ObjectiveIt has been demonstrated that alcohol continues to be the most prevalent reason causing traffic accidents. This study aimed to evaluate the alcohol levels in the traffic accident cases admitted to Ege University Hospital.MethodsBlood samples of the 4,266 cases were analyzed with the enzymatic immunoassay method between 2016 and 2017. The data was obtained from the hospital automation system and analyzed statistically.ResultsOf all cases (n = 20,058) admitted to the hospital department, the rate of traffic accidents was 21.3% (n = 4,266). The mean age was 32.38 +/- 12.4, and 70.4% of the cases were male. Ethyl alcohol was positive in 9.6% (n = 409) of the cases, and 33.1% (n = 136) of them were in the 25-34 age range. A significant increase was observed in 2017 compared to 2016 (p < 0.05). About 87.6% (n = 360) of the positive cases were above the legal limit of 0.50 g/L for drivers except for professional drivers. About 57.7% (n = 237) of the positive traffic accident cases were passengers, and 27.2% (n = 112) were motor vehicle and motorcycle drivers.ConclusionsThis paper presented an updated ethyl alcohol determination in the traffic accidents of the Izmir region in order to have an overall picture of Turkey. Continuous and systematic follow-up studies may help to monitor the alcohol use problem.

Objective:As a result of using different scales while preparing disability reports, different rates are determined for the same person. Comparing the disability rates measured by the Scales in the annex of “Capacity to Work and Vocational Permanent Disability Rates Detection Operations Regulations (CVPR)” and the Scale in the annex of “Regulation on Disability Criteria on Classification and Health Board Reports to Be Issued to the Disabled (RDC)”, this research aims at identifying differences between the scales, thereby indicating various hardships experts encounter.Methods:The cases’ gender, occupation and trauma history as well as the request letters, the injuries related to the event and the examination findings were recorded forms. After calculating each case’s profit loss rate in profession and disability rate, the components that led to different rates were evaluated.Results:It was found that following trauma, tibial, fibula and vertebral injuries occurred at most. The most frequent sequelas (52%) were a limitation of movement at a joint and a gait disorder. Since the disorders of almost half of the cases (48.1%) identified with sequela in CVPR do not appear in the A Scale, the discretionary power was enacted. The school environment is the place where children mostly are away from parental supervision for the first time and for a long period of time. Children may be injured by accidents like burns at the school. Thus, making them vulnerable when security measures are not taken accordingly. This study aimed to discuss the trauma severity and medicolegal aspects of a scald injury in the school. This is a case of a 7-year-old male who had burn injury by spilling hot water at the school canteen with a medicolegal perspective by related literature. A second degree burn that covers about 30% of his body surface was determined in his first medical records. Sepsis and renal insufficiency had been developed in the process. Three years after the accident, his tissue losses and psychological symptoms due to the accident were examined for his forensic report. Findings conspicuously showed that accidents might cause permanent physical and psychological damages in individuals, their families and also significant costs for the society. Accidents and their negative impacts may be prevented by simple measures like educating the school staff, families and children about first aid for burn injuries. These impacts are subject to both punishment and compensation lawsuits. In this regard, expert witnesses and forensic reports are important to prevent miscarriages of justice. © 2021 by Society of Pediatric Emergency and Intensive Care Medicine.

Aim: It was aimed to examine the records of individuals 65 years of age and older for which forensic reports were prepared, to identify injury characteristics in older people and to draw attention to the precautions to be taken to prevent older people from traumas in this study. Materials and Methods: Among the total of 2471 forensic reports which are prepared between 01 January 2014 and 31 December 2015 in Department of Forensic Medicine of Ege University Faculty of Medicine, the reports of 101 cases who were 65 years of age and older at the time of event were evaluated retrospectively. The data were evaluated by transferring to statistical program. Results: Of the 101 cases evaluated, 63 (62.4%) were male. The mean age was 72.8±6.04 and maximum age was 89 years old. The traffic accidents were the most common cause of trauma (82.3%). The extremities were the most affected body region. Conclusions: When the damages caused by similar traumas are compared in older people with other age groups, they would be much harmful in older people. Therefore, even taking the necessary measures to prevent traffic accidents which were determined as the most important cause of trauma in the cases aged 65 years and older, would be an effective step in reducing mortality and morbidity. Explaining the characteristics of traumas occurring in older people is thought to contribute to the determination of the measures to be taken in this regard.

Erektil disfonksiyon, cinsel ilişki için yeterli ereksiyonun elde edilememesi veya sürdürülememesidir. Bu olgu sunumunda; trafik kazasına bağlı multitrav-malı olguda gelişen erektil disfonksiyonu adli-tıbbi açıdan değerlendirmek ve yaralama suçlarının adli tıp açısından değerlendirilmesi amacıyla kullanılan kıla-vuza katkıda bulunmak amaçlandı.Tıbbi evrakta; olaya bağlı pelvik kırıklar, mesane ve üretra rüptürü ve diğer yaralanmaları olduğu kayıtlıydı. Olgunun Anabilim Dalımıza başvurusu sonrası istenen Üroloji konsültasyonu sonucunda; mevcut bulgularla ereksiyonu gerçek-leştirmek için organik açıdan yeterli olmadığı anlaşıldı.Travmanın erektil disfonksiyona yol açtığı belirlenen olguda, yaralanmanın ürolojik açıdan kişinin organlarından birinin işlevinin sürekli yitirilmesine yol açtığı sonucuna varıldı.Erektil disfonksiyonun, cinsel fonksiyon bozuklukları başlığı altında ilgili kılavuzda yer almasının, adli rapor düzenleme sürecine yar-dımcı olacağı düşünülmektedir., Erectile dysfunction is the inability to maintain or to obtain adequate erec-tion for sexual intercourse. in this case report; it was aimed to evaluate erectile dysfunction which developed after a traffic accident due to multi-trauma and to contribute to the guideline for the evaluation of the crimes in forensic medicine.Medical records included pelvic fractures, bladder and urethra ruptures as well as other injuries. Urology consultation was requested after the application of the case to our department. As a result of this, regarding the current find-ings, it was found that the case was not enough to perform erection in terms of organic aspects.It was concluded that the trauma led to the permanent loss of one of the or-gans in terms of urology.The inclusion of the erectile dysfunction in the relevant guidelines under the heading of sexual dysfunctions is thought to be helpful in the process of forensic report-making.

Amaç: Trafik kazaları hem ceza hukuku hem de tazminat hukuku açısından sorumluluk doğurabilmektedir. Kalıcı sekellerin ve bu sekellere bağlı maluliyet oranlarının değerlendirilmesinde, Adli Tıp uzmanlarına büyük görevler düşmektedir. Bu araştırmada; trafik kazalarına bağlı maluliyet raporları hakkında Yargıtay’ın bozma kararları incelenerek, göz önünde bulundurulması gereken hususların değerlendirilmesini amaçladık. Gereç ve Yöntem: Trafik kazası sonucu meydana gelen maluliyet oranı hesaplamalarında kullanılan yasal mevzuat ve “trafik kazası” kavramları anahtar kelime olarak kullanılarak, “https://karararama.yargitay.gov.tr/YargitayBilgiBankasiIstemciWeb/” adresinden ulaşılan kararlar tarandı. Yargıtay’ın trafik kazasına bağlı maluliyet oranı raporlarında, ilk derece mahkemelerince alınan kararları bozma nedenleri araştırıldı. Bulgular: Yargıtay 17. Hukuk Dairesi’nin 376 bozma kararına ulaşıldı. Haksız fiilin gerçekleştiği tarihte yürürlükte olmayan tüzüğe/yönetmeliğe göre rapor düzenlenmesinin en sık (n=262, %69,7) bozma nedeni olduğu görüldü. Bunu sırasıyla; alınan raporlar arasında çelişkili oranlar olması, hangi tüzüğün/yönetmeliğin esas alındığının belli olmaması/anlaşılamaması, yazım hatası olması, hükme esas alınacak maluliyet raporu alınmaması ve illiyet bağı ile ilgili sorunların izlediği saptandı. Sonuç: Maluliyet raporlarının; anlaşılır, gerekçeli ve ilgili mevzuata uygun olarak düzenlenmesi esas olmalıdır. Yürürlükteki mevzuata uygun olarak düzenlenmemiş, uygun şekilde gerekçelendirilmemiş, illiyet bağı konusuna dikkat edilmemiş, çelişki bulunan ve denetime elverişli olmayan raporlar hak kayıplarına, yargılama sürecinin uzamasına neden olabilmektedir.

Son yillarda ani kardiyak olum oranlarinda artis gorulmektedir. Gunumuzde, implante edilebilir kardiyoverter defibrilator (implantable cardioverter defibrilator, ICD) ani kardiyak olumleri onlemede kullanilan bir cihazdir. Travma sonucunda cihazda birtakim problemler meydana gelebilmektedir. Kablo kirigi bu problemlerden biridir. Literaturde, kunt travmaya bagli ICD komplikasyonlarinin, ozellikle kablo kiriginin oldukca nadir goruldugu belirtilmektedir. Bu olgu sunumunda; eski esi tarafindan sol kolu cekilerek kunt travmaya maruz kalan ve olusan kablo kirigi nedeniyle uygunsuz soklara maruz kalan kadin olgu medikolegal acidan degerlendirildi. Olguda, gogse direkt bir travma olmadan, sol kolun cekilmesine bagli olarak lead kirigi meydana gelmistir. Hafif siddetteki bir travmanin bile, yasamsal tehlike olusturabilecek, ciddi klinik sonuclara neden olabilecegine dikkat cekmek amaclandi.

Background Serotonergic dysfunction is implicated in depression, psychiatric disorders and suicidal behaviors. The first and rate-limiting step in the synthesis of serotonin is catalyzed by tryptophan hydroxylase (TPH) which is encoded by TPH1 and THP2 genes. Genetic association studies have revealed contradictory results about the effect of the TPH1 A218C (rs1800532) polymorphism on suicidal behavior in different populations. Material and method In this study, we investigated A218C polymorphism in 109 suicide attempters and 98 healthy controls. Socio-demographic characteristics of participants were obtained through questionnaire. DNA was extracted from peripheral blood and genotyping was performed by Real Time PCR. Fisher's exact test was used to evaluate the significance of the difference among the independent variables. Hardy-Weinberg equilibrium was tested using Pearson's goodness-of-fit chi-squared test. Results The frequency of A allele was significantly higher in suicide attempters than controls (46.33% vs. 35.71%, p =0.0357). However, there were no differences in genotype frequencies of this locus between participants having attempted suicide and controls ( p >0.05). Among males, frequencies of CC genotype and C allele were found to be significantly higher in controls ( p =0.0125, p =0.0298). With regard to the female subjects and female controls, no significant association was detected between suicidal behavior and genotype/allele frequencies ( p >0.05). Conclusion Our results provide evidence that A allele of TPH1 A218C polymorphism may be associated with suicidal behavior in Turkish population.

Amaç: Bu çalışmada, İzmir Karşıyaka Devlet Hastanesi Adli Tıp Polikliniğince 2015 yılı içinde düzenlenen adli raporlar incelenerek bölgemizin adli olgu profilinin belirlenmesi ve elde edilecek bulgularla adli olayların önlenmesi konusunda çalışan kişi ve kurumlara katkıda bulunmak amaçlanmıştır.Yöntem: Bu çalışma, 01.01.2015-30.11.2015 tarihleri arasındaki 11 aylık dönemde düzenlenen toplam 1022 adet adli raporun retrospektif olarak incelenmesiyle yapılmıştır. Olguların sosyodemografik özellikleri, rapor tarihi, olay türü (yaralanma şekli), yaralanan vücut bölgesi ve yaralanma derecesi (sonuç bölümünde verilen kararlar yönünden) ayrı ayrı kodlanıp bilgisayar ortamına (SPSS for Windows 17.0 istatistiksel paket programı) aktarılarak analiz edilmiş ve benzer çalışmalarla karşılaştırılmıştır.Bulgular: 1022 olgunun 716 (%70,1)'sı erkek, 306 (%29,9)'sı ise kadındı. Olguların büyük bölümünün genç ve orta yaş grubunda olduğu ve polikliniğimize yaz aylarında (335), en fazla ise Haziran ayında (131) başvurduğu belirlendi. Bin yirmi iki olgunun en büyük kısmını, % 61,2 (626) ile yaralama eylemine (darp) bağlı olgular ve %27,4 (279) ile trafik kazalarına bağlı yaralanmalar oluşturmakta idi. Yaralama eylemi nedeniyle başvuran olguların, %57,3 (586)'ü künt travmalar, %2,8 (29)'i kesici delici aletler, %1,1 (11)'i ateşli silahlar ile yaralanmıştı. Tek bölgede olan yaralanmaların en sık baş-boyun (n=307, %30,0) ve ekstremite (n=250, %24,5) bölgesinde olduğu, en sık kırılan tek kemiğin ise burun kemiği (n=64, %6,3) olduğu belirlendi.Sonuç: Çalışmamızda elde ettiğimiz bulgular genel anlamda benzer çalışmalar ile uyumlu bulunmuştur. Adli olaylarda istatistiki bulguların saptanması, bölgesel profillerin ve veri tabanının oluşturulması, bu tür olayların önlenmesi çalışmalarında önemli bir role sahiptir (Örneğin, aile içi şiddet, kadına yönelik şiddet, suça sürüklenen çocuklar, trafik kazaları, etkili eylem-darp olaylarında eğitim ve öfke kontrolü gibi çalışmalar)., Objective: The aim of this study was to determine the criminal profile of our district by the analysis of the criminal reports arranged by İzmir Karşıyaka State Hospital Department of Forensic Medicine in 2015 and by these findings to make a contribution to professionals and institutions that work for the prevention of criminal cases. Methods: This study was conducted via retrospective analysis of total of 1022 criminal reports prepared during 11 months between 01.01.2015 and 30.11.2015. Socioeconomic characteristics of the cases, dates of the reports, type of the incident (type of injury), injured body parts and severity of the injury (according to decisions in the conclusion part of the report) were coded one by one, transferred to a computerized system (SPSS for Windows 17.0 package program) for analysis and compared with the results of similar studies. Results: A total of 1022 cases consisted of 716 male (70.1%), and 306 (29.9%) female patients. Most of the cases were in the young and middle-age groups. Most of the patients (n=335) were admitted during summer months, and notably in June (n=131). Majority of these 1022 cases constituted of injuries due to physical assaults (n=626; 61.2%), and traffic accidents (n=279; 27.4%), blunt traumas (n=586; 57.3%), stab wounds (n=29; 2.8%), and gunshot wounds (n=11;1.1%). In the single body-part injuries, head and neck (n=307, 30.0% ) and extemity (n=250, 24.5%) injuries were the leading regions that effected. Nasal bone was the most commonly broken bone (n=64, 6.3%). Conclusion: Our study results were found to be generally compliant with those of similar studies. Determination of the statistical findings and the creation of district profiles and database in criminal cases has an important role in the prevention efforts of these incidents. (training and anger management in the incidents like; domestic violence, violence against women, children driven to crime, traffic accidents, effective action-assault, etc.).

Bu çalışmada Ege Üniversitesi Tıp Fakültesi asistan doktorlarına yönelik bir anket düzenlenerek, e-reçete kullanımına bakış açılarının değerlendirilmesi, e-reçete kullanımının iş kolaylığı, zaman ve ilaç israfının engellemesi açısından beklentilere ne kadar cevap verdiği, yükümlülüklerinin farkındalığı ve güvenilirlik açısından değerlendirilmesi amaçlanmıştır. Katılımcıların %51`i e-reçete eğitimi aldığını, %86`sı e-reçete kullanmanın getirdiği yükümlülükleri bilmediğini belirtmiştir. 73 kişi e-reçete kullanımını güvenli bulmadığını, 85 (%85) kişi e-reçete kullanmanın zaman tasarrufu sağlamadığını, 72 kişi e-reçete kullanmanın ilaç israfını engellemediğini ve yine 72 kişi de e-reçete kullanımının hasta açısından olumlu sonuçları olacağını düşünmediğini bildirmiştir. E-reçete uygulamasının pratikteki kullanımının artmasından sonra, amacına ulaşıp ulaşmayacağının zamanla anlaşılacağı ve bir takım eksiklikler mevcutsa da yeri geldikçe giderilebileceği umulmaktadır., In this study it was aimed to evaluate the viewpoints of the assistant doctors working in Ege University Medicine Faculty, evaluate how the e-prescriptions facilitate work, save time, prevent the extravagance of using drugs, and evaluate the awareness of the doctors about the concomitant responsibilities and the reliability of e-prescriptions. 51% of the volunteers stated that they had had an education about the e-prescription. %86 of the volunteers stated that they didn`t know the concomitant responsibilities of the e-prescription. 73 volunteers stated that the e-prescription isn`t reliable, 85 volunteers (85%) stated that the e-prescription didn`t save any time, 72 volunteers stated that the e-prescriptions didn`t prevent the extravagance of using drugs and again 72 volunteers stated that they thought no positive results would be obtained by using the e-prescription. The implementation of the e-prescription is hoped both to be understood whether it will be successful or not and to overcome the existing deficiencies.

Amaç: Cinsel suçlar, tüm dünyada her yaş grubundaki insanı tehdit eden toplumsal bir sorundur. Cinsel suçların, mağdurda birçok ruhsal hastalığa zemin hazırladığı bilinmektedir. Bu çalışmada, Ruh Sağlığı ve Hastalıkları Anabilim Dalı Erişkin Adli Kurul'da raporları düzenlenen cinsel saldırı mağdurlarının demografik özellikleri, suç konusu olayların özellikleri ve düzenlenen raporların sonuçları değerlendirilerek literatürdeki diğer çalışmalarla karşılaştırılması amaçlandı.Gereç ve Yöntem: Adli Makamlar tarafından 2012 - 2014 yılları arasında EÜTF Ruh Sağlığı ve Hastalıkları Anabilim Dalı Erişkin Adli Kurul'a rapor düzenlenmesi nedeniyle dosyası ile birlikte gönderilen olgulara ait kayıtlar; olguların demografik özellikleri, rapor istem makamı, olayın türü ve tarihi, saldırgana ait bilgiler, olguların muayene bulguları ile düzenlenen raporların sonucu açılarından retrospektif olarak incelenerek, elde edilen veriler Windows için SPSS 18 istatistik programı ile değerlendirildi.Bulgular: 01 Ocak 2012 - 31 Aralık 2014 tarihleri arasında rapor düzenlenen 311 olgunun 12'si (%3,9) erkek, 299'u (%96,1) kadındı. Olay tarihindeki yaşları 6 ila 89 arasında ve ortalaması 26,58+12,22 idi. Raporların %37'sinin (n=115) Ağır Ceza Mahkemeleri tarafından istendiği görüldü. Gerçekleştiği beyan edilen cinsel saldırıların %56,9'u (n=177) nitelikli olup, mağdurların %80,7'sinin (n=251) kendisinin şikâyetçi olduğu, saldırganların tümünün erkek olduğu, bunların %70,1'inin (n=218) mağdurun çevresindeki kişilerden olduğu tespit edildi. Olay sonucunda olguların %61,4'ünde (n=191) bir, %34,1'inde (n=106) ise birden fazla psikiyatrik bozukluk meydana geldiği saptandı.Sonuç: Cinsel saldırı mağdurlarında tespit edilen ruhsal rahatsızlıklar ile cinsel saldırıların her yaş grubundaki başta kadın ve çocuklara karşı, çoğu zaman erkekler tarafından gerçekleştirildiği bilgisi literatürdeki diğer çalışmalarla uyumlu bulunmuştur., Objective: Sex crimes remain a social concern affecting people of all ages globally. It is known that sex crimes pave the way for many mental disturbances. In this study, it was aimed to investigate the demographics of sexual assault victims, features of sexual assaults and the results of the reports evaluated by Ege University Faculty of Medicine Adult Judicial Council and to compare those to other studies in literature.Materials and Methods: Records of cases sent with their files for report preparation to Ege University, Faculty of Medicine, Department of Psychiatry, Judicial Council between 2012 - 2014 were examined retrospectively and the data were evaluated with SPSS 18 for Windows.Results: 12 (%3.9) of 311 cases which were reported between 01.01.2012-31.12.2014 were male and 299 (%96.1) were female. Ages at the time of the event ranged between 6 and 89 and the average was 26.58+12.22 %37 (n=115) of the reports were requested by High Criminal Courts. %56.9 (n=177) of the sexual assaults claimed to occur were qualified/aggravated, %80.7 (n=251) of the victims presses charges themselves, all of the offenders were male, %70.1 (n=218) of the offenders were people from the neighborhood of the victim. As a result of the event, %61,4 (n=191) of the victims developed one, %34.1 (n=106) developed more than one psychiatric disorders.Conclusion: Mental disorders sexual assault victims are diagnosed with and the fact that sexual assaults are performed by mostly men against women and children of all ages were consistent with the literature.

Shaken baby syndrome (SBS) is a serious type of physical abuse characterized by sub - dural hematoma and retinal hemorrhage. This syndrome is frequently seen in children younger than 2 years of age. Most of the time it results in death or severe neurological damage. In these cases, external visible injuries are generally absent; often there no history of shaking or trauma. Clinical symptoms and findings are nonspecific. Therefore, most of the cases are missed when SBS is not considered in the differential diagnosis. In this article, we reported two SBS cases admitted to our hospital with a history of head trauma and convulsions, resulting in death in one case.

Amaç: Çocuk istismarı olgularının tanınmasında ve yönetilmesinde en uygun yaklaşım çok merkezli bir ekip çalışması ile sağlanabilir. Bu çalışmada çocuk istismarı tanısı alan olgularımızın demografik ve klinik özelliklerinin değerlendirilmesi amaçlandı. Gereç ve Yöntem: Ağustos 2009-2010 yılları arasında Ege Üniversitesi Çocuk Koruma Birimi’ne yönlendirilip çocuk istismarı tanısı alan olguların demografik ve klinik özellikleri geriye dönük olarak değerlendirildi. Bulgular: Bu çalışmada çocuk istismarı tanısı alan yaşları 1-17 yaş arasında değişen 46’sı (%51,7) kız, 48’i (%48,3) erkek toplam 89 çocuk değerlendirildi. Cinsel, fiziksel, duygusal istismar ve ihmal oranları sırasıyla %49, %25, %11, %14 bulundu. İstismarı gerçekleştiren kişi fiziksel istismar olgularının %67’sinde baba iken, cinsel istismar olgularının %59’unda yabancı birisinin olduğu saptandı. Fiziksel istismar kız ve erkek cinsiyette eşit sıklıkta iken, cinsel istismar olgularının çoğunluğunu (%56,8) kız çocukları oluşturmaktaydı. Çıkarımlar: Ülkemizde çocuk istismarı ve ihmaline ilgi ve duyarlılığın artması ile birlikte tanı alan olgu sayısında artış olmuştur ve pek çok hastanede çocuk koruma birimleri “multidisipliner” olarak hizmet vermeye başlamıştır. Bütün bunlar olumlu gelişmeler olsa da, ülkemizde çocuk istismarı olgularının tanınması ve doğru yönlendirilmesi konusunda daha alınacak çok yol vardır., Aim: Cases of child abuse and neglect (CAN) should be ideally managed by a multidisciplinary team. In the present study, our aim was to review the demographic and clinical features of cases of child abuse and neglect followed-up by the Child Protection Team of Ege University, Izmir, Turkey. Material and Method: The data of the cases of CAN referred to the multidisciplinary team of Ege University between August 2009 and 2010 were retrospectively reviewed. The demographic and clinical characteristics of the cases were summarized. Results: There were a total of 89 CAN cases evaluated by the team in our hospital during the last 12 months. The age of the cases ranged from 1 to 17 years, and 43 (48.3%) of them were male, while 46 (51.7%) were female. The sexual, physical and emotional abuse rates were 49%, 25% and 11% respectively, while 14% of them were diagnosed as pure neglect cases. The fathers were offenders in 67% of cases of physical abuse and in 9% of cases of sexual abuse. However, an extra-familial person was the perpetrator in 59% of the cases of sexual abuse. Conclusions: Recently, the number of reported CAN cases in our country has increased after the increase in the awareness of the public about this subject and multidisciplinary child protection teams have been established within hospitals. However, there is a long way to go, although remarkable improvements have been observed in the last decade

Yüz, kişilerin karşılaşmada ilk göze çarpan vücut bölümüdür. Yaralanma nedeniyle yüz bölgesinde oluşan izler kişilerin sosyal hayatını önemli derecede etkileyeceğinden, Türk Ceza Kanunu’nda (TCK) Madde 87, 89 ve 95’te “yüzde sabit iz” ve “yüzün sürekli değişikliği” kavramları tanımlanmış ve suçun ağırlaştırıcı sebebi olarak düzenlenmiştir. Yüz bölgesindeki yaralanmalar en sık trafik kazaları, darp, ateşli silahlar nedeniyle meydana gelir. Yüzde yüksek basınçlı maddelerle meydana gelen yaralanmalar oldukça nadirdir. Genellikle dışarıdan bakıldığında yara önemsiz gibi görünmekte, cilt altında meydana gelen hasar daha belirgin olmaktadır. Hasarın patofizyolojisinde, maddenin dokuda oluşturduğu distansiyon, kimyasal irritasyon ve meydana gelen sekonder enfeksiyon önemli rol almaktadır. Yüksek basınçlı su yaralanmalarının çoğu yaralanma özelliği ve iyileşme süreci açısından iyi seyirlidir. Ancak bazen suyun mekanik kuvveti, yüz bölgesinde göz, sinir, kemik yaralanmalarına neden olabilmektedir. Bu olgu sunumunda, yüz bölgesinde basınçlı su ile meydana gelen yaralanmanın TCK kapsamında değerlendirilmesi, sonuçları ile basınçlı sistemleri kullanan sektörlerde alınabilecek önlemler ve bilgilendirmenin öneminin tartışılması amaçlanmaktadır., Face is the most noticeable body part of a person. Due to the fact that injuries in the facial region affect the social life of an individual severely, in the Turkish Penal Code Articles 87, 89 and 95 define the concepts of “distinct mark on the face” and “distinct facial change”, and these are accepted as aggravating circumstances. Facial injuries most commonly occur due to traffic accidents, assaults and firearm injuries. Facial injuries with high – pressure agents are extremely rare injuries. When viewed from outside, the injury generally seems insignificant, but the damage occurring under the skin becomes increasingly apparent. In the patho-physiology of the injury, the distension formed by the substance in the tissue, chemical irritation and the resulting secondary infection all have important roles. Most of the high-pressured water injuries have good courses in the aspect of the properties and the healing process of the injuries. However, sometimes the mechanical force of water may cause eye, nerve and bone injuries in the face. In this case report, it is aimed to evaluate the injuries occurring in the face due to pressurized water within the scope of the Penal Code and to discuss the measures to be taken in the sectors using pressurized water systems and the importance of giving information on this subject.

Giriş: İzmir’de adli otopsisi yapılan 65 yaş ve üzeri olguların özelliklerini inceleyerek istatistiksel bir veri elde etmek ve ölüm nedeninin sağlıklı şekilde ortaya konabilmesi için adli otopsilerin önemine dikkat çekmek istenmiştir. Gereç ve Yöntem: 2004-2007 yılları arasında, Adli Tıp Kurumu İzmir Grup Başkanlığında otopsisi yapılan, yaşı 65 yaş ve üzeri olan 934 olgu retrospektif olarak incelenmiştir. Veriler SPSS programı ile değerlendirilmiştir. Bulgular: Araştırmaya katılan 934 olgunun 650’si erkek, 284’ü kadındır. Olguların yaş ortalaması 73.9±6.7’dir. En sık görülen ölüm sebepleri arasında; doğal nedenli ölümlerde 331 olguyla kardiyovasküler hastalıklar, kazalarda 94 olguyla trafik kazaları, cinayetlerde 31 olguyla ateşli silah yaralanmaları, intiharlarda 60 olguyla ası yer almaktadır. Sonuç: Tüm dünyada olduğu gibi ülkemizde de yaşlı nüfus hızla artmaktadır. Bu çalışmada elde edilen verilere bakıldığında; gerek doğal gerekse doğal olmayan ölüm nedenleri açısından yaşlılarda elde edilen bulgular genç erişkin otopsileriyle benzerlik göstermiştir. Ancak yaşlılık döneminde ölüm nedeni ve ölüm orijininin sağlıklı şekilde ortaya konabilmesi için, bu yaş grubunda adli otopsiler ve onlardan elde edilecek verilerin önemli olduğu görüşü desteklenmiştir., Introduction: We aimed to obtain statistical data and reveal the causes of deaths reliably, and to draw attention to the importance of forensic autopsies by examining the features of the autopsies performed on elderly people aged 65 and over in Izmir. Materials and Method: In this study, 934 cases aged 65 and over whose autopsies were performed at The Council of Forensic Medicine Branch of Izmir, between 2004 and 2007 were analyzed retrospectively. Data were analyzed by SPSS for Windows. Results: 650 of 934 cases were male, 284 of them were female. Average age of the cases was 73.9±6.7. The leading causes of deaths were cardiovascular diseases (n=331), traffic accidents (n=94), firearm injuries (n=31) and hangings (n=60) in the natural, accidental, homicidal and suicidal groups respectively. Conclusion: Old population is increasing rapidly in our country, as in all of the world. Results of this study showed that both natural and unnatural causes of deaths of the elderly as revealed in autopsies showed similarities with the results of younger autopsies. This study has supported the view that data obtained from forensic autopsies are of importance in order to reveal the causes and origins of deaths in the old age reliably.

Alcoholism is a complex genetically influenced disorder which refers to alcohol abuse and alcohol dependence. There are controversial results on the role of gene polymorphisms in alcohol dependence in the literature. Differences in population groups and selective inclusion criteria for alcohol dependence may affect results. In this study, we investigated the role of ADH1B Arg48His (rs1229984) and, ADH1C Ile350Val (rs698) gene polymorphisms in Turkish population. 100 healthy volunteers and 75 patients who were admitted to Ege University Alcohol Dependence Unit enrolled in the study. We found significant increase both in ADH1B (Arg48His) polymorphism Arg allele and Arg/Arg genotype frequency in patients. No profound connection between alcohol dependence and ADH1C Ile350Val gene polymorphism was detected. Alcohol dependence is an important health problem that depends on many genetic and environmental factors but we think that it is possible to interpret genetic risk for developing early diagnostic methods and treatment strategies by comprehensive linkage and association studies.

Distinguishing between injuries resulted from electrical current versus thermal energy is not only a difficult, but also a controversial issue in forensic medicine practice.In this study, an electrical current and a cautery were applied to dorsal skins of 10 rats and biopsies were taken from the injured sites as well as normal skin. In the histologic sections; some planimetric variables such as the perimeter, area, diameter equivalent circle, minimum feret, maximum feret, and the circular form factor of the nuclei located in normal and injured epidermis were measured with the aid of the computer-assisted image analysis.When compared with normal skin, all of the variables -nuclear area, perimeter, diameter equivalent circle, minimum feret, maximum feret, and circular form factor seemed to be decreased both in the electrical current- and cautery-applied skin samples.The differences between the variables measured in normal skin and in electrical- or cautery-applied skin samples were statistically significant (P < 0.05). However none of the variables showed any meaningful differences between the electrical- and cautery-applied areas.It was concluded that the nuclear changes due to electrical current and thermal injury are identical and morphometric analysis seems not to make any further contributions in the differentiating from each other. Therefore, conventional and more established methods for detection of metallization would be more effective.

Our study was designed to determine whether the hormone and enzyme levels that we have mentioned in serum specimens can be used in postmortem examinations of the cases of hanging or not. Our research includes the postmortem results of the thyroid tissue pathologic examinations, serum levels of triiodothyronine (fT3), thyroxine (fT4), thyrotropin (TSH), thyroglobulin, parathormone (PTH), calcitonin, and amylase in cases of death due to hanging. The mean of fT3 and thyroglobulin levels were higher in hanging cases than those of sudden death cases. This difference was statistically significant (P < 0.05). In our study, we obtained data demonstrating that high postmortem levels of free T3 and thyroglobulin may be indicators of vital reaction in hanging cases. In such cases of death, it seems that applying pressure on the neck region where the thyroid gland is located causes the leakage of thyroglobulin as well as fT3, the active form of the thyroid hormone.

Trafik kazaları sadece bedensel yaralanmalara neden olmamakta aynı zamanda önemli ruhsal ve nörolojik bozukluklara da yol açmaktadır. Posttravmatik parkinsonizm, kafa travmalarının meydana geldiği trafik kazalarından sonra nadiren görülen patolojik bir durumdur. Belirtiler klasik Parkinson hastalığından genellikle biraz farklıdır. Klasik triad (bradikinezi, tremor, rijidite) oluşmaz, tek bir belirti ile kalabilir. Bu çalışmada 2001 yılında geçirdiği trafik kazası sonrasında posttravmatik parkinsonizm ve demansiyel bulgular saptanan 68 yaşındaki bir erkek olgu tartışılmıştır. Kesin rapor istemi ile Anabilim Dalımıza başvuran olguda kazadan bir ay sonra yürüme güçlüğü saptanması üzerine Nöroloji Anabilim Dalından konsültasyon istenmiş, nöropsikiyatrik değerlendirme sonucu; karmaşık dikkat, konsantrasyon, planlama, organizasyon, konstrüksiyon, mental fleksibilite, yakın bellek işlevleri yönünden ılımlı-orta derecede (CDR 0,5-1) kognitif bozukluk ile trafik kazasından önce bulgu yokken travmaya bağlı olarak gelişen subdural hematom sonrası parkinsoniyel bulgular ve kognitif yıkım (demansiyel bulgular), bilateral ılımlı dişli çark rijiditesi, astezi-abazi tipi yürüme bozukluğu tespit edilmiştir. Bu olgu trafik kazası sonrası ender olarak ortaya çıkabilen posttravmatik parkinsonizme dikkat çekilmesi ve ayırıcı tanısında dikkat edilecek ilkelerin tartışılması açısından irdelenmiştir., Traffic accidents cause not only physical injuries also cause the important mental and neurological deficits. Posttraumatic parkinsonism is a pathological situation which is seen rearly after the traffic accidents with head trauma. The symptoms of posttraumatic parkinsonism are a little different than Parkinson disease’s. The classical triad (bradykinesia, tremor, rigidity) can not occur at posttraumatic parkinsonism, only one symptom can be seen in this disease. In this study, we introduced 68 years old male patient who had demantial symptoms and posttraumatic parkinsonism after the traffic accident in 2001. A neurology consultation was wanted for the case in our study because of his walking problems. The result of the neuropsychological evaluation was complicated careful attention, concentration, planning, organization, construction, mental fleksibility and moderate- middle degree (CDR 0.5-1) deficit of close memory. Also, parkinsoniel diagnoses and cognitif deficit (demantial diagnoses), bilateral moderate rigidity, astheny- abasia like walking problems were found related to the subdural hematoma which had occured at the traffic accident. This case was investigated to attract attention to posttraumatic parkinsonism which is seen rarely in the literature. Besides, we wanted to discuss other diagnoses which semptoms like posttraumatic parkinsonism.

Gunumuzde teknolojinin hizla gelismesinin cok buyuk avantajlar sunmasinin yaninda, yapilan arastirmalar son yillarda guvenlik ihlallerinde artis oldugunu gostermektedir. Bu nedenle, bu calismada Ege Universitesi Tip Fakultesi ogrenci ve hekimlerinden olusan gonullu katilimcilarin demografik verilerinin elde edilmesi, internet kullanma sikligi ve amaci, internet uzerinden yaptiklari islemler, kullandiklari gercek bilgiler gibi internet ve sosyal ag kullanimlari hakkinda bilgi edinmek, kullandiklari sosyal aglara duyduklari guven ile internet ve sosyal ag uzerinden yasadiklari sorunlarin saptanarak degerlendirilmesi amaclandi. Bu calismada, 54u hekim, 46si tip fakultesi ogrencisi olmak uzere toplam 100 gonullu katilimciya, internet ve sosyal ag kullanimlari hakkinda sorular iceren 29 soruluk bir anket uygulanarak, anket formlarinin verileri istatistik programi ile analiz edildi. Calismaya katilanlarin yas ortalamasi 25.5±4.8 olup, %54u kadindi. Katilimcilarin %10'u internet kullanimiyla ilgili kursa giderek egitim aldigini, %89u her gun internet kullandigini, %84u internete evinden baglandigini belirtti. Internet uzerinden sosyal ag kullandigini soyleyen 87 (%87) katilimcinin 57si (%65.5) ise her gun kullandigini bildirdi. Sosyal aglarda gercek bilgilerini kullandigini belirtenlerin orani %86 idi. Yaptiklari paylasimlar sonucu katilimcilarin %47si rahatsiz edildigini belirtirken, %5i profilinin baskasi tarafindan ele gecirildigini, %7si ise zarara ugradigini belirtti. Rahatsiz edildigini belirten %47'lik grubun %95.7sinin gercek kimlik bilgilerini veren kisiler oldugu saptandi. Internet kullaniminin, sagladigi faydalarin yani sira bazi dezavantajlari da bulunmaktadir. Internet ortaminda kisisel bilgilerin paylasilmasiyla, bu bilgiler sosyal aglara uye kisiler tarafindan gorulebilmekte, bilisim teknolojisi suclularinin isini kolaylastirmaktadir.
