring in young adults without underlying lung disease. Although tobacco smoking is a well-documented risk factor for spontaneous pneumothorax, an association between electronic cigarette use (that is, vaping) and spontaneous pneumothorax has not been noted. We report a case of spontaneous pneumothoraces correlated with vaping. Case presentation An 18-year-old Caucasian man presented twice with recurrent right-sided spontaneous pneumothoraces within 2 weeks. He reported a history of vaping just prior to both episodes. Diagnostic testing was notable for a right-sided spontaneous pneumothorax on chest X-ray and computed tomography scan. His symptoms improved following insertion of a chest tube and drainage of air on each occasion. In the 2-week follow-up visit for the recurrent episode, he was asymptomatic and reported that he was no longer using electronic cigarettes. Conclusions Providers and patients should be aware of the potential risk of spontaneous pneumothorax associated with electronic cigarettes.

The American Academy of Pediatrics published expanded guidelines for infant safe sleep in 2011, expanding the definition from “back to sleep” to “safe to sleep,” more fully describing risk factors and guidelines. In 2016, the guidelines were revised to promote “providers modeling safe sleep behavior” to the highest level of recommendation. Previous studies have addressed the difficulty in creating clear, consistent communication between health care providers and families during an infant’s inpatient stay. This institutional update describes an interprofessional and family-centered quality improvement project to improve sleep safety for hospitalized infants through a multimodal approach. Five family-centered interventions were designed: a designated safe sleep web page, a clear bedside guide to safe sleep, additional training for nursing staff in motivational interviewing, a Kamishibai card audit system, and electronic health record smart phrases. These coordinated interventions reflect advantages of an interprofessional and family-centered approach: building rapport and achieving improvements to infant sleep safety.
