Giriş ve Amaç: Maksillofasiyal travmalı hastalarda bozulan anatomi ve travmaya sekonder gelişen ödem nedeniyle hava yolu yönetimi hızla müdahale edilmesi gereken bir durumdur. Travma hastasının değerlendirilmesinde ve yönetilmesinde öncelikle, hava yolu güvenliği sağlanmalı ve servikal omurga stabilize edilmelidir. Bu yazıda; ateşli silah yaralanması sonucu maksillofasiyal travma gelişen iki olguda uygulanan anestezi yönetiminin literatür eşliğinde tartışılması amaçlandı.Gereç ve Yöntem: Bu çalışmada iki hastanın hastane bilgi sistemindeki kayıtları ve anestezi kayıtları incelendi. Hastaların perioperatif özellikleri, cerrahi endikasyonu, anestezi tekniği ve komplikasyonları kaydedildi.Bulgular ve Sonuç: Maksillofasiyal travmalı hastalar; yüz kemikleri ve yumuşak dokulardaki hasarlanmaya bağlı olarak kısa sürede gelişebilen hava yolu ödemi, kan, pıhtı, yabancı cisim, kırık kemik fragmanları gibi hasarlı doku veya travmaya neden olan ana materyalin hava yolunu kapatabilmesinden kaynaklı aspirasyon riski nedeniyle hızla hava yolunun garantiye alınması gereken özellikli hasta grubudur. Yazımızda ateşli silah yaralanması sonucu maksillofasiyal travma gelişen iki olgudaki hava yolu yönetimini tartışmayı amaçladık., Introduction and Aim: Airway management is a condition that requires rapid intervention in patients with maxillofacial trauma due to impaired anatomy and edema secondary to trauma. In the evaluation and management of the trauma patient, airway safety should be ensured and the cervical spine should be stabilized. In this article; It was aimed to discuss the anesthesia management applied in two cases with maxillofacial trauma as a result of gun shot injury, in the light of the literature.Materials and Methods: In this study, records of two patients in the hospital information system and anesthesia records were examined. Perioperative characteristics, indications for surgery, anesthesia technique and complications of the patients were recorded. Results and Conclusion: Patients with maxillofacial trauma; Airway edema, which can develop in a short time due to damage to facial bones and soft tissues, damaged tissue such as blood, clot, foreign body, broken bone fragments, or because of the risk of aspiration due to the main material causing trauma can close the airway, it is a special patient group that needs to be guaranteed the airway quickly. In our article, we aimed to discuss the airway management in two cases with maxillofacial trauma as a result of gun shot injury.

Cardiac autonomic neuropathy is a frequent complication of type 2 diabetes mellitus. Cardiac autonomic neuropathy, in which sympathetic tone predominates over parasympathetic activity, increases both cardiovascular morbidity and mortality and unfortunately has no definitive treatment. Sodium-glucose cotransporter-2 inhibitors have been suggested to reduce sympathetic nervous system activity, based on the results from previous studies. In this study, we aimed to investigate the effect of 24-week treatment with dapagliflozin, a sodium-glucose cotransporter-2 inhibitor, on cardiac autonomic function measures in patients with type 2 diabetes mellitus and cardiac autonomic neuropathy.Dapagliflozin 10 mg/day (n=42) or non-sodium-glucose cotransporter-2 inhibitor oral antidiabetic(s) (n=38) was added to the treatment of patients whose glycemic control could not be achieved with existing treatments. The patients with definite or confirmed cardiac autonomic neuropathy diagnosed by cardiovascular autonomic reflex tests underwent 24-hour Holter-electrocardiogram recordings to obtain heart rate variability and heart rate turbulence parameters before starting additional medication and after a 24-week treatment period.In-group analyses showed that dapagliflozin 10 mg/day for 24 weeks improved heart rate variability and heart rate turbulence parameters and decreased the frequency of ventricular premature beats relative to their baseline values. No such findings were observed in the control group despite similar glycemic control. Comparisons between dapagliflozin group and the control group showed that these effects of dapagliflozin were significantly better than non-sodium-glucose cotransporter-2 inhibitor oral antidiabetics.Dapagliflozin improves measures of cardiac autonomic function compared to the control group in type 2 diabetic patients with cardiac autonomic neuropathy. This intergroup benefit, demonstrated for the first time, may be promising for the regression of cardiac autonomic neuropathy with sodium-glucose cotransporter-2 inhibitors.

ÖzetKonstriktif perikardit (KP), elastik olmayan perikardın, kalp dolumunu engellemesi nedeniyle ortaya çıkan bir diyastolik kalp yetmezliği şeklidir. KP, açıklanamayan kalp yetmezliğinin ayırıcı tanısında, özellikle korunmuş sol ventrikül ejeksiyon fraksiyonlu kalp yetmezliği hastalarında düşünülmelidir. KP gelişimiiçin risk faktörleri; önceden kalp ameliyatı ve radyasyon tedavisini içerir, ancak çoğu vaka halen idiyopatik olarak kabul edilmektedir. Teşhisi zor olabilir ve sıklıkla kardiyak görüntüleme ve hemodinamik kateterizasyon ile desteklenen ekokardiyografik değerlendirme gerektirir. Komplet cerrahi perikardiyektomi, kronik KP için tek etkili tedavi yöntemidir. Genellikle kardiyak manyetik rezonans görüntüleme ile tanımlanan subakut enflamatuar KP’li hasta alt grubu, anti-enflamatuar tedavilere cevap verebilmektedir., Constrictive Pericarditis (CP) is a form of diastolic heart failure that occurs when non-elastic pericardium prevents heart filling. CP should be considered in the differential diagnosis of unexplained heart failure, especially in patients with heart failure with preserved ejection fraction. Although previous cardiacsurgery and radiation therapy are the most frequently observed risk factors for CP development, most cases are still considered idiopathic. Diagnosis can be difficult and often requires echocardiographic evaluation supported by cardiac imaging and hemodynamic catheterization. Complete surgical pericardiectomy is the only effective treatment for chronic CP. A subacute subset of patients with subacute inflammatory CP, usually diagnosed by cardiac magnetic resonance imaging, is able to respond to anti-inflammatory treatments.

Background The SARS-CoV-2 pandemic is still ongoing and a major challenge for health care services worldwide. In the first WSES COVID-19 emergency surgery survey, a strong negative impact on emergency surgery (ES) had been described already early in the pandemic situation. However, the knowledge is limited about current effects of the pandemic on patient flow through emergency rooms, daily routine and decision making in ES as well as their changes over time during the last two pandemic years. This second WSES COVID-19 emergency surgery survey investigates the impact of the SARS-CoV-2 pandemic on ES during the course of the pandemic. Methods A web survey had been distributed to medical specialists in ES during a four-week period from January 2022, investigating the impact of the pandemic on patients and septic diseases both requiring ES, structural problems due to the pandemic and time-to-intervention in ES routine. Results 367 collaborators from 59 countries responded to the survey. The majority indicated that the pandemic still significantly impacts on treatment and outcome of surgical emergency patients (83.1% and 78.5%, respectively). As reasons, the collaborators reported decreased case load in ES (44.7%), but patients presenting with more prolonged and severe diseases, especially concerning perforated appendicitis (62.1%) and diverticulitis (57.5%). Otherwise, approximately 50% of the participants still observe a delay in time-to-intervention in ES compared with the situation before the pandemic. Relevant causes leading to enlarged time-to-intervention in ES during the pandemic are persistent problems with in-hospital logistics, lacks in medical staff as well as operating room and intensive care capacities during the pandemic. This leads not only to the need for triage or transferring of ES patients to other hospitals, reported by 64.0% and 48.8% of the collaborators, respectively, but also to paradigm shifts in treatment modalities to non-operative approaches reported by 67.3% of the participants, especially in uncomplicated appendicitis, cholecystitis and multiple-recurrent diverticulitis. Conclusions The SARS-CoV-2 pandemic still significantly impacts on care and outcome of patients in ES. Well-known problems with in-hospital logistics are not sufficiently resolved by now; however, medical staff shortages and reduced capacities have been dramatically aggravated over last two pandemic years.

Cardiac autonomic neuropathy is a serious microvascular complication of type 2 diabetes mellitus that affects a significant portion of patients. Due to decreased parasympathetic activity, the sympathetic nervous system becomes dominant, causing several problems that lead to increased cardiovascular morbidity and mortality. Sodium-glucose cotransporter-2 inhibitors have been shown to reduce sympathetic nervous system activity previously. This is a promising finding for restoring the impaired sympathovagal balance in cardiac autonomic neuropathy. The aim of this study is to evaluate the effect of at least 6 months of sodium-glucose cotransporter-2 inhibitor treatment on sympathetic nervous system activity with sympathetic activity index and heart rate variability parameters in patients with type 2 diabetes mellitus.Holter-electrocardiogram recordings of 50 patients who were using an sodiumglucose cotransporter-2 inhibitor (empagliflozin or dapagliflozin) for at least 6 months and 50 patients who did not were analyzed retrospectively. The sympathetic activity index and heart rate variability parameters of these 2 groups, which were similar in terms of age, gender, hemoglobin A1c, and duration of diabetes, were compared.The ratio of low-frequency to high-frequency power reflecting the sympathovagal balance [-1.495 (-2.165/-1.196) vs. -1.224 (-1.619/-0.863), P=.008] and sympathetic activity index [1.44 (1.06/2.76) vs. 2.47 (1.42/3.68), P=.009] was lower in the sodiumglucose cotransporter-2 inhibitor group than in the control group. In addition, the sympathetic activity index was correlated with the ratio of low-frequency to high-frequency power (r=0.418, P.001).Sodium-glucose cotransporter-2 inhibitor treatment for at least 6 months was found to result in lower values of sympathetic activity index and the ratio of low-frequency to high-frequency power in patients with type 2 diabetes mellitus. These findings indicate lower sympathetic nervous system activity, which supports the sympathoinhibitor effects of sodiumglucose cotransporter-2 inhibitors.

Covid-19 salgınının günlük yaşantımıza etkileri halihazırda devam ederken tüm dünyada olduğu gibi Türkiye’de de sosyal ve ekonomik saikler okulların açılmasını adeta bir zorunluluk haline getirmiştir. Türkiye’de okullar yüz yüze eğitim için yeni eğitim öğretim yılına hazırlıklarını tamamlamaya çalışmaktadır. Salgının yayılımı açısından en kritik kamusal alanlardan biri olan okullarda sürecin nasıl yürütüleceği ve okullar açıkken salgının ne derece kontrol altında tutulabileceği konusu toplumun her kesiminde merak ve endişe uyandırmaktadır. Bu minvalde elinizdeki bu çalışmanın amacı Covid-19 salgını sürecinde kapalı kalan okulların yeni eğitim öğretim dönemine ne kadar hazır olduklarının değerlendirilmesi ve bu bağlamda politika önerilerinin ortaya konmasıdır. Bu kapsamda ilk olarak okulların mevcut fiziksel koşullarının sağlık önlemleri açısından yeterliği ele alınmıştır. Okulların tam kapasite ile yüz yüze yeniden açılması sürecinde yöneticileri, öğretmenleri, öğrencileri ve velileri ilgilendiren mevcut ve muhtemel problemler değerlendirilmiş ve çözüm önerilerine değinilmiştir. Akabinde eğitim-öğretim planlama, müfredat ve içeriklere dair analizler ile rezilyans uygulamalarının etkililiğini artıracak tavsiyelere yer verilmiştir. Sonuç bölümünde politika uygulayıcıları ve eğitim paydaşları için uygulanabilir ve somut politika notları sunulmuştur

Background: Conventional methods for evaluation of block success are time consuming and need patient cooperation. The aim of this study was to evaluate whether the Perfusion Index and Plethysmographic Variability Index can be used to predict brachial nevre block success.Materials and Methods: Thirty-four patients, aged 18-65 years, who were transferred to elective or emergency surgery were included. Brachial plexus block was applied. Those with cardiac disease, chronic obstructive pulmonary disease and asthma were excluded from the study. Bedside, non-invasive, serial plethysmographic variability index (PVI) and perfusion index (PI) measurements of the patients were made using the Radical-97 Pulse Oximeter device from the fourth finger tip of the blocked hand.Results: When the PVI values measured from the blocked arm were compared; The difference between 0 and 5 minutes, 5th and 10th minutes and 0 to 10 minutes was statistically significant. When PI values were compared as 0 and 5 minutes, 5 and 10 minutes, 0 and 10 minutes, the changes were statistically significant. Preeclampsia increases the risk of cardiovascular mortality and morbidity both during pregnancy and long term after the labor by causing cardiac changes that may lead to atrial and ventricular arrhythmias.We aimed to investigate noninvasive predictors of atrial and ventricular arrhythmias and cardiac structural changes in preeclampsia.The study included 34 preeclampsia patients as the study group and 33 healthy pregnant women as the control group. The presence of fragmented QRS morphology, P dispersion, QT dispersion, Tp-e/QTc ratio, inter- and intra-atrial electromechanical delay, left ventricular mass index was evaluated in the groups by 12 lead- ECG and standard and tissue Doppler echocardiography.Left ventricular mass index and relative wall thickness, and E/Em ratio was significantly higher in preeclampsia. Inter- and intra-atrial electromechanical delay and Tp-e were prolonged, and P dispersion, QT dispersion, and Tp-e/QTc ratio were increased significantly in patients with preeclampsia. In addition, fragmented QRS morphology was seen in 76.5% of patients with preeclampsia while it was present in only 27.3% of the control group (Preeclampsia causes significant cardiac structural and electrocardiographic alterations that may increase the risk of atrial and/or ventricular arrhythmias. A more thorough and earlier cardiac assessment and closer follow-up of these patients would be useful to avoid further complications.

Background Obese non-alcoholic fatty liver disease (NAFLD) was found to increase the risk of developing atrial fibrillation (AF) regardless of the metabolic syndrome subgroups that may accompany it. In this study, the effect of NAFLD on the structural and electrical functions of the heart was investigated using tissue Doppler echocardiography (TDE) in non-obese NAFLD patients without any known risk factors for AF.Material and methods The study included 43 female patients (31.3±3.8 years), who had stage 2–3 hepatosteatosis detected by liver ultrasonography and diagnosed as non-obese NAFLD (patient group), and 31 healthy women (control group, 32.5±3.6 years). In addition to standard echocardiographic parameters, inter- and intra-atrial electromechanical delay (EMD) were evaluated by TDE.Results Interatrial EMD (PA lateral – PA tricuspid) and intraatrial EMD (PA septum – PA tricuspid) were significantly longer in patient group (16.1±3.4 vs. 12.5±2.3 ms, pConclusion Inter-atrial and intra-atrial EMD were detected in young women with non-obese NAFLD. In addition, at the subclinical level, structural and functional impairment was detected However, large-volume prospective studies are required to cobfirm these findings regarding the development of AF in non-obese NAFLD patients.

Sol koroner arterin pulmoner arterden (ALCAPA) anormal kökeni, nadir görülen doğuştan kalp anomalisidir. Genellikle klinik seyir; ileri sol kalp yetmezliği ve yaşamın ilk aylarında ortaya çıkan mitral kapak yetersizliğidir. Bununla birlikte, bazı durumlarda sağ koroner arterden oluşan kollateraller miyokard perfüzyonu için yeterlidir. ALCAPA'lı hastalarda aritmi veya erişkin yaşamda ani kalp ölümü ilk klinik başvuru olabilir. Göğüs ağrısı ve çarpıntı şikayetleri ile acil servise başvuran ve sol koroner arterde anormal köken saptanan 66 yaşında kadın hastayı sunmaktayız., Abnormal origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital heart anomaly. Usually the clinical course; advanced left heart failure and mitral valve insufficiency that occurs in the first months of life. However, in some cases collaterals consisting of the right coronary artery are sufficient for myocardial perfusion. Arrhythmia or sudden cardiac death in adult life may be the first clinical presentation in patients with ALCAPA. We present a 66-year-old female patient who was admitted to the emergency department with complaints of chest pain and palpitations and was found to have an abnormal origin in the left coronary artery.

Introduction:Obesity is usually related to insulin resistance and glucose metabolism disorders. The relationship between insulin resistance and epicardial adipose tissue and atrial electromechanical delay has been described in previous studies.Aim:This study aims to demonstrate the effects of metformin on epicardial adipose tissue and electromechanical delay in patients using metformin for insulin resistance.Materials and methods:A total of 30 patients using metformin for insulin resistance were included in the study. Pre-treatment and post-treatment epicardial adipose tissue and electromechanical delay were evaluated.Results:There was a statistically significant decrease in epicardial adipose tissue thickness after 3 months of metformin therapy (6.4 ± 2.1 versus 4.7 ± 2.0; p = 0.008). Furthermore, the inter-atrial and intra-atrial electromechanical delay also significantly decreased after 3 months of metformin monotherapy (23.6 ± 8.2 versus 18.1 ± 5.8; p < 0.001, 9.1 ± 2.9 versus 6.3 ± 3.6; p = 0.003, respectively).Conclusion:In this study, we show that metformin monotherapy significantly decreases epicardial adipose tissue thickness and electromechanical delay in obese children.

Bu olgu sunumunda 12 yaşında , Fenilketonüri (FKÜ) tanısı almış hastanın vitreo-retinal cerrahi sırasındaki anestezi yönetimini paylaşmak istedik. Ülkemizde akraba evliliğinin yüksek oranda olması nedeniyle FKÜ gibi otozomal resesif kalıtılan hastalıklar daha sık görülmektedir. Klasik FKÜ görülme sıklığı dünyada 1:10000 iken bu oran ülkemizde 1:6094’dir. Her yıl ülkemizde 250-300 çocuk bu hastalıkla doğmakta ve her 20-25 kişiden biri bu hastalığı taşımaktadır. Bölgemizde akraba evliliğinin yüksek sıklıkta olması ve FKÜ hastalarında maksilla hipoplazisi,mental retardasyon,mikrosefali ve B12 vitamin metabolizması bozukluğu gibi geniş perspektifte klinik özelliklerin olması nedeniyle genel anestezi tecrübemiz paylaşılmıştır. In this case report, we wanted to share the anesthesia management for vitreo-retinal surgery of a 12 year-old patient with Phenylketonuria (FKU).Autosomal recessive diseases like FKU are more common because of the high proportion of consanguineous marriages in our country . The incidence of classical FKU is 1: 10000 in the world while it is 1: 6094 in our country. Every year 250-300 children in our country are born with this disease and every 20-25 people carry this disease. Our general anesthesia experience has been shared because of the high frequency of consanguineous marriages in our region and the clinical features in a wide perspective such as maxillary hypoplasia, mental retardation, microcephaly and vitamin B12 metabolism disorder in FKU patients.

Background: The aim of this retrospective study was to evaluate the indications, characteristics and anesthesia management of epidermoid bullosa patients who underwent surgery in our hospital. Materials and Methods: In this study, records of this patient group in the hospital information system and anesthesia records were examined. Demographic data, preoperative characteristics, surgical indication, anesthesia technique, blood transfusion and complications were recorded. Results: A total of 19 patients were included in the study. Since some of the patients had multiple operations, 25 surgical data were used. All patients were operated due to contracture release and pseudo syndactyly. All patients underwent general anesthesia. No new postoperative lesions were observed. Conclusions: In this patient group, also known as butterfly disease, attention should be paid to the management of the patient perioperatively and be prepared for the complications that may occur in each stage of anesthesia. It should be kept in mind that even minor traumas in epidermoid bullosa patients may cause lesions that affect patients' lives. And our priority should be the first rule of medicine, “Primum Non Nocere”, Amaç: Bu retrospektif çalışmanın amacı, hastanemizde ameliyat olan epidermoid bülloza hastalarının ameliyat endikasyonlarını, özelliklerini ve anestezi yönetimini değerlendirmektir.Materyal ve metod: Bu çalışmada bu hasta grubunun hastane bilgi sistemindeki kayıtları ve anestezi kayıtları incelendi. Hastaların demografik verileri, preoperatif özellikleri, cerrahi endikasyonu, anestezi tekniği, kan transfüzyonu ve komplikasyonları kaydedildi.Bulgular: Toplam 19 hasta çalışmaya dahil edildi. Hastaların bir kısmı birden fazla defa ameliyat olduğundan 25 ameliyat verisi kullanıldı. Hastaların tamamının kontraktür açılması ve psödosindaktili nedeniyle opere olduğu görüldü. Hastaların tamamında genel anestezi uygulandı. Hiçbirinde postoperatif yeni lezyon gözlenmedi. Sonuç: Kelebek hastalığı olarak da bilinen bu hasta grubunda peroperatif hasta yönetimine dikkat edilmeli anestezinin her aşamasında oluşabilecek komplikasyonlara karşı hazırlıklı olunmalıdır. Epidermoid Bülloza hastalarında minör travmalarla bile hastaların hayatını etkileyen lezyonlar oluşabileceği hatırda tutulmalı. Ve önceliğimiz tıbbın ilk kuralı olan “önce zarar verme” olmalıdır.

Aim: Transcatheter aortic valve implantation has been used widely as an alternative treatment method for patients with symptomatic aortic stenosis carrying high surgical risk. Despite technological advances and increased experience, its early and late mortality rate is still relatively high and it is important to predict upcoming cardiac events and mortality for the management of patients. In this study, we aimed to investigate the clinical and laboratory parameters affecting mortality after these procedures performed in our clinic. Material and Methods: The patients who underwent TAVI due to severe AS between May 2015 and March 2019 were investigated retrospectively. Demographic, echocardiographic and laboratory data of all patients were recorded. During the follow-up period (mean: 556 days), patients were divided into 2 groups; deceased patients (group 1, n=13) and living patients (group 2, n=34). The data of the patients before TAVI procedure were compared between the two groups.Results: Among demograhic data, the ratio of chronic kidney disease was significantly higher in the deceased group. Among echo parameters, left ventricular ejection fraction was significantly lower and pulmonary artery pressure was significantly higher in the deceased group. Of laboratory parameters, blood urea nitrogen and creatinine levels were significantly higher and toral protein levels were significantly lower in the deceased group. Additionally, leukocyte, uric acid, CRP and CRP/albumin ratio were significantly higher and albumin was significantly lower in the deceased patients.Conclusion: The presence of chronic kidney disease, low left ventricular EF, high pulmonary artery pressure, high CRP levels, increased CRP/albumin ratio and high uric acid levels preceding TAVI may be helpful to determine the risk of mortality after the TAVI procedure.

Hayat boyu öğrenme ve hayat boyu öğrenme kavramına yönelik çeşitli uygulamalar, Avrupa Birliği tarafından ERASMUS+ kapsamında desteklenmekte; ülkelerin ulusal ajansları ve sivil toplum kuruluşları aracılığıyla projelendirilmektedir. Bu projelerden biri de Hayat Boyu Öğrenme: Media Sapiens – Medya aracılığıyla gelir artırma isimli projedir. Genel amaç olarak medya kanallarının kullanılarak gelir sağlama yollarının geliştirilmesini hedefleyen proje kapsamında 2-10 Kasım 2014 tarihileri arasında proje ortaklarının katıldığı 10. eğitim programı gerçekleştirilmiştir. Bu doğrultuda çalışmamızda adı geçen proje kapsamında gerçekleştirilen etkinlikler, kazanılan deneyimler ve bu deneyimlerin değerlendirilmesine çalışmanın sonuç bölümünde yer verilmektedir.

Background/aim In the literature, there is a paucity of data about the effects of fibromyalgia syndrome (FMS) on myocardial function and electrophysiological properties of atrium and ventricles. In this study, we investigated cardiac functions and noninvasive predictors of arrhythmias in patients with FMS. Materials and methods The study included 43 female patients diagnosed with FMS and 30 age- and sex-matched healthy subjects. The presence of fragmented QRS (fQRS) morphology, P dispersion, QT dispersion, inter- and intraatrial electromechanical delay was evaluated in the groups with 12-lead ECG and standard and tissue Doppler echocardiography. Results Among electrocardiographic parameters, P dispersion, QT dispersion, and the ratio of presence of fQRS morphology were found to be significantly higher in the study group as compared to the control group. In lateral and septal, the ratio of the early transmitral flow velocity to the early diastolic tissue velocity (E/Em) was significantly higher in the study group. Additionally, intra- and interatrial electromechanical delay was found significantly prolonged in the study group. Conclusion FMS is found to be associated with significant cardiac electrical alterations that may indicate the increased risk of atrial and ventricular arrhythmias in this group of patients.
