INTRODUCTION: The immunomodulatory roles of vitamin D and vitamin D binding protein (VDBP) are in interest with incidence or outcome of coronavirus disease-2019 (COVID-19). This study aimed to investigate the association between the severity of COVID-19 with VDBP, total 25-hydroxy vitamin D (25(OH)D), and its metabolites free vitamin D (VDfree) and bioavailable vitamin D (VDbio).METHODS: Study group consisted of 68 COVID-19 patients and 20 healthy subjects. Patients were subgrouped as asymptotic, mild/moderately pneumonia, or severe pneumonia. Plasma total 25(OH)D was quantitated by liquid chromatography with mass spectrometry and serum VDBP by a polyclonal sandwich enzyme immunoassay. In addition, routinely used laboratory parameters in follow-up were recorded. VDfree and VDbio were calculated using total 25(OH)D, VDBP, and albumin levels.RESULTS: Plasma total 25(OH)D (13.3± 5.7 vs 30.3±13.3 ng/dL), VDfree [2.18 (1.52-3.44) vs 4.34 (3.74-6.48) pg/mL], and VDbio [1.86 (1.09-2.81) vs 4.28 (3.45-6.34) nmol/L)] levels were lower in COVID-19 patients (pDISCUSSION AND CONCLUSION: COVID-19 patients have lower plasma 25(OH)D levels and lower 25(OH)D metabolites VDfree, VDbio which are physiologically active. Additionally, serum VDBP concentrations significantly decrease in critically ill patients which needs further studies to be associated in the etiopathogenesis of the disease severity.Keywords:COVID-19, vitamin D binding protein, 25(OH)D, free vitamin D, bioavailable vitamin D

Tetralogy of Fallot is the most common cyanotic congenital heart disease. The present case is an adult COVID-19patient with a history of Tetralogy of Fallot, who is a healthcare worker. High Flow Nasal Oxygen therapy was used due torespiratory distress. In addition, Mesenchymal Stem Cells were applied while the disease was in critical process. The clinicaland laboratory improvement were observed in the patient from the second day of treatment of Mesenchymal Stem Cells. Response to the antiviral treatments given to the patient could not be obtained. HFNO and MSCs were used in the treatment of the patient. Clinical and laboratory improvement was observed in the patient from the second day of treatment of MSCs.

Background/aim: Remdesivir, which was first developed for the treatment of Ebola disease but failed to meet expectations, has become hope in the fight against the COVID-19 pandemic. This study aimed to evaluate risk factors for mortality and prognosis of adult moderate/severe COVID-19 patients treated with remdesivir, and safety and tolerability of 5 days of remdesivir treatment. Materials and methods: This multicenter prospective observational study was conducted in 14 centers in Turkey. Pregnancy or breastfeeding, multiorgan failure, or usage of vasopressors for septic shock, ALT>5 x the upper limit of the normal range, or eGRF, Health Institutes of Turkey [COVID-19-PMSRMD], Health Institutes of Turkey (Project ID: COVID-19-PMSRMD).

Çok ilaca dirençli gram negatif bakteri enfeksiyonları tüm dünyada büyük bir sorun olarak her geçen gün önemini artırmaktadır. Karbapenem dirençli enfeksiyonlarda, tedavi seçenekleri sınırlı kaldığından, mortalite ve morbidite yüksek seyretmektedir. Hastanelerde enfeksiyon kontrol uygulamaları ile karbapenem dirençli mikroorganizmaların yayılımının önlenmesi, bu enfeksiyonların yönetiminde önemli bir basamaktır. Bu makalede, karbapenem dirençli gram negatif bakterilerin kontrolü için hastanelerde alınacak önlemler derlenmiştir.

Coronavirus disease 2019 (COVID-19), caused by the SARS-CoV-2 virus, has become a global health threat. COVID-19 often involves the respiratory tract, however, it is also associated with several neurological conditions (seizures, convulsions, changes in consciousness, encephalitis, cerebral hemorrhage, and cerebral thrombosis). Cavernous malformations (CMs) are vascular lesions that occur in the central nervous system and the body. Symptomatic CMs most manifest through headaches, seizures, hemorrhage, or focal neurological deficits. The co-occurrence of COVID-19 and cavernous hemangioma hemorrhage has not been reported in the literature. The association between COVID-19 and intracerebral hemorrhage remains controversial. However, a relationship with COVID-19 infection in patients with intracerebral hemorrhage should always be considered during this pandemic. The case of a 37-year-old female patient is reported here. The patient presented to the emergency room with a headache that persisted for 2 days along with cavernous hemangioma hemorrhage and tested positive for COVID-19., SARS-CoV-2 virüsünün neden olduğu Coronavirus hastalığı 2019 (COVID-19), küresel bir sağlık tehdidi haline geldi. COVID-19 genellikle solunum yolunu tutar; bununla birlikte çeşitli nörolojik durumlarla (nöbetler, konvülsiyonlar, bilinç değişiklikleri, ensefalit, serebral hemoraji ve serebral tromboz) ilişkili olduğu da bulunmuştur. Kavernöz malformasyonlar (KM), merkezi sinir sisteminde ve vücutta meydana gelen vasküler lezyonlardır. Semptomatik KM'ler en yaygın olarak baş ağrısı, nöbetler, kanama veya fokal nörolojik defisitlerle kendini gösterir. Literatürde COVID-19 ile kavernöz hemanjiyom kanamasının birlikte görülmesi bildirilmemiştir. COVID-19 ile intraserebral hemoraji arasındaki ilişki tartışmalıdır. Bununla birlikte, intraserebral hemorajili hastalarda COVID-19 enfeksiyonu ile bir ilişki bu pandemi sırasında her zaman dikkate alınmalıdır. 37 yaşında bir kadın hastanın vakası burada bildirilmektedir. COVID-19 enfeksiyonlu olgularda hastaların hayat kalitesini ciddi anlamda etkileyen tat ve koku kaybının bir an önce düzelmesi için homeopati uygulaması yaptığımız üç hastanın klinik takibini ortaya koymayı amaçladık. Yaşları sırasıyla 27, 52 ve 53 olan üç erkek hastadan alınan bilgilere dayalı olarak koku ve tat kayıplarına yönelik olarak homeopati uygulanmıştır. Verilen tedaviler sonrası hastalarda dramatik düzelmelerin olduğu kaydedilmiştir. Her üç hastanın konvansiyonel standart tedavilerine devam edilirken, beraberinde veya sonrasında yapılan homeopati ile koku ve tat kayıplarının tama yakın düzeldikleri görsel analog ölçek değerlendirmesine göre gösterilmiştir. Koku ve tat kaybı bulgularıyla seyredebilen COVID-19 pandemisinde, hastaların hızla normal yaşama dönebilmelerini sağlamak için homeopati ile tedavi üzerine olgularla bir farkındalık oluşturulmak istenmiştir. Bu olgu serisi, bu konuda fikir sunmak üzere kaleme alınmıştır. Ancak “ağızların tadını kaçıran” COVID-19 ile daha kapsamlı mücadelede, geniş kapsamlı kohort çalışmalarıyla ileri araştırmalara ihtiyaç vardır., We aimed to reveal the clinical follow-up of three patients that we applied homeopathy in order to improve the loss of taste and smell that affects the quality of life in patients with COVID-19 infection. Homeopathy was applied for loss of smell and taste based on information from three male patients aged 27, 52 and 53, respectively. Dramatic improvements were noted in patients. While conventional standard treatments of all three patients are being continued, homeopathy accompanied by or afterwards showed that the loss of smell and taste disappeared completely according to the visual analog scale assessment. In Covid-19 pandemic, which can be followed by loss of smell and taste, it was aimed to raise awareness with patients on homeopathy to ensure that patients can return to normal life quickly. However, further research is needed in the broader struggle with Covid-19, which "spoils the taste", with extensive cohort studies.

Amaç: Akut Myeloid Lösemi (AML) tanısı alan ve remisyon indüksiyon kemoterapisi verilen hastalarda gelişen invaziv fungal enfeksiyonların (İFE) mortalitesi yüksek seyretmektedir. Bu hastalarda antifungal profilaksi kullanımı önerilmektedir ancak antifungal profilaksinin, başlanma zamanı konusunda net bilgi yoktur. Bu çalışmada, remisyon indüksiyon kemoterapisi alan hastalarda, kemoterapi ile başlanan antifungal profilaksinin etkinliği ile hasta nötropeniye girdikten sonra başlanan başlanan posakonazol profilaksisinin etkinliğini değerlendirmek amaçlanmıştır.Gereç-Yöntem: AML tanısı ile remisyon indüksiyon kemoterapisi alan hastalarda, hasta nötropeniye girdikten sonra başlanan posakonazol profilaksisi (Grup 1) ile kemoterapi rejimiyle birlikte posakonazol profilaksisi (Grup 2) arasındaki farklar karşılaştırıldı. Hastaların demografik verileri ve gelişen İFE’lar kaydedildi.Bulgular: Çalışma süresince, AML tanısı ile remisyon indüksiyon kemoterapisi alan 104 hasta çalışmaya alındı. Birinci grupta 46 hasta, ikinci grupta ise 58 hasta vardı. Profilaksi etkinliği karşılaştırıldığında ise birinci grupta 22 (%47.8) ikinci grupta 19 (32.8) hastada başarısızlık görüldü. Her iki grubun demografik verileri benzer idi ancak 21 günden uzun nötropeni varlığı grup 1’de daha fazla bulundu. Ayrıca grup 1’de ortalama nötropeni süresi daha uzun idi (p
