Abstract

Introduction and Objective: The literature has suggested that many medications commonly used in long term care settings could be simplified or eliminated. While the importance of minimizing medications is well understood there is a lack of specific recommendations to assist with this task. The purpose of this study was to evaluate the usefulness of a medication reduction protocol in a long term care facility. Methods: A medication review, with a set of predetermincd criteria to guide decision-making, was applied to 54 skilled nursing care residents, over a 7-month period. The protocol examined certain medications commonly used in long term care and specified criteria to make an intervention. Changes in medication use, cost savings, and adverse effects were examined 3 months after the medication changes were initiated. Results and Conclusions: A total of 84 changes were made on 81% (N = 44) of residents, averaging 1.6 changes per patient. Interventions resulted in an 8% reduction in the number of medications and a 12% reduction in the number of doses per patient per day over a 3-month period. Ninety-four percent of the changes were still in effect at 3-month follow-up. Overall, this resulted in an annual cost saving of $2190 for 54 residents. This study suggests that many medications commonly used in long term care can be safely eliminated and drug scheduling can be optimized. [ABSTRACT FROM PUBLISHER]

12. Visceral Injuries: An Unusual Complication of an Electrical Burn

Abstract

A case of high-voltage electrical injury with massive retroperitoneal muscle necrosis, focal hepatic coagulation necrosis, acute pancreatitis, and an acute coagulopathy with factor V, factor X, and platelet deficits occurred. Visceral involvement by electrical current is unusual, and to our knowledge, hepatic necrosis from this type of injury has not been described previously.

13. Serum Corticosteroid Binding Following Thermal Injury1

Abstract

An adsorption technique has been adapted to measure both the total corticosteroid-binding capacity (CBC) and corticosteroid-binding globulin (CBG) capacity of rat serum following thermal injury. A maximum increase in the rat serum CBC was seen 4 days following a third-degree burn. This increase in CBC activity was accounted for by a twofold increase in the activity of the CBG fraction. Changes in corticosterone levels paralleled alterations in the CBG capacity. Increased levels of CBG were still indicated when CBG capacity was corrected for changes in serum globulin levels following a burn. The binding by non-CBG serum elements remained relatively constant postburn.In contrast to the rat, no significant alteration in CBC or CBG capacity occurred in human burn patient serum. As a result, the level of cortisol in excess of CBC was as much as 5 times normal in the early postburn period and remained elevated up to 40 days after injury. It is concluded that the human, unlike the rat, does not respond with an increase in serum corticosteroid-binding activity following the stress of a severe burn.

The Veterans Administration's (VA) Department of Medicine and Surgery, the largest of the civilian federal health care systems, is under continuous White House, congressional, and public pressure to provide greater accountability and increased cost-effectiveness for its $5 billion-plus annual budget. Responding to these pressures, the VA is evaluating in ten of its 172 medical centers a new system of resource allocation and financial management known as a multilevel care system. Abstract

We studied 454 patients with prostatic adenocarcinoma who were assigned a preliminary clinical stage on the basis of serum acid phosphatase, routine bone survey and physical examination. Subsequently, they were assigned a final clinical stage after radioisotopic bone scanning, lymphangiography and staging pelvic lymph node dissection. Only 53, 54, 57 and 26 per cent, respectively, of patients initially assigned the preliminary clinical stage of IB, II, III or IVA remained at that stage after the additional studies.

Abstract

The ability of spleen cells from rats subjected to a standard burn or surgical injury to induce a graft-versus-host (GVH) reaction was measured by means of a modified popliteal node assay. Results showed a severe depression of the GVH reaction for a period of about 1 week. This depression was not abolished by prior donor adrenalectomy.
