Aim: In polycystic ovarian syndrome, the ovaries become stiffer due to chronic anovulation. We aimed to compare tissue elasticity in terms of shear wave velocities measured using acoustic radiation force impulse imaging technique between the ovaries of polycystic ovarian syndrome women and non-polycystic ovarian syndrome women. Material and methods: The study was designed as a retrospective data analysis of women who underwent transvaginal ultrasound and acoustic radiation force impulse imaging in a university hospital between July 2014 and March 2015, for various reasons. There were 32 polycystic ovarian syndrome patients and 32 patients without a diagnosis of polycystic ovarian syndrome. Age, body mass index, fasting glucose levels, cycle day 3 follicle stimulating hormone, luteinizing hormone, thyroid stimulating hormone, prolactin, antimullerian hormone levels, and menstrual patterns with clinical hyperandrogenism were evaluated. On the menstrual cycle days 2–4, by performing a transvaginal ultrasound scan, the ovarian volumes and antral follicle counts in both ovaries were recorded for each woman. The ultrasound system was converted into the elastography mode, and acoustic radiation force impulse imaging was performed. Shear wave velocity (m/sec) was measured at least 5 times for each ovary, and the mean value was calculated for each polycystic ovarian syndrome and non-polycystic ovarian syndrome woman. Results: Age, body mass index, fasting glucose levels, cycle day 3 follicle stimulating hormone, luteinizing hormone, thyroid stimulating hormone, and prolactin levels were similar between the groups (p >0,05). A 44-year-old virgin with abdominal mass and anemia wasadmitted to our hospital. The patient complained of constipationand a palpable mass in the abdomen for about 4 years. Ontransabdominal ultrasonography, a giant, complex, solid, lobulatedmass 152x142x81mm in size was observed. Exploratorylaparotomy was performed. There was a giant, multiple lobulatedirregular-shaped solid mass occupying the whole abdomen,reaching the xiphioid and infiltrating the omentum. The size of themass arising from the left utero-ovarian ligament was approximately30x15cm. The histopathologic report confirmed the diagnosis ofleiomyoma with myxoid degeneration., Kırkdört yaşında abdominal kitle ve anemisi olan kadın hastaneyebaşvurdu. Kadının 4 yıldır kabızlık ve karında palpe edilen kitleşikayeti vardı. Abdominal ultrasonografide 152x142x81mmbüyüklüğünde kompleks, solid lobule yapıda kitle izlendi.Laparotomi planlandı. Operasyonda ksifoide kadar uzanan dev,multi-lobule, düzensiz sınırları olan sert kitle görüldü. Kitleninbüyüklüğü yaklaşık 30x15cm idi ve sol utero-ovaryen ligamenttenkaynaklanıyordu. Patoloji raporu miksoid degenerasyon gösterenfibrom olarak geldi.

13-19. yy.lar arasında Anadolu'da oluşmuş ve İslam kültürünün özelliklerini yansıtmış olan Dîvan şiiri, üç kıtaya yayılmış olan Osmanlı İmparatorluğu'nun geniş kültürel birikimine sahip olmuştur.Yayıldığı bu geniş coğrafya ve kültür birikimiyle zengin bir şiir dili oluşmuş ve etkisini günümüze kadar taşımıştır.Altı asır boyunca çeşitli evrelerden geçen Dîvan şiirinde Osmanlı Devleti'nin etkisiyle birçok ülke ve şehirden söz edilir. Bu yer adları kimi zaman sadece coğrafi mekân olarak kullanılırken kimi zaman da tarihi ve ekonomik önemiyle şiirde yer bulmuştur.Halifelik makamı oluşuyla Bağdat, kumaşıyla meşhur Halep, baharatıyla Hindistan, kılıcıyla Şam bunlardan bazılarıdır.Ancak bazı şehirler vardır ki onlar şiirimizin geleneğini ve hayal dünyasını temsil etmektedir. Sevgilinin eşiği ve köyünü tavaf eden şair için Kâ'be, sevilenin inci dişleri için Aden ve Bahreyn, kırmızı, can yakan dudaklar için Bedehşan, benzersiz olan o güzelin güzelliği ve kokusu için Çin şairlerin kimi zaman teşbih, iham, tevriye gibi sanatlarla kullandığı şehirlerin başında gelir.Bu çalışmayla şiirimizde kullanılan yer isimlerinin coğrafi ve tarihi özelliklerinden bahsedip dîvan taramalarıyla bu şehir ve ülkelerin Dîvan şiirimizdeki önemini örnekleyecek metin ve beyitler kullandık. Composed between 13-19 century in Anatolia and reflected features of Islamic cultures, Classical Turkish Poetry has had a wide cultural knowledge of Ottoman Empire which has outspreaded to three continent.With its this wide spreaded in geography and its cultural richness, ıt has had a rich language and continued to keep its effectiveness.In Classical Turkish Poetry which passed through various periods during the six centuries, several countries and cities have been mentioned by the influences of Ottoman Empire. These places' names were used for just geographical location and sometimes were replaced in poetry to their historical and economic importance.Baghdad important for caliphate outority, Halep famous for fabric, Indian for special spices, Damascus for saber are some of the examples of these.But there are some cities that symbolize our customs and fictions of poetry. For poetry who circumambulate to this dear's step and villiage for belaved's pearl teeth Aden and Bahreyn for red and vicious lips. Bedehshan for her unique beauties and odour Chinese poetries use art such as comporision doubt and implied.For this work we use text and verse which sample the importance of classical poetry which searching classical poetry and we mention the historical and geographical features of places' names in poetry. 95

Giriş ve amaç: Gebeliğin en sık medikal komplikasyonu olan gestasyonel diyabet, ilk kez gebelik sırasında tanı konulan glukoz intoleransı olarak tanımlanır. Gebe kadınların yaklaşık %3-5'ini etkileyebilmektedir. Diyabet, kardiyovasküler hastalıklar ve koroner aterosklerotik lezyonlar için majör bir risk faktörüdür. Aterosklerozun subklinik erken dönemde saptanabilen bulgusu; noninvazif olarak karotis intima media kalınlığında (KİMK) artmanın ve endotel-bağımlı vazodilatasyonda (FMD) bozulmanın gösterilmesidir.Kardiyovasküler hastalıkların erken bulgularından olan aterosklerozun öncül sebebi endotel hasarı ve disfonksiyonudur. Bu çalışmada gestasyonel diyabet öyküsü olan ve 1 yıl içinde doğum yapmış hastalarda KİMK ve FMD ile endotel yapı ve fonksiyonlarının belirlenmesi ve buna bağlı kardiyovasküler hastalık riskinin araştırılması planlandı.Gereç ve yöntem: Çalışmamız; Ağustos 2012 – Ağustos 2013 tarihleri arasında Marmara Üniversitesi Tıp Fakültesi Pendik Eğitim ve Araştırma Hastanesi, Kadın Hastalıkları ve Doğum Kliniği, Jinekoloji polikliniğine başvuran, 1 yıl içinde doğum yapmış olan ve gestasyonel diyabet öyküsü olan 74 hasta ve yine 1 yıl içinde doğum yapmış olan ancak gestasyonel diyabet öyküsü olmayan 65 kişilik kontrol grubu oluşturularak toplam 139 kişi ile gerçekleştirildi. GDM tanısı için Dünya Sağlık Örgütü'nün belirlediği tanı kriterleri kullanıldı. İki grubun demografik, metabolik, hormonal özellikleri, kardiyovasküler risk faktörleri ve endotel disfonksiyonunun belirteçlerinden olan KİMK ve FMD ölçümleri karşılaştırıldı. Çalışma prospektif olarak yapıldı. Vaka-kontrol çalışması olarak planlandı.Bulgular: GDM öyküsü olan grupta FMD değeri, GDM öyküsü olmayan gruptan anlamlı (p < 0,05) olarak daha düşüktü. GDM öyküsü olan grupta karotis intima media kalınlığının (KİMK) ortalama değeri, GDM öyküsü olmayan gruptan anlamlı (p < 0,05) olarak daha yüksekti. GDM öyküsü olan grupta, obez olmayan hastalar ile kıyaslandığında, obez hasta grubunda KİMK değeri anlamlı (p < 0,05) olarak daha yüksek; FMD değeri de anlamlı (p < 0,05) olarak daha düşüktü. KİMK değeri ile yaş, BMİ değeri, bel/kalça oranı, sistolik basınç, diastolik basınç, insülin, Hba1c, AKŞ, total kolesterol, Tg, LDL, 75 gr OGTT, Hba1c, AKŞ arasında anlamlı (p < 0,05) pozitif korelasyon; FMD ve HDL değeri arasında anlamlı (p < 0,05) negatif korelasyon mevcuttu.FMD değeri ile BMİ değeri, bel/kalça oranı, sistolik basınç, diastolik basın, insülin, Hba1c, AKŞ, total kolesterol, Tg, LDL ve 75 gr OGTT arasında anlamlı (p < 0,05) negatif korelasyon; HDL ile arasında anlamlı (p < 0,05) pozitif korelasyon mevcuttu. Sonuçlar: Gestasyonel diyabet öyküsünün; diyabetin endotel yapı ve fonksiyonu üzerindeki olumsuz etkisini gösteren belirteçlerden KİMK değerinde yükselme ve FMD değerinde düşüklük ile birlikte olduğu saptanmıştır. Gestasyonel diyabet öyküsü olan kadınlarda saptadığımız bu durumun tespiti, kadınların gelecek yaşamlarında gelişebilecek koroner arter hastalığı riski nedeniyle, klinik bulgu vermeden gerekli önlemlerin alınabilmesine yardımcı olur. GDM öyküsü olan hastalar arsında obez olan hastaların obez olmayanlara göre daha daha yüksek KİMK ve daha düşük FMD değerine sahip olmasının; obezitenin bu hasta grubunda endotel fonksiyon bozukluğunu arttırdığı sonucuna vardık. Çalışmamızda GDM öyküsü olan ve olmayan gruplar arasında kardiyovasküler risk faktörleri ve demografik veriler dağılımının benzer nitelikte olması, sonuçta tespit ettiğimiz endotelyal disfonksiyon farklılığını doğrudan GDM öyküsü olmasına bağlı olduğunu düşündürtmüştür. Introduction and Objective: Gestational diabetes mellitus, the most common medical complication of pregnancy, is defined as glucose intolerance with initial onset or recognition during pregnancy. It could affect 3-5% of pregnant women. Diabetes is a major risk factor for cardiovascular and coronary atherosclerotic lesions. The early recognition of subclinical atherosclerosis is investigating the increase in carotid intima media thickness (CIMT) and the impairment in endothelial dependent vasodilation (FMD- Flow-mediated Dilatation) by noninvasive techniques.The endothelial damage and dysfunction are early markers for atherosclerosis and cardiovascular diseases. This study was aimed to assess and evaluate the risk of endothelial dysfunction by using CİMT and FMD measurements in patients diagnosed with gestational diabetes in their previous births in the last one year.Material and method: Our study includes 139 patients who have consulted Marmara University Hospital Gynecology and Obstetrics Department, Gynecology Outpatient Clinic between August 2012 and August 2013. 74 of them have been diagnosed with gestational diabetes and have given birth within the last year. 65 of them were selected as the control group whom also have given birth within the last year but haven't been diagnosed with gestational diabetes. The diagnostic criterias of World Health Organisation has been used to diagnose gestational diabetes. Demographic, metabolic, hormonal factors, cardiovascular risk factors and CIMT and FMD results were compared. Our study is a prospective study which is outlined as a case control study.Results: The FMD value of GDM positive group was significantly reduced than the control group (p < 0,05). The average value of carotid intima media thickness (CIMT) of GDM positive group was significantly higher than control group (p < 0,05). The non-obese women in study group with GDM history, had a statistically significant lower CIMT and higher FMD (p < 0,05), when compared to those of obese patients. A significant positive correlation between CIMT and age, BMI, waist/hip ratio, systolic blood pressure, diastolic blood pressure, insulin, HbA1c, fasting blood glucose (p < 0,05) was found. Also a significant negative correlation between CİMT and FMD, HDL was detected (p < 0,05). A significant negative correlation between FMD value and BMI, waist/hip ratio, systolic blood pressure, diastolic blood pressure, insulin, HbA1c, fasting blood glucose, total cholesterol, triglyceride, 75 grams oral glucose intolerance test levels (p < 0,05) was calculated. Also a significant positive correlation between FMD value and HDL levels (p < 0,05) was seen.Conclusion: It has been estimated that previous GDM history is accompanied along with findings which shows unfavorable effects of diabetes on endothelial structure and function, an increase in CIMT and a decrease in FMD levels. The early diagnosis of these conditions in pregnant women with GDM can help us to take precautions to decrease the risk of developing coronary artery disease in the future. The higher levels of CIMT and lower levels of FMD in obese patients confirm that obesity increases endothelial dysfunction. 97

Objective: Oocyte donation (OD) cycles have been used extensively over the last decades due to high success regarding live birth rate (LBR). We evaluated the reproductive outcomes of fresh and vitrified sibling oocytes in terms of fertilization rates, blast ratio, clinical pregnancy rates, and LBR., Materials and Methods: This retrospective cohort study was conducted at a tertiary in vitro fertilization (IVF) center. A total of 7515 metaphase II (MII) donor oocytes from 304 donor cycles for 609 oocyte recipients undergoing embryo transfers with either fresh or cryopreserved-thawed donor oocytes. Donor cycles that provided both 12 fresh MII oocytes to be used for one recipient and at least 12 MII oocytes which were suitable for vitrification to be used for another recipient at another time were analyzed. Fertilization rates, blastocyst ratios, clinical pregnancy rates (CPR), LBR were evaluated as main outcome measures., Results: When the fresh and cryopreserved-thawed OD cycles were compared, there was no significant differences between two groups in terms of age of the recipient (41.9 ± 5.7 and 40.3 ± 6.8, p = NS), number of MII oocytes (12.1 ± 0.3 and 12.6 ± 0.8, p = NS), number of 2 pronuclear (PN) (9.1 ± 1.6 and 9.7 ± 2.0, p = NS), blastocyst ratio (58.9 ± 21.7 and 51.3 ± 21.2, p = NS) and number of transferred embryos (1.9 ± 0.3 and 1.9 ± 0.3, p = NS). There was no significant difference between fresh and cryopreserved-thawed sibling donor oocyte cycles in terms of CPR (66.8 % and 60.7 % respectively, p = NS) or LBR (59.5 % and 55.1 %, respectively; p = NS). Miscarriage and multiple gestation rates were similar between groups (p = NS).
