Objective: Testicular torsion is an urological emergency that requires early intervention. When torsion is diagnosed late, the possibility of organ loss increases. In this study, we investigated the role of preoperative hematological parameters in predicting orchiectomy. Material and Methods: A total of 136 patients who had undergone surgery due to testicular torsion were included in this study. Patients are divided into two groups as orchiectomy (n=48) and testicular fixation (n=88). The groups were compared with age, neutrophil/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR), platelet count (PLT), mean platelet volume (MPV), neutrophil count, lymphocyte count, symptoms of duration, side and the degree of torsion. Results: The mean age was 18.2 ± 9.6 in orchiectomy group and 17.2 ± 6.1 in testicular fixation group (p=0.569). The symptoms of duration was statistically higher in orchiectomy group (57.4±35.7 vs. 8.7±7.4 (p=0.001)). NLR was 5.03±2.93 in orchiectomy group and 6.72±3.51 in testicular fixation group (p=0.005). MPV was found to be significantly higher in the orchiectomy group (p=0.001). There were no differences between the groups in terms of the PLR (p=0.137), PLT (p=0.251), neutrophil count (p=0.309) and lymphocyte count (p=0.895). In multivariate analysis, the degree of torsion and the duration of symptoms were found to be predictive for orchiectomy (p=0.003, p<0.001). Objective: The aim of this study was to document the use of general anesthesia for patients who underwent percutaneous nephrolithotomy (PNL) during a 3-year period and to examine the current discussion related to anesthesia techniques in the context of the literature. Methods: Patients scheduled for PNL between 2015 and 2017 were assessed retrospectively. Patient demographic data, the characteristics of the renal stones, the duration of the operation, blood transfusion requirements, and complications of the PNL procedure and general anesthesia were evaluated. Results: A total of 521 patients were included in this study. The mean age was 48.32±0.61 years. The mean stone size was 22.48±0.47 mm. The mean duration of the operation was 106.30±1.56 minutes, and 79.07% of patients were stone-free after the procedure. The mean fluoroscopy time was 23.30±1.45 seconds and the mean irrigation fluid volume was measured as 8.70±0.23 L. The decrease in the hemoglobin and hematocrit levels after the procedure was statistically significant (p<0.0001), with a transfusion rate of 4.99%. Fever after surgery and hemorrhage not requiring blood transfusion were the major complications (13.4% and 10.74%, respectively) seen, using the Clavien classification system. Difficult intubation (1.2%), post-extubation laryngospasm (2.3%), refractory nausea (1.5%), bronchospasm (0.38%), ischemic electrocardiography changes (0.19%), and delirium (0.19%) were the major anesthesia problems. Conclusion: General anesthesia is a safe and effective method for PNL with well-known risks. Regional anesthesia techniques have also been reported in PNL procedures as an alternative to general anesthesia in recent years. Further clinical trials with large patient groups are needed to demonstrate the safety and efficacy of regional anesthesia in PNL. [ABSTRACT FROM AUTHOR]

Objective: In this study, we aimed to evaluate the change of quality of life of patients with localized and metastatic prostate cancer after treatment. Materials and methods: Twenty-eight patients (mean age 62.45±7.11 years) who were treated with radical prostatectomy and 14 patients (mean age 69.94±8.39 years) who were treated with orchioectomy between December 2007 and June 2010 were included in the study. Patients were asked to complete EORTC QOL-30 instrument before (T0) and 1st (T1), 3rd (T2), 6th (T3), and 12th (T4) months after the treatment. QOL-C30 scores were calculated and converted to 0-100 scale. Score changes of QOL-C30 were the change in scores of related time period from basal score (T1-T0, T2-T0, T3-T0, and T4-T0). Optimum time to evaluate quality of life was defined by determining the score which was closest to the basal score. In addition, the affected quality of life parameters were determined. Results: The mostly affected parameter was the social functioning in radical prostatectomy group. Quality of life got worse in each time period. Total quality of life was the most affected parameter in orchioectomy group. Quality of life significantly increased in first month measurement, and this increase continued in 3rd, 6th, and 12th months. In both groups, quality of life deteriorated from baseline mostly in 1st month after the treatment. Quality of life was closest to the basal values in 12th month measurements. Conclusion: Patients' quality of life should be assessed at 3rd month following treatment of localized and metastatic prostate cancer. Although quality of life of metastatic prostate cancer patients was initially worse than that of localized prostate cancer patients, improvement of quality of life in patients with metastatic cancer was more obvious.
