Eccrine angiomatous hamartoma is a rare, benign skin tumor typically occurring on the hands and feet. It is usually congenital in origin but, less commonly, it may also appear later in life. Eccrine angiomatous hamartoma may appear in various colours and forms: a macule, papule, plaque or nodule, resembling angioma. Associated symptoms include hyperhidrosis within the lesion. Definitive diagnosis requires histopathological examination. The most characteristic microscopic features include eccrine glands hyperplasia, and vascular proliferation and dilatation. ABSTRACT: Background: Eccrineangiomatoushamartoma (EAH) is a rare benign disease that comprises vascular and eccrine malformations. Pain is one of the most common symptoms and surgical resection is generally required. Objective: To report a novel surgical technique that can alleviate severe pain caused by a large lesion in an EAH patient. Methods: On magnetic resonance imaging (MRI), a subcutaneous abnormal signal was detected in the right hip and groin area of an 18-year-old male. The lesion was carefully elevated from the normal tissue above the plane of the deep fascia and was sutured in situ. Additionally, a small sample of the central lesion tissue was collected for histological examination. Results: The visual analog scale score of pain decreased from 9 points to 2 points at 1week and to 0 points at 2 weeks after the surgery. The only complication observed was local numbness, which recovered spontaneously. No recurrence of pain was observed after 1 year of surgery. Histological findings demonstrated that the numbers of eccrine sweat glands and abnormal vessels decreased post-surgery. Conclusion: The surgical procedure used in our case may represent a novel, easy, safe, and effective option for the treatment of a large, severely painful EAH.

The lesion was devoid of hair, had follicular prominences, and showed increased localized sweating. Histopathology revealed an increased number of eccrine glands in the myxoid stroma with multiple vascular elements in the dermis, favoring the diagnosis of eccrine angiomatous hamartoma. This case illustrates a relatively uncommon clinical presentation of eccrine angiomatous hamartoma as a hairless, hyperhidrotic plaque in the head and neck region.

First reported in 2006, eccrine angiokeratomatous hamartoma is a very rare vascular malformation of the skin, with only few described cases. It has a peculiar histopathology with features deriving from the combination of two different vascular malformations of the skin: solitary angiokeratoma and eccrine angiomatous hamartoma. In the past, other authors described similar hamartomatous lesions with features deriving from verrucous venous malformation and eccrine angiomatous hamartoma. We believe that these lesions are clearly overlapping from clinical, histopathological, and immunohistochemical points of view and the term "eccrine angiokeratomatous hamartoma" should be used to indicate the whole spectrum of these lesions as suggested by Kanitakis et al. Herein we present two cases of this rare vascular hamartoma, with clinical, histopathological and immunohistochemical characterization. In addition, for the first time we report a complete and detailed review of the literature to clarify the clinical, epidemiological, and histopathological features of this unique entity. A 71-year-old woman was referred to our clinic with a 2-month history of pain and exudate from her right first toe. Physical examination revealed a subungual nodule that elevated the nail plate and produced distal ony-cholysis. We performed onychectomy and removed the nodule. A histopathological specimen from the nodule showed hyperkeratosis and acanthosis in the epidermis, papillomatosis and dilated vessels in the superficial dermis, and aggregation of eccrine glands in the mid-dermis. Based on these findings, a diagnosis of eccrine angiomatous hamartoma (EAH) was made. EAH is a rare cutaneous hamartoma that is histologically characterized by proliferation of eccrine glands and vascular components. To our knowledge, EAH occurring in the subungual region has not been reported. The pathogenesis of acquired EAH has not been completely elucidated, but several cases have been reported to be caused by external stimuli. Although the subungual area generally has no sweat glands, our case suggests that a traumatic stimulus can induce EAH anywhere including regions where sweat glands are normally not found. Eccrine angiomatous hamartoma (EAH) is an exceedingly rare benign tumor-like lesion prevalent in childhood, which can produce pain and marked sweating. Histological features include proliferation of eccrine sweat glands and angiomatous capillary channels. It may be rarely associated with underlying pathological conditions. A 15-year-old female patient presented with multiple tender reddish papules and nodules coalescing to form plaques of 10 × 8 cm over the anterior aspect of the right lower thigh since birth. It was associated with hypertrichosis, hyperhidrosis, pain, and occasional bleeding on trauma. Histopathological examination of the lesion showed increased proliferation of both eccrine and angiomatous channels. Ultrasonography and Color Doppler of the right thigh showed dilatation of the great saphenous vein (GSV) above the right knee, with evidence of grade 3 reflux at saphenofemoral junction. These lesions are more often present at birth or appear during early infancy and childhood and present as solitary nodule or plaque with occasional pain and sweating. They are generally present on the extremities, mostly the palms and soles. We report here a case of EAH in a 20-year-old female who presented with a solitary angiomatous plaque close to the lower eyelid of the right eye with occasional pain and sweating for its rarity.

Background: Eccrine angiomatous hamartoma is a rare benign cutaneous malformation with a diverse clinical appearance, therefore likely to be misdiagnosed and underreported. Main observations: A 44-year-old man presented with a congenital erythematous hyperhidrotic plaque on the left upper back measuring 18 x 25 cm. No pain or tenderness nor hypertrichosis were observed. Histopathology was consistent with the mucinous variant of eccrine angiomatous hamartoma. Intralesional injection of botulinum toxin type A greatly reduced localized sweating, improving patient quality of life. Conclusions: This article describes a novel clinical presentation of eccrine angiomatous hamartoma: large, erythematous, and slightly indurated plaque localized on the upper back. Background: Eccrine angiomatous hamartoma (EAH) is a rare benign cutaneous tumor characterized by the proliferation of eccrine glands and capillaries. Objective: The aim of this study was to summarize the clinicopathological characteristics of EAH. Methods: A retrospective chart review was performed on all patients diagnosed with EAH from 1977 to 2012 in the Union Hospital, Wuhan, P.R. China, and the clinicopathological features were compared with the cases reported in the literature. Results: A total of 26 patients with EAH were identified. The male:female ratio was 1.2:1. EAH most commonly presents as a solitary (80.8%) plaque (50.0%) on the lower extremities (61.5%). Most patients presented with hyperhidrosis localizing to the lesion. Although most patients did not have major pain or anatomic deformity, one patient had severe pain as well as difficulty walking and moving, necessitating leg amputation. The histopathological findings showed typical features of EAH. Conclusion: EAH is a rare but characteristically benign skin hamartomatous condition. A 29-year-old male patient presented to us with multiple swellings on his left leg associated with localized sweating and occasional throbbing pain. Based on the clinical features and histopathology, a diagnosis of Eccrine angiomatous hamartoma was made. The patient showed good response with regression of lesions after four intralesional injections of ethoxysclerol (polidocanol). We present this as a case of late onset eccrine angiomatous hamartoma with multiple lesions, responding to treatment with intralesional ethoxysclerol.

The article presents case study of one-and-half-year old boy who was referred for pulsed-dye laser (PDL) treatment for port-wine stain on his left thigh and how the parents reported an episodic increase in erythema accompanied by sweating. It mentions that skin biopsy from the plaque showed papillomatosis and irregular acanthosis of epidermis. It also mentions that diagnosis of eccrine angiomatous hamartoma was based on clinicopathological correlation.

ABSTRACT Eccrine angiomatous hamartoma (EAH) is a rare tumoral lesion that consists of vascular and eccrine malformation and often occurs in the distal extremities of children. Although EAH is benign, a therapeutic excision may be required for symptomatic or cosmetic considerations. We hereby report a typical case of EAH presenting as a painful and rapidly growing plaque on the right thigh. Hamartomas are proliferations resulting from errors in embryonic development, product of an abnormal arrangement of tissues that are proper to an organ. Hamartomas can be grouped in pures (pure eccrine nevus and eccrine hamartoma) and mixed, the varieties include angiomatous component, mucinous and lipomatous. Eccrine angiomatous hamartoma is a rare skin benign tumor; these are the combination of a proliferation in the eccrine normal glands and hypertrophic glands characterized by the association of underlying rich vascular stroma. It usually appears at birth, but can also appear later in childhood and rarely in the puberty, being less common in elder people. Predominates in acral areas and there have been reports of widespread distribution, can be single or multiple, bilateral and symmetric cases. Morphologically, it manifests as a neoformation unique or multiple, or with the appearance of papules or nodules and occasionally as a little lifted plate; the tone varies from reddish brown or yellowish or as the skin tone. May be associated with hyperhidrosis or hirsutism. It is usually asymptomatic but sometimes can be painful. The eccrine angiomatous hamartoma is generally a benign lesion, which does not need to be treated aggressively, the removal is indicated for cosmetic reasons or severe pain associated. A 34-year-old male presented with a swelling on the volar surface of the third digit of his right hand. This swelling was associated with pain and erythema. Ultrasound-guided needle biopsy was performed. Cytologic and histologic preparations together confirmed the diagnosis of a rarely encountered mixed epithelial and mesenchymal proliferation, an eccrine angiomatous hamartoma.
