Leishmaniasis is a disease caused by a protozoan and transmitted by sandfly species in several emerging countries. Visceral leishmaniasis is a serious complication, especially in immunosuppressed patients, and is uncommon after liver transplantation. We report the case of a 48-year-old female patient who underwent liver transplantation owing to polycystic liver disease. Six months after the procedure, she was hospitalized with diarrhea, acute kidney failure, and leukopenia. She had been off steroids for 3 months and was taking mycophenolate and tacrolimus. She had already been treated for cytomegalovirus, which was negative on admission. During hospitalization, fever, splenomegaly, ascites, and pancytopenia appeared. Serology for Leishmania by indirect immunofluorescence was negative. Then, bone biopsy and molecular testing for Leishmania diagnosed it as visceral leishmaniasis. Amphotericin therapy was initiated with resolution of fever after 4 days of treatment and gradual recovery from pancytopenia. This case highlights the challenge of early diagnosis of visceral leishmaniasis in liver transplant recipients with diarrhea and leukopenia, which can be caused by immunosuppression or more prevalent viral diseases. Late onset of fever, splenomegaly, and a first negative serologic test also made early diagnosis difficult. The aim of the report is to emphasize the suspicion of visceral leishmaniasis in symptomatic patients from emerging countries and to question the benefit of including protozoan screening in liver transplant donors and recipients in endemic areas.

Injection drug use and heterosexual transmission from male key populations to their female partners have been the dominant modes of HIV transmission in Vietnam. A demonstration project was conducted to offer immediate antiretroviral therapy (ART) (regardless of CD4 cell count) to HIV-seropositive partners in serodiscordant couples. During March–December 2013, couples HIV testing and counselling (HTC) and immediate ART were offered in two drug use-affected provinces. Of 256 couples receiving couples HTC, 146 (57%) were serodiscordant; 134 (92%) seropositive partners initiated ART irrespective of CD4 cell count. Of these, 86% were male; 57% reported ever using illicit drugs; median CD4 cell count before ART initiation was 374 cells/mm3. Consistent condom use was reported by 58, 70 and 71% among HIV-positive partners at months 0, 3 and 12. Background: Maintaining essential HIV services has being a Global challenge during the COVID-19 crises. Myanmar has 54 million inhabitants. Neighbor of China, Thailand, India and Bangladesh it was impacted by COVID-19, but came up with a comprehensive and effective response, following WHO recommendations. The HIV Prevalence is 0.58% and it is concentrated among key population. A HIV Contingency Plan was developed to face this challenge. Methodology: The programme-based cross-sectional descriptive study with analysis of routinely collected data from MoHS data system, between 2019 and 2020 was conducted, comparing first six months of 2019 and 2020. Results: HIV outreach activities and HIV testing were slightly affected after detection of first COVID-19 case, till mid May 2020. After that, outreach activities resumed. Introduction of HIV self-testing was initiated. 72% of more than 21,000 PWID on MMT were receiving take home dose up to 14 days and 60% of ART patients were receiving 6 months ARV dispensing. Conclusion: Essential HIV services were maintained.

Mulheres de corpo e alga, cooperativa criada na comunidade da Barrinha, Icapuí-CE utiliza algas marinhas criando produtos, gerando renda e resíduos. O objetivo é identificar os teores de proteínas totais (PT), de carboidratos solúveis totais (AST), umidade e cinzas nos resíduos, antes e depois da adição de diferentes concentrações de S. cerevisiae, em diferentes tempos de exposição, buscando o tempo de fermentação adequado para melhorar a composição nutricional. Foi dividido em duas etapas: levedura nas concentrações de 0%, 5%. 10% e 15% e expondo em estufa à 36°C nos tempos de 0h, 24h, 48h e 72h. A estatística foi feita usando o software Assistat®, montado em experimento fatorial, com três repetições. Observou-se que o melhor tempo para aumento de PT foi 24h na concentração de 10% (C2). Na segunda etapa, ao tempo e a concentração ideal da primeira etapa foi adicionado três tipos de fonte de nitrogênio: Sulfato de amônio 2,5%, a Uréia a 2,5%; e Sulfato de amônio e Uréia (1,25%, cada) e, após a FSS foram feitos testes de PT, AST, umidade e cinzas. A suplementação com uréia apresentou maior PT possível devido a uma maior disponibilidade de nitrogênio para formação de proteínas, comportamento parecido com os AST. A umidade mais adequada a legislação foi a amostra C3 atribuida a biomassa microbiana e as cinzas não apresentaram diferença significativa possivelmente devido ao consumo de AST e compostos nitrogenados para fabricação das proteínas. O resíduo, sendo mais estudado, pode servir como uma alternativa para o produtor rural, como ração animal.

Background This study was conducted to determine the seroprevalence of HIV among pregnant women in Brazil and to describe HIV testing coverage and the uptake of antenatal care (ANC). Methods Between October 2010 and January 2012, a probability sample survey of parturient women aged 15-49 years who visited public hospital delivery services in Brazil was conducted. Data were collected from prenatal reports and hospital records. Dried blood spot (DNS) samples were collected and tested for HIV.We describe the agespecific prevalence of HIV infection and ANC uptake with respect to sociodemographic factors. Results Of the 36,713 included women, 35,444 (96.6%) were tested for HIV during delivery admission. The overall HIV prevalence was of 0.38% (95% confidence interval [CI]: 0.31-0.48), and it was highest in: the 30 to 39 year-old age group (0.60% [0.40- 0.88]), in the Southern region of Brazil (0.79% [0.59-1.04]), among women who had not completed primary (0.63% [0.30-1.31]) or secondary (0.67% [0.49-0.97]) school education, and among women who self-reported as Asian (0.94% [0.28-3.10]). The HIV testing coverage during prenatal care was of 86.6% for one test and of 38.2% for two tests. Overall, 98.5% of women attended at least 1 ANC visit, 90.4% attended at least 4 visits, 71% attended at least 6 visits, and 51.7% received ANC during the 1st trimester. HIV testing coverage and ANC uptake indicators increased with increasing age and education level of education, and were highest in the Southern region. Conclusions Brazil presents an HIV prevalence of less than 1% and almost universal coverage of ANC. However, gaps in HIV testing and ANC during the first trimester challenge the prevention of the vertical transmission of HIV. More efforts are needed to address regional and social disparities. Introdução Este estudo foi realizado com o objetivo de determinar a soroprevalência do HIV entre as mulheres grávidas no Brasil e descrever a cobertura de exames de HIV e a integração dos cuidados pré-natais (CPN). Métodos Entre outubro de 2010 e janeiro de 2012, foi realizada uma pesquisa de probabilidade por amostragem direcionada a mulheres grávidas com idade entre 15 e 49 anos, que utilizaram serviços de parto em hospitais públicos do Brasil. Os dados foram coletados a partir de relatórios pré-natais e registros hospitalares. Amostras de sangue foram coletadas e submetidas a exames de HIV. Descrevemos a prevalência da infecção pelo HIV específica de acordo com a idade e a absorção dos CPN em relação a fatores demográficos. Resultados Das 36.713 mulheres incluídas, 35.444 (96,6%) foram submetidas a exames de HIV durante a admissão para o trabalho de parto. A prevalência global de HIV foi de 0,38% (intervalo de confiança [IC] de 95%: 0,31-0,48), e foi maior no grupo com a faixa etária entre 30 e 39 anos de idade (0,60% [0,40-0,88]), da região Sul (0,79% [0,59-1,04]), entre as mulheres com o ensino fundamental incompleto (0,63% [0,30-1,31]) ou ensino médio incompleto (0,67% [0,49-0,97]), e entre as mulheres que se identificam como asiáticas (0,94% [0,28-3,10]). A cobertura do exame de HIV durante os CPN foi de 86,6% para um exame e de 38,2% para dois exames. No geral, 98,5% das mulheres foram atendidas em pelo menos 1 consulta de CPN, 90,4% compareceram a pelo menos 4 consultas, 71% compareceram a pelo menos 6 visitas, e 51,7% receberam CPN durante o 1o trimestre. A cobertura de exames de HIV e os indicadores de captação de CPN aumentaram de forma proporcional ao aumento da idade e do nível de educação, e foram maiores na região Sudeste. Conclusões O Brasil apresenta uma prevalência de menos de 1% e cobertura praticamente universal de CPN. No entanto, as lacunas nos exames de HIV e CPN durante o primeiro trimestre representam um desafio à prevenção contra a transmissão vertical do HIV. São necessários mais esforços a fim de reduzir as disparidades regionais e sociais.

O presente trabalho tem por objetivo o estudo aprofundado do regime de invalidades, aplicável ao direito societário e, em especial, à deliberação de aprovação de contas, viabilizando propositura de ação de responsabilidade contra o administrador que age com culpa ou dolo no exercício de suas funções. A escolha do tema se deu após a análise de uma decisão judicial que nos pareceu injusta e descabida. Daí, ao estudar o assunto mais amiúde, chegamos à conclusão de que a questão tem sido muito pouco explorada em nossa doutrina e, deveras, aplicada por nossa jurisprudência sem o cuidado e profundidade que entendemos devida, surgindo, então, a motivação para a escolha do tema. O estudo realizado procura demonstrar que, em relação ao regime de invalidades, não é possível se falar num direito societário moderno e mais justo, seguindo os atuais ditames da boa-fé objetiva, sem pensar na aplicação do regime de nulidades, quando o caso concreto o exigir. This paper aims to study in detail the invalidity regime, applicable to corporate law and, in particular, to the approval of accounts, making it possible to file a claim of liability against the trustee who acts guilty or willful misconduct in the performance of his duties. The choice of topic came after an analysis of a court decision that seems to be unfair and inappropriate. Hence, when we study the subject more often, we come to the conclusion that the question has been very little explored in our doctrine and, indeed, applied by our jurisprudence without the care and depth that we understand it should be, thus, arising the motivation for the choice of the theme. The study seeks to demonstrate that, in relation to the invalidity regime, it is not possible to speak of a modern and fairer corporate law, following the current dictates of objective good faith, without considering the application of the nullity regime, when the concrete case demand.

Objective: Rapid HIV testing and counselling in community settings delivered by trained peers can allow easier access to and uptake of HIV testing and care. The programme 'Live Better Knowing' offers rapid HIV testing to key populations across Brazil. This cross-sectional study aims at describing the HIV prevalence in population subgroups and at assessing the association of sex work (SW) and/or drug use with a positive HIV result. Methods: Between January 2013 and April 2016, key populations were invited for HIV rapid oral fluid tests (DPP HIV- 1/2 Bio-Manguinhos/Fiocruz, Rio de Janeiro, Brazil) by peers. HIV testing was offered in identified venues and areas where these populations socialize. Participants were interviewed using a brief questionnaire about their socio-demographic and risk behaviour. Multivariable logistic regression was used to assess the effect of SW and/or drug use on the risk of HIV. Results: Among the 43,358 participants, 42.9% were ciswomen, 2.9% were transvestites, 2.7% were transgender, 19.8% were men who have sex with men (MSM) and 31.9% were heterosexual men. Overall, 52.1% were first-time HIV testers. The HIV prevalence among the ciswomen, transvestites, transgender people, MSM and heterosexual men was 0.9%, 7.2%, 4%, 3.4% and 1%, respectively. Individuals who performed SW and used drugs had the highest HIV prevalence within each population group (ciswomen 1.7%, transvestites 8.1%, transgender 7.4%, MSM 5.9% and heterosexual men 2.6%). The adjusted odds ratios with 95% confidence intervals for selling sex combined with drug use were 2.90 (1.21-6.96) in ciswomen, 25.01 (2.67-234.57) in transgender people and 2.42 (1.31-4.48) in MSM. Conclusion: This outreach programme attracted persons with high risk practices who had never been tested. Our findings suggest that transvestites and transgender people (particularly those who sell sex) constitute a high risk population for HIV in urgent need of enhanced prevention, treatment and care services.

Objective To describe the antiretroviral therapy status of people living with HIV (PLHIV) who died of AIDS-related causes between 2009 and 2013. Methods We conducted a cross-sectional population-based study. Data were obtained by linking the mortality information system and the national ART dispensing database. Trends were modelled using linear regression analysis. Results A total of 61 425 AIDS-related deaths were registered in Brazil between 2009 and 2013. Median age at death was 41 years (IQR: 33-49) and 65.7% (40 337) of deaths were among men; 47.2% (29 004) of PLHIV who died during the study period had never started treatment 7.0% (4274) had discontinued it 15.9% (9775) were on ART for 6 months or less and 29.9% (18 372) were on ART for more than 6 months. Only 1.3% of PLHIV were on third-line ARV regimens when they died. Conclusions AIDS-related mortality remains a challenge even in a context of sustained universal access to antiretroviral treatment due to failure of service provision not to therapy failure. Robust health policies closing gaps in the HIV continuum of care are crucial to further reduce mortality.

Announced in 2010 at the International AIDS Conference in Vienna and pioneered by the Joint United Nations Programme on HIV/AIDS (UNAIDS) and the World Health Organization's (WHO) leadership at the global level, Treatment 2.0 is a new approach to the HIV response that encourages innovation, efficiency, and sustainability. Building upon WHO and UNAIDS' ”3 by 5” Initiative, Treatment 2.0 focuses upon scale-up and universal access of life-saving ART treatment through strategic investments and innovations in five priority pillars that include: 1) Optimize drug regimens; 2) Provide point-of-care (POC) and other simplified diagnostic and monitoring tools; 3) Reduce costs; 4) Adapt service delivery; and 5) Mobilize communities [1-3]. The Treatment 2.0 approach is in line with UNAIDS' 2011-2015 Strategy: Getting to Zero with the vision of, ”Zero New infections; Zero discrimination; and Zero AIDSrelated deaths”, and as well as the four strategic directions for the health sector response outlined under WHO's Global Health Sector Strategy on HIV/AIDS 2011-2015 [4,5]. At the Sixty-fourth World Health Assembly (WHA) in May 2011, it was formally announced that Vietnam has taken the leadership to pilot Treatment 2.0 in two of its provinces, with support from both the WHO and UNAIDS country offices [6,7]. Given that Vietnam is one of the few countries with a concentrated epidemic to pilot Treatment 2.0, the outcomes and experiences of this initiative can provide valuable insight to other countries who may consider implementation. The objectives of this article are therefore to: 1) Describe the early process for translating Treatment 2.0 concept in Vietnam's context; and 2) Highlight early progress and challenges.

Introduction Men who have sex with men (MSM) and transgender women (TGW) in Brazil experience high rates of HIV infection. We examined the clinical and economic outcomes of implementing a pre‐exposure prophylaxis (PrEP) programme in these populations. Methods We used the Cost‐Effectiveness of Preventing AIDS Complications (CEPAC)‐International model of HIV prevention and treatment to evaluate two strategies: the current standard of care (SOC) in Brazil, including universal ART access (No PrEP strategy); and the current SOC plus daily tenofovir/emtracitabine PrEP (PrEP strategy) until age 50. Mean age (31 years, SD 8.4 years), age‐stratified annual HIV incidence (age ≤ 40 years: 4.3/100 PY; age > 40 years: 1.0/100 PY), PrEP effectiveness (43% HIV incidence reduction) and PrEP drug costs ($23/month) were from Brazil‐based sources. The analysis focused on direct medical costs of HIV care. We measured the comparative value of PrEP in 2015 United States dollars (USD) per year of life saved (YLS). Willingness‐to‐pay threshold was based on Brazil's annual per capita gross domestic product (GDP; 2015: $8540 USD). Results Lifetime HIV infection risk among high‐risk MSM and TGW was 50.5% with No PrEP and decreased to 40.1% with PrEP. PrEP increased per‐person undiscounted (discounted) life expectancy from 36.8 (20.7) years to 41.0 (22.4) years and lifetime discounted HIV‐related medical costs from $4100 to $8420, which led to an incremental cost‐effectiveness ratio (ICER) of $2530/YLS. The network of HIV counseling and testing centers in Sao Paulo, Brazil is a major source of data used to build epidemiological profiles of the client population. In this paper we propose two methods to give a first rough estimate of the actual number of hepatitis C virus (HCV)-infected individuals (prevalence) taking into account the notification rate of newly diagnosed infections (incidence of notification) and the size of the liver transplantation waiting list (LTWL) of patients with liver failure due to chronic HCV infection. Both approaches, when applied to the Brazilian HCV situation converge to the same results, that is, the methods proposed reproduce both the prevalence of reported cases and the LTWL with reasonable accuracy. We use two methods to calculate the prevalence of HCV that, as a first, and very crude approximation, assumes that the actual prevalence of HCV in Brazil is proportional to the reported incidence to the official notification system with a constant denoted $$\kappa $$ . In the paper we discuss the limitations and advantages of this assumption. With the two methods we calculated $$\kappa $$ , which reproduces both the reported incidence and the size of the LTWL. With the value of $$\kappa $$ we calculated the prevalence I(a) (the integral of which resulted in 1.6 million people living with the infection in Brazil, most of whom unidentified). Other variables related to HCV infection (e.g., the distribution of the proportion of people aged a who got infected n years ago) can be easily calculated from this model. These new variables can then be measured and the model can be recursively updated, improving its accuracy.

OBJETIVO: Descrever o perfil de usuários de drogas injetáveis vivendo com HIV/Aids e estimar a prevalência de hepatites B e C nesse grupo. MÉTODOS: Estudo transversal realizado com 205 pessoas vivendo com HIV/Aids, usuários de drogas injetáveis em acompanhamento em três unidades de atendimento da rede pública do Município de São Paulo, em 2003. Foi selecionada amostra não-probabilística, obtida de forma consecutiva e voluntária, nos dias em que compareciam para consulta nas unidades de atendimento. Por meio de entrevistas, foram levantados dados pessoais e informações sobre comportamento sexual, uso de drogas e conhecimento de hepatites. Foram realizados testes para detecção da infecção pelos vírus das hepatites B e C. RESULTADOS: Dos entrevistados, 81% eram homens e 19% mulheres, com idade média de 39 anos (dp=6,1) e seis anos de educação formal (dp=2,0). Não havia diferença em relação ao estado marital entre os sexos, 48% eram solteiros, 42% casados e 8% divorciados. A idade média do primeiro uso de tabaco, álcool e drogas ilícitas foi 13, 15 e 18 anos, respectivamente. Prevalências de hepatites B e C foram, respectivamente, de 55% (IC 95%: 49;63) e 83% (IC 95%: 78;88). Antes de usar droga injetável pela primeira vez, 80% dos respondentes não tinham ouvido falar de hepatites B e C. CONCLUSÕES: A alta prevalência de hepatites B e C e o baixo nível de conhecimento sobre a doença justificam a inclusão de esclarecimentos sobre as infecções hepáticas e de vacinação contra hepatite B nas estratégias de redução de danos pelo HIV. OBJECTIVE: To describe the profile of injecting drug users living with HIV/AIDS and estimate hepatitis B and hepatitis C prevalence rates within this group. METHODS: Cross-sectional study conducted with 205 injecting drug users living with HIV/AIDS receiving attention in three public health clinics in the city of Sao Paulo, in 2003. A non-probabilistic sample of volunteers was selected consecutively on the days respondents appeared for their appointments at the clinics. Personal data and information on sexual behavior, drug use and knowledge of hepatitis was collected through interviews. Tests were conducted to detect infections of the hepatitis B and C viruses. RESULTS: Out of the interviewees, 81% were men and 19% women, with an average age of 39 (SD = 6.1) and six years of formal education (SD = 2.0). There was no difference in marital status between the sexes, of which 48% were single, 42% were married, and 8% were divorced. The average ages for first use of tobacco, alcohol and illegal drugs were 13, 15 and 18, respectively. Hepatitis B and C prevalence were 55% (95% CI: 49;63) and 83%(95% CI: 78;88), respectively. Eighty percent of respondents had not heard of Hepatitis B and C prior to the first time they used injecting drugs. CONCLUSIONS: The high prevalence rates of Hepatitis B and C and low level of knowledge regarding the diseases justify the inclusion of information about hepatitis infections and the hepatitis B vaccines in HIV harm reduction strategies.

OBJETIVO: Analisar elementos da vulnerabilidade à infecção pelo HIV entre mulheres usuárias de drogas injetáveis. MÉTODOS: Foram realizadas 13 entrevistas semi-estruturadas com mulheres usuárias (ou ex-usuárias) de drogas injetáveis, moradoras da Zona Leste do município de São Paulo, no ano de 2002. O roteiro das entrevistas abordou quatro eixos temáticos: contexto socioeconômico e relações afetivas, uso de drogas, prevenção contra a infecção pelo HIV e cuidados com a saúde. As entrevistas foram analisadas por meio de análise de conteúdo. RESULTADOS: A pobreza, ausência de vínculos afetivos sólidos e continuados, expulsão da casa da família de origem e da escola, exposição à violência, institucionalização, uso de drogas, criminalidade e discriminação foram constantes nos relatos das entrevistadas. Esses elementos dificultaram a adoção de práticas de prevenção ao HIV como o uso de preservativos, seringas e agulhas descartáveis, e a busca de serviços de saúde. CONCLUSÕES: A vulnerabilidade ao HIV evidencia a fragilidade da vivência efetiva dos direitos sociais, econômicos e culturais, o que demanda políticas voltadas para o bem-estar social de segmentos populacionais específicos como mulheres (crianças e adolescentes), de baixa renda, moradores da periferia, com pouco acesso a recursos educacionais, culturais e de saúde. Este acesso é dificultado especialmente àquelas que são discriminadas por condutas como o uso de drogas.

Hepatitis C is considered one of the most neglected diseases in world. Worldwide about 150 million people are chronically infected by hepatitis C virus (HCV), and 60% to 70% of them will develop severe liver disease. This article describes Brazil’s response to hepatitis C, from the first steps in 1993 to a national program in 2002. We reviewed the available literature, most of it in Brazilian Portuguese, and compiled them in order to share this experience with those seeking some pragmatic solutions. After 12 years, the national program has achieved universal coverage of treatment, resulting in saved lives and resources for the health system. There is abundant evidence that the HCV epidemic deserves attention. The overall consequence of long-term HCV infection is a negative impact on the health care economy. The Brazilian experience can be adapted to many countries in the world, in compliance with the 2010 World Health Organization World Health Assembly Resolution.

Aims. A survey of cocaine users was undertaken to study (i) the frequency of reported drug overdoses among cocaine users; and (ii) the frequency of witnessing drug overdoses in the same population. Design and setting. A cross-sectional study as part of the World Health Organization (WHO) Multi-city Study among injecting drug users (IDUs), phase II, was conducted in Santos Metropolitan Region, State of Sao Paulo, Brazil, in 1999. Participants. Three hundred and ninety-six exclusive users of cocaine in the Santos Metropolitan Region, Sao Paulo State, Brazil were surveyed concerning their past experience with drug overdoses. Findings. Eighty (20%) of the cocaine users reported having experienced one or more overdoses, and 50% reported that they knew one or more other cocaine users who had died of an overdose. On multivariate analysis, being female and having spent time in jail were associated with an increased likelihood of having had one or more overdoses. Conclusion. Cocaine overdoses are an important and under-recognized health problem in the Santos Metropolitan Region, and possibly in other areas of Brazil.

Supplemental Digital Content is Available in the Text., Background: Few studies have assessed the effects of antiretroviral therapy (ART) to prevent HIV transmission in Asian HIV epidemics. Vietnam has a concentrated HIV epidemic with the highest prevalence among people who inject drugs. We investigated the impact of expanded HIV testing and counseling (HTC) and early ART, combined with other prevention interventions on HIV transmission. Methods: A deterministic mathematical model was developed using HIV prevalence trends in Can Tho province, Vietnam. Scenarios included offering periodic HTC and immediate ART with and without targeting subpopulations and examining combined strategies with methadone maintenance therapy and condom use. Results: From 2011 to 2050, maintaining current interventions will incur an estimated 18,115 new HIV infections and will cost US $22.1 million (reference scenario). Annual HTC and immediate treatment, if offered to all adults, will reduce new HIV infections by 14,513 (80%) and will cost US $76.9 million. Annual HTC and immediate treatment offered only to people who inject drugs will reduce new infections by 13,578 (75%) and will cost only US $23.6 million. Annual HTC and immediate treatment for key populations, combined with scale-up of methadone maintenance therapy and condom use, will reduce new infections by 14,723 (81%) with similar costs (US $22.7 million). This combination prevention scenario will reduce the incidence to less than 1 per 100,000 in 14 years and will result in a relative cost saving after 19 years. Conclusions: Targeted periodic HTC and immediate ART combined with other interventions is cost-effective and could lead to potential elimination of HIV in Can Tho.

Viral hepatitis is a disease of great concern to public health that is now met by a favorable momentum to combat the global epidemic. This article is intended to highlight the importance of viral hepatitis in the Vietnam population as well in the group of people living with HIV/AIDS (PLWHA). We reviewed available data on epidemiology and response on hepatitis B virus (HBV) and hepatitis C virus (HCV) and HIV coinfection in Vietnam. The hepatitis B surface antigen (HBsAg) prevalence in the general population ranged from 5.7% to 24.7%. The anti-HCV prevalence ranged from 0.38% to 4.3% in the general population, while among people who inject drugs (PWIDs) it ranged from 31% to 97.2%. The HBV prevalence among PLWHA is similar to the general population, while HCV/HIV coinfection is concentrated in some groups. Anti-HCV prevalence among HIV-infected PWIDs can be as high as 98.5%. Developing policies for diagnosis and treatment of chronic HBV and HCV infections are critical priorities in order to prevent clinical progression to cirrhosis and liver cancer.

A new approach for the estimation of the Basic Reproduction Ratio R0 for HIV among intravenous drug users (IVDU) is proposed. This approach is based in an adaptation of the models proposed by Ross and Macdonald for vector-borne infections. A straightforward adaptation of Macdonald's model is presented first: biological vectors are replaced by needles and syringes and we consider a homogeneous population of IVDUs; next we present a modified model where several heterogeneities are considered. Some of those heterogeneities are due to intrinsic differences between needles and syringes and biological vectors; others, such as those related to movements of individuals between communities, should apply to both biological vectors and injection apparatuses. An example of the calculations of R0 for a real IVDUs community is presented.

As countries in Asia strive to meet their universal access targets, harm-reduction programmes are yet to be scaled up to reach effective levels of coverage. Resource tracking and estimation of resource needs and gaps is critical to inform the financing decisions of major donors of harm-reduction programmes in the region. This study aimed at estimating the financial resource needs and gaps for scaling-up harm reduction in the region, building on previous research conducted by the Independent Commission on AIDS in Asia. The overall resource need for achieving universal access in the target population in 2009 was US $0.5 billion, with NSP and OST accounting for nearly 70% of the overall regional resource need. A significant resource gap, approximately 90%, of the resource need in 2009, was identified for harm reduction in the region, representing less than 2% of the overall global resource need to address AIDS. Additional resources will be required to support the introduction and scaling-up of integrated, comprehensive harm-reduction programmes that provide a full range of services to reduce HIV transmission among people who inject drugs.

BackgroundRecent studies have sought to describe HIV infection and transmission characteristics around the world. Identification of early HIV-1 infection is essential to proper surveillance and description of regional transmission trends. In this study we compare people recently infected (RI) with HIV-1, as defined by Serologic Testing Algorithm for Recent HIV Seroconversion (STARHS), to those with chronic infection.Methodology/principal findingsSubjects were identified from 2002-2004 at four testing sites in São Paulo. Of 485 HIV-1-positive subjects, 57 (12%) were defined as RI. Of the participants, 165 (34.0%) were aware of their serostatus at the time of HIV-1 testing. Introduction : The World Health Organization recently released guidelines on the use of pre-exposure prophylaxis (PrEP) for prevention of HIV infection among men and transgender women (TGW) who have sex with men based on results of randomized clinical trials. The aim of this commentary is to discuss the opportunities and challenges of incorporating PrEP into the Brazilian continuum of HIV care and prevention for men who have sex with men (MSM) and TGW. Discussion : Key aspects of the AIDS epidemic among MSM and TGW in Brazil and the comprehensive Brazilian response to the epidemic are presented. The universal access to health care provided through the Brazilian Unified Health System (SUS) and the range of prevention and care services already available countrywide to HIV-positive individuals and at-risk MSM and TGW are identified as the main facilitators for the implementation of PrEP. Limited PrEP awareness among MSM, TGW and health care providers, low HIV testing frequency and low HIV risk perception among MSM and TGW represent the core challenges to be addressed. Data generated by demonstration projects in Brazil will provide an important contribution to PrEP rollout in Brazil. Conclusions : The implementation of PrEP in Brazil is feasible. A synergistic rollout of treatment as prevention and PrEP will maximize public health and individual benefits of the country’s comprehensive response to the AIDS epidemic. Centros de Testagem e Aconselhamento em DST/HIV (CTA) oferecem, entre outras atividades, a realização de testes sorológicos anti-HIV e VDRL. Estes serviços dispõem de coleta sistemática de dados permitindo conhecer o perfil epidemiológico dos indivíduos infectados. O presente estudo descreve o perfil epidemiológico das pessoas com sorologia positiva que utilizaram um dos maiores CTA do país, durante os anos de 2001 e 2002. Foram utilizados dados de sorologia positiva e das entrevistas (pré e pós teste) realizadas de rotina. Obsevou-se 470 (4%) resultados positivos entre 10657 testes sorológicos realizados, sendo que grande parte (81,4%) dos usuários com sorologia positiva são do sexo masculino. Understanding the role of injection drug users (IDUs) in the epidemic and the political response thereto is a key factor in the control of the epidemic in Brazil. This paper summarizes some of the most important analyses of the Brazilian response to the HIV/AIDS epidemic among and from IDUs. Key elements of the response include the support of the Brazilian Universal Public Health System, the provision of universal access to highly active antiretroviral therapy, and the creation of harm reduction projects that are politically and financially supported by the federal government. The response among and from IDUs is a key element in overall control of the HIV/AIDS epidemic. The response to the epidemic among and from IDUs has been headed in the correct direction since its beginning and is now being intensively expanded.

Santos Metropolitan Region (SMR), State of Sao Paulo, historically is well known as being one of the areas with the largest number of AIDS cases in Brazil, especially among injection drug users (IDUs). The main objective of this study was to assess the effects of changes in drug-using patterns among IDUs on trends in HIV infection among IDUs in the 1990s.During 1991 through 1992 (wave 1; n = 214), 1994 through 1996 (wave 2; n = 135), and 1999 (wave 3; n = 108), we conducted three cross-sectional studies of IDUs. All participants were interviewed and tested for antibodies to HIV.The overall sample population was 69% male, and 87% of the sample population was under 40 years old. Eighty-four percent of the population had less than 9 years of education. HIV seroprevalence was 63% in wave 1, 65% in wave 2, and 42% in wave 3 ( p.001). Smoking of crack cocaine increased from 11% in wave 1 to 60% in wave 2 and 67% in wave 3 ( p.001). The prevalence of frequent injections (5 per day) decreased from 42% in wave 1 to 30% in wave 2 and 15% in wave 3 ( p.001).HIV prevalence decreased as injection frequency decreased and crack cocaine use increased. In SMR, patterns of drug use have been affecting the HIV epidemic more than scant public health intervention.

As part of Phase II of the World Health Organisation (WHO) Injecting Drug Use Multi-City Study, Rapid Assessment and Response (RAR) methodology was used in Sao Vicente City (Sao Paulo State, Brazil) to analyse the situation regarding injecting drug use and HIV infection. Over a period of 8 weeks, analysis of existing information, focus groups (four) and depth interviews (45) were conducted, with qualitative data analysis taking a further 12 weeks. This work was undertaken by a team of six researchers (working mostly part-time on the project), and the research process was overseen by an Advisory Community Group. Key topics addressed included: the general drug use situation; HIV/AIDS and drugs; and harm reduction. Through the use of qualitative analysis it was found that there was: a negative moral status associated with drug use and drug users; prejudices against this group; and a lack of information about health services and HIV prevention programmes within Sao Vicente city. In regard to risk behavior, injecting drug users reported awareness of risks and methods of prevention. However, safer sex was considered difficult by all participants. Interventions were proposed by the research team to minimize problems and gaps in provision, and some of these have already occurred. A review of existing quantitative data found that some key institutions do not collect systematic data on drug use. Conclusions of the study include the need for interventions and the progress on Harm Reduction proposals and AIDS prevention among IDUs.

To study the prevalence of HIV and infections with related transmission patterns the authors interviewed and obtained blood samples from 220 injecting drug users (IDUs) sampled by snowballing from the city of Santos in the state of Sao Paulo Brazil where the estimated number of IDUs (10000) comprises about 2% of the entire population. Seroprevalence of HIV hepatitis B and C viruses (HBV and HCV) syphilis and HTLV (1 and 2) was assessed and compared with that in 197 blood donors from the same city matched for age and gender. Risk behavior related to HIV was assessed by a standard questionnaire applied to the IDU sample. Univariate and multivariate analyses of the risk factors were performed. Seroprevalences found were 62% for HIV 75% for HCV 75% for HBV 34% for syphilis and 25% for HTLV (1 and 2) among IDUs which compare with 0.0% 2% 23% 12% and 1% for blood donors respectively. The risk for parenterally transmitted infections in this IDU community was higher than that for sexually transmitted infections (odds ratio [OR] for syphilis 3.57; hepatitis B 10.0; hepatitis C 100). The results of the multivariate risk analysis showed that daily rate of ID use >5 times/day (OR = 6.73) not changing behavior to avoid AIDS (OR = 3.28) ID use >15 days/month (OR = 2.72) and ID use in the last 2 months (OR = 2.23) were the risk behaviors significantly associated with HIV infection. The epidemic of HIV/AIDS linked to injecting drug usage is one of the most explosive in recent years. After a historical epicentre in Europe, South and North America, at present it is clearly the main cause of dissemination of the epidemic in Eastern Europe and some key Asian countries. Recently, 10 African countries reported the spread of HIV through people who inject drugs (PWID), breaking one of the final geographical barriers to the globalization of the epidemic of HIV among and from PWID. Several countries of the Asia and Pacific Region have HIV epidemics that are driven by injecting drug usage. Harm reduction interventions have been implemented in many countries and potential barriers to implementation are being overcome. Harm reduction is no longer a marginal approach in the Region; instead, it is the core tool for responding to the HIV/AIDS epidemic among PWID. The development of a comprehensive response in the Region has been remarkable, including scaling up of needle and syringe programmes (NSPs), methadone maintenance treatment (MMT), and care, support and treatment for PWID. This development is being followed up by strong ongoing changes in policies and legislations. The main issue now is to enhance interventions to a level that can impact the epidemic. The World Health Organization (WHO) is one of the leading UN agencies promoting harm reduction. Since the establishment of the Global Programme on AIDS, WHO has been working towards an effective response to the HIV epidemic among PWID. WHO's work is organized into a number of components: establishing an evidence base; advocacy; development of normative standards, tools and guidelines; providing technical support to countries; ensuring access to essential medicines, diagnostics and commodities; and mobilizing resources. In this paper, we trace the course of development of the HIV/AIDS epidemic among and from PWID in the Western Pacific and Asia Region (WPRO) as well as WHO's role in supporting the response in some of the key countries: Cambodia, China, Lao PDR, Malaysia, the Philippines and Viet Nam.

Indonesia has an explosive HIV/AIDS epidemic starting from the beginning of this century, and it is in process to build its response. Reported AIDS cases doubled from 2003 – 2004, and approximately 54% of these cases are in people who inject drugs. Indonesia is the 4th largest country in population in the world, a predominantly Muslim country with strong views on drug users and people living with HIV/AIDS. Globally speaking, Indonesia has one of the most explosive epidemics in recent years. IHPCP (Indonesia HIV/AIDS Prevention and Care Project) is a joint support project (primarily AusAID-based) that works in partnership with the Government of Indonesia. IHPCP has been a key player of in the country's response, particularly pioneering NSP; stimulating and supporting methadone programs, and being key in promoting ARV for people who currently inject drugs. The project works via both the public health system and NGOs. It is still early to measure the impact of current interventions; however, this paper describes the current status of Indonesia's response to the HIV/AIDS crisis among people who inject drugs, and analyses future challenges of the epidemic in Indonesia.

Several strategies aim at characterizing the AIDS epidemic in different parts of the world. Among these, the identification of recent HIV-1 infections using the recently described serologic testing algorithm for recent human immunodeficiency virus (HIV) seroconversion (STARHS) strategy was employed in four testing sites of the City of Sao Paulo Public Health Department (CSPPHD). Those identified as recently infected were invited to participate in a prospective clinical and laboratory evaluation study. We describe the establishment of the patient identification network and the success in enrolling the participants, as well as their clinical and laboratory characteristics. From May to December 2002, 6,443 persons were tested for HIV in the four participating sites, of whom 384 (5.96%) tested HIV-1 positive; 43 (11.2%) of them were identified as recently infected. Twenty-two were successfully enrolled in the follow-up study, but three of them did not meet clinical and/or laboratory criteria for recent HIV-1 infection. After these exclusions, the laboratory findings revealed a median CD4+ T lymphocyte count of 585 cells/microL (inter-quartile range 25-75% [IQR], 372-754), a CD8+ T lymphocyte count of 886 cells/microL (IQR, 553-1098), a viral load of 11,000 HIV-RNA copies/mL (IQR, 3,650-78,150), log10 of 4.04 (IQR 3.56-4.88). The identification of recent HIV infections is an extremely valuable way to evaluate the spread of the virus in a given population, especially when cohort studies, considered the gold standard method to evaluate incidence, are not available. This work demonstrated that establishing a network to identify such patients is a feasible task, even considering the difficulties in a large, resource-limited country or city.
