Aim: The literature on the effects of the fibromyalgia syndrome (FMS) association with hepatitis C virus (HCV) on quality of life and vitamin D levels is limited. We aimed to evaluate the frequency of fibromyalgia in patients with chronic hepatitis C virus (HCV) and also to analyze the relationship between the presence of fibromyalgia and vitamin D parameters. Methods: This controlled, cross-sectional study of 123 participants (72 patients with HCV and 51 in the control group) was conducted in a tertiary hospital. The study enrolled age- gender and BMI-matched control groups. Patients were diagnosed with fibromyalgia and arthritis as per the 2010 American College of Rheumatology diagnostic criteria. Fibromyalgia Impact Questionnaire (FIQ), Nottingham Health Profile (NHP) and the level of vitamin D were performed in all patients. Results: The presence of fibromyalgia, the level of vitamin D and the score of FIQ were statistically different between HCV (+) and control groups. The score of social isolation was higher in the HCV (+) group, whereas all other scores were similar in both groups according to the NHP scale. Based on the presence or absence of fibromyalgia, all scores of FIQ and NHP scales were higher in a subgroup with fibromyalgia. This study documented a prevalence of 56.9% of FMS in patients with HCV (+) in comparison to the control group Conclusion: This study documented a high prevalence of 56.9% of FMS in patients with HCV (+) in comparison to the control group. While emphasizing raising awareness of fibromyalgia, its relationship with HCV, vitamin D, quality of life, age, gender, BMI, and tender points was revealed via regression analysis.

Aim:The Juvenile Spondyloarthritis Disease Activity Index (JSpADA) is the only disease activity score specifically validated for children with enthesitis-associated arthritis (ERA). It was developed to address the need for an effective measurement tool to assess disease activity in this population. We aimed to evaluate the clinical course of patients with ERA using JSpADA and to compare the effects of treatment modalities using JSpADA.Methods:This cross-sectional observational study enrolled 61 patients with ERA who were followed up between January 2020 and 2023. Clinical features, treatment options, and JSpADA were noted in electronic medical files. The effectiveness of treatment modalities was compared by JSpADA.Results:The median age of onset of the group was 10 [interquartile range (IQR), 9-15] years. The study cohort included three groups of patients: 1) DMARD received (n=34); 2) biologic drug received (n=14); 3) DMARD and biological combination received (n=13). Forty-three cases (70%) presented with peripheral arthritis, including enthesitis, whereas 18 (30%) patients had axial involvement. At disease onset, the median JSpADA scores were 2 (IQR, 2-3), 2.5 (IQR, 2-3), and 3.5 (IQR, 2.5-5) in groups 1, 2, and 3, respectively (p=0.27). At the first year of follow-up, there was a significant improvement in the disease activity of groups 1 and 2 (p=0.02 and p=0.04). However, there was no significant reduction in JSpADA values in the third group.Conclusion:In patients with ERA, intermittent JSpADA evaluation during visits can guide the objective and accurate follow-up and treatment response of patients.

Konteyner taşımacılığının, denizyolu ticaretindeki önemi her geçen gün artmaktadır. Konteyner hacminin etkili tahmini ise liman planlaması ve işletimi için bir karar desteği sağlamaktadır. Bu nedenle liman yönetimlerinin geleceğe yönelik planları açısından tahminleme çalışmaları önemli bir rol oynamaktadır. Bu çalışmada, Antalya’da bulunan Port Akdeniz Limanı için yapılan tahmin modellerinde Ocak 2008-Aralık 2017 (120 ay) dönemi konteyner istatistikleri veri seti olarak kullanılmıştır. Liman işletmesinin yük talep tahmini, konteyner bazında ve mevsimsel farklılıklar dikkate alınarak, Ocak 2018-Aralık 2019 (24 ay) dönemi için yapılmıştır. Gri Tahmin ve Box-Jenkins yöntemlerinin kullanıldığı çalışmada, konteyner tahminleri Gri Model (1,1) ve ARIMA (0,1,0)x(0,1,1)12 modelleri ile analiz edilmiştir. Tahmin sonuçları başarı kriterleri ile değerlendirildiğinde, Gri Model (1,1)’in MAPE ve MAE değerlerinin daha düşük olduğu gözlemlenmiştir. Ancak hem RMSE ve MSE hem de sapma değerleri dikkate alındığında ise; ARIMA (0,1,0)x(0,1,1)12 modelinin daha iyi ve uygun tahmin değerleri verdiği tespit edilmiştir.

The deliberate self-harm behaviour which defined as attempting to own body resulting in tisue damage without conscious desire of peolple to die, is a major public health problem worldwide. The causes of deliberate self- harm, risk factors, the relationship between mental disorders and treatment strategies are not fully known. Deliberate self- harm can be observed together with psychiatric disorders such as borderline personality disorder, histrionic personality disorder, eating disorders and mood disorders. Also, deliberate self-harm must be distinguished from suicidal behavior. Psychologi-cal trauma has been suggested as a risk factor for deliberate self- harm behavior. Trauma and traumatic events have long been associated with deliberate self- harm behavior. This study investigates the relationship between shipping connectivity, environmental quality, financial development, and economic growth among 14 countries in the Regional Comprehensive Economic Partnership (RCEP) from 2006 to 2019. Using panel-corrected standard error, Dynamic Seemingly Unrelated Regression, and Driscoll–Kraay estimation methods, the analysis reveals that shipping connectivity significantly contributes to financial development and economic growth, while also exerting a negative impact on environmental quality. These findings suggest that the maritime sector can have significant impacts not only on economic growth and financial development but also on environmental sustainability. In countries where maritime shipping has increased, particularly with the growth of trade, positive outcomes are observed in terms of financial development and economic growth, while negative impacts on environmental quality are also evident. This study provides insights for policymakers to develop strategies that maximize economic benefits while reducing environmental harm in order to achieve sustainable development in the maritime sector. Objectives: We aimed to outline the demographic data, clinical spectrum, and treatment approach of sarcoidosis in a large group of patients and sought to figure out the variations of early-onset (EOS) and late-onset paediatric sarcoidosis (LOS). Methods: The study followed a retrospective-descriptive design, with the analysis of medical records of cases diagnosed as paediatric sarcoidosis. Results: Fifty-two patients were included in the study. The median age at disease onset and follow-up duration were 83 (28.2–119) and 24 (6–48) months, respectively. Ten (19.2%) cases had EOS (before 5th birthday) and 42 (80.7%) cases had LOS. The most common clinical findings at the time of the disease onset were ocular symptoms (40.4%) followed by joint manifestation (25%), dermatological symptoms (13.5%), and features related to multi-organ involvement (11.5%). Anterior uveitis was the most common (55%) one among ocular manifestations. Patients with EOS displayed joint, eye, and dermatological findings more commonly than patients with LOS. The recurrence rate of disease in patients with EOS (5.7%) and LOS (21.1%) were not statistically different (P = .7). Conclusions: Patients with EOS and LOS may present with variable clinical features and studies addressing paediatric sarcoidosis cases in collaboration between disciplines will enhance the awareness of this rare disease among physicians and assist early diagnosis with lesser complications. Objective: Based on the pathophysiological similarities between alcohol use disorder (AUD) and obsessive compulsive spectrum disorders, as well as the close association between traumatic experiences and addiction, the goal of this study was to assess the relationship between obsessive beliefs and childhood traumas in patients with AUD by comparing them with the control group Material and Method: The data of 60 AUD cases who applied to the Alcohol and Substance Treatment Center outpatient clinic consecutively and 56 healthy control groups were evaluated. Structured Clinical Interview for DSM-5, Obsessive Beliefs Questionnaire, Childhood Trauma Questionnaire, Hamilton Anxiety Inventory, and Hamilton Depression Inventory were applied to both groups. The Addiction Profile Index, which evaluates the severity of addiction, was applied to the patient group. Data were evaluated with SPPS 22. Results: Obsessive beliefs, traumatic experiences, depression and anxiety scores were found to be significantly higher in AUD cases than in the control group. No significant relationship was found between traumatic experiences and obsessive beliefs. The most important predictors of obsessive beliefs were found to be anxiety and depression scores. Conclusion: The current study is important since it is the first to assess how obsessive beliefs and childhood traumas relate to AUD patients. Further analysis in a larger sample in this area will be helpful in identifying risk factors and creating preventive interventions for AUD, a disorder that is difficult to treat and relapse. Abstract Background The field of transitional care for chronic conditions in adolescents, notably juvenile idiopathic arthritis (JIA), is rapidly growing. Transitioning these patients to adult healthcare systems presents significant challenges in practical implementation. Consequently, it would be appropriate for each country to develop a transition program tailored to its specific infrastructure. To pursue this goal, a Delphi study was conducted to identify the key components of transitional care in JIA. Methods Three panels and two rounds were held consisting of adolescents and young adults, parents, and clinicians (pediatric or adult rheumatologists). As a result, feedback on acceptance of the key statements of transitional care was obtained using the Delphi method. Results Out of 102 contacted, 88 (86.3%) participants responded to the Round 1 survey, which included 48 clinicians, 20 youths, and 20 parents. In Round 2, the number of clinicians dropped to 29, while the number of youths and parents remained constant. Based on expert opinions, 29 statements were selected for the first round. Statements that received ≥ 70% approval in the first round advanced to the next round. Sixteen statements did not achieve ≥ 70% approval. Of the remaining, 12 were reviewed in the second round, while four were excluded. Conclusion Although consensus has been reached on the basic transitional care issues for JIA patients, several issues still need to be agreed upon. Acceptance and applicability of the final 20-item checklist in clinical practice are critical for advancing JIA transition care in Turkey.

Prompt engineering techniques are instructions that enable large language models (LLMs) to solve real-world problems more effectively. These techniques enhance the capabilities of LLMs to generate accurate and efficient responses. Our study examines the challenge of acquiring comprehensive and efficient information in the metaverse through the application of various prompt engineering techniques. The main objective is to improve the accuracy and effectiveness of metaverse-related responses by leveraging LLM capabilities. In this study, 100 questions were generated using GPT, GEMINI, QWEN, and MISTRAL language models focusing on the metaverse. Our experiments indicated that responses often included unrelated information, highlighting the need for prompt engineering techniques. We applied knowledge-based, rule-based, few-shot, and template-based prompt engineering techniques to refine the responses. The performance of GPT, GEMINI, QWEN, and MISTRAL models were evaluated based on criteria including accuracy, timeliness, comprehensiveness, and consistency. Our findings reveal that prompt engineering techniques significantly enhance the efficacy of LLMs in providing improved information retrieval and response generation, aiding users in efficiently acquiring information in complex environments like the metaverse.
