Background: The World Health Organization's “End TB Strategy” aims to end global tuberculosis (TB) epidemic through a holistic combination of health and social interventions placing the patients and communities at the heart of the response. This study aimed to assess the effectiveness of utilizing school children as ambassadors in TB advocacy. Materials and Methods: We adopted a quasi-experimental intervention design where students' awareness level was assessed before and after the intervention. A total of 185 student ambassadors were trained to conduct interventions in schools, and 920 students were randomly selected to assess the impact of the ambassador's intervention. A structured questionnaire was used to assess the correct and incorrect knowledge on specific aspects of TB. This intervention study was implemented in a phased manner which involved a participatory formative phase. A student-friendly and culturally relevant educational materials and activities for providing TB knowledge for the study student population were developed. Data collected from the baseline and end-line evaluation surveys were analyzed using STAT Ver. 16.0.– Stata Corp., June 2016, USA. Results: A significant increase (>80%; P < 0.05) in the correct knowledge on diagnosis and prevention for TB was noted among sampled students (n = 818) before and after intervention. Reduction in incorrect knowledge, like understanding TB as hereditary disease, was found to be less (50%; P < 0.05). Conclusions: School students lead intervention could significantly improve correct knowledge on TB and could be replicated.

Objectives Informing research participants of the results of studies in which they took part is viewed as an ethical imperative. However, there is little guidance in the literature about how to do this. The Fluoxetine Or Control Under Supervision trial randomised 3127 patients with a recent acute stroke to 6 months of fluoxetine or placebo and was published in the Lancet on 5 December 2018. The trial team decided to inform the participants of the results at exactly the same time as the Lancet publication, and also whether they had been allocated fluoxetine or placebo. In this report, we describe how we informed participants of the results.Design In the 6-month and 12-month follow-up questionnaires, we invited participants to provide an email address if they wished to be informed of the results of the trial. We re-opened our trial telephone helpline between 5 December 2018 and 31 March 2019.Setting UK stroke services.Participants 3127 participants were randomised. 2847 returned 6-month follow-up forms and 2703 returned 12-month follow-up forms; the remaining participants had died (380), withdrawn consent or did not respond.Results Of those returning follow-up questionnaires, a total of 1845 email addresses were provided and a further 50 people requested results to be sent by post. Results were sent to all email and postal addresses provided; 309 emails were returned unrecognised. Seventeen people replied, of whom three called the helpline and the rest responded by email.Conclusion It is feasible to disseminate results of large trials to research participants, though only around 60% of those randomised wanted to receive the results. The system we developed was efficient and required very little resource, and could be replicated by trialists in the future.Trial registration number ISRCTN83290762; Post-results.

6. Qualitative Case Study Analysis: Increasing the Utilization Services for a School-Based Health Center from the Perspective of Teachers and School Administrators

School-based health centers (SBHCs) are a model of healthcare where services are delivered in clinics on school campuses (Kjolhede et al., 2021; Harris et al., 2016), with research indicating the delivery model has several educational benefits (Kjolhede et al., 2012; Arenson et al., 2019; Knop et al., 2016). Research findings supporting the use of SBHC; yet, these centers report low utilization of services. Teachers were identified as a main source of referral for students to SBHCs due to the amount of time teachers spend with students (Wade et al. 2008), connecting them as a driving source of inquiry for increasing further use of services for SBHCs. This study explores how to increase the use of services for a site-specific SBHC from the perspective of teachers and school administrators. The researcher used a case study methodology with data collected through eight semi-structured interviews with teachers and administrators at the research site. The findings report several themes contributing to underutilizing services for the SBHC: lack of information and communication on processes and procedures, under-developed relationships between SBHC and teachers and administrators, absence of teacher empowerment, and need for teacher and parent engagement with the SBHC. The findings were transferable with other SBHC studies; a formative concept of empowerment is unique to this study. compared to the three data sources used to create codes for the literature. Based on the findings, three recommendations were generated to improve the problem of practice: the research site should develop a formalized process for its SBHC and share information on the resources and processes concerning the SBHC with teachers; cultivate trust and develop meaningful relationships between teachers and the SBHC; and, empower teachers, structurally with policy development and psychologically with creating meaning on the impact and importance of referring students to the SBHC, within SoE. The implications of this study concerning SBHCs and increasing use of services were in alignment and transferable to previous findings; however, most of the literature discusses the importance of relationships with teachers and SBHC staff, but few studies identify empowerment as a strategy to increase the use of services for SBHCs. Empowerment was a seminal concept revealed in this study regarding elementary education in the field of preventative healthcare services. Additional studies are needed to explore what elements and activities of empowerment had the greatest impact on referral of services for SBHCs. In closing, information, trust, communication, and teacher empowerment are elements for consideration for increasing the use of services for SBHCs. 7. A Scholarly Dialogue: Writing Scholarship, Authorship, Academic Integrity and the Challenges of AI

- Abstract
Concerns about the role of technology and the quality of student writing in higher education are not new. Historically, writing scholars have been at the forefront of initiatives that scrutinise and integrate new technologies in higher education. This article contends that writing scholars are again uniquely equipped to assist students, teachers in all disciplines, and institutions of higher education in navigating the challenges created by the public availability of generative artificial intelligence (AI). Adopting a dialogical approach, the article brings together six scholars from diverse disciplinary backgrounds within the broad umbrella of writing studies. Through the lens of writing scholarship, this chorus of critical voices illuminates the important questions posed by, and possible responses to, AI in higher education. Although AI complicates key issues in higher education such as academic integrity, assessment, and authorship, writing scholarship provides an essential framework for educators to respond to these challenges. Collectively, these scholars map the history of writing scholars' responses to technological change in higher education and suggest how writing scholars can contribute to the debates and discussions concerning the impact of AI on higher education.

Specialized endocrine cells produce and release steroid hormones that govern development, metabolism and reproduction. In order to synthesize steroids, all the genes in the biosynthetic pathway must be coordinately turned on in steroidogenic cells. In Drosophila, the steroid producing endocrine cells are located in the prothoracic gland (PG) that releases the steroid hormone ecdysone. The transcriptional regulatory network that specifies the unique PG specific expression pattern of the ecdysone biosynthetic genes remains unknown. Here, we show that two transcription factors, the POU-domain Ventral veins lacking (Vvl) and the nuclear receptor Knirps (Kni), have essential roles in the PG during larval development. Vvl is highly expressed in the PG during embryogenesis and is enriched in the gland during larval development, suggesting that Vvl might function as a master transcriptional regulator in this tissue. Vvl and Kni bind to PG specific cis-regulatory elements that are required for expression of the ecdysone biosynthetic genes. Knock down of either vvl or kni in the PG results in a larval developmental arrest due to failure in ecdysone production. Furthermore, Vvl and Kni are also required for maintenance of TOR/S6K and prothoracicotropic hormone (PTTH) signaling in the PG, two major pathways that control ecdysone biosynthesis and PG cell growth. We also show that the transcriptional regulator, Molting defective (Mld), controls early biosynthetic pathway steps. Our data show that Vvl and Kni directly regulate ecdysone biosynthesis by transcriptional control of biosynthetic gene expression and indirectly by affecting PTTH and TOR/S6K signaling. This provides new insight into the regulatory network of transcription factors involved in the coordinated regulation of steroidogenic cell specific transcription, and identifies a new function of Vvl and Knirps in endocrine cells during post-embryonic development.

9. Intervention Characteristics Considered in Health Educators' Adoption Decision-Making Process

It is well known that perceptions of intervention characteristics (e.g., cost, source, evidence strength and quality) are a critical link from dissemination to implementation. What is less known is the process by which researchers understand the characteristics most valued by key intermediaries (i.e., real-world decision-makers), particularly in the federal system of Cooperative Extension. In Extension, university-based specialists are available to assist county-based agents in program selection, delivery, and evaluation. For this work, a sequential explanatory mixed-methods design was used to conduct surveys and semi-structured interviews, informed by the Diffusion of Innovations theory and Consolidated Framework for Implementation Research. Educators and specialists were recruited across 47 states to identify characteristics of health promotion interventions that facilitate the adoption decision-making process. Analysis of intervention attribute importance survey data was conducted through a one-way ANOVA with Bonferroni post hoc test to determine individual variable differences between responses. Interviews underwent a conventional content analysis. In total, 121 educators and 47 specialists from 33 states completed the survey. Eighteen educators and 10 specialists completed interviews. Educators and specialists valued components such as the community need for the intervention, and potential reach compared with other components including previous delivery settings and external funding of the intervention (p < 0.05). Qualitative data indicated divergence between educators and specialists; educators valued understanding the intervention cost (time and training) and specialists valued the evidence base and external funding available. Intervention developers should communicate information valued by different stakeholders to improve the adoption of evidence-based interventions.

10. Development and evaluation by a cluster randomised trial of a psychosocial intervention in children and teenagers experiencing diabetes: the DEPICTED study

Objective: To develop and evaluate a health-care communication training programme to help diabetes health-care professionals (HCPs) counsel their patients more skilfully, particularly in relation to behaviour change. Design: The HCP training was assessed using a pragmatic, cluster randomised controlled trial. The primary and secondary analyses were intention-to-treat comparisons of outcomes using multilevel modelling to allow for cluster (service) and individual effects, and involved two-level linear models. Setting: Twenty-six UK paediatric diabetes services. Participants: The training was delivered to HCPs (doctors, nurses, dietitians and psychologists) working in paediatric diabetes services and the effectiveness of this training was measured in 693 children aged 4–15 years and families after 1 year (95.3% follow-up). Interventions: A blended learning programme was informed by a systematic review of the literature, telephone and questionnaire surveys of professional practice, focus groups with children and parents, experimental consultations and three developmental workshops involving a stakeholder group. The programme focused on agenda-setting, flexible styles of communication (particularly guiding) and a menu of strategies using web-based training and practical workshops. Main outcome measures: The primary trial outcome was a change in glycosylated haemoglobin (HbA1c) levels between the start and finish of a 12-month study period. Secondary trial outcomes included change in quality of life, other clinical [including body mass index (BMI)] and psychosocial measures (assessed at participant level as listed above) and cost (assessed at service level). In addition, patient details (HbA1c levels, height, weight, BMI, insulin regimen), health service contacts and patient-borne costs were recorded at each clinic visit, along with details of who patients consulted with, for how long, and whether or not patients consulted on their own at each visit. Patients and carers were also asked to complete an interim questionnaire assessing patient enablement (or feelings towards clinic visit for younger patients aged 7–10 years) at their first clinic visit following the start of the trial. The cost of the intervention included the cost of training intervention teams. Results: Trained staff showed better skills than control subjects in agenda-setting and consultation strategies, which waned from 4 to 12 months. There was no effect on HbA1c levels (p = 0.5). Patients in intervention clinics experienced a loss of confidence in their ability to manage diabetes, whereas controls showed surprisingly reduced barriers (p = 0.03) and improved adherence (p = 0.05). Patients in intervention clinics reported short-term increased ability (p = 0.04) to cope with diabetes. Parents in the intervention arm experienced greater excitement (p = 0.03) about clinic visits and improved continuity of care (p = 0.01) without the adverse effects seen in their offspring. The mean cost of training was £13,145 per site or £2163 per trainee. There was no significant difference in total NHS costs (including training) between groups (p = 0.1). Conclusions: Diabetes HCPs can be trained to improve consultation skills, but these skills need reinforcing. Over 1 year, no benefits were seen in children, unlike parents, who may be better placed to support their offspring. Further modification of this training is required to improve outcomes that may need to be measured over a longer time to see effects. Trial registration: Current Controlled Trials ISRCTN61568050. Using a panel of eight monoclonal antibodies directed against the G, F and NP proteins of respiratory syncytial virus, 167 virus isolates from nasopharyngeal washing cultures at British Columbia Children’s Hospital during two consecutive epidemics were subgrouped. Slides made and frozen at the time of virus isolation or prepared from recovered frozen passage material, were assayed by indirect immunofluorescence. Of 85 strains tested in 1987–88, 54 (64%) were subgroup A, and 31 (36%) subgroup B. By contrast, of 82 strains tested in 1988–89 five (6%) were subgroup A and 77 (94%) subgroup B. Review of patient charts did not reveal significant differences in clinical course of patients infected with the two subgroups, but the risk of infection with subgroup A was significantly greater than the risk of subgroup B infection in younger patients.

Measurements of the branching fractions of $B_{s}^{0}\to {{K}^{*\pm }}{{K}^{\mp }}$ and $B_{s}^{0}\to {{K}^{*\pm }}{{\pi }^{\mp }}$ decays are performed using a data sample corresponding to $1.0\ {\rm f}{{{\rm b}}^{-1}}$ of proton-proton collision data collected with the LHCb detector at a centre-of-mass energy of $7\;{\rm TeV}$ , where the ${{K}^{*\pm }}$ mesons are reconstructed in the $K_{S}^{0}{{\pi }^{\pm }}$ final state. The first observation of the $B_{s}^{0}\to {{K}^{*\pm }}{{K}^{\mp }}$ decay and the first evidence for the $B_{s}^{0}\to {{K}^{*-}}{{\pi }^{+}}$ decay are reported with branching fractions \begin{eqnarray*}\begin{array}{rcl} \bf{\mathcal{B}}\left( B_{s}^{0}\to {{K}^{*\pm }}{{K}^{\mp }} \right) & = & \left( 12.7\pm 1.9\pm 1.9 \right)\times {{10}^{-6}}, \\ \bf{\mathcal{B}}\left( B_{s}^{0}\to {{K}^{*-}}{{\pi }^{+}} \right) & = & \left( 3.3\pm 1.1\pm 0.5 \right)\times {{10}^{-6}}, \\ \end{array}\end{eqnarray*} where the first uncertainties are statistical and the second are systematic. In addition, an upper limit of $\mathcal{B}\left( {{B}^{0}}\to {{K}^{*\pm }}{{K}^{\mp }} \right)\lt 0.4\ (0.5)\times {{10}^{-6}}$ is set at $90\%\ (95\%)$ confidence level.

The focus of this study centered around superintendent's responses to the emergency remote learning period caused by the COVID-19 global pandemic. Forms of remote learning have been around since the 1700s, albeit, in many different forms. Technological advances of the 21st Century have significantly increased the ability of schools to provide instruction to students not on campus. Results of this study identified two elements, reception of instruction, and delivery of instruction, as critical components to superintendents leading a district in relation to remote learning. Background: Codominant stems are a common condition of concern on landscape trees. Understanding the impact of varying degrees of stem reduction can assist practitioners in making informed pruning decisions. Methods: To determine this, a single reduction or removal cut was made on each pair of codominant stems on medium-aged Acer rubrum 'October Glory' at approximately 25%, 50%, 75%, and 100% of the diameter relative to the basal diameter at the shared union or left as nonpruned controls. Changes in stem diameter ratio, trunk diameter growth, sprout production, wound closure, and aesthetics were documented for 4 years starting in 2020. Results: Stem diameter ratio was significantly reduced in codominant stems pruned to a relative diameter of 50% or 75% after a single growing season. The 75% treatment reduced growth for 2 growing seasons. There were no significant differences in change of stem diameter ratio with any treatment after 3 growing seasons. Larger pruning cuts resulted in the growth of more sprouts and more exposed wood after 3 years. Aesthetics of trees following pruning were acceptable for nonpruned, 25%, and 50% treatments in all years but were rated as not acceptable or moderately acceptable for 75% and 100% treatments after 2 growing seasons. Trees with the 75% treatment became aesthetically acceptable after 3 growing seasons. Conclusions: Pruning codominant stems of A. rubrum 'October Glory' with a cut that is 50% or 75% the diameter of the basal diameter at the shared union is appropriate at reducing stem diameter ratio, minimizing exposed wood, and maintaining overall aesthetic appearance after 2 to 3 years. Determining the structural origins of amyloid fibrillation is essential for understanding both the pathology of amyloidosis and the rational design of inhibitors to prevent or reverse amyloid formation. In this work, the decisive roles of peptide structures on amyloid self-assembly and morphological diversity were investigated by the design of eight amyloidogenic peptides derived from islet amyloid polypeptide. Among the segments, two distinct morphologies were highlighted in the form of twisted and planar (untwisted) ribbons with varied diameters, thicknesses, and lengths. In particular, transformation of amyloid fibrils from twisted ribbons into untwisted structures was triggered by substitution of the C-terminal serine with threonine, where the side chain methyl group was responsible for the distinct morphological change. This effect was confirmed following serine substitution with alanine and valine and was ascribed to the restriction of intersheet torsional strain through the increased hydrophobic interactions and hydrogen bonding. We also studied the variation of fibril morphology (i.e., association and helicity) and peptide aggregation propensity by increasing the hydrophobicity of the peptide side group, capping the N-terminus, and extending sequence length. We anticipate that our insights into sequence-dependent fibrillation and morphological diversity will shed light on the structural interpretation of amyloidogenesis and development of structure-specific imaging agents and aggregation inhibitors.

Most of the light from blazars, active galactic nuclei with jets of magnetized plasma that point nearly along the line of sight, is produced by high-energy particles, up to around 1 TeV. Although the jets are known to be ultimately powered by a supermassive black hole, how the particles are accelerated to such high energies has been an unanswered question. The process must be related to the magnetic field, which can be probed by observations of the polarization of light from the jets. Measurements of the radio to optical polarization—the only range available until now—probe extended regions of the jet containing particles that left the acceleration site days to years earlier, and hence do not directly explore the acceleration mechanism, as could X-ray measurements. Here we report the detection of X-ray polarization from the blazar Markarian 501 (Mrk 501). We measure an X-ray linear polarization degree Π_X of around 10%, which is a factor of around 2 higher than the value at optical wavelengths, with a polarization angle parallel to the radio jet. This points to a shock front as the source of particle acceleration and also implies that the plasma becomes increasingly turbulent with distance from the shock.
