Background and Objectives: Endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) are both well-established and effective treatments for dysplasia and early cancer in Barrett’s esophagus (BE). This study aims to compare the short- and long-term outcomes associated with these procedures in treating Barrett’s neoplasia. Materials and Methods: This single-center retrospective cohort study included 95 patients, either EMR (n = 67) or ESD (n = 28), treated for Barrett’s neoplasia at Sahlgrenska University Hospital between 2004 and 2019. The primary outcome was the complete (en-bloc) R0 resection rate. Secondary outcomes included the curative resection rate, additional endoscopic resections, adverse events, and overall survival. Results: The complete R0 resection rate was 62.5% for ESD compared to 16% for EMR (p < 0.001). The curative resection rate for ESD was 54% versus 16% for EMR (p < 0.001). During the follow-up, 22 out of 50 patients in the EMR group required additional endoscopic resections (AERs) compared to 3 out of 21 patients in the ESD group (p = 0.028). There were few adverse events associated with both EMR and ESD. In both the stratified Kaplan–Meier survival analysis (Log-rank test, Chi-square = 2.190, df = 1, p = 0.139) and the multivariate Cox proportional hazards model (hazard ratio of 0.988; 95% CI: 0.459 to 2.127; p = 0.975), the treatment group (EMR vs. ESD) did not significantly impact the survival outcomes. Conclusions: Both EMR and ESD are effective and safe treatments for BE neoplasia with few adverse events. ESD resulted in higher curative resection rates with fewer AERs, indicating its potential as a primary treatment modality. However, the survival analysis showed no difference between the methods, highlighting their comparable long-term outcomes.

Abstract Background The role of the renin-angiotensin system in gastric physiology and disease has as yet been sparsely explored. The first aim of the study was to investigate the baseline presence and location of angiotensin II receptors (AT1R and AT2R) in the stomach of the Mongolian gerbil. A second aim was to elucidate whether the presence of H. pylori infection is associated with changes in the expression of these receptors. Methods H. pylori-negative and H. pylori-infected (strain SS1 or TN2GF4) male Mongolian gerbils were investigated. The stomachs were examined at six or 12 months after inoculation by the use of immunohistochemistry, western blot and microscopic morphometry. Results AT1R and AT2R were located in a variety of cells in the gerbil gastric wall, including a subpopulation of endocrine cells in the antral mucosa and inflammatory cells infiltrating H. pylori-infected stomachs. Gerbils infected with the SS1 strain showed a significantly increased antral AT1R protein expression and an increased number of infiltrating polymorphonuclear leucocytes (PMNs) at 12 months. The AT1R protein expression correlated with the number of PMNs and the antral expression of myeloperoxidase. Conclusions Angiotensin II receptors are present in a variety of cells in the gastric wall of the Mongolian gerbil. The results indicate an influence dependent on the H. pylori strain on the gastric AT1R expression and a relationship between gastric AT1R expression and mucosal PMNs infiltration.

Adiponectin is an adipokine with a modulatory role in metabolism and exerting both anti- and pro-inflammatory effects. Levels of adiponectin are increased in serum and synovial fluid from patients with rheumatoid arthritis (RA). Adiponectin is able to stimulate the production of different pro-inflammatory factors from peripheral blood mononuclear cells (PBMCs) and fibroblast-like synoviocytes (FLS) from subjects with established RA. As increased circulating adiponectin levels are a risk factor for future development of RA in subjects with obesity, we hypothesize that adiponectin is implicated in the development of RA at an early stage by initiating the pro-inflammatory processes associated with the disease pathogenesis. Therefore, we aimed to determine if adiponectin is able to induce pro-inflammatory responses in cells involved in the pathogenesis of RA, but collected from subjects without any known inflammatory disease. PBMCs and FLS were obtained from non-inflamed subjects and stimulated with 5 μg/ml human recombinant adiponectin. Supernatants collected after 48 h were analyzed for the production of 13 chemokines and 12 cytokines using multiplex assay and ELISA. Adiponectin significantly stimulated the production of CXCL1, CXCL5, and interleukin (IL)-6 in both PBMCs and FLS, whereas it induced CCL20, CCL4, CCL3, CCL17, tumor necrosis factor (TNF), granulocyte-macrophage colony-stimulating factor and IL-10 only in PBMCs, and CXCL8, CXCL10, CCL5, CCL11, and CCL2 only in FLS. Pre-stimulation with TNF of FLS from non-inflamed subjects did not significantly enhance the release of most pro-inflammatory factors compared to adiponectin alone. Our findings indicate that PBMCs and FLS from non-inflamed subjects react to adiponectin stimulation with the secretion of several pro-inflammatory chemokines and cytokines. These results suggest that adiponectin is able to initiate pro-inflammatory responses in cells from non-inflamed subjects and support the hypothesis that adiponectin is implicated in the early phases of RA pathogenesis.

Assessment of forest health is very vital because forests form the largest terrestrial ecosystems on earth. The greenness of vegetation is one of the essential factors used in evaluating the health of forest reserves. This study is aimed at assessing the health of fifteen forest reserves in Southeastern part of Nigeria using meteorological data and MOD13A1-derived Normalized Difference Vegetation Index (NDVI) and Enhanced Vegetation Index (EVI). Related portions of the monthly MOD13A1 data, derived for the years 2010, 2014, and 2018, were downloaded, and the monthly mean values of the vegetation indices (NDVI and EVI) were estimated for each of the forest reserves using the Spatial Analysis Module in ArcGIS software. The computed monthly mean values of NDVI range from 0.094 to 0.790 while that of EVI ranges from 0.11 to 0.624 and the rainfall data range from 0 to 780.2 mm/month within the period of study. Analyses of the correlation coefficients between monthly rainfall data and NDVI, monthly rainfall data and EVI, and that of NDVI and EVI range from −0.827 to 0.584; −0.715 to 0.914, and 0.598 to 0.980. The obtained results indicate that some of the forest reserves are moderately healthy while some areas are under great stress. We can conclude that satellite remote sensing is a veritable tool in the assessment, management, and monitoring of forest health especially where there is little or no terrestrially acquired forest inventory data.

The fungus Mucor indicus is able to produce ethanol from xylose as well as dilute-acid lignocellulosic hydrolyzates. The fungus completely assimilated 10 g/L xylose as the sole carbon and energy source within 32 to 65 h at an aeration rate of 0.1 to 1.0 vvm. The highest ethanol yield was 0.16 g/g at 0.1 vvm. Xylitol was formed intermediately with a maximum yield of 0.22 g/g at 0.5 vvm, but disappeared towards the end of experiments. During cultivation in a mixture of xylose and glucose, the fungus did not assimilate xylose as long as glucose was present in the medium. The anaerobic cultivation of the fungus in the hydrolyzate containing 20% xylose and 80% hexoses resulted in no assimilation of xylose but complete consumption of the hexoses in less than 15 h. The ethanol yield was 0.44 g/g. However, the xylose in the hydrolyzate was consumed when the media was aerated at 0.067 to 0.333 vvm. The best ethanol yield was 0.44 g/g at 0.067 vvm. The results of this study suggest that M. indicus hydrolyzate can be first fermented anaerobically for hexose assimilation and subsequently continued under oxygen-limited conditions for xylose fermentation.

Only few studies have been devoted to the actions of the renin-angiotensin system (RAS) in the human gastrointestinal tract. The present study was undertaken to elucidate the expression and action of RAS in the human esophageal mucosa. Mucosal specimens with normal histological appearance were obtained from healthy subjects undergoing endoscopy and from patients undergoing esophagectomy due to neoplasm. Gene and protein expressions of angiotensin II (Ang II) receptor type 1 ([AT.sub.1]) and type 2 ([AT.sub.2]) and angiotensin-converting enzyme (ACE) were analyzed. In vivo functionality in healthy volunteers was reflected by assessing transmucosal potential difference (PD). Ussing chamber technique was used to analyze the different effects of Ang II on its [AT.sub.1] and [AT.sub.2] receptors. Immunoreactivity to [AT.sub.1] and [AT.sub.2] was localized to stratum superficiale and spinosum in the epithelium. ACE, [AT.sub.1], and [AT.sub.2] were found in blood vessel walls. Transmucosal PD in vivo increased following administration of the [AT.sub.1] receptor antagonist candesartan. In Ussing preparations mean basal transmural PD was -6.4 mV, epithelial current ([I.sub.ep]) 34 [micro]A/[cm.sup.2], and epithelial resistance ([R.sub.ep]) 321 [ohms] x [cm.sup.2]. Serosal exposure to Ang II increased PD as a result of increased [I.sub.ep], whereas [R.sub.ep] was constant. Ang II given together with the selective [AT.sub.1]-receptor antagonist losartan, or [AT.sub.2] agonist C21 given alone, resulted in a similar effect. Ang II given in presence of the [AT.sub.2]-receptor antagonist PD123319 did not influence PD, but [I.sub.ep] decreased and [R.sub.ep] increased. In conclusion, Ang II receptors and ACE are expressed in the human esophageal epithelium. The results suggest that [AT.sub.2]-receptor stimulation increases epithelial ion transport, whereas the [AT.sub.1] receptor inhibits ion transport and increases [R.sub.ep]. potential difference; epithelial current; epithelial resistance doi: 10.1152/ajpgi.00255.2009.

The effects of binding of tetradecyl betainate to Salmonella typhimurium on the rate of hydrolysis is investigated. The hydrolysis rate was signifiant but retarded compared to tetradecyl betainate (B14) in solution. Hydrolysis of substance bound to S. typhimurium was increased in an alkaline environment and by heat, consistent with a model based on the insertion of amphiphilic QQCs into the bacterial outer membrane.
