Abstract

Background COVID-19 pandemic is having a collateral health effect on delivery of surgical therapies and reports from management and maintaining surgical care are little. Purpose Investigate the impact of COVID-19 in our cardiac surgical ward and practice by scoping review of the last two years of activity, between 1st March 2019 and 28th February 2021. Methods Our Hospital is a private institution accredited with the National Health Service with a global volume load of 750 cardiac cases per year. We analized the pre-pandemic and the ongoing pandemic period on volume activity, the capacity of satisfying the needs of cardiosurgical patient and the mortality rate. Results We observed a minimal cardiac surgery caseload decline of 14.8%, probably due to environmental factor, the absence of E.D. in our institute, the rapid re-organization pathways, the hospital surveillance, the supply of personal protective equipment, the informative measures to improve patient and personnel welfare. In order to enhance the pshycological impact of this devastating event, we invited all the health workers to fill a questionnaire regarding concerns and worries about the risk of transmitting the infection to their families or to aquire it themselves. Conclusion During this devastating pandemic the rapid response in re-organization pathways and hospital surveillance allowed us to preserve the cardiac surgery volume of life-saving and non deferrable cases. Abstract

Osteoporosis affects approximately 40-50% of adult patients with beta-Thalassemia Major (beta TM). Recent data have implicated an altered modulation of the osteoprotegerin (OPG)/receptor activator of NFkB ligand (RANKL) system in the pathogenesis of beta TM-osteoporosis. OPG/RANKL system acts downstream from IL-1 alpha, IL-6 and TNF-alpha and it may be the final actor mediating the effects of these cytokines on the regulation of both postmenopausal and metabolic bone resorption. However, to date, there are no data on circulating levels of these pro-resorptive cytokines in beta TM patients. We investigated the potential relationships among these cytokines, several markers of bone turnover and bone mineral density (BMD) in beta TM patients. IL-1 alpha, IL-6 and TNF-alpha, OPG and RANKL serum levels, hemato-urinary bone remodeling markers and bone mineral density (BMD) at L2L4 and femoral neck as well as erythropoietin (EPO), 17beta-estradiol, and free-testosterone levels were measured in 30 well treated beta TM patients and in 20 healthy subjects, matched for age, sex and BMI with the patients. beta TM patients showed an altered bone turnover, with increased deoxypyridinoline (D-PYR) levels (P

Abstract

Objective: To investigate whether MS is associated with erectile dysfunction (ED) among obese non diabetic individuals. Methods: A cross-sectional study was carried out to examine the association between the cluster of abnormalities related to the MS and ED as evaluated by the International Index of Erectile Function (IIEF). Fifty consecutive obese [i.e. body mass index (BMI) ≥30 kg/m2], non-diabetic whites (age 42.1±11.3 yr, BMI 43.3±8.7 kg/m2) were recruited. Results: The prevalence of MS as well as that of any MS component were not different between subjects with or without ED. Neither the prevalence of ED (34.3% vs 33.4%, p=0.6), nor IIEF score (21.5±3.9 vs 21.7±3.7, p=0.8), were different between patients with or without MS. IIEF was similar across subgroups of individuals stratified according to the number of MS components and was not related to HOMAIR index. Hypogonadism was observed in 30.8% and 28.1% individuals with and without MS (p=0.58). Testosterone and BMI levels were inversely related (r=−0.3, p=0.04). Conclusion: Among obese non-diabetic individuals the risk of developing ED is independent of the presence of MS factors. Testosterone levels progressively decrease with increasing body weight.

Abstract

Objective: To evaluate in a group of postmenopausal women the effects of long-term raloxifene treatment on breast density using a digitized analysis of mammograms and on insulinlike growth factor-1 (IGF-1), insulinlike growth factor binding protein-3 (IGFBP-3), and sex hormone-binding globulin (SHBG) plasma levels. Design: Seventy healthy postmenopausal women with normal body weight were enrolled in this study and were divided into two groups based on their bone status, evaluated by dual-energy x-ray at the lumbar spine (L2-4). Fifty women (chronological age 52.4 ± 4.1 y, menopausal age 42.1 ± 3.9 y), in whom the L2-4 T score was less than −2.5 SD, were treated with raloxifene HCl 60 mg/day orally for 2 years. The other 20 women (chronological age 53.6 ± 3.5 y, age at menopause 43.1 ± 3.6 y), in whom the L2-4 T score ranged between −1 and −2.5 SD, were enrolled as controls. All 70 women received calcium (1 g/d orally) and cholecalciferol (880 UI/d orally) supplementation. Moreover, all women followed a normocaloric and personalized diet. All women had mammography at baseline and after 2 years of therapy. The mammographic images on traditional support (radiography) were acquired by using a film scanner and were then elaborated by means of ad hoc software. Moreover, assessments of IGF-1, IGFBP-3, and SHBG plasma levels were obtained at baseline and after 24 months. Results: After 24 months of therapy, there was a significant variation in the raloxifene-treated group with respect to baseline in the distribution of gray classes of radiographic images. In particular, an attenuation of graphic trace with a reduction of the areas with the lowest and most elevated gray classes was observed. In the control group, no significant variations of graphic traces were observed. Moreover, raloxifene treatment significantly reduced IGF-1 and increased IGFBP-3 and SHBG plasma levels at 24 months. During follow-up, IGF-1, IGFPB-3, and SHBG levels did not change significantly in the control group. Conclusions: Long-term treatment with raloxifene in a population of postmenopausal women is able to reduce breast density. Such an effect could perhaps explain the reduction in the incidence of mammary carcinoma observed in the Multiple Outcomes of Raloxifene Evaluation study probably due to the direct antiestrogenic activity of raloxifene on mammalian tissue and/or its indirect activity increasing SHBG levels or modifying the IGF-1/IGFBP-3 ratio.

Abstract

The tagged quasi-free np→npπ+π− reaction has been studied experimentally with the High Acceptance Di-Electron Spectrometer (HADES) at GSI at a deuteron incident beam energy of 1.25 GeV/nucleon (s∼2.42 GeV/c for the quasi-free collision). For the first time, differential distributions of solid statistics for π+π− production in np collisions have been collected in the region corresponding to the large transverse momenta of the secondary particles. The invariant mass and angular distributions for the np→npπ+π− reaction are compared with different models. This comparison confirms the dominance of the t-channel with ΔΔ contribution. It also validates the changes previously introduced in the Valencia model to describe two-pion production data in other isospin channels, although some deviations are observed, especially for the π+π− invariant mass spectrum. The extracted total cross section is also in much better agreement with this model. Our new measurement puts useful constraints for the existence of the conjectured dibaryon resonance at mass M∼2.38 GeV and with width Γ∼70 MeV.

Abstract

The pion form factor has been measured in the space-like q 2 region 0.014 to 0.26 (GeV/ c ) 2 by scattering 300 GeV pions from the electrons of a liquid hydrogen target. A detailed description is given of the apparatus, data analysis and corrections to the data. The mean square charge radius extracted from the data is model-dependent. We find that a form which includes a realistic description of the form factor phase gives a similar results to the naive pole form, and conclude 〈r 2 π 〉 = 0.438±0.008 fm 2 .

Abstract

La donazione di abiti a Palazzo Morando, Museo della Moda del Comune di Milano, è stata l'occasione per esaminare questi manufatti anche dal punto di vista sartoriale inserendo lo studio nella cornice della cultura materiale, che mette in dialogo le fonti materiali con quelle documentarie del XVIII secolo. Attraverso l'analisi della trattatistica dei secoli XVI-XVIII, il saggio ricostruisce il contesto storico in cui gli abiti oggetto della donazione sono stati concepiti, prodotti, indossati; ripercorrendo la storia della sartoria dal Medioevo all'Epoca moderna, questo studio mette in evidenza il ruolo delle donne nell'ambito dei nuovi mestieri riconducibili alla sartoria, i nuovi rituali connessi al rapporto fra sarto e cliente, il valore economico delle vesti.
