Abstract

Giving surgery patients the drug enoxaparin for up to 21 days after surgery can lower their risk of developing thromboembolism, according to a study of 332 patients. Thromboembolism occurs when a blood clot forms in a large vein and then travels to another part of the body. It often occurs in cancer patients who have abdominal surgery.

Abstract

Forty milligrams (mg) of enoxaparin every day appears to reduce the risk of thromboembolism in critically ill patients. Because hospital patients are often inactive, they are at risk of blood clots that travel to other parts of the body and cut off the blood supply. This condition is called thromboembolism. Researchers randomly assigned 1,102 hospital patients to take 40 mg of enoxaparin, 20 mg of enoxaparin or a placebo every day for 6 to 14 days. Enoxaparin is a form of low-molecular weight heparin. The risk of thromboembolism was reduced by more than half in those who took 40 mg.

Abstract

A blood clotting disorder may increase the risk of obstetrical complications in some pregnant women. These complications include preeclampsia, stillbirth, fetal growth retardation and placental abnormalities. Researchers analyzed blood samples for evidence of gene mutations in 110 women who had an obstetrical complication and 110 who had a normal pregnancy. Half of the women with obstetrical complications had a gene mutation that caused blood clotting abnormalities, compared to 17% of the women with a normal pregnancy.

Abstract

Red blood cells (RBCs) in beta-thalassemia show enhanced aggregability and form large clusters as opposed to normal rouleaux. The size of the clusters may reach and even exceed the width of the small blood vessels and thus interfere with blood flow and microcirculation. The RBC aggregates require a greater shear stress to be dispersed. Blood transfusions in thalassemic patients decrease the aggregability of the RBCs. This suggests that the improvement of RBC rheological function may be useful in treating the disorder.

Patients with an indwelling central venous catheter are prone to development of thrombotic complications. Thrombocytopenia in patients undergoing bone marrow transplantation (BMT) generally is protracted. The management of thrombosis in thrombocytopenic patients is difficult because heparin and warfarin are relatively contraindicated because of the high risk of major bleeding. Low molecular weight heparin (LMWH) is a new class of antithrombotic drug. Enoxaparin (Rhone Poulenc Rorer, Antony, France) is an LMWH that has been shown to be effective in the treatment and prophylaxis of venous thrombosis. Enoxaparin, with its high antithrombotic to anticoagulant ratio, may be safer than standard heparin in patients at high risk of hemorrhagic complications. Methods. The authors report the cases of five thrombocytopenic patients, undergoing autologous BMT, in whom venous thrombosis developed related to a Hickman catheter. Results. All the patients were treated with Enoxaparin and recovered promptly without hemorrhagic complications. Conclusions. The authors suggest that Enoxaparin is an effective drug in treating thrombocytopenic patients in whom acute venous thrombosis develops. Cancer 1992; 70:1895-1898.
