Bir fıkıh mezhebi olarak Zeydiyye hakkında da bilgi veren genel nitelikli bazı çağdaş yayınlarda Zeyd b. Ali’den (öl. 122/740) mezhebin kurucu imamı, ona nispet edilen el-Mecmûʿu’lfıḳhî’den de mezhebin en önemli kaynağı olarak söz edilmektedir. Buna karşın son zamanlarda yapılan Zeydiyye konulu müstakil çalışmalar, mezhebin Sünnî fıkıh mezhepleri gibi doktriner bir yapıya kavuştuğu müteaḫḫirûn dönem Zeydî literatüründe Zeyd b. Ali’nin hukuk doktrinine bağlılık anlamı içermeyen yaygın bir Zeydîlik anlayışının varlığını fark etmemizi sağlamıştır. Bu iki bakış arasındaki zıtlıktan yola çıkan çalışmamızda Zeydî fürû-i fıkıh literatürü açısından Zeyd b. Ali’nin hangi konumda görüldüğünü anlamak için onun Zeydî mezhep görüşünün oluşumundaki etkisinin açığa çıkarılması hedeflenmiştir. Yöntem olarak onun fıkhî görüşleri ile Zeydiyye’nin en güvenilir fıkıh metinlerinden Kitâbu’l-Ezhâr’daki müftâbih görüşler arasındaki farklılıklar mezhebin oluşum sürecindeki önemli fakihlerden el-Müeyyed-Billâh’ın (öl. 411/1020) Şerḥu’t-Tecrîd’i üzerinden yorumlanmıştır. Örneklem olarak istiftâḥ duasının zamanı, iftitâḥ tekbirinde ve kıraat sırasında ellerin durumu ve kunut dualarının okunuşu gibi namazın kılınış şeklinde öne çıkan ve her Müslümanın tanık olduğu ihtilaflı fıkhî meseleler incelenmiştir. In some contemporary publications of a general nature that also give information about Zaydiyya as a fiqh madhhab, Zayd b. ʿAlī (d. 122/740) is mentioned as the imam of the madhhab, and al-Majmūʿ al-fiqhī attributed to him is cited as the most crucial source of the madhhab. On the other hand, recent independent studies on Zaydiyya have made us realize the existence of a widespread understanding of Zaydiism, which does not mean adherence to Zayd b. ʿAlī’s legal doctrine in the Zaydī literature of the later period which the madhhab gained a doctrinal structure like the Sunnī fiqh madhhabs. Our study, which is based on the contrast between these two perspectives, aims to reveal the influence of Zayd b. ʿAlī in the formation of the Zaydī madhhab’s view to understand his position in terms of furūʿ al-fiqh literature. As a method, the differences between his opinions and the muftābih views of Kitāb al-Azhār, which is among the most reliable fiqh texts of Zaydiyyah have been interpreted through the Sharḥ al-Tajrīd by al-Muʾayyad bi-llāh (d. 411/1020) who is one of the essential faqīhs in the formation process of the madhhab. We examined, as examples, the controversial fiqh issues witnessed by every Muslim and attracting attention in the form of ritual prayer (ṣalāt), such as the introductory duʿā (al-istiftāḥ), the state of the hands during the takbīr al-iḥrām and recitation of the Qur’ān (al-qirāʾāt) at standing (qiyām), and the recitation of the duʿā l-qunūṭ.

Bu çalışmada müteaḫḫirûn dönem Zeydî fıkhında ortaya çıkmış fukahâ tasnifi ve buna ilişkin ıstılahlar konu edilmiştir. Konunun işlenmesinde Zeydî fıkıh birikiminin doktriner bir fıkhî ekol olarak formüle edilmesini amaçlayan çalışmalar arasında önemli bir yeri bulunan Bülûġu’l-ereb ve künûzü’ẕ-ẕeheb fî-maʿrifeti’l-meẕheb adlı eser esas alınmıştır. Konu ile ilişkisi bakımından Zeydî fıkıh çalışmalarının değerlendirildiği bir girişten sonra konu üç ana başlıkta işlenmiştir. Birinci başlıkta eser tanıtılmış, ikinci ve üçüncü başlıklarında da Zeydî fukahânın tasnifinde kullanılan yöntem ve ıstılahlar incelenmiştir. Eserin müellifi Kāsımî İmâmeti döneminde yaşamış olan Ali b. Abdullah eş-Şehârî’dir (öl. 1190/1776). Zeydî literatür arasında önem atfedilen birçok kaynaktan istifade edilerek hazırlanan eserin en önemli özelliği o zamana kadar mezhep doktrini üzerine yazılanların en kapsamlısı olmasıdır. Eserin birinci cüzünde ana tema olarak tabi olunması gereken hak mezhebin Zeydiyye olduğu anlatılmaktadır. İkinci cüzünde ise çalışmamızın konusu olan Zeydî fukahânın tabakaları anlatılmıştır. Fukahâ tabakası en başta selef-i ṣâliḥîn ve ḫalef-i ṣâliḥîn olmak üzere ikiye ayrılır. Zeydî fakîhlerin ilk tabakası 3/10. asrın başlarına kadar uzanan selef tabakasıdır. Bu tabaka sâbıḳûn ve müteaḫḫirûn olmak üzere iki kısma ayrılmıştır. Sâbıḳûn kısmı Hz. Ali (öl. 40/661), iki oğlu Hz. Hasan (öl. 49/669) ve Hz. Hüseyin (öl. 61/680) ile başlayıp bunların soyundan gelen on iki imamın evladı ile sona ermektedir. Zeyd b. Ali (öl. 122/740) de on iki imamdan biridir. Selefin Kāsım er-Ressî (öl. 246/860) ile başlayan müteaḫḫirûn kısmındaki fakîhlerin en önemlileri ehlü’n-nüṣûṣ veya eimmetü’n-nüṣûṣ diye anılan altı imamdır. Böyle isimlendirilmelerinin sebebi usul ve fürû alanında mezhep görüşlerinin tespitinde bunların sözlerinin esas alınmış olmasıdır. Tabakanın başındaki Ressî sâbiḳûnun üzerinde icmâ ettikleri ilkelerin ve ʿitretin bütün bilgisinin birçok kanaldan kendisine ulaştığı bir noktada bulunur. Ressî’nin torunu Hâdî-İlelhak (öl. 298/911), mezhepteki konumu bakımından en önemli imamdır. Halef tabakası da ehl-i taḥṣîl ve ehl-i naẓar olmak üzere iki kısma ayrılmıştır. Muḥaṣṣilûn veya eimmetü’t-taḥṣîl diye de söz edilen birinci kısmın alt kategorisi yoktur. İkinci kısım ise ehl-i taḳrîr ve müẕâkirûn olmak üzere iki alt kısma ayrılmıştır. Muḥaṣṣilûn tabakası ehlü’n-nüṣûṣun son tabakasına yetişenlerle başlayıp üç önemli imamdan biri olan Abdullah b. Hamza (öl. 614/1217) ile son bulur. Muḥaṣṣilûnun diğer önemli iki imamı ise Ahmed b. el-Hüseyin (öl. 411/1020) ile kardeşi Yahyâ b. el-Hüseyin’dir (öl. 424/1032). İlk dönemi itibariyle bir imamlar fıkhı (meẕâhibü’l-eimme) halinde olan Zeydî fıkhını tek bir mezhep haline getiren ve onu son dönemdeki klasik biçimine kavuşturan, bu tabaka fakîhlerinin taḥṣîlü’l-meẕheb adı verilen fıkıh faaliyetidir. Ehl-i taḳrîr ile müẕâkirûn fakîhleri aynı dönemde yaşamışlardır. Bu dönem muḥaṣṣilûn tabakasının son asrı olan 6/12. asrın başlarından Şehârî’nin yaşadığı 12/18. asrın sonlarına kadar devam eden yaklaşık 700 yıllık bir zaman dilimidir. Ehl-i taḳrîrin mezhepteki rolleri çoğunlukla ehl-i tahṣîlin çalışmalarını gözden geçirip isabetli olanlarını onaylamak ve hatalı olanlarını da tashih etmekten ibaret olduğundan bu şekilde isimlendirilmişlerdir. Kitâbu’l-Ezhâr müellifi İbnü’l-Murtazâ (öl. 840/1437) eh-i takrîr içinde öne çıkan fakîhlerden biridir. Zeydî fukahânın çoğunluğu müẕâkirûn tabakasına mensuptur. Bunlar ehl-i naẓarın en alt tabakasıdır. Çalışmada ulaşılan önemli bir sonuç olarak şunu ifade etmek mümkündür: Zeydî fukahâ tasnifi ve kullanılan ıstılahlar açısından Şeharî’nin eseri ile aynı konuya yer veren modern Zeydî çalışmalardaki bazı bilgi ve kanaatler arasında farklılık vardır. Dolayısıyla çalışmamızda bu esere dayalı olarak yapılan tespitlerin ve ortaya konulan sonuçların diğer Zeydî klasikler üzerine yapılacak yeni çalışmalarla teyit edilmesi ve Zeydî fıkhına ilişkin mevcut kanaatlerin gözden geçirilmesi gerekir.

Çalışmamızda müteahhirûn dönem Yemen-Zeydî ilim geleneğine ait Dülâme’nin (öl. 1179/1765) Şüzûru’z-zeheb fî tahkîki’l-mezheb adlı eserin tahkikli neşri hedeflenmiştir. İshâk b. Yûsuf’un et-Tefkîk li-Ukûdi't-teşkîk adlı risalesine zeyl olarak kaleme alınan eserde mezhebin ve Zeyd b. Ali’ye nispetinin mahiyeti, mezhebin usulü, Zeydî fıkıh metinlerindeki tahric ve tercihle ilgili ıstılahlar gibi Zeydî fıkhına dair önem arz eden kritik konular ele alınmıştır. Aynı dönemde kaleme alınmış Şehârî’nin (öl. 1190/1776) Bülûğu’l-ereb ve künûzü’z-zeheb fî maʿrifeti’l-mezheb isimli çalışmasıyla uyumlu bir mezhep anlayışına sahip olduğu görülen eser, Zeydiyye mezhebinin doktriner yapısının anlaşılması bakımından mühim bir kaynaktır. Soru cevap tarzında hazırlanan eseri önemli kılan diğer bir husus da hem Zeydî toplumun önde gelen âlimlerinden hem de sûfî kişilikleri ile maruf Hanefî ulemâdan öğrenim görmüş bir fakih tarafından yazılmış olmasıdır. Eserin ele aldığı konular hususi olarak Zeydiyye fıkhıyla ilgili olmakla birlikte genel olarak mezhep olgusunun mahiyeti açısından da önem arz etmektedir. Tahkik çalışmamızda müellif nüshası bulunmayan eserin metin inşasında aralarında 1238/1823 tarihli nüshanın da bulunduğu en eski istinsah tarihli dört tanesi esas alınarak tercih yöntemi kullanılmıştır.

Patients with solid malignancies are more vulnerable to severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infection than the healthy population. The outcome of SARS-CoV-2 infection in highly immunosuppressed populations, such as in patients with hematological malignancies, is a point of interest. We aimed to analyze the symptoms, complications, intensive care unit admissions, and mortality rates of patients with hematological malignancies infected with SARS-CoV-2 in Turkey.In this multicenter study, we included 340 adult and pediatric patients diagnosed with SARS-CoV-2 from March to November 2020. Diagnosis and status of primary disease, treatment schedules for hematological malignancies, time from last treatment, life expectancy related to the hematological disease, and comorbidities were recorded, together with data regarding symptoms, treatment, and outcome of SARS-CoV-2 infection.Forty four patients were asymptomatic at diagnosis of SARS-CoV- 2 infection. Among symptomatic patients, fever, cough, and dyspnea were observed in 62.6%, 48.8%, and 41.8%, respectively. Sixty-nine (20%) patients had mild SARS-CoV-2 disease, whereas moderate, severe, and critical disease was reported in 101 (29%), 71 (20%), and 55 (16%) patients, respectively. Of the entire cohort, 251 (73.8%) patients were hospitalized for SARS-CoV-2. Mortality related to SARS-CoV-2 infection was 26.5% in the entire cohort; this comprised 4.4% of those patients with mild disease, 12.4% of those with moderate disease, and 83% of those with severe or critical disease. Active hematological disease, lower life expectancy related to primary hematological disease, neutropenia at diagnosis of SARS-CoV-2, ICU admission, and first-line therapy used for coronavirus disease-2019 treatment were found to be related to higher mortality rates. Treatments with hydroxychloroquine alone or in combination with azithromycin were associated with a higher rate of mortality in comparison to favipiravir use.Patients with hematological malignancy infected with SARS-CoV-2 have an increased risk of severe disease and mortality.Solid malignite hastalarının şiddetli akut solunum yolu enfeksiyonu-koronavirüs-2 (SARS-CoV-2) enfeksiyonuna sağlıklı bireylerden daha yatkın oldukları gösterilmiştir. Bu verilerin ardından oldukça yoğun immünosupresif olan hematolojik malignite hastalarında SARS-CoV-2 infeksiyonu sonuçları ilgi konusu olmuştur. Biz de bu makalede Türkiye’de hematolojik malignite tanısı ile takip ve tedavi edilirken SARS-CoV-2 enfeksiyonu saptanan hastaların semptom, komplikasyon, yoğun bakım ünitesine yatış ve mortalite oranlarını değerlendirmeyi amaçladık.Çok merkezli çalışmamıza Mart-Kasım 2020 tarihleri arasında SARS-CoV-2 enfeksiyonu tanısı alan erişkin ve pediatrik 340 hastayı dahil ettik. Hastaların hematolojik malignite tanıları, hastalık statusları, tedavileri, son tedaviden enfeksiyona kadar geçen süre, komorbiditeleri, yaşam beklentileri değerlendirildi. Semptomları, oluşan komplikasyonlar ve sonuçlar analiz edildi.Kırk-dört hasta semptomsuz olarak enfeksiyonu geçirirken, semptomatik hastaların ateş, öksürük, ve dispne oranları sırasıyla %62,6, %48,8 ve %41,8 idi. Altmış altı hasta (%20) hastalığı hafif geçirirken, orta, ciddi ve kritik hastalık tanısı alanların oranı sırasıyla %29, %20 ve %16 idi. Tüm kohortta ölüm oranı %26,6 idi; ölüm hafif hastalığı olanlarda %4,4 , orta derece hastalık geçirenlerde %12 ve ciddi/kritik hastalık geçirenlerde ise %83 olarak saptandı. Aktif hematolojik hastalık olması, primer hematolojik hastalığa bağlı düşük hayat beklentisi, SARS-CoV-2 tanısı aldığında nötropenik olmak, yoğun bakıma alınmış olmak ve ilk sıra koronavirüs hastalığı-2019 tedavisi yüksek ölüm riski ile ilişkilendirilen faktörlerdendi. Tek başına veya azitromisin ile kombine olarak hidroksiklorokin kullanan hastaların sadece favipiravir kullananlara göre ölüm riskleri daha yüksek saptandı.SARS-CoV-2 infeksiyonu geçiren hematolojik malignite hastalarında daha yüksek oranda ciddi hastalık ve ölüm riski gözlenmektedir.

Castleman disease is a rare lymphoproliferative disease also known as angiofollicular lymph node hyperplasia. It is classified as hyaline vascular and plasmacytic variants histologically but characteristics of both types can coexist. Most unicentric cases of the disease are hyaline vascular while most multicentric cases are of the plasmacytic type. Although the pathogenesis is not completely understood, the role of interleukin (IL)-6 in unicentric disease and the roles of IL-6 and human herpes virus-8 in multicentric disease are well defined. Unicentric disease is typically localized and symptoms are minimal and treated locally. Multicentric disease is systemic and clinically characterized by generalized lymphadenopathy, splenomegaly, anemia, and systemic inflammatory symptoms. Systemic therapies are primarily given. Several malignant diseases including lymphomas, POEMS syndrome, follicular dendritic cell sarcomas, paraneoplastic pemphigus, Kaposi sarcoma, and amyloidosis can be associated with Castleman disease. In this paper, recent information about Castleman disease, which is a rare disease, is summarized.Castleman hastalığı, anjiyofolliküler lenf nodu hiperplazisi olarak da bilinen nadir bir lenfoproliferatif hastalıktır. Histolojik olarak hiyalin vasküler ve plazmasitik varyant olarak sınıflandırılır ancak nadiren iki tipe ait özellikler bir arada bulunabilir. Unisentrik hastalığı olan olguların çoğu hiyalin vasküler, multisentrik hastalığı olan olguların çoğu ise plazma hücreli histolojik tipindedir. Patogenezi tam olarak anlaşılmamıştır fakat unisentrik hastalıkta interlökin (IL)-6’nın, multisentrik hastalıkta IL-6 ve human herpes virüs-8’in rolü iyi tanımlanmıştır. Unisentrik hastalık tipik olarak lokalizedir, semptomlar minimaldir ve tek başına lokal tedavi uygulanır. Multisentrik hastalık sistemik bir hastalıktır ve klinik olarak yaygın lenfadenopati, splenomegali, anemi ve sistemik inflamatuar semptomlarla karakterizedir. Başlıca sistemik tedaviler uygulanır. Lenfomalar, POEMS sendromu, folliküler dendritik hücreli sarkomlar, paraneoplastik pemphigus, Kaposi sarkomu, amiloidoz Castleman hastalığı ile ilişkili olabilir. Bu yazıda nadir bir hastalık olan Castleman hastalığı ile ilgili güncel bilgiler özetlenmiştir.

Bone marrow aspirate for morphology and cytogenetics and qualitative reverse transcriptase polimerase chain reaction on peripheral blood cells is mandatory for the diagnosis of chronic myeloid leukemia (CML). Bone marrow biopsy and fluorescence in situ hybridisation may be necessary in selected patients. Performing other tests and diagnostic procedures depends on characteristics of the individual patient. Although flow cytometry is an essential tool in the diagnosis and monitoring of many hematological malignancies, it has a limited role in CML. In this study, we evaluated the CD45 side scatter results of our CML patients at diagnosis and during follow up. Totally 56 CML patients (22 female and 34 male) in chronic phase treated with imatinib were included. Patients were also evaluated after 8 (3-19) months follow up. Complete blood cound parameters and CD45/SSC results of the patients at diagnosis and follow up were evaluated retrospectively. Çalışmamızda kliniğimizde Ocak 2008-Aralık 2018 tarihleri arasında, Hodgkin lenfoma (HL) tanısı alan hastaların; demografik, histopatolojik, prognostik özelliklerini belirlemeyi ve bu belirteçlerin sağkalım üzerindeki ilişkisini ortaya koymayı amaçladık. Çalışmamızda Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Hematoloji Bilim Dalı’nda 2008-2018 yılları arasında, Dünya Sağlık Örgütü (WHO) 2008 sınıflamasına göre Hodgkin lenfoma tanısı almış olan 130 hastanın verileri retrospektif olarak değerlendirildi. Çalışmaya dahil edilen 130 hastanın %61.5’i (n=80) erkek, yaş ortalaması 46.5±15.8 (20-89) yıldı. Tanı sırasında hastaların %37.7’si erken evre, %62.3’ü ileri evreydi. En sık tanı evre Ⅱ’de (%34.6) konulmuştu. Hastalardan %93.1’i klasik HL, %6.9’u nodüler lenfosit predominant HL tanısı almıştı. Klasik HL tanılı hastaların %49.6’si nodüler sklerozan HL (NSHL), %15.7’si mikst selüler HL, %7.4 lenfositten zengin HL, %3.3’ü lenfositten fakir HL tanılıydı, %24 ise klasik HL tanılı ancak alt tipi belirtilmemişti. NSHL tüm hastalarda, kadınlarda ve erkeklerde en sık görülen histolojik alt gruptu. Evrelere göre genel sağkalım (OS) ve relapssız sağkalımlara (RFS) bakıldığında evre Ⅰ’de en yüksek, evre Ⅳ’te en düşüktü. Tüm hastalara göre bakıldığında 5 yıllık OS %88.7, 5 yıllık RFS %83.9, 10 yıllık OS %82.2, 10 yıllık RFS %82.4 olarak bulundu. In our study, the data of 130 patients who were diagnosed with Hodgkin lymphoma in Eskişehir Osmangazi University Faculty of Medicine Hematology Department between 2008-2018 according to the World Health Organization (WHO) 2008 classification were analyzed retrospectively. 61.5% (n = 80) of the 130 patients included in the study were male, the mean age was 46.5 ± 15.8 (20-89) years. At the time of diagnosis, 37.7% of the patients were in the early stage and 62.3% were in the advanced stage. The most common diagnosis was made in stage (34.6%). 93.1% of the patients were diagnosed with classical HL and 6.9% with nodular lymphocyte predominant HL. Of the patients with classic HL, 49.6% were diagnosed with nodular sclerosing HL (NSHL), 15.7% with mixed cellular HL, 7.4% with lymphocyte-rich HL, 3.3% with lymphocyte-poor HL, 24% with classic HL but no subtype specified. NSHL was the most common histological subgroup in all patients, women, and men. Looking at overall survival (OS) and relapse-free survival (RFS) by stages, it was highest in stage Ⅰ and lowest in stage Ⅳ. Glanzmann thrombasthenia is an inherited disease causing bleeding episodes due to platelet dysfunction. The standard treatment for moderate-severe bleeding is platelet transfusion. Recombinant factor VIIa (rFVIIa) is successfully used in bleeding episodes and invasive procedures. Here, we present a patient with Glanzmann thrombasthenia, whose bleeding episodes could only be controlled by rFVIIa. The patient is a 28 years old male, who has had frequent bleeding episodes unresponsive to local hemostatic agents and tranexamic acid and had an anaphylactoid reaction to platelet transfusion. We started the patient on a low-dose (20 μg/kg) rFVIIa once a week. The patient has no spontaneous bleeding since then. This is the first case report of a Glanzmann thrombasthenia patient on routine prophylaxis with low-dose rFVIIa.

As a result of the population aging, the incidence of both hematologic and non-hematologic diseases of elderly gets higher, requiring bone marrow (BM) aspirations and biopsies be implemented also in the geriatric patients. Our aim in this study is to address BM examination in the geriatric patients aged 85 and over, describing its indications and morbidity and discussing the established diagnoses and outcomes of applied therapies based on BM examinations in comparison to the available literature which is actually limited in number particularly in this patient group. We have retrospectively reviewed the BM aspiration/biopsy results of 114 patients aged ≥ 85 years who were followed-up by hematology department of a university hospital from 2010 to 2020. The patients were selected through the internal data handling system of the hospital using the entry codes corresponding to BM aspiration and biopsy and those at the age of 85 or older were identified. Demographic features, primary diagnosis, any comorbidities, complete blood count (CBC) details, erythrocyte sedimentation rate (ESR), red blood cell indices, C-reactive protein (CRP), serum ferritin, vitamin B12, and folic acid levels, any abnormal results in serum biochemistry testing, the indication(s) for and the result(s) from BM aspiration/biopsy, and the treatments applied based on those result(s) were recorded for each patient as well as the rates of and reasons for mortality. In our cohort of 114 patients with a mean age of 86.3±1.7 (85-93) years, 64.9% (n=74) were males. None of the patients experienced any serious complication during BM aspiration and biopsy. First three indications in our cohort for BM aspiration/biopsy in decreasing order of frequency were cytopenia in 48.2% (n=55), anemia + elevated ESR in 18.4% (n=21), and leukocytosis + anemia + thrombocytopenia in 15.8% (n=18). BM aspiration/biopsy results gave rise to a pathologic or hematologic diagnosis in 85.9% (n=98) and 78.9% (n=90) of patients, respectively. Most frequent hematologic malignancies were myelodysplastic syndrome (MDS), acute myeloid leukemia (AML), and multiple myeloma (MM). Kaplan-Meier analysis revealed hemoglobin (Hb) and LDH levels as prognostic factors with an impact on mortality. Prognostic factors with an impact on mortality based on multivariate stepwise Cox regression analysis, on the other hand, were uric (p= 0.018, hazard ratio (HR)= 1.134, 95% confidence interval (CI) = 1.022-1.258), LDH (p=0.092, HR=1.001, 95% CI= 1.000-1.002), and platelet levels (p=0.007, HR=1.000, 95% CI=1.000-1.000). BM examination should definitely be performed in patients aged ≥ 85 years, particularly in the event of cytopenia, unexplained anemia, and elevated ESR both for diagnostic purposes and to prolong life expectancy through administration of modified therapies depending on the performance status. In such patients uric acid, LDH and platelet levels should be closely followed-up as independent prognostic variables which effect mortality.

Background/aim: hyperparathyroidism is an endocrine disorder characterized by hypercalcemia. Because of calcium’s effects on parathyroid glands, bone, intestines, and kidneys, it has an important place in homeostasis. The results of studies regarding hyperparathyroidism hemostasis are conflicting. Thromboelastography helps to evaluate all steps of hemostatic system. Our aim in this study was to investigate the possible role of hemostatic mechanisms in the development of thrombosis in hyperparathyroid patients with the modified rotation thromboelastogram (ROTEM). Materials and methods: twenty-two patients with primary hyperparathyroidism (PHPT) and 20 healthy controls were involved. This study was conducted in Eskisehir Osmangazi University Faculty of Medicine, Endocrinology and Hematology clinics for 2 years. The complete blood count, fibrinogen, D-dimer levels, prothrombin time, activated prothrombin time, and ROTEM parameters [clot formation time (CFT), clotting time (CT), and maximum clot formation (MCF)] were determined by two activated tests, INTEM and EXTEM analyses. A thromboelastographic evaluation was performed in the preoperative and postoperative (3 months after surgery) periods. Results: in INTEM assay, the CT (p = 0.012) and CFT (p = 0.07) values were increased in preoperative PHPT patients compared with the control group. Although there was a decrease in the postoperative CT and CFT values, no statistical difference was found. Conclusion: the prolongation of the CT and CFT values were consistent with a hypocoagulable state in patients with PHPT. Hyperparathyroidism causes a hypocoagulable state that can be successfully assessed by ROTEM. Hemostatic changes, do not seem to have an effect on increased cardiovascular mortality., NA

Mantle hücreli lenfoma (MHL), Hodgkin dışı lenfoma vakalarının %10’dan daha azını oluşturur. Başlıca bağımsız risk faktörleri; MHL Uluslararası Prognostik İndeksi (MIPI) ve Ki-67 tümör proliferasyonu indeksidir. Bu çalışmanın amacı; bölümümüzde takip ve tedavisi yapılan MHL hastalarının geriye dönük olarak değerlendirilmesidir. Çalışmaya tıbbi kayıtlarına ulaşılabilen toplam 27 MHL hastası dahil edildi. Bu veriler incelenerek hastaların, MIPI skoru, kemik iliği tutulumu, ekstranodal tutulum, tedavi protokolleri, tedaviye yanıtı, nüks durumu ve nakil öyküsü kaydedildi. Bulgular: Hastalar ortalama 64,4 (1-246) ay süreyle takip edilmişti. Hastaların 5’ine (%18) otolog, 2’sine (%8) allogeneik hematopoietik kök hücre nakli yapılmıştı. Birinci basamak tedavi sonrası 17 (%63) hastada tam yanıt, 3 (%11) hastada kısmi yanıt elde edilirken 7 (%26) hastada progresyon saptandı. Birinci basamak tedavi sonrası medyan progresyonsuz sağkalım 29 (3-120) ay olarak hesaplandı. MIPI skorlamasına göre düşük riskli hasta grubunun medyan sağkalımı, yüksek riskli hasta grubuna göre daha uzun (194 ay vs 126 ay, p=0.04), orta riskli hasta grubunun progresyonsuz sağkalımı yüksek riskli hasta grubuna göre daha uzun (41 ay vs 3 ay, p=0.025) bulundu. Sonuç olarak, çalışmamızda hastaların medyan tanı yaşı, çoğunun evre III-IV olması, MIPI skoru yüksek hastalarda progresyonsuz sağkalımın kısa olması literatürle uyumlu bulgular olarak saptandı. Merkezimizde en çok tercih edilen birinci basamak tedavi R-hyper-CVAD/MTX-AraC olup literatüre kıyasla medyan progresyonsuz sağ kalım süresi daha kısa bulundu. MHL’de giderek artan hedefe yönelik tedavi kullanımı dikkate alındığında, bu tür tedavilerle eskiden beri mevcut olan tedavilerin kıyaslanması açısından sonuçlarımızın önemli olduğu kanaatindeyiz., Mantle cell lymphoma (MCL) comprises less than 10% of non-Hodgkin's lymphoma cases. Major independent risk factors are identified as the MCL International Prognostic Index (MIPI) and Ki-67 proliferation index of tumor. The aim of this study is retrospectively evaluate MCL patients treated and followed-up in our department. Materials and Methods: This study included 27 MCL patients medical records of whom could be reached. The data was reviewed to figure out MIPI score, bone marrow involvement, extranodal involvement, treatment protocols, treatment response, relapse status, and transplant history. Patients were followed-up for a mean duration of 64.4 (1-246) months. Subsequent to the first-line treatment, complete response was achieved in 17 (63%) and partial response was achieved in 3 (11%) patients, whereas 7 (26%) patients experienced disease progression. Median PFS following first-line treatment was 29 (3-120) months. Based on the classification by MIPI scoring, low-risk patients had significantly longer median survival than that of high-risk patients (194 months vs 126 months, p=0.04), and the patients at moderate-risk had significantly longer PFS compared to high-risk patients (41month vs 3 months, p=0.025). Median age, stage III-IV, and short duration of PFS in patients with high MIPI score were our findings in parallel with available literature. R-hyper-CVAD/MTX-AraC was mostly preferred first-line treatment at our center with a shorter median PFS compared to literature. Given the expanding use of target-driven therapies for MCL, we believe our results are noteworthy in comparing such therapies with pre-existing therapies.

Otolog kök hücre nakli, immünokemoterapi sonrası relaps Hodgkin ve non-Hodgkin lenfomada uzun süreli sağkalımı arttıran standart bir tedavi şeklidir. Başarıyı etkileyen en önemli faktör hastalığın kemosensitif ya da kemorezistan olmasıdır. Ancak transplant ilişkili morbidite/mortalite işlem başarısını halen önemli ölçüde etkilemektedir. Transplant ilişkili morbidite/mortalite açısından minimal kalıntı hastalığın yok edilmesi ile toksisite arasında bir denge sağlanması önemlidir. Bu amaçla yüksek doz rejimlerin terapötik penceresini arttırmaya yönelik çabalar devam etmektedir. Bununla birlikte bir yüksek doz tedavinin diğerine üstünlüğünden bahsetmek için kesin kanıtlar mevcut değildir. Farklı rejim arayışları devam etmekle birlikte özellikle karmustin temininde bir dönem yaşanan sıkıntı rejim değişikliğini zorunlu kılmıştır. Bu süreçte merkezimizde BEAM yerine BuCyE protokolü tercih edilmiştir. Çalışmamızda BuCyE’nin BEAM’a kalıcı bir alternatif olma potansiyelinin değerlendirilmesi amaçlanmıştır. Çalışmaya otolog nakil işlemleri 2006-2017 yılları arasında gerçekleştirilen 20 hasta dahil edildi.Hastalar 2 gruba ayrılarak demografik, klinik ve laboratuvar özellikleri açısından değerlendirildi. BuCyE uygulanan hasta sayısı 10 idi. Karşılaştırma amacıyla kullanılan BEAM grubuna seçilen hastalar verilerine ulaşılması mümkün olan hastalara ait isim listesinden rastgele seçildi. Sağkalım süresi; kaybedilen hastalar için nakil günü ile ölüm tarihi arası, yaşayan hastalar için nakil günü ile değerlendirme tarihi arası olarak hesaplandı. Çalışmamızda birçok parametre için veriler 2 grupta da benzer olmakla birlikte sağkalım BEAM lehine bulundu (55.25±15.29 vs 12.12±4.02 ay, p=0.02). Hasta sayısı ve takip süresi kesin sonuçlar çıkarmak için yetersiz olmakla birlikte verilerimiz sağkalım süresi ve merkezimizin mevcut tecrübesi itibariyle BEAM protokolünün lenfoma için BuCyE’ye tercih edilecek hazırlama rejimi olduğunu düşündürmüştür. Ancak prospektif karşılaştırmalı çalışmaların yokluğunda bir hazırlama rejimini tavsiye etmek mevcut kanıt düzeyleri düşük olduğundan güçtür. Devam eden klinik çalışmalara katılım önemlidir., Autologous stem cell transplantation is a type of standard therapy which increases the long term survival in post immunochemotherapy relapsed Hodgkin and non-Hodgkin lymphoma. Success is most commonly affected by the presence of chemosensitive or chemoresistant disease. However, transplant related morbidity/mortality still affects the success of the procedure. In the era of transplant related morbidity/mortality it is important to provide a balance between minimal residual disease and toxicity. To manage this goal trials on increasing the therapeutic window of high dose regimens are continuing. Unfortunately, we do not have conclusive evidence to say that one high dose regimen is better than the other. Different regimen trials are still going on but lack of carmustine for a while led to a compulsory change in regimen preference. During this period BuCyE was preferred instead of BEAM in our center. In this study, our aim was to decide whether BuCyE can be a permanent alternative to BEAM. Twenty patients who underwent autologous transplantation between 2006-2017 were included in the study. Patients were divided into 2 groups demographic, clinical and laboratory characteristics were evaluated. The number of patients in BuCyE group was 10. BEAM group which is used for comparison was chosen randomly from the list of patients with accessible data. Survival was calculated as the time between day of transplantation and data collection or exitus. Many of the parameters were similar between 2 groups in our study but survival was longer in favor of BEAM group (55.25±15.29 vs 12.12±4.02 months, p=0.02). Despite number of patients and follow up period is not enough to make definitive conclusions our data suggested that BEAM was superior to BuCyE in lymphoma in terms of survival and center experience. In the absence of prospective trials it is difficult to suggest a conditioning regimen due to low level of evidence. It is important to participate in ongoing clinical trials.

Chronic myeloproliferative neoplasms (MPN) are clonal disorders of hematopoietic stem/progenitor cell characterized by thrombohemorrhagic complications and a tendency to transform into acute leukemia. The pathogenesis of thrombosis in MPN is complex and results from a multifaceted interplay of clinical and disease-related factors. Rotational thromboelastometry (ROTEM) provides the complete and rapid information about all stages of the coagulation process. Here, we assess ROTEM parameters as a screening of coagulation profile in patients with MPNs. In particular, higher mean maximum clot firmness values were found in Essential thrombocythemia and Polycythemia vera patients when compared to healthy controls. Rotational thromboelastometry may be able to detect MPN patients who are susceptible to thrombotic and/or hemorrhagic complications. The predictive value of ROTEM for thrombosis remains to be established to classify subsets of patients at prominent risk who may benefit from prophylaxis with antithrombotic drugs.

Bu makalede, Mustafa Ahmed ez-Zerkā’nın “el-Fıkhu’l-İslâmî fi sevbihi’l-cedîd” adlı kitap projesinin kapsamı konusunda yeni bir kanaat ortaya konulması ve müellifin bu proje kapsamında yayımlamış olduğu ilk iki eserin yeni baskılarında yapılan değişikliklere dikkat çekilmesi hedeflenmiştir. Bu amaçla evvela müellifin kitaplarının ön sözlerindeki açıklamalardan hareket edilerek oradan elde edilen bilgilerin eserlerin muhtevası ile karşılaştırmalı bir şekilde yorumlanmasıyla projenin amaçları ile sonuçları arasındaki ilişki sorgulanmıştır. Daha sonra, müellifin “el-Medhal ilâ nazariyyeti’l-iltizâmi’l-‘âmme” adlı eserine yeni baskıda eklemiş olduğu “Nusûsu mevâddi’n-nazariyyeti’l-âmmeti li’l-iltizâmât fi’l-fıkhi’l-İslâmî” adlı kanun tasarısının bu projeyle ilişkisinin belirlenmesine çalışılmıştır. Vardığımız kanaate göre bu tasarı, müellifin yaptığı nihai değişiklikte başlangıçtaki plandan farklı bir biçimde ortaya çıkmış olsa da, daha önce projenin dördüncü cildi için planlanmış olan asıl çalışmanın yerine ikame edilmiş bir metindir. Zerka’nın İslam fıkhını yeni bir üslupla sunma çabasının somut neticelerinden bir olan bu metin, hem onun bu çabasının doğru anlaşılabilmesi hem de İslam fıkıh tarihinin son döneminin en önemli gelişmelerinden biri olan İslam fıkhına dayalı kanun yapma faaliyetinin daha ileri bir noktaya taşınabilmesi bakımından farkında olunması gereken bir özelliğe sahiptir.

Valproik asit cocuklarda epilepsi tedavisinde kullanilmaktadir. En sik gorulen yan etkileri hepatotoksisite, anemi ve trombositopenidir. Valproik asit bazi hematolojik hastaliklarda tedavi amaciyla kullanilmaktadir. Epileptik hastalarda fetal hemoglobin uzerine olan etkileri tam olarak arastirilmamistir. Bu vaka kontrol calismasinda en az alti aydir valproik asit tedavisi alan epilepsi hastasi cocuklar ve saglikli gonulluler dahil edilmistir. Fetal hemoglobin degerleri, ilac kan duzeyleri arastirilmistir. Kirk dort epilepsi hastasi cocuk ve 57 saglikli cocuk arastirmaya dahil edildi. Ortalama fetal hemoglobin (HbF) yuzdesi epilepsi hastalarinda ve kontrol grubunda sirasiyla 0.48 ±0.96 (%) ve 0.25 ±0.61 (%) saptandi. Kan valproik asit duzeyi ile HbF duzeyi arasinda anlamli bir iliski belirlendi. Calismamiz valproik asit tedavisinin HbF duzeyini etkilemedigini gosterdi. Epileptik hastalarda HbF yuksekligi belirlenmesi durumunda dikkatli bir arastirma gerekmektedir.

Granulositik sarkom, myeloid seri progenitor hucrelerini iceren ekstrameduller alanlarda yer alan tumoral bir kitledir. Genellikle kemik, periost, yumusak dokuda lokalize olurlar. Granulositik sarkom, cok yaygin olarak, akut myeloid loseminin klinik bir baslangic bulgusu olarak kendini gosterebilir. 42 yasinda olan ve bogaz agrisi nedenli klinigimize basvuran olgumuzun fizik muayenesinde tonsiller hipertrofik, hepatosplenomegali ve multipl konglemere ozellikte lenfadenopatileri mevcuttu. Laboratuvar incelemesinde, hemoglobin 7.2 gr/dl, lokosit 144x109/L platelet 36x109/L saptandi. Periferik yayma ve kemik iligi aspirasyon bulgulari ile akut veya kronik myeloid losemi dusunulmeyen hastaya lenfoma on tanisi ile eksizyonel lenf nodu biyopisi yapildi ve biyopsi sonucu granulositik sarkom ile uyumlu saptanarak 7+3 remisyon-induksiyon kemoterapisi baslandi. Hasta progresyon ve sepsis nedenle ex oldu. Sonuc olarak; lokomoid reaksiyonla gelen ve patolojik lenfadenopatileri olan hastalarda tabloya granulositik sarkomun eslik edebilecegi dusunulmeli ve doku tanisi ile on tani desteklenmelidir.

Imatinib mesylate, BCR-ABL proteinini hedef alan ilk secici tirozin kinaz inhibitorudur (TKI). Nilotinib ve dasatinib ise imatinib direncli KML tedavisinde kullanilan 2.kusak tirozin kinaz inhibitorleridir. Bu tirozin kinaz inhibitorlerinin en sik gorulen yan etkileri miyelosupresyon, bulanti, kusma, ishal ve grade 1-2 cilt dokuntuleridir. Oz: Santral sinir sistemi (SSS) lenfomalari primer ve sistemik tutulumun eslik ettigi sekonder lenfomalar olarak ikiye ayrilir. Bu calismada SSS lenfomalarinin klinik ve manyetik rezonans goruntuleme (MRG) bulgularini literatur bilgileri esliginde sunmayi amacladik. Aralik 2011 ile Haziran 2017 arasinda farkli histolojik subtiplere sahip SSS lenfoma tanisi olan hastalarin klinik ve kontrastli serebral MRG incelemeleri retrospektif degerlendirildi. Hastalarin demografik ozellikleri, lenfoma subtipleri, immunitesi, serum laktat dehidrogenaz degerleri, klinik ve MRG bulgulari kaydedildi. MRG goruntulerde lezyon sayisi, lokalizasyonu, sinyal ozellikleri, difuzyon ozellikleri, kontrastlanma paterni, hemoraji ve kelebek gorunumu varligi degerlendirildi. Calismaya SSS lenfoma tutulumu olan 23 hasta dahil edildi. (erkek/kadin:9/14, yas ortalamasi 53.6±18.8 dagilimi 15-81) ve primer (n=3,%13) ve sekonder (n=20, %87) olarak iki gruba ayrildi. Hastalarin tumunde non-Hodgkin lenfoma ve subtip olarak en sik diffuz buyuk B hucreli lenfoma (%65.2) saptandi. En sik semptom bas agrisi ve halsizlikti. SSS lenfomalarinda siklikla parankimal (%56.5) ve leptomeningeal (%47.8) tutulum izlendi. Primer SSS lenfomalarinda parankimal tutulum butun hastalarda izlenirken, leptomeningeal tutulum sekonder SSS lenfomalarinda daha sik saptandi (%55). SSS lenfomali 13 hastada 33 parankimal lezyon saptandi. Parankimal lezyonlar cogunlukla supratentoryal (%87.9) ve lokalizasyon olarak da en sik beyaz cevher (%57.6) yerlesimliydi. Primer SSS lenfoma grubunda tum lezyonlar T1 agirlikli goruntulerde (AG) hipointens, T2AG’de heterojen hipo-hiperintens ve difuzyon kisitlamasi izlenirken cogunlukla homojen noduler kontrastlanma mevcuttu(%83). Sekonder SSS lenfoma grubunda ise lezyonlarin cogunda T1AG’de hipointens (%74.1), T2AG’de hiperintens (%51.9), difuzyon kisitlamasi (%96.3) ve cogunlukla homojen noduler (%55.6) kontrastlanma izlendi. SSS lefomalarinda klinik ve radyolojik goruntuleme ozelliklerinin bilinmesi; tanida, prognozda, tedavi cevabini degerlendirmede ve sagkalimi belirlemede oldukca onemlidir.

Recovery and immunity to hepatitis B virus (HBV) are marked by antibody to hepatitis B core antigen (anti-HBc) with or without antibody to hepatitis B surface antigen (anti-HBs) in the absence of hepatitis B surface antigen (HBsAg). In the profoundly immune compromised individual, HBV may reactivate even in the presence of serologic evidence of resolved infection. The loss of anti-HBs followed by reactivation with development of HBsAg is known as reverse sero-conversion. A 62-year-old female patient with the diagnosis of IgG type of multiple myeloma had received bortezomib-based chemotherapy, and autologous hematopoietic stem cell transplantation (HSCT) was performed thereafter. A 56-year-old male patient with the diagnosis of chronic lymphocytic leukemia had received 6 cures of rituximab-endoxan chemotherapy. Prior to chemotherapy, HBsAg was negative, anti-HBs positive, anti- HBc positive and HBV-DNA was negative in both patients. Approximately one year after chemotherapy, HBV reverse sero-conversion developed in both patients. Resolved HBV infection with undetectable HBV-DNA before chemotherapy or HSCT did not confer HBV reverse sero-conversion. Prior to the creation of regular follow-up or prophylaxis schemes of patients with resolved HBV infection, in whom immune suppressive and anti-cancer treatments or HSCT will be performed, close follow-up of patients for HBV reverse sero-conversion even in late stages after immune suppressive and anti-cancer treatments or HSCT seems beneficial, especially in regions with intermediate or high endemicity for HBV.

The pathogenesis of chronic spontaneous urticaria (CSU) is incompletely understood. There is a growing interest in the role of the coagulation cascade in chronic urticaria. Rotational thromboelastometry (ROTEM) assay enables the global assessment of coagulation status. In the present study, we aimed to test the coagulation profile in children with CSU using ROTEM and correlate these parameters with those of a healthy group. A total of 24 children with active CSU (11 girls and 13 boys) 8 to 17 years of age and age-matched and sex-matched 30 healthy control participants were enrolled in the study. ROTEM assays (intrinsic thromboelastometry and extrinsic thromboelastometry) were used to measure and analyze coagulation time, clot formation time, and maximum clot firmness. The CSU patients and controls did not differ in age, sex, erythrocyte, neutrophil, and platelet counts. Also, ROTEM parameters did not show any difference between the 2 groups. ROTEM is increasingly being used as a tool for monitoring coagulation status. In this study, ROTEM parameters did not show any difference between CSU patients and the healthy group. Further studies are needed to confirm our findings on a larger number of CSU patients.

Hereditary spherocytosis (HS) is a familial hemolytic disorder associated with a variety of mutations that lead to defects in red blood cell (RBC) membrane proteins. There is increasing evidence that hypercoagulability occurs in chronic hemolytic anemia. In this study, changes in the coagulation profile in children with HS were investigated using rotational thromboelastometry. A total of 21 children with HS and 28 healthy children were enrolled in the study between October 2010 and October 2018. Complete blood count, prothrombin time, activated partial thromboplastin time, and fibrinogen level were ascertained, while rotational thromboelastometry assays were used to measure and analyze coagulation time, clot formation time, and maximum clot firmness. Akut miyeloid losemi (AML)li cogu hastada losemik hucreler kemoterapi sonrasi kaybolur. Ancak minimal kalinti hastalik (MKH) nedeniyle losemi nuksu gozlenebilir. MKH’nin akim sitometrik olarak takibi prognostik acidan bilgi saglar. Fakat relaps aninda immunfenotipik kaymalar olabilir ve akim sitometri ile MKH degerlendirilmesini kisitlayabilir. Bu calismada AML hastalarindaki antijen degisikliklerinin saptanmasi amaclanmistir. Calismada Eylul 2002 ve Kasim 2016 arasinda AML tanisi alan 19-77 yas arasi geriye donuk olarak degerlendirildi. Kemik iligi ornekleri tani ve relaps aninda elde edildi. Hastalarin 34’u de novo, 6’si sekonder AML idi. Kemik iligi ornekleri K3EDTA iceren tuplere alindi. Phycoerhtyrine (PE) ve fluorescein isothiocyanate (FITC) (eBioscience and BD Bioscience, San Jose, California) yuzey antijenleri laboratuvarimizda kullanilan rutin panele gore kullanildi. Antijen degisiklikleri 2 (n=7), 3 (n=6), 4 (n=6), 5 (n=4) ya da 6 (n=1) antijende gozlendi. Antijen degisiklikleri 18 antijenin 16’sinda (%88.9) saptandi. Hicbir hastada degisiklik gozlenmeyen antijenler sadece CD20 ve CD45’ti. AML’li hastalarda relaps sirasinda immunfenotipik kayma sikligi yuksektir. MKH degerlendirilirken relapsta gelisen antigen degisiklikleri goz onunde bulundurulmalidir.

Background: Circulating endothelial cells (CEC) are defined in conditions which vascular damage is seen in course of disease such as systemic vasculitis, coronary artery disease and chronic renal failure. CEC is thought to be an indicator of vascular damage (1). Behcet disease (BD) is a systemic vasculitis mostly known with recurrent oral and genital ulceration, uveitis and mucocutaneus lesions. On the other hand vascular involvement such as deep venous thrombosis, cerebral sinus thrombosis and pulmonary artery aneurysm, is an important clinical finding of disease which may cause mortality (2). Objectives: Our aim in this study was to analyse CEC levels in patients with Behcet disease, to compare them between patients with vascular (Group 1) and mucocutaneous (Group 2) involvement. Also we have compared the results of Behcet patients with patients with thrombosis due to other causes (Group 3) and healthy controls (Group 4). Each group involved 20 participant. Methods: Blood samples of the patients and healthy controls are drawn into tubes containing etylene-diamine-tetra-acetic acid (EDTA). A panel of monoclonal antibodies, including anti-CD45 to exclude hematopoietic cells, anti-CD31, -CD34, -CD36, -CD105, -CD106, -CD133,and –CD 146 and appropriate analysis gates were used to enumerate resting and activated CECs and circulating and endothelial progenitor cells (CEP). A hundred microlitre complete blood was added and incubated for 20 minutes at room temperature in the dark. After incubation for 10 minutes with erythrocyte lysing solution at room temperature, centrifugation at 1,800 rpm for 5 minutes was performed. Supernatant was removed and washed with phosphate buffer saline (PBS) for two times. Pellet was resuspended with PBS and 300.000–400.000 cells were counted with FACSCalibur flow cytometry device. Results: Mean age, sex distribution and duration of disease were similar in all groups (Table 1). CEC levels did not show a statistically significant difference between all groups. CEPs, activated CECs (aCEC) and resting CECs (rCEC) were also compared between groups. CEPs were higher in Behcet patients with thrombosis similar to patients with thrombosis due to other causes (p:0.042). Activated CECs levels did not show a difference between groups (p>0.05). Resting CECs are higher in Groups 1 and 3 than Groups 2 and 4. The detailed analysis of CEC, CEP, activated and resting CECs between groups is listed in Table 2. Conclusion: Increased levels of aCECs may be an indicator of active vascular involvement in BD. But in the current study aCEC levels did not show a difference between groups but none of the patients had active vascular involvement which may cause of this sameness. CEP and resting CEC levels were elevated in both groups of patients with thrombosis. So CEC may be a marker for vascular damage but it is not specific for BD. Aim: The most seen complication of multiple myeloma (MM) is renal insufficiency (RI). Although MM is known as one of the causesof reversible end-stage renal disease, these patients are usually not suitable for autologous stem cell transplantation (ASCT). We aimed in this study to reveal the clinical course of MM patients with renal insufficiency that underwent ASCT. Materials and Methods: We included 25 MM patients with RI who has undergone ASCT in this study. Creatinine levels more than 2 mg/dL was defined as renal insufficiency at the time of diagnosis. For survival analysis, we included patients with a minimum 100 days post-transplantation follow-up.Results: Median age was 56.6±7.9 (42-65) years. Melphalan was given 140 mg/m 2 . Nine patients (36%) required dialysis at the time of diagnosis. Six patients became dialysis-free with induction therapy. After ASCT, none of the patients needed dialysis. Cox regression analysis showed a significant increase in disease-free survival (DFS) and overall survival (OS) times compared to patients with very good partial response in patients with complete response on the 100 th day of ASCT (for DFS; 15.7 months vs 63.7 months, p=0.009 and for OS; 37.9 months vs 97.3 months p=0.01, respectively). Conclusion: There are studies showing that ASCT performed with reduced dose of melphalan is a renal protective and effective method in patients with renal impairment. Our study confirms this thesis, as well as emphasizing the significant effect of CR on DFS and OS on the 100 th day after transplantation in these patients.
