Abstract

Introduction The absence of standardized reporting for sleep medicine exams across different laboratories can lead to misinterpretation, diagnostic inconsistencies, and suboptimal treatment strategies. This document seeks to establish guidelines for the development of sleep study reports, covering recordings from studies of types 1 to 4, and represents the official position of Associação Brasileira do Sono (ABS; Brazilian Sleep Association) on the standardization of polysomnography (PSG) and cardiorespiratory polygraphies.

Abstract

This document “Proposed management model for the use of telemonitoring to positive airway pressure adherence” was prepared by a special commission of the Brazilian Association of Sleep Medicine, with the objective of recommending a follow-up model for patients undergoing positive airway pressure therapy using telemonitoring. This proposal was prepared based on a survey and analysis of the most up-to-date national and international literature and uses the best available evidence to facilitate the standardization of care by Sleep Science specialists with potential benefit for patients. Among the conclusions of the document, it is emphasized that telemonitoring is an important tool that allows health professionals trained in sleep-disordered breathing to remotely monitor PAP therapy, allowing prompt and, when necessary, daily adjustments to be made in order to increase adherence to treatment. The authors also conclude that the privacy of the data received and shared during the provision of telemonitoring must be respected by the physician or health professional trained in sleep, with the authorization of the patient and/or person responsible, who should be made aware of the short-, medium- and long-term provision of the service.

Abstract

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Associação Fundo de Incentivo à Psicofarmacologia (AFIP) A apneia obstrutiva do sono (AOS) está associada a alterações comportamentais, cognitivas, metabólicas e no sistema cardiovascular, sendo o CPAP (Continuous Positive Airway Pressure) o principal tratamento. No entanto, os benefícios do tratamento com CPAP são limitados devido à baixa adesão à terapia. Considerando-se isso, foram desenvolvidos os aparelhos de pressões flexíveis com o objetivo de aumentar o conforto e a adesão do paciente ao tratamento. Entre eles estão o Cpap C-Flex Plus, que realiza uma redução na pressão inspiratória/expiratória, e o CPAP com SensAwake, parâmetro que possibilita a redução da pressão quando se sugere que o paciente está em vigília. Estudos que avaliam parâmetros polissonográficos e de adesão desses equipamentos comparados ao CPAP Pressão Fixa são escassos. Objetivo: Comparar o efeito das diferentes modalidades de CPAP nos parâmetros do sono e na adesão em pacientes com AOS moderada e grave. Desenho do estudo: Estudo randomizado, controlado e duplo-cego. Métodos: Pacientes com diagnóstico recente de AOS e com índice de apneia e hipopneia (IAH) ≥ 20 eventos/hora, idade 30-65 anos, foram randomizados e utilizaram CPAP por 30 dias em cada modalidade (CPAP Pressão Fixa, C-Flex Plus e SensAwake), com washout de sete dias entre as modalidades. Ao final de cada modalidade, o participante realizou polissonografia (PSG) com o CPAP em uso, foram aplicados os instrumentos escala de Epworth, FOSQ, Pittsburgh e escala visual analógica e foi avaliada a adesão (relatório de adesão) ao CPAP. Resultados: Dos 23 pacientes avaliados, 15 foram recrutados para o estudo. A média de idade e o índice de massa corpórea (IMC) dos participantes foram de 42.4 ±15.1 anos e 29.9 ±4.8 kg/m2, respectivamente. A média do IAH foi de 43.5±22.9 eventos/hora. O índice de despertar (ID) foi significativamente menor no SensAwake comparado ao CPAP Pressão Fixa (10±5.2 versus 12.7±5.5 eventos/hora, p=0,05) e o índice de distúrbio respiratório (IDR) da modalidade SensAwake foi também significativamente menor quando comparado ao do C-Flex Plus (3.5±3.6 versus 5±3.1eventos/hora, p=0,02). Já a adesão à terapia foi maior no C-Flex Plus (C-Flex Plus: 5.8 ± 1.2 versus SW: 4.4 ± 1.1 versus Pressão Fixa: 4.67 ± 1.48 horas, p = 0.01). Houve melhora na qualidade de sono com o uso do CPAP Pressão Fixa, em comparação com o momento inicial (p=0,01).
