15. Åtgärdsvalsstudie Ostkustbanan Stockholm-Uppsala : tidhållning tågtrafik

Abstract

I åtgärdsvalsstudien (ÅVS) redovisas åtgärder som Trafikverket föreslås genomföra för att förbättra leveranskvaliteten avseende punktlighet och robusthet på järnvägssträckan Stockholm-Uppsala utifrån de brister som identifierats. Åtgärdsförslagen bedöms kunna genomföras på kortare sikt (inom cirka sju år). Tidsperioden som studerats är år 2019-2022. Studien har genomförts över verksamhetsområdesgränserna inom Trafikverket och i samverkan med SJ, Green Cargo, MTR Pendeltågen och Mälartåg, Svensk kollektivtrafik, Mälardalstrafik, Region Uppsala, Region Stockholm och Tågföretagen. Brister har identifierats genom såväl kvantitativa (bearbetning av data från uppföljningssystemen Lupp och ofelia) som kvalitativa (möten med arbets- och referensgrupperna samt intervjuer) analyser. Genereringen av åtgärder har tagits fram på ett likartat sätt. Åtgärder som förordas i rapporten är indelade i infrastrukturåtgärder respektive åtgärder av administrativ karaktär. Åtgärder i infrastrukturen avser konkreta åtgärder för utpekade anläggningsdelar, men också åtgärder som antingen minskar risken för att störningar uppstår eller åtgärder som kan bidra till att minska konsekvenser av inträffade störningar. Åtgärder av administrativ karaktär avser bland annat tydliga mål som hänger ihop nationellt och lokalt/regionalt och som är nedbrutna till verksamheter, vilket bidrar till att styra mot och verka för måluppfyllelse. Här tas också analysarbete, rutiner, arbetssätt och samverkan, samt sådant som rör baskontraktet upp. Det förespråkas att samtliga åtgärder ska hanteras samlat i ett åtgärdspaket. Detta eftersom orsakerna till merförseningar och inställda tåg visar på stor variation vilket leder till att det inte finns enstaka åtgärder som löser alla brister. Åtgärdspaketet bör genomföras inom ett samlat projekt i samverkan mellan alla berörda delar av Trafikverket. För att nå framgång i detta projekt är det helt nödvändigt att genomförandet planeras i samverkan med berörda järnvägsföreta

17. The Impact of Gastroparesis on Surgical Therapy for Gastroesophageal Reflux Disease

Abstract

Gastroesophageal reflux disease (GERD) and gastroparesis frequently present together. This concurrence represents both a diagnostic and surgical management challenge. There is controversy regarding the etiological relationship between these conditions, which extends into controversy on the proper surgical management of concurrence. Patients with gastroparesis and medically refractory GERD are often referred for consideration of antireflux surgery (ARS), but there is disagreement as to whether gastroparesis impacts surgical outcome. This review critically evaluates data concerning whether gastroparesis affects reflux control, the impact of ARS on gastroparesis symptoms and gas-bloat syndrome, physiologic changes in gastric emptying after ARS, and the role of pyloric drainage surgery in the patient undergoing ARS. Many of the studies on these topics are limited in sample size and not designed to specifically determine the impact of gastroparesis on ARS outcomes. Consequently, the available data is heterogeneous, and studies draw conflicting conclusions. Based on this controversial and limited literature, clinical reasoning, and anecdotal experience, we discuss surgical management of concurrent GERD and gastroparesis.

Abstract

BACKGROUND: The notion that gas-bloat syndrome (GBS) after magnetic sphincter augmentation (MSA) is less detrimental has not been substantiated by data. This study aimed to identify the incidence, natural history, risk factors, and impact on outcomes of GBS after MSA. STUDY DESIGN: Records of patients who underwent MSA at our institution were reviewed. GBS was defined as a score of 4 or more on the gas bloat-specific item within the GERD health-related quality-of-life (GERD-HRQL) questionnaire. Preoperative clinical and objective testing data were compared between those with and without GBS at 1 year using univariate followed by multivariable analysis. GBS evolution over time and its impact on outcomes were assessed in those with 1- and 2-year follow-up. RESULTS: A total of 489 patients underwent MSA. At a mean (SD) follow-up of 12.8 (2.1) months, patient satisfaction was 88.8%, 91.2% discontinued antisecretory medications, and 74.2% achieved DeMeester score normalization. At 1 year, 13.3% of patients developed GBS, and had worse GERD-HRQL scores and antisecretory medication use and satisfaction (p < 0.0001). DeMeester score normalization was comparable (p = 0.856). Independent predictors of GBS were bloating (odds ratio [OR] 1.8, p = 0.043), GERD-HRQL score greater than 30 (OR 3, p = 0.0010), and MSA size 14 or less beads (OR 2.5, p = 0.004). In a subgroup of 239 patients with 2-year follow-up, 70.4% of patients with GBS at 1 year had resolution by 2 years. The GERD-HRQL total score improved when GBS resolved from 11 (7 to 19) to 7 (4 to 10), p = 0.016. Patients with persistent GBS at 2 years had worse 2-year GERD-HRQL total scores (20 [5 to 31] vs 5 [3 to 12], p = 0.019). CONCLUSIONS: GBS affects 13.3% of patients at 1 year after MSA and substantially diminishes outcomes. However, GBS resolves spontaneously with quality-of-life improvement. Patients with preoperative bloating, high GERD-HRQL scores, or small MSA devices are at greatest risk of this complication. [ABSTRACT FROM AUTHOR]

21. Sliding and Paraesophageal Hiatal Hernias are Distinct Diseases: Surgeons are Responsible for Delineating the Differences

Abstract

The outcomes for sliding and paraesophageal hiatal hernias (PHH) are jointly reported. By combining outcomes of sliding hernia and PHH repair, surgeons are conflating the outcomes of surgical management for GERD with the outcomes of PHH repair. PHH is a distinct clinical entity from sliding hernia, requiring a more complex operation with higher risk of complications and, ultimately, comparatively diminished outcomes. The practice of combining PHH and sliding hernia surgical data confounds the true outcomes of ARS and contributes to misconceptions that reduce referral rates. Current and future research assessments must be based on accurate discrimination between the 2 anatomic presentations. Surgeons have a responsibility to ensure this distinction is clearly drawn when disseminating their ARS outcomes.

24. Abdominal Wall Hernias in Patients with Hiatal Hernias: Predictors of Association in a Large Cohort of Patients

Abstract

Introduction: Hiatal hernia and abdominal wall hernia (AWH) have a common theme of pathogenesis and etiology. This suggests a potential relationship between these 2 types of hernia. However, no large studies have examined such an association. We designed the current study to characterize this association and identify factors associated with this coexistence.Methods: A database of patients seen at a large multi-center health network serving 3 states in the mid-Atlantic region of the United States was queried for patients with a documented diagnosis of hiatal hernia (HH) or paraesophageal hernia (PEH). This population was then divided into groups based on whether or not they had a history of another type of AWH (ventral, inguinal/ femoral, umbilical, or incisional hernia). Demographic and clinical data were compared between groups using univariate, followed by multivariable logistic analysis.Results: The final study population consisted of 28 114 patients (66.5% female) with HH/PEH. The prevalence of AWH in this population was 8.2% (n = 2299). Of these 69.1% had only one AWH with the remaining 30.9% having two or more hernias. Multivariable analysis showed male patients with HH/PEH were more likely to have AWH (OR: 1.97; 95% CI: 1.810-2.162, P<0 .0001). The odds of AWH were higher in patients aged 50 to 80 years old compared to younger patients (OR: 1.527; 95% CI: 1.348-1.730, P<0 .0001). Patients with any comorbidity were also more likely to develop AWH (OR: 1.764; 95% CI: 1.566-1.986, P< 0.0001), with obstructive sleep apnea (OSA) being the comorbidity with the highest risk (OR: 1.295; 95% CI: 1.161-1.444, P<0 .0001), followed by obesity (OR: 1.267; 95% CI: 1.156-1.388, P<0 .0001).Conclusion: The prevalence of an AWH in patients with a HH/PEH was 8.2%. Although HH/PEH are more prevalent in females, males with HH/PEH are at higher risk for AWH. Obesity, OSA, tobacco use, hypertension, diabetes, and hypothyroidism are independent risk factors for presence of an AWH in patients with HH/PEH.

Abstract

Neurodegenerative diseases are one of the most important contributors to morbidity and mortality in the elderly. In Europe, over 14 million people are currently living with dementia, at a cost of over 400 billion EUR annually. Recent advances in diagnostics and approval for new pharmaceutical treatments for Alzheimer's disease (AD), the most common etiology of dementia, heralds the beginning of precision medicine in this field. However, their implementation will challenge an already over-burdened healthcare systems. There is a need for innovative digital solutions that can drive the related clinical pathways and optimize and personalize care delivery. Public-private partnerships are ideal vehicles to tackle these challenges. Here we describe the Innovative Health Initiative (IHI) public-private partnership project PROMINENT that has been initiated by connecting leading dementia researchers, medical professionals, dementia patients and their care partners with the latest innovative health technologies using a precision medicine based digital platform. The project builds upon the knowledge and already implemented digital tools from several collaborative initiatives that address new models for early detection, diagnosis, and monitoring of AD and other neurodegenerative disorders. The project aims to provide support to improvement efforts to each aspect of the care pathway including diagnosis, prognosis, treatment, and data collection for real world evidence and cost effectiveness studies. Abstract

Magnetic sphincter augmentation (MSA)was introduced as an alternative to laparoscopic Nissen fundoplication (LNF). This reproducible, outpatient procedure addresses the etiology of gastroesophageal reflux disease by implanting a ring of magnetic beads across the esophagogastric junction (EGJ). MSA is designed to resist effacement of the lower esophageal sphincter (LES) and, similar to LNF, results in restoration of anti-reflux barrier competency by increasing overall length, intraabdominal length and resting pressure of the sphincter. However, the novel use of magnets to augment the physiology of the LES poses unique challenges to the physiology of the EGJ and esophagus. These impacts are best revealed through manometry. The degree of restrictive forces at the EGJ, as measured by intrabolus pressure and integrated relaxation pressure, is higher after MSA compared with LNF. In addition, contrary to the LNF, which retains neurohormonal relaxation capability during deglutition, the magnetic forces remain constant until forcibly opened. Therefore, the burden of overcoming EJG resistance is placed solely on the esophageal body contractile force, as measured by distal contractile integral and distal esophageal amplitude. The main utility of preoperative manometry is in determining whether a patient's esophagus has sufficient contractility or peristaltic reserve to adapt to the challenge of an MSA. Manometric thresholds predictive of MSA outcomes deviate from those used to define named Chicago Classification motility disorders. Therefore, individual preoperative manometric characteristics should be analyzed to aid in risk stratification and patient selection prior to MSA. [ABSTRACT FROM AUTHOR]

Abstract

Magnetic sphincter augmentation (MSA) was introduced in 2007 as an alternative surgical procedure for patients with gastroesophageal reflux disease (GERD). The majority of data since MSA's introduction has focused on short and intermediate-term results, demonstrating safety and high efficacy in terms of reflux symptom control, freedom from proton pump inhibitor use and normalization of distal esophageal acid exposure. However, GERD is a chronic condition that demands a long-term solution. Limited available data from studies reporting outcomes at 5 years or later following MSA demonstrate that the promising short- and mid-term efficacy and safety profile of MSA remains relatively constant in the long term. Compared with Nissen fundoplication, MSA has a much lower rate of gas-bloat and inability to belch at a short-term follow-up, a difference that persists in the long-term. The most common complaint after MSA at a short-term follow-up is dysphagia. However, limited data suggest dysphagia rates largely decrease by 5 years. Dysphagia is the most common indication for dilation and device removal in both early- and long-term studies. However, the overall rates of dilation and removal are similar in short- and long-term reports, suggesting the majority of these procedures are performed in the short-term period after device implantation. The indications and standard practices of MSA have evolved over time. Long-term outcome data currently available are all from patient cohorts who were selected for MSA under early restricted indications and outdated regimens. Rapporten beskriver nuvarande bromsprocenttabeller, deras historiska bakgrund, nuvarande tillämpning samt en modell för framtagning av bromsprocenttabeller enligt den svenska modellen. Rapporten behandlar inte förhållande på sträckor med ERTMS/ETCS. Järnvägsfordons största tillåtna hastighet (sth) utifrån bromsförmåga vid trafikering anges i bromsprocenttabeller. Bromsprocenttabeller har ett direkt samband med projekteringsregler för signalsystemet, förbeskedsavstånd och tillhörande retardationstabeller.

42. Potentiating V gamma 9Vd2 T cell proliferation and assessing their cytotoxicity towards adherent cancer cells at the single cell level

Abstract

V gamma 9V delta 2 T cells is the dominant gamma delta T cell subset in human blood. They are cytotoxic and activated by phosphoantigens whose concentrations are increased in cancer cells, making the cancer cells targets for V gamma 9V delta 2 T cell immunotherapy. For successful immunotherapy, it is important both to characterise V gamma 9V delta 2 T cell proliferation and optimise the assessment of their cytotoxic potential, which is the aim of this study. We found that supplementation with freshly thawed human serum potentiated V gamma 9V delta 2 T cell proliferation from peripheral mononuclear cells (PBMCs) stimulated with (E)-4-Hydroxy-3-methyl-but-2-enyl diphosphate (HMBPP) and consistently enabled V gamma 9V delta 2 T cell proliferation from cryopreserved PBMCs. In cryopreserved PBMCs the proliferation was higher than in freshly prepared PBMCs. In a panel of short-chain prenyl alcohols, monophosphates and diphosphates, most diphosphates and also dimethylallylmonophosphate stimulated V gamma 9V delta 2 T cell proliferation. We developed a method where the cytotoxicity of V gamma 9V delta 2 T cells towards adherent cells is assessed at the single cell level using flow cytometry, which gives more clear-cut results than the traditional bulk release assays. Moreover, we found that HMBPP enhances the V gamma 9V delta 2 T cell cytotoxicity towards colon cancer cells. In summary, we have developed an easily interpretable method to assess the cytotoxicity of V gamma 9V delta 2 T cells towards adherent cells, found that V gamma 9V delta 2 T cell proliferation can be potentiated by media-supplementation and how misclassification of non-responders may be avoided. Our findings will be useful in the further development of V gamma 9V delta 2 T cell immunotherapy.
