Introduction: Necrotizing soft tissue infections (NSTI) are increasing, posing a significant risk of morbidity and mortality. Due to nonspecific symptoms, a high index of suspicion is crucial. Treatment involves a multidisciplinary approach, with broad-spectrum antibiotics, early surgical debridement, and life support. This study analyzes the characteristics, demographics, complications, and treatment of NSTI in a hospital in Madrid, Spain. Methods: A retrospective observational study was conducted, including all surgically treated NSTI patients at our center from January 2016 to December 2022, examining epidemiological and clinical data. The Laboratory Risk Indicator for Necrotizing Fasciitis (LRINEC) was prospectively calculated for all patients. Results: Twenty-two patients (16 men, 6 women, mean age 54.8) were included. Median time from symptom onset to emergency room visit was 3.5 days. All reported severe treatment-resistant pain; sixteen had fever exceeding 37.8 °C (72.7%). Skin lesions occurred in twelve (54.5%), and thirteen had hypotension and tachycardia (59.1%).Treatment involved resuscitative support, antibiotherapy, and radical debridement. Median time to surgery was 8.25 h. Intraoperative cultures were positive in twenty patients: twelve Streptococcus pyogenes, four Staphylococcus aureus, one Escherichia coli, and four polymicrobial infection. In-hospital mortality rate was 22.73%. Conclusions: We examined the correlation between our results, amputation rates and mortality with LRINEC score and time to surgery. However, we found no significant relationship unlike some other studies. Nevertheless, a multidisciplinary approach with radical debridement and antibiotic therapy remains the treatment cornerstone. Our hospital stays, outcomes and mortality rates align with our literature review, confirming high morbimortality despite early and appropriate intervention. Presentaban lesiones cutáneas 12 pacientes (54,5%) y 13 hipotensión y taquicardia (59,1%). El tratamiento consistió en soporte vital, antibioterapia y desbridamiento radical. El tiempo medio hasta la cirugía fue de 8,25 horas. Los cultivos intraoperatorios fueron positivos en 20 pacientes: 12 Streptococcus pyogenes, cuatro Staphylococcus aureus y uno Escherichia coli. En cuatro casos la infección fue polimicrobiana. La mortalidad intrahospitalaria fue del 22,73%. Conclusiones: El enfoque multidisciplinar, incluyendo desbridamiento radical y terapia antibioterapia, sigue siendo la piedra angular del tratamiento de las NSTI. En este estudio no encontramos correlación significativa entre nuestros resultados, tasas de amputación y mortalidad con la puntuación de la escala LRINEC ni el tiempo hasta la cirugía. Nuestros resultados, estancia hospitalaria y tasa de mortalidad coinciden con lo reportado por la literatura, confirmando la alta morbimortalidad de estas infecciones a pesar del tratamiento precoz.

Resumen: Introducción: Las metástasis vertebrales son un problema muy frecuente y asocian un deterioro importante de la calidad de vida en los pacientes oncológicos. El objetivo de esta revisión es determinar el encaje de las técnicas quirúrgicas mínimamente invasivas dentro del manejo de esta entidad. Métodos: Se realizó una revisión bibliográfica en las bases de datos Google Scholar, PubMed, Scopus y Cochrane. Se revisaron los artículos publicados en los últimos 10 años que fueran de una relevancia y calidad adecuadas. Resultados: Tras el cribado de los 2.184 trabajos identificados inicialmente en las distintas bases de datos, se incluyeron un total de 24 artículos en esta revisión. Conclusión: La cirugía mínimamente invasiva de columna es especialmente útil en pacientes oncológicos frágiles con metástasis vertebrales por la reducida comorbilidad que presentan las técnicas que se engloban en ella en comparación con la de la cirugía abierta convencional. Los avances en tecnología aplicada a la cirugía, como la navegación y la robótica, mejoran la precisión y reducen las complicaciones de esta técnica. The objective of this review is to address the issue of how minimally invasive surgery can play an important role in treating this pathology. Methods: A literature review was performed, searching in the Google Scholar, PubMed, Scopus and Cochrane databases. Relevant and quality papers published within the last 10 years were included in the review. Results: After screening the 2184 initially identified registers, a total of 24 articles were included for review. Conclusion: Minimally invasive spine surgery is specially convenient for fragile cancer patients with spinal metastases, because of its reduced comorbidity compared to conventional open surgery. Technological advances in surgery, such as navigation and robotics, improve accuracy and safety in this technique.

Resumen: Las metástasis espinales representan una importante carga sobre la calidad de vida en los pacientes afectados por una enfermedad oncológica activa, debido a la alta incidencia de síndromes dolorosos, deformidad espinal y deterioro neurológico. La cirugía juega un papel determinante a la hora de mejorar la calidad de vida mediante el control del dolor, el restablecimiento de la función neurológica y el mantenimiento de la estabilidad espinal, además de contribuir a la respuesta de la terapia médica. La cirugía mínimamente invasiva (MIS) es una opción de tratamiento en determinados pacientes con alto riesgo quirúrgico, ya que tiene una baja tasa de complicaciones, de sangrado intraoperatorio, de estancia hospitalaria y ofrece resultados similares a la cirugía abierta.Presentamos en esta revisión el papel de la MIS en esta enfermedad, y algunos casos tratados en nuestro centro hospitalario. Resumen: Introducción: El enclavado endomedular mediante abordaje infrapatelar (IP) se considera el gold estándar del tratamiento de las fracturas diafisarias de tibia, sin embargo, se ha asociado a mayor dificultad para la reducción y complicaciones como la mala alineación en procurvatum y al dolor anterior de rodilla. El abordaje suprapatelar (SP) surge como alternativa para solventar estos aspectos, asociándose también con un menor tiempo intraoperatorio y menor dosis y tiempo de fluoroscopia. Material y métodos: Estudio retrospectivo comparativo entre: un grupo de 22 fracturas intervenidas mediante abordaje SP y otro de 30 fracturas intervenidas por abordaje IP trastendinoso. Se analizaron variables perioperatorias y aspectos clínicos, radiológicos y funcionales en consulta a los 3, 6 y 12 meses. Resultados: No se encontraron diferencias entre grupos en cuanto a tiempo intraoperatorio, anemización, calidad de la reducción, o complicaciones en el seguimiento, entre otros. A los 12 meses, 12 (54,5%) casos SP y 16 (53,3%) IP presentaban dolor anterior de rodilla, sin diferencias significativas. En las escalas de evaluación, se registraron diferencias significativas en la IKDC (International Knee Documentation Committee) a favor de la técnica SP (88,45 [76,44-91,1] vs. IP 69 [49,95-80]) (p = 0,006), sin diferencias significativas en otras escalas funcionales analizadas. Conclusiones: En consonancia con lo hasta ahora descrito en la literatura, el presente estudio apoya la tendencia hacia el enclavado SP por mejorar los resultados funcionales (IKDC) a medio plazo respecto a la técnica tradicional IP, sin aumentar las complicaciones. Asimismo se percibe por los cirujanos una mayor facilidad técnica para la reducción y sencillez en la obtención de imágenes radiológicas intraoperatorias. Material and methods: Retrospective comparative study between a group of 22 fractures treated by SP approach and another of 30 fractures intervened by IP transtendinous approach. Perioperative variables were analyzed, as well as clinical, radiological, and functional aspects in outpatient visits at 3, 6 and 12 months. Results: No differences were found between groups in terms of intraoperative time, anemization, quality of reduction or complications during follow-up, among others. At 12 months, 12 (54.5%) SP cases and 16 (53.3%) IP presented anterior knee pain, without significant differences. In the evaluation scales, significant differences were recorded in the IKDC (International Knee Documentation Committee) in favor of the SP technique 88.45 (76.44–91.1) vs. IP 69 (49.95–80) (p = .006), with no significant differences in other functional scales analyzed. Conclusions: According to what has been described so far in the literature, the present study supports the tendency toward SP nailing by improving the functional results (IKDC) in the medium term compared to the traditional IP technique, without increasing complications. Likewise, surgeons perceive greater technical ease for reduction and simplicity in obtaining intraoperative radiological images.

Resumen: Introducción: La alta prevalencia de la patología del antepié genera largas listas de espera quirúrgica (LEQ). Hemos detectado un considerable número de pacientes que renuncian a la cirugía, lo que crea una importante distorsión en nuestra actividad y un elevado gasto de recursos. Nuestro objetivo es estudiar los factores relacionados con estas renuncias, así como comparar con otras patologías de alta prevalencia y tratamiento quirúrgico ambulatorio: síndrome del túnel carpiano (STC) y la meniscopatía interna (MI). Material y métodos: Estudio retrospectivo de las renuncias a la intervención sobre 2.399 pacientes incluidos en LEQ de la Unidad de Pie y Tobillo de nuestro centro para cirugía del antepié, entre enero/2014 y marzo/2018, ambos inclusive. Resultados: Hemos encontrado 389 renuncias, lo que supone un 16,22% de las inclusiones en LEQ, siendo un 84,83% mujeres. Las patologías con mayor tasa de renuncia han sido: metatarsalgia de Morton (24%) y hallux rigidus (20,16%). La patología más frecuente, el hallux valgus, registra un 15,96% de renuncias que se producen mayoritariamente entre los seis y nueve meses. En el STC y la MI, el índice de renuncias ha sido del 17,42 y del 8,92% respectivamente, con mayores tasas de renuncia en los tres primeros meses. Conclusiones: La renuncia a una intervención programada sobre el antepié registra una alta frecuencia en nuestro medio, que puede relacionarse con factores como el tipo de patología, historia natural, respuesta a intervenciones ortopédicas, el tiempo en LEQ, y otros no determinados sobre los que debemos profundizar, para racionalizar y establecer prioridades en nuestras LEQ. Abstract: Introduction: The high prevalence of forefoot pathology generates long surgical waiting lists (SWL). We have detected a considerable number of patients who withdraw surgery, which creates an important distortion in our activity and high expenditure of resources. Our objective is to study the factors related to these resignations, as well as, compare them with other pathologies of high prevalence and ambulatory surgical treatment: carpal tunnel syndrome (CTS) and internal meniscopathy (IM). Material and methods: Retrospective study of the surgical cancellations on 2,399 patients included in the SWL of the Foot and Ankle Unit of our center for forefoot surgery, between January/2014 and March/2018, both included. Results: We have found 389 renunciations, which represent 16.22% of the inclusions in SWL, with 84.83% of women. The pathologies with the highest rate of resignation have been Morton metatarsalgia (24%) and hallux rigidus (20.16%). The most frequent pathology, hallux valgus, records 15.96% of resignations that occur mostly between 6 and 9 months. In the CTS and IM, the resignation rate has been 17.42 and 8.92%, respectively, with higher resignation rates in the first 3 months. Conclusions: The withdrawal of a scheduled intervention on the forefoot registers a high frequency in our environment, which can be related to factors such as the type of pathology, its natural history, response to orthopedic interventions, time in LEQ, and other non-specific ones on which we must investigate, to rationalize and establish duties in our SWL.

El flujo en vertido en lámina libre constituye uno de los métodos de disipación de energía más utilizados en el sobrevertido de presas. Este artículo presenta el análisis temporal y frecuencial de registros de presiones medidos en la solera de un cuenco de disipación de un aliviadero de vertido libre. Las mediciones han sido realizadas en un dispositivo experimental de tamaño relativamente grande (altura de caída de hasta 2.84 m y velocidades del chorro incidente entre 5.3 y 7.6 m/s). Se han analizado los comportamientos en el dominio del tiempo y de la frecuencia de los cuatro tipos de chorros de aliviaderos de vertido libre clasificados en estudios previos. El estudio se centra en el punto de estancamiento y en las distribuciones de presiones en su entorno. Finalmente, se ha realizado una comparativa de las frecuencias obtenidas con resultados existentes en la bibliografía para chorros circulares y rectangulares.

Young rabbit females selected for growth rate can have nutritional needs which may not be met by the common practice of feed restriction during rearing in commercial rabbit production. The aim of this study was to analyse the effect of two different feeding programmes: restricted and ad libitum feeding, applied in young rabbit females for one month at the end of rearing, on reproductive performance, body condition and circulating metabolic hormones and metabolites in a rabbit line selected by growth rate in 3 consecutive reproductive cycles. Thus, twenty-four 16-week-old does were randomly assigned to a group in which the daily recommended nutrient intakes were satisfied (fed restricted: 130 g/day, n=13) or a group fed to satiety (ad libitum: 235.5 g/day, n=11) during one month. Then, all does were inseminated in 3 consecutive cycles using a 42-day reproductive cycle. Measurements of does’ body weight, perirenal fat thickness and plasma leptin, non-esterified-fatty-acids (NEFA), beta-hydroxybutyrate (BOHB) and fructosamine were performed at artificial insemination (AI), parturition and weaning time in 3 consecutive cycles. Reproductive performance of does was evaluated based on fertility, litter size at parturition, prolificacy and productivity. Differences in body weight were found only in the 1st cycle, ad libitum fed females being heavier than restricted ones. Nevertheless, body weight variances disappeared in later cycles. No differences were found in perirenal fat thickness. Finally, in ad libitum fed females slight differences were found in plasma levels of NEFAs (452 vs. 258 μekv/L and 527 vs. 306 μekv/L for 1st and 2nd cycles) and BOHB (0.26 vs. 0.03 mM for 2nd cycle), but disappeared in the 3rd reproductive cycle. Fertility, prolificacy and productivity was not significantly affected by the feeding programme. Nevertheless, total litter size showed to be higher in ad libitum fed females at second parturition (8.7 vs. 5.9 kits). Therefore, the evaluated feeding programmes until first AI in females selected by growth rate had no effect on their reproductive outcomes, as the global reproductive performance was not affected.

Reproductive performance is greatly affected by environmental factors such as temperature. Heat stress (HS) during pregnancy and lactation can influence not only foetal growth but also postnatal development of kits. The aim of this study was to test the effect of HS during gestation and lactation on postnatal growth till Spanish commercial liveweight. To investigate this, 32 primiparous non lactating rabbit does were exposed to 1 of 2 environmental treatments: high temperature (between 25 and 36°C, HS group; n=16) or thermoneutral conditions (between 14 and 20°C, TN group; n=16). Does were allowed to acclimate 30 d before the artificial insemination. At birth, kits were allocated into 4 groups: HS was only applied during gestation (G group; n=54); HS was applied during gestation and lactation period (GL group; n=85); HS was only applied during lactation period (L group; n=60); and TN was applied during gestation and lactation period (C group; n=77). All litters were kept under each experimental environment until weaning at day 30. Then, litters were moved to TN temperatures until slaughter at day 63. Campylobacteriosis and salmonellosis are the most frequently reported zoonoses and among the most common causes of diarrhoeal illness in the European Union and the United States, and their incidence appears to be increasing. Campylobacter species are routinely found in poultry, swine, cattle, dairy cows and sheep. So far, there are few descriptions of Campylobacter isolation from rabbits. Rabbit meat is a common item in the Mediterranean diet. In this context, the aim of the present study was to investigate the occurrence of Campylobacter spp. in healthy rabbits reared in intensive farms in the Alto Palancia region, eastern Spain. Caecal contents from 70 healthy does reared on 7 different farms were collected. Bacteriological culture was performed in accordance with ISO 10272-1:2006. All samples tested negative for Campylobacter spp. To our knowledge, this is the first study in which comprehensive monitoring was specifically carried out in order to provide data on the occurrence of thermophilic Campylobacter spp. in large intensive rabbit farms in Spain. However, further microbiological studies throughout the Spanish territory are needed to determine the prevalence and risk of other foodborne pathogens in rabbits at farm level.

Selection for growth rate is negatively related with reproductive fitness. The aim of this work was to analyse the causes of fertility failure in rabbit does selected for growth rate and characterised for reproductive deficiencies (line R). In the experiment, 82 does were divided into 2 groups: naturally mated (NM) and artificially inseminated (AI), to relate luteinizing hormone (LH) concentration with ovulation induction and pregnancy rate by laparoscopic determination. Additionally, in 38 of these females ovulation rate and metabolites determination (leptin, NEFA, BOHB and glucose) were analysed and perirenal fat thickness measurement and live body weight (LBW) determined. The results showed that all ovulated does (both NM and AI) presented higher concentrations of LH than non-ovulated females. In addition, non-ovulated females showed high levels of leptin and BOHB, as well as LBW. Females from line R have an inherit reduced fertility due to ovulation failure as a consequence of a reduction in LH release, which could be explained by a heavier body weight and higher leptin concentrations.

This study was conducted to elucidate the effect of different freezing extenders on two lines selected for hyperprolificacy and longevity (H and LP, respectively). In extender A, dimethyl sulphoxide (Me2SO) and sucrose were used as cryoprotectants. In extenders B and C, the sucrose was replaced by 20% egg yolk, and in extender C the Me2SO was substituted by acetamide. Semen was packaged in 0.25 ml plastic straws and cooled at 5°C for 45 min, and then was frozen in liquid nitrogen vapour for 10 min before being plunged into the liquid nitrogen. Thawing was carried out by immersing the straws in a water bath at 50°C for 10 s. Frozen-thawed semen characteristics and reproductive parameters were affected by freezing. Extender C showed significantly lower post-thawing quality traits than any of the three extenders. Acrosome integrity was significantly improved when Me2SO was used as cryoprotectant. Sucrose replacement by 20% egg yolk had no effect on acrosome integrity but provided significantly lower sperm motility and viability. Freezing extender affected fertility rate, total born, number of implantation sites and gestational losses, obtaining better results when extender A was used. The acrosomal integrity after frozen-thawed process showed a significant correlation with fertility at 12th day and also at birth, indicating that an increase in acrosomal integrity leads to an increase in both fertilities (12th day and at birth). A positive correlation between motility of semen and implantation sites was found. The post-thawing quality traits of semen were not affected by the genetic line, although LP line showed higher total born and lower foetal and gestational losses. The findings of this study suggest that freezing extender composition has a significant effect on the success of rabbit sperm for preservation, and when Me2SO was used as permeable cryoprotectant sucrose provided better protection compared with egg yolk and improved reproductive traits, and, on the other hand, the male genotypes used in the present study had no effect on frozen-thawed sperm parameters but negatively affected some of the reproductive parameters.

La endocarditis infecciosa predispone al absceso esplénico, de incidencia mal definida y que aumenta la morbimortalidad. No está estandarizado el momento para esplenectomía cuando se requiere intervención por endocarditis. Se describe la experiencia con endocarditis y patología esplénica asociada e indicación quirúrgica valvular y esplénica. Material y métodos: Revisión retrospectiva de la base de datos prospectiva de endocarditis, definida por criterios modificados de Duke. Análisis de pacientes diagnosticados de infarto/absceso esplénico mediante imagen/estudio post mortem. En los casos de esplenectomía, se practicó en la misma intervención después del procedimiento valvular. Resultados: Entre enero de 1995 – julio de 2011 se recogieron 737 episodios de endocarditis; 62 (8,41%) pacientes presentaron infarto/absceso esplénico con fiebre persistente y dolor abdominal; 6 (8,9%) requirieron esplenectomía; 5 (83%) eran varones. La edad media fue 52 (27–72). EuroSCORE logístico medio fue 37,42% (18,83–60,93%). La endocarditis fue mitral (3), aórtica (2) y multivalvular (mitral y tricúspide). Se aislaron Enterococcus spp (2), Staphylococcus aureus (1), estreptococos del grupo viridans (1), Kingella kingae (1) y hemocultivos negativos (1). El absceso esplénico se diagnosticó por tomografía (4); en 2 la intervención fue urgente, sin imagen. Se practicó sustitución valvular. Tres (50%) fallecieron. En los 6 se confirmó absceso esplénico por histopatología. El seguimiento de los supervivientes a la intervención con esplenectomía fue de 16, 22 y 36 meses, sin recidiva. Conclusiones: Debe sospecharse absceso esplénico en los pacientes con endocarditis, fiebre y dolor abdominal. La esplenectomía y la intervención valvular pueden realizarse en el mismo acto dependiendo de la condición del paciente.

La infección vascular es potencialmente letal, afectando a cualquier territorio del aparato circulatorio. Este estudio observacional ofrece datos de seguimiento a largo plazo sobre el comportamiento de los homoinjertos vasculares criopreservados en esta patología. Metodología: Análisis retrospectivo de datos prospectivos de la base de datos departamental. Análisis de supervivencia, reinfección, reintervención y preservación de extremidades. Los pacientes con sospecha clínica de infección vascular fueron tratados mediante sustitución del tejido infectado por homoinjerto vascular criopreservado. Se registraron comorbilidades, indicación quirúrgica y causante microbiológica. Resultados: Desde octubre de 1992 – junio de 2011 se intervinieron 42 pacientes (37 varones, 88%). Edad media 63 ± 11,2. La infección fue primaria (16/38%) y protésica (26/62%). La intervención fue electiva (46%), urgente (33%) y de urgencia vital (21%). Los estafilococos (13/31%), infección polimicrobiana (6/14%) y el cultivo negativo (5/12%) fueron los diagnósticos microbiológicos más frecuentes. En 27 (64%) se implantó un homoinjerto y en 15 (36%) un homoinjerto compuesto. Se implantaron 68 homoinjertos (39 ilíacas, 14 bifurcados, 12 aortas torácicas). Hubo 21 intervenciones sobre el sector aortofemoral (50%). No hubo pérdidas en el seguimiento. La estancia media fue de 30,5 días. Excluyendo la mortalidad hospitalaria (23,8%) el 46,9% falleció durante el seguimiento. La mediana de supervivencia fue 9,9 años (intervalo de confianza [IC] 95%: 3,9–15,9). Hubo 8 reoperaciones por motivos no infecciosos. Durante el seguimiento no hubo reinfecciones vasculares. Conclusiones: Este estudio a largo plazo contribuye a mantener la indicación para el implante de homoinjertos vasculares criopreservados en las infecciones vasculares. Los resultados demuestran un comportamiento satisfactorio en esta población multimórbida.

Abstract Introduction Anorexia Nervosa (AN) is a complex psychiatric illness, characterized by a high risk of developing cardiovascular complications. Given the high risk of vascular diseases in patients with AN, we can assume that patients with severe AN have a high risk of developing ischemic stroke. However, to the best of our knowledge, no reports of patients with AN presenting with ischemic stroke have been published, other than a report of the development of IS during refeeding therapy in patients with severe AN. Case presentation The present case report is aimed at describing the characteristics of an ischemic stroke occurring in a 19-year-old university student who had a 6-month history of AN. She was a non-smoker, had no relevant medical history and no family history of stroke. Upon hospital admission due to symptoms of stroke (aphasia and facial droop), she exhibited severe malnutrition with a BMI of 12.8 kg/m2. Computerized tomography imaging revealed occlusion of the left M2 branch and a congruous extensive area of hypoperfusion. Further investigations ruled out all common causes of stroke: she had no vascular stenosis, no heart diseases or arrhythmias, and no shunts, and gave negative results in autoimmune, toxicological and thrombophilia screenings. Conclusion Clinicians should suspect development of severe complications, including ischemic stroke, in patients with severe AN. Further extensive group studies or group-based studies are needed to elucidate the etiology of ischemic stroke in patients with severe AN. This will enable us to develop more precise and effective interventions. Graphical Abstract
