Objective. Although there are many randomized trials about unprotected left main percutaneous coronary intervention (ULMPCI), there are few data from Turkey. Therefore, we aimed to present the first data about ULMPCI, determine SYNTAX and EuroSCORE II scores, clinical and demographic data of patients; monitor mortality, morbidity and need for revascularization during 1-year follow-up. Methods and Results. A retrospective 1-year follow-up study of 53 patients who underwent ULMPCI was conducted. Primary endpoint was occurrence of major adverse cardiovascular and cerebrovascular events (MACCE), based on sudden cardiac death (SCD), myocardial infarction (MI), target vessel revascularization (TVR), target lesion revascularization (TLR), stent thrombosis and stroke. Complete revascularization rate was 100%. At 1- year follow-up of the patients, acute coronary syndrome and related cardiac death occurred in 1 (1.9%) of the patients at 8 months, and 1 (1.9%) patient developed restenosis at 9 months. The rate of life without MACCE was 96.2%. A comparison of MACCE rates between diabetic and nondiabetic patients revealed MACCE occurred in 2 patients with diabetes, but this was not statistically significant (p = 0.111). One of these patients was in the low–middlerisk group according to SYNTAX score and the other was in the high-risk group (p = 0.216). Conclusions. In selected patient groups with left main coronary artery disease (patients with low–middle SYNTAX score and high risk of coronary artery bypass grafting surgery, CABG), PCI is a safe and effective treatment option alternative to CABG with high process success and low MACCE rate.

Objective: Myocardial infarction (MI) with non-obstructive coronary arteries (MINOCA) is a relatively new term that is characterized by clinical evidence of MI with normal or near-normal coronary arteries on coronary angiography (QCA). To date, there have been no population-based studies on the prevalence of MINOCA in Turkey. The aim of this nationwide study was to document the prevalence and demographics of MINOCA in a Turkish population. Methods: MINOCA-TR is national, multi-center, prospective, all-comer study that was conducted in 32 hospitals. All consecutive patients who were ≥18 years old, diagnosed with MI according to the Third Universal Definition of Myocardial Infarction, and had undergone QCA were included in the study. Patients with stable coronary artery disease, unstable angina pectoris, a history of revascularization, and type 4/5 MI were excluded. Results: A total of 1793 patients who were diagnosed with MI and had undergone QCA were screened between March 2018 and October 2018, of whom 1626 (mean age: 61.5+-12.5 years, 70.7% male) were enrolled from 32 centers. The prevalence of MINOCA was 6.7% (n=109) in the overall study population. Compared with non-MINOCA patients, those with MINOCA were younger, had a higher prevalence of the female gender, and had a history of flu. Background and objective: We aimed to investigate biochemical factors affecting coronary collateral circulation development in an elderly population aged 75 years and over. Material and methods: The study group consisted of patients with a prior coronary angiography for stable coronary artery disease (CAD). Patients with total occlusion of at least one vessel were included in the study. Enrolled patients were divided into two groups, good collateral (GC; n = 73) and bad collateral (BC; n = 55), in accordance with the Cohen-Rentop’s classification system. Results: In comparison to the GC group, bilirubin levels were significantly lower (p < 0.001), and fasting plasma glucose (FPG) levels were significantly higher in the BC group (p = 0.026). Low-density lipoprotein cholesterol (LDL-C) and high-density lipoprotein cholesterol (HDL-C) levels were significantly lower in the BC group when compared to the GC group (p = 0.002 and p < 0.001, respectively). Backward elimination stepwise logistic regression analysis identified bilirubin and FPG as variables that strongly predicted the presence of a well-developed coronary collateral circulation and a poorly developed coronary collateral circulation, respectively. Conclusion: Bilirubin and FPG were seemed as the most important factors affecting coronary collateral circulation development in patients with stable CAD who were older than 75 years.

Acute pericarditis (AP) following blunt thoracic trauma is rare and difficult to diagnosis. A 43-year-old man with offered to the emergency department (ED) after falling from height before a week ago. The ECG performed in the ED was abnormal, ECG revealed PR segment depression in leads DII, DIII, aVF, and V3-6 and a preliminary diagnosis of acute inferolateral STEMI was presumed. Patients have evidence of systemic inflammation, including leukocytosis, elevated erythrocyte sedimentation rate. We are reporting a case of post-traumatic acute pericarditis presenting with PR-segment depression and normal cardiac enzymes mimicking acute STEMI. Keywords: ECG, Pericarditis, Trauma
