Still, no studies captured patients’ bottom-up perspectives regarding healthcare at the time of COVID-19 at a micro-level in a range of different countries. We, therefore, explored the perspectives of patient representatives in all six World Health Organisation (WHO) regions and extracted suggestions for care redesign after the pandemic.Methods: We conducted semi-structured interviews with patient representatives until saturation. Thematic analysis followed a modified form of meaning condensation. We established rigour by transcript checking, inter-coder agreement, quote variation and standardised reporting.Results: Disadvantaged people experienced an unprecedented inequity in healthcare from limited access to physical violence. The narratives revealed the extent of this inequity, but also opportunities for health workers to act and improve. Stigmatisation from COVID-19 differed between cultures and countries and ranged from none to feeling “ashamed” and “totally bashed”. While experienced as indispensable in the future, patients refused telehealth when they were given “bad news”, such as having an eye removed because of melanoma, and in end-of-life care. Patient representatives redefined their role and became indispensable influencers throughout the pandemic and beyond.Conclusion: We reached out to patient representatives with diverse perspectives, including those who represent minorities and marginalised patient populations. Since preferences and personal meanings drive behaviour and could be foundations for targeted interventions, they must be considered in all groups of people to increase society’s resilience as a whole. Exosomes also participate in relocation of functional lncRNAs between cells. Methods: In the present study, we evaluated expression of LINC00355, LINC00958, UCA1-201, UCA1-203, and MALAT1 lncRNAs in urinary exosomes isolated from transitional cell carcinoma (TCC) of bladder, non-malignant urinary disorders, and normal subjects. Results: LINC00355, UCA1-203, and MALAT1 expression was significantly higher in TCC patients compared to controls (non-malignant or normal samples). However, UCA1-201 expression was significantly decreased in TCC patients compared with controls. LINC00355 and MALAT1 expression was significantly lower in cigarette smokers and opium-addicted TCC patients, respectively. On the other hand, LINC00355 expression tended to be higher in opium-addicted TCC patients. The proposed panel of lncRNAs (composed of UCA1-201, UCA1-203, MALAT1, and LINC00355) had 92% sensitivity and 91.7% specificity for diagnosis of bladder cancer from normal samples. Conclusion: Transcript levels of lncRNAs in urinary exosomes are potential diagnostic biomarkers in bladder cancer. Keywords: lncRNA, exosome, bladder cancer

5. Drift ion-acoustic waves in a nonuniform rotating magnetoplasma with two-temperature superthermal electrons

Abstract

The theory of low-frequency (in comparison with the ion cyclotron frequency), long wavelength, electrostatic drift ion-acoustic waves (IAWs) is studied in a nonuniform rotating magnetoplasma with two temperature superthermal electrons. In the linear limit, the coupling of IAWs and drift waves by the density inhomogeneity is shown to produce a new wave mode which typically depends on the density gradient, the rotational frequency and the spectral indexes of superthermal electrons. In the nonlinear regime, an evolution equation for the drift IAWs is derived by the dispersion approach, and using the Jacobi elliptic function expansion technique its exact solitary and periodic wave solutions (namely, cnoidal and dnoidal) are also obtained. The properties of these solutions are numerically examined and it is found that they are significantly modified by the effects of the background density gradient, the superthermality of electrons and the Coriolis force associated with the rotational motion of ions., Comment: 10 pages, 3 figures; Revised version to appear in Physica Scripta (2022)

Abstract

Intrauterine infection/inflammation (IUI) is a frequent complication of pregnancy leading to preterm labor and fetal infllammation. How inflammation is modulated at the maternal-fetal interface is unresolved. We compared transcriptomics of amnion (a fetal tissue in contact with amniotic fluid) in a preterm rhesus macaque model of IUI-induced by lypopolysaccharide with human cohorts of chorioamnionitis. Bulk RNA-seq amnion transcriptomic profiles were remarkably similar in both rhesus and human subjects and revealed that induction of key labor-mediating genes such as IL1 and IL6 was dependent on NF-kB signaling and reversed by the anti-TNF antibody Adalimumab. Inhibition of collagen biosynthesis by IUI was partially restored by Adalimumab. Interestingly, single-cell transcriptomics, flow cytometry, and immuno-histology demonstrated that a subset of amnion mesenchymal cells (AMCs) increase CD14 and other myeloid cell markers during IUI both in the human and Rhesus macaque. Our data suggest that CD14+ AMCs represent activated AMCs at the maternal-fetal interface.

Abstract

The impact of a range of different threats has resulted in the listing of six out of seven sea turtle species on the IUCN Red List of endangered species. Disease risk analysis (DRA) tools are designed to provide objective, repeatable and documented assessment of the disease risks for a population and measures to reduce these risks through management options. To the best of our knowledge, DRAs have not previously been published for sea turtles, although disease is reported to contribute to sea turtle population decline. Here, a comprehensive list of health hazards is provided for all seven species of sea turtles. The possible risk these hazards pose to the health of sea turtles were assessed and “One Health” aspects of interacting with sea turtles were also investigated. The risk assessment was undertaken in collaboration with more than 30 experts in the field including veterinarians, microbiologists, social scientists, epidemiologists and stakeholders, in the form of two international workshops and one local workshop. The general finding of the DRA was the distinct lack of knowledge regarding a link between the presence of pathogens and diseases manifestation in sea turtles. A higher rate of disease in immunocompromised individuals was repeatedly reported and a possible link between immunosuppression and environmental contaminants as a result of anthropogenic influences was suggested. Society based conservation initiatives and as a result the cultural and social aspect of interacting with sea turtles appeared to need more attention and research. A risk management workshop was carried out to acquire the insights of local policy makers about management options for the risks relevant to Queensland and the options were evaluated considering their feasibility and effectiveness. The sea turtle DRA presented here, is a structured guide for future risk assessments to be used in specific scenarios such as translocation and head-starting programs.

Abstract

Background & Aim: Approximately 75% of patients with cancer experience acute nausea and vomiting (24 hours after chemotherapy). The aim of this study was to determine the effect of auricular acupressure in relieving acute nausea and vomiting among women with breast cancer receiving chemotherapy . Methods & Materials: This study was a posttest only randomized clinical trial conducted on 48 women with breast cancer receiving chemotherapy in Imam Khomeini and Hazrat Rasoul Akram hospital. Before chemotherapy and after obtaining informed consent, the patients were randomly allocated into intervention and control groups. Intervention group received routine medications for controlling nausea and vomiting as well as auricular acupressure. Meanwhile, the control group received only the routine medications. The data were gathered using demographic and Morrow Standard questionnaire; and were compared using statistical tests in the SPSS-21 . Results: T he results showed that using auricular acupressure led to a decrease in the number and the intensity of nausea at acute phases (P=0.001). Acupressure application did not lead to any complications in the patients . Conclusion: According to the results of this study, using auricular acupressure on Shenmen, Point zero, stomach, Brainstem, and cardia points can relieve nausea and vomiting at acute phase of chemotherapy. It is suggested that nurses use this pressure technique as a complementary treatment for relieving nausea and vomiting caused by chemotherapy. &nbsp

Abstract

Introduction: One of the most frequent issues nurses encounter with after or during cancer-related surgeries is pain. For postoperative pain relief, different nonpharmacologic interventions, such as nurse-patient communication, mental support, preoperative education, and consultation can be used. Doing such interventions may decrease postoperative pain. However, the research results regarding the effect of such measurements on pain intensity is contradictory. So in this research study the effect of a pain management plan on pain intensity after cancer surgery was assessed. Materials and Methods: The present study is a pre- and posttest case and control clinical trial, which was conducted in Sayyed-Al-Shohada Hospital of Isfahan University of Medical Sciences in 2011. Care program consisted of pain education, communication with the patient, and pain measurement. Seventy patients were sampled based on the inclusion criteria and randomly assigned in 2 groups. Data were collected using American Pain Society-patient outcome questionnaire, which measured pain intensity. Results: In the experimental group, the mean score of pain intensity before surgery and in the first 12-24 h after surgery was less than the control group. Also comparing pain intensity mean differences before and in the first 12 h, before and in the first 24 h indicated that the experimental group had lower scores than the control group, but these differences were not statistically significant. In both the groups, in the first 24 h following surgery the mean score of pain intensity decreased significantly. Discussion: Results of the present research study suggested that a nursing pain management program consisting consultation, education, and pain assessment may have a clinical effect on cancer patient pain intensity following surgery. However, these results were not statistically significant. This might be due to the limited sample size as well as conducting the program in a short period of time. It is recommended that effects of such a program on the pain intensity will be examined further with a larger sample and in a longer period of time.
