Abstract

Acute appendicitis (AA) is the predominant condition responsible for acute abdominal pain across all age demographics. The purpose of this research is to determine if the hemoglobin, albumin, lymphocyte, and platelet (HALP) and modified HALP (m-HALP) scores differ between complicated and uncomplicated appendicitis in patients diagnosed with AA who have applied to the emergency department (ED). Additionally, this study aims to investigate whether HALP and m-HALP scores are superior to other biomarkers.

Abstract

In this study, the efficiency of using zeolite-based adsorbents in a solid phase extraction (SPE) procedure of alpha-tocopherol and beta-sitosterol isolation from Sunflower Oil Deodorizer Distillate (SuDOD) without pre-treatment was investigated. The results showed that 99.2% a-tocopherol and 97.3% beta-sitosterol were successfully isolated as pure fractions from SuDOD, when desilicated ZSM-5-type zeolite (DSiZSM-5) was used as adsorbent on a SPE. A simple and rapid HPLC method for simultaneous alpha-tocopherol and beta-sitosterol analysis was developed and validated according to AOAC guidelines. It was found that the inclusion of a DSiZSM-5 SPE step increased the precision of the alpha-tocopherol and beta-sitosterol analysis. In conclusion, DSiZSM-5 zeolite was proven to be an efficient adsorbent which can be used not only for the recovery of alpha-tocopherol and beta-sitosterol from SuDOD in industrial scale, but also in a laboratory scale clean-up method prior to the analysis of alpha-tocopherol and beta-sitosterol.

Methods: 20 NCCLs were restorated with Universal Single Bond (Self Etch) (3M, USA) and Grandio (Voco, Germany), 20 NCCLs were restorated with Universal Single Bond (Total Etch) and Grandio. After diode laser application, 20 NCCLs were restorated with Universal Single Bond and Grandio, 20 NCCLs were restorated with Universal Single Bond (Total Etch) and Grandio. The restorations were clinical evaluated by two examiners at baseline, for 6 and 18 months using modified United States Public Health Service (USPHS) Criteria. Chi-square test was used for statistical analyse. Results: The lowest rates of retention was found in the Group 1. There was no statistically significant difference among the groups in marginal discoloration, marginal integrity, sensitivity (P > 0.05). Conclusion: Diode laser application prior to the restoration of teeth with NCCL don't reduce the retention rate of restorations, may reduce hypersensitivity and may affect the success of restoration. However, further laboratory and clinical studies are required.

Abstract

OBJECTIVES In this study, it was aimed to investigate whether or not platelet‑rich plasma (PRP) causes intra-abdominal adhesions and therefore, whether or not PRP can be used safely in intra-abdominal operations. METHODS Of the total of 35 animals, 5 were used as donors for the preparation of platelet‑rich plasma (PRP). The surgical procedures were performed on the remaining 30 animals. These rats were randomized and divided into 3 groups of 10. In Group 1, no adhesion induction was performed. Adhesion was induced by cecal abrasion and peritoneal resection model in Groups II and IIII. In Group 2, no treatment was given. In Group 3, 1 cc PRP was applied on the cecum. The rats were sacrificed on postoperative day 21. RESULTS According to adhesion scores, the difference between the sham and PRP groups was not statistically significant. There was also no significant difference between the control and PRP groups, but the adhesion scores in the PRP group was lower than those in the control group. On histopathological evaluation, the difference between the sham and PRP groups was not statistically significant. There was also no significant difference between the control and PRP groups, but the average fibrosis and inflammation scores in the PRP group were lower than those in the control group. CONCLUSION The results of the present study have demonstrated that PRP neither reduced nor exacerbated postoperative adhesions. Thus, PRP can be used safely in experimental and clinical studies where it will be applied intra-abdominally (Tab. 2, Fig. 3, Ref. 11).

Abstract

Although paraoesophageal hernias remain 30% asymptomatic, when diagnosed they have to be operated because of their fatal complications such as ischaemia, bleeding and perforation. The mortality rate increases to twenty times in emergency cases when it is compared with elective ones. Site of gastric perforation may be directed towards bursa omentalis as well as to peritoneal cavity. A case of a volvulated paraoesophageal gastric herniation ruptured to bursa omentalis is discussed with the review of the literature.

Abstract

AIM The purpose of this study was to determine the correlation between pulp stones and renal stones. This study also aims to report associations between the presence of pulp stone and gender, age, tooth type, dental arches and sides. PATIENTS AND METHODS Data were collected through radiographic examination of bitewing radiographs of 116 kidney stone patients and a similar number of age-matched controls, referred to the Department of Oral and Maxillofacial Radiology, Faculty of Dentistry, Erciyes University. Two oral radiologists examined the radiographs to identify pulp stones. The Chi-squared and Mann Whitney U tests were used to investigate the correlations between the presence of pulp chamber calcification and age, gender, dental status and kidney stone. RESULTS Pulp chamber opacities were detected in 199 (19.3%) out of the 1031 examined teeth, and in 84 (72.4%) out of the 116 kidney stone patients. There was no statistically significant difference between the study and control group (p = 0.882). The occurrence of pulp stones was significantly higher in molars than premolars and similar prevalences were found between dental arches and sides. CONCLUSION In this study, no correlation was found between the presence of pulp stones and kidney stones in the investigated group. Therefore, the presence of pulp stones does not seem to be correlated with that of kidney stones.

Methods: Enamel specimens were prepared and randomly distributed into three groups according to fissure sealant (Aegis, Helioseal F, Helioseal Clear). Each group was then subdivided according to adhesive system (Clearfil S3, Single Bond, no adhesive). A universal testing machine was used to measure lTBS, and data were analysed using one- way ANOVA and Tukey's tests. Results: lTBS values for all Aegis subgroups were significantly lower than for comparable Helioseal F and Helioseal Clear subgroups (p < 0.05). No differences were observed in lTBS of Helioseal F and Helioseal Clear (p > 0.05). In the Helioseal Clear group, lTBS values for Single Bond were significantly higher than for Clearfil S3 (p < 0.05). However, no significant differences were found between the lTBS values of the adhesive subgroups in the Aegis or Helioseal F groups (p > 0.05). Conclusions: Sealant lTBS values may be affected by material content. The addition of an adhesive may improve lTBS values of sealant to enamel.

Abstract

Purpose: To evaluate the accuracy of dilatation and curettage (D&C) and Pipelle biopsy for the diagnosis of endometrial pathologies and determine whether the amount of endometrial tissue obtained using these techniques is sufficient for further histopathology of hysterectomy specimens. Materials and Methods: Patients undergoing hysterectomy for various indications were evaluated via Pipelle endometrial biopsy or D&C from 2009-2011. A total of 267 women were included with 78 women enrolled in the Pipelie group and 189 in the D&C group. Uterine findings were grouped as normal, hyperplasia, focal lesion, atypia, and atrophy. Histological sections from the Pipelle biopsy or D&C specimens were compared to each other and hysterectomy specimens. Results: The concordance rate between Pipelle biopsy and hysterectomy was 62% and between D&C and hysterectomy was 67%. The sensitivity of Pipelie biopsy and D&C for detecting hyperplasia was 41.7% and 45%, respectively, and for detecting atypia was 71.4% for both techniques. The sensitivity of detecting atrophic endometrial tissue was significantly higher in the D&C group at 80% compared to 37.5% in the Pipelie biopsy group (p = 0.030). All other parameters were similar in both groups. Conclusion: Pipelle biopsy and D&C were equally successful for diagnosing endometrial pathologies. Neither Pipelle biopsy nor D&C was adequate for detecting focal endometrial pathologies and endometrial hyperplasia. In contrast, both techniques were sufficient for the diagnosis of atypia. The Pipelle biopsy technique is a reasonable pre-hysterectomy procedure that is more economical, less invasive, and can easily be performed in multiple clinics. [ABSTRACT FROM AUTHOR]

37. Alarming rise of cesarean births: a single center experience.

Abstract

BACKGROUND AND OBJECTIVES, The increase in cesarean section rates requires detailed investigation worldwide. The goal of this study was to analyze the distribution of indications and rates of cesarean sections in a developing country and to introduce the measures for controlling increased cesarean deliveries. MATERIAL AND METHODS, Electronic medical records of the patients who underwent cesarean section were retrospectively evaluated between the years of 2006 and 2008. RESULTS, Total of 42,547 vaginal delivery, 104 instrumental vaginal delivery and 28357 cesarean section were performed. The instrumental delivery and cesarean section rates were 0.14 and 39.9%, respectively. The most common indication was repeat cesarean that was present in 9224 patients (32.5%) followed by fetal distress in 6427 patients (22.6%). CONCLUSIONS, Encouraging vaginal delivery for patients with previous cesarean by community based national approaches seems the leading measure to control the increased rates of cesarean section in developing countries. Abstract

Environmental guidelines in the automotive industry greatly emphasize the recycling, remanufacturing, and recovering of end-of-life vehicles (ELVs). Given the principle of extended producer responsibility, developing an effective reverse logistics network is the most significant digit ahead of the industry. However, initial attempts addressing the reverse logistics network design (RLND) problem were short-sighted, focusing on cost minimization. Undoubtedly, the whole concept of recycling was founded on the pillars of sustainability. Accordingly, reverse logistics network design must be motivated by long-term environmental and societal benefits. This fact has become even more prominent in the current pandemic environment as COVID-19 has added serious uncertainties and risks to the supply chain processes. This paper reiterates the essence of sustainability goals and proposes a multi-objective fuzzy mathematical model to RLND problem for ELVs under such a fragile and fuzzy environment. The coverage of the proposed model is to optimally determine the locations and numbers of the facilities and the flows among them concerning environmental, social, and economic aspects. Hence, the model aims to reach a robust compromise solution that leads to a resilient network design. A real case study on the ELV market in Istanbul/Turkey proves the merit of the developed model.

Objective: To examine the amount of change in color and color parameters of a composite resin (Filtek P60) polymerized by five different polymerization methods. Methods: A Teflon mold (6mm in diameter, 2mm in height) was used to prepare the composite resin discs (n=10). G1: Polymerization with inlay oven; G2: Polymerization with HQTH and autoclave; G3: Polymerization with LED and autoclave; G4: Polymerization with HQTH; G5: Polymerization with LED. Colorimetric values of the specimens before and after polymerization were measured using a spectrophotometer. The CIE L*a*b color system was used for the determination of color difference. Analysis of variance (ANOVA) was used to analyze the data for significant differences. Tukey's HSD test and paired two-tailed tests were used to perform multiple comparisons (a=.05). Results: There were no significant differences in total color change (?E*ab) among the polymerization groups (P>.05). Abstract

In this study, the efficiency of using zeolite-based adsorbents in a solid phase extraction (SPE) procedure of a-tocopherol and ß-sitosterol isolation from Sunflower Oil Deodorizer Distillate (SuDOD) without pre-treatment was investigated. The results showed that 99.2% a-tocopherol and 97.3% ß-sitosterol were successfully isolated as pure fractions from SuDOD, when desilicated ZSM-5-type zeolite (DSiZSM-5) was used as adsorbent on a SPE. A simple and rapid HPLC method for simultaneous a-tocopherol and ß-sitosterol analysis was developed and validated according to AOAC guidelines. It was found that the inclusion of a DSiZSM-5 SPE step increased the precision of the a-tocopherol and ß-sitosterol analysis. In conclusion, DSiZSM-5 zeolite was proven to be an efficient adsorbent which can be used not only for the recovery of a-tocopherol and ß-sitosterol from SuDOD in industrial scale, but also in a laboratory scale clean-up method prior to the analysis of a-tocopherol and ß-sitosterol. Abstract

Change in urine color is due to several diseases or metabolic activity. Detection of the urine color change and diagnosis of the color change related diseases are performed at the hospitals after several comprehensive tests. In this study, a urine color detecting device, which can be used at home healthcare, has been developed. The device is equipped with a homogeneous light source and a power supply that supplies power to this source, a light intensity detecting sensor, a USB-to-Serial convertor module. The device is connected to a portable computer via this module and light intensity measurements are operated by a software run on the computer. The synthesis and optical properties of the 5,5',6,6'-tetraphenyl-2,2'-bi([1,3]dithiolo [4,5-b] [1,4]dithiinylidene)-2,3-dichloro-5, 6-dicyano-p-benzoquinone (DDQ) complex thin film were investigated by the optical characterization. The optical constants such as refractive index, extinction coefficient and absorption coefficient were determined using the transmittance T(lambda) and reflectance R(lambda) spectra and the refractive index dispersion was analyzed using single oscillator of Wemple-Didomenico model. The single oscillator energy Eo and the dispersion energy E-d were calculated. The effect of temperature on refractive dispersion and optical band gap E-g is also discussed. 48. Evaluation of ST-segment and T-wave changes associated with NSTE-ACS in patients with RBBB: a nested case-control study design.

Abstract

Patients presenting with suspected acute coronary syndrome (ACS) in the emergency department (ED) require rapid and accurate electrocardiographic (ECG) evaluation. This study aims to assess conventional ECG markers for diagnosing non-ST-elevation ACS (NSTE-ACS) in patients with chest discomfort and right bundle branch block (RBBB). A nested case-control design was employed to compare patients with RBBB admitted to the ED for suspected cardiac ischemia, focusing on those who developed NSTE-ACS versus those who did not. The sample consisted of 352 patients with suspected ACS and RBBB identified on ECG. Among them, 88 were diagnosed with NSTE-ACS. In the presence of RBBB, ST-segment elevation on the isoelectric line and/or positive T-waves on the ECG were significantly associated with the diagnosis of NSTE-ACS (p < 0.05). In patients who developed NSTE-ACS, the likelihood of an isoelectric ST-segment was 3.48 (95% CI 2.07-5.82) times higher, the likelihood of positive T-waves was 4.16 (95% CI 2.51-6.91) times higher, and the combination of an isoelectric ST-segment with positive T-waves was 4.81 (95% CI 2.28-8.25) times higher (p < 0.05). In addition, ST-segment depression and non-negative T-waves were significantly more frequent in patients who developed NSTE-ACS, with odds ratios of 5.78 (95% CI 3-11.3), compared to those who did not (p < 0.05). The odds ratios for ST-segment and T-wave changes were 2.81 and 3.47, respectively, with an 80% correct classification rate for predicting NSTE-ACS. Clinicians should closely monitor the presence of an isoelectric ST-segment and positive T-waves in patients with RBBB to assess for potential cardiac ischemia., Competing Interests: Declarations. Abstract

Somatic mutations and polymorphisms may play a role in multiple myeloma (MM) susceptibility and survival. One of the immune checkpoint inhibitors is P-selectin glycoprotein ligand-1 (PSGL-1); the majority of tumor-infiltrating leukocytes express PSGL-1, causing T cell and immune inhibition via PSGL-1 mediator molecules. We aimed to investigate the effect of variable number of tandem repeat (VNTR) polymorphism in the second exon of the PSGL-1 gene on MM susceptibility, response to treatment and survival in our patient group. A total of 238 patients diagnosed with MM between January 2010 and January 2021 and 162 healthy individuals as a control group were included in this cross-sectional study. The genotypes of the VNTR polymorphism in the second exon of the PSGL-1 gene were statistically compared between patients and healthy controls; the statistically significant effects of the genotypes on response to first-line treatment and survival were examined. The AC genotype was significantly higher in healthy controls compared to patients diagnosed with MM (p < 0.001). The median PFS in patients with AA/AB/AC was 56 months, while it was 100 months in patients with BB/CC. The hazard ratio of 1.34 for PFS was found to be clinically significant and having the BB/CC genotype could provide a longer PFS compared to others, but it was not statistically significant due to the sample size. Abstract

Background: Incisional hernia is frequently observed after open colorectal cancer surgery, and should be considered a serious short- and long-term health issue. The present study evaluated the efficacy of small-bite abdominal closure in reducing the incidence of incisional hernia in this patient group., Methods: An RCT was conducted between June 2019 and June 2022. A total of 173 patients who underwent open colorectal cancer surgery were assigned randomly to one of two groups to undergo fascial closure with either small bites (87) or conventional bites (86). The incisional hernia rate was accepted as the primary outcome, and surgical-site infection as the secondary outcome., Results: The incisional hernia rates at 1 year were 7 and 27% in the small- and conventional-bite groups respectively (P < 0.001). This rate increased to 9 and 31% at the end of the second year (P < 0.001). Surgical-site infections occurred in 18% of the small-bite group and 31% of the conventional-bite group (P = 0.03). Compared with the conventional-bite group, the small-bite group had higher suture/wound length ratios (mean(s.d.) 5.18(0.84) versus 3.67(0.57); P < 0.001) and a longer fascial closure time 14.1(4.64) versus 12.9(2.39) min; P = 0.03)., Conclusion: Small-bite closure with 5-mm tissue bites placed 5 mm apart reduced the incidence of incisional hernia and surgical-site infection after open colorectal cancer surgery., (Published by Oxford University Press on behalf of BJS Foundation Ltd. 2024.)
