Background: Liver related pathologies including Hepatocellular Carcinoma (HCC) is a universal problem. Innate immunity receptors were accused in the etiopathogenesis of HCC with many conflicts. TLR4 is one of pathogen recognition receptors involved in the pathogenesis of many diseases and malignancies. TLR4 receptor polymorphisms were investigated in HCV related morbidities along with inconclusive results Objectives: to study the role of TLR4 rs 2149356 and rs 1927914 genotypes polymorphisms in HCV related HCC development. Methodology: 200 Chronically infected HCV patients were enrolled in this study. they were divided according to lab and clinical data into 100 CHC group and 100 HCC patients who were compared to health individual. The blood samples obtained were further proceed to full lab and TLR4 genotyping by RFLP-PCR technique Results: GT genotype and T allele of TLR4 rs 2149356 at 95% CI of 0.38 (0.21-0.70) was significantly increased in control group than in HCC and CHC groups. At 0.32(0.17-0.63) TLR4 rs 1927914 C allele and CT genotype was significantly increased in Controls than diseased groups while T allele is significantly increased in HCC than control group. Conclusions: TLR4 genotypes may play a protective role against HCC development among chronic HCV patients.

Chronic hepatitis C (CHC) is a leading cause of cirrhosis and hepatocellular carcinoma (HCC). This study aimed to study the association of IL28B, toll-like receptor (TLR) 7, cytomegalovirus and advanced liver disease.Four groups were included; control (n = 125, 25.9%), CHC (n = 114, 23.6%), liver cirrhosis (n = 120, 24.8%), and HCC (n = 124, 25.7%).In CHC group, patients were mainly F1 (60%) followed by F2. IL28B genotype CC percentage was higher in control group than the CHC and cirrhosis groups. CT and TT genotypes were higher in the CHC and cirrhosis groups than control group. The C allele was higher in the control group than the CHC, cirrhosis and HCC groups and the opposite with the T allele. Control and CHC had same TLR7 alleles. Cirrhosis patients and HCC had lower TLR 7 A allele and higher G allele than the control group. Both cirrhosis and HCC groups had statistically significant higher percentage of the AG and GG genotypes than the control group. Patients with HCC had higher cytomegalovirus infection percentage than cirrhosis and CHC group (38.7% vs 20% vs 16.7%), respectively.IL28B, TLR7 SNPs and cytomegalovirus infection are risk factors for advanced liver disease in hepatitis C patients.

Background: Liver diseases including HCC are life-threatening morbidities. Mechanisms of HCC are still unclear. Cytomegalovirus (CMV) is DNA virus characterized by latency and cellular transformation. Genetic polymorphisms especially SNPs are the most common genetic variation. It plays a critical role in regulation of cellular interaction and cytokine production. CMV infection and its role in HCC is under investigation. Objectives: to profile IL28B single-nucleotide polymorphism in development of HCC in chronic HCV, and identify the relationship between human cytomegalovirus infection, IL28B SNPs and HCC. Methodology: 120 blood samples were obtained from HCV patients; divided according to clinical, radiological and laboratory data into three equal groups, forty patients each: HCV, HCC, LC groups, and 40 normal persons as controls. SNPs of interleukin-28 (IL28B rs12979860) by RFLP and CMV viral DNA by real-time polymerase chain reaction were investigated. Results: IL28B /CC genotype was increased significantly in controls and TT genotype in HCC group. A positive correlation among HCV viral load, CMV with the genotype IL28B rs12979860/TT especially in HCC group. Conclusions: IL28B rs12979860/TT is expressed in HCC patients making its role in development of HCC in CMV positive HCV patients cannot be denied.

Till now the exact mechanism and effect of biogenic silver nanoparticles on fungus is an indefinable question. To focus on this issue, we prepared hydrothermal assisted thyme coated silver nanoparticles (T/AgNPs) and their toxic effect on Candida isolates is described. The role of thyme (Thymus Vulgaris) in the reduction of silver ions and stabilization of T/AgNPs are estimated by Fourier transforms infrared spectroscopy, structure and size of present silver nanoparticles were detected via atomic force microscopy as well as high-resolution transmission electron microscopy. The biological activity of T/AgNPs was observed against Candida isolates from COVID-19 Patients. Testing of virulence of Candida species using Multiplex PCR. T/AgNPs proved highly effective against Candida albicans, Candida kruzei, Candida glabrata and MIC values ranging from 156.25 to 1,250 µg/mL and MFC values ranging from 312.5 to 5,000 µg/mL. The structural and morphological modifications due to T/AgNPs on Candida albicans were detected by TEM. It was highly observed that when Candida albicans cells were subjected to 50 and 100 µg/mL T/AgNPs, a remarkable change in the cell wall and cell membrane was observed.

Background As physicians in a referral hospital, we observed the association between history of enteric fever and somatic disorders associated with low mood. At the Al-Hussein University Hospital, Cairo and the National Liver Institute Hospital, Menoufia, we receive patients from all over Egypt, including rural areas where enteric fever is endemic. Aim Here in, 60 Egyptian patients referred to us for evaluation of different somatic disorders are reported. Methods After extensive evaluations, the patients' symptoms were function-related. Also, their typhoid carrier states were documented, and the severity of depression using Hamilton-D (HAM-D) questionnaire was evaluated and recorded. All patients were treated with ceftriaxone, 2 gm, IV, daily for 15 days. The clinical evaluation and Hamilton score were reassessed at the end of the treatment and 6 weeks thereafter. The patients did not receive any anti-depressant nor anti-anxiety treatment during their course. Typhoid carrier was defined by documenting the history of typhoid fever that was diagnosed by culturing the Salmonella species, and not by serology, isolated from stool culture along with febrile condition, plus the absence of fever in the past 3 weeks. The Widal test was not accepted as a criterion for enrollment. Results Patients showed clinically significant improvement in the somatic complaints, and their HAM-D score immediately post-treatment that was consolidated for 6 weeks post-treatment completion. Conclusion In this study, the typhoid carrier was associated with the psychosomatic depression that improved by antibiotic therapy.

Aim To assess the relationship between vitamin D receptor (VDR) gene polymorphisms at FokI site and hepatocellular carcinoma (HCC) in chronic liver disease patients. Methods The study enrolled 70 patients with chronic liver disease; of them 20 patients had HCC. Specific primer sequences of the VDR FokI C > T single nucleotide polymorphisms rs10735810, was used to amplify VDR gene. Allele specific restriction enzymes FokI was used to digest the amplified PCR products and the bands of specific PCR-RFLP fragments were resolved on 2% Ethidium bromide agrose gel electrophoresis. FokI genotypes was identified by their specific banding pattern on the gel electrophoresis via ultraviolet illumination. Results In chronic liver disease the frequency of FF, Ff and ff genotypes were 50%, 43% and 7% respectively. The wild type F and the mutant f allele had a frequency of 71%, 29% respectively. In HCC group the frequencies of FokI FF, Ff and ff genotypes were 55%, 30% and 15%; relative to 48%, 48% and 4% in non HCC respectively. F allele had a frequency of 72%, and 70%; and f allele had 28% and 30% respectively. VDR FokI genotypes and allele frequencies were not significantly associated with HCC in chronic liver diseases. The carriage of FokI polymorphisms ff genotype was associated with increased tumor size and higher AFP levels. FokI polymorphisms did not show any significance association with tumor stage, portal vein invasion or lymph node metastasis. Conclusion VDR FokI genotypes and allele frequencies were neither significantly associated with HCC, HCV nor were risk for progression of chronic liver disease into decompensated liver disease.

Aim To evaluate the possible association between vitamin D receptor single nucleotide polymorphisms and hepato cellular carcinoma in Egyptian patients with chronic liver disease. Method Specific primer sequences of the vitamin D receptor gene rs1544410, rs7975232, and rs731236 were used to detect vitamin D receptor polymorphisms in blood sample by polymerase chain reaction restriction fragment polymorphism. Allele specific restriction enzymes BsmI, ApaI and TaqI were used to digested PCR amplified products and the bands of specific PCR-RFLP fragments were resolved by 2% Ethidium bromide agrose gel electrophoresis. Genotypes and alleles were identified by their specific banding pattern on the gel electrophoresis via ultraviolet illumination. Result The frequencies of the 3 single nucleotide polymorphisms BsmI, ApaI, and TaqI genotype and the haplotype distribution were similar in patients with chronic liver disease either with or without hepato-cellular carcinoma. Neither of vitamin D receptor gene polymorphisms were a risk or protective factor for HCC; nor did these polymorphisms alter the risk for HCC development by coexisting HCC with chronic hepatitis C virus infection. TaqI polymorphism was associated with increased risk of progression of chronic liver disease to liver cell failure and decompensated liver disease. Conclusion vitamin D receptor single nucleotide polymorphisms at BsmI, ApaI, and TaqI did not provide significant association with HCC cancer risk or protection in the overall analysis in Egyptian population with chronic liver disease.

IntroductionThe development and progression of hepatocellular carcinoma (HCC) is a multistage process involving the deregulation of genes that are crucial to cellular processes. Multiple risk factors are correlated with HCC. MicroRNA is differentially expressed in development of different types of malignancies, including hepatic malignancy. Single nucleotide polymorphisms (SNPs) are the most common sequence variation in human genome. SNPs in miRNAs may affect transcription, processing or target recognition and result in malignant diseases. Aim: To determine the association between micro-RNA gene polymorphisms and development of HCC in Egyptian Patients.Material and methodsThis study included 200 individual who were matched in age and sex. Tumor staging was done using BCLC staging system. Quantification and genotyping of Micro-RNA were performed.ResultsAmong the 200 patients, 2 groups were described: group I included 90 HCC patients with a male majority (72.2%) and 110 controls in group II. Diabetes and hypertension were detected as risk factors. Three microRNA SNPs were assayed. There was a significant association between rs10061133 miR-499b and the risk of HCC. The genotypes GG or G allele were associated significantly to an increased risk of HCC (GG: OR (95% CI) = 2.91 (1.23-4.22), p = 0.013; G allele: OR (95% CI) = 1.79 (1.12- 2.15), p = 0.026) compared with the genotype of AA or AG or A allele.ConclusionsThere is an association between the mi-RNA SNPs and the susceptibility to HCC, aiming to explore some roles and mechanisms of SNPs within miRNAs in the occurrence and development of HCC.

Background Emergence of 2019-nCoV attracted global attention and WHO declared COVID-19 a public health emergency of international concern. Therefore we aimed to explore the severity and atypical manifestations of COVID-19 among children. Methods This is an observational cohort study conducted on 398 children with confirmed COVID-19 by using real-time reverse transcriptase polymerase chain reaction assay for detection of 2019-nCoV nucleic acid during the period from March to November 2020. Patients were subdivided regarding the severity of COVID-19 presentation into Group I (Non-severe COVID-19) was admitted into wards and Group II (Severe COVID-19) admitted into the PICU. Results Non- severe cases were 295cases (74.1%) and 103cases (25.9%) of severe cases. There was a significant difference between age groups of the affected children (P P Conclusion Most children had a non-severe type of COVID-19 and children with severe type had higher levels of D-dimer, hypoxia, shock and mechanical ventilation.

Background: Spontaneous bacterial peritonitis (SBP) is associated with the highest mortality among end-stage cirrhotic liver disease patients. Neutrophil CD11b expression increases on the neutrophil surface within 5 min of exposure to bacteria. Paracentesis remains the only accepted method for accurate evaluation of patients, with many drawbacks, hence, a diagnostic noninvasive marker with a very high sensitivity and high diagnostic accuracy is very necessary. Aim of the study: to evaluate the neutrophil CD11b as a non-invasive biomarker for the diagnosis of SBP, comparing its sensitivity and specificity to other traditional methods. Patients and Methods: 200 patients who had liver cirrhosis with ascites were recruited to the Hepatology department inpatient wards of the National Liver Institute, Menoufia University. They were divided into Group I: 100 patients with SBP and Group II: 100 patients with non SBP ascites. All studied patients were subjected to full clinical examination, abdominal ultrasound, paracentesis, and laboratory investigations including ascetic fluid (AF) examinations. The CD11b expression and its mean fluorescence intensity (MFI) were assessed on peripheral blood neutrophils by flowcytometry. Results: There was a significant increase in the MFI of CD11b in the SBP group compared to the non SBP group. At cut off &gt, 20 for MFI of CD11b with a sensitivity of 100% and specificity of 100% can discriminate between SBP and non SBP cases followed by ascetic fluid TLC examination at a cut off 0.26 (&times, 103) with a sensitivity of 92%, and specificity of 96%, then, AF neutrophil count at cut off 0.25 (&times, 103) with a sensitivity of 80%, specificity of 100%, and AF culture examination with a sensitivity of 56% and specificity of 100%. Conclusion: The measurement of CD11b MFI on peripheral blood neutrophils is a useful non-invasive marker with high sensitivity and specificity to predict SBP compared with other methods. Further large-scale studies are needed to study the value of CD11b MFI level in the SBP follow-up therapy.

Background: High number of losses may occur among health care workers (HCWs) who are the key contributors in the COVID-19 battle. Objectives: To explore the base-line knowledge and perception of HCWs who will join working in our Isolation Unit, correct the wrong believes and investigate their safety at the end of their quarantine period. Methods: The study was implemented on 206 HCWs [133 nurses, 28 physicians, 8 pharmacists, and 37 technicians and housekeepers] by assessing HCWʼs knowledge and attitude via on-line questionnaire, improve misconceptions via health care program prior to start their work in the unit and inspect the COVID-19 infection by laboratory and radiological investigations. The inclusion criteria were being HCWs in the Isolation Unit of COVID-19, National Liver Institute, Menoufia University, Egypt for in the period from of June 10th to August 10th, 2020. Results: Base-line questionnaire results revealed good knowledge and positive attitude of HCWs towards 16-COVID-19 itemsin spite of ignoring the incubation period of the disease. There was significant positive attitude of the HCWs groups, and the total group attitude showed a significant good knowledge score. HCWs had highly significant good knowledge score and positive attitude towards the importance of training on PPE donning and doffing. Continuous health education on infection control practices and importance of social distancing during the isolation period was emphasized. At the end of the quarantine period, the infection rate was 5.8%. The HCWs with positive SARS-CoV-2 showed negative COVID-Ab results, significant changes in their C-T scan findings and lab findings. Conclusion: HCWs with goodknowledge, perception and who follow strict precautions in an Isolation Unit are the basis of success in winning the Battle against SARS-CoV aiming at keeping HCWs safety with optimum patient care and less harm to the health facilities.

Background Duodenoscope-emerging infection especially drug-resistant bacteria is considered a major concern nowadays. Different approaches were attempted to overcome this problem, like double high-level disinfection procedure. We performed a systematic review and meta-analysis to evaluate risk difference for positive cultures from duodenoscopes between double high-level disinfection (dHLD) and single (standard) high-level disinfection (sHLD). Main body A thorough literature search (in October and November 2019) for studies comparing dHLD and sHLD for duodenoscopes was performed by 3 researchers in the Web of Science, Scopus, PubMed, and Cochran databases. The search terms were “duodenoscope,” “ERCP endoscope,” “disinfection,” “sterilization,” and “reprocessing,” and only randomized clinical trials with the English language were accepted. Four trials were identified studying dHLD, and only 2 clinical trials comparing dHLD with standard sHLD were found reporting 6193 duodenoscope cultures. Overall sHLD cultures were 2972, and dHLD cultures were 3221; overall positive cultures were 140 in sHLD and 161 in dHLD. The results of a meta-analysis using the random-effect model showed no significant risk difference (RD) between the 2 procedures for duodenoscope positive cultures (p = 0.53, RD 0.003, 95% CI “− 0.007–0.013”). Conclusions Double HLD offered no significant difference over single HLD for duodenoscope disinfection. An alternative strategy to overcome duodenoscope-transmitted infection is a big issue to be resolved.

Background: Viral infections, congenitally or perinatally acquired, have been associated with neonatal cholestasis. Investigators have suggested a similar link to biliary atresia (BA).

Background: A good survival rate among patients with beta thalassemia major (beta-TM) has led to the appearance of an unrecognized renal disease. Therefore, we aimed to assess the role of serum cystatin-C as a promising marker for the detection of renal glomerular dysfunction and N-acetyl beta-D-glucosaminidase (NAG) and kidney injury molecule 1 (KIM-1) as potential markers for the detection of renal tubular injury in beta-TM children. Methods: This case-control study was implemented on 100 beta-TM children receiving regular blood transfusions and undergoing iron chelation therapy and 100 healthy children as a control group. Detailed histories of complete physical and clinical examinations were recorded. All subjected children underwent blood and urinary investigations. Results: There was a significant increase in serum cystatin-C (p &lt, 0.001) and a significant decrease in eGFR in patients with beta-TM compared with controls (p = 0.01). There was a significant increase in urinary NAG, KIM-1, UNAG/Cr, and UKIM-1/Cr (p &lt, 0.001) among thalassemic children, with a significant positive correlation between serum cystatin-C, NAG and KIM-1 as regards serum ferritin, creatinine, and urea among thalassemic patients. A negative correlation between serum cystatin-C and urinary markers with eGFR was noted. Conclusion: Serum cystatin-C is a good marker for detection of glomerular dysfunction. NAG and KIM-1 may have a predictive role in the detection of kidney injury in beta-TM children.

Fulminant hepatic failure is a life-threatening disease. Hepatitis A virus (HAV) can cause fulminant hepatic failure and death in about 0.2% of cases. Extensive destruction of infected hepatocytes by immune-mediated lysis is thought to be the cause. We aimed to evaluate the use of steroid therapy in children with fulminant HAV. This study included 33 children with fulminant HAV in two groups. Steroid group: comprised of 18 children who received prednisolone (1 mg/kg/d) or its equivalent dose of methylprednisolone, and the nonsteroid group: comprised another 15 children who did not receive steroid therapy. Age and sex were matched for both groups (P > .05), and they were comparable regarding baseline clinical and laboratory characteristics. Of the steroid group, 15 patients survived and 3 died, while in the nonsteroid group, 4 patients survived and 11 died (P = .001). Of the living patients, 15 of 19 (78.9%) received steroids while only 3 of 14 (21.4%) of the dead patients received steroids (P = .001). Stepwise regression analysis showed that steroid therapy was the only independent variable associated with recovery (P = .001). Steroid therapy in children with fulminant HAV associated significantly with improved outcome and survival. Future studies on a larger population size are strongly recommended.

Aim: The diagnosis of biliary atresia (BA) is challenging as no single preoperative test is 100% accurate, especially for distinguishing it from other causes of neonatal cholestasis (NC). Intercellular adhesion molecule (ICAM) elevation was reported in BA as a part of the immune-mediated inflammatory process. The use of ICAM-1 as a discriminative tool between BA and other causes of NC has never been addressed before. This study was to evaluate the diagnostic potentials of ICAM-1 in BA versus other forms of NC. Methods: For this purpose, serum ICAM-1 (sICAM-1) and ICAM-1 expression, in liver biopsy using immunohistochemistry, were estimated in 30 patients with BA and compared to that in 20 patients with other forms of NC. sICAM-1 levels were compared to that in 20 healthy controls.
