Objective To evaluate the responsiveness in terms of correlation of the Hospital Universitario La Princesa Index (HUPI) comparatively to the traditional composite indices used to assess disease activity in rheumatoid arthritis (RA), and to compare the performance of HUPI-based response criteria with that of the EULAR response criteria. Methods Secondary data analysis from the following studies: ACT-RAY (clinical trial), PROAR (early RA cohort) and EMECAR (pre-biologic era long term RA cohort). Responsiveness was evaluated by: 1) comparing change from baseline (Delta) of HUPI with Delta in other scores by calculating correlation coefficients; 2) calculating standardised effect sizes. The accuracy of response by HUPI and by EULAR criteria was analyzed using linear regressions in which the dependent variable was change in global assessment by physician (Delta GDA-Phy). Results Delta HUPI correlation with change in all other indices ranged from 0.387 to 0.791); HUPI's standardized effect size was larger than those from the other indices in each database used. In ACT-RAY, depending on visit, between 65 and 80% of patients were equally classified by HUPI and EULAR response criteria. However, HUPI criteria were slightly more stringent, with higher percentage of patients classified as non-responder, especially at early visits. HUPI response criteria showed a slightly higher accuracy than EULAR response criteria when using Delta GDA-Phy as gold standard. Conclusion HUPI shows good responsiveness in terms of correlation in each studied scenario (clinical trial, early RA cohort, and established RA cohort). - Abstract
The authors present their experience in diagnosing and treating interstitial nephritis with the development of chronic renal failure in a patient with generalized sarcoidosis, by involving the intrathoracic and peripheral lymph nodes, liver, spleen, subcutaneous fat, lung, as well as with the severe salivary and lachrymal gland lesions that imitate the clinical picture of Schogren's disease.

- Abstract
The study of the effects of intensive treatment on activity of free-radical oxidation in lymphocyte membranes was conducted in 15 rheumatoid arthritis (RA) patients with systemic involvement, stage II-III activity who had failed previous basic therapy because of unefficiency or intolerance. The synchroneous programmed intensive therapy was performed in 3 stages. Stage 1: three sessions of plasmapheresis with 3 day intervals, synchronous introduction of 30 mg methotrexate and 500 mg methylprednisolone. Stage 2: the same procedures once a week for the following 3 weeks. Stage 3: a monthly intensive therapy for 3 months. After the end of stage 3 all the patients received methotrexate 7.5 mg weekly per os for 6 months. The above intensive therapy of RA patients had an antiinflammatory effect, changed peroxidation reactions in lymphocyte membranes by lowering the level of pro-oxidants and stimulation of antioxidant system. No positive changes in the lipid status occurred.

- Abstract
Features of clinical course and symptoms of systemic lupus erythematosus are analysed in SLE patients with sepsis (n = 11) which was defined as a systemic inflammatory reaction to verified infection with polyorganic insufficiency. Statistic analysis was made with the program MS Excel 7.0. Sepsis, activating factors, antibacterial and immunosuppressive therapy are discussed. Outcomes are described. Differential diagnosis with paraneoplastic syndrome is illustrated.
