1. Eyelid dermatitis in patch-tested adult patients: a systematic review with a meta-analysis

Abstract

Abstract Eyelid dermatitis (ED) affects a cosmetically significant area and leads to patients’ distress. Despite ongoing and recent research efforts, ED remains a multidisciplinary problem that needs further characterization. We aimed to evaluate the atopic eyelid dermatitis (AED) frequency in ED patients and to perform their clinical profiling. PubMed databases were searched from 01.01.1980 till 01.02.2024 to PRISMA guidelines using a search strategy: (eyelid OR periorbital OR periocular) AND (dermatitis or eczema). Studies with patch-tested ED patients were included. Proportional meta-analysis was performed using JBI SUMARI software. We included 65 studies across Europe, North America, Asia and Australia, with a total of 21,793 patch-tested ED patients. AED was reported in 27.5% (95% CI 0.177, 0.384) of patch-tested ED patients. Isolated ED was noted in 51.6% (95% CI 0.408, 0.623) of 8453 ED patients with reported lesion distribution, including 430 patients with isolated AED. Our meta-analysis demonstrated that the AED frequency in patch-tested ED patients exceeded the previous estimate of 10%. Isolated AED was noted in adult patients, attending contact allergy clinics. Future studies are needed to elucidate the global prevalence and natural history of isolated AED in adults.

2. Foreign Language Teachers as Agents of Using a Coursebook

Abstract

The article highlights the concept of foreign language teacher agency which can manifest itself in the teachers` decision to introduce changes to the coursebooks they use. The most significant characteristics of the teacher agentic position in this area are clear motives and goals for change, a detailed analysis of the context and of the tools (including coursebooks) that are necessary for positive learner outcomes, an autonomous decision to choose and use the teaching materials that can facilitate effective learning. All these require teacher self-regulation and reflection. The article aims to find out if foreign language teachers are inclined to change coursebooks and why some teachers reject any changes, what factors affect their decision, and what kind of changes are common. The present research is based on the teachers` answers to a questionnaire that was offered to 182 foreign language school teachers from 2015 up to the beginning of 2020. All the respondents were divided into two groups: professionally active teachers in the community (32 teachers) and those who do not actively participate in the regional professional activities (150 teachers). The results of the study prove that the teacher`s readiness and ability to introduce coursebook changes are a significant indicator of the overall teacher agency. Though the study has a few limitations, its outcomes can be beneficial for both pre-service and in-service teacher education in terms of coursebook evaluation and material choice/development skills.

3. Interactive Mobile Home Tasks vs. Individual Home Tasks in University Foreign Language Education at the Upper-Intermediate Level

Abstract

Homework, being an indispensable part of learning, needs special planning and modification in line with the latest educational development. In this view, we assume that mobile devices can provide ample opportunities for students to interact and collaborate with their group-mates to enhance learning outcomes in university foreign language education. Interactive mobile homework assignments combined with individual ones can noticeably improve these outcomes from different perspectives. They engage students in communicative foreign language usage, encouraging both receptive and productive speech acts, student-to-student cooperation in learning through communication, peer-teaching, and pooling resources which promote the amplification of the learning environment through content and language sharing. On the other hand, interactive mobile homework assignments also have certain risks that may decrease the quality of learning, such as possible error exchange, a superficial approach, a switch to use of native language, and chatting, etc. Nevertheless, interactive mobile homework assignments keep students on the task for a longer period of time because they require each student’s pre-task preparation, longer oral exchanges during the “while phase” and post-task individual summary, therefore providing more practice. The study described in the article highlights the characteristics of the interactive mobile homework tasks, suggests a list of homework task types, both individual and interactive, and compares the results of their practical application in university foreign language education. The authors attempt to identify a trend in their contribution to the learning outcomes achieved by the students while completing their homework, either interactively or individually.

4. A University Foreign Language Curriculum for Pre-Service Non-Language Subject Teacher Education

Abstract

The article focuses on the problems of teaching foreign languages (FL) to pre-service subject (other than foreign languages) teachers and highlights their possible solutions. A sustained discontent of both university teachers and students with the outcomes of FL classes gave impetus for a systematic, detailed study of the university FL education of the target group of learners. There was an acute need to develop well-grounded guidelines for the related course objectives, its content and adequate techniques intended for the target group of learners. The authors claim that the generally accepted English for Specific Purposes (ESP) or Content and Language Integrated Learning (CLIL) models of university FL education cannot be applied in this context for a number of reasons. A limited amount of classroom time, lack of updated teaching materials, mixed-ability classrooms, and unclear perspectives of the future application of the FL skills made the study urgent. Therefore, the teachers of the English Language Department of the Petrozavodsk State University (Russia) developed a project that is described in the article; 152 students and six teachers participated in its implementation. The objective of the project was to introduce reasonable changes into the approaches, the aims, the content, and the techniques for rational and effective FL teaching and learning in this context. The authors identified priorities of the university FL education in line with the context and the target learner characteristics. The article presents arguments in favor of the introduced changes, describes the phases of the project work, and gives details of all the constituents of the proposed curriculum. Based on the outcomes of the project, the authors conclude that it is necessary to undertake further research steps in this direction.

Abstract

Today, we can see a great interest in vitamin D because it participates in the regulation of many metabolic processes, and its deficiency is associated with the development of various diseases. Chronic spontaneous urticaria (CSU) is the disease that affects the quality of patients life, and the existing strategy of patient management is not always sufficiently effective. Nowadays, there is enough information about the role of vitamin D deficiency and the severity of CSU. That is why it is important to study not only therapeutic schemes, but also a role of genetic variability that may have an impact on vitamin D levels. Such studies will help to personalize the treatment schemes for patients with CSU. At the same time, the focus of these studies should be not only on the receptor and vitamin D binding protein, but also on P450 system, which plays a key role in the vitamin D metabolism.

Abstract

Asthma is a severe and chronic disabling disease affecting more than 300 million people worldwide. Although in the past few drugs for the treatment of asthma were available, new treatment options are currently emerging, which appear to be highly effective in certain subgroups of patients. Accordingly, there is a need for biomarkers that allow selection of patients for refined and personalized treatment strategies. Recently, serological chip tests based on microarrayed allergen molecules and peptides derived from the most common rhinovirus strains have been developed, which may discriminate 2 of the most common forms of asthma, that is, allergen- and virus-triggered asthma. Abstract

Background: cold urticaria (ColdU), that is, the occurrence of wheals or angioedema in response to cold exposure, is classified into typical and atypical forms. The diagnosis of typical ColdU relies on whealing in response to local cold stimulation testing (CST). It can also manifest with cold-induced anaphylaxis (ColdA). We aimed to determine risk factors for ColdA in typical ColdU. Methods: an international, cross-sectional study COLD-CE was carried out at 32 urticaria centers of reference and excellence (UCAREs). Detailed history was taken and CST with an ice cube and/or TempTest(R) performed. ColdA was defined as an acute cold-induced involvement of the skin and/or visible mucosal tissue and at least one of: cardiovascular manifestations, difficulty breathing, or gastrointestinal symptoms. Results: of 551 ColdU patients, 75% (n = 412) had a positive CST and ColdA occurred in 37% (n = 151) of the latter. Cold-induced generalized wheals, angioedema, acral swelling, oropharyngeal/laryngeal symptoms, and itch of earlobes were identified as signs/symptoms of severe disease. ColdA was most commonly provoked by complete cold water immersion and ColdA caused by cold air was more common in countries with a warmer climate. Ten percent (n = 40) of typical ColdU patients had a concomitant chronic spontaneous urticaria (CSU). They had a lower frequency of ColdA than those without CSU (4% vs. 39%, p = .003). We identified the following risk factors for cardiovascular manifestations: previous systemic reaction to a Hymenoptera sting, angioedema, oropharyngeal/laryngeal symptoms, and itchy earlobes. The absolute basophil count (cells/L) can be determined by manual counting of peripheral blood smears or using cell counting chambers as well as by automated hematology analyzers and fluorescence flow cytometry. Manual basophil counting of peripheral blood smears is currently regarded as the reference method, although the limitations of this method (distribution, observer, and statistical errors) are widely recognized. Automated hematology analyzers offer an advantage of larger numbers of counted cells and high throughput but are characterized by inconsistent analytical performance for basophil enumeration. Flow cytometric enumeration of circulating basophils using panels of monoclonal antibodies is being developed as novel candidate reference method for the absolute basophil count in peripheral blood. Basophil counting using fluorescence flow cytometry is characterized by high precision and statistical superiority. Emerging innovative technologies for absolute cell counts include imaging flow cytometry, mass cytometry, and on-chip blood counting, but their analytical performance for absolute basophil counts is yet to be established. Here, we describe various techniques for absolute basophil counting in peripheral blood including manual basophil counts in smears and hemocytometers and flow cytometric methodologies using double-platform, bead-based, and volumetric approaches.

Abstract

Special conditions associated with laboratory autoimmune testing are not well compatible with recent developments in regulatory frameworks such as EN/ISO 15189 accreditation or in vitro diagnostic medical device regulation (IVD-R). In addition, international recommendations, guidelines and disease criteria are poorly defined with respect to requirements on autoantibody testing. Laboratory specialists from Austria, Belgium, Croatia, Estonia, Finland, France, Germany, Greece, Hungary, Italy, Norway, Poland, Portugal, South Africa, Spain, Sweden, Switzerland, and The Netherlands collected information, reported national experience, and identified quality issues in relation to autoantibody testing that require consensus on interpretation of the regulatory frameworks and guidelines. This process has been organized by the European Autoimmunity Standardisation Initiative (EASI). By identifying the critical items and looking for a consensus, our objective was to define a framework for, in particular, EN/ISO accreditation purposes. Here, we present a review of current publications and guidelines in this field to unify national guidelines and deliver in this way a European handout on quality control and accreditation for laboratories involved in autoantibody testing. We focus on quality items that can be checked during accreditation visits. Abstract

Background and objective Numerous autoimmune diseases (AIDs) have been linked to chronic spontaneous urticaria (CSU). Here, we provide the first extensive and comprehensive evaluation of the prevalence of AIDs in patients with CSU and vice versa . Methods A Pubmed and Google Scholar search was performed to identify studies reporting the prevalence of various AIDs in CSU and vice versa published before April 2017. Results The prevalence of individual AIDs in CSU is increased (≥ 1% in most studies vs ≤ 1% in the general population). AIDs with relatively high prevalence in the general population are also quite common in CSU patients, whereas those with low prevalence remain a rare finding in CSU. The rates of comorbidity in most studies were ≥ 1% for insulin-dependent diabetes mellitus, rheumatoid arthritis (RA), psoriasis and celiac disease (CD), ≥ 2% for Graves' disease, ≥ 3% for vitiligo, and ≥ 5% for pernicious anemia and Hashimoto's thyroiditis. Organ-specific AIDs are more prevalent in CSU than systemic (multiorgan or non organ-specific) AIDs. > 2% of CSU patients have autoimmune polyglandular syndromes encompassing autoimmune thyroid disease (ATD) and vitiligo or pernicious anemia. Antithyroid and antinuclear antibodies are the most prevalent AID-associated autoantibodies in CSU. > 15% of CSU patients have a positive family history for AIDs. The prevalence of urticarial rash in AID patients is > 1% in most studies. This rash is more prevalent in eosinophilic granulomatosis with polyangiitis, ATD, systemic lupus erythematosus, RA and CD. Conclusions CSU patients have an increased risk of AIDs, especially adult female patients and those with a positive family history and a genetic predisposition for AIDs, who should be screened for signs and symptoms of AIDs.

Urticaria is a common disorder with diverse clinical presentations. Correct recognition of the different clinical patterns helps clinical assessment and treatment. The management of urticaria involves recognition of relevant disease associations, treatment of any identifiable external causes including infection, avoidance of drug, food and physical triggers and the appropriate use of pharmacological therapies. These can be divided into first, second and third-line approaches. The choice of treatment will be influenced by many factors including drug-licensing, safety, pattern of disease, its severity, pharmaco-economic considerations and patient preference. Possible new therapeutic approaches on the horizon include new H1 antihistamines, H4 receptor antagonists, a histidine decarboxylase inhibitor under development and biological agents that target histamine-releasing autoantibody production and function.

Abstract

The recommendations for the definition and diagnosis presented in this position paper are the result of a panel consensus meeting held in December 2008 in Berlin. This consensus meeting was a joint initiative of EAACI (European Academy of Allergology and Clinical Immunology) Dermatology Section, the EU-funded network of excellence, GA2LEN (Global Allergy and Asthma European Network), the EDF (European Dermatology Forum) and UNEV (urticaria network e.V.). The aim of these recommendations is to improve the diagnosis and management of patients with physical urticaria or cholinergic urticaria and to promote research and a better understanding of these diseases. Our recommendations used the paper produced by a 1996 expert meeting (1) and they acknowledge the latest changes in our understanding of physical urticarias and cholinergic urticaria as well as the recent development of novel diagnostic tools. In addition, this consensus paper highlights areas of need for further research.

Abstract

In this article we investigate the dynamics of a five-link, passive bipedal robot. The passivity in this context stands for the ability of the robot to walk autonomously down an inclined surface without any external source of energy. Previous research efforts in passive walking were limited to four link models with knees or 2-link models without knees with a variety of mass distributions. In this paper we analyze the dynamics of a five-link robot with knees and upper body. We were successful in detecting three limit cycles that include three distinct upper body motions. We have investigated the structural stability of these cycles subject to variations in the upper body length. The results demonstrated that the stability can be improved with addition of linear dampers in the hip joints of the model. Also, our investigation demonstrated that erect body posture is only achievable when torsional springs are placed in the hip joints.

Abstract

Background Symptomatic dermographism is the most common type of physical urticaria. It can be severe and poorly controlled with H1 antihistamines in some patients. Photochemotherapy (psoralen plus ultraviolet [UV] A) may help the itch of dermographism but the effect of narrowband (NB) UVB therapy has not been previously studied. Objectives We sought to examine the clinical efficacy of NB UVB therapy for itch and whealing in symptomatic dermographism and to assess the duration of the effect during 3 months of follow-up. Methods Eight patients (6 female) were enrolled into an open uncontrolled prospective study. Intensity of itching and whealing was assessed with visual analog scales and the whealing response was evaluated by testing with a dermographometer at pressures of 20, 36, and 60 g/mm 2 on the upper aspect of the back. NB UVB phototherapy was given for 6 weeks 3 times weekly starting at 50% of minimal erythema dose with 20% to 0% increments as tolerated. Fexofenadine (180 mg/d) was taken during the run-in period and subsequently throughout the study and follow-up as required. Patients were followed for 3 months with regular assessments every 6 weeks after completion of phototherapy. Results All patients showed an improvement in itching at the end of NB UVB treatment (mean [SD] reduction 52.3% [31.6%]). Subjective assessment of whealing revealed a significant improvement in all but two patients (mean [SD] reduction 71% [54%]). There was a small and statistically significant improvement in cumulative dermographometer-induced wheal widths at the end of phototherapy ( P = .038). A time trend for the relapse of symptoms within 12 and 18 weeks after completing phototherapy was significant for both visual analog scale scores but not for dermographometer-induced whealing. Limitations The apparent rarity of antihistamine-resistant symptomatic dermographism limited the study to a small number of participants. The severity of the condition did not permit a controlled and blinded study design. Conclusions NB UVB phototherapy is an effective second-line treatment for patients with severe symptomatic dermographism responding poorly to fexofenadine. This therapy can lead to subjective relief of pruritus and whealing and objective reduction of whealing. NB UVB phototherapy may restore symptom control with antihistamines in some patients.

Abstract

Urticaria is an acquired illness characterized by itchy hives, angioedema, or both. It is a histaminergic, mast cell–driven disorder presenting with different clinical phenotypes, causes, triggers, and aggravating factors. It can be short-lived or very prolonged and mild or severe but will eventually remit without long-term damage. Management is primarily aimed at alleviating symptoms, rather than influencing its natural history. The differential diagnosis of urticarial rashes includes urticarial vasculitis and hereditary or acquired autoinflammatory syndromes. Although the majority of patients presenting with angioedema without wheals will have spontaneous idiopathic urticaria, a minority will be bradykininergic with completely different heritability, pathogenesis, prognosis, and management. Anaphylaxis is an acquired acute systemic illness characterized by respiratory difficulty, fall in blood pressure, urticarial rash, vomiting, or diarrhea. It is a mast cell–mediated disorder with multiple immunological or nonimmunological causes and may be multifactorial, often requiring emergency treatment with epinephrine.
