37. Association between short and long sleep durations and cardiovascular outcomes: a systematic review and meta-analysis

Abstract

Background: A shorter sleep duration has been identified as a risk factor for cardiovascular diseases and increased mortality. It has been hypothesized that a short sleep duration may be linked to changes in ghrelin and leptin production, leading to an alteration of stress hormone production. Here, we conducted a systematic review and meta-analysis to investigate the potential relationship between a sleep duration and cardiovascular disease mortality.Methods: We conducted a comprehensive search of Ovid Medline In-Process and other non-indexed citations, Ovid MEDLINE, Ovid Embase, Ovid Cochrane Central Register of Controlled Trials, and Scopus from database inception to March 2017. Observational studies were included if the studies reported hazard ratios or odds ratios of the associations between sleep durations (short and long) and cardiovascular disease mortality. Data were extracted by a reviewer and then reviewed by two separate reviewers. Conflicts were resolved through consensus. Using the DerSimonian and Laird random effects models, we calculated pooled hazard ratios and pooled odds ratios with 95% confidence intervals (CI). Subgroup analyses were performed to explore potential sources of heterogeneity. The quality of the included studies and publication bias were assessed.Results: In total, our meta-analysis included 19 studies (31 cohorts) with a total of 816,995 individuals with 42,870 cardiovascular disease mortality cases. In pooled analyses, both short (risk ratio 1.19; 95% CI 1.13 to 1.26, P<0.001, I2=30.7, Pheterogeneity=0.034), and long (risk ratio 1.37; 95% CI 1.23 to 1.52, P<0.001, I2=79.75, Pheterogeneity<0.001) sleep durations were associated with a greater risk of cardiovascular disease mortality.Conclusions: Both short (<7 hours) and long sleep durations (>9 hours) can increase the risk of overall cardiovascular disease mortality, particularly in Asian populations and elderly individuals. Future epidemiological studies would ideally include objective sleep measurements, rather than self-report measures, and all potential confounders, such as genetic variants.

Abstract

A recent article advanced the argument that New South Wales mandatory reporting legislation requiring teachers to report suspected child abuse operates to require harm caused to children by school bullying to be reported by teachers to government child protection agencies. The article raises important issues concerning the nature and scope of mandatory reporting legislation in New South Wales and throughout Australia. In this article, we explore the argument's strengths and weaknesses. We conclude that, due to the nature and context of mandatory reporting laws, the nature of child protection systems, the differences between abuse perpetrated by parents or caregivers as opposed to bullying inflicted by children, and the presence of schoolbased responses to bullying, the more persuasive view is that the laws do not and should not require harm caused by school bullying to be reported by teachers to government child protection agencies.

46. Birth to three years: A review of the literature around the care and education of young children and babies

Abstract

Birth to three years is a critically important time for children's learning, health and wellbeing. From birth (and before birth) children are active learners, capable of interacting, as competent persons, with their world. Evidence from a range of disciplines and theoretical perspectives demonstrates the importance of the first years of life for children's present and future lives. This brief, plain language review is designed as a resource for practitioners who work with children - to provide quick-glance evidence of policy, research and practice in support of the best possible start for our youngest citizens. It follows a comprehensive review of birth to three conducted by David, Goouch, Powell and Abbott (2003) as part of the United Kingdom's Birth to Three Matters Project and Framework for Effective Practice. While the current review is more annotative than that of David and colleagues (2003), the two reviews concur that very young children should be respected as people in their own right in a society that recognizes and caters for their capabilities; and with warm, responsible people who provide opportunities for their holistic learning, health and wellbeing. The review shows that: children live in the present and what happens in the present impacts on their future; children's lives are diverse and the contexts of their lives are changing; children are active and competent learners; children are citizens and the care and education of children and those around them should be a shared responsibility.

47. Young children enacting governance : child’s play?

Abstract

Schools, homes and communities are increasingly perceived as risky spaces for children. This concern is a driving force behind many forms of governance imposed upon Australian children by well-meaning adults. Children are more and more the subjects of both overt and covert regulation by teachers and other adults in school contexts. Are children, though, passive in this process of governance? It is this issue that is the focus of this paper. In order to respond to the question of how young children enact governance in their everyday lives, video-recorded episodes of naturally occurring interactions among children in a preparatory classroom were captured. These data were then transcribed and analysed using the methods of conversation analysis and membership categorisation analysis. This paper shows a number of strategies that the children used when enacting governance within their peer cultures in the classroom. It focuses specifically on how adult and child-formulated rules and social orders of the classroom were drawn upon and developed in order to control and govern during the interaction. This paper illustrates that children are not passive in enacting governance, but actively and competently enact governance through their peer cultures. These findings are significant for educators to consider, as they help to develop an understanding of the complex social orders that children are continually constructing in the early childhood classroom.
