Background/aim: To emphasize the role of cochlear implantation CI in the auditory rehabilitation of patients with otosclerosis OS and share our surgical experiences on this rare group of patients. Materials and methods: Retrospective analysis of the patients who have a diagnosis of otosclerosis and implanted between January 1998-May 2019 was performed. Preoperative and postoperative clinical, radiological, audiological and surgical findings are presented. Results: Among 2195 patients who have been implanted in our institution, 12 0.54% met the diagnostic criteria of OS according to their preoperative clinical, radiological, audiological and peroperative surgical findings. Electrode insertion was performed via "round window membrane and cochleostomy"in 8 and 4 patients, respectively. No major complications occured. All patients showed satisfactory performances by means of audiometric scores postoperatively. Nonauditory stimulation NAS which manifested as "facial twitching"was a challenging problem in one patient during the surgery and subsided after the operation. Conclusion: Our experience on CI in patients with OS revealed that the implantation was a relatively safe procedure and had satisfactory impact on audiological performances.

Amaç: Geç teşhis edilen ve tedavi ile düzelmeyen ani sensörinöral işitme kaybı (ASNİK) çocukların dil ve psikolojik gelişimi üzerinde olumsuz bir etkiye sahip olabilir, bu nedenle tanı konur konulmaz tedaviye başlanmalıdır. Bu çalışmanın amacı ASNİK ile takip ve tedavi edilen çocuk hastalarda etiyoloji, prognostik faktörlerin belirlenmesi ve tedavi sonuçlarının karşılaştırılmasıdır.Gereç ve Yöntem: ASNİK tanısı ile tedavi ve takip edilen 28 hasta çocuğun dosyaları retrospektif olarak incelendi. Pür ton odyometrisinde (PTO), 500 Hz, 1000 Hz, 2000 Hz ve 4000 Hz saf ton eşiklerinin ortalaması saf ses ortalaması (SSO) olarak kabul edildi. Hastaların iyileşme durumları Siegel kriterlerine göre belirlendi. Odyometrik eğri türleri artan, azalan ve düz tip olarak gruplara ayrıldı.Bulgular: ASNİK'li hastaların yaşları 7 ile 18 arasında değişmekteydi (ortalama 14,89 ± 3,24). Tedavi öncesi SSO 55,27 ± 12,39 dB (min-maks: 38,5 ila 85,25 dB), tedavi sonrası SSO 23,13 ± 18,22 dB (min-maks: 5 ila 72,5 dB) idi. Odyometrik eğri türleri: azalan grup (n: 11, %39,2), yükselen grup (n: 5, %17,9), düz grup (n: 12, %44,9) şeklinde idi. Tedavi sonrası 18 (%64,3) hastada tam iyileşme, 8 (%28,6) hastada kısmi iyileşme saptanırken 2 (%7,1) hastada iyileşme olmadı.Sonuç: Çalışmamızda sistemik steroid ile tedaviye yanıt oranı yüksek saptandı. Kulak çınlaması varlığı ve tedavi öncesi inen tip odyometrik eğri iyi prognoz faktörleri olarak saptandı., Aim: Sudden sensorineural hearing loss (SSNHL) may have a negative impact on the language and psychological development of children, especially if it is not diagnosed early and treated promptly. This study were aimed to determine and compare the etiological factors, treatment outcomes and prognostic factors in the pediatric patients who were followed up with the diagnosis of SSNHL.Materials and Methods: The files of 28 children were analyzed retrospectively. In pure tone audiometry, the average of pure tone thresholds of 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz were accepted as pure tone averages (PTAV). Patients' recovery status was determined according to Siegel criteria. Audiometric curve types were evaluated as ascending, descending, and flat.Results: The ages of patients with SSNHL was mean ± SD 14.89 ± 3.24 (min-max: 7 and 18). The pre-treatment PTAV was mean ± SD 55.27 ± 12.39 dB HL (min-max: 38.5 and 85.25 dB HL) and the PTAV after treatment was mean ± SD 23.13 ± 18.22 dB HL (min-max: 5 and 72.5 dB HL). Audiometric curve types were detected as descending curve (n: 11, 39.2%), ascending curve (n: 5, 17.9%), flat curve (n: 12, 44.9%). Eighteen (64.3%) patients had complete recovery, 8 (28.6%) patients had partial recovery, and 2 (7.1%) patients had no recovery after the treatment.Conclusion: The response to treatment was found to be high. Descending audiometric curve type was found as a positive prognostic factor. Although the presence of tinnitus was a better prognostic factor than the presence of vertigo.

WOS: 000486388800005, Introduction: Idiopathic facial paralysis or Bell's palsy is the most common type of peripheral facial paralysis. Advanced age and systemic diseases, such as diabetes and hypertension are considered as negative prognostic factors for Bell's palsy still controversial. Here, we aimed to compare the epidemiologic features and prognostic factors of patients with Bell's palsy aged >= 65 years. Materials and Method: Records of patients with Bell's palsy (age, >= 65 years) who were admitted to our clinic between January 2008 and December 2017 were evaluated. Results: We included 89 (72.4%) patients with Bell's palsy. The patients' ages varied between 65 and 91 (72.70 +/- 5.9) years. Paralysis was in the right side in 59.6% (n = 53) and in the left side in 40.4% (n = 36) of the patients. Three (3.4%) patients had progressive facial paralysis. One (1.1%) had positive family history. After at least 6 months of follow-up, 71 (79.8%), 10 (11.2%), 6 (6.7%), and 2 (2.25%) patients were diagnosed with grade 1 and 2, 3, 4, and 5 Bell's palsy, respectively. Furthermore, a statistically significant correlation was observed between the initial paralysis grade and neutrophil-to-lymphocyte ratio (p=0.001). Conclusion: Although old age is considered as a negative prognostic factor in patients with BR we observed satisfactory recovery rates in geriatric patients who received appropriate treatment. In addition, a significant correlation was observed between the initial grade of facial paralysis and neutrophil-to-lymphocyte ratio upon admission, which can be considered as a prognostic factor.

BACKGROUND Idiopathic facial paralysis or Bell`s palsy is the most common type of peripheral facial paralysis. Children with Bell`s palsy is an uneasy situation for the family and physician with questions about the etiology, treatment options and the healing process. Here, we aimed to compare the epidemiologic features and prognostic factors of patients with Bell`s palsy aged < 18 years. METHODS Records of patients with Bell`s palsy who were admitted to our clinic between January 2008 and December 2017 were evaluated. RESULTS Forty-seven patients with Bell`s palsy were included to this study. The patients` ages varied between 7 and 17 (14.7±2.5) years. At the end of at least 6 months of follow-up, 32 (68.1%) of the patients presented with House Brackmann (HB) grade 1 facial paralysis, while 12 (25.5%) of them had grade 2 and 3 (6.4%) of them had grade 3 facial paralysis. Mean neutrophil-to-lymphocyte ratio (NLR) in patients with advanced grades (grade 4, 5, 6) was higher, compared to that of patients with grade 2 and 3 (4.10 ± 1.06 vs 1.34 ± 1.02 (p < 0.001). CONCLUSIONS In our study, the response rate to treatment was high. In differential diagnosis, congenital anomalies, malignancy, trauma, middle ear infection and surgery should be considered. In addition, NLR at admission can be considered as a prognostic factor.

Pseudoankylosis of temporomandibular joint (TMJ) is characterized by limited mouth opening and impaired mandibular mobility induced by pathologic factors outside the joint itself, usually leading to compromised speech, swallowing, and breath functions. Multiple surgical approaches or reconstructive procedures have been proposed to resolve the joint pseudoankylosis and restore the mandibular movement for these affected patients. Free forearm flap has been widely used in reconstruction for various congenital or acquired defects or deformities; however, this flap has been rarely employed for TMJ pseudoankylosis in the literature. Here, the authors reported that noma-induced TMJ pseudoankylosis was diagnosed and successfully treated by extended free forearm flap with length over 10 cm in a Chinese female patient.

7. Transoral approach to a huge neurofibroma of the parapharyngeal space: a case report

Neurofibroma of the parapharyngeal space accounts for less than 2% of all parapharyngeal space neoplasms. A 38-year-old man presented with complaints of snoring and left-sided nasal obstruction. Medical history also revealed numerous huge skin neurofibromas and multiple café au lait spots all over the body. Pharyngeal endoscopic examination disclosed a firm, nonpulsatile submucosal mass, pushing the left pharyngeal region approximately 5 cm medially, and extending to the inferior part of the epiglottis. The tumor was removed by transoral excision without any subsequent complications. Histological and immunohistochemical examinations revealed a neurofibroma.

Choanal atresia is the unilateral or bilateral obstruction of the passage between nasal cavity and nasopharynx. Transnasal surgery of choanal atresia in a patient with unilateral choanal atresia diagnosed accidentally during operation of septorinoplasti for septal deviation and external nasal deformity in Plastic Surgery Clinic is presented. A 25-year-old female referred to plastic surgery clinic with external nasal deformity and difficulty in breathing through nose. Although concha bullosa at middle concha and polipoid degeneration at lower concha are observed in computerized tomography of paranasal sinus, the patient referred to our clinic. In physical examination, due to advanced septal deviation, endoscopic examination of left nasal cavity could not be established. It was planned to examine the left nasal cavity in operating room after septoplasty. In the endoscopic examination after septoplasty, choanal atresia at left nasal cavity observed. Atresia was repaired with transnasal approach. At follow-up period, the choanal opening was intact. Unilateral choanal atresia is a disorder which can reach at the adolesant ages without diagnosis. Although the symptoms of the disease can be interfere in lots of diseases, the diagnosis is simple with nasal endoscopic examination. If nasal endoscopic examination is difficult because of nasal septal deviation, it must be performed in operating room before ending the operation to reveal the additional disorders.
