Objective (s): Health system responsiveness and meeting patients' expectations are important in creating public trust in the health system. This study was conducted to determine the relationship between health system responsiveness and public trust in the health system in Gorgan, Iran. Methods: This cross-sectional study was conducted in 2020. The sample size in this study was 450 households. To collect data, a health system responsiveness questionnaire including 32 questions in 8 dimensions, and a public trust questionnaire including 25 questions in 6 dimensions were used. The validity and reliability of the instruments were confirmed. The path analysis was used in AMOS software to determine the relationship between responsiveness and public trust in the health system. Results: The mean score of responsiveness was 3.4 (±0.53) out of 5, the highest score was related to social support and the lowest was related to the right to choose a doctor. The mean score of public trust was 3.3 (± 0.52) out of 5, the highest score was related to the quality of care, and the lowest was related to macro health policies. The hypothesis test findings showed a positive and significant relationship between all dimensions of responsiveness and public trust in the health system. In general, 64% of the variance in public trust is explained by the health system responsiveness. Conclusion: The health system responsiveness and public trust in the health system were evaluated at an average level, and responsiveness plays an important role in influencing public trust in the health system. Therefore, the government should play its role in properly managing citizens' needs in order to regulate its credibility and most importantly to maintain public trust.

Abstract Background Assessing the quality of health services gives insights to managers about the status of services delivered by them, especially from the client’s perspective. Although various tools have been developed to measure the quality of primary health care (PHC), no specific tool was found in this field in Iran. Therefore, the present study was conducted to develop and validate the quality assessment tool of PHC in Iran. Methods This methodological study was conducted in 2021. In the first step, based on a literature review, an initial questionnaire was designed, and its face validity, content validity, construct validity, and reliability were evaluated. Descriptive tests, Kolmogorov-Smirnov, exploratory factor analysis, Kaiser-Myer-Olkin (KMO), and Cronbach’s alpha were performed by using SPSS 22. Results The initial questionnaire included 33 items, of which three items were removed due to inconsistency with factorization. The final questionnaire consisted of 30 items and nine dimensions: interaction, efficiency, timeliness, accuracy, consultation, tangibility, safety, accessibility, and environment. The KMO and Cronbach’s alpha index values were 0.734 and 0.82, respectively, indicating acceptable reliability and validity. The developed dimensions represented about 73% of the total variance. Conclusion The designed questionnaire has relatively good validity and reliability and can be used to measure the quality of PHC and to identify weaknesses in service delivery. However, researchers should carefully examine it to enhance its applicability as a standard tool for measuring PHC quality.

Abstract Background Cancer results from the accumulation of mutations in critical genes, such as DNA repair genes. But these genes are a double-edged sword, because the basis of current cancer treatment is DNA damage from chemotherapy and radiation, and the repair system can slow the healing process by repairing the induced damage. Therefore, any substance that can reduce the DNA repair capacity of cancer cells can make the cells more sensitive to treatment. Metformin and curcumin, as low-complication compounds, can play this role well. Methods In the present work, changes in the expression of CASP3, BAX, and BCL2L1 apoptotic genes, and nine genes involved in DNA repair pathways (XRCC1, XRCC2, XRCC3, XRCC4, XRCC5, XRCC6, XRCC7, BRCA1 and BRAC2) were measured comparatively by real-time PCR in AGS gastric cancer cell line under single and co-treatments with metformin and curcumin. Results Our findings showed that co-treatment of metformin and curcumin induced decreasing the expression of anti-apoptotic BCL2L1 and increasing expression of proapoptotic CASP3 and BAX. Metformin decreased the expression levels of seven genes, while curcumin did not alter the expression levels. The co-treatment of metformin and curcumin showed that although the XRCC2, XRCC3, XRCC4, XRCC5, XRCC6, XRCC7, BRCA1, and BRCA2 were down-regulated, there was no difference between metformin and co-treatment for mRNA levels. Conclusion Our results suggest that metformin increases the sensitivity of cancer cells to anticancer drugs by suppressing several DNA repair pathways and that curcumin may induce apoptosis.

Introduction and purpose: Despite the availability of vaccines, the use of face masks remains a recognized and effective measure in preventing the spread of COVID-19. However, it is crucial to comprehend the underlying factors influencing people's adherence to preventive behaviors, particularly mask usage within communities, as negligence and disregard for this matter persist. Therefore, the present study aimed to identify the reasons for not using masks by people during the COVID-19 pandemic from the point of view of health experts. Methods: This research employed a qualitative approach and content analysis in 2019. A targeted sampling method was employed, and 15 healthcare experts from Kashan and Aran and Bidgol, Iran, participated in this study. Participants were selected to represent maximum diversity in terms of age, gender, and education level. Semi-structured interviews were conducted as the primary method of data collection. The duration of the interviews varied between 20-30 min. The interview transcripts were carefully documented, extracted, and segmented into meaningful units. To enhance the research's reliability and confirm the accuracy of the findings, credibility criteria were employed to validate the collected data. Results: Out of the interview participants, 66.6% held a bachelor's degree, 13.4% possessed a master's degree, and 20% had a doctorate degree. Around 80% of the participants were male, with 53.5% falling within the 40-50 age range, and all were married. During the data analysis process, 40 core concepts sharing common characteristics were categorized into six subclasses, which were further condensed into three main classes based on abstract commonalities. The research findings were classified into three main categories: individual factors, interpersonal factors, and environmental factors. The subcategories included lack of awareness, behavioral and belief patterns, physical barriers, subjective norms, economic barriers, environmental barriers, inappropriate mask structure, and health policies and guidelines. Conclusion: Based on the findings of this research, in addition to individual factors (e.g., awareness and beliefs regarding preventive behaviors), interpersonal factors, abstract norms, and economic and environmental factors can significantly influence mask usage within a society. This finding highlights the necessity of government organizations' support to address these challenges effectively.

Abstract Background Quality healthcare services are considered one of the most effective vehicles for healthcare managers to achieve organizational goals. Therefore, this study aimed to combine the findings of comparable studies to identify consistencies and contradictions in the quality of outpatient services in Iran. Methods The current systematic review and meta-analysis study was conducted in 2022 according to PRISMA guideline. All relevant English and Persian studies were searched in databases, including Web of Sciences, PubMed, Scopus, Scientific Information Database, and Magiran. No year restriction was applied. The quality of the studies was assessed by the 22-item Strengthening the Reporting of Observational Studies in Epidemiology checklist. The meta-analysis was conducted by using Open Meta Analyst, and between-study heterogeneity was investigated with I-squared statistic. Results Of the 106 retrieved articles, seven studies with a total sample size of 2600 were included in the meta-analysis. The pooled estimate of mean for overall perception was 3.95 (95% CI: 3.34- 4.55, P< 0.001, I2= 99.97), while the pooled estimate of the mean for the overall expectation was 4.43 (95% CI: 4.11- 4.75, P< 0.001, I2= 99.93). The highest and lowest perception mean scores were related to tangibility (3.52, Gap= -0.86) and responsiveness (3.30, Gap= -1.04) dimensions. Conclusion Responsiveness was identified as the weakest dimension. Therefore, managers are recommended to design suitable workforce-development programs which focus on the provision of timely and prompt services, polite and courteous interactions with patients, and prioritization of patients’ needs. Moreover, training public sector practitioners along with incentives can fill up the existing gaps.

Background and Aim: Medical tourism refers to travel for receiving health services abroad. The COVID-19 crisis affected a wide range of tourism-related businesses, including medical tourism, and caused a decrease in the supply and demand of health services in this market. This study aimed to investigate the impact of the COVID-19 pandemic on Medical Tourism in Iran. Materials and Methods: This cross-sectional study was conducted in Iran in 2021. The data used in this study were extracted from the information system of the Ministry of Health from 2018 to 2021, which includes the number, origin, and destination of medical tourists, per capita revenue, and monthly revenue two years before and one and a half years after the COVID-19 pandemic. Introduction and purpose: One of the dangers that environmental health experts face at work is violence, the incidents of which are observed every year. The type and extent of this violence depend on the ethics of the operator. This s tudy aimed to determine the s tate of violence among environmental health experts in Kashan and Aran va Bidgol, Isfahan, Iran. Methods: In this descriptive s tudy, 53 environmental health inspectors working in the centers affiliated with Kashan University of Medical Sciences, Kashan, Iran, were selected. The data collection tool was a 58-item reliability-validated ques tionnaire consis ting of four parts, including physical violence (18 items), psychological violence (12 items), sexual violence (12 items), and racial violence (12 items), and there were three open ques tions at the end of the ques tionnaire. After the ques tionnaire was completed, information was recorded in SPSS software, and analysis was performed using independent t, Chi-square, and Fisher s tatis tical tes ts. Results: According to the results, 30% of the subjects experienced a physical attack, 90% verbal violence, and 7% racial violence in the pas t three years. No cases of sexual violence were reported. Moreover, the curator's insult to the inspectors had no relationship with the inspectors' behavior and was influenced by factors related to the curator himself and cultural and social factors. Conclusion: Based on the results, a combination of personnel training and improving their communication skills, as well as behavioral training for attendants, would be effective in reducing cases of violence.

Background and Aim: In recent years, public trust in health system has been considered one of the performance evaluation indicators of health systems. However, most of the research on public trust in health system is related to developed countries, and research in this regard is limited in Iran. A better understanding of trust in health system and some influential factors helps to develop targeted interventions to increase trust. The present study was conducted to investigate the level of public trust in health system in Rasht County. Materials and Methods: In this cross-sectional study, 680 households (510 urban and 170 rural) were included. The data collection tool was a questionnaire to measure public trust in health system with 30 items and six dimensions, whose validity and reliability were confirmed. Mann-Whitney and Kruskal-Wallis tests were used to compare and analyze the difference in public trust in health system between groups. Results: The overall score of trust in health system was 66.7 out of 112. The highest score for the quality-of-care dimension was 16.38 out of 28, and the lowest for the cooperation quality of providers dimension was 7.47 out of 12. Among service providers, the highest trust was in nurses and specialist doctors, and the least was in traditional medicine service providers. Background: Outsourcing is the strategy of reducing expenses and improving organizational quality whose evaluation in healthcare organizations is of special importance. This study aims to investigate the status of outsourced pharmacies of Alborz University of Medical Sciences based on personnel perspectives and available documentation. Methods: This was a descriptive analytical and cross-sectional study conducted in outsourced pharmacies of 10 teaching hospitals of Alborz University of Medical Sciences. Expenses, revenues and working time were derived from the hospitals’ information centers and the documents. 128 relevant officials were selected by census to investigate the views of managers and authorities of clinical wards on outsourcing. Authors analyzed data using descriptive statistics, t-test and chi-square test through SPSS22 software. All cost was converted to US dollar by using the average exchange rate in 2019 (1USD=110.000IR). Results: The average satisfaction of managers and staff with outsourcing pharmacies were 4.20 (82 %) and 4.45 (80 %) out of 5. The highest level belonged to medicine expiration date with the average of 4.64, and the lowest score was related to medicinal status at the time of delivery with the average of 4. Consumption expenses for a bed in outsourced pharmacies regarding all the hospitals were 123 $, and the profitability of the pharmacy for one bed was134 $ (5.540.900 Riyals). Conclusion: Based on the results, the following components had a relatively proper quality and an acceptable quality in all the respects of the staff’s view regarding the conditions in the pharmacies. They can be inferred as the most salient characteristics of the present study in investigating the quality of outsourcing the pharmacies of the teaching hospitals in Alborz medical sciences. The important components include: The time spent by executive managers of the pharmacy’s affairs, decrease of the manager’s workload, improvement of management, increase in management’s satisfaction in the organization and etc.

Coronavirus 2019 infection (COVID-19) has a broad spectrum of clinical complications, some unrecognized. Herein, a case of a diabetic patient with multiple episodes of hemoptysis 2 months following her recovery from SARS-CoV-2 infection is reported. The initial computed tomography (CT scan) revealed the left lower lobe collapsed secondary to bronchial narrowing and obliteration. Bronchoscopy was performed, indicating necrotic endobronchial tissue, which was confirmed histopathologically as invasive mucormycosis. Bronchial necrosis due to mucormycosis is an unusual presentation of COVID-19-associated pulmonary mucormycosis. The accurate diagnosis could be challenging as it can resemble other pathologies such as malignancies. Therefore, it is crucial to identify this fatal complication in patients with prolonged COVID-19 and lung collapse.

Background and Aim: With the outbreak of the COVID-19 pandemic, the performance of hospitals were affected, and changes were made in the utilization of hospital services. Analyzing hospital performance data during the COVID-19 pandemic can provide insights into service utilization patterns and care outcomes for managers and policymakers. This study was conducted to investigate the impact of COVID-19 on selected outcome indicators in the hospitals of Shahid Beheshti University of Medical Sciences, Tehran. Materials and Methods: This research was descriptive-analytical and of the time series analysis type. Six outcome indicators were considered: hospitalization rate, bed occupancy rate, the average length of stay, emergency visits, laboratory tests, and imaging requests. Related data from 12 affiliated hospitals from 2017-2019 (pre-COVID) and 2020 (post-COVID) were obtained from the hospital's intelligent management system. The data were analyzed using R software's interrupted time series analysis method. Results: The hospitalization rate (P=0.015), bed occupancy rate (P=0.04), and the number of laboratory tests (P=0.003) significantly increased immediately after the outbreak of the pandemic. In contrast, emergency visits (P=0.034) have significantly decreased. The bed occupancy rate and the number of imaging requests showed no significant change. The decrease in emergency room visits within one year after the pandemic was significant, but the changes in other outcome indicators were non-significant (P>0.05). Conclusion: Understanding the changes and impact of a major event on hospital outcome indicators is necessary for decision-makers to effectively plan for resource allocation and effective pandemic response. The outbreak of COVID-19 has caused a change in performance and hospital outcomes by affecting the supply and demand of services. In a year after the pandemic's beginning, except for emergency visits, the other indicators have not experienced significant changes. Preservation of essential services such as emergency room visits is recommended in the strategy of rapid response to an epidemic outbreak and public campaigns to encourage people to seek medical care if needed in future waves of the pandemic.

BackgroundPayments to physicians by the pharmaceutical industry are common, but recent evidence shows that these payments influence physician prescribing behavior in the form of increased prescription of brand-name drugs, expensive and low-cost drugs, increased prescription of payer company drugs, etc. Considering that these payments increase drug costs for patients and health systems, there is a public interest in controlling them. Therefore, this study aimed to identify and propose policy options for managing physician-pharmaceutical industry interactions in the context of Iran's health system.MethodsIn the first phase, a systematic search was conducted to identify relevant policies and interventions in Web of Science, PubMed, and ProQuest databases from 2000 to 2022. Then, the opinions of the research team and an expert group (physicians, health policy and transparency experts, and industry representatives) were used to categorize the interventions and propose policy options along with their advantages, disadvantages, and implementation considerations.ResultsIn the search, 579 articles were retrieved, and 44 articles were found suitable for the final analysis. Twenty-nine interventions and strategies were identified, and based on these; Five policy options were identified: prohibition, restriction, physician self-regulation, voluntary industry disclosure, and mandatory industry disclosure.ConclusionThe proposed policies in our study include advantages, challenges, and implementation considerations based on up-to-date evidence that can help policymakers use them to manage COI in physician-pharmaceutical industry interactions in Iran's health system. A combination of measures seems to help manage COI: firstly, using self-regulating physicians and industry to institutionalize transparency, and in the next step, implementing mandatory industry disclosure policies and establishing restrictions on some financial interactions.

Gastric cancer has a weak prognosis and its usual treatments depend on surgery and chemotherapy. These treatments suffer from some drawbacks such as high rates of local recurrence and metastasis, low survival rates, and significantly decreased life quality. Therefore, new therapeutic methods for improved gastric cancer care with minimal side effects seem necessary. Currently, combinatorial treatments for cancer are preferred and recently, metformin (Met) and curcumin (Cur) have been interesting options for this type of therapy. The aim of the present study was to investigate anticancer effects of metformin and curcumin in both single and combinatorial treatment forms on AGS gastric cancer cell line. In comparison to single treatments with each substance, the results of co-treatments with metformin and curcumin indicated synergistic inhibitory effects on cell viability, wound healing, cell migration and invasion, and primary tumor formation. To determine the selective effect of combination of “Met + Cur” on cancerous cells, very low doses of 8 anticancer drugs (cisplatin, carboplatin, oxaliplatin, epirubicin, doxorubicin, docetaxel, paclitaxel, and methotrexate) used in MTT assay were comparatively tested on AGS cancer cells and normal HDF cells for 48 and 72 hours. The results indicated that the combination of “Met + Cur” significantly increased cytotoxic effects of all anticancer drugs of AGS cells. It is while in normal HDF cells, combination of “Met + Cur” along with anticancer drugs had no effect. This can be inferred as selectively additive effect.

This paper aims to identify, describe and analyze the effective factors in creating visual order in Jiroft soapstone works based on documentary studies and with the aim of identifying different types of painted works. Accordingly, the authors try to answer the question with documentary studies and descriptive-analytical method in order to answer the question: Artist or craftsman of Halilrood archaeological site, heir or founder of which artistic experience? Since the designs performed on these works are ultimately designed and executed in beauty, balance, fit and balance, it can be assumed that their creators, in addition to being aware of the artistic application of lines and designs, also have little knowledge of visual knowledge and basics in the field. Findings show that engineering and visual understanding of elements at the level of objects, in the direction of structural interaction of their line and role, and achieving unity in works of art has been one of the most important goals of their construction. The artist / craftsman of Halilrood has a relatively clear knowledge of his work as a work of art and has tried to create a relationship between design and background, perspective, diversity and statism by repeating figures and elements.

Introduction: Competency-based education seeks to bridge the gap between the labor market and the academic education system. Due to the problems of graduates of this field in the professional contexts, this study endeavored to up to date the curriculum of the field based on the expected job qualifications of the health system. Methods: This qualitative and applied study with the focus-group discussion method was conducted in the Faculty of Management and Medical Education of Shahid Beheshti University of Medical Sciences in the academic year 2019. To discuss the group, 13 faculty members and experts, graduates as well as two hospital managers were selected to identify the differences between academic learning and the professional requirements, and then to evaluate the content and topics of the Master of Health Services Management course. At the end of the group's consensus, notes were taken on the agreed results. Results: The data analysis revealed that the essential competencies are like this: leadership, effective teamwork, problem-solving, system thinking, and quality improvement. Changes were suggested in the content of existing courses, like working abilities with STATA software, expertise to design solutions by systems thinking methodologies, awareness of the ultimate challenges of the decisions without supporting evidence, and capacity to compare and criticize strategic planning process and outcomes. Conclusion: Findings revealed that modifications to current courses' content were suggested by adding new curriculum approaches to cover the required skill set. Given changes in the formal curriculum is a time-consuming process, transforming the content of existing courses is recommended.

Background: Insurances are the most important financial sources of hospitals but sometimes insurance companies deduct amounts as deductions from the sum of requested amounts after investigating financial documents sent to hospitals monthly. The current study aims at determining the level and reasons for insurance bill deductions in AL Zahra Hospital in Isfahan. Methods: This applied study is descriptive-cross sectional in method and has been conducted retrospectively. The statistical population of this study contained file information of hospitalized patients who have been referred to Isfahan AL Zahra Hospital in 2016. The sample size determined by Cochran Formula and 400 files has been investigated. Random sampling employed. The information has been extracted from patients through a checklist whose reliability and validity has been confirmed by insurance experts and management specialists. Qualitative methods of interview used to identify the most important reasons for increasing the deduction sums and statistical indices and descriptive scales (frequency, percent etc) have been analyzed. Results: The most and the least percent of insurance deductions are related to Armed Forces Insurance (36.8 %) and Social Security Insurance Organization (3.64 %) respectively. Also, the most deductions of hospitalized files were related to visiting deductions of hospitalized patients’ doctor and drug and consumables deduction, other services, surgeon and anesthetist commission, experiments and pathology, radiology and sonography are in next rates and the least deductions is related to doctors 2k visit cost, hoteling, and surgery room. Conclusion: According to high percent of insurance deductions and investigation of its reasons, it has been revealed that hospitalized files related to visit by resident year 1 and 2 and absence of resident physician in center and the academic status of hospital that managers have to clarify this issue and use residents year 3 and 4, attending and resident physicians to reduce difference between hospitals and insurance companies and consider documentation as one of main elements of their agenda. So training all individuals who are influential on patients’ bills such as doctors and nurses is essential.

Background and Objectives: Awareness about prescribing pattern and comparing that with standards of the World Health Organization (WHO) can affect health policy-makers in adopting policies and implementing necessary interventions for rational use of drugs. The aim of this study was to determine the pattern of drug prescribing in outpatients covered by Social Security Organization in Semnan province. Material and Methods: In this study, 952,712 prescriptions in Semnan province in 1396 were investigated and indicators of use of drugs include average price of each prescription, average number of drugs in each prescription, percentage of prescriptions containing 4 drugs or more, percentage of prescriptions containing antibiotics, injections and corticosteroids, physicians by specialty and by the length of activity were extracted and compared with those provided by the WHO standards. Results: The average price of prescriptions was 254,377 IRR and average number of drugs per prescription was 2.65. Generally, 26.9 percent of the prescriptions included 4 or more drugs. Percentage of the prescriptions containing antibiotics, injections and corticosteroids was 55 percent, 29 percent and 11.1 percent, respectively. All of the examined indicators were higher than the recommended level by the WHO. Conclusion: This study showed some evidence of irrational drug prescription especially in the polypharmacy, antibiotics and injectable drugs. Therefore, it is suggested that adequate supervision of the prescribing and training of prescribing principles for physicians should be considered in continuing education programs.

Methods: This descriptive-analytical study was performed on 307 nurses selected through random sampling in 2016. Data gathering tools were two questionnaires of ethical climate and organizational citizenship behavior. Data analysis was done through SPSS20 and using descriptive statistics, t-test, One- Way ANOVA, correlation and multiple regression tests. Results: Mean scores of ethical climate and citizenship behavior were respectively 3.51 ±0.55 and 3.49 ± 0.45 that reflects the favorable view of nurses towards ethical climate and citizenship behavior. Head nurses had the highest mean scores in the dimensions of ethical climate. Among the dimensions of citizenship behavior, the dimension of conscience gained the highest mean score among nurses. The aim of this study was to evaluate the relationship of quality of services provided to type 2 diabetic patients with self- assessment in comprehensive health service centers. Methods: This descriptive-analytic cross-sectional study was conducted on 203 patients with type 2 diabetes referred to ten comprehensive health service centers of south Tehran/ Iran in 2018 selected through convenience sampling method. Data collection tool was a questionnaire for comprehensive assessment of quality of health services. Data were analyzed through SPSS22 and using t-test, ANOVA and logistic regression statistical tests. Results: From the perspective of patients, mean score of quality of services was 7.47. Among different health services, the highest mean score of quality was related to trust and continuity of service while, support and prevention gained the lowest mean score of quality. There was a significant statistical relationship between the quality of service and self-assessment of diabetes control in the past 12 months (P= 0.007). Also, a significant relationship was found between the quality of service and diabetes complication (P = 0.04). With each unit increase of the quality of service, the odds of a standard visit increased by 27% (P = 0.013). Conclusion: Design and implementation of interventions in order to increase the social support of patients and sharing their experiences with each other as well as improving the dimensions of prevention and safety in care are essential. The concern of accurately representing the elements of the world around, in a work of art under the technical skill of the artist, sometimes repels the interaction between these elements for the sake of desirable generality. Consequently, relying on the strict representation of the elements of the scene and the lack of attention to design principles in the work will not be sufficient to achieve the target audience. The artist uses the necessary visual arrangements to make sense of the scene in the body and soul of the viewer and to convey the sense of content through the necessary measures. Departure of Iranian paintings from walls to manuscript forms a profound link between Persian literature and Iranian art. The Mongol conquest of Iran was a turning point in the history of the painting of this land. Another process of adaptations and innovations began in the late 13th century AD. With the introduction of bibliography at the courts, there was a restriction on the social function of painting, but instead, the themes and artistic expression became wider'. The Iranian painter has exhibited some of the most prominent works of literature of this region in the form of design and color, and has contributed his wishes, ambition and imagination. Visual representation of artistic discipline and artistic arrangements in Iranian painting, especially in the schools of Herat and Tabriz, represent the Iranian artist's awareness of the principles of design and the visual study of the valuable works of Iranian painting art is a visible evidence of the application of design principles in this field. The artist achieves visual unity by utilizing visual qualities, artistic tactics, and the principles and rules required in the process of creating a work. The principles of image organizing, harmony, diversity, balance, proportion, mastery, movement and the ability to form a single whole are the guides of the artist. Iranian and Indian-Islamic sources have mainly benefited from the term framework rather than the contemporary term of composition. For example, Mirza Muhammad Haidar Dughlatin the discussion of Behzad -1450-1535 AD wrote as follows: “His pen is tighter and his framework is better than his”. In the works of Tabriz I school, the harmony between image and text, caused horizontal and vertical combinations of events. Symmetrical and sometimes diagonal structures are also visible in these paintings. In some of these works, the artist uses clever tactics to create such a link between the existing elements, the composition and the narrative that the keen eye, after understanding and receiving it, rejoices and camouflaging these measures and applying hidden geometry, doubles the enjoyment of the work. The purpose of this study was to combine composition in Iranian painting with emphasis on the content and principles of visual traditions in a descriptive-analytical method. The present study seeks to draw on the legacy of the fine art of Iranian painting, through extensive research in the form of books and articles, and the author's collection and analysis of the aforementioned cases, conventional, institutional, and authentic tricks and arrangements from the perspective of creating a sense of theme in Iranian painting. In the field of visual traditions, the necessary books and articles are sufficiently accessible, and the conformity of the visual values of Iranian painting with these principles has been the way to achieve the purpose of the present study. Therefore, the results show that Iranian art has long been trying to express deep spiritual thoughts and has been seeking the truth by crossing the boundaries of time and place as elements of perception of reality. The Western artist used all the formal arrangements and techniques of landscapes to create spirituality from the visual elements. The Iranian-Islamic artist, however, tried to immaterialize the world of work. The viewer's feeling against a stimulus will itself be a criterion for measuring the success of a project. The basis of design theory is to discover the conditions under which the viewer's sensory reactions are quite receptive and positive. Design theory is based on the assumption that human sense inherently searches for a particular order within visual stimuli. The result is that the Iranian painter is aware of the principles of visual discipline and does not purport to convey a mere depiction of the elements of the scene in order to convey a sense of variety of themes. Choosing the right frame, the right geometry, and the right composition, along with applying visual qualities and artistic arrangements have been guidance of emotionally believing text and image adaptation. Each theme requires its own structure, composition and geometry, and contemporary themes have often used common tricks and tactics.

Abstract Background Measuring patients’ perception from health service quality as an important element in the assessment of service quality has attracted much attention in recent years. Therefore, this study was conducted to find out how the patients evaluated service quality of clinics at teaching hospitals affiliated with Tehran University of Medical Sciences in Iran. Methods This cross-sectional study was conducted in Tehran in 2017 and 400 patients were randomly selected from four hospitals. Data were collected using a questionnaire, the validity and reliability of which were confirmed in previous study. In order to analyze the data, T-test, ANOVA, and Pearson correlation coefficient were calculated using SPSS 23. Results The results indicated that among eight dimensions of health service quality, the patients were more satisfied with physician consultation, services costs and admission process. The highest and lowest mean scores were related to physician consultation (Mean = 4.17), and waiting time (Mean = 2.64), in that order. The total mean score of service quality was 3.73 (± 0.51) out of 5. Outpatient services were assessed as good, moderate and weak by 57.5, 40 and 2.5% of the patients, respectively. The novel coronavirus disease (COVID-19) has rapidly spread across the world and caused a pandemic, and still continues to evolve. In Iran, the first cases of COVID-19 were officially announced between February 19 and 23, 2020 and it soon became clear that Iran is one of the countries that is worst-hit by COVID-19 outbreak (1, 2). It is now evident that most cases of COVID-19 disease develop mild respiratory and constitutional symptoms (3), while some cases are asymptomatic (3, 4). Involvement of other organs, including liver and kidneys has been reported in patients with COVID-19 (5). Many questions remain unanswered about associations and presentations of COVID-19. Â

Background and Objective: Public private partnership (PPP) model has been recently introduced as an approach to improve functional efficiency and solving governments’ health care shortages in various aspects. The number of studies focused critically and explanatory on this model are scarce. Furthermore, lack of study organization and structural coherence for deducing practical consequences are still bold in this field. This review summarizes and categorizes these shortcommings. Materials and Methods: This study is designed in review manner: search was conducted in Pubmed, Google scholar, SID.ir and Scopus with all of the combinations of the two keyword groups with the time period search restriction of 2010 to 2017 published studies: group one included “Public private partnership”, “Public private cooperation”, “Privatization”, “Public private initiatives” and their abbreviations; and group two included “Health”, and “Hospital”. After stepwise evaluations and appropriate screening, of 145 assessed papers in the first place, 23 papers were finally selected for data extraction. After reading the full-text of included articles, all of the mentioned information about the problems and challenges of PPPs were extracted. Then, the drafted results for each study were evaluated and compared with each other and organized as main subgroups. Afterwards, the frequency of repeats and emphasis of each subgroup was calculated for all of the 23 studies. Finally, a description review was adapted and registered for each subgroup using related articles. Results: Analysis and assessment of the papers included in this study pointed to several discrete issues which were categorized and discussed in 12 main subgroups based on their conceptual coherence: complexity, low-quality evaluation and supervision, lack of transparency, inadequate coordination, neglecting justice and public interest, lack of flexibility, lack of experience and practical knowledge, insufficeient financial resources, lack of appropriate risk assessment, inadequate cooperation of stakeholders, partner choice restrictions, and paying insufficient attention to the staff. Conclusion: The most important problems and challenges currently confronting health care PPPs were assessed to be complexity, low-quality evaluation and supervision, lack of transparency, and inadequate coordination. All of the items highlight the need to enhance the comprehension of both agreement sides regarding the nature, structure, and complexity of these types of cooperation; as well as complete and precise clarifications for sides, community, and other stakeholders. The aim of this study was to determine the extent of burnout and the relationship between demographic characteristics and burnout dimensions among primary health care providers. Methods: This cross-sectional descriptive-analytic study was conducted in 2019. The statistical sample included all primary health care providers (n=335) working in one of the medical colleges in Markazi province. Data were collected using Maslach Burnout Inventory (MBI). Simple and multiple regression tests were used. Data were entered into SPSS version 23 software. Results: The mean score of emotional exhaustion, depersonalization, and personal accomplishments was 15.5, 4.9, and 33.8, respectively. Among health care providers, 21.8% had high emotional exhaustion, 23%, high depersonalization, and 47.5% had low personal accomplishments. The results of multiple regression tests indicated a significant relationship between gender, work experience, and place of service with burnout. Conclusion: Some strategies such as conducting training courses, providing advisory services, and optimizing human resources management, especially for women, urban employees, and those with high work experience, are suggested to reduce burnout. Background: The aim of this study was to assess the knowledge, attitude and practice of health science students of Shahid Beheshti University of Medical Sciences about the outbreak of water and foodborne diseases in 2017. Methods: In this descriptive and analytic study, 335 students from school of Health and school of Health, Safety and Environment (HSE) were randomly assigned to the study. Data collecting tool was a researcher-made questionnaire containing 61 questions. The data were analyzed using SPSS 18 software. Results: The mean and standard deviation of knowledge, attitude and practice of students were (66.15 ± 61.15), (66.63 ± 14.68) ± 66.33 and (±21.55) ±65.96, respectively. Only 13.7, 6.6 and 24.5% of students had a good knowledge, attitude and good performance regarding the subject. Objective (s): Treatment cost and lack of insurance are the principal reasons for patients absconding or leaving without permission from a hospital. Here we aimed to investigate the effect of the Health Transformation Plan (HTP) on patients absconding from a large general hospital in Tehran, Iran. Methods: In this descriptive study, we considered March 2011 to April 2014 as the period before and June 2014 to March 2017 as the period after the introduction of the HTP. The data of 701 absconded patients were collected. For data analysis, the Mann-Whitney test and Interrupted Time Series model were used in STATA.12 software. Results: The proportion of absconding patients immediately has decreased significantly since the implementation of the HTP (P = 0.002, β = 0.342). However, during the 3-year period after the implementation of the HTP, the patient absconding rate has increased monthly by 0.12% (p = 0.019). Investigating the difference in the patient absconding rate before and after the HTP, we found that the proportion of patients absconding has significantly increased since the HTP implementation (0.011 slope; p = 0.019). Conclusion: Immediately after the implementation of the HTP, the proportion of patients absconding from hospital dropped significantly. This suggests that the HTP has been effective at reducing the patient absconding rate by creating insurance coverage and reducing costs. However, the reasons for a reversal of this trend during the third year of the HTP should be further investigated.

Background & Objectives: One of the most valuable assets of any organization is its brand because it causes more profit margins, better cooperation channels and other benefits for the organization. The aim of this study was to investigate the factors affecting brand identity of hospitals in a semi- governmental hospital in Tehran/ Iran. Methods: In this descriptive-analytic study, 403 hospitalized patients in a semi-governmental hospital were selected by available sampling method in 2016. The data gathering tool was a researcher-made questionnaire based on the Kapferer model (including 30 questions in 6 domains of brand personality, brand physique, brand culture, relationship, self-image and reflection). Data were analyzed using one-sample t-test in SPSS 16 software. Results: The highest mean scores were attributed to the importance of service quality (4.21), known cases (4.15) and easy access (4.02). Introduction: The present study aims to evaluate the Three-Dimensional Diffusion-Weighted reversed fast imaging with steady state free precession (3D DW-PSIF) sequence with respect to imaging of the peripheral nerves; the tibial, medial, and lateral plantar nerves in the lower extremity, ulnar and median nerve in the upper extremity, sciatic nerve, brachial plexus, and lumbosacral plexus, and also to compare its usefulness with the current two-dimensional sequences on a 1.5 T MR scanner. Methods: A total of 25 healthy subjects underwent MR imaging of peripheral nerves, 5 subjects in each area. In each imaging sequence, including T2W SPAIR and 3D DW-PSIF, images were evaluated for ability to identify the nerves in the related area using a 3-score scale (0-2). Then, by summing up the conspicuity scores, a total certainty score was recorded for each sequence. Background & Objectives: The ability to attract tourists in international health markets depends on the quality standards. The aim of this study was to determine the state of educational hospitals of Shahid Beheshti University of Medical Sciences based on organization-oriented standards of joint commission international. Methods: This Descriptive study was performed in four selected teaching hospitals of Shahid Beheshti University of Medical Sciences in Tehran. Data collection was carried out using the questionnaires of the organization-oriented standards of joint commission international including 76 questions in 6 areas of improving quality and patient safety, infection prevention and control, leadership and guidance, safety and facility management, training and staff competence and information and communication management. Descriptive statistics were used as mean and percentage. Data were entered into SPSS20 software. Results: The compliance rate of organization-oriented standards in the hospitals was 81.2% that indicated the relative readiness with regard to the standards of joint commission international in attracting medical tourists. The highest compliance rate of standards was related to safety and facility management (84.4%) and the lowest compliance rate was related to quality improvement and patient safety (75.2 %). Conclusion: Hospitals should pay more attention to the quality improvement and patient safety in order to meet international standards and obtain an international accreditation certificate, and they are supposed to improve standards in this area through corrective actions. Background: One of the main goals of health sector evolution plan is reducing the amount of out of pocket payment by patients receiving hospital services in public sectors. This study aimed to assess the amount of out of pocket payment by inpatients in public hospitals affiliated to Shahid Beheshti University of Medical Science in Tehran city in 2015. Materials and Methods: In this cross-sectional study, 405 discharged patients from four public and teaching hospitals were selected using convenient sampling method. Data were collected using a researcher made checklist and investigating the hospital bills. In order to data analysis, descriptive statistics and regression analysis were utilized through SPSS Software version16. Results: The amount of out of pocket payment was 10.2 percent included 9.9 percent formal payment, 0.2 percent bought & brought goods and 0.1 percent informal payment. The most portion of the hospital costs was related to medical supplies and pharmaceuticals, surgeries services and hosteling with 32.6, 20.6 and 17.36, respectively. Type of hospital, having surgery, average length of stay, family size and gender had significant effect on the out of pocket payment amount (p ≤ 0.05). Conclusion: The amount of out of pocket payment by inpatient in public hospitals was in accordance with goal of the health sector evolution plan, which reveals the appropriate government support. It is recommended to tailor and implement enormous plans regarding outpatients and private sector in national level to attain sustained reduction in out of pocket payment.

Abstract Background The inappropriate use of drugs due to irrational prescriptions is a common problem in Iran, but there is little evidence of prescription patterns in rural family physicians. This study aimed to explore the prescribing pattern and rational drug use indicators for family physicians using Index of Rational Drug Prescribing (IRDP) in Kermanshah Province, Iran. Methods In this retrospective study, 352,399 prescriptions from 184 family physicians in 103 primary health care (PHC) centers were examined. As stated, an analysis was done for rational use indicators suggested by World Health Organization (WHO): e.g., the percentage of prescriptions containing antibiotics, injections, and those prescribed by a generic name and from a national essential medicine list, plus the average number of drugs per prescription; these factors were all taken into account. Rational drug use was studied with the IRDP. Results The average number of drugs per prescription was 3.14 (± 1.2) and the average cost per prescription was 116,740 IRR (USD 3.6). Around 19% of prescriptions had more than four drugs, while the percentage of prescriptions involving antibiotics and injections was 52.1% and 24.4%, respectively. There was 95.1% drugs prescribed by their generic name and 95.9% were retrieved from the essential drugs list. The value of the IRDP was 3.70 out of 5. Conclusion The findings of this study showed that some degree of irrational drug prescribing exists among family physicians, especially in terms of injections, antibiotics, and polypharmacy. It is recommended that there be continuing education programs for physicians regarding rational prescribing for different kinds of medical indications. Clinical practice guidelines should also assist with the rational use of medicine.

Background: Now a day, antibiotic resistance is a global problem. A way to solve this problem is production of alternative drugs. In this regard, today so many researches on antimicrobial peptides against pathogens are being done. LL-37 peptide is one of these peptides that is cationic and has antibacterial, antiviral, and anticancer activity. This project aimed to study the antimicrobial effectiveness of peptide LL-37 on multiple-drug-resistant (MDR) Acinetobacter baumannii. Methods: First, the LL-37 gene was linked to the pET-32a vector; then, recombinant DNA was transformed into the host bacteria and inducted to produce proteins. After the production and purification of recombinant proteins, to activate the protein, dialysis was performed in phosphate buffered saline (PBS). Then, the efficiency of peptide LL-37 on multiple-drug-resistant Acinetobacter baumannii was tested using common laboratory tests such as minimum inhibitory concentration (MIC). Findings: The minimum inhibitory concentration of LL-37 on Acinetobacter baumannii ATCC19606 was 1.5 µg/ml. In addition, the activity test showed that the recombinant proteins could inhibit the growth and decay the bacteria. Conclusion: The results of this study show that LL-37 protein, in comparison to other peptides and drugs in other studies, is more efficient and in low concentration can cause destruction of bacteria. This can herald a bright future for the treatment of infections caused by multiple-drug-resistant Acinetobacter baumannii.

Background and purpose: Reducing maternal mortality by 75% between 1990 and 2015 was the fifth goal of the Millennium Development Goals Declaration in 2000 (MDG 5) and all countries were committed to achieve this goal. This study aimed to assess Iran’s success in achieving this target. Materials and methods: Data was obtained from national data sources and international reports or estimations including World Report from United Nations on maternal mortality trends, the World Bank database, World Health Organization and UNICEF reports, Maternal Mortality Surveillance System, Demographic and Health Surveys 2000 and 2010, and yearbooks of National Organization for Civil Registration and Statistical Center of Iran. Data was analyzed using Excel software to produce descriptive statistics. Results: Findings showed that Islamic Republic of Iran with an average annual reduction of 6.4% between 1990 and 2015 and a total reduction of 80% in maternal mortality during the past 25 years is one of the nine countries that have fully achieved the fifth Millennium Development Goal. Conclusion: Coordinated efforts of Iran for reducing maternal mortality during the last 25 years could be an inspiration for other countries to reduce preventable maternal mortality.

Introduction: Human resources are the most strategic resource and the most significant input for health systems. Their behavior and motivation can strongly affect the overall performance of the health systems. The aim of this study was to determine the factors that affect motivation in frontline employees at teaching hospitals affiliated with Shahid Beheshti University of Medical Sciences (SBMU) in Tehran, Iran. Methods: This cross-sectional study was conducted in 2015. The participants (nurses, physiotherapists, radiology and laboratory technicians, operating room and pharmacy staff) were 300 employees selected by the stratified random sampling method from two general and teaching hospitals. The data collection instrument was a questionnaire that consisted of 42 questions in the 7 domains of motivational factors. Data analysis was performed using descriptive statistics and independent samples t-test by SPSS software, version 23. Results: The findings indicated that working relationships (mean of 3.95) were the main determinant of job motivation of frontline employees. Job content (3.76) career development (3.75), social respect (3.75), and autonomy (3.30) were the next four affective factors. Recognition and remuneration had the least influence on the job motivation of frontline employees. The results of the Friedman test indicated that the difference between the mean scores of different dimensions was significant (χ 2 (6) = 607.00, p < 0.001). Conclusion: A hospital that seeks to engender increased motivation of its frontline employees should have in place a human resources strategy that includes facilitating communication between personnel and management, supporting employees in the community, and promoting social respect for health professions, providing educational opportunities and career development, development of appropriate promotional policies, employee participation in goal setting, facilitating a good working environment and job security, job enrichment, and delegation.

OBJECTIVES This study investigated the knowledge of Iranian women about HIV/AIDS and whether they had accepting attitudes towards people living with human immunodeficiency virus (HIV), and sought to identify factors correlated with their knowledge and attitudes. METHODS The data analyzed in the present study were taken from Iran’s Multiple Indicator Demographic and Health Survey, a national survey conducted in 2015. In total, 42,630 women aged 15-49 years were identified through multi-stage stratified cluster random sampling and interviewed. Associations of the socio-demographic characteristics of participants with their knowledge and attitudes were examined using multiple logistic regression analysis. RESULTS The majority (79.0%) of Iranian women had heard about HIV/AIDS, but only 19.1% had a comprehensive knowledge. In addition, only 15.4% of women had accepting attitudes toward people with HIV. Being older, married, more highly educated, and wealthier were factors associated with having more comprehensive knowledge of HIV/AIDS, and living in urban areas was associated with having more positive attitudes toward people with HIV. CONCLUSIONS The relatively poor knowledge of Iranian women and the low prevalence of accepting attitudes toward people living with HIV highlight the need to develop policies and interventions to overcome this issue, which would be a basis for further prevention of HIV/AIDS in Iran.

The main purpose of this descriptive- correlational research was to identify the factors influencing on Managerial ability of administrators and board of directors of agricultural production cooperatives. The statistical population consisted of all administrators and board of directors of agricultural production cooperatives in Harsin Township (N=444). Samples were selected using stratified sampling technique (n= 206). Based on findings, the managerial ability of respondents was at moderate level. The locus of control of the majority was internal. It means that they attribute their successes and failures to themselves. Based on exploratory factor analysis the main managerial styles of participants were regulated, inefficient and participatory. The most important factors affecting on managerial ability were locus of control, educational level, goal of cooperation and sustainability and participatory management style.

INTRODUCTION: The educational service is expensive and having the right financial information is one of the most important tools for managing financial resources. Therefore, due to the importance of this issue, this study aimed to determine the cost of educational services for medical sciences students at Alborz University of Medical Sciences. METHODOLOGY: A cross-sectional study was conducted between March 20, 2018, and March 20, 2019, at Alborz University of Medical Sciences. The current and capital cost data were extracted from the university's financial database, and the cost price of services provided was calculated using the activity-based cost model. RESULTS: The mean annual cost of services per student was $4778, and the mean cost of education per hour was $113. The total cost price of services provided per student at the school was 65% for wages and salary, 26% for depreciation of building and equipment, and 9% for consumable goods and services. Furthermore, the share of different cost centers to the total cost price of services provided per student was 82% for educational services, 11.9% for student welfare services, and 6.1% for research services. CONCLUSION: Costs of personnel, especially pays to faculty members and the costs of capital expenditures, are the most important costs in higher education institutions. It seems reducing of pay to faculty members through novelty approach of education and evaluation, reducing capital expenditures through use effective of buildings and equipment that can be placed higher education institutions in the path to productivity.

This research aims to identify and prioritise the professional needs of novice teachers using the Delphi technique. This qualitative, emergent design employs semi-structured interviews to collect data and the Atride-Sterling style for thematic analysis. Twelve novice teachers with different educational qualifications were purposively selected, and the data were analysed using MAXQDA, which revealed five inclusive themes, 23 organising themes and 79 basic themes. The results indicated that the professional needs of novice teachers, based on their priority, include economic, social, administrative, emotional, and educational needs, which are interconnected. The results also indicate that the needs of novice teachers are in line with Maslow's hierarchy of needs. By capturing holistic data on needs rather than just professional needs, our study's approach has uncovered something obvious but missing from the extant literature on the needs of novice teachers: only when their basic needs are met can they focus on their role as teachers. In the West, where salaries may be lower than similar professions, teachers still make enough to meet their basic needs. In Iran, this is not always the case. This study suggests new priorities in novice teacher support that are essential before professional educational support can be effective.

زمينه و هدف: يکي از اهداف برنامه تحول سلامت، ترويج زايمان طبيعي و کاهش ميزان سزارين است. اين مطالعه با هدف تعيين ميزان دستيابي يکي از بيمارستان هاي دانشگاهي شهر تهران به اهداف بسته ترويج زايمان طبيعي در طرح تحول سلامت انجام شده است. مواد و روش ها: اين مطالعه مقطعي در سال 1394 انجام شد. داده هاي مورد نياز مطالعه از سيستم اطلاعات بيمارستاني جمع آوري شد و آمار زايمان هاي انجام شده به روش طبيعي و سزارين به تفکيک سالهاي 92 و 93 براي کليه متخصصين زنان و زايمان در بيمارستان مورد مطالعه در نرم افزار اکسل 2013 وارد و با استفاده از روشهاي آمار توصيفي تحليل شد .داده هاي مربوط به رضايت بيماران و رايگان شدن زايمان طبيعي از مستندات بيمارستان و پرونده هاي بيماران استخراج گرديد. يافته ها: از ميان پنج پزشک متخصص شاغل در بيمارستان، چهار پزشک موفق شده اند درصد سزارين به کل زايمان را بين 3 تا 7 درصد نسبت به سال پايه کاهش دهند. اما هيچ يک از پزشکان موفق به دستيابي به هدف کاهش 10 درصدي سزارين در پايان سال93 نسبت به سال پايه نشده اند و در مجموع بيمارستان مورد نظر تنها 2 درصد کاهش در ميزان سزارين ها داشته است. همچنين رضايت بيماران نسبت به سال گذشته 9/1 درصد افزايش يافته بود و صورت حساب هاي زايمان طبيعي همگي رايگان بود. نتيجه گيري: به نظر مي رسد برخي از عوامل نظير آموزشي بودن بيمارستان و ارجاع زايمان هاي پيچيده به اين بيمارستان رفرال ، باعث عدم موفقيت بيمارستان مورد مطالعه در دستيابي به هدف برنامه ترويج زايمان طبيعي و کاهش 10 درصدي ميزان سزارين باشد.

Background and Aim: Over the past decade, the number of private centers providing health care services in Iran has had a continuous growth and the highly competitive market in private hospitals industry has caused increasing pressure on them to provide services with a higher quality. The purpose of this study was to assess the quality of services being provided in private hospitals of Tehran as judged by patients. Materials and Methods: This cross-sectional study was conducted in 2010 in Tehran. The study sample comprised 983 patients, randomly selected from 8 private hospitals. The survey instrument was the standard SERVQUAL questionnaire. Wilcoxon, t-test, and Kruskal-Wallis tests were used for statistical analyses. Results: The total mean scores of patients' expectation and perception were 4.92 and 4.02, respectively. The highest figures were related to the tangible factors dimension and the lowest ones to the empathy dimension. The differences between the perception and expectation for all the 22 items and the 5 dimensions were found to be statistically significant(p< 0.001). Conclusion: SERVQUAL is valid and reliable in hospital settings as a standard tool for the assessment of quality. The results of the present study have revealed the areas that need to be improved as far as the patients are concerned. Measuring the opinion of patients can help facilitate the provision of hospital services, and improve the quality of such services and patients' satisfaction.

زمينه و هدف: فرار بیمار از بیمارستان، یک مسئله بهداشتی درمانی مهم با هزینه‌های اقتصادی، اجتماعی و بهداشتی است. این مطالعه با هدف بررسی میزان فرار بیماران از بخش اورژانس و بار مالی آن در یک بیمارستان عمومی در شهر تهران انجام شد. مواد و روش‌ها: این مطالعه توصیفی و گذشته‌نگر در بخش اورژانس یک بیمارستان بزرگ عمومی و آموزشی شهر تهران انجام شد. با استفاده از پرونده‌های بیماران فراری در محدوده زمانی یکم مهر ۱۳۹۲ تا ۳۱ شهریور ۱۳۹۳، اطلاعات مورد نیاز جمع آوری شد. تحلیل داده‌ها با استفاده از روش‌های آمار توصیفی انجام شد. موازين اخلاقي از جمله محرمانه بودن نام بيماران و بیمارستان مورد پژوهش رعايت گرديد. يافته‌ها: براساس یافته‌ها، 774 نفر از بیماران بدون تسویه حساب و اطلاع به کارکنان، بیمارستان را ترک کرده بودند و نرخ فرار از اورژانس در این مطالعه 4/2 درصد بود. میزان فرار در بین بیماران جوان، مرد، متاهل و بدون بیمه بیشتر بود. میانگین صورتحساب پرونده هر بیمار فراری ۱،۴۴۹،۷۳۸ ریال و بیمارستان مذکور در مجموع مبلغ ۱،۱۲۲،۰۹۷،۲۱2 ریال متحمل خسارت مالی ناشی از فرار بیماران شده بود. نتيجه گيري: شناسایی افراد پرریسک برای مراقبت‌های بیشتر، کنترل منظم بخش‌های اورژانس، تجهیز این بخش به نیروی انسانی جهت رسیدگی سریع‌تر به وضعیت بیماران، ارتباط مناسب و تعامل با بیمار و همراهان او، آشنا کردن بیمار به سیاست ترخیص به میل شخصی و ثبت دقیق اطلاعات بیمار برای کاهش ریسک فرار یا دسترسی داشتن به او پس از فرار از استراتژی‌های پیشگیری و کاهش این رفتار است.

زمينه و هدف: ترک شغل کارکنان یکی از مسائل مهم مدیریتی در بخش بهداشت و درمان طی سالهای اخیر می‌باشد و مدیران بخش سلامت می‌توانند با استفاده از کیفیت زندگی کاری، از ترک شغل و جابجایی‌های شغلی جلوگیری کنند. این مطالعه با هدف بررسی رابطه بین کیفیت زندگی کاری و تمایل به ترک شغل در بین کارکنان یکی از بیمارستانهای مرجع تحت پوشش دانشگاه علوم پزشکی تهران انجام شد. مواد و روش‌ها: این مطالعه مقطعی در سال 1393 انجام شد. جامعه مورد پژوهش کلیه کارکنان بیمارستان آموزشی درمانی مورد نظر بودند که نمونه‌ای به تعداد 126 نفر از کارکنان به صورت طبقه‌ای- تصادفی انتخاب شدند. برای گردآوری داده‌ها از پرسشنامه کیفیت زندگی کاری والتون شامل 32 سوال در 8 بُعد و پرسشنامه تمایل به ترک شغل با 3 سوال استفاده شد. تحلیل یافته‌ها با استفاده از روش‌های آمار توصیفی، ضریب همبستگی و رگرسیون چند متغیره انجام شد. يافته‌ها: بین کیفیت زندگی کاری و تمایل به ترک شغل یک رابطه معکوس معنی‌دار وجود داشت و دو بعد قانون گرایی و یکپارچگی و انسجام اجتماعی پیش بینی کننده تمایل کارکنان به ترک شغل بودند. نتيجه گيري: جهت ماندگاری و حفظ نیروی کار مجرب و ماهر، مدیریت بیمارستان باید حمایت‌های مدیریتی و بهبود سرپرستی، رعایت عدالت و انصاف در سازمان، تلاش برای ایجاد نظام پرداخت منصفانه، سازگاری و تعادل بین کار و سایر جنبه‌های زندگی را در برنامه‌های بهبود کیفیت زندگی کاری برای کارکنان در اولویت قرار دهد.

Background & Objective: Diagnosis and treatment of tuberculosis requires high utilization of resources and costs and assessing the utilization of health care resources is always important for budgeting and long-term planning in a highly dynamic health care system. This study aimed to investigate the resources and service utilization pattern of patients with smear positive tuberculosis in the Azadshahr district, Golestan province. Methods: In this retrospective and cross-sectional study, all new patients with smear positive pulmonary tuberculosis who were registered and successfully completed their treatment at the health network of Azadshahr County, from April 2013 to October 2015, were entered to the study (45 patients). Data were collected from the patients’ records at the health care facilities, telephone interviews with patients and registry book of patients in the tuberculosis unite of health network. Results: The average duration of treatment and DOTS (Directly Observed Treatment, Short-Course) implementation was 195 and 158 days per patient, respectively. The average drug consumption was 741 pills and the average number of X-ray and laboratory tests (smear test) was 1.7 and 4.4 per patient, respectively. Each patient had been visited on average 8.8 times by physician and 27% of patients were hospitalized, their average length of stay was 1.8 days. Conclusion: Our findings showed that the utilization pattern of resources and services in patients with tuberculosis was approximately consistent with protocols developed by World Health Organization (WHO) and Ministry of Health of Iran often.
