39. Dementia and mild cognitive impairment in China: From the public health perspective.

Abstract

Background: China has the largest population of patients with dementia and mild cognitive impairment (MCI) in the world, imposing a heavy burden on the public and healthcare systems. The major concerns from the public health perspective include: prevalence, health economic burden, risk factors, and management strategy. Method: Statistical analyses were performed on the largest and most comprehensive dementia and MCI cohort study (fig. 1) in China. Systemic review was also done on the studies from the past 10 years. Result: For the prevalence, our data showed that, the overall prevalence of dementia was 6.0% and of MCI was 15.5%. This represented 15.07 million individuals with dementia and 38.77 million individuals with MCI (fig. 2). For the health economic burden, our systematic review showed that the annual total costs associated with dementia would be $248.71 billion in 2020, $507·49 billion in 2030, $1·00 trillion in 2040, and $1·89 trillion in 2050 in China; on a global scale, these costs would be $1·33 trillion in 2020, $2·54 trillion in 2030, $4·83 trillion in 2040, and $9·12 trillion in 2050 (fig. 3). For the risk factors, our study found that MCI and dementia shared 9 similar, modifiable risk factors, including rural residence, fewer years of education, living alone, smoking, hypertension, hyperlipidaemia, diabetes, and heart and cerebrovascular disease. Conclusion: The above public health data indicated that dementia and MCI population has become a heavy burden not only in China but around the world, which called for us to develop systematic management strategies, including: 1) prevention strategy targeting the MCI population by controlling for the modifiable risk factors and increasing public awareness; 2) monitoring strategy for all people with MCI by anti‐dementia national surveillance network that does regular cognitive evaluation and training; 3) early diagnosis and treatment strategy for people with dementia by trained dementia professionals; 4) care strategy by an integrative dementia nursing system. Abstract

The apolipoprotein E (APOE) ε4 allele is a genetic risk factor for Alzheimer's disease, whereas educational attainments have protective effects against cognitive decline in aging and patients with Alzheimer's disease. We examined the possible effects of years of education and APOE genotype on the topological properties of the functional network in normal aging, mild cognitive impairment and Alzheimer's disease. The years of education showed a significant, negative association with the local efficiency, clustering coefficient and small-worldness of functional networks in APOE ε4 noncarriers but not in ε4 carriers. These associations were mainly observed in normal aging and were reduced in mild cognitive impairment and Alzheimer's disease. Moreover, regions of the inferior frontal gyrus, temporal pole, and cuneus also showed correlations between education and nodal degree. Our findings demonstrated that the protective effects of education persist in APOE ε4 noncarriers but diminish in ε4 carriers. Abstract

Objective: To evaluate the state of evidence for the beneficial and harmful effects of Tai Chi on non-motor disorders in post-stroke patients. Design: Systematic review and meta-analysis of published studies. Subjects: Stroke survivors who received conventional rehabilitation therapy or Tai Chi training. Data sources: We searched seven electronic literature databases and one clinical registry platform to collect data from randomized controlled trials published up to July 26, 2020. Results: A total of 11 randomized controlled trials with 723 stroke survivors met the inclusion criteria, of which six were included in the meta-analysis. Among the 11 studies, one was assessed as "low", eight were assessed as "moderate", and only two were assessed as "high" for the assessment of methodologic quality. Compared to patients who received conventional rehabilitation therapy, those who received Tai Chi training showed greater improvement in scores of depression (standardized mean difference (SMD) [95% confidence interval (CI)] = 0.36 [0.10, 0.61], Grading of Recommendations Assessment, Development, and Evaluation [GRADE]: very low). There were no differences in the improvements in post-stroke global mental disorders (mean difference (MD [95% CI] = 6.15 [−3.05, 15.36], GRADE: moderate) or sleep disorders (MD [95% CI] = 0.33 [−1.51, 1.81], GRADE: low) between Tai Chi and control groups. Conclusion: Tai Chi may alleviate post-stroke depression in stroke survivors but has no clear effects on post-stroke cognitive and sleep disorders. [ABSTRACT FROM AUTHOR]

Abstract

We report a PS1 gene mutation (Val 97Leu) in a Chinese familial Alzheimer's disease (FAD) pedigree and a cell model of FAD built by transfecting PS1 v97L mutants into human neuroblastoma SH-SY5Y cells. To test our hypothesis that the PS1 v97L mutation is pathogenic, we investigated possible alterations in transport regulation and intracellular Ca(2+) homeostasis in endoplasmic reticulum (ER). Grp78 is an ER-resident chaperone mediating the unfolded protein response (UPR) and is a key regulator of ER stress transducers. KDEL is a 4-amino-acid retention sequence made of Lys-Asp-Glu-Leu-COO. KDEL is a "resident" sequence as protein residence in ER is consistently associated with KDEL at the C-extremity. Our group used KDEL recognizing anti-Grp78 monoclonal antibody to detect the level of Grp78. We found increased KDEL level in all the transfected cells including cells transfected with PS1 V97L genes, wild-type and the mock. However cells with PS1 V97L mutation expressed a relatively lower KDEL compared with the wild-type and the mock, and a significantly lower Grp78 level compared with the wild-type, the mock and control. These results suggest that PS1 V97L mutation impedes intracellular transport regulation in ER. PS1 V97L mutation mediates increased ER Ca(2+) content in human neuroblastoma SH-SY5Y cells. The increased intracellular Ca(2+) release is due to depleted Ca(2+) storing content of ER but not due to extracellular environment as capacitative Ca(2+) entry (CCE) is invariant. PS1 V97L mutation interferes with intracellular Ca(2+) homeostasis. Abnormal transport regulation and Ca(2+) homeostasis attributed to PS1 V97L mutation may be associated with the pathology of Chinese familial FAD.
