Abstract

Myocarditis and/or pericarditis (also known as myopericarditis) are inflammatory diseases involving the myocardium (with non-ischemic myocyte necrosis) and/or the pericardial sac. Myocarditis/pericarditis (MPC) may present with variable clinical signs, symptoms, etiologies and outcomes, including acute heart failure, sudden death, and chronic dilated cardiomyopathy. Possible undiagnosed and/or subclinical acute myocarditis, with undefined potential for delayed manifestations, presents further challenges for diagnosing an acute disease and may go undetected in the setting of infection as well as adverse drug/vaccine reactions. The most common causes of MPC are viral, with non-infectious, drug/vaccine associated hypersensitivity and/ or autoimmune causes being less well defined and with potentially different inflammatory mechanisms and treatment responses. Potential cardiac adverse events following immunization (AEFIs) encompass a larger scope of diagnoses such as triggering or exacerbating ischemic cardiac events, cardiomyopathy with potential heart failure, arrhythmias and sudden death. The current published experience does not support a potential causal association with vaccines based on epidemiologic evidence of relative risk increases compared with background unvaccinated incidence. The only evidence supporting a possible causal association of MPC with a vaccine comes from case reports. Hypersensitivity MPC as a drug/vaccine induced cardiac adverse event has long been a concern for postlicensure safety surveillance, as well as safety data submission for licensure. Other cardiac adverse events, such as dilated cardiomyopathy, were also defined in the CDC definitions for adverse events after smallpox vaccination in 2006. In addition, several groups have attempted to develop and improve the definition and adjudication of post-vaccination cardiovascular events. We developed the current case definitions for myocarditis and pericarditis as an AEFI building on experience and lessons learnt, as well as a comprehensive literature review. 38. Immunization to Protect the US Armed Forces: Heritage, Current Practice, and Prospects.

Abstract

Americans serving with the US Armed Forces need protection from the dangerous infections that they can contract during training, based on occupation, during overseas deployment, or because of underlying health status. For over 230 years, the military health-care system has immunized troops to protect them personally and to help them accomplish their missions. Military researchers have invented, developed, and improved vaccines and immunization delivery methods against more than 20 diseases. This article consolidates content from several previous historical reviews, adds additional sources, and cites primary literature regarding military contributions and accomplishments. Discussion emphasizes smallpox, typhoid fever, tetanus, influenza, meningococcal disease, adenovirus, yellow fever, pneumococcal disease, and anthrax. Delivery issues include documentation, simultaneous immunization, seroscreening, safety surveillance, jet injection, and cold-chain management. Immunization policies for each major US conflict are described. Military immunization programs need to be individualized on the basis of personal contraindications and prior immunity. The proper conduct of military immunization programs respects the need for detailed education of military personnel, maximizes quality in immunization delivery, and supports quality clinical care to prevent and treat adverse events after immunization. 39. Anaphylaxis Complicating Routine Childhood Immunization: Hemophilus Influenzab Conjugated Vaccine

Abstract

Increasing numbers of vaccines are being introduced into the infant and childhood immunization schedules. Immediate hypersensitivity reactions to include anaphylaxis can occur during the immunization visit. Determining the cause of a vaccine-associated immediate hypersensitivity reaction may require multiple specific skin test and challenge procedures. We report the case of a 4-month-old Caucasian male who developed an urticarial rash within 10 minutes of receiving his 4-month immunizations, diphtheria-tetanus-acellular pertussis (DTaP), inactivated polio (IPV), and the diphtheria mutant protein (CRM197) conjugated Hemophilus influenzaetype b (HbOC). The child was referred for an allergy-immunology consultation in order to determine the cause of the reaction and a future vaccine management strategy. Negative prick skin tests were obtained with full-strength vaccine and subsequent dose challenges with DTaP and IPV vaccines were tolerated without reaction. Prick skin testing with full-strength HbOC was negative, but dose challenge with 0.05 mL resulted in an anaphylactic reaction within 10 minutes of the first challenge dose. The reaction included generalized hives, nasal congestion and diffuse wheezing. Subsequent intradermal skin testing with four different Hemophilus influenzaetype b (Hib) vaccines at 1:100 dilution revealed isolated skin test reactivity to HbOC but not the other protein conjugated Hib vaccines. Skin testing with CRM197alone (the protein conjugate in HbOC) was positive, confirming the source of the reaction. The patient tolerated subsequent Hib immunization with a CRM-free preparation without complications. Our case report suggests that the mutant diphtheria protein CRM197may be an allergenic protein in some patients who develop anaphylaxis to the HbOC vaccine. The availability of vaccines with different protein conjugates provides an alternative vaccination strategy for those patients with specific sensitization. The question of cross-reactions and risk for anaphylaxis with other vaccines containing the CRM197remains to be determined. The new pneumococcal vaccine (Prevnar™) contains CRM197.
