Zusammenfassung Hintergrund In der chirurgischen Augenheilkunde existieren in der Regel kurze Operationszeiten und somit viele Wechsel zwischen den einzelnen Operationen, die keiner spezifischen Vergütung unterliegen. Da in Kliniken der Maximalversorgung konsekutiv häufig unterschiedliche Operationen mit variabler Dauer durchgeführt werden, Notfalloperationen eingeschoben werden müssen und Weiterbildung von Kolleginnen und Kollegen praktiziert wird, ist es besonders wichtig, möglichst kurze Überleitungszeiten zu generieren, um sowohl ausreichend Operationszeit zu haben als auch möglichst viele Fälle behandeln zu können. Ziel dieser Arbeit ist es, die Effizienz der Operationsleistung einer Universitäts-Augenklinik zu evaluieren. Patienten und Methodik Die im Jahr 2021 durchgeführten Operationen der MHH-Augenklinik wurden hinsichtlich Spektrum, Anzahl, OP-Dauern, Wechselzeiten und Prozesszeiten evaluiert. Personell war jeder OP-Saal mit einer Assistenzärztin oder einem Assistenzarzt der Anästhesie, einer Anästhesiepflege, 2 OP-Pflegenden, einem Operateur und 20 % anästhesiologisch oberärztlicher Supervision ausgestattet. Auf Basis eines theoretischen Konzepts, welches einen erhöhten Personalschlüssel bei gleichbleibender Infrastruktur vorsieht, wurde berechnet, wie viele Operationen mehr durchgeführt werden können, wenn die Überleitungszeit halbiert wird, und ob der finanzielle Mehraufwand dabei kompensiert werden kann. Ergebnisse Bei insgesamt n = 2712 während regulärer Dienstzeiten (244 Arbeitstage) in 2 OP-Sälen durchgeführten Operationen (durchschnittlich täglich n = 11,1; wöchentlich n = 53,6 und monatlich n = 237,1) betrug die durchschnittliche OP-Dauer 37 min und die Überleitungszeit 43 min. Die Operationssäle wurden damit nur zu 51 % der Gesamtbetriebszeit durch chirurgische Arbeiten ausgelastet. Hauptprozeduren waren mit n = 1350 die Vitrektomien und mit n = 1308 Kataraktoperationen. Das angepasste Personalkonzept sah pro OP-Saal eine zusätzliche OP-Pflegekraft sowie für beide OP-Säle insgesamt eine zusätzliche Ärztin oder einen Arzt der Anästhesie vor; die Zusatzkosten für diesen Personalaufwand berechneten sich auf ca. 300.000 € pro Jahr. Die Halbierung der Überleitungszeit von 43 min auf 21 min durch dann mögliche überlappende Einleitung und paralleles Arbeiten, was bis dato nicht möglich ist, ergibt pro OP-Saal eine zusätzliche OP-Zeit von 100 min, sodass mindestens vier OPs zusätzlich durchgeführt werden können. Bei stringenter Durchführung und gleichen räumlichen Strukturen mit stabilen Fixkosten könnten somit n = 976 OPs pro Jahr mehr durchgeführt werden. Abzüglich der angepassten Personalkosten, der zusätzlichen Materialkosten für OP und Anästhesie von 557.042 € und den stationären Hotelleriekosten von 600.663 € bei durchschnittlichen Liegezeiten von 2,8 Tagen würde ein Mehrerlös von etwa Faktor 2,4 der zusätzlichen Personalkosten erzielt werden. Berücksichtigt werden in dieser Kalkulation die derzeitige Fallpauschale von 3739,40 € und ein durchschnittlicher Casemix-Index der Augenklinik von 0,649 (Gesamtmehrerlös 2.155.449 €; Deckungsbeitrag II 701.389 €) für das betrachtete chirurgische Patientenkollektiv im Jahr 2021. Schlussfolgerungen Eine Erhöhung des Personalaufwands im OP-Saal für chirurgische Fächer wie der Augenheilkunde mit kurzen Eingriffen und vielen Wechseln lohnt sich betriebswirtschaftlich auch für ein Universitätsklinikum, um überlappende Überleitungen durch Anästhesie und OP-Pflege zu ermöglichen und zu optimieren. Dieses sollte daher auch entgegen standardisierter Personalbestückungen der OP-Säle erwogen werden, um vorhandene Ressourcen mit ihren Fixkosten möglichst optimal zu nutzen.

Outpatient procedures at a university hospital are generally considered to be unprofitable. In the present publication we evaluate the turnover and costs of the university eye outpatient department of the Hannover Medical School (MHH) in terms of a cost unit accounting as well as providing a summary of the workload.Given the data of the hospital information system (IS-H/i.s.h.med from SAP) and a proprietary software (TimeElement), all patient contacts in the year 2019 were evaluated. The latter software is applied in a standardized manner to record the patient flow of our outpatient service in real time electronically. The total costs consist of personnel, material and room costs including infrastructure of the MHH and are compared to the flat-rate revenues according to the university outpatient contract (HSA) as well as further revenues from internal referral services, self-pay patients, outpatient surgery and cooperation contracts for intravitreal injections (IVOM).With an average full-time equivalent (FTE) headcount of 10.63 assistant physicians, 3.6 specialist physicians, and 21 nonphysicians (plus 4 Federal Volunteer Service, BUFDI) in our policlinic, we have determined € 2,927,022 in personnel costs, including overheads, for the entire year. Including infrastructure (€ 524,942), material and equipment costs with overheads and internal cost allocation of € 258.657, the total costs in 2019 resulted in € 3,710,621. In contrast, the total income in 2019 was € 3,524,737 generated through the abovementioned patient segments, resulting in a deficit of € -185,884 (5%). Our data provide evidence that regular outpatient revenues are insufficient and are mainly balanced by outpatient surgery, IVOMs and self-pay patients. In total, there were 19,453 patient contacts during regular office hours (with 17,305 billable cases). At n = 9943, the majority of the contacts were HSA visits; however, only 82% of the cases could effectively be charged due to multiple visits per quarter. The median total patient attendance was 3.21 h (mean 3.38 h). On average, 78 patient contacts were counted per working day. The analysis with TimeElement unveiled a median of n = 2 physician contacts per patient (mean n = 1.91). The median duration per interaction with a physician was 17.98 min (mean 23.23 min). For diagnostics, we counted a median of n = 2 interactions per patient (mean n = 2.31), with an entire interaction lasting a median of 18.30 min (mean 22.60 min). In total n = 37,363 individual diagnostic procedures were recorded in 2019, with SD-OCT being the primary procedure at n = 10,888.The cost/turnover calculation showed a marginal financial loss through our ophthalmological outpatient department. Thus, the costs of a university eye outpatient department in Lower Saxony do not seem to be sufficiently covered by direct outpatient revenues. Maintaining quarterly flat rates for all cases of the outpatient department would require a fee of about € 214 in our setting to remain cost neutral. Currently, the lower flat rates in the HSA area are compensated by other areas. Obviously, the high content-related workload in our setting requires a high personnel expenditure with a considerable personnel cost contribution of nearly 80%.HINTERGRUND: Allgemein gelten Hochschulambulanzen in Universitätskliniken als defizitär. In der vorliegenden Publikation wird der Ansatz unternommen, im Sinne einer Kostenträgerrechnung Umsatz und Kosten der Hochschulambulanz der Medizinischen Hochschule Hannover (MHH) zu evaluieren sowie eine Aufstellung der Arbeitsleistung zu unternehmen.Mithilfe der Daten des klinikeigenen Informationssystems (SAP) und einer eigenen Software (TimeElement), welche standardisiert angewendet wird, um den Patientenflow in unserer Hochschulambulanz in Echtzeit online zu erfassen, wurden alle Patientenkontakte des Jahres 2019 evaluiert. Die Gesamtkosten summieren sich aus Personal‑, Material- und Raumkosten inklusive Infrastruktur und werden den pauschalierten Erlösen nach Hochschulambulanzvertrag (HSA-Bereich) sowie weiteren Erlösen aus Konsilleistungen, Selbstzahlern, ambulantem Operieren und Kooperationsverträgen für intravitreale Injektionen (IVOMs) gegenübergestellt.Bei durchschnittlich 10,6 assistenzärztlichen und 3,6 fachärztlichen Stellen sowie 21 nichtärztlichen Stellen (plus 4 BUFDIs [Bundesfreiwilligendienst]) in unserer Hochschulambulanz errechnen sich 2.927.022 € Personalkosten inklusive Overhead für das Gesamtjahr. Zuzüglich der Infrastruktur (524.942 €) sowie Material- und Sachkosten einschließlich Overhead sowie interner Leistungsverrechnung (ILV) in Höhe von 258.657 € beliefen sich die Gesamtkosten in 2019 auf 3.710.621 €. Dem standen Einnahmen von 3.524.737 € aus den oben genannten Bereichen gegenüber, womit sich ein Defizit von −185.884 € (5 %) ergab. Auffallend sind die deutlich unzureichende Finanzierung der Hochschulambulanz über die Fallpauschale und die notwendige Querfinanzierung aus den Bereichen des ambulanten Operierens, der IVOMs und der Selbstzahler. Insgesamt kam es zu den regulären Sprechzeiten zu 19.453 Patientenkontakten bei 17.305 abrechenbaren Fällen. Mit n = 9943 waren der Großteil der Kontakte HSA-Besuche. Abrechenbare Fälle resultierten daraus allerdings nur in 82 % aufgrund mehrfacher Besuche pro Quartal. Die gesamte Anwesenheit betrug im Median 3,21 h (durchschnittlich 3,38 h). Durchschnittlich wurden 78 Patientenkontakte pro Arbeitstag gezählt. Dabei ergab die Analyse mittels TimeElement im Median pro Patient 2 Arztkontakte (durchschnittlich n = 1,91). Die gesamte Dauer ärztlicher Interaktionen betrug im Median 18 min (durchschnittlich 23 min). In der Funktionsdiagnostik zählten wir im Median ebenfalls 2 Interaktionen pro Patient (durchschnittlich n = 2,31), wobei die gesamten Interaktionen im Median 18 min dauerten (durchschnittlich 23 min). Insgesamt wurden innerhalb der Funktionsdiagnostik n = 37.363 Einzelleistungen im Jahr 2019 gezählt, wobei mit n = 10.888 die optische Kohärenztomographie (SD-OCT) die Hauptleistung darstellte.In einer reinen Kosten‑/Umsatz-Rechnung ergibt sich an der MHH ein leicht defizitäres Ergebnis der Hochschulambulanz. Somit erscheinen die Kosten einer universitären, augenheilkundlichen Hochschulambulanz in Niedersachsen durch die direkten ambulanten Einnahmen nicht ausreichend gedeckt zu sein. Eine Beibehaltung von quartalsweisen Fallpauschalen für alle Fälle der Ambulanz würde in unserem Setting eine Honorierung von ca. 214 € notwendig machen, um die Kosten zu decken. Aktuell werden die zu niedrigen Pauschalen im HSA-Bereich von den anderen Bereichen kompensiert. Die hohe medizinische Arbeitsleistung in unserem Setting erfordert einen ebenfalls hohen Personalaufwand mit einem erheblichen Personalkostenanteil von annähernd 80 %.

Background Diverticulosis is increasing worldwide as a public health problem. The Combined Overview on Diverticular Assessment (CODA) score, merging Diverticular Inflammation and Complication Assessment (DICA) and few clinical parameters, may reliably predict the occurrence of acute diverticulitis and surgery due to complications. Thus, the aim of the study is to confirm the value of DICA classification and to develop and validate the CODA endoscopic-clinical score. Methods A number of 2198 patients, at the first diagnosis of diverticulosis/diverticular disease were enrolled in a multicentre, prospective, international cohort study. Participants were scored according to DICA classifications. A 3-year follow-up was performed. Survival methods for censored observation were used to develop and validate the CODA score for predicting diverticulitis and surgery. Results The 3-year cumulative probability of diverticulitis and surgery was ≤4%, and ≤0.7% in CODA A; 10%and >2.5% in CODA C, respectively. The 3-year cumulative probability of diverticulitis and surgery was of 3.3% (95% CI 2.5% to 4.5%) in DICA 1, 11.6% (95% CI 9.2% to 14.5%) in DICA 2 and 22.0% (95% CI 17.2% to 28.0%) in DICA 3 (p < 0.001), and 0.15% (95% CI 0.04% to 0.59%) in DICA 1, 3.0% (95% CI 1.9% to 4.7%) in DICA 2 and 11.0% (95% CI 7.5% to 16.0%) in DICA 3 (p < 0.001), respectively. The CODA score showed optimal discrimination capacity in predicting the risk of surgery in the development (cstatistic: 0.829; 95%CI 0.811 to 0.846) and validation cohort (c-statistic: 0.943; 95% CI 0.905 to 0.981). Conclusions DICA endoscopic classification was confirmed to have a significant predictive value in terms of acute diverticulitis occurence/recurrence and risk of surgery. CODA score could provide a new risk stratification tool useful for everyday clinical practice and also with a significant public health impact in terms of treatment effectiveness and decision making. Key messages • DICA endoscopic classification of diverticular disease is a clear predictor of the outcome of diverticulosis/diverticular disease. • The CODA score, combining DICA and few clinical parameters, may reliably predict the occurence of acute diverticulitis and surgery due to complications.

In ophthalmologic surgery, there are usually short operation times and thus many changes between the individual operations, which are not subject to remuneration. As in maximum care hospitals consecutive different operations with different durations are often performed, emergency operations have to be inserted and further training of colleagues is practiced, it is particularly important to generate the shortest possible transfer times in order to have both sufficient operation time and to be able to treat as many cases as possible. The aim of this work is to evaluate the efficiency of the surgical performance of a university eye hospital.The surgeries performed in 2021 at the MHH Eye Clinic were evaluated with respect to the spectrum, number, surgery duration, transfer times and process times. In terms of personnel, each operating room was staffed with one assistant anesthesiologist, one nurse anesthetist, two operating room nurses, one surgeon, and 20% senior anesthesiologist supervision. Based on a theoretical concept, which provides an increased staffing ratio while maintaining the same infrastructure, it was calculated how many more surgeries could be performed if the transfer time was halved and whether the additional financial expense could be compensated.With a total of n = 2712 surgeries performed during regular duty hours (244 working days) in 2 operating rooms (average daily n = 11.1; weekly n = 53.6 and monthly n = 237.1), the average surgery duration was 37 min and the transition time 43 min. This means that the operating rooms were used for surgery for 51% of the total operating time. Main procedures were vitrectomy with n = 1350 and cataract surgery with n = 1308. The new personnel concept provided one additional operating room nurse per operating room and one additional anesthesiologist for both operating rooms. The additional costs for this personnel expenditure were calculated at approx. 300,000 € per year. The halving of the transfer time from 43 min to about 21 min through possible overlapping induction and parallel work, which was not possible until now, results in an additional operation time of about 100 min per operating room, so that at least 4 additional operations can be planned and performed. In this way, with stringent implementation and the same spatial structures with stable fixed costs, n = 976 more operations could be performed, which, minus the personnel costs, the additional material costs for surgery and anesthesia of 557,042 € and the inpatient hotel costs of 600,663 €, with an average length of stay of 2.8 days, would result in an additional revenue of about 2.4 times the additional personnel costs at the current flat rate of 3739.40 € and an average case mix index of the MHH Eye Hospital of 0.649 (total revenue: 2,155,449 €; profit margin II: 701,389 €) for the considered surgical patient collective in 2021.An increase of the personnel expenditure in the operating room for surgical subjects such as ophthalmology with shorter interventions and many changes is economically worthwhile also for a large hospital in order to enable and optimize overlapping transfers of anesthesia and surgical care. Background CONKO-006 was designed for patients with pancreatic adenocarcinoma with postsurgical R1 residual status to evaluate the efficacy and safety of the combination of gemcitabine and sorafenib (GemSorafenib) compared with those of gemcitabine + placebo (GemP) for 12 cycles. Patients and methods This randomised, double-blind, placebo-controlled, multicenter study was planned to detect an improvement in recurrence-free survival (RFS) from 42% to 60% after 18 months. Secondary objectives were overall survival (OS), safety and duration of treatment. Results 122 patients were included between 02/2008 and 09/2013; 57 were randomised to GemSorafenib and 65 to GemP. Patient characteristics were wellbalanced (GemSorafenib/GemP) in terms of median age (63/63 years), tumour size (T3/T4: 97/97%), and nodal positivity (86/85%). Grade 3/4 toxicities comprised diarrhoea (GemSorafenib: 12%; GemP: 2%), elevated gamma-glutamyl transferase (GGT) (19%; 9%), fatigue (5%; 2%) and hypertension (5%; 2%), as well as neutropenia (18%; 25%) and thrombocytopenia (9%; 2%). By August 2017, 118 (97%) RFS event had occurred. There were no difference in RFS (median GemSorafenib: 8.5 versus GemP: 9.4 months; p = 0.730) nor OS (median GemSorafenib: 17.6 versus GemP: 17.5 months; p = 0.481). Landmark analyses suggest that patients who received more than six cycles of postoperative chemotherapy had significantly longer OS (p = 0.021). Conclusion CONKO-006 is the first randomised clinical trial to include exclusively patients with PDAC with postsurgical R1 status thus far. Sorafenib added to gemcitabine did neither improve RFS nor OS. However, postoperative treatment exceeding six months seemed to prolong survival and should be further investigated in these high-risk patients. BackgroundIn response to the COVID-19 pandemic, endoscopic societies have recommended reduction of endoscopic procedures. In particular non-urgent endoscopies should be postponed. However, this might lead to unnecessary delay in diagnosing gastrointestinal conditions.MethodsRetrospectively we analysed the gastrointestinal endoscopies performed at the Central Endoscopy Unit of Saarland University Medical Center during seven weeks from 23 March to 10 May 2020 and present our real-world single-center experience with an individualized rtPCR-based pre-endoscopy SARS-CoV-2 testing (“PECo”) strategy.ResultsAltogether 359 gastrointestinal endoscopies were performed. The PECo strategy enabled us to conservatively handle endoscopy program reduction (44% reduction as compared 2019). The results of COVID-19 rtPCR from nasopharyngeal swabs were available in 89% of patients prior to endoscopies. Apart from six patients with known COVID-19, all other tested patients were negative. The frequencies of endoscopic therapies and clinically significant findings did not differ between patients with or without SARS-CoV-2 tests.ConclusionA reasonable reduction of the endoscopy program in the setting of structured SARS-CoV-2 testing is feasible and safe. The PECo strategy allows continuation of endoscopic procedures in a region with intermediate frequency of COVID-19 when hospital capacities are not overwhelmed by the pandemic. Thus, the study might help to develop new strategies during future waves of COVID-19 or local outbreaks.

Liver fibrosis is the common consequence of chronic liver diseases (CLD). Recently liver stiffness measurements (LSM) ≥ 9.1 kPa, as determined by transient elastography (TE), were demonstrated to predict significant fibrosis (stages ≥ F2) in a population-based setting. The PNPLA3 (adiponutrin) p.I148M polymorphism enhances the risk of liver injury. The aim of our study was to investigate the association between the procholestatic ABCB4 polymorphism c.711AT and LSM ≥ 9.1 kPa in humans as well as the interaction between ABCB4 and PNPLA3 in a mouse model of chronic cholestasis. Prospectively, we recruited 712 patients with CLD (278 women, age 50 ± 13 years) with available TE results; liver biopsy results were available in 165 individuals. none 55 si First published: 22 December 2016 Background: High-quality data on the management of autoimmune hepatitis (AIH) are scarce. Despite published guidelines, management of AIH is still expert based rather than evidence based. Aim: To survey expert hepatologists, asking each to describe their practices in the management of patients with AIH. Methods: A survey questionnaire was distributed to members of the International AIH Group. The questionnaire consisted of four clinical scenarios on different presentations of AIH. Results: Sixty surveys were sent, out of which 37 were returned. None reported budesonide as a first line induction agent for the acute presentation of AIH. Five (14%) participants reported using thiopurine S-methyltransferase measurements before commencement of thiopurine maintenance therapy. Thirteen (35%) routinely perform liver biopsy at 2 years of biochemical remission. If histological inflammatory activity is absent, four (11%) participants reduced azathioprine, whereas 10 (27%) attempted withdrawal altogether. Regarding the management of difficult-to-treat patients, mycophenolate mofetil is the most widely used second-line agent (n = ~450 in 28 centres), whereas tacrolimus (n = ~115 in 21 centres) and ciclosporin (n = ~112 in 18 centres) are less often reported. One centre reported considerable experience with infliximab, while rescue therapy with rituximab has been tried in seven centres. Conclusions: There is a wide variation in the management of patients with autoimmune hepatitis even among the most expert in the field. Although good quality evidence is lacking, there is considerable experience with second-line therapies. Future prospective studies should address these issues, so that we move from an expert- to an evidence- and personalised-based care in autoimmune hepatitis. mixed R. Liberal; Y. S. de Boer; R. J. ZusammenfassungDie nicht-alkoholische Fettlebererkrankung (NAFLD) gehört aktuell zu den häufigsten Leberkrankheiten in der Welt. Die wichtigsten exogenen Determinanten der NAFLD sind Übergewicht, Diabetes und metabolisches Syndrom. In den letzten Jahren konnte nachgewiesen werden, dass die Träger bestimmter Genvarianten nicht nur häufiger NAFLD entwickeln, sondern auch ein erhöhtes Risiko für eine fortschreitende Leberschädigung aufweisen. In Analysen sehr großer Patientenkohorten mit NAFLD konnten drei Polymorphismen, PNPLA3 p.I148M, TM6SF2 p.E167K und MBOAT7 rs641738, als sichere Risikofaktoren für die Entstehung einer Fettleber identifiziert werden. Diese Varianten sind auch mit Komplikationen der NAFLD, wie zum Beispiel Leberzirrhose und Leberkrebs, assoziiert. In dieser Übersicht werden ausgewählte Studien zu diesen Genpolymorphismen vorgestellt. Wir diskutieren ihre möglichen pathophysiologischen Mechanismen und stellen das Konzept einer genbasierten Leberkrankheit, PASH (PNPLA3-assoziierte Steatohepatitis), vor.

Hepatitis C virus infection is causing chronic liver disease, cirrhosis, and hepatocellular carcinoma. By combining direct-acting antivirals (DAAs), high sustained virologic response rates (SVRs) can be achieved. Resistance-associated substitutions (RASs) are commonly observed after DAA failure, and especially nonstructural protein 5A (NS5A) RASs may impact retreatment options.1-3 Data on retreatment of DAA failure patients using first-generation DAAs are limited.4-7 Recently, a second-generation protease- and NS5A-inhibitor plus sofosbuvir (voxilaprevir/velpatasvir/sofosbuvir [VOX/VEL/SOF]) was approved for retreatment after DAA failure.8 However, this and other second-generation regimens are not available in many resource-limited countries or are not reimbursed by regular insurance, and recommendations regarding the selection of retreatment regimens using first-generation DAAs are very important. This study aimed to analyze patients who were re-treated with first-generation DAAs after failure of a DAA combination therapy.
