A koronavírus-betegség megjelenése óta világszerte nyomon követik a fertőzésekhez kapcsolódó közvetlen halálozást, és naponta számolják a vírus okozta halálesetek számát. A koronavírus okozta világjárvány nemcsak mindennapi életünket, de a teljes egészségügyi rendszer működését átrendezte. A kórházi felvételre való fokozott igény kapcsán a különböző országok vezetői számos, az egészségügyi ellátást befolyásoló rendkívüli intézkedést vezettek be. Az átrendeződés mind direkt, mind indirekt módon negatív hatással volt a hirtelen szívhalál epidemiológiai mutatóira, a laikus elsősegélynyújtók cardiopulmonalis resuscitatióra való hajlandóságára és a félautomata defibrillátor használatára, ezek a negatív hatások azonban széles határok között mozognak a különböző kontinenseken és országokban. A laikusok és az egészségügyi dolgozók védelme, illetve a pandémia terjedésének megakadályozása érdekében a korábbi alapszintű és emelt szintű újraélesztésre vonatkozó, az Európai Újraélesztési Tanács által megfogalmazott ajánlások számos változtatáson estek át. Az extracorporalis cardiopulmonalis resuscitatio alkalmazásának gyakorisága egyre nő mind a kórházon belüli, mind a kórházon kívüli keringésmegállás ellátásában. A legújabb újraélesztési irányelvek is egyre inkább beépítik javaslataik közé az extracorporalis keringéstámogató eszközök használatát elhúzódó újraélesztés esetén, bizonyos szelektált betegcsoportokban. Mindezek ellenére kevés bizonyíték áll rendelkezésünkre a beavatkozás hatékonyságáról, és még számos nyitott kérdés maradt megválaszolatlanul azt illetően, hogy milyen körülmények között, mely betegcsoportokban van a leginkább létjogosultsága ennek a modalitásnak. Fontos kérdés a terápia időzítése és helyszíne, illetve lényeges az extracorporalis technikákat alkalmazó személyzet megfelelő kiképzése. Összefoglalónkban röviden áttekintjük, hogy a jelenlegi szakirodalom és ajánlások szerint milyen esetekben merülhet fel az extracorporalis újraélesztés megkezdése, milyen típusú mechanikus keringéstámogatás az elsőként választandó az újraélesztés alatt, milyen tényezőket gondoljunk át alkalmazása során, illetve milyen szövődményekre számítsunk. A kórházon belüli keringésmegállás túlélési esélye még mindig csak 15–34% körüli. A kórházi személyzet elengedhetetlen feladata, hogy a betegek vitális paramétereinek monitorozásával a beteg állapotának romló trendjét észleljék, és megfelelő beavatkozásokkal a keringésmegállást megelőzzék. A betegek állapotának rosszabbodására utaló jelek, tünetek (például légzésszám, pulzusszám, vérnyomás, oxigénszaturáció, tudat stb. változása) összegzéséből képzett pontrendszereknek és azok protokollokban megfogalmazott változatának használata segíthet abban, hogy a „periarrest” állapotú betegek időben felismerhetők legyenek. Ha azonban a keringésmegállás mégis bekövetkezik, a kórházon belüli reanimáció szabályait követve, csapatmunkában dolgozva az ellátószemélyzetnek képesnek kell lennie a jó minőségű mellkasi kompressziók és a korai defibrillálás biztosítására. Ehhez rendszeres képzésre, megfelelő infrastruktúrára, valamint rendszerszintű csapatmunkára van szükség. A jelen összefoglaló röviden bemutatja a kórházi újraélesztés kezdeti szakaszának kihívásait és annak integrálását a kórházon belüli sürgősségi ellátás komplex rendszerébe. A kórházon kívüli váratlan keringésmegállások túlélési aránya egész Európában alacsony. Az utóbbi évtizedben fény derült arra, hogy a túlélés javításának legfőbb kulcsa a keringésmegállás pillanatában jelen lévő laikusok bevonása az ellátásba. A laikus segélynyújtók a korai felismerésen és a korai mellkaskompresszión túl a korai defibrillálás elvégzésébe is bevonhatók. A felnőttek alapszintű újraélesztése egyszerű és könnyen elsajátítható készségek sorozatából áll, amely akár kisiskoláskortól tanítható. A gyakorlat azonban azt mutatja, hogy éles helyzetben az újraélesztés megkezdését a készségek felejtése mellett a nem technikai készségek – például helyzetfelismerés, gyors döntéshozatal, kommunikáció – hiánya és emocionális tényezők is erősen befolyásolják. Ennek felismerése és a technikai vívmányok használata új szemléletet hozott mind az oktatásban, mind az ismeretek gyakorlatba való átültetése terén. A jelen összefoglaló a kórházon kívüli felnőtt alapszintű újraélesztés aktuális szakmai irányelvét és az újraélesztés oktatásával kapcsolatos szakmai újdonságokat (ezen belül a nem technikai készségek jelentőségét) tekinti át, kitérve a COVID–19-járvány hatására is. A fejlett országokban a hirtelen szívhalál napjainkban is a vezető kardiovaszkuláris halálokok közé tartozik. Európában gyakorisága évente mintegy 84/100 000 fő. Az elmúlt évtizedekben számos rizikófaktort azonosítottak, amelyeknek ismeretével a prognózis javítható. Írásunkban a kórházon belül (IHCA), illetve a kórházon kívül (OHCA) kialakult hirtelen szívhalál incidenciáját, major prognosztikai faktorait és a túlélést javító lehetséges eszközöket mutatjuk be. Ezentúl összegeztük a 2021-es új irányelveket az alap- és emeltszintű újraélesztési protokollokat illetően.

Tanulmányunk vidéki kontextusban, a Székelyföld (Románia) példáján elemzi azt a kérdést, hogy hogyan és milyen mértékben mutathatók ki az új gazdaság (new economy) jelenségének komponensei az újszerű kisvállalkozásokban. A 2019-2020 között végzett kutatási programunkban 30 interjút - 17 nő és 13 férfi, 25-35 évesek - elemeztünk, amelyeket innovatív vállalkozások alapítóival és vezetőivel készítettünk. Témánk fontosságát az adja, hogy a vizsgált terület alapvetően vidéki, gazdaságilag marginális és hátrányos helyzetű Románia legtöbb régiójához képest. Az innováció lényege ebben az összefüggésben nem a csúcstechnológia alkalmazása, hanem a térségi társadalmi-gazdasági kontextushoz viszonyítva tapasztalt újszerűség. A tanulmány a szakirodalmi szempontok bemutatása után elemzi az új gazdaság jellemzőinek 4 összetevőjét (1. A habitus elemeinek beépítése, 2. a személyes meggyőződések és értékek szerepe, 3. a reflexív magatartás szerepe, 4. a társadalmi közeg szerepe a vállalkozás fejlesztésében és működtetésében), amelyek a vállalkozók személyes jellemzőinek előtérbe helyezésével változást hoznak a térségi vállalkozói gyakorlatban.

A gyermekek médiafogyasztási szokásai az elmúlt években jelentősen megváltoztak. A lineáris médiaszolgáltatások helyett egyre hangsúlyosabb szerepet kapnak a lekérhető médiaszolgáltatások, valamint a videómegosztó platformokon közzétett tartalmak. A kereskedelmi közleményekkel kapcsolatos, lineáris és lekérhető médiaszolgáltatásokra vonatkozó uniós szabályozási minimumokat egységesítette az audiovizuális médiaszolgáltatásokról szóló irányelv (AVMS-irányelv) 2018. évi módosítása. Az audiovizuális médiaszolgáltatások egységes európai piacának kielégítő működése mellett szükséges a gyermekvédelem megfelelő szintjének biztosítása, beleértve a gyermekek kereskedelmi közleményekkel szembeni védelmét is. A módosított AVMS-irányelv tárgyi hatályának a videómegosztóplatform-szolgáltatásokra történő kiterjesztésével az Európai Unió reagált a megváltozott médiafogyasztási szokásokra és a médiakonvergenciára, továbbá előirányozta a kiskorúak fokozott védelmét, beleértve a platformokon megjelenő kereskedelmi közleményeket és az ezeket tartalmazó felhasználók által közzétett tartalmakat is.

Összefoglaló. A koronavírus-betegség (COVID–19) okozta közvetlen mortalitáson túl, a járvány közvetett úton is hatással lehet a hirtelen szívhalálra. Egyre növekvő számú közlemény foglalkozik a járványnak a hirtelen szívhalálra kifejtett közvetett hatásával. A kijárási korlátozások és az egészségügyi rendszerek átszervezése hozzájárulhatott ahhoz, hogy a járvány alatt mind a kórházon kívüli, mind a kórházon belüli szívhalál előfordulása megemelkedett. Közegészségügyi intézkedések, mint a korlátozások és a kórházak átszervezése, megváltoztathatják az egészségügyi szolgáltatásokhoz való hozzáférést, ezért hozzájárulhattak az elmúlt évben tapasztalt emelkedett számú szívmegálláshoz. Közleményünk célja a SARS-CoV-2-járvány hirtelen szívhalálra kifejtett hatására vonatkozó, a nemzetközi irodalomban jelenleg megtalálható tanulmányok összefoglalása, melyek a kórházon kívüli szívmegállás előfordulásának háromszoros emelkedéséről számoltak be a járványt megelőző évhez képest. Általánosságban elmondható, hogy a kórházon kívüli szívmegállás a járvány ideje alatt nagyobb gyakorisággal járt nem sokkolandó ritmussal, hosszabb idő telt el a mentők kiérkezéséig, alacsonyabb volt a szemtanú által megkezdett újraélesztés, a spontán keringés visszatérésének, valamint a kórházi elbocsátásnak a gyakorisága. A járványnak a kórházon belüli szívmegállásra kifejtett hatása kevésbé vizsgált az irodalomban. Míg a hirtelen szívhalált követő mortalitás néhány kutatásban jelentős emelkedést mutatott, addig máshol nem volt különbség a járványt megelőző időszakhoz képest. A COVID–19-pandémia ideje alatt jelentősen megnövekedett kórházon kívüli és belüli szívmegállás hátterében a járványnak közvetett úton is szerepe lehet, a fertőzés közvetlen hatása mellett. However, there are a growing number of publications suggesting that the prevalence and outcome of sudden cardiac death may also be indirectly affected by the pandemic. Public health measures, such as lockdowns and reorganisation of hospitals, can alter the access to healthcare services and therefore might have contributed to the excess number of cardiac arrests which were seen over the last year. Our aim was to review the currently available publications regarding the impact of the COVID-19 pandemic on out-of-hospital and in-hospital cardiac arrests. A recent study reported a 3-fold growth in the incidence of out-of-hospital cardiac arrests during the 2020 COVID-19 period compared to the year before. In general, the number of non-shockable rhythms increased, bystander-witnessed cases and bystander-initiated cardiopulmonary resuscitation were reduced and ambulance response times were significantly delayed during the pandemic. Return of spontaneous circulation and survival to discharge substantially decreased compared to the time before the pandemic. The difference between the rate of mortality following in-hospital cardiac arrest during and before the pandemic is controversial according to published data. The incidence of out-of-hospital and in-hospital cardiac arrests significantly increased during the pandemic compared to previous years suggesting direct effects of COVID-19 infection and indirect effects from new public health measures. Összefoglaló. Az áramütés súlyos esetben hirtelen halállal vagy több szervrendszer kiterjedt károsodásával járhat. A magasfeszültségű áramütés (>1000 V) általában súlyosabb égési sérülésekkel és magasabb kórházi mortalitással jár, mint az alacsonyfeszültségű, de a sérülések súlyosságát a feszültségen kívül a test ellenállása, az áramexpozíció ideje, az áram fajtája, erőssége és útja is befolyásolja. A kritikus állapotú vagy súlyos égési sérüléseket szenvedett betegek sürgősségi ellátása komplex és multidiszciplináris szemléletet igényel. A súlyos szövődményekkel járó áramütéses balesetek ugyanakkor a fejlett országokban ritkák: az áramütés következtében sürgősségi osztályon jelentkező betegek döntő többsége panaszmentesen vagy minor panaszokkal kerül felvételre. A ritmuszavarok az áramütéses balesetek messze leggyakoribb cardialis szövődményei, és rendszerint közvetlenül az áramütés után jelentkeznek. Az elektromos áram kamrafibrillációt vagy asystoliát is kiválthat, mely a baleset helyszínén ellátás nélkül halálhoz vezethet. Bár sok helyen elterjedt gyakorlat az áramütést szenvedett betegek rutinszerű monitorozása, a klinikailag releváns arrhythmiák összességében ritkák, és a felvételi EKG alapján diagnosztizálhatók, ezért EKG-monitorozás csak meghatározott rizikófaktorok esetén szükséges. Jelen munkánk célja összefoglalni az áramütést szenvedett betegek optimális sürgősségi ellátásával kapcsolatos legfontosabb szempontokat, különös tekintettel az áramütéses balesetet követően fellépő cardialis szövődményekre és arrhythmiákra, valamint az EKG-monitorozás indikációira. Orv Hetil. 2020; 161(47): 1979–1988. Summary. Electrical accidents (EA) may cause sudden death or severe injuries of multiple organs. High voltage injuries (>1000 V) are associated with more severe burn injuries and higher in-hospital mortality than low voltage injuries, however, the severity of complications depends on several other factors like resistance of the body, duration of current exposition, intensity, type and pathway of current. Critically ill patients with severe burns and/or other injuries require a multidisciplinary intensive treatment. However, such complications are rare in the developed countries: most patients present in the emergency department with no or minor symptoms and do not require hospital admission. Arrhythmias are the most frequent cardiac complications after EA. Electrical current may cause ventricular fibrillation or asystolia which can lead to death on the scene. In patients presenting in the emergency department, clinically relevant arrhythmias are rare and can be diagnosed by a 12-lead ECG, therefore a systematic monitoring may not be indicated. Aim of our work is to review the most frequent complications after an electrical accident with special focus on cardiac complications and arrhythmias. The complex, multi-jurisdictional regulation of child protection in Hungary, coordinating the activities of the competent bodies in this field, emerged during the period of the Austro-Hungarian Monarchy (1867–1918). Legislation reflected the change in the way childhood were perceived, recognising that children need additional protection in accordance with their age-related physical and psychological characteristics. In the narrower framework of child protection, administrative child protection dealt with children who had been declared abandoned by the authorities, while judicial child protection dealt with juveniles and children who had been debauched. The two areas were in some ways linked, the legislator’s aim was to ensure cooperation between the public administration and the justice system for the protection of children.

Abstract: Post-resuscitation brain injury forms the leading cause of death of patients after successful cardiopulmonary resuscitation that explains why post-resuscitation neuroprotection is the most important part of post-resuscitation therapy. The goals of the neuroprotection tools available today are preventing the evolution of primary and formation of secondary brain injury. We are going to summarize the neuroprotective possibilities that we can reach today. We will discuss the role of pharmacologic strategies including sedation, the modalities of upholding brain perfusion, the monitoring of proper hemodynamic variables and the practice of targeted temperature management. It is very important to avoid hypo- and hyperoxia, to keep normocapnia, normoglycemia and to control seizures during the management of post-cardiac arrest patients. There is still a lack of evidence to prove which pharmacologic agent may be effective in postresuscitation neuroprotection, however, there are some promising results regarding thiamine. Hemodynamic management guided by higher level hemodynamic monitoring may be beneficial in enhancing brain perfusion but more clinical studies are needed to investigate its usefulness. Targeted temperature management constitutes the main element of post-resuscitation neuroprotection, however, the details of its implementation raise several questions. Összefoglaló. A landiolol intravénásan alkalmazandó, kifejezetten cardioselectiv, gyors hatású és rövid felezési idejű béta-1-receptor-blokkoló, mely elsősorban negatív chronotrop és inotrop hatással bír, vérnyomáscsökkentő hatása elhanyagolható. Főleg hemodinamikailag instabil állapotú, supraventricularis ritmuszavarban szenvedő betegek kamrafrekvenciájának csökkentésére használható. Nagy esetszámú, randomizált vizsgálatok igazolták hatékonyságát szívműtétek után jelentkező pitvarfibrilláció megelőzésében, valamint súlyos akut szívelégtelenségben és szívműtétek posztoperatív szakában jelentkező pitvari tachyarrhythmiák kezelésében. Ezek mellett kisebb vizsgálatok alapján a használata biztonságosnak tűnik akut myocardialis infarctusban, hatékony szeptikus állapotú, pitvarfibrillációban szenvedő betegek kamrafrekvencia- és ritmuskontrolljára, valamint nem cardialis műtétek esetén a pitvarfibrilláció prevenciójára és kezelésére. Sikerrel alkalmazható elektromos vihar esetén is, és jól használható angiográfiás coronaria-CT-vizsgálat előtt az optimális szívfrekvencia elérésére. A gyógyszer 2016 óta Európában, 2018 óta Magyarországon is elérhető. Orv Hetil. 2022; 163(2): 53-62. Summary. Landiolol is an intravenous, selective beta-1-receptor blocking agent with rapid onset of action and ultra-short half-life that has a predominant negative chronotropic and only mild negative inotropic effect without significant reduction of blood pressure. Landiolol is indicated to control the ventricular heart rate predominantly in patients with hemodynamic instability due to supraventricular tachyarrhythmia. Large randomized controlled trials have proven the efficacy of landiolol in the prevention of atrial fibrillation and atrial tachyarrhythmias in severe acute heart failure or post-cardiac surgery. Based on lower case-number studies, the administration of landiolol has been proven to be efficient and safe in rhythm and rate control in atrial fibrillation complicating acute myocardial infarction, sepsis, and in the prevention of atrial fibrillation in non-cardiac surgery. Landiolol may be used in electrical storm, and even during coronary CT-angiography to achieve an optimal heart rate for imaging. The drug is available in Europe since 2016 and in Hungary since 2018. Proper hemodynamic management is necessary among post-cardiac arrest patients to improve survival. We aimed to investigate the effects of PiCCO™-guided (pulse index contour cardiac output) hemodynamic management on mortality in post-resuscitation therapy. In this longitudinal analysis of 63 comatose patients after successful cardiopulmonary resuscitation cooled to 32–34 °C, 33 patients received PiCCO™, and 30 were not monitored with PiCCO™. Primary and secondary outcomes were 30 day and 1 year mortality. Kaplan–Meier curves and log-rank tests were used to assess differences in mortality among the groups. Interaction effects to disentangle the relationship between patient’s condition, PiCCO™ application, and mortality were assessed by means of Chi-square tests and logistic regression models. A 30 day mortality was significantly higher among PiCCO™ patients, while 1 year mortality was marginally higher. More severe patient condition per se was not the cause of higher mortality rate in the PiCCO™ group. Patients in better health conditions (without ST-elevation myocardial infarction, without cardiogenic shock, without intra-aortic balloon pump device, or without stroke in prior history) had worse outcomes with PiCCO™-guided therapy. Catecholamine administration worsened both 30 day and 1 year mortality among all patients. Our analysis showed that there was a complex interaction relationship between PiCCO™-guided therapy, patients’ condition, and 30 day mortality for most conditions.

Összefoglaló. Bevezetés: A cardiovascularis halálokok közül világszerte nagy jelentőségű a hirtelen szívhalál. Annak ellenére, hogy a cardiopulmonalis resuscitatio és a postresuscitatiós intenzív osztályos kezelés is komoly metodikai és technikai fejlődésen ment keresztül az elmúlt időszakban, kevés az olyan validált pontrendszer, amely jól becsülné a beteg intenzív osztályra kerülésekor a mortalitási rizikót. Célkitűzés: A sikeres újraélesztést követő intenzív osztályos kezelés kezdetekor felmért, a cardiogen shock rizikóstratifikációjára alkalmazott CardShock Risk Score (CSRS) és az általunk hozzáadott, specifikus súlyozófaktorokkal (iniciális ritmus, inotropigény) módosított CardShock Risk Score (mCSRS) összevetése a mortalitás előrejelzésében post-cardiac arrest szindrómás betegeknél. Módszerek: Retrospektív vizsgálatunk során 172, kórházon kívül sikeresen újraélesztett és klinikánkon ellátott consecutiv betegből a CSRS- és mCSRS-pontrendszerek segítségével végül 123 beteg adatait elemeztük. A CSRS- és mCSRS-változók és a korai/késői mortalitás közötti összefüggést Cox-regressziós analízissel vizsgáltuk. A pontszámok alapján 3 csoportba (1–3, 4–6, 7+) soroltuk a betegeket. Az összevont csoportok túlélését log-rank teszttel hasonlítottuk össze. Eredmények: A betegpopuláció átlagéletkora 63,6 év volt (69% férfi), és a hirtelen szívhalál hátterében 80%-ban akut coronaria szindróma állt. A korai/késői mortalitást leginkább a felvétel utáni neurológiai állapot, a szérumlaktátszint, a vesefunkció, az iniciális ritmus és a beteg katecholaminigénye határozta meg. A mCSRS alkalmazását követően mind az „1–3” és a „4–6” (p≤0,001), mind a „4–6” és a „7+” (p = 0,006) csoportok között szignifikáns különbséget találtunk a túlélésben. Although there have been immense methodological and technical advances in the field of cardiopulmonary resuscitation and following intensive care in the last decade, currently there are only a few validated risk-stratification scoring systems for the quick and reliable estimation of the mortality risk of these patients at the time of admission to the intensive care unit. Objective: Our aim was to correlate the mortality prediction risk points calculated by CardShock Risk Score (CSRS) and modified (m) CSRS based on the admission data of the post-cardiac arrest syndrome (PCAS) patients. Methods: The medical records of 172 out-of-hospital resuscitated cardiac arrest patients, who were admitted at the Heart and Vascular Centre of Semmelweis University, were screened retrospectively. Out of the 172 selected patients, 123 were eligible for inclusion to calculate CSRS and mCSRS. Based on CSRS score, we generated three different groups of patients, with scores 1 to 3, 4 to 6, and 7+, respectively. Mortality data of the groups were compared by log-rank test. Results: Mean age of the patients was 63.6 years (69% male), the cause of sudden cardiac death was acut coronary syndrome in 80% of the cases. The early and late mortality was predicted by neurological status, serum lactate level, renal function, initial rhythm, and the need of catecholamines. Using mCSRS, a significant survival difference was proven in between the groups “1–3” vs “4–6” (p≤0.001), “4–6” vs “7+” (p = 0.006). Conclusion: Compared to the CSRS, the mCSRS expanded with the 2 additional weighting points differentiates more specifically the low-moderate and high survival groups in the PCAS patient population treated in our institute. Background: The coronavirus disease (COVID-19) brought several challenges in medical education. The aim of our study was to investigate whether virtual trainings (VT) organized during the COVID-19 pandemic at our university were effective in replacing in-person bed-side education in intensive therapy and anaesthesiology among fifth-year medical students, both from students' and instructors' perspectives. Methods: This was a cross-sectional study consisting of three parts: a 20-item students' questionnaire filled out by students participating in VT, a 22-item instructors' questionnaire filled out by instructors taking part in virtual education and a 20-item knowledge test completed by students participating in VT, as well as by students visiting bed-side trainings (BT) during the same semester, before COVID-19 pandemic. The questionnaires focused on effectiveness, content, self-preparedness, technical background and interactivity of VT. Instructors' and students' responses given to the common questions, as well as the knowledge test results were compared. Mann-Whitney U test was used for group comparisons and binary logistic regression was performed to analyse the influence of previous health-care experience on students' feeling of self-preparedness. Results: 113 students (68% response rate) and 29 instructors (97% response rate) filled out the questionnaires. The majority of students found our VT useful and effective; however, a considerable number of participants felt disadvantaged by taking a virtual course instead of bed-side learning sessions and would recommend to keep virtual distance learning methods combined with BT. Instructors found VT overall effective and deemed the transfer of their knowledge satisfactory; however, they described worse interactivity and contact with students during virtual sessions compared to in-person teaching. Instructors showed a clearer consensus that VT should not replace BT in the future, while students’ answers were more divided in this regard. Previous health-care experience did not influence students' feeling of self-preparedness. One hundred and twenty-seven students (56 after VT and 71 after BT) completed the end-of-semester knowledge test. Students participating in VT performed better than students visiting BT (median score VT:83.5 vs BT:77.3; p=0.015). Conclusions: The new curriculum incorporating virtual practice sessions was effective in maintaining continuous education of intensive therapy and anaesthesiology among fifth-year medical students during the COVID-19 outbreak.

This systematic review and meta‐analysis aimed to investigate the association between maternal overnutrition and offspring's insulin sensitivity—following the Preferred Reporting Items for Systematic Reviews and Meta‐analyses statement. Studies published in English before April 22, 2019, were identified through searches of four medical databases. After selection, 15 studies aiming to explore the association between prepregnancy body mass index (ppBMI) or gestational weight gain (GWG) of non‐diabetic mothers and their offspring's insulin sensitivity (fasting insulin or glucose level and Homeostatic Measurement Assessment for Insulin Resistance [HOMA‐IR]) were included in the meta‐analysis. Associations of ppBMI and GWG with offspring's insulin sensitivity were analysed by pooling regression coefficients or standardized differences in means with 95% confidence intervals (CIs). Maternal ppBMI showed significant positive correlations with the level of both fasting insulin and HOMA‐IR in offspring (standardized regression coefficient for fasting insulin: 0.107, CI [0.053, 0.160], p < 0.001 and that for HOMA‐IR: 0.063, CI [0.006, 0.121], p = 0.031). However, the result of the analysis on coefficients adjusted for offspring's actual anthropometry (BMI and adiposity) was not significant. Independent from ppBMI, GWG tended to show a positive correlation with insulin level, but not after adjustment for offspring's anthropometry. Offspring of mothers with excessive GWG showed significantly higher HOMA‐IR than those of mothers with optimal GWG (p = 0.004). Our results demonstrate that both higher ppBMI and GWG increase the risk of offspring's insulin resistance, but the effect of ppBMI on insulin sensitivity in offspring may develop as consequence of their adiposity.

Annak ellenére, hogy a cardiopulmonalis resuscitatio és a postresuscitatiós intenzív osztályos kezelés is komoly metodikai és technikai fejlődésen ment keresztül az elmúlt időszakban, kevés az olyan validált pontrendszer, amely jól becsülné a beteg intenzív osztályra kerülésekor a mortalitási rizikót. Célkitűzés: A sikeres újraélesztést követő intenzív osztályos kezelés kezdetekor felmért, a cardiogen shock rizikóstratifikációjára alkalmazott CardShock Risk Score (CSRS) és az általunk hozzáadott, specifikus súlyozófaktorokkal (iniciális ritmus, inotropigény) módosított CardShock Risk Score (mCSRS) összevetése a mortalitás előrejelzésében post-cardiac arrest szindrómás betegeknél. Dogs (Canis familiaris) are excellent models of human behavior as during domestication they have adapted to the same environment as humans. There have been many comparative studies on dog behavior; however, several easily measurable and analyzable psychophysiological variables that are widely used in humans are still largely unexplored in dogs. One such measure is rapid eye movement density (REMD) during REM sleep. The aim of this study was to test the viability of measuring REMD in dogs and to explore the relationship between the REMD and different variables (sex, age, body size, and REM sleep duration). Fifty family dogs of different breeds and ages (from 6 months to 15 years old) participated in a 3-h non-invasive polysomnography recording, and the data for 31 of them could be analyzed. The signal of the electro-oculogram (EOG) was used to detect the rapid eye movements during REM sleep, and REMD was calculated based on these data. The duration of REM sleep had a quadratic effect on REMD. Subjects' REMD increased with age, but only in male dogs with short REM sleep duration. Furthermore, in the case of dogs with short REM sleep, the interaction of body mass and REM sleep duration had a significant effect on REMD. No such effects were found in dogs with long REM duration. These results suggest that relationships may exist between REMD and several different variables.

Although a positive link between sleep spindle occurrence and measures of post-sleep recall (learning success) is often reported for humans and replicated across species, the test–retest reliability of the effect is sometimes questioned. The largest to date study could not confirm the association, however methods for automatic spindle detection diverge in their estimates and vary between studies. Here we report that in dogs using the same detection method across different learning tasks is associated with observing a positive association between sleep spindle density (spindles/minute) and learning success. Our results suggest that reducing measurement error by averaging across measurements of density and learning can increase the visibility of this effect, implying that trait density (estimated through averaged occurrence) is a more reliable predictor of cognitive performance than estimates based on single measures.

Background Proper basic life support (BLS) is key in improving the survival of out-of-hospital cardiac arrest. BLS skills deteriorate in three to 6 months after training. One method to improve skill retention may be using the “testing effect” to test skills at the end of a BLS course. The aim of our study was to investigate whether either testing or the timing of such testing after BLS training have any influence on skill retention. Methods This was a post-test only, partial coverage, prospective quasi-experimental study designed to evaluate a BLS training course among 464 fifth year medical students at Semmelweis University in the first semester of 2013/2014. Groups were systematically but non-randomly assigned to either a control group that took no exam or one of two experimental groups that took an exam (N = 179, NoExam group; N = 165, EndExam group – exam at the end of the BLS training; N = 120, 3mExam group – exam 3 months after the BLS training). The ability to perform ten prescribed essential BLS steps was evaluated during a skill retention assessment 2 months after the course in the NoExam, 2 months after the course (and the exam) in the EndExam and 5 months after the course (2 months after the exam) in the 3mExam group to measure skill retention and the effect of our intervention. Scores were calculated for each BLS step, and also summed up as a total score. We used Kruskal-Wallis test to assess differences in skill retention. Results Overall, NoExam and EndExam groups showed similar skill retention. The mean total score (and many of the sub-scores) of students was significantly higher in the 3mExam group compared to both the NoExam and the EndExam groups, and there was no difference in the total score (and many of the sub-scores) of the latter two groups. The 3mExam group had less variability in total scores (and many of the sub-scores) than the other two groups. Conclusion Our study provides evidence that testing these skills 3 months after BLS training may be more effective than either testing immediately at the end of the course or no testing at all.

Post-resuscitation brain injury forms the leading cause of death of patients after successful cardiopulmonary resuscitation that explains why post-resuscitation neuroprotection is the most important part of post-resuscitation therapy. The goals of the neuroprotection tools available today are preventing the evolution of primary and formation of secondary brain injury. We are going to summarize the neuroprotective possibilities that we can reach today. We will discuss the role of pharmacologic strategies including sedation, the modalities of upholding brain perfusion, the monitoring of proper hemodynamic variables and the practice of targeted temperature management. It is very important to avoid hypo- and hyperoxia, to keep normocapnia, normoglycemia and to control seizures during the management of post-cardiac arrest patients. There is still a lack of evidence to prove which pharmacologic agent may be effective in postresuscitation neuroprotection, however, there are some promising results regarding thiamine. Hemodynamic management guided by higher level hemodynamic monitoring may be beneficial in enhancing brain perfusion but more clinical studies are needed to investigate its usefulness. Targeted temperature management constitutes the main element of post-resuscitation neuroprotection, however, the details of its implementation raise several questions. A tudomány mai állása szerint a jelenleg elérhető neuroprotekciós módszerek célja, hogy megelőzzék a primer agykárosodás kiterjedését és a szekunder agykárosodás kialakulását. Közleményünkben részletezzük a ma rendelkezésre álló neuroprotekciós lehetőségeket, külön kitérve az egyes farmakológiai ágensek és ezen belül a szedáció szerepére, az agyi perfúzió fenntartásának lehetőségeire, a megfelelő hemodinamikai paraméterek monitorozására és kiválasztására, valamint a célhőmérséklet-orientált kezelésre. A postresuscitatiós betegek ellátása során fontos, hogy kerüljük a hypoxiát és hyperoxiát, normocapniára és normoglykaemiára törekedjünk, valamint a görcstevékenységet azonnal uraljuk. Egyelőre nincs még egyértelmű bizonyíték arra vonatkozóan, hogy melyik gyógyszer alkalmazása segíthet a postresuscitatiós neuroprotekcióban. A tiamin ez irányú vizsgálata során biztató eredmények születtek. Az agyi perfúzió javítása céljából fontos lehet a magasabb szintű hemodinamikai monitorozás és az általa vezérelt terápia, de ennek bizonyítása szintén további tanulmányok elvégzését igényli. European climate change objectives aim to reduce CO2 emissions, promote the spread of renewable energy sources (RES), and stimulate energy efficiency (EE). The situation of local and regional governance is crucial in the implementation of community objectives. A multi-year research project was implemented in East Hungary by the University of Debrecen and Eszterh&aacute, zy University. It focuses on the relevance of local communities in the formation of adequate responses to the fore-mentioned challenges. A broad range of primary data collections (questionnaires and interviews) were implemented in two rural counties of the country. The first objective was to elicit existing plans concerning RES and EE at the municipality level and to measure the rate of public participation in the elaboration of them. Secondly, the scopes of the implemented projects were examined in the target area. According to the results, local leaders seem to be committed to environmental issues, but communities rarely possess energy/climate-change plans and projects. The main motivators of project implementation are financial. Because of poor financial conditions, only EU co-financed projects are realized. As subsidies come through the national government, political factors are crucial. From a governance point of view, national and county levels are decisive, and local specialties are not taken into consideration.

In the last fifteen years mild therapeutic hypothermia became an accepted and widespread therapeutic method in the treatment of successfully resuscitated patients due to sudden cardiac death. Based on the available evidence therapeutic hypothermia is part of the resuscitation guidelines, however, many aspects of its therapeutic use are based on empirical facts. In particular, the subjects of intense debate are the ideal target temperature and the benefit of hypothermia in patients found with non-shockable rhythm. Hypothermia affects almost all organ systems and, therefore, early detection and treatment of side effects are essential. The aim of the authors is to summarize the clinical role and pathophysiologic effects of therapeutic hypothermia in the treatment of resuscitated patients based on current evidence and their practical experience. Recently dogs ( Canis familiaris ) have been demonstrated to be a promising model species for studying human behavior as they have adapted to the human niche and developed human-like socio-cognitive skills. Research on dog behavior, however, has so far almost exclusively focused on awake functioning. Here we present a self-developed non-invasive canine polysomnography method that can easily be applied to naive pet dogs. N = 22 adult pet dogs (with their owners present) and N = 12 adult humans participated in Study I. From these subjects, N = 7 dogs returned on two more occasions for Study II. In Study I, we give a descriptive analysis of the sleep electroencephalogram of the dog and compare it to human data. In order to validate our canine polysomnography method in Study II, we compare the sleep macrostructure and the EEG spectrum of dogs after a behaviorally active day without sleep versus passive day with sleep. In Study I, we found that dogs' sleep EEG resembled that of human subjects and was generally in accordance with previous literature using invasive technology. In Study II, we show that similarly to previous results on humans daytime load of novel experiences and sleep deprivation affects the macrostructural and spectral aspects of subsequent sleep. Our results validate the family dog as a model species for studying the effects of pre-sleep activities on the EEG pattern under natural conditions and, thus, broaden the perspectives of the rapidly growing fields of canine cognition and sleep research.

Objectives The diagnostic values of the aVR lead or “Vereckei algorithm,” and the lead II R-wave peak time (RWPT) criterion, recently devised for the differential diagnosis of wide QRS complex tachycardias (WCTs), were compared. Methods A total of 212 WCTs (142 ventricular tachycardias [VTs], 62 supraventricular tachycardias [SVT], and eight preexcitation SVTs) from 145 patients with proven electrophysiologic diagnoses were retrospectively analyzed by seven examiners blinded to the electrophysiologic diagnoses. Results The overall test accuracy of the Vereckei algorithm was superior to that of the RWPT criterion (84.3% vs. 79.6%; p = 0.0003). The active role of sleep in memory consolidation is still debated, and due to a large between-species variation, the investigation of a wide range of different animal species (besides humans and laboratory rodents) is necessary. The present study applied a fully non-invasive methodology to study sleep and memory in domestic dogs, a species proven to be a good model of human awake behaviours. Polysomnography recordings performed following a command learning task provide evidence that learning has an effect on dogs’ sleep EEG spectrum. Furthermore, spectral features of the EEG were related to post-sleep performance improvement. Testing an additional group of dogs in the command learning task revealed that sleep or awake activity during the retention interval has both short- and long-term effects. This is the first evidence to show that dogs’ human-analogue social learning skills might be related to sleep-dependent memory consolidation.

Therapeutic use of hypothermia has come to the frontline in the past decade again in the prevention and in mitigation of neurologic impairment. The application of hypothermia is considered as a successful therapeutic measure not just in neuro- or cardiac surgery, but also in states causing brain injury or damage. According to our present knowledge this is the only proven therapeutic tool, which improves the neurologic outcome after cardiac arrest, decreasing the oxygen demand of the brain. Besides influencing the nervous system, hypothermia influences the function of the whole organ system. Beside its beneficial effects, it has many side-effects, which may be harmful to the patient. Before using it for a therapeutic purpose, it is very important to be familiar with the physiology and complications of hypothermia, to know, how to prevent and treat its side-effects.
