Background Pre-existing cardiovascular disease (CVD) or cardiovascular risk factors have been associated with an increased risk of complications following hospitalisation with COVID-19, but their impact on the rate of recovery following discharge is not known.Objectives To determine whether the rate of patient-perceived recovery following hospitalisation with COVID-19 was affected by the presence of CVD or cardiovascular risk factors.Methods In a multicentre prospective cohort study, patients were recruited following discharge from the hospital with COVID-19 undertaking two comprehensive assessments at 5 months and 12 months. Patients were stratified by the presence of either CVD or cardiovascular risk factors prior to hospitalisation with COVID-19 and compared with controls with neither. Full recovery was determined by the response to a patient-perceived evaluation of full recovery from COVID-19 in the context of physical, physiological and cognitive determinants of health.Results From a total population of 2545 patients (38.8% women), 472 (18.5%) and 1355 (53.2%) had CVD or cardiovascular risk factors, respectively. Compared with controls (n=718), patients with CVD and cardiovascular risk factors were older and more likely to have had severe COVID-19. Full recovery was significantly lower at 12 months in patients with CVD (adjusted OR (aOR) 0.62, 95% CI 0.43 to 0.89) and cardiovascular risk factors (aOR 0.66, 95% CI 0.50 to 0.86).Conclusion Patients with CVD or cardiovascular risk factors had a delayed recovery at 12 months following hospitalisation with COVID-19. Targeted interventions to reduce the impact of COVID-19 in patients with cardiovascular disease remain an unmet need.Trail registration number ISRCTN10980107.

Introduction: Police officers are exposed to a variety of traumatic events, which can be physical or psychological. This study aimed to identify factors that influence Post-traumatic growth (PTG) in South Korean police officers, according to age group. Methods: Raw data were collected from September 26 to October 9, 2017, for 269 police officers who are employed at 10 police offices in Seoul. PTG was affected significantly by age, marital status, monthly income, and police rank in pain perception and social support variables among general characteristics. Results: Factors that affected PTG in the ‘20~29’ age group were resilience and pain perception, but in the ‘30~39’ age group, only Pain perception was significant, and in the ‘over 50’ age group, social support and pain perception were significant. Conclusions: The development of mental health programs for police should consider the age group of the patients. Mental health care should also be continuous.

A high proportion of HIV-positive patients in South Africa receive concomitant efavirenz (EFV) and isoniazid (INH) therapy. EFV is metabolised in the liver via CYP2B6, and genetic polymorphism of CYP2B6 is known to result in slowed metabolism of the drug. INH is also metabolised in the liver, causing inhibition of a pathway that plays an important role in slow EFV metabolisers. Concomitant INH use therefore affects plasma levels of EFV. EFV is well known to cause neuropsychiatric side-effects on initiation, and a recent adult case series described late-onset neurotoxicity in the form of subacute ataxia and encephalopathy in patients treated with EFV for a median of 2 years, in association with toxic plasma levels of the drug. We have seen an increase in cases of EFV toxicity presenting to our neurology referral unit. All cases have been in the context of recent initiation of concomitant INH. We therefore conducted a retrospective case record audit to describe these seven cases with the additional advantage of tertiary-level assessment. We outline the clinical features and investigation results, as well as outcomes after EFV was stopped. Our main objectives are to highlight the probable role of concomitant INH use in the development of this syndrome, and to suggest that only limited work-up may be warranted in suspected cases.

This article presents the ENglish Reading Online (ENRO) project that offers data on English reading and listening comprehension from 7,338 university-level advanced learners and native speakers of English representing 19 countries. The database also includes estimates of reading rate and seven component skills of English, including vocabulary, spelling, and grammar, as well as rich demographic and language background data. We first demonstrate high reliability for ENRO tests and their convergent validity with existing meta-analyses. We then provide a bird's-eye view of first (L1) and second (L2) language comparisons and examine the relative role of various predictors of reading and listening comprehension and reading speed. Across analyses, we found substantially more overlap than differences between L1 and L2 speakers, suggesting that English reading proficiency is best considered across a continuum of skill, ability, and experiences spanning L1 and L2 speakers alike. We end by providing pointers for how researchers can mine ENRO data for future studies.

Teacher education programs are increasingly taking up commitments to prepare new teachers for equitable teaching. Despite best intentions, programs feel challenged to help candidates translate these commitments into classroom practice. Using a context-specific teacher education framework, we conducted a mixed-methods study of seven urban-focused programs to understand how they targeted preparation for urban contexts. We found that while programs offer multiple opportunities to learn about content embedded in context, fewer opportunities exist for candidates to practice in context, and that faculty play a critical bridging role in designing practice opportunities that are informed by program vision.

Since 2012, the UC Sustainable Agriculture Research and Education Program (SAREP) has worked with the Yolo County Department of Agriculture to support farm to school activities in Yolo County. In 2015, SAREP partnered with the Yolo County Department of Agriculture to deepen engagement with Yolo County growers and increase direct sales to Yolo County schools. SAREP tracked the volumes and prices of produce purchased by five school districts for the 2014–2015 baseline year and the 2015–2016 school year. Analysis was completed for three school districts for common produce items purchased, increases in in-season purchasing and direct grower versus distributor sales. For these districts, 17 produce items were in the top 10 for at least one of the districts; the five most common were apples, bananas, lettuce, oranges and strawberries, four of which are available locally for some or all of the school year. Districts purchased between 50% and 75% of their produce in season by the end of year two. All districts increased their purchases directly from growers. Findings suggest how services for growers and school food buyers can contribute to more local procurement.

Geological carbon sequestration (GCS) involves injecting CO[Formula: see text] into subsurface geological formations for permanent storage. Numerical simulations could guide decisions in GCS projects by predicting CO[Formula: see text] migration pathways and the pressure distribution in storage formation. However, these simulations are often computationally expensive due to highly coupled physics and large spatial-temporal simulation domains. Surrogate modelling with data-driven machine learning has become a promising alternative to accelerate physics-based simulations. Among these, the Fourier neural operator (FNO) has been applied to three-dimensional synthetic subsurface models. Despite its good accuracy in simulating CO[Formula: see text] plume migration, it requires large computational resources in training and also lacks generalizability. Here, to further improve performance, we have developed a nested Fourier-DeepONet by combining the expressiveness of the FNO with the modularity of a deep operator network (DeepONet). This new framework is twice as efficient as a nested FNO for training and has at least 80% lower GPU memory requirement due to its flexibility to treat temporal coordinates separately. These performance improvements are achieved without compromising prediction accuracy. In addition, the generalization and extrapolation ability of nested Fourier-DeepONet beyond the training range has been thoroughly evaluated. Nested Fourier-DeepONet outperformed the nested FNO for extrapolation in time with more than 50% reduced error. It also exhibited good extrapolation accuracy beyond the training range in terms of reservoir properties, number of wells, and injection rate.

Background The adoption of avoidance diets by adult-onset asthmatics has not previously been studied. We hypothesized that avoidance diets would associate with adult-onset asthma, allergy, and aspirin-exacerbated respiratory disease (AERD).Methods A total of 1247 subjects with adult-onset asthma (age range: 31–91) from the Finnish national registry, and age- and sex-matched controls (n = 1970) participated in a questionnaire study in 1997. We estimated the association between asthma/allergy/AERD and avoidance diets, adjusting for potential confounding factors and validated the results in two retrospective cohorts of 5080 rhinitis/rhinosinusitis patients and 167 AERD patients from 2019 to 2020.Results The presence of asthma positively associated with adoption of any avoidance diet (adjusted OR [CI95%] 1.24 [1.02–1.51], p = 0.029) as did allergic disease and self-reported AERD within the asthmatic group (1.79 [1.29–2.48], p = 0.001 and 1.69 [1.15–2.49], p = 0.007, respectively). Asthmatics and allergic asthmatics were more likely to report avoidance of fish, fruits and vegetables, and spices (p ≤ 0.03) compared to controls and non-allergic asthmatics. The adjusted OR for multiple diets among AERD patients was 2.57 [1.34–4.95] p = 0.005. In the validation, 26.2% of the allergic asthmatics and 10.8% of AERD patients had documented avoidance diets.Conclusions Our study shows a positive association between avoidance diets and adult-onset asthma, and with allergic disease or AERD within asthmatic patients. Although we lack information on the reason patients chose to observe a specific diet, our results reinforce the importance of asking patients about their diet and if needed, giving dietary advice for adult asthma patients to help them avoid the adoption of unnecessarily restrictive diets.

Numerous primary studies and a recent meta-analytic confirmatory factor analysis (Meta-CFA; Said-Metwaly, Fernández-Castilla, Kyndt, & Van den Noortgate, 2018) have shown that Torrance Tests of Creative Thinking -- Figural (TTCT-F) consists of two factors. However, recent research has raised questions regarding factor analysis of the TTCT-F due to poor discriminant validity and artificial clustering of indices. Additionally, the original meta-analysis did not include databases of dissertations and theses. In this study, we replicated and extended the meta-CFA study of the TTCT-F using 56 correlation matrices from 45 studies and a total of 11,142 participants. We investigated whether the problem of discriminant validity, participants' age, the use of Form A or B, and the use of standardized or raw scores influence the factor structure. Our results show that a two-factor structure remains stronger than a one-factor structure in both replication and extension studies, and this conclusion is robust to score type, participant age, and issues related to discriminant validity. However, further research is needed to determine if this structure applies to less commonly used Form B, which had different factor loadings from Form A. We discuss the implications of our results for creativity research, the psychometrics of TTCT-F, and gifted identification.

A perceived increase in the number of isolates of Moraxella catarrhalis from the respiratory secretions of patients intubated in the pediatric intensive care unit prompted a review of the clinical profiles of such patients and restriction enzyme analysis of the strains involved. Over two months, of 192 patients admitted to the unit, 154 were intubated. Of the 46 for whom endotracheal tube specimens were submitted to the laboratory, M catarrhalis was isolated in 12. M catarrhalis was not felt to be a significant respiratory pathogen by the attending medical staff in any of the patients from whom it was isolated. In only two patients (17%) could nosocomial acquisition be firmly invoked. Restriction enzyme analysis of the 12 strains ruled out the presence of an epidemic strain. Isolation of M catarrhalis from intubated children does not necessarily imply pathogenicity nor an outbreak situation.

A 19-year-old immunocompetent cystic fibrosis patient with recurrent neck abscesses due to a multiresistant Pseudomonas cepacia is described. After 13 drainage procedures over a two-year period, a trial of interferon-gamma therapy to enhance monocyte function was attempted. The patient had one minor recurrence but has otherwise been symptom free for almost two years. P cepacia is an unusual cause of extrapulmonary abscess formation. Such abscesses may not present with classical signs of inflammation, are likely to be multiresistant and to require surgical drainage. Immunotherapy may be justified in the immunocompetent host when infection is refractory to medical and surgical therapy.

Persons with serious mental illnesses experience high rates of medical comorbidity, especially diabetes. This study examined initial implementation feasibility, acceptability, and appropriateness of a new 6-month Multicomponent Intervention for Diabetes risk reduction in Adults with Serious mental illnesses (MIDAS) among persons in residential care facilities (RCFs). We conducted a mixed-methods study using four types of quantitative and qualitative data sources (administrative data; structured facility-level observations; resident assessments including blood-based biomarkers, 24-h dietary recalls, and self-report physical activity; and focus groups/interviews with staff and participants), to assess evidence of and factors affecting intervention feasibility, acceptability, and appropriateness. It was feasible to provide a high percentage of MIDAS class sessions (mean 50 of 52 intended sessions delivered) and make nutrition-related RCF changes (substitutions for healthier food items and reduced portion sizes). Class attendance rates and positive feedback from residents and staff provided evidence of MIDAS acceptability and appropriateness for addressing identified health needs. The residents who attended [greater than or equal to] 85% of the sessions had greater improvement in several desired outcomes compared to others. Implementing a fully integrated MIDAS model with more extensive changes to facilities and more fundamental health changes among residents was more challenging. While the study found evidence to support feasibility, acceptability, and appropriateness of individual MIDAS components, some challenges for full implementation and success in obtaining immediate health benefits were also apparent. The study results highlight the need for improving health among RCF populations and will inform MIDAS adaptations designed to improve intervention fit and effectiveness outcomes.

This article examines the long-overlooked governmental body, the local community college governing board, through the lens of descriptive representation, to assess what variables are associated with minority representation. Higher education governing boards have long lacked diversity, let alone any type of proportional representation, but an examination of descriptive representation of local community college governing boards, which oversee a large portion of minority undergraduates, has been missing from the literature. Using OLS regression and logit methods for analysis of original data, the results provide evidence that those boards are far from descriptively representative and that social and environmental characteristics impact board diversity. The findings indicate that board size and the percentage of Black residents can play a significant role in the selection of Blackboard members, establishing an important empirical foundation in light of descriptive representation scholars' findings that descriptive representation can lead to substantive benefits. Given the important role of community college governing bodies that serve approximately half of the nation's minority undergraduates, this research is important as a stepping stone for further study of community college governing boards, who serves on them and why that matters.

Introduction: HyperArc (HA) auto‐planning offers simplicity for the end user and consistently high‐quality SRS plans. The "Ask For It" (AFI) optimization strategy offers a manual planning technique that, when coupled with R50%Analytic, can be guided to deliver a plan with an intermediate dose spill "as low as reasonably achievable" and high target dose conformity. A direct comparison of SRS plan quality obtained using the manual planning AFI strategy and HA has been performed. Methods: Using a CT data set available from the Radiosurgery Society, 54 PTVs were created and used to generate 19 individual SRS/SRT cases. Case complexity ranged from single PTV plans to multiple PTV plans with a single isocenter. PTV locations ranged from relative isolation from critical structures to lesions within 1.5 mm of the optic apparatus and abutting the brainstem. All cases were planned using both the AFI and HA optimization strategies as implemented in the Varian Medical Systems Eclipse Treatment Planning System. A range of treatment plan quality metrics were obtained including Intermediate Dose Spill (R50%), Conformity Indices CIRTOG and CIPaddick, PTV Dose Coverage (Dn%), PTV Mean Dose, and Modulation Factor. The Wilcoxon Signed Rank Sum non‐parametric statistical method was utilized to compare the obtained plan quality metrics. Results: Statistically significant improvements were found for the AFI strategy for metrics R50%, CIRTOG, CIPaddick, and PTV Mean Dose (p < 0.001). HA achieved superior coverage for Dn% (p = 0.018), while the Modulation Factors were not significantly different for AFI compared to HA optimization (p = 0.13). Conclusion: This study provides evidence that the AFI manual planning strategy can produce high‐quality planning metrics similar to the HA auto‐planning method. [ABSTRACT FROM AUTHOR]
