Bolesti mitralne valvule su vrlo često asipmtomatske i najčešće se otkrivaju slučajno. S vremenom opterećuju srčane komore i mijenjaju njihovu strukturu i funkciju. Najvažnija metoda u dijagnostici i evaluaciji stupnja bolesti je ehokardiografija. Do danas su razvijene brojne tehnike popravka mitrale valvule. Kako bi se odabrala čim prikladnija metoda za pacijenta, potrebno je koristiti Funkcionalnu Klasifikaciju i pažljivo intraoperativno analizirati valvulu. S razvojem i usavršavanjem različitih tehnika popravka mitralne valvule, ujedno dolazi i do sve većih dilema. Mnoga istraživanja pokušala su usporediti implantaciju neokordi i resekcijske tehnike, ali još nisu dobiveni podaci o nekim značajnijim razlikama ili prednosti isključivo jedne tehnike. U ovoj retrospektivnoj studiji, proučili smo 53 pacijenta koji su zaprimljeni i liječeni na Zavodu za kardijalnu kirurgiju Kliničkog Bolničkog Centra (KBC) Rijeka u razdoblju od rujna 2011. godine do rujna 2019. godine. Jedna skupina je podvrgnuta resekcijskim tehnikama popravka mitralne valvule, a druga implantaciji umjetnih kordi (neokordi). Većina rezultata odgovara rezultatima dosadašnjih istraživanja, a najznačajniji rezultat uočen je kod veličine implantiranih protetskih prstena, Mitral valve diseases are usually asymptomatic and are most often discovered accidentally. Over time, they burden the heart chambers and change their structure and function. The most important method in the diagnosis and evaluation of the degree of the disease is echocardiography. To date, a number of mitral valve repair techniques have been developed. In order to select the most appropriate method for the patient, it is necessary to use the Functional Classification and carefully analyze the valve intraoperatively. With the development and improvement of various mitral valve repair techniques, there are also growing dilemmas. Many studies have tried to compare chordal replacement and resection techniques, but no data have yet been obtained on some significant differences or advantages of a single technique. In this retrospective study, we studied 53 patients who were admitted and treated at the Department of Cardiac Surgery of the Clinical Hospital Center (CHC) Rijeka in the period from September 2011 to September 2019. One group underwent resection techniques to repair the mitral valve, and the other underwent chordal replacement. Most of the results correspond to the results of previous research and the most significant result was observed with the size of the implanted prosthetic rings.

"Slagalica nasljeđa" - priručnik za opismenjavanje iz medicinske genetike ima tri namjene. Prije svega, on je edukativna slikovnica za studente, liječnike i pacijente, ali i druge zainteresirane pojedince jer su u njoj kroz ilustracije objašnjene osnove genetike čovjeka, kao i osnove medicinske genetike. Od toga kako prepoznati osobu s genetičkim poremećajem, kako nastaju i koje vrste genetičkih poremećaja postoje pa sve do toga na koji ih način možemo dijagnosticirati. Nadalje, nakon svake ilustracije na pojedinoj stranici nalaze se definicije 79 pojmova iz medicinske genetike koje čine tezaurus za studente, liječnike i pacijente koji se na bilo koji način susreću s genetičkim poremećajima. Naposljetku, ova knjiga sadrži i primjere rečenica u koje su ubačeni stručni pojmovi iz medicinske genetike, a koji su namijenjeni studentima prilikom savladavanja komunikacijskih vještina na kolegiju Medicinska genetika, ali i liječnicima prilikom informiranja svojih pacijenata o (mogućem) genetičkom poremećaju. Uz kreatoricu ideje i urednicu izdanja, doc. dr. sc. Ninu Perezu, autori izdanja su studenti šeste godine Integriranog preddiplomskog i diplomskog sveučilišnog studija Medicina i prof. dr. sc. Saša Ostojić.

5. Cardiomyocytes calpain 2 expression: Diagnostic forensic marker for sudden cardiac death caused by early myocardial ischemia and an indicator of the duration of myocardial agonal period?

- Abstract
Sudden cardiac death (SCD) is an unexpected natural death of cardiac etiology and occurs within one hour of the onset of cardiac symptoms in an apparently healthy subject or within 24 h if death is not witnessed. The diagnosis of early myocardial ischemia (EMI) or acute myocardial infarction (AMI) after death is a challenge for forensic pathologists especially when death occurs in a short period of time after the onset of myocardial ischemia. 2+ homeostasis caused by myocardial ischemia during SCD can lead to the activation of calcium-activated non-lysosomal cysteine protease, including calpains. They serve as a proteolytic unit for cell balance and also participate in the processes of apoptosis and necrosis. Agony is a period that precedes somatic death that differs from cellular agony which may evolve for hours after somatic death lasting differently depending on the cell type and mechanism of death. We hypothesize that the expression of calpain 2 in cardiomyocytes could be a specific and sensitive diagnostic forensic marker for SCD caused by EMI and an indicator of the duration of myocardial agonal period. We will conduct a retrospective study that will prove this hypothesis on the respondents who died of SCD by EMI and AMI, instant death by head gunshot and hanging. There is no data on such an analysis in the available literature. The standard hematoxylin-eosin staining will be used to detect cardiac tissue damage. The expression of calpain 2 in cardiomyocytes will be analyzed immunohistochemically. In SCD caused by EMI we expect lower level of calpain 2 expressionin comparison to AMI due to shorter duration of dying. Similar, we predict in the remote region lower calpain 2 expression than in the region of ischemia for both EMI and AMI. In instant death caused by perforating traumatic brain injury we expect mild or no calpain 2 expression throughout the whole myocardium because of very short (immediate) duration of dying. In death caused by hanging calpain 2 expression throughout the whole myocardium should be strong because of longer cellular agonal period. We expect that our results would indicate the immediate activation of calpain 2 in different causes of cardiomyocytes death. From the degree of expression of calpain 2 we could conclude about the duration of cardiomyocytes agony so calpain 2 could be used as a marker for the assessment the duration of somatic and cellular agony., (Copyright © 2021 Elsevier Ltd.
