Abstract

The Genotype List (GL) String grammar for reporting HLA and Killer-cell Immunoglobulin-like Receptor (KIR) genotypes in a text string was described in 2013. Since this initial description, GL Strings have been used to describe HLA and KIR genotypes for more than 40 million subjects, allowing these data to be recorded, stored and transmitted in an easily parsed, text-based format. After a decade of working with HLA and KIR data in GL String format, with advances in HLA and KIR genotyping technologies that have fostered the generation of full-gene sequence data, the need for an extension of the GL String system has become clear. Here, we introduce the new GL String delimiter ?, which addresses the need to describe ambiguity in assigning a gene sequence to gene paralogs. GL Strings that do not include a ? delimiter continue to be interpreted as originally described. This extension represents version 1.1 of the GL String grammar.

Abstract

The transplantation of hematopoietic stem cells (HSC) from a healthy individual can be the only curative therapy for patients with malignant or non-malignant diseases of the blood system. Current requirements for the transplantation of HSC demand to match the five human leucocyte antigen (HLA) loci A, B, C, DRB1 and DQB1 on allele level between patient and donor. Given the heterogeneous i.e. non-allele level HLA data usually available in the files of big unrelated donor registries, the estimation of five locus haplotype frequencies in allele level resolution is highly desirable in order to be used in various applications improving patient care. A tailored and methodically extended implementation of the frequently employed expectation-maximization algorithm is needed to achieve this aim due to the complexity introduced by ambiguous or missing HLA data found in the daily practice of unrelated donor search. The HLA haplotype frequencies achieved with this new methodology are validated against comparable haplotype frequencies published in the literature. The haplotype frequencies of German donors presented here are more precise when used for predicting the degree of HLA compatibility between donors and patients.

Abstract

Prior research has identified five common genetic variants associated with narcolepsy with cataplexy in Caucasian patients. To replicate and/or extend these findings, we have tested HLA-DQB1, the previously identified 5 variants, and 10 other potential variants in a large European sample of narcolepsy with cataplexy subjects.

Abstract

OBJECTIVE Narcolepsy with cataplexy is tightly associated with the HLA class II allele DQB1*06:02. Evidence indicates a complex contribution of HLA class II genes to narcolepsy susceptibility with a recent independent association with HLA-DPB1. The cause of narcolepsy is supposed be an autoimmune attack against hypocretin-producing neurons. Despite the strong association with HLA class II, there is no evidence for CD4+ T-cell-mediated mechanism in narcolepsy. Since neurons express class I and not class II molecules, the final effector immune cells involved might include class I-restricted CD8+ T-cells. DESIGN HLA class I (A, B, and C) and II (DQB1) genotypes were analyzed in 944 European narcolepsy with cataplexy patients and in 4043 control subjects matched by country of origin. All patients and controls were DQB1*06:02 positive and class I associations were conditioned on DQB1 alleles. RESULTS HLA-A*11:01 (OR = 1.49 [1.18-1.87] P = 7.0*10-4), C*04:01 (OR = 1.34 [1.10-1.63] P = 3.23*10-3), and B*35:01 (OR=1.46 [1.13-1.89] P = 3.64*10-3) were associated with susceptibility to narcolepsy. Analysis of polymorphic class I amino-acids revealed even stronger associations with key antigen-binding residues HLA-A-Tyr9 (OR = 1.32 [1.15-1.52] P = 6.95*10-5) and HLA-C-Ser11 (OR=1.34 [1.15-1.57] P = 2.43*10-4). CONCLUSIONS Our findings provide a genetic basis for increased susceptibility to infectious factors or an immune cytotoxic mechanism in narcolepsy, potentially targeting hypocretin neurons.

Abstract

Aim: The completeness and resolution of the HLA typing of registered volunteer donors is usually not sufficient to determine their suitability for a concrete patient. Therefore, knowing the probability for each ambiguity in a donor to turn out to be a match is indispensable for planning the search strategy at large and for deciding each single step. Methods: Since the update to version 2 in late 2009, OptiMatch® has been using HLA-A, -B, -C, -DRB1, -DQB1 high resolution haplotype frequencies (HF) to predict the probability that each donor will be a 10/10 or 9/10 match as well as the probability of identity for each of the five loci individually. The HF used had been derived from over 145.000 active donors of ZKRD typed for the five loci at intermediate or high resolution. We retrospectively validated the predictions performing the same calculation for over 38,500 CT samples requested between January 2000 and March 2011 where no high resolution information was available for HLA-A, -B, -C, -DRB1 and -DQB1 at the time of request, but was revealed by the CT result. We grouped predictions in intervals of 5%, then compared the expected and observed number of matches per interval and performed a weighted linear regression. Results: The R2 were >.995, regression coefficients >.94 and absolute intercepts <.05 for all seven variants of predictions carried out [Fig. 1]. Display Omitted Conclusions: The predictive power of the OptiMatch® algorithm exceeds our own expectations. With an adequate base of haplotype frequencies it is an excellent tool for speedy, economic and tailored unrelated donor searches. [Copyright &y& Elsevier]
