Summary: Background: The protection of fourth dose mRNA vaccination against SARS-CoV-2 is relevant to current global policy decisions regarding ongoing booster roll-out. We aimed to estimate the effect of fourth dose vaccination, prior infection, and duration of PCR positivity in a highly-vaccinated and largely prior-COVID-19 infected cohort of UK healthcare workers. Methods: Participants underwent fortnightly PCR and regular antibody testing for SARS-CoV-2 and completed symptoms questionnaires. A multi-state model was used to estimate vaccine effectiveness (VE) against infection from a fourth dose compared to a waned third dose, with protection from prior infection and duration of PCR positivity jointly estimated. Findings: 1298 infections were detected among 9560 individuals under active follow-up between September 2022 and March 2023. Compared to a waned third dose, fourth dose VE was 13.1% (95% CI 0.9 to 23.8) overall; 24.0% (95% CI 8.5 to 36.8) in the first 2 months post-vaccination, reducing to 10.3% (95% CI −11.4 to 27.8) and 1.7% (95% CI −17.0 to 17.4) at 2–4 and 4–6 months, respectively. Relative to an infection >2 years ago and controlling for vaccination, 63.6% (95% CI 46.9 to 75.0) and 29.1% (95% CI 3.8 to 43.1) greater protection against infection was estimated for an infection within the past 0–6, and 6–12 months, respectively. A fourth dose was associated with greater protection against asymptomatic infection than symptomatic infection, whilst prior infection independently provided more protection against symptomatic infection, particularly if the infection had occurred within the previous 6 months. Duration of PCR positivity was significantly lower for asymptomatic compared to symptomatic infection. Interpretation: Despite rapid waning of protection, vaccine boosters remain an important tool in responding to the dynamic COVID-19 landscape; boosting population immunity in advance of periods of anticipated pressure, such as surging infection rates or emerging variants of concern. La lectura de Hombres de caminos (1988), novela del escritor piurano Miguel Gutiérrez Correa, ha transitado por juicios valorativos generales; no existe, sin embargo, un estudio crítico profundo, a pesar de que han pasado más de tres décadas desde su publicación. En este artículo, se postula que esta novela establece una continuidad de la violencia colonial encarnada en la figura/imagen del bandolero/bandolerismo. Para comprobar esta hipótesis, en primer lugar, se explicará que dicha continuidad se perpetúa como núcleo temático a partir de la narración de la gesta bandoleril en Piura, de tal modo que la novela va a construir su narrativa a partir del movimiento del sujeto mestizo bandolero del siglo XIX hacia un sujeto mestizo indígena de los inicios de la Colonia (o viceversa). En dicho desplazamiento, se evidenciará que el discurso de la lucha de clases que se asoma a partir de la gesta del bandolerismo es en realidad una prolongación del discurso de las razas del siglo XVI. En segundo lugar, a nivel de estructura, se demostrará que la continuidad se establece a través de un encadenamiento narrativo que simula las “polémicas de las Indias” y las tensiones entre la oralidad que se llevaron a cabo durante la época colonial.

Studies indicate that training in the surveillance of child development can aid in the identification and/or prevention of deficiencies. The more people involved (family members, caregivers, health professionals, and educators), the better the quality of child development. The objective of this project was to promote health surveillance of the development of children in primary care, in association with family health teams in São Carlos, by means of the training of community health workers (CHWs) and occupational therapy graduate students at the Federal University of São Carlos. The extension activity was based on meaningful learning, divided into three phases. Firstly, the researchers taught the students how to use the booklet "Every time, it's time to care." In the second phase, students undertook the training of CHWs, and in the third phase, the CHWs will use the contents of the booklet to work with families in their areas. The first two phases of the project have so far been completed. Problem-solving workshops used the booklet as supporting material, and considered nine key themes: care for pregnant women, bonds and affection, vaccination schedule, child development, nutrition, hygiene, management of childhood illness, accident prevention, and rights of the child. These important issues encountered in everyday life enabled the participants to identify the themes with aspects of their personal lives, and to connect theory with practice. The problem of prompt detection of problems related to early childhood care was also highlighted. The final phase of the work is being completed, and is expected to help to enhance the empowerment of families.

The objective of this integrative revision is to understand the Brazilian scientific production of the thematic focus on the family quality of life (FQoL) of people with disabilities, as well as the participating population, referential theory, and instruments used in the studies. The data survey was done in the BVS, Scielo, and PubMed databases, and in the Capes' Periodic portal, whose inclusion criteria were articles published from 2007 to 2018, available fully online, in Portuguese or English. These articles portrayed the theme of life quality of families of people with disabilities and articles from research done in Brazilian contexts. The results showed that only three of the 19 selected articles to the study analyzed the theme of FQoL directly including the aspects related to the construct. They also pointed out that WHOQOL-Bref was the most used instrument to evaluate the FQoL (n=11), followed by semi structured interviews (n=5) and that only 10,5%, i.e., two articles used the appropriated definition of the FQoL. The research determined that the thematic research is still in the beginning stage in the scientific national production, large use of individual constructs of life quality, and sparse presence of specific measures instruments to measure FQoL.
