Introduction: The electrocardiogram (ECG) is a useful initial tool for the detection of left ventricular hypertrophy (LVH) despite its low sensitivity. However, there are no data in Colombian population on the behavior of the different electrocardiographic criteria. Materials and method: A cross-sectional study was carried out in two specialized cardiology centers in Medellín since 2011, where ECGs of 51 patients who met the inclusion criteria were analyzed. Six criteria were calculated, including the Peguero-Lo Presti, while LVH was defined based on two-dimensional transthoracic echocardiography and M mode. Results: 18 patients with electrocardiographic criteria of LVH and confirmed with echocardiography were analyzed and were compared with 33 controls. LVH was found more frequently in older age, arterial hypertension (HTN) and chronic kidney disease. ROC curves were made determining that the best criteria for the electrocardiographic diagnosis of LVH identified by echocardiography is the Peguero-Lo Presti criteria (ROC: 0.81). The specificities of all the criteria were 81-93%, with no significant differences between them. Conclusion: The electrocardiographic criteria with the best diagnostic performance to identify LVH diagnosed by echocardiography is the Peguero-Lo Presti criteria, compared to the traditional criteria used in the evaluated population. Introducción: Los individuos con hiperreación cardiovascular poseen un riesgo cinco veces superior de padecer hipertensión. En el contexto cubano no están descritos los cambios en la actividad eléctrica ventricular del electrocardiograma, durante la prueba isométrica del peso sostenido, y no se conoce si estos pueden estar influenciados por el grado de reactividad cardiovascular.Objetivo: Describir los parámetros de la actividad eléctrica ventricular en condiciones basales y durante el ejercicio isométrico, en adultos jóvenes de ambos sexos con diferente grado de reactividad cardiovascular.Métodos: La muestra estuvo constituida por 130 individuos de ambos sexos, y se dividió en dos grupos: Normorreactivos e hiperreactivos. A todos se les realizó un electrocardiograma digital en derivación DII; se utilizó el programa LabChart 8 en un polígrafo ADInstruments. Se estudiaron las siguientes variables: Complejo QRS, intervalo QT corregido (QTc), intervalo JT, intervalo Tpeak-Tend, amplitud del segmento ST y de la onda T en reposo, en el ejercicio isométrico y durante la recuperación.Resultados: Entre las mujeres normorreactivas e hiperreactivas se encontraron diferencias significativas en la amplitud de la onda T, tanto basales, durante el ejercicio isométrico, como en la recuperación. Entre los hombres de ambos grupos solo se encontraron diferencias significativas en el intervalo JT, durante el ejercicio.Conclusiones: Los cambios producidos en la regulación autonómica de la función cardíaca durante el ejercicio isométrico y la recuperación, se reflejaron en la actividad eléctrica ventricular en los individuos normorreactivos e hiperreactivos de ambos sexos, principalmente en el intervalo JT y la altura de la onda T.

Heart rate variability (HRV) is an indicator of autonomic activity. As such, it is said to be able to detect signs of disease and can be used to estimate disease status. Recently, the usefulness of the QT interval has been discussed. The reason for this is that QT interval variability is said to be a risk marker for arrhythmia and cardiovascular death. However, this is only a possibility, and whether it actually functions as a risk marker has not yet been established. All S, Q, R, T and P peaks from electrocardiograms (ECGs) were obtained with several Matlab algorithms and compared, and then RR and QT intervals were calculated. On the other hand, it is generally known that there are clear differences in QT interval and RR interval between men and women and with age. Therefore, this project codified an algorithm for HRV analysis implemented in Matlab to see if there are similar differences in QT, RR interval indicators between men and women and by age. For this purpose, several indicators were implemented in Matlab and applied to the QT and RR intervals based on linear and nonlinear algorithms.

Introduction:Left ventricular hypertrophy is an independent risk factor for cardiovascular disease. There are various electrocardiographic criteria for diagnosis, with different sensitivities and specificities.Objective:To determine the diagnostic value of electrocardiographic criteria for left ventricular hypertrophy in comparison to transthoracic echocardiography in adults with arterial hypertension.Material and method: Observational, descriptive, retrospective cross-sectional design, diagnostic test type, which included adult patients with a diagnosis of arterial hypertension admitted to the Medical Clinic rooms of the Hospital de Clínicas, Paraguay, from August 2022 to August 2023. Demographic variables, electrocardiographic criteria (Sokolow-Lyon, Cornell, Lewis, Peguero Lo Presti), and echocardiographic criteria for left ventricular hypertrophy were determined.Results:Five hundred seventeen electrocardiograms and echocardiograms of hypertensive patients were evaluated. According to the Sokolow-Lyon criteria, the sensitivity was 16% and the specificity was 70% for the diagnosis of left ventricular hypertrophy; by Cornell criteria, the sensitivity was 43% and specificity 87%; by Lewis criteria, the sensitivity was 26% and specificity 76% and by Peguero Lo Presti criteria. the sensitivity was 63% and the specificity 87%.Conclusion: The electrocardiogram criterion of left ventricular hypertrophy with the greatest sensitivity was that of Peguero Lo Presti and those with the greatest specificity were those of PegueroLo Presti and Cornell.Keywords:hypertension, left ventricular hypertrophy, electrocardiography, echocardiography, diagnosis, Introducción: la hipertrofia ventricular izquierda es un factor de riesgo independiente de enfermedad cardiovascular. Existen diversos criterios electrocardiográficos para el diagnóstico, con distintas sensibilidades y especificidades. Objetivo: determinar el valor diagnóstico de los criterios electrocardiográficos de hipertrofia del ventrículo izquierdo en comparación a la ecocardiografía transtorácica en personas adultas con hipertensión arterial. Material y método: diseño observacional, descriptivo, retrospectivo de corte transversal, tipo prueba diagnóstica, que incluyó a pacientes adultos con diagnóstico de hipertensión arterial internados en las salas de Clínica Médica del Hospital de Clínicas, Paraguay, desde agosto del 2022 a agosto del 2023. Se determinaron las variables demográficas, criterios electrocardiográficos (Sokolov-Lyon, Cornell, Lewis, Peguero Lo Presti) y ecocardiográficos de hipertrofia ventricular izquierda. Resultados: se evaluaron 517 electrocardiogramas y ecocardiografías de pacientes hipertensos. Según criterio de Sokolov-Lyon la sensibilidad fue 16% y la especificidad 70% para el diagnóstico de la hipertrofia ventricular izquierda; por criterios de Cornell la sensibilidad fue 43% y especificidad 87 %; por criterios de Lewis la sensibilidad fue 26% y especificidad 76% y por criterios de Peguero Lo Presti la sensibilidad fue 63% y la especificidad 87%. Conclusión: el criterio por electrocardiograma de hipertrofia ventricular izquierda con mayor sensibilidad fue de Peguero Lo Presti y los de mayor especificidad fueron los de Peguero Lo Presti y Cornell.

Spontaneous pneumothorax is a rare entity, and its association with electrocardiographic changes is infrequent. In these cases, the most frequent abnormalities are ST-segment elevation, T-wave inversion and poor R wave progression in precordial leads. We present the case of a 25-year-old man who experienced a left spontaneous pneumothorax secondary to a subpleural bleb. Upon admission, his electrocardiogram revealed ST-segment depression in the inferior leads and aVF. These findings resolved following management of pneumothorax through thoracotomy., El neumotórax espontáneo es una entidad poco frecuente, y es poco frecuente su asociación con los cambios electrocardiográficos. En estos casos, las anormalidades más frecuentes son la elevación del segmento ST, la inversión de la onda T y la pobre progresión de la onda R en derivaciones precordiales. Presentamos el caso de un varón de 25 años quien presentó un neumotórax espontáneo izquierdo, a su ingreso el paciente tuvo un electrocardiograma con infradesnivel en el segmento ST en derivaciones inferiores, hallazgos que resolvieron posterior al tratamiento del neumotórax con una toracotomía

El treball que es presenta a continuació se centra en l'exploració de solucions innovadores en el monitoratge de pacients fora de l'hospital, amb èmfasi en l'autonomia energètica dels dispositius mèdics portables. Es proposa un estudi teòric i pràctic per construir un prototip d'electrocardiògraf alimentat per l'energia de les ones de radiofreqüència. Es revisen tècniques de captació d'energia i s'analitzen les particularitats d’aquestes aplicades a la radiofreqüència. Es realitza un estudi sobre l'electrocardiograma com a diagnòstic mèdic i l'arquitectura d'un electrocardiògraf convencional. La part final aborda la construcció del prototip, incloent la comparació de components, la creació de la placa de circuit imprès i els primers passos en la programació del microcontrolador per verificar el seu funcionament. L'objectiu és desenvolupar una solució que aprofiti l'energia de l'entorn per a dispositius de monitoratge, millorant l'autonomia i la llibertat dels pacients en règim no hospitalari., El trabajo que se presenta a continuación se centra en la exploración de soluciones innovadoras en la monitorización de pacientes fuera del hospital, con énfasis en la autonomía energética de los dispositivos médicos portátiles. Se propone un estudio teórico y práctico para construir un prototipo de electrocardiograma alimentado por la energía de las ondas de radiofrecuencia. Se revisan técnicas de captación de energía y se analizan las particularidades de estas aplicadas a la radiofrecuencia. Se realiza un estudio sobre el electrocardiograma como diagnóstico médico y la arquitectura de un electrocardiograma convencional. La parte final aborda la construcción del prototipo, incluyendo la comparación de componentes, la creación de la placa de circuito impreso y los primeros pasos en la programación del microcontrolador para verificar su funcionamiento. El objetivo es desarrollar una solución que aproveche la energía del entorno para dispositivos de monitorización, mejorando la autonomía y la libertad de los pacientes en régimen no hospitalario., The work presented below focuses on exploring innovative solutions in monitoring patients outside the hospital, with an emphasis on the energy autonomy of portable medical devices. A theoretical and practical study is proposed to build a prototype electrocardiogram powered by the energy of radiofrequency waves. Energy harvesting techniques are reviewed, and the specificities of their application to radiofrequency are analysed. A study is conducted on the electrocardiogram as a medical diagnosis tool and the architecture of a conventional electrocardiogram. The final part addresses the construction of the prototype, including component comparisons, the creation of the printed circuit board, and the initial steps into programming the microcontroller to verify its operation. The objective is to develop a solution that harnesses environmental energy for monitoring devices, enhancing the autonomy and freedom of patients undergoing non-hospital care.

L'objectiu d'aquesta treball de fi de grau és dissenyar i desenvolupar un sistema de monitorització de la salut del pacient portàtil basat en sense fil mitjançant el microcontrolador ESP32. El sistema pretén proporcionar un seguiment continu i en temps real dels signes vitals com la freqüència cardíaca, la temperatura corporal i la saturació d'oxigen en sang (SpO2) per als pacients d'una manera no intrusiva i còmoda. El microcontrolador ESP32 s'escull pel seu baix consum d'energia, les seves capacitats de comunicació sense fil integrades i l'extens suport de la comunitat de desenvolupadors. Aquestes característiques el converteixen en una opció ideal per a un dispositiu portàtil que requereix una llarga durada de la bateria i una comunicació perfecta amb sistemes externs. El sistema proposat utilitza diversos sensors, inclòs un mòdul frontal ECG, un sensor de temperatura i un mòdul SpO2, tots compatibles amb l'ESP32. Aquests sensors proporcionen mesures precises i fiables dels signes vitals, que després es processen i es transmeten sense fil a una estació de control remota. El sistema també incorpora capacitats d'emmagatzematge i anàlisi de dades, que permeten als professionals sanitaris controlar les tendències de salut dels pacients i detectar anomalies en temps real. A més, facilita la intervenció oportuna i millora l'atenció al pacient. El procés de desenvolupament implica dissenyar els components de maquinari i microprogramari, garantir una integració perfecta, optimitzar el consum d'energia i validar la precisió i la fiabilitat del sistema mitjançant proves exhaustives. Els resultats d'aquesta treball pretenen contribuir al creixent camp de la tecnologia sanitària portàtil, proporcionant una solució rendible i escalable per al control remot del pacient. L'impacte potencial d'aquest sistema inclou la millora dels resultats dels pacients, la reducció dels costos sanitaris i la millora de l'assistència sanitària tant en entorns clínics com a domicilis., El objetivo de este trabajo de fin de grado es diseñar y desarrollar un sistema portátil de monitorización de la salud del paciente basado en tecnología inalámbrica utilizando el microcontrolador ESP32. El sistema tiene como objetivo proporcionar un control continuo y en tiempo real de los signos vitales, como la frecuencia cardíaca, la temperatura corporal y la saturación de oxígeno en la sangre (SpO2) para los pacientes de una manera conveniente y no intrusiva. El microcontrolador ESP32 se elige por su bajo consumo de energía, capacidades inalámbricas integradas y amplio apoyo de la comunidad de desarrolladores. Estas características lo convierten en una opción ideal para un dispositivo portátil que requiere una batería de larga duración y una comunicación fluida con sistemas externos. El sistema propuesto utiliza varios sensores, incluido un módulo frontal de ECG, un sensor de temperatura y un módulo SpO2, todos compatibles con el ESP32. Estos sensores brindan mediciones precisas y confiables de los signos vitales, que luego se procesan y transmiten de forma inalámbrica a una estación de monitoreo remota. El sistema también incorpora capacidades de almacenamiento y análisis de datos, lo que permite a los profesionales de la salud monitorear las tendencias de salud de los pacientes y detectar anomalías en tiempo real. The aim of the system is to allow non-invasive monitoring of vital signs, including heart rate, temperature and blood oxygen saturation (SpO2) in a continuous and real time manner for patients. I chose the ESP32 microcontroller because of its low power consumption, built-in wireless functionality and broad support from a large developer community. It can considerably serve as an excellent candidate for a wearable gadget that demands continuous battery use and smooth communication with other systems. The proposed system uses multiple sensors, among which an ECG front-end module, a temperature sensor and SpO2 modules that are compatible with the ESP32. These sensors give precise measurements of vital signs that are further processed and transmitted via wireless services to a remote monitoring office. The system also integrates data storage and analysis features, allowing medical practitioners to observe with trends of patient’s health as well as detects abnormalities in real time. In addition, it allows for quicker intervention and better patient care. The hardware and firmware design, integration issues in an efficient manner so that the system is accurate but also very reliable. This research expands to the developing area of wearable health technology by offering a low-cost and scalable means for remote patient monitoring. The possible consequences of the proposed system may encompass better outcomes for patients, lower costs to health care and an optimized management in both clinical as well home environments.

Presentamos el caso de una paciente que ingresó a emergencia por angina inestable, con electrocardiograma inicial sin signos de isquemia aguda y evolución clínica favorable. Durante la hospitalización desarrolló el patrón electrocardiográfico de Wellens, señalado en la literatura como un hallazgo infrecuente, poco identificado y de pronóstico ominoso. Este patrón electrocardiográfico se detalla en derivadas precordiales, sugiriendo una lesión significativa de una arteria epicárdica principal; la paciente tuvo alteraciones electrocardiográficas similares en las derivadas laterales altas, en quien se confirmó el compromiso de bifurcación coronaria no descrita previamente en este escenario.

Foundation: teaching of clinical electrocardiography during doctor´s training in Cuba has been characterized by the insufficient content treatment in the subjects of the curriculum, despite its importance for professional performance.Objective: to propose a curricular strategy for content treatment of clinical electrocardiography in the medical training program. Methods: the research was developed at the Cienfuegos University of Medical Science. The theoretical analysis allowed establishing the requirements for the treatment of electrocardiography as content of the training program. These requirements, together with the diagnosis which confirmed the potential offered by the curriculum of the medical degree to address the content, allowed the design of a curricular strategy oriented towards this objective.Results: the curricular strategy proposes a didactic clinical electrocardiography content sequencing in each year or cycle of the curriculum, modifying the role of the teacher and the student in its teaching-learning process, by granting a special connotation at the curricular level by mainstreaming this content treatment and the teaching tasks that students can solve throughout the career. Expert´s assessment allowed us to confirm the validity of the curricular strategy as its relevance, feasibility, sustainability and transferability were verified.Conclusions: the strategy presented will help to resolve the medical career curriculum insufficiency in terms of electrocardiography content.

Introducción: el intervalo QTc prolongado predispone a arritmias graves. Diversos medicamentos, entre ellos los antirretrovirales, pueden prolongarlo. Los objetivos fueron determinar las características demográficas, clínicas y la frecuencia del intervalo QTc prolongado en pacientes con VIH. Métodos: estudio observacional, prospectivo, con grupo control. Se incluyeron varones y mujeres, mayores de 18 años, portadores de infección por VIH, que acudieron al Hospital Nacional (Itauguá, Paraguay) durante 2020. Actuaron como grupo control los estudiantes de Medicina. Se excluyeron todos los sujetos que no dieron su consentimiento y los portadores de arritmias. Se midieron variables demográficas, clínicas, laboratoriales y electrocardiograma de 12 canales en reposo. El estudio contó con la aprobación del Comité de Ética de la Universidad Privada del Este (Paraguay). Resultados: ingresaron al estudio 39 pacientes con VIH y 39 controles sanos. La edad media de los casos fue 37 ± 11 años, siendo 59% del sexo masculino. La comorbilidad más frecuente en los casos fue la obesidad (7,6%). Los valores medios de urea, creatinina, K, Ca y Mg en los casos se hallaban en rango normal. Se detectó 18% de QTc prolongado en casos y 0% en los controles. Estos sujetos con alteración electrocardiográfica se hallaban todos en tratamiento antirretroviral y antibiótico múltiple de conocida asociación con QTc prolongado. Conclusión: la frecuencia de QTc prolongado en pacientes con VIH fue del 18% y en controles sanos fue del 0%. Se recomienda el control periódico del electrocardiograma en pacientes con VIH en tratamiento con fármacos que prolongan el intervalo QT.

Pulmonary embolism takes place as a consequence of the migration of a thrombus towards the pulmonary vascular tree. Patients frequently report chest pain that can mimic an acute coronary syndrome. We present the case of a 57-year-old patient who came to the emergency department with acute chest pain and dyspnea. He was diagnosed as a non-ST segment elevation acute coronary syndrome, due to the electrocardiographic changes that he presented at admission, but the echocardiographic findings showed increased pulmonary pressures, and the invasive coronary angiography, normal coronary arteries; therefore, a pulmonary angiography with multislice tomography was performed, which confirmed the diagnosis of bilateral pulmonary embolism. To evaluate left ventricular hypertrophy diagnosed with transthoracic echocardiography and its relationship with electrocardiographic changes. Methods: An analytical observational study was carried out in patients treated by the cardiology service at the General Hospital of Barranquilla MiRed IPS, from October 2020 to March 2021. Results: From a total of 111 patients, 63.1% (n = 70) were female, the mean age was 64.1 ± 11.8; 21.6% (n = 24) had hypercholesterolemia, 26.1% (n = 29) took 3-hour weekly walks, and 44.1% (n = 49) were obese. 90.1% (n = 100) received treatment, being losartan the most widely used with 52.3% (n = 58), 77.5% (n = 86) complied with the treatment, and 67.6 % (n = 75) showed normal creatinine values 0-1.0 mg / dL. A significant difference was established between the categories of the electrocardiographic patterns, 39.0% (n = 30) with left ventricular hypertrophy had poor R wave progression in V3 (p <0.000), and 67.6% (n = 67) of those who did not show ventricular hypertrophy left, had other electrocardiographic findings, (p <0.000). The patients with concentric hypertrophy pattern corresponded to 76.6% (n = 59), (p <0.000); in those with eccentric hypertrophy, 23,4% (n=18) had a positive Sokolow-Lyon index on electrocardiography. None of the electrocardiographic findings was sufficiently accurate in estimating left ventricular hypertrophy. Conclusion: Low sensitivity in the electrocardiographic indices for the diagnosis of left ventricular hypertrophy is reported in this study, with a great difference in its diagnostic power compared to echocardiogram. This case is of a 67-year-old male patient treated in the emergency department for a chronic hypokalemia, treated with fluid therapy, ranitidine, amlodipine, and ondansetron in a time of 20 minutes, after the administration of the latter, he presented with sudden death. Fundamento: en la electrocardiografía es importante tener la habilidad de interpretar, además de confianza para lograr una definición certera en el diagnóstico y tratamiento. Objetivo: implementar un taller utilizando el aprendizaje basado en la simulación clínica para la interpretación del electrocardiograma. Métodos: se realizó un estudio de tipo descriptivo y longitudinal en la Facultad de Medicina de la Universidad Nacional Autónoma de México, durante 2018. Se utilizaron métodos teóricos: análisis-síntesis e inducción–deducción; y empíricos: una encuesta y un examen de interpretación de electrocardiograma antes y después del taller aplicado. Resultados: en la evaluación sobre la identificación del electrocardiograma hubo una mejoría significativa posterior a la realización del taller en aspectos como: la interpretación del ritmo, del eje cardíaco, de ondas y en la asignación de diagnósticos en un menor tiempo según el postest. Se constató la diferencia de resultados tanto en confianza, aptitudes obtenidas, resolución de dudas y de aprendizaje reflexivo centrados en la génesis de cada aspecto del electrocardiograma, lo que permitió al equipo de trabajo ubicarlos como puntos clave en la adquisición de habilidades prácticas e intelectuales. Conclusiones: la implementación del taller permitió una mejoría significativa en las habilidades de los estudiantes para la interpretación del electrocardiograma, lo cual condicionó que se sintieran más confiados en sus apreciaciones; por lo que se evidenció la efectividad de su aplicación.

32. Prognostic value of the Tpeak-Tend interval for in-hospital subacute ventricular arrhythmias in tako-tsubo syndrome

The clinical value of electrocardiogram (ECG) repolarization parameters associated with ventricular arrhythmias (VAs) in tako-tsubo syndrome is still under debate. We aimed to evaluate ECG predictors of subacute VAs, defined as those occurring after the first 48 hours from admission.This single-center observational study enrolled patients admitted to the cardiology department between 2012 and 2018 with a confirmed diagnosis of tako-tsubo syndrome. Data collection included a 12-lead ECG on admission and at 48 hours, continuous telemetry monitoring, blood testing, transthoracic echocardiography, and coronary angiography during hospitalization. VA events were defined as: premature ventricular contractions ≥ 2000 within a 24-hour window of telemetry monitoring, ventricular fibrillation, sustained ventricular tachycardia (VT), polymorphic VT, and nonsustained VT.A total of 87 patients (age 72 ± 12 years) were enrolled. During a median of 8 days of hospitalization, subacute VAs were documented in 22 patients (25%) after a median of 91 hours from admission. Subacute VAs were associated with an increase in mortality during hospitalization (P = .030). The corrected global (mean of the 12-lead ECG values) Tpeak-Tend interval at 48 hours from admission was an independent predictor of subacute VAs and was statistically superior to the standard corrected QT interval (Z test, P = .040). A cutoff of 108 msec for the corrected global Tpeak-Tend yielded a 71% sensitivity and 72% specificity for subacute VAs.In patients with tako-tsubo syndrome, subacute VAs are associated with repolarization alterations that can be identified on conventional ECG using the Tpeak-Tend interval.

Introducción: Los esquemas de quimioterapia en el tratamiento de cáncer de mama incluyen Antraciclinas con una efectividad de respuesta alta, sin embargo en algunos casos el potencial efecto terapéutico se ve limitado por la Cardiomiopatía Inducida por Antraciclinas (CIPA). El objetivo del estudio fue establecer la prevalencia longitudinal de esta entidad. Métodos: Este estudio observacional, longitudinal fue realizado en el Instituto Oncológico Nacional “Dr. Juan Tanca Marengo” -SOLCA-Guayaquil. El período de exposición fue abril a diciembre del 2008. El período de observación terminó el 30 de junio del 2020. Incluyó mujeres >18 años con cáncer de mama, en tratamiento con antraciclinas, clasificadas por riesgo Cardiovascular de Framingham en Riesgo Bajo (A), riesgo moderado (B) y riesgo alto (C). Se midió las Alteraciones electrocardiográficas (AEKG) basales, a las 6 horas y a las 24 horas y desarrollo de CIPA en seguimiento a 12 años. La muestra fue no probabilística tipo censo. Se utiliza estadística descriptiva con Intervalo de confianza para proporciones. Resultados: Ingresaron al estudio 50 casos son AEKG. El desarrollo de AEKG a 6 horas en 18/50 casos (36% IC95% 34.1-37.9%), estas AEKG persistieron hasta las 24 horas. Las AEKG se presentaron en un 18.52% en mujeres con Riesgo A, 52.38% en Riesgo B y 100% en Riesgo C. El desarrollo de CIPA a 12 años fue de 3.57% (2.27-4.87%) en CIPA-Subaguda y de 7.14% (5.34-8.95% en CIPA crónica. CIPA Subaguda+ crónica 10.71% (8.55-12.88%). Conclusiones: En este estudio se concluye que los eventos de cardiotoxicidad aguda fueron detectados por cambios electrocardiográficos y que en el seguimiento a largo plazo fueron evidentes en un porcentaje un poco mayor a la reportada en la literatura internacional.

Methods: an observational, descriptive, longitudinal and retrospective study was conducted in hypertensive patients. The sample consisted of 64 patients, who underwent complementary humoral examinations, electrocardiograms and echocardiographic studies. Descriptive statistics was used. Results: the age group between 60 and 69 years old (45,31 %) and male patients (56,25 %) predominated. In 59,37 % of the patients, the time of the evolution of the disease was not specified; and 78,12 % were in the third stage of the disease. Glomerular filtration decreased in 39,06 %, and 23,44 % showed elevated levels of creatinine. Left ventricular hypertrophy was the main electrocardiographic alteration, appearing in 50 % of the cases. Concentric and eccentric hypertrophies were the main echocardiographic alterations; each of them was present in 37,5 % of the patients. Conclusions: older male adults were prone to develop high blood pressure, who showed advanced stages of the disease. Increased of creatinine and decrease of glomerular filtration were common. Patients presented left ventricular hypertrophy as an electrocardiographic finding, with patterns of concentric and eccentric hypertrophies on the echocardiogram.

Antecedentes: El uso de medicamentos con potencial de prolongar el intervalo QT se incrementa con la edad y con las enfermedades cardíacas. 36. Diagnostic value of R wave in aVL lead in left ventricular hypertrophy

Introduction: left ventricular hypertrophy (LVH) is defined as the increase in the thickness of the wall or the interventricular septal portion (or both). This entity represents an important risk factor that is related to the increase in mortality. Its diagnosis is based on voltage criteria that become visible on the is based on voltage criteria that become visible on the electrocardiogram, such as that the R-wave voltage criterion in lead aVL (RaVL). Objective: to evaluate the diagnostic utility of the R-wave voltage criterion in lead aVL (RaVL) for left ventricular hypertrophy. Methods: an observational, analytical and cross-sectional study was conducted, where 127 patients were studied. Clinical and electrocardiographic variables were studied. Results: left ventricular hypertrophy was identified by echocardiography in 69 % of patients. The voltage criterion of the R-wave in lead aVL (RaVL) had a low sensitivity (14,9 %), high specificity (92,5 %) and high positive predictive value (81,3%). The Odds ratio of the R-wave in lead aVL (RaVL) for concentric left ventricular hypertrophy was 0,85 while for the severe one it was 6,13. Conclusions: the voltage criterion of the R-wave voltage in lead aVL (RaVL) was more useful to confirm the cases that in reality present left ventricular hypertrophy and not to deny such existence, but not to discriminate between both types of left ventricular hypertrophy. This criterion is very useful to detect severe forms of this entity.

Introduction: the electrocardiographic study of cardiovascular diseases is part of the medical science syllabuses; and in many occasions it is difficult for students to understand it. Objective: to prepare a multimedia on clinical electrocardiography for medical science students. Method: a study based on technological innovation was carried out in the academic course 2017-2018, at Santiago de Cuba University of Medical Sciences. Different application software was used for multimedia development; as well as theoretical and empirical methods. A survey was applied to assess the quality and applicability to 120 third-year students and 30 professors. Results: 83,87 % of students were female; where the 60 % of professors were male, 46,67 % had between 6 and 10 years of experience as professors and 50 % hold the Master of Science degree. More than the 63 % of students and professors considered as good all the dimensions included. Only poor evaluations were given respect the applicability as a means to facilitate learning and consolidation of knowledge (6,67 %); and product functionality by professors (6,67 %); students evaluated as poor the update of the staff that made used of it (2,15 %), and product functionality (8,60 %). Conclusions: this multimedia is a useful tool in the teaching-learning process for the training of basic electrocardiography, since it provides professors with an auxiliary means and supplies updated information on cardiovascular diseases.

Foundation: teaching of clinical electrocardiography during doctor's training in Cuba has been characterized by the insufficient content treatment in the subjects of the curriculum, despite its importance for professional performance. Objective: to propose a curricular strategy for content treatment of clinical electrocardiography in the medical training program. Methods: the research was developed at the Cienfuegos University of Medical Science. The theoretical analysis allowed establishing the requirements for the treatment of electrocardiography as content of the training program. These requirements, together with the diagnosis which confirmed the potential offered by the curriculum of the medical degree to address the content, allowed the design of a curricular strategy oriented towards this objective. Results: the curricular strategy proposes a didactic clinical electrocardiography content sequencing in each year or cycle of the curriculum, modifying the role of the teacher and the student in its teaching-learning process, by granting a special connotation at the curricular level by mainstreaming this content treatment and the teaching tasks that students can solve throughout the career. Expert's assessment allowed us to confirm the validity of the curricular strategy as its relevance, feasibility, sustainability and transferability were verified. Conclusions: the strategy presented will help to resolve the medical career curriculum insufficiency in terms of electrocardiography content. [ABSTRACT FROM AUTHOR]

This paper presents the results of the preliminary research aimed at a complex psychophysiological assessment of the emotional state of football fans. The issue of mob violence and aggressive behavior of some football fans at football stadiums and the prevention of aggressive and destructive behavior of football spectators in the run up to the FIFA World Cup 2018 make this research relevant. The purpose of this study is to compare the emotional state of a football fan before and after teaching them conscious self-regulation methods and to evaluate the effectiveness of the application of complex psychophysiological diagnostics using the biofeedback device during the training session. The sample for this research is 19-27 year-old football fans with a fan experience from seven to ten years. During the experiment, we have modeled a situation at a football stadium via showing participants football-themed video clips that would persuade them to react in a positive or negative way. Afterwards, the participants were asked to fill in questionnaires, which are aimed at describing their emotional response and self-evaluating their behavior. This study has shown that there is a direct statistically proven connection between the emotional response of a football fan and their subjectively estimated settling time. Projective techniques such as the Hand Test of Edwin Wagner and a modified version of Lüscher Colour Test proposed by Liudmila Sobchik were used to indicate a steady decrease in affective ratio and emotional excitement of a group which was taught a self-regulation technique. The complex psychophysiological assessment of football fans, including the measurement of heart rate, galvanic skin responses, and a photoplethysmography has shown the photoplethysmography to be the most reliable method of indicating the directional change of an emotional state. As a result, it became clear that it is possible to use this technique to develop self-regulation skills using biofeedback. In conclusion, the prospects of the current research is in enlarging the sample in terms of quantity and age, including extra psychophysiological markers of emotional states in our methodology, and approving a method of a self-regulation via biofeedback based on the plethysmography. Este trabajo de Final de Grado (TFG) se enfoca en el desarrollo de una aplicación para dispositivos Garmin con el objetivo de contar los golpeos de raqueta durante una actividad deportiva. En un contexto donde la tecnología portátil desempeña un papel fundamental en la vida cotidiana de los deportistas, la creación de aplicaciones personalizadas para dispositivos Garmin se ha convertido en un campo de estudio en crecimiento. La aplicación desarrollada se basa en el Garmin Software Development Kit (SDK) y se implementa con un enfoque metodológico sólido que abarca el diseño del entorno, el desarrollo del código, pruebas exhaustivas y la evaluación de resultados en dispositivos Garmin físicos. La relevancia de este proyecto se manifiesta en su capacidad de mejorar la experiencia del los usuarios de dispositivos Garmin, particularmente aquellos interesados en deportes de raqueta. La aplicación proporciona una solución efectiva para contar y analizar los golpeos de manera precisa, contribuyendo así al avance del conocimiento en el campo del desarrollo de aplicaciones para wearables. Este trabajo se estructura en cuatro puntos principales que cubren desde la concepción inicial de la idea hasta la implementación y la obtención de resultados experimentales. This work is structured into four main sections that cover from the initial conception of the idea to implementation and the acquisition of the experimental result. Brugada syndrome (BrS) has been associated with risk of ventricular fibrillation and sudden cardiac death (SCD). Its risk stratification remains challenging as the only accepted factor is the presence of resuscitated cardiac arrest or arrhythmogenic syncope and the majority of patients are diagnosed in the asymptomatic phase. Moreover, the only treatment available to prevent SCD is the implantation of a cardiac defibrillator, which can lead to adverse events such as inappropriate shocks. In this study, we present Machine Learning (ML)/supervised classification tools for BrS risk stratification based on the automatic analysis of long-term high-resolution electrocardiographic information. For this purpose, 12-lead ECG 24h Holter and clinical variables from 64 Brugada subjects were used. ECG signals were preprocessed with a signal-averaging algorithm to reduce noise and obtain individual ECG beats for delineation, resulting in 11 ECG biomarkers. Subsequently, 4 different ML/supervised algorithms based on Decision Tree, XGBoost, K-Nearest Neighbors and support vector machine algorithms were tested. AUC results were around 90%, however sensitivity results were around 50%. The results do not efficiently predict BrS symptomatic patients at risk of SCD, which is mainly caused by the reduced number of symptomatic patients. El síndrome de Brugada (SB) es una rara canalopatía cardiovascular hereditaria asociada con el riesgo de fibrilación ventricular y muerte súbita (MS). Su estratificación de riesgo es un desafío y la mayoría de los pacientes son diagnosticados en la fase asintomática siendo el ´único tratamiento contra la MS la implantación de un desfibrilador eléctrico. En este estudio, presentamos herramientas de clasificación supervisada para la estratificación del riesgo del SB basadas en el análisis automático de información electrocardiográfica de alta resolución. Para ello, se utilizaron señales de ECGs de 24 horas y 12 derivaciones y variables clínicas de 64 sujetos con SB. Las señales de ECG se preprocesaron promediando la señal para reducir el ruido y obtener latidos individuales para el cálculo de 10 biomarcadores. Posteriormente, se probaron 4 algoritmos de clasificación supervisada diferentes basados en Random Forest (RF), Extreme Gradient Boosting (XGBoost), K-Nearest Neighbors (KNN) y Support Vector Machine (SVM). Los resultados del área bajo la curva fueron alrededor del 90%, sin embargo, los resultados de sensibilidad fueron alrededor del 50%. Los resultados no predicen de manera eficiente a los pacientes sintomáticos en riesgo de MS debido al reducido número de pacientes sintomáticos. Aquest treball de fi de grau es centra en el disseny d'un dispositiu no invasiu per diferenciar entre persones diabètiques i no diabètiques mitjançant l'anàlisi del comportament cardíac. L'objectiu principal és comprendre com la diabetis afecta la freqüència cardíaca, així com desenvolupar un dispositiu electrònic capaç de discriminar basant-se en les mesures de la freqüència cardíaca. S'ha examinat la conveniència d'utilitzar l'electrocardiografia (ECG) o la fotopletismografia (PPG) per a aquest propòsit, amb un enfocament específic en el disseny de hardware i deixant la part de software per a futures investigacions. La PPG s'ha presentat com l'opció preferida degut al confort dels pacients, malgrat que fan falta estudis pràctics per corroborar-ho. A més, s'ha analitzat la base de dades DICARDIA per investigar els efectes en la freqüència cardíaca en persones diabètiques, tant amb com sense condicions cardiovasculars preexistents, durant proves d'esforç. Els resultats indiquen una disminució significativa en les freqüències cardíaques i en la recuperació de la freqüència cardíaca en persones diabètiques, coincidint amb les expectatives inicials., Este trabajo de fin de grado se centra en el diseño de un dispositivo no invasivo para diferenciar entre personas diabéticas y no diabéticas mediante el análisis del comportamiento cardíaco. El objetivo principal es comprender cómo la diabetes afecta la frecuencia cardíaca y desarrollar un dispositivo electrónico capaz de discriminar basándose en el ritmo cardíaco. Se evaluó la conveniencia de utilizar la electrocardiografía (ECG) o la fotopletismografía (PPG) para este propósito, centrándose específicamente en el diseño de hardware y dejando el software para futuras investigaciones. La PPG se presenta como la opción preferida debido a la comodidad del paciente, aunque son necesarios estudios prácticos para corroborarlo. Además, se analizó la base de datos DICARDIA para investigar los efectos en la frecuencia cardíaca en personas diabéticas, tanto con como sin condiciones cardiovasculares preexistentes, durante pruebas de esfuerzo. Los resultados indican una disminución significativa en las frecuencias cardíacas y en la recuperación de la frecuencia cardíaca en personas diabéticas, lo cual concuerda con las expectativas iniciales., This final degree project focuses on designing a non-invasive device to differentiate between diabetic and non-diabetic individuals by analysing their cardiac behaviour. The primary goal is to understand how diabetes affects heart rate and to develop an electronic device for effective discrimination based on heart rate measurements. It is assessed the suitability of electrocardiography (ECG) or photoplethysmography (PPG) for this purpose, with a specific focus on hardware design, leaving the software aspect for future research. PPG emerges as the preferred choice due to the comfortability given to the patient, however practical studies are needed to prove it fully. Furthermore, DICARDIA database was examined to investigate how diabetes impacts heart rate in individuals, both with and without pre-existing cardiovascular conditions, during exercise stress tests. The results indicate significantly lower heart rates and heart rate recovery in diabetic individuals, aligning with initial expectations.
