Abstract: Aim of the study Radiolabelled meta-iodobenzylguanidine (MIBG) is an effective option in treatment of neuroblastoma (NBL) tumours. We studied feasibility, toxicity and efficacy of upfront 131 I-MIBG and induction treatment in stage 4 NBL patients. Patients and methods Retrospective, multi-centre (AMC and EMC) pilot regimen (1/1/2005–2011). Newly diagnosed stage 4 NBL patients, were treated with 2 courses of 131 I-MIBG, GPOH 2004 NBL protocol, myeloablative therapy (MAT) and autologous stem cell rescue (ASCT). 131 I-MIBG was administered in a fixed dose. Response rate (RR) was defined as complete remission, very good partial response and partial response. Results Thirty-two patients, (median age [range] 2.9 [0–11.4] years), 21 received 131 I-MIBG therapy, 11 did not because of: MIBG non-avid (N = 5) and poor clinical condition (N = 6). In 95% of eligible patients 131 I-MIBG treatment was feasible within 2 weeks from diagnosis. Interval between chemotherapy courses was 25 days ( 131 I-MIBG group) versus 22 days (chemotherapy group). No stem cell support was needed after 131 I-MIBG therapy. Stem cell harvest in both groups was feasible, neutrophil recovery was comparable, but platelet recovery post MAT, ASCT was slower for 131 I-MIBG-treated patients. RR post 131 I-MIBG was 38%, post MAT + ASCT was 71% ( 131 I-MIBG group), 36% (chemotherapy group) and overall 59%. Conclusions Induction therapy with 131 I-MIBG before the HR GPOH NB 2004 protocol is feasible, tolerable and effective in newly diagnosed stage 4 NBL patients. 131 I-MIBG upfront therapy induces early responses.

Abstract: Background: Neuroblastoma is a rare malignant disease and patients with high-risk neuroblastoma have a poor prognosis. Retinoic acid has been shown to inhibit growth of human neuroblastoma cells and has been considered as a potential candidate for improving the outcome. Methods: The objective was to evaluate effects of retinoic acid after consolidation with high-dose chemotherapy and bone marrow transplantation as compared to placebo or no further treatment in patients with high-risk neuroblastoma. We searched the databases CENTRAL, MEDLINE, and EMBASE from inception to 01 October 2014 and included randomized controlled trials. Results: We identified one relevant randomized controlled trial with 50 participants receiving retinoic acid and 48 participants receiving no further therapy. There was no statistically significant difference between the treatment groups in overall survival (hazard ratio 0.87, 95% confidence interval 0.46–1.63, P=0.66) and event-free survival (hazard ratio 0.86, 95% confidence interval 0.50–1.49, P=0.59). We did not identify results for other outcomes, including toxicity. Conclusion: The difference in overall and event-free survival between treatment alternatives was not statistically significantly different. Based on the currently available evidence, we are uncertain about the effects of retinoic acid after bone marrow transplantation in patients with high-risk neuroblastoma.

In theory, rapid COJEC could reduce the risk of drug resistance and it has been considered as a potential candidate for improving the outcome. Methods: The objective was to evaluate effects of rapid COJEC compared to standard induction chemotherapy in patients with high-risk neuroblastoma. We searched the databases CENTRAL, MEDLINE, and EMBASE from inception to 11 November 2014 and included randomized controlled trials. Results: We identified one relevant randomized controlled trial with 130 participants receiving rapid COJEC and 132 participants receiving standard OPEC/COJEC induction chemotherapy. There was no statistically significant difference between the treatment groups in complete response (risk ratio 0.99, 95% confidence interval 0.71 to 1.38, P=0.94) and treatment-related mortality (risk ratio 1.21, 95% confidence interval 0.33 to 4.39, P=0.77). A statistically significant difference in favor of the standard treatment arm was identified for the following early toxicities: febrile neutropenia, septicemia, and renal toxicity. Conclusion: The differences in complete response and treatment-related mortality between treatment alternatives were not statistically significantly different. Based on the currently available evidence, we are uncertain about the effects of rapid COJEC induction chemotherapy in patients with high-risk neuroblastoma.

Abstract: A miniaturized reactor system based on a small-scale calorimeter (25 mL vessel) equipped with different chemical sensors is presented in this contribution. Apart from the sensor for the detection of the thermal power, the system was equipped with two chemical sensors. In comparison to established calorimeters, this newly designed combination as a whole significantly extends the range of application. The miniaturized chemical sensors can be chosen from a pool of potentiometric and amperometric ones, measuring pH value, redox potential, nitrate and glucose concentrations, as well as dissolved carbon dioxide and oxygen content. The miniaturized sensors were developed by the Kurt-Schwabe-Institute (Meinsberg) and the electrical circuit of the system was designed by IMM (Mittweida). The reactor was tested for batch cultivations using Paracoccus pantotrophus DSM 65 as a model culture.

Abstract: International audience; The Fourth Metronomic and Anti-angiogenic Therapy Meeting was held in Milan 24-25 June 2014. The meeting was a true translational meeting where researchers and clinicians shared their results, experiences, and insights in order to continue gathering useful evidence on metronomic approaches. Several speakers emphasised that exact mechanisms of action, best timing, and optimal dosage are still not well understood and that the field would learn a lot from ancillary studies performed during the clinical trials of metronomic chemotherapies. From the pre-clinical side, new research findings indicate additional possible mechanisms of actions of metronomic schedule on the immune and blood vessel compartments of the tumour micro-environment. New clinical results of metronomic chemotherapy were presented in particular in paediatric cancers [especially neuroblastoma and central nervous system (CNS) tumours], in angiosarcoma (together with beta-blockers), in hepatocellular carcinoma, in prostate cancer, and in breast cancer. The use of repurposed drugs such as metformin, celecoxib, or valproic acid in the metronomic regimen was reported and highlighted the potential of other candidate drugs to be repurposed. The clinical experiences from low- and middle-income countries with affordable regimens gave very encouraging results which will allow more patients to be effectively treated in economies where new drugs are not accessible. Looking at the impact of metronomic approaches that have been shown to be effective, it was admitted that those approaches were rarely used in clinical practice, in part because of the absence of commercial interest for companies. However, performing well-designed clinical trials of metronomic and repurposing approaches demonstrating substantial improvement, especially in populations with the greatest unmet needs, may be an easier solution than addressing the financial issue. Metronomics should always be seen as a chance to come up with new innovative affordable approaches and not as a cheap rescue strategy.

Abstract: At first sight the problem of the iR drop and its compensation in electrochemical polarization measurements seems to have only minor significance, but it has actually troubled electrochemists and corrosion scientists for more than a hundred years. For reducing the iR drop in the electrochemical cell, its computation, experimental determination and numerical or automatic electronic compensation numerous scientific and experimental work has been carried out and appropriate suggestions were submitted. These efforts led to commercially available potentiostats with sophisticated facilities for automatic iR compensation. Nevertheless, to date these possibilities have been utilized with a certain hesitancy. Many users underestimate the iR drop, regarding it often merely as a marginal problem, which only has to be taken into account in experiments with very high currents or extremely low conductivity of the electrolyte. Furthermore, there are also doubts and prejudices regarding modern methods of iR compensation, resulting from previous unpleasant experiences or reports on failed experiments with inappropriate equipment or imperfect methods. Reduction or automatic compensation of the iR drop is necessary more frequently than generally assumed and also in most cases possible. On the other hand the application of the different methods is still not completely uncomplicated and requires special experimental experience and care. The aim of the present work is it therefore to give a comprehensive retrospective overview of the ohmic drop problem and the relevant activities to overcome it.

Abstract: SummaryAim: 131I-meta-iodobenzylguanidine (131I-MIBG) therapy has been used in neuroblastoma treatment for many years but its value in high intensive first line treatment protocols is not exactly known. Patients, methods: Stage 4 neuroblastoma patients >1 year with 123I-MIBG positive residual disease (primary tumour and/or metastasis) after complete induction chemotherapy according to the German neuroblastoma trial NB97 were retrospectively analyzed. Results: One-hundred-eleven patients had 123I-MIBG positive residual disease after complete induction chemotherapy. Forty patients received 131I-MIBG therapy using a median activity of 0.44 GBq/kg body weight. By univariate analysis, patients who underwent 131I-MIBG therapy had a better 3-year event free survival (3-y-EFS 46±8%) and 3-year overall survival (3-y-OS 58±9%) than 71 patients without 131I-MIBG therapy (3-y-EFS 19±5%, p=0.003; 3-y-OS 43±6%, p=0.037). However, subgroup analysis of 66 patients who underwent high dose chemotherapy with autologous stem cell transplantation (ASCT) during treatment found a very similar outcome with 131I-MIBG therapy (3-y-EFS 49±9%, 3-y-OS 59±10%) and without 131I-MIBG therapy (3-y-EFS 33±9%, p=0.171; 3-y-OS 59±9%, p=0.285) due to the dominating effect of ASCT. By multivariate analysis, 131I-MIBG therapy had no impact on EFS (p=0.494) and OS (p=0.891). Only ASCT, external beam radiation therapy and MYCN amplification were important for EFS and OS. Conclusions: An independent advantage of I-131-MIBG therapy could not be proven in this retrospective analysis. The ongoing German Neuroblastoma Trial NB2004 will address the influence of 131I-MIBG therapy with emphasis on tumour dosimetry.

Abstract: M. malmoense could be cultivated in sputum samples of a 49-year-old patient with destructive pulmonary disease. The conventional antituberculous therapy (started because initially a presumptive diagnosis of tuberculosis was established) was altered to ethambutol, rifabutin, clarithromycin and ciprofloxacin, followed by a long-time therapy with azithromycin or clarithromycin. But till now it was not possible to eradicate the mycobacteria from the respiratory tract (insufficient compliance, interruptions of the therapy due to side effects, excessive smoking). Infections due to M. malmoense are rare events. Many patients have disposing underlying diseases. In most cases it is a pulmonary infection. The most frequent used antibiotics are rifampicin (or rifabutin), ethambutol and clarithromycin.

Abstract: Defekte oder funktionsbedingte Unterbrechungen in der Polymerbeschichtung von metallischen Werkstoffen sind bevorzugte Ausgangspunkte verstarkten Korrosionsangriffs. Die von diesen Stellen ausgehende Unterwanderung von Beschichtungen ist von groser praktischer Relevanz und daher auch nach wie vor Gegenstand eingehender wissenschaftlicher Untersuchungen. Neben bereits seit langerer Zeit angewandten Untersuchungsmethoden bietet die elektrochemische Rauschmessung eine weitere Moglichkeit, die Delamination und Korrosionsphanomene in der Umgebung von Defekten in Beschichtungen in Abhangigkeit von den Substrat- und Beschichtungsmaterialien, dem Korrosionsmedium, den Umgebungsbedingungen sowie weiteren Einflussgrosen in situ zu untersuchen und sehr fruhzeitig nachzuweisen. In der vorliegenden Arbeit werden Ergebnisse von derartigen Rauschmessungen an zwei sehr unterschiedlichen Korrosionssystemen vorgestellt. Electrochemical noise measurements on partly coated surfaces of different materials Defects or for a special function necessary interruptions in the polymer coating on metallic materials are preferred sites of increased corrosion attack. The delamination of coatings originating from those irregularities is of great practical relevance and thus now as before also subject of intensive scientific studies. Apart from already established research methods, the measurement of electrochemical noise offers an additional research method for assessment of delamination and corrosion phenomena around defects in the coating in situ in dependency on the substrate and coating materials, the corrosive agent and environment, as well as on other influencing parameters. In this paper results of such kind of noise measurements on two quite different corrosion systems are presented.

Abstract: Scratches or any other defects in the polymer coating on metallic materials are preferred sites of increased corrosion attack. The undermining of coatings originating from such a damage is of great practical relevance and thus now as before also subject of scientific studies. In the paper a new method for in situ measurements of corrosion processes under coatings by using a sensor array will be presented. The progress of undermining originating from a scribe was determined by resistance and impedance measurements between individual sensor elements and the metallic substrate in dependency on the substrate material and other parameters. Eine neue Methode zur Bewertung der von einem Ritz in der Beschichtung auf einem Metallblech ausgehenden Korrosion Kratzer oder sonstige Defekte in der Polymerbeschichtung metallischer Werkstoffe sind bevorzugte Ausgangspunkte fur verstarkten Korrosionsangriff. Die von einer solchen Beschadigung ausgehende Unterwanderung von Beschichtungen ist von groser praktischer Bedeutung und daher nach wie vor auch Gegenstand wissenschaftlicher Untersuchungen. In dem Beitrag wird eine neue Methode zur In-situ-Messung der Korrosionsprozesse unter Beschichtungen mittels eines Sensorarrays vorgestellt. Das Fortschreiten der von einem Ritz ausgehenden Unterwanderung wird durch Widerstands- und Impedanzmessungen zwischen einzelnen Sensorelementen und dem Metallsubstrat in Abhangigkeit vom Substratmaterial und anderen Einflussgrosen bestimmt.

Abstract: Summary Dendritic cell (DC) development and function is critical in the initiation phase of any antigen-specific immune response against tumours. Impaired function of DC is one explanation as to how tumours escape immunosurveillance. In the presence of various soluble tumour-related factors DC precursors lose their ability to differentiate into mature DC and to activate T cells. Gangliosides are glycosphingolipids shed by tumours of neuroectodermal origin such as melanoma and neuroblastoma. In this investigation we address the question of whether gangliosides suppress the development and function of monocyte-derived DC in vitro. In the presence of gangliosides, the monocytic DC precursors showed increased adherence, cell spreading and a reduced number of dendrites. The expression of MHC class II molecules, co-stimulatory molecules and the GM-CSF receptor (CD116) on the ganglioside-treated DC was significantly reduced. Furthermore, the function of ganglioside-treated DC was impaired as observed in endocytosis, chemotactic and T cell proliferation assays. In contrast to monocytic DC precursors, mature DC were unaffected even when higher doses of gangliosides were added to the culture. With regard to their carbohydrate structure, five different gangliosides (GM2, GM3, GD2, GD3, GT1b), which are typically shed by melanoma and neuroblastoma, were tested for their ability to suppress DC development and function. Suppression was induced by GM2, but not by the other gangliosides. These data suggest that certain gangliosides impair DC precursors, implying a possible mechanism for tumour escape.

Abstract: Summary Background The role of P-glycoprotein (Pgp) associated multidrug resistance for neuroblastoma patients is controversial. Therefore we asked whether at all the typical functional features of the multidrug resistance phenotype could be found in neuroblastoma cells and studied the prognostic relevance of Pgp expression. Patients and methods Tumor touch preparations and tumor cell infiltrated bosne marrow smears of 62 neuroblastoma patients were investigated. The expression of Pgp was determined by a highly sensitive immunosandwich technique. Drug resistance studies were performed by exposing cells to Pgpdependent cytostatic drugs in tissue cultures. Intracellular drug accumulation was examined by rhodamine-123 fluorescence microscopy. Results Pgp expression was demonstrable for the SK-N-SH cell line, but not detectable in CHP-100 and ten other neuroblastoma cell lines by immunocytochemistry. In tissue cultures, SK-N-SH cells showed a relative resistance to vincristine and adriamycin (45.1 and 12.7-fold resp.) and reduced intracellular accumulation of rhodamine-123 which could be normalized by the Pgp blocker verapamil. Pgp expression was detected by immunocytochemistry in 14 out of 62 tumors (22.6%). No correlation was found to the stage of the disease (P = 0.33), histopathological grading (P = 0.82), N-myc oncoprotein expression (P = 0.76) or N-myc oncogene amplification (P = 0.20). Kaplan–Meier analysis of event free survival for stage 4 tumors revealed a weak trend of inferior survival for patients with Pgp positive tumors (logrank analysis: P = 0.069). Conclusions Though Pgp expression is detectable and functional in neuroblastoma cells, but its presence does not provide much information to the complex phenomenon of chemotherapy resistance in patients.

Abstract: Electrochemical noise analysis has been used increasingly for investigation of electrochemical systems in recent years. In this paper results of noise measurements on different potentiometric electrochemical sensors are presented. The experimental arrangement consists mainly of a high-resolution multimeter and a series connected differential amplifier with a very high input resistance. Miniaturized pH glass electrodes, pH thick film electrodes, ISFET pH sensors as well as miniaturized reference electrodes have been investigated by means of electrochemical noise measurements. The results confirm that measurement of electrochemical noise can be used as an additional method for testing and characterization of electrochemical sensors.

Abstract: Tumor specimens of 203 infants with neuroblastomas of different clinical stages — registered in successive multicenter clinical trials of the German Society of Pediatric Oncology — could be examined for N-myc amplification, chromosome 1-ploidy and — structure, CD44 std. expression (in tumor tissue, and also in patient‘s sera).Eightyseven (= 43%) of these infants had a non-localized, disseminated neuroblastoma, mainly involving sympathetic nerve tissue, lymphnodes, liver, skin, bone marrow and bones (46 patients were classified into the 4s group, 41 patients in the true 4 group).If the clinical classification between stage 4 and stage 4swas neglected, then 17 of these infants (= 20%) had N-myc amplification (4—64 copies) with 16 already dead. Seven of 9 examined patients with true stage 4 had chromosome 1p aberrations (with N-myc amplification in 5), and among the dead there were 2 with CD44 negative expression.In another series, serum CD44 std. was measured by ELISA, and the highest (significantly different) Kruskal-Wallis mean rank values (147.8) were found in infants (n = 6) with stage 4s compared to the low mean-rank-value of 71.9 in patients with stage 4 (n = 65). Stage 1—3 patients (n = 42) had values of 99.8—88.6.Thus, infants with disseminated neuroblastomas, showing non-diploidy, normal chromosome 1p structure, non-N-myc amplification and high CD44 std. expression in tumor tissue, and also high CD44 std. values in serum, will have the highest chance of survival due to tumor-non-progression.On the other hand, N-myc amplification in the tumor cells was found to be characteristic for stage 4s neuroblastoma patients with tumor progression (n=6). Therefore, 4s neuroblastoma-patients with N-myc amplified tumors should be aggressively treated like true stage 4 tumor patients!

Abstract: In Germany and Austria, more than 90% of pediatric cancer patients are enrolled into nationwide disease-specific first-line clinical trials or interim registries. Essential components are a pediatric cancer registry and centralized reference laboratories, imaging review, and tumor board assistance. The five-year overall survival rate in countries where such infrastructures are established has improved from20% before 1950 to80% since 1995. Today, treatment intensity is tailored to the individual patient's risk to provide the highest chances of survival while minimizing deleterious late effects. Multicenter clinical trials are internationalized and serve as platforms for further improvements by novel drugs and biologicals.

Abstract: Electrochemical Noise of Magnesium in Organic Solvents The electrochemical noise of the system Mg/n-C4H9OH, NaClO4 is analyzed on different conditions. The suitability of this method is to be investigated for the determination of selfdischarge of galvanic cells. Gravimetric measurements, radiometric methods and visual judgement of anode surfaces are taken for this hitherto. The long-term stability of the system without polarization up to 75 days is confirmed by constant noise amplitude and comparative EIS-measurements. The system activated by constant polarization showed a corrosive attack indicated by an increasing current noise amplitude within some hours. Finally passivation occurred after advanced destruction of the oxidic surface layer. The noise amplitude diminished. Salt deposit caused this event, corroborated by SEM shottings. Analysis of potential noise in nonaqueous electrolytes is inconclusive owing to inherent noise of used reference electrodes of 2. kind.

Abstract: On the Anodic Behaviour of Magnesium in Non Aqueous Elektrolytic Solutions The paper presents results of electrochemical studies on systems of the composition magnesium/non aqueous electrolyte. By means of curentless stability- and polarization measurements as well as impedance characteristics the behaviour of the system Mg/n-C4H9OH/0,2M NaClO4 is described in particular. Cyclic voltammograms and impedance spectra demonstrate a passivating layer on the metal surface prior to the first polarization starts. It determines the anodic behaviour. Deviations in the Bode diagram at high frequencies can be traced back to spurious capacities within the measuring cell.

Abstract: Gene expression data from microarrays are being applied to predict preclinical and clinical endpoints, but the reliability of these predictions has not been established. In the MAQC-II project, 36 independent teams analyzed six microarray data sets to generate predictive models for classifying a sample with respect to one of 13 endpoints indicative of lung or liver toxicity in rodents, or of breast cancer, multiple myeloma or neuroblastoma in humans. In total, >30,000 models were built using many combinations of analytical methods. The teams generated predictive models without knowing the biological meaning of some of the endpoints and, to mimic clinical reality, tested the models on data that had not been used for training. We found that model performance depended largely on the endpoint and team proficiency and that different approaches generated models of similar performance. The conclusions and recommendations from MAQC-II should be useful for regulatory agencies, study committees and independent investigators that evaluate methods for global gene expression analysis.

Abstract: Children with malignant disease have an increased risk for bacterial infections. We investigated a possible correlation between septic episodes and decreased IgG subclass levels in 63 patients. At diagnosis 13 of 50 children showed decreased IgG subclass levels: 10x IgG4, 2x IgG1, and 1x IgG3 + IgG4 were reduced. Bone marrow infiltration by tumor cells did not increase the frequency of subclass reductions (4/25 with, 9/25 without bone marrow infiltration). The time course of subclass levels was followed during 37 febrile episodes (mainly fever of unknown origin, septicemia, pneumonia) of 23 children under cytostatic therapy. 6 patients showed transient low IgG subclasses: 2x IgG4, 1x IgG1, 1x IgG3, 1x IgG2 + IgG4, and 1x IgG1 + IgG3 + IgG4. Children with decreased IgG subclass levels appeared to occur more independently of leucopenia. In general, febrile episodes in children with subclass decreases did not last a longer period and did not occur more frequently than in children without IgG subclass deficiencies. In conclusion, the determination of IgG subclasses in cancer children at diagnosis or during chemotherapy did not add substantial information of prognostic or therapeutic relevance.

Abstract: A 69-year-old man with liver cirrhosis presented to a surgeon, complaining of pain and swelling involving the right knee joint. A cloudy yellow synovial fluid was aspirated. The examination revealed polymorphonuclear leukocytes and Listeria monocytogenes was grown in the culture. The patient was treated with oral cotrimoxazole. Further follow-up was uneventful. The literature on listerial joint infections is briefly reviewed.

Abstract: Part II demonstrates the results of MRI follow up studies of 23 children with leukemia. Differentiation between leukemic cells and normal hematopoiesis by MRI is impossible. The state of disease can only be determined together with bone marrow biopsy. Clinical remission precedes the normalization of bone marrow signal by weeks. In non-responders, however, cytological findings and MRI findings seem to correlate at the same time. During therapy, areas of osteonecrosis may develop. They do not necessarily contradict continuing complete remission. But this does not affect the prognosis and complete remission is still a possibility. Diagnosis of relapse by MRI is only possible in correlation with studies during the different states of disease especially at clinical remission.

Abstract: The results of MRI studies performed on the medullary cavity in the knee region of 15 children with leukemia and 5 healthy children are reported. By the age of a few years the signal intensities and the relaxation times of bone marrow begin to resemble fat. Early leukemic infiltration can therefore be more easily recognised in the knee region than in the spine using simple T1 weighted Spin-Echo images. We observed an abnormal signal pattern in all our patients which fell into three groups: (a) diffuse uniform (b) diffuse non-uniform and (c) patchy. We have not been able to correlate these into age, sex or the risk factor determined by clinical or laboratory methods. The diffuse patterns seem to dominate in cases of ALL, the patchy forms in AML. No correlation could be found between the blast levels established by the iliac crest biopsy and the results of MRI.

Abstract: Impedance sensors in thick film technology have been tested as a tool for testing cell-mediated effects. The screen printed Pt electrodes have a width of 250-400 mum. Electrodes and the surrounding ceramic chip substrate could be homogeneously grown with L-929 and Hela cells. The performance of a screen printed interdigitated electrode structure (IDES) was compared with that of thin film structures with the same layout geometry. The thick film impedance sensors allowed to correctly record the morphological response of confluent Hela cell layers to stimulation with histamine. A thick film conductivity sensor also revealed impedance values which were dependent on cell growth on the electrode surface, even at a very low frequency range of ap1Hz.

Abstract: (131)I-meta-iodobenzylguanidine ((131)I-MIBG) therapy has been used in neuroblastoma treatment for many years but its value in high intensive first line treatment protocols is not exactly known.Stage 4 neuroblastoma patients1 year with (123)I-MIBG positive residual disease (primary tumour and/or metastasis) after complete induction chemotherapy according to the German neuroblastoma trial NB97 were retrospectively analyzed.One-hundred-eleven patients had (123)I-MIBG positive residual disease after complete induction chemotherapy. Forty patients received (131)I-MIBG therapy using a median activity of 0.44 GBq/kg body weight. By univariate analysis, patients who underwent (131)I-MIBG therapy had a better 3-year event free survival (3-y-EFS 46 +/- 8%) and 3-year overall survival (3-y-OS 58 +/- 9%) than 71 patients without (131)I-MIBG therapy (3-y-EFS 19 +/- 5%, p = 0.003; 3-y-OS 43 +/- 6%, p = 0.037). However, subgroup analysis of 66 patients who underwent high dose chemotherapy with autologous stem cell transplantation (ASCT) during treatment found a very similar outcome with (131)I-MIBG therapy (3-y-EFS 49 +/- 9%, 3-y-OS 59 +/- 10%) and without (131)I-MIBG therapy (3-y-EFS 33 +/- 9%, p = 0.171; 3-y-OS 59 +/- 9%, p = 0.285) due to the dominating effect of ASCT. By multivariate analysis, (131)I-MIBG therapy had no impact on EFS (p = 0.494) and OS (p = 0.891). Only ASCT, external beam radiation therapy and MYCN amplification were important for EFS and OS.An independent advantage of I-131-MIBG therapy could not be proven in this retrospective analysis. The ongoing German Neuroblastoma Trial NB2004 will address the influence of (131)I-MIBG therapy with emphasis on tumour dosimetry.

Abstract: Availability of statistically sufficient numbers of tumor samples and other biomaterials in high quality together with corresponding clinical data is crucial for biomedical research. Tumor repositories from individual scientists are mostly not sufficient to satisfy these criteria, especially since pediatric tumors are rare. In 2000 three centralized tumor repositories (neuroblastoma in Cologne, nephroblastoma in Würzburg, hepatoblastoma, brain tumors in Bonn) have been established by the "German Competence Net Pediatric Oncology und Hematology". The aim was to collect biomaterial including tumor samples, normal tissue, and blood in high quality for research and diagnostic purposes at a central institution. Informed consent of the parents or patients is a prerequisite for scientific use of the samples and is requested by the therapy trial. The samples are collected according to accepted standards and shipped in the specially designed Tumorbox. The tumor repository organizes the distribution of the samples to the cooperating diagnostic laboratories. The number of collected tumor samples has increased over the years. In 2000, samples from 200 patients were collected while the patient number increased to 321 in 2005. Over the years the tumor repositories collected more than 7,150 samples (fresh frozen tumor, fresh frozen normal tissue, and blood). Through links with clinical trial databases the samples can be connected with clinical data. 12 of 14 applications for tumor material to be used in specific scientific projects have been approved by an independent supervisory board. The establishment of central tumor repositories represents a major step for biomedical research activities and quality control in pediatric oncology.

Abstract: M. malmoense could be cultivated in sputum samples of a 49-year-old patient with destructive pulmonary disease. The conventional antituberculous therapy (started because initially a presumptive diagnosis of tuberculosis was established) was altered to ethambutol, rifabutin, clarithromycin and ciprofloxacin, followed by a long-time therapy with azithromycin or clarithromycin. But till now it was not possible to eradicate the mycobacteria from the respiratory tract (insufficient compliance, interruptions of the therapy due to side effects, excessive smoking). Infections due to M. malmoense are rare events. Many patients have disposing underlying diseases. In most cases it is a pulmonary infection. The most frequent used antibiotics are rifampicin (or rifabutin), ethambutol and clarithromycin.

Abstract: The sonic hedgehog (Shh) and the Wnt signalling pathways are involved in the development of medulloblastomas (MBs), the most frequent malignant brain tumours in children. Components of these two developmental and cancer-associated pathways, including (Patched) PTCH, SMOH, adenomatous polyposis coli (APC), beta-catenin and AXIN1 show somatic mutations in sporadic MBs. In this study we analysed SUFU (human Suppressor of Fused), which acts as a negative regulator of both the Shh and Wnt signalling pathways and therefore represents a putative tumour suppressor gene, to find out if it is also involved in the pathogenesis of sporadic MBs. We screened 145 primitive neuroectodermal tumours (PNETs) including 90 classic MBs, 42 of the desmoplastic variant and two medullomyoblastomas as well as 11 MB cell lines for mutations using single-strand conformational polymorphism (SSCP) and sequencing analysis. 18% of the MBs exhibited allelic losses on chromosome 10q. In contrast to a previous report, in which truncating mutations of SUFU have been identified in 9% of MBs, we were not able to identify somatic mutations of SUFU in our large tumour panel. We uncovered single nucleotide polymorphisms (SNPs) in exon 4, 8, 11 and in intron 2 in the SUFU gene. Expression analysis by competitive reverse transcription-polymerase chain reaction (RT-PCR) revealed no difference in SUFU mRNA levels of both MB subtypes and normal foetal or adult cerebellar tissues. Our results indicate that genetic alterations of the SUFU gene, do not contribute significantly to the molecular pathogenesis of MBs.
