Introduction: We are facing a pandemic with a great impact worldwide, as a result of the rapid spread of the novel coronavirus (COVID-19). The medical community is still getting to know behavior of this virus and the consequences from a population point of view. All this knowledge is extremely dynamic, so some behaviors are still not well established. Otorhinolaryngologists have a central role in the management of this situation, in which they must assess the patient, avoid contamination to and by health professionals and other patients. Thus, the recommendations of the Brazilian Association of Otorhinolaryngology and Cervical-Facial Surgery (ABORL-CCF) have the main objective of reducing the spread of the new coronavirus during otorhinolaryngological care and assisting in the management of these patients. Methods: Review of the main recommendations of national and international scientific societies, decisions by government agencies and class councils. The topics will be related to the general aspects of COVID-19, personal protective equipment, care in patient assistance, endoscopic exam routines and the management of sinonasal, otological and pediatric evaluations related to COVID-19. Results: The use of personal protective equipment is considered crucial in routine ENT care. We recommend postponing appointments, exams and elective surgeries to reduce the spread of COVID-19. Similarly, we recommend changing routines in several areas of otolaryngology. Additionally, guidance is provided on the use of telemedicine resources during the pandemic period. Conclusions: We are still at the beginning of the COVID-19 pandemic and scientific evidence is still scarce and incomplete, so these ABORL-CCF recommendations for otorhinolaryngologists may be updated based on new knowledge and the pattern of the new coronavirus spread. Resumo: Introdução: Estamos diante de uma pandemia de grande impacto mundial como resultado da rápida propagação do novo coronavírus, COVID-19. A comunidade médica está ainda conhecendo o comportamento desse vírus e as repercussões do ponto de vista populacional. Todo esse conhecimento é extremamente dinâmico, por isso algumas condutas ainda não estão bem estabelecidas. O otorrinolaringologista tem um papel central no manejo dessa situação em que deve avaliar o paciente e evitar a contaminação dos profissionais da saúde e dos demais pacientes. Dessa forma, as recomendações da Associação Brasileira de Otorrinolaringologia e Cirurgia Cérvico-Facial (ABORL-CCF), têm por objetivo principal reduzir a propagação do novo coronavírus durante o atendimento otorrinolaringológico e auxiliar no manejo desses pacientes. Método: Revisão das principais recomendações das sociedades científicas nacionais, internacionais, decisões de órgãos governamentais e de conselhos de classe. Os tópicos serão relativos aos aspectos gerais do COVID-19, equipamentos de proteção individual, cuidados no atendimento ao paciente, as rotinas dos exames endoscópicos e o manejo de aspectos nasossinusais, otológicos e pediátricos relacionados ao COVID-19. Resultados: É considerado crucial o uso de equipamento de proteção individual no atendimento otorrinolaringológico de rotina. Recomendamos postergar atendimentos, exames e cirurgias eletivas para diminuir a propagação do COVID-19. Da mesma forma, recomendamos mudança de rotinas em diversas áreas da otorrinolaringologia. Além disso, orientações sobre o uso do recurso da telemedicina durante o período de vigência da pandemia. Conclusões: Estamos ainda no início da pandemia do COVID-19 e as evidências científicas são ainda escassas, por isso essas recomendações da ABORL-CCF para os otorrinolaringologistas podem sofrer atualizações baseadas nos novos conhecimentos e no padrão de disseminação do novo coronavírus.

Abstract Introduction Over the years the immune system suffers many morphologic and functional alterations, which result in a peak of function in puberty and a gradual decrease in the elderly. Aim Treat patients 55 years or older with allergic rhinitis with immunotherapy and then analyze the response to allergens. Materials and Methods From June 2009 to July 2010, 104 charts of patients 55 years or older with allergic complaints were evaluated. The patients were selected by anamnesis, physical examination, and otorhinolaryngologic exam. The patients had cutaneous test for mites before and after 1 year of sublingual specific immunotherapy. The cutaneous response was classified as negative (absent), light, moderate, or severe. Results Before vaccination, 42 (40.4%) patients were classified as having a severe form of allergy and 62 (59.6%) as having a moderate allergy. After the specific therapy, 40 (38.4%) patients were classified as negative (absent), 37 (35.6%) as light, 19 (18.3%) as moderate, and 8 (7.7%) as severe responses. Conclusion Immunotherapy, a desensitization technique, is indicated in cases which patients cannot avoid the exposure to allergens and in situations where pharmacologic therapy is not ideal. Specific immunotherapy to treat the allergic rhinitis in elderly patients was efficient and had no collateral effects, and in addition to the clinical benefit, improvement in the cutaneous test could also be observed.

As evolutivas alterações no uso e ocupação do solo, decorrentes do processo de urbanização, resultam em modificações no clima das cidades, afetando o desenvolvimento do meio e a qualidade de vida da população. A temperatura da superfície é um importante parâmetro biofisico relacionado, dentre outros fatores, com o tipo de cobertura do solo e que influencia indiretamente a temperatura do ar. Palmas, capital do Tocantins, teve sua implantação no ano de 1989, passando por processos acelerados de crescimento populacional e expansão do seu Plano Diretor, sendo necessário o monitoramento das condições climáticas da região. Esse estudo propõe a obtenção e análise da temperatura de superfície em Palmas, a partir de imagens de satélites do TM Lansat 5 para os anos de 1991, 2003 e 2010 e da utilização do algoritmo Surface Energy Balance Algorithm for Land - SEBAL visando identificar a variação multitemporal da temperatura da superfície, as mudanças ocorridas na cobertura da superfície e a ocorrência de Ilhas de Calor. Os resultados obtidos permitiram verificar que, no decorrer dos anos de estudo, houve uma intensificação na temperatura da superfície da região e que há a ocorrência de ilhas de calor com amplitude térmica de 11°C.

PURPOSE: To evaluate the deposition of collagen fibers at pig's vocal folds after topical use of mitomycin or 5-fluorouracil, when partial exeresis of mucosa layer had been promoted by CO2 laser. METHODS: There were used 18 Larger white pigs which were anesthetized and submitted to mucosa fragment's exeresis, bilaterally, at its free border. The animals were divided into 3 groups, each one with 6 animals: control group, without topical drug application; mitomycin group; and 5-fluorouracil group. After 30 days, the animals were subjected to euthanasia, and samples of the vocal folds were collected and stained by picrosirius red technique with polarization for quantification of total collagen deposition. RESULTS: In control group, the mean rate of right vocal fold's collagen deposition at submucosa consisted in a 3428.66 micrometers area. There was found an area whose size had, in average, 2196.36 micrometers, in mitomycin group, and 2269.19 micrometers, in 5-fluorouracil group. CONCLUSION: Mitomycin and 5-fluorouracil had promoted beneficial change in vocal fold's cicatrization with less collagen deposition, but there was no significant statistically difference when they were compared between themselves.OBJETIVO: Avaliar a deposição das fibras de colágeno total em pregas vocais suínas após o uso tópico de mitomicina ou 5-fluorouracil nas exéreses parciais de mucosa com laser de CO2. MÉTODOS: Foram utilizados 18 porcos da raça Larger white anestesiados e submetidos à exérese de fragmento de mucosa de borda livre da prega vocal direita e prega vocal esquerda. Os animais foram divididos em 3 grupos com 6 animais cada: grupo controle, sem aplicação de medicação tópica; grupo mitomicina, com uso tópico dessa substância; grupo 5-fluorouracil, uso tópico. Após 30 dias do experimento os animais foram submetidos à eutanásia, coletadas amostras das pregas vocais e coradas pela técnica do picrosirius red com polarização para a quantificação computadorizada da deposição do colágeno total. RESULTADOS: No grupo controle, a média da área do colágeno depositado na submucosa da prega vocal direita foi de 3428,66 micrômetros. No grupo mitomicina a média foi de 2196,36 micrômetros. No grupo 5-fluorouracil, a média foi de 2269,19 micrômetros. CONCLUSÃO: A mitomicina e o 5-fluorouracil promoveram mudança benéfica na cicatrização da prega vocal, com menor deposição de colágeno, porém, quando comparados entre si, eles não apresentaram diferença estatisticamente significante.

INTRODUÇÃO: A sutura dos tecidos e sua cicatrização é um dos fundamentos básicos da cirurgia e a procura de substâncias que melhorem este processo é desafio constante. O uso de substâncias retiradas de plantas tem sido testado há muitos anos, contudo sem ainda terem comprovação científica consolidada. OBJETIVO: Comparar as alterações macroscópicas e histológicas proporcionadas pelo uso do extrato bruto da Jatropha gossypiifolia L. intraperitonial, na cicatrização de suturas realizadas na bexiga urinária de ratos. MÉTODOS: Quarenta ratos da linhagem wistar, adultos, machos foram distribuídos em dois grupos, experimento e controle. Incisão longitudinal de 1cm na parede ventral da bexiga e síntese em plano único com pontos separados de poliglactina 910 5-0 (ethicon) foi realizada nos dois grupos. No controle, instilou-se na cavidade peritoneal água destilada na proporção de 1ml/kg de peso, enquanto no grupo Jatropha utilizou-se o extrato bruto de Jatropha gossypiifolia L. na proporção de 1ml/kg, que representava 200mg/kg de peso do fitoterápico. Cada grupo foi subdividido em dois subgrupos de 10 animais, sendo estes submetidos à eutanásia no 3º e 7º dia pós-operatório. Foi feita análise macroscópica e histológica comparativa entre os subgrupos. RESULTADOS: No 3º dia foi observada diferença estatisticamente significante nas variáveis inflamação aguda, neoformação vascular e colagenização, sendo a primeira, maior no grupo controle e as duas últimas no grupo Jatropha; no 7º dia as variáveis inflamação aguda e proliferação fibroblástica apresentaram-se mais intensas, com significado estatístico, no grupo controle. CONCLUSÃO: Não se observou efeito favorecedor cicatrizante do extrato bruto da Jatropha gossypiifolia L., intraperitonealmente aplicado, na bexiga urinária de ratos.INTRODUCTION: The suture of tissues and their healing process is one of the basic fundamentals of surgery, and the research on substances which could improve the process is an ongoing challenge. The use of plants has been tested but till nowadays without scientific demonstration. PURPOSE: To compare the macroscopic and histological alterations done by the use of intraperitoneal Jatropha gossypiifolia L. gross extract, in the healing process of sutures performed on the bladder of rats. METHODS: Forty adult, male, Wister rats were distributed into two animal groups. One centimeter longitudinal incision on the bladder ventral wall, and single plane synthesis with separated polyglactine 910 5-0 (Ethicon) stitches was done in all the animals. In the control group an intraperitoneal cavity instillation of distilled water at a ratio of 1ml/kg/weight, and a 1ml/kg/weight of Jatropha gossypiifolia L. extract for the Jatropha group was done representing 200 mg of the substance. Each group was subdivided into two with 10 animals in each, being submitted to euthanasia on 3th and 7th post-operative days. Comparative histological, macroscopic and statistical analysis were undertaken between the subgroups. RESULTS: Statistical significant difference was observed in the acute inflammation changes, vascular neoformation and bonding on day 3; the first one was greater in the control group, and the latter two greater in the Jatropha group; acute inflammation variables and fibroblastic proliferation presented to be more intense on day 7, with statistical significance favoring the control group. CONCLUSION: No favorable healing effect was observed with the administration of single intraperitoneal dose of Jatropha gossypiifolia L. gross extract on the sutures of bladder in rats.
