Abstract

Background/Aim: Ulcerative Colitis (UC) is an inflammatory bowel disease which is common in many areas of the world including Egypt. A lot of controversy regarding the pathogenesis of UC exist. The current study is an attempt to detect some pathogenic bacteria in UC patients. Materials and methods: Endoscopic colonic biopsies obtained from 40 patients with ulcerative colitis and 20 controls were analyzed by means of real-time PCR technique for the presence of Clostridium difficile, Helicobacter Pylori (H. pylori) and pathogenic Escherichia Coli (E. coli) which are positive for KPC and/or OXA-48. Results: All patients and control samples were negative for Clostridium difficile. Three of the 40 patient samples (7.5%) and none of the 20 controls were positive for H. pylori with no significant difference between the two groups. KPC-positive E. coli were detected in 11 of the 40 patients (27.5%) and in none of the controls with a significant difference between the two groups (P=0.01). All patients and control samples were negative for OXA-48 positive E. coli. Conclusion: Although this study does not support the claim that Clostridium difficile and/or H. pylori have a role in UC, it greatly suggests that pathogenic E. coli may be involved in one way or another in the course of UC. Abstract

Hepatitis C virus (HCV) infection is known to be one of the leading causes of hepatocellular carcinoma (HCC) all over the world. Previously, multiple studies have confirmed a decreased rate of HCC occurrence or recurrence in the cases of hepatitis C associated cirrhosis after treatment with interferon, in comparison to the untreated cases, even in the absence of clearance of HCV. Treatment programs with direct-acting antivirals (DAAs) as a new method for HCV treatment and cure in 2014, with higher safety and efficacy, were considered as an important step in the treatment of patients with history of HCC, improving their overall prognosis. Recently, reports coming from various European centers claimed that the risk of HCC increased following DAAs therapy, especially in cases with previous HCC. Moreover, other studies revealed that the recurrence of HCC after DAAs treatment was more aggressive. Even though others were not able to conclude the same results, the role of DAA therapy in recurrence of HCC in patients with previous HCC after sustained virological response (SVR) achievement remains questionable. 26. Detection of Helicobacter pylori oipA and dupA genes among dyspeptic patients with chronic gastritis

Abstract

Helicobacter pylori (H. pylori): is a microbe with wide genetic diversity that infects the stomach of most people in developing countries, leading to several clinical outcomes among different individuals such as gastritis, ulcers, or gastric cancer. Outer inflammatory protein A (oipA) and duodenal ulcer promoting (dupA) genes are among the possible virulence factors which determine the patient outcome. Aim: To detect oipA and dupA genes of H. pylori among dyspeptic Egyptian patients, and to investigate their correlation with the varying degrees of the associated chronic gastritis. Methods: The study enrolled 50 patients with dyspepsia, attending the Gastrointestinal Endoscopy unit of the Gastroenterology and Tropical Departments at Ain Shams University Hospital for upper gastrointestinal endoscopy, in the period between, June and, December 2019. Four antral gastric biopsies were taken from each patient for polymerase chain reaction assay to detect the virulence genes oipA, dupA, and cagA and for histopathological assessment. Results: Forty patients were H. pylori positive by histopathology and PCR. cagA, oipA, and dupA were identified in 6 (15%), 13 (32.5%), 9 (22.5%) of biopsies, respectively. Both cagA and oipA genes were highly significantly associated with increasing the severity of gastritis. Only oipA virulence gene showed a highly significant association with gastroduodenitis. There was a highly significant moderate association between cagA and oipA genes. Conclusion: oipA could be a virulence biomarker that serves a great value in predicting the progress of gastric mucosal damage in patients with chronic gastritis, and targeting antimicrobial therapy in those patients to prevent severe gastroduodenal diseases.

29. Assessment the Role of Non Invasive Biomarkers as an Early Predictors of Mucosal Healing in Ulcerative Colitis Patients Treated with the Biological Therapy.

Abstract

Background: Ulcerative colitis (UC) is characterized by confluent mucosal inflammation and erosions starting from the anal verge and extending to a variable extent, where patients often complain of diarrhea associated with rectal bleeding, abdominal tenderness, and weight loss. Numerous disease biomarkers have been studied in ulcerative colitis (UC) such as Erythrocyte sedimentation rate(ESR) and C- reactive protein(CRP). On the other hand, fecal calprotectin and Lactoferrin are more specific and sensitive but expensive and still not commonly used in clinical practice due to the need for on demand specimen collection. Objective: To evaluate the role of the non-invasive biomarkers as an early predictor of mucosal healing in ulcerative colitis patients treated with biological therapy. Methods: Twenty adult patients with ulcerative colitis who received biological therapy (after adequate screening) were included in our study. The patients met the indications for treatment with biological therapy according to European Crohn and Colitis Organization guidelines. The patients were presented to our IBD clinic at tropical medicine department, Ain shams university in the period from 1/2018 to 9/2021. Results: There is a strong relationship between clinical response and mucosal healing at 6 months of starting biological therapy as 100% of patients who achieved mucosal healing showed clinical response. Median N/L ratio, median PLT/L ratio and CRP/albumin ratio were 1.58, 425 and 1.19 respectively in patients who achieved mucosal healing at 6 months after 3 months of starting biological therapy with statistical significance (p-value = 0.019, 0.019 and 0.008) respectively in comparison to the patients who didn't achieve mucosal healing. Both median of L/M ratio and PLT/M ratio were decreased in patients achieved mucosal healing at 6 months after 3 months of starting biological therapy (3.43 and 660) respectively in comparison to patients who didn't achieve mucosal healing but it didn't reach any statistical significance. Conclusion: Non invasive biomarkers at 3 months like N/L ratio, PLT/L ratio, L/M ratio and PLT/M ratio may be used as non invasive predictors of mucosal healing at 6 months after starting treatment with biological treatment. 32. Role of transient elastography and controlled attenuation parameter measurements in predicting portal hypertension in Egyptian patients with hepatocellular carcinoma candidate for liver resection.

Abstract

Background and aim Hepatic resection for hepatocellular carcinoma (HCC) is a main therapy for the treatment of surgical candidate patients. However, careful patient selection is required to balance the risk of postoperative liver failure and the potential benefit of long-term outcomes. We investigated the role of transient elastography and controlled attenuation parameter (CAP) in predicting portal hypertension in those patients, to assess if they could replace an invasive procedure [hepatic venous pressure gradient (HVPG)]. Moreover, their role in prediction of HCC recurrence after surgical resection was assessed. Patients and methods The study was performed at Ain Shams University hospitals, Tropical Medicine Department. A total of 30 patients with HCC, being candidate for surgical resection, were included in our study. We divided them into two groups according to the presence or absence of portal hypertension. Assessment of portal hypertension was done by laboratory markers, upper gastrointestinal endoscopy, and HVPG measurements. Transient elastography and CAP measurements were done for all patients. Correlations were done between transient elastography readings and CAP readings with other parameters of portal hypertension. Patients who underwent surgical resection were followed up for 6 months after liver resection to detect HCC recurrence. Results A positive correlation between transient elastography, CAP, and HVPG was found. Receiver operating characteristic curves of transient elastography, CAP, and a combination between transient elastography+platelet were drawn to differentiate between the two groups. The best cutoff point for transient elastography to detect significant portal hypertension was more than 18, with sensitivity of 93.75%, specificity of 100.0%, and area under the curve (AUC) of 99.8. Moreover, the cutoff point for CAP to detect significant portal hypertension was more than 217, with sensitivity of 87.5%, specificity of 64.29%, and AUC of 81.2%. However, the cutoff point of the platelet count was 166, with sensitivity of 100%, specificity of 86.67%, and AUC of 100%. The combination between transient elastography and platelets showed sensitivity of 87.5%, specificity of 100.0%, and AUC of 95.5%. Moreover, transient elastography showed a significant role in predicting early recurrence of HCC, with P value of 0.011. Conclusion Transient elastography and CAP can be used as a useful tool in evaluating portal hypertension and also can be used in predicting early recurrence of HCC after surgery.
