5. Childhood Disintegrative Disorder (CDD): Symptomatology of the Norwegian Patient Population and Parents' Experiences of Patient Regression

Abstract

Childhood Disintegrative Disorder (CDD) is a rare and little researched developmental disorder characterised by regression in language and social skills after a period of seemingly normal development until at least the age of 2 years. The study contacted all parents of CDD patients in Norway to assess patient symptomatology and parents' experiences of regression via questionnaire or interview. There were 12 participants. Symptomatology was in-line with previous studies, with universal regression in language and social skills and onset predominantly at 2-4 years. Regression was connected to feelings of 'loss' and uncertainty over the prognosis for CDD patients. The study supported CDD diagnostic criteria and showed that CDD patient regression has profound implications for parental well-being.

Abstract

Bandwidth estimation and congestion control for real-time communications (i.e., audio and video conferencing) remains a difficult problem, despite many years of research. Achieving high quality of experience (QoE) for end users requires continual updates due to changing network architectures and technologies. In this paper, we apply reinforcement learning for the first time to the problem of real-time communications (RTC), where we seek to optimize user-perceived quality. We present initial proof-of-concept results, where we learn an agent to control sending rate in an RTC system, evaluating using both network simulation and real Internet video calls. We discuss the challenges we observed, particularly in designing realistic reward functions that reflect QoE, and in bridging the gap between the training environment and real-world networks., Comment: Workshop on ML for Systems at NeurIPS 2019

Abstract

Failure to accurately measure the outcomes of an experiment can lead to bias and incorrect conclusions. Online controlled experiments (aka AB tests) are increasingly being used to make decisions to improve websites as well as mobile and desktop applications. We argue that loss of telemetry data (during upload or post-processing) can skew the results of experiments, leading to loss of statistical power and inaccurate or erroneous conclusions. By systematically investigating the causes of telemetry loss, we argue that it is not practical to entirely eliminate it. Consequently, experimentation systems need to be robust to its effects. Furthermore, we note that it is nontrivial to measure the absolute level of telemetry loss in an experimentation system. In this paper, we take a top-down approach towards solving this problem. We motivate the impact of loss qualitatively using experiments in real applications deployed at scale, and formalize the problem by presenting a theoretical breakdown of the bias introduced by loss. Based on this foundation, we present a general framework for quantitatively evaluating the impact of telemetry loss, and present two solutions to measure the absolute levels of loss. This framework is used by well-known applications at Microsoft, with millions of users and billions of sessions. Abstract

User surveys for Quality of Experience (QoE) are a critical source of information. In addition to the common "star rating" used to estimate Mean Opinion Score (MOS), more detailed survey questions (problem tokens) about specific areas provide valuable insight into the factors impacting QoE. This paper explores two aspects of the problem token questionnaire design. First, we study the bias introduced by fixed question order, and second, we study the challenge of selecting a subset of questions to keep the token set small. Based on 900,000 calls gathered using a randomized controlled experiment from a live system, we find that the order bias can be significantly reduced by randomizing the display order of tokens. The difference in response rate varies based on token position and display design. It is worth noting that the users respond to the randomized-order variant at levels that are comparable to the fixed-order variant. The effective selection of a subset of token questions is achieved by extracting tokens that provide the highest information gain over user ratings. This selection is known to be in the class of NP-hard problems. We apply a well-known greedy submodular maximization method on our dataset to capture 94% of the information using just 30% of the questions.

Abstract

User-perceived quality-of-experience (QoE) in internet telephony systems is commonly evaluated using subjective ratings computed as a Mean Opinion Score (MOS). In such systems, while user MOS can be tracked on an ongoing basis, it does not give insight into which factors of a call induced any perceived degradation in QoE -- it does not tell us what caused a user to have a sub-optimal experience. For effective planning of product improvements, we are interested in understanding the impact of each of these degrading factors, allowing the estimation of the return (i.e., the improvement in user QoE) for a given investment. To obtain such insights, we advocate the use of an end-of-call "problem token questionnaire" (PTQ) which probes the user about common call quality issues (e.g., distorted audio or frozen video) which they may have experienced. In this paper, we show the efficacy of this questionnaire using data gathered from over 700,000 end-of-call surveys gathered from Skype (a large commercial VoIP application). We present a method to rank call quality and reliability issues and address the challenge of isolating independent factors impacting the QoE. Finally, we present representative examples of how these problem tokens have proven to be useful in practice.

14. Fedratinib for the treatment of myelofibrosis: a critical appraisal of clinical trial and "real-world" data.

Abstract

Fedratinib is a predominantly JAK2 inhibitor that has shown efficacy in untreated and ruxolitinib-exposed patients with myelofibrosis (MF). Based on randomized clinical trial data, it is approved for use in patients with International Prognostic Scoring System (IPSS) or Dynamic International Prognostic Scoring System (DIPSS) intermediate-2 or high-risk disease and is distinguished from ruxolitinib in that it can be administered without dose reduction in patients with thrombocytopenia, to a platelet count above 50,000/µL. In these trials, fedratinib achieved significant spleen volume reduction in ~30–45% of patients and improvement in total symptom scores in 35–40% with good tolerability. In contrast, recently published real-world data suggest that these responses may not be as robust outside clinical trials. In the context of routine clinical practice spleen responses are documented in only 13–68%, with varying degrees of symptom improvement. This may be due to the lack of a uniform definition of ruxolitinib failure, which may influence the timing of initiating fedratinib as a second-line treatment and result in a more prolonged exposure to ruxolitinib prior to intitaing fedratinib treatment. We suggest that given the growing number of drugs available for use in MF, recognizing the failure of first-line (and potentially subsequent) treatments is critical to allow timely transition to potentially more active agents, as highlighted by the data pertaining to fedratinib. 28. Dare to be different, dare to progress : a case study of a Key Stage 4 Pupil Referral Unit 2009-12

Abstract

‘Dare to be different, dare to progress’ explores the educational experiences of a group of 14 – 16 year old students referred to a pupil referral unit (PRU) during the period 2009 – 2012 using both quantitative and qualitative data sources. The quantitative data gathered from school files, the Local Authority’s school performance data and police records enabled a statistical exploration of recorded information pertaining to issues of attainment and progress, attendance, exclusions, deprivation factors and contacts with police. The qualitative data enabled rich contextual information and was gathered from fieldwork involving the researcher’s observations, four group interviews involving four teachers and four teaching assistants and thirteen paired interviews with nine volunteer students. Willis (1977) showed how ‘the lads’ used their social class identities to forge social class relations acting out resistant behaviours in the process in preparation for and perpetuating a working-class lifestyle or culture. This study resonated with my work at the PRU and the impetus to take it further developed during a secondment to the local Youth Offending Service where I observed a high proportion of PRU students were also known to the police and other children’s services. Bourdieu’s (1984, 1990, 1992) works on habitus, capital and field were influential in shaping the theoretical and conceptual framework around ‘class’ and ‘culture’. Skeggs’ (1997, 2004) concepts of inscription and identity formation and Quinn’s (2010) concept of imagined social capital contributed to this too. The research is unique to a particular group of young people in a particular setting and combined statistics, field notes, photographs and dialogue thus indicating ethnographic case study methodology (Merriam 1988). The research found that ‘class’ remains the strongest indicator of educational achievement and cultural capital, but the cultural influences of youth and identity, and deprivation alongside low aspirations and expectations exacerbate the situation.

37. Understanding the performance of Internet video over residential networks

Abstract

Video streaming applications are now commonplace among home Internet users, who typically access the Internet using DSL or Cable technologies. However, the effect of these technologies on video performance, in terms of degradations in video quality, is not well understood. To enable continued deployment of applications with improved quality of experience for home users, it is essential to understand the nature of network impairments and develop means to overcome them. In this dissertation, I demonstrate the type of network conditions experienced by Internet video traffic, by presenting a new dataset of the packet level performance of real-time streaming to residential Internet users. Then, I use these packet level traces to evaluate the performance of commonly used models for packet loss simulation, and finding the models to be insufficient, present a new type of model that more accurately captures the loss behaviour. Finally, to demonstrate how a better understanding of the network can improve video quality in a real application scenario, I evaluate the performance of forward error correction schemes for Internet video using the measurements. I show that performance can be poor, devise a new metric to predict performance of error recovery from the characteristics of the input, and validate that the new packet loss model allows more realistic simulations. For the effective deployment of Internet video systems to users of residential access networks, a firm understanding of these networks is required. This dissertation provides insights into the packet level characteristics that can be expected from such networks, and techniques to realistically simulate their behaviour, promoting development of future video applications.

Abstract

Computer architectures can use custom logic in addition to general pur- pose processors to improve performance for a variety of applications. The use of custom logic allows greater parallelism for some algorithms. While conventional CPUs typically operate on words, ne-grained custom logic can improve e ciency for many bit level operations. The commodi ca- tion of eld programmable devices, particularly FPGAs, has improved the viability of using custom logic in an architecture. This thesis introduces an approach to reasoning about the correctness of compilers that generate custom logic that can be synthesized to provide hardware acceleration for a given application. Compiler intermediate representations (IRs) and transformations that are relevant to genera- tion of custom logic are presented. Architectures may vary in the way that custom logic is incorporated, and suitable abstractions are used in order that the results apply to compilation for a variety of the design parameters that are introduced by the use of custom logic.

Abstract

Objectives APS-associated heart valve disease (HVD) is well described. Nonetheless, limited data exist on clinical parameters associated with the course of primary APS (pAPS) patients with HVD. The goal of this study was to assess clinical features and related outcomes in patients with APS-associated HVD. Methods In this multicentre retrospective study, we identified 33 pAPS patients with HVD (pAPS-HVD group) and compared their clinical course with 128 pAPS patients with normal heart valves on echocardiography (pAPS-control group). Results pAPS-HVD patients had more cerebrovascular events (56.3% vs 25%, P  = 0.005) and livedo reticularis (24.2% vs 7.8%, P  = 0.013) than pAPS-controls. Furthermore, catastrophic-APS (CAPS) (12.1% vs 2.4%, P  = 0.034), recurrent thrombosis (33.3% vs 4.7%, P  < 0.001) and need for advanced therapy (i.e. IVIG, plasmapheresis or rituximab) were more frequent in pAPS-HVD patients. Anti-β2-glycoprotein 1 IgG (84.8% vs 63.2%, P  = 0.034), anti-cardiolipin IgG (90.9% vs 64.8%, P  = 0.005) and triple positive aPL (75.8% vs 56.5%, P  = 0.047) were commoner in pAPS-HVD patients vs pAPS-controls. Ten of the 33 patients with pAPS-HVD underwent valve surgery, which was associated with male gender, smoking, arterial limb ischaemia and livedo reticularis. Conclusion pAPS-HVD patients had a more severe APS clinical course including CAPS and thrombotic events as well as a specific serology, namely IgG isotype aPL antibodies and triple positivity. Abstract

As the popularity of dance as a health-enhancing activity for adults over the age of 65 has grown, experimental research that aims to identify the physical, psychological, and social impacts of dancing is now prevalent in academic journals. Consistently, the participant's voice is left out of this research.Photo-elicitation interviews were used within an arts-informed methodology. Ten female dancers aged 66-77 years who attend the same weekly, creative dance class participated.Thematic analysis of interview transcripts revealed an overarching theme of psychosocial resilience and five sub-themes: connecting with others, challenging perceptions of aging, emotional vulnerability, the importance of creativity, and opportunity to exercise autonomy.The results offer novel insight into the impact of creative dancing on subjective wellbeing and suggest that creative dancing can foster psychosocial resilience. The relationship between creative dance and psychosocial resilience should be explored further in other creative dance groups and older male dancers.
