8p23 deletion syndrome is characterized by congenital heart disease, diaphragmatic hernia, growth restriction, microcephaly, intellectual disability, behavioral problems, and abnormal genitalia. Prenatal findings are generally related to abnormal ultrasound findings and few cases have been reported in the literature. We present the prenatal diagnosis of 8p23 deletion syndrome with sonographic features of fetal growth restriction, short long bones, increased right ventricular wall thickness and small ventricular dimensions without an obvious structural fetal heart anomaly. The deletion size of the present case was one of the largest reported prenatally with a 10.8 MB deletion in the 8p23.3p23.1 region that did not include the critical GATA4 gene. The large deletion in our case included the SOX7, Tankyrase 1 (TNKS) and Microcephalin 1 (MCPH 1) genes. We assume that classical phenotypic features of micrognathia, low-set ears, flat and broad nasal bridge that we observed in our case may be due to these deletions. Microarray analysis of the chromosomes should be performed to diagnose chromosome aberrations such as 8p23 deletion syndrome in obscure ultrasonographic findings prenatally.

The aim of this analysis was to study the postnatal short-term outcome of cases with aneurysmal septum primum (ASP) and non-restrictive foramen ovale in otherwise structurally normal hearts. This is a retrospective review of fetuses with ASP referred between 2016 and 2018 to the University Hospital of Cerrahpasa, Department of Fetal Cardiology for cardiac scanning. Prenatal and postnatal clinical features and outcomes for each case were ascertained from the departmental database and from individual clinical hospital records. We presented twenty-four cases which also had postnatal echocardiographic examination in our hospital. At the time of diagnosis of ASP, the mean maternal age was 31.1 ± 5.7 years, the mean gestational age was 28.9 ± 5.9 weeks, the mean birth week was 37.4 ± 3.1 weeks, and the mean birth weight was 2940.8 ± 736.6 g. The postnatal first 3 months prevalences of atrial septal aneurysm, patent foramen ovale and, secundum atrial septal defect (ASD) were 12.5%, 12.5%, 20.8% among fetuses with ASP, respectively. ASP may persist after birth and may increase the frequency of secundum ASD. These data suggest that fetuses with ASP should be followed by a pediatric cardiologist in postnatal life.

PURPOSE To construct nomograms for the fetal cerebellar vermis and brainstem structures obtainable from the midsagittal plane of the brain by two-dimensional sonography. METHODS This was a prospective cross-sectional study of 434 healthy fetuses in low-risk singleton pregnancies between 18 and 35 gestational weeks. The following parameters were evaluated in the midsagittal cranial plane; cerebellar vermis anteroposterior diameter (APD), craniocaudal diameter (CCD), pons, midbrain and medulla oblongata APD and tectum length. The measurements were presented as growth charts according to gestational age. RESULTS The mean ± SD, and 5%, 50%, 95% centile charts according to gestational age for vermis APD and CCD, pons, midbrain and medulla oblongata APD and tectum length were constructed. Pertussis is an important cause of morbidity and mortality in infants under two months of age and these high risk babies are dependent on maternally derived antibodies until completion of their first immunization series. This study aimed to evaluate the vaccine response of late preterm and term newborns as well as their mothers who underwent combined tetanus-diphtheria toxoid and acellular pertussis (Tdap) vaccination during pregnancy.A total of 70 pregnant women were administered Tdap vaccine (Boostrix®, GSK) between 27 and 33 gestational weeks of pregnancy. The IgG antibodies against pertussis toxin (PT) and filamentous hemagglutinin (FHA) in maternal blood before vaccination and in both maternal and umbilical cord blood after vaccination were evaluated using the in-house ELISA method. The geometric mean concentrations (GMC) and placental transfer ratios of antibodies were measured.Participants' with a mean age of 29.59 ± 4.70 years received Tdap vaccine at an average 28.6 ± 1.31 gestational weeks. Average pre and post vaccination levels of anti-PT IgG GMCs and anti-FHA IgG GMCs were 8.01 IU/ml vs 39.48 IU/ml (p = 0.001) and 122.24 IU/ml vs 183.97 IU/ml (p 0.001), respectively. The anti-PT and anti-FHA IgG GMCs of cord blood after vaccination was 25.15 IU/ml and 118.77 IU/ml, respectively (p 0.001 and p = 0.064). Placental transfer ratios of anti-PT ve anti-FHA IgG antibodies were detected as 0.65 and 0.62, respectively.Immunization of pregnant women with Tdap at the third trimester results in high maternal and infant antibody levels. Maternal immunization during each pregnancy seems to be the best strategy in revealing the highest maternal and infant antibodies and in narrowing the gap between birth and immune system maturation in infants. Pregnant women in our country should also get the Tdap vaccine during pregnancy especially in the early third trimester.

To investigate the copeptin levels in women with intrahepatic cholestasis of pregnancy (ICP) compared to women with uncomplicated pregnancies.This cross-sectional study was conducted in 40 pregnant women with ICP and 38 randomly selected healthy pregnant women, who formed the control group. Serum copeptin concentrations were measured using an enzyme-linked immunosorbent assay.Maternal age, body mass index at assessment, and gestational age at blood sampling were similar between the two groups. Duration of pregnancy was shorter and mean birth weight was significantly lower in the ICP group compared to the control group. Total bile acid, alanine aminotransferase, aspartate aminotransferase, and gamma glutamyl transferase levels were significantly higher in the ICP group than in the control group. There was no significant difference in copeptin concentrations (2.54 (2.05) versus 2.43 (1.98) ng/ml; p = .5).Serum copeptin concentrations did not vary between the pregnancies complicated by ICP and the healthy pregnancy control group.

Amaç: AileviAkdeniz Ateşi (AAA) tanısı olan, gebe olguların maternal ve fetal sonuçlarınındeğerlendirilmesi amaçlanmıştır. Gereç ve Yöntem :Çalışmamız 2013-2017 yılları arasında üniversitemizin perinataloji kliniğindegebelikleri takip edilmiş 40 AAA olgusunun dosya verilerinin retrospektif olarakincelenmesiyle yapıldı. Hastaların klinik özellikleri , obstetrik ve perinatalsonuçları değerlendirildi. Bulgular:Ortalamahasta yaşı 30.75 ±6.15, nulliparite oranı %42,5 bulundu. Ortalama hastalıksüresi 8,36 ± 5.44 yıl olarak bulundu. Gebelik sırasında 7 hasta atak geçirdi.Hastaların % 90 ’ ı medikal tedavi almaktaydı. Ortalama doğum haftası 37,3±4.5,ortalama doğum kilosu 3115±547 grbulundu. Preterm doğum, fetal büyüme kısıtlılığı ,preeklampsi görülme oranları sırasıyla; %10 ,%5 , %10, olarak bulundu. Fetal ve neonatal kayıp, ile konjenital anomalilidoğum gözlenmedi. Sonuç : AAA tanılı gebelikolgularının yönetiminde multidisipliner yaklaşım ile takip ve tedavininsağlanması olumlu maternal ve fetalsonuçlar ile ilişkilidir., Objective: Toevaluate maternal and fetal outcomes of pregnancies diagnosed with FamilialMediterranean Fever (FMF) Methods: Retrospective analysis of 40 pregnanciesdiagnosed with FMF who received antenatal care at our perinatology unit between2012 -2017 included in this clinical study. Clinical characteristics ofpatients, perinatal and maternal outcomes were evaluated.Results: Themean patient age was 30.75 ±6.15 and the nulliparity rate was 42,5 %. The meanduration of illness was found to be 8,36 ± 5.44 years. 7 patients had an attackduring pregnancy. 90 % of the patients received medical treatment. Mean gestationalage at delivery was 37,3±4.5 , mean birth weight was 3115±547 g. Preterm delivery, fetalgrowth restriction and preeclampsiarates were 10, 5, 10 % respectively. Fetal and neonatal loss, congenitalanomalous birth was not observed.Conclusion:Multidisciplinary approach to the care of pregnancy diagnosed with FMF isassociated with good maternal and fetal outcomes.

The aim of the current study was to determine serum endocan levels in patients with threatened preterm labor and to assign whether endocan levels in patients with true preterm labor who give birth within 7 days differ from those of false preterm labor and uncomplicated pregnancy.This cross-sectional study was conducted on 58 patients diagnosed with threatened preterm labor and 31 healthy pregnant women matched for gestational age. Patients with threatened preterm labor were divided into two groups; preterm delivery (28) and term delivery (30) groups. Maternal serum endocan levels were measured with the use of an enzyme-linked immunosorbent assay kit.The median serum endocan level (pg/mL) in patients with threatened preterm labor was significantly higher than that of women with uncomplicated pregnancies (725, IQR 619-823 versus 310, IQR 218-423;The maternal serum endocan level may be a useful marker to define high risk group for preterm delivery in patients with threatened preterm labor and similar cervical length measures.

To evaluate placental elasticty in women with gestational diabetes mellitus (GDM) and non-diabetic controls. Thirty-three pregnant women with GDM according to the current criteria of the American Diabetes Association and 43 healthy pregnant women who were admitted to the antenatal clinic were recruited for this case-control study. Elasticity values of both the peripheral and the central parts of the placentas of the patients in both groups were determined by shear wave elastography (SWE) imaging. Mean elasticity values of both the central and the peripheral part of the placentas were significantly higher in GDM pregnancies (p 0.001). No difference was observed in the mean elasticity values of the central and the peripheral part of the placentas in two groups (p 0.05). SWE imaging technology might provide a quantitative assessment of the morphological pathologies of placentas in pregnant women with GDM.

Background: Toxoplasma Gondii, Rubella, and Cytomegalovirus (CMV) are the most common causes of congenital infections worldwide. There is not a consensus on these infectious agents should be screened during pregnancy. The aim of this study is to determine the prevalence of toxoplasma, rubella, and CMV infections in order to know the need for antenatal screening.Methods: This retrospective cross-sectional study was performed on 1309 ambulatory pregnant patients who applied to the obstetric clinic of a university hospital between October 2016 and April 2018. Documents of patients in the first trimester were reviewed and serologic data of Toxoplasma gondii, CMV, Rubella infections were retrieved from the computer database.Results: Of 1309 pregnant women, positivity for anti-Toxoplasma IgG antibody was 352(26.9%), while 17(1.3%) of the subjects tested were positive for the anti-Toxoplasma IgM antibody. These positivities of the pregnant women for anti-Rubella IgG and IgM were 1147(87.6%) and (0.1%), respectively. These positivities of the pregnant women for anti-CMV IgG and IgM were 1163(88.8%) and 17(1.3%), respectively.Conclusions: We detected high rates of immunity against Rubella and CMV but low rates of immunity against Toxoplasma in this retrospective cohort of pregnant women. Due to high rates of seropositivity against Rubella and CMV, routine nationwide screening may not be necessary.

Objective: We aimed to determine the fetal cerebro-placental Doppler indices and modified myocardial performance index (Mod-myocardial performance index (MPI)) in this homogenous group of postdated pregnancies.Methods: A total of 92 singleton pregnant women were included in this prospective study. Objective With the widespread use of ultrasonography for fetal screening, the detection and management of congenital urinary tract abnormalities has become crucial. In this study, we aimed to describe the clinical approaches in patients with prenatally detected urinary tract abnormalities. Material and Methods This study is a retrospective, single-center study performed at a perinatology unit of a university hospital, between 2010 and 2016. The outcomes of 124 patients who were prenatally diagnosed as having urinary tract abnormalities are reported. Variables included in the analysis were fetal sex, birth week and weight, persistency, and necessity surgery after birth for renal pelvic dilatation. Low-risk renal pelvic dilatation was determined as an anterior-posterior (AP) diameter of 4-7 mm at 16-28 weeks, 7-10 mm after 28 weeks, whereas high-risk dilatation was defined as AP measurements of ≥7 mm at 16-28 weeks, ≥10 mm after 28 weeks, respectively. Results The majority of patients consisted of male fetuses with bilateral pelviectasis (62.9%, 20.2%, respectively). The mean age was 28.8±6.4 years. Hastaların klinik karakteristikleri, obstetrik ve perinatal sonuçlarıdeğerlendirildi.Bulgular: Hastaların ortalama yaşı 29,1±4,6 yıl ve nulliparite oranı %45,2 idi. Gebelik sırasında hastalığınalevlenmesi hastaların %11,1’ inde görüldü. Lupus antikoagulanı,antikardiyolipin IgG ve IgM antikorlarının pozitifliği sırasıyla; %23,8 , %18,2ve %18,2 olarak saptandı. Weevaluated lupus pregnancies that were followed and delivered from 2002 to 2016.Results: The mean patient age was 29.1 ±4.6 years, and the nulliparity rate was 45.2%. Disease flare-up occurred in11.1% of patients. Lupus anticoagulants and anticardiolipin IgG and IgMantibodies were positive in 23.8%, 18.2%, and 18.2% patients, respectively. Themean gestational age at delivery was 37.2 ± 3.7, and the mean birth weight was2813 ± 856 g. Fetal growth restriction, preterm delivery, stillbirth, andpreeclampsia rates were 16.6%, 10.3%, 10.3%, and 9.5%, respectively.Conclusion:A multidisciplinary approach to the care of pregnant women with SLE ismandatory for good maternal and fetal outcomes.

To evaluate the distribution and the obstetric outcomes of pregnancies with different types of rheumatic diseases managed in our unit. Pregnancies of 162 women with rheumatic diseases, seen for their antenatal care at our department for the period between 2013 and 2017 were included in this retrospective clinical study. Obstetric and perinatal outcomes were main outcome measures. The most encountered rheumatic diseases were SLE (37.7%) followed by Behcet’s disease (20.4%) and rheumatoid arthritis (17.3%) in our series. The mean maternal age was 30.6 ± 5.3 and the rate of nulliparity was 38.3% in the overall group. Disease activation occurred in 14.1% of patients. Mean gestational age at delivery was 37.4 ± 3.1 and mean birth weight was 3004 ± 762 g. Stillbirth, neonatal death, fetal growth restriction, preeclampsia and preterm delivery rates were 1.2, 2.4, 17.3, 7.4 and 17.9%, respectively. Antiphospholipid syndrome had the highest incidences for fetal growth restriction (42.9%), preeclampsia (28.6%) and delivery ≤ 34 gestational weeks (42.9%). Pathologic uterine artery Doppler velocimetry was identified in 15 cases (15/162, 9.3%) in which 10 (66.7%) developed preeclampsia and/or fetal growth restriction during follow-up. A majority of women with rheumatic diseases have successful pregnancies and deliver healthy babies, with the close and appropriate rheumatological, obstetric and neonatal monitoring.

To determine the serum levels of HIF-1 α, progranulin, and syndecan-1 in preeclampsia (PE) and normal pregnancy, and to compare whether these markers demonstrate any difference between early-onset PE (EO-PE) and late-onset PE (LO-PE).This cross-sectional study was conducted on 27 women with EO-PE, 27 women with LO-PE, and 26 healthy normotensive pregnant controls matched for gestational age. Maternal levels of serum HIF-1 α, progranulin, and syndecan-1 were measured with the use of an enzyme-linked immunosorbent assay kit.Statistical analysis revealed significant differences between the control and the PE groups in progranulin (p .001) and syndecan-1 (p .001) levels. There were no significant differences in the serum HIF-1 α levels between these groups (p= .069). When PE patients were evaluated by considering subgroups; statistical analysis revealed significant inter-group differences in all biomarkers. Serum progranulin levels were significantly higher in LO-PE compared with the other two groups (EO-PE versus LO-PE and LO-PE versus controls p = .000). Control group presented significantly higher syndecan-1 levels, than EO and LO-PE (p .001). HIF-1 α levels positively correlated with progranulin levels (r = .439, p= .000).Serum progranulin may have potential to be used as a biomarker for the differentiation of EO-PE and LO-PE. The co-operative action between HIF-1 α and progranulin might play a key role in the pathogenesis of LO-PE. The predominant feature of LO-PE seems to be an inflammatory process, whereas in EO-PE placentation problem seems to be the main pathology.

In this study, we evaluated the placental elasticity in vivo by shear-wave elastography in pregnant women under follow-up for intrauterine growth restriction (IUGR) and compared the elasticity values to normal pregnancies.This prospective study included 42 pregnant women with a possible diagnosis of intrauterine growth restriction based on obstetrical grayscale and Doppler ultrasonography and 42 women with a normal pregnancy during the 2nd and 3rd trimester. During follow-up examinations, seven fetuses showed an increased growth and were delivered with a birth-weight above the 10 percentile. However, for statistical purposes we included these seven patients in the IUGR group due to prospective nature of the study. All patients initially underwent obstetrical grayscale and Doppler ultrasonography with measurement of resistivity and pulsatility indices from uterine arteries. Subsequently, elasticity values of the peripheral and central part of the placentas from fetal and maternal surfaces were measured by shear-wave elastography. Following delivery, Apgar scores at 1st and 5th minute, birth weight were collected. For statistical analysis, Mann-Whitney U test was used. ROC curves were plotted and cut-off values for elasticity values were analyzed.Median elasticity values of the central part of the placentas from maternal (28kPa vs 6kPa) and fetal sides (21.5kPa vs 5kPa) were significantly higher in IUGR pregnancies compared to the control group (p0.001). Similarly, median elasticity values of peripheral part of placentas from maternal (22kPa vs 5.35kPa) and fetal sides (22.5kPa vs 5.3kPa) were significantly higher in IUGR pregnancies compared to the control group (p0.001).Placental stiffness values are significantly higher in patients with IUGR. Shear-wave elastography can be used as a non-invasive, complementary method to gray-scale and Doppler ultrasound for diagnosing IUGR.

One hundred and seventy-two twin-pregnant patients were enrolled. The estimated foetal weight was calculated using 19 different formulas. Ong's formula (0.954 (95%CI = 0.938/0.966)), which was designed specifically for twins, produced the highest Cronbach's alpha value followed by Hadlock II (0.952 (95%CI = 0.935/0.965)), Hadlock I (0.952 (95%CI = 0.935/0.964)), Hadlock III (0.952 (95%CI = 0.935/0.964)), Hadlock IV (0.952 (95%CI = 0.935/0.964)) and our formula (0.952 (95%CI = 0.935/0.964)), which produced the same Cronbach's alpha values for twin A. For twin B, our formula produced the highest Cronbach's alpha value (0.961 (95%CI = 0.948/0.972) followed by Hadlock II (0.960 (95%CI = 0.946/0.971)), Hadlock I (0.960 (95%CI = 0.946/0.970)), Hadlock III (0.960 (95%CI = 0.946/0.970)) and Hadlock IV (0.960 (95%CI = 0.946/0.970)). In conclusion, our formula (AC, FL) performed well in predicting the foetal weights in twin pregnancies (>24 weeks) in our study. However, it should be tested in other populations. Hadlock II (AC, FL) produced a comparable performance to Hadlock I (BPD, HC, AC, FL), Hadlock III (BPD, AC, FL) and Hadlock IV (HC, AC, FL). Hadlock II may be preferable in twin pregnancies since it is based on AC and FL only.

Mean maternal serum ANP and BNP levels of women with GDM were significantly lower than the control group (12.9 ± 9.9 versus 34.8 ± 16.9 pg/ml, p 0.001; 416.6 ± 209.7 versus 629.7 ± 162.2 mg/dl, p 0.001, respectively). Maternal serum ANP and BNP levels were negatively correlated with insulin levels, HbA1c and HOMA-IR values (p 0.05).Maternal serum ANP and BNP levels are significantly lower in patients with GDM. These biomarkers might be valuable in clinical setting for identifying high-risk women for developing diabetes during pregnancy.
