We use sequence analysis on data from the Korean Labor & Income Panel Study (1998–2019) to investigate trajectories of women’s labour market participation in the eight years after first childbirth. We pay special attention to the type of employment through which mothers participate in the labour market, distinguishing between regular full-time employment, non-regular employment, self-employment, and non-employment. After creating employment sequences, we use cluster analysis to reveal patterns of employment trajectories and average marginal effects derived from multinomial logistic regression to identify women’s characteristics on the distinct trajectories. We find that women of younger cohorts are less likely to solely focus on family and childcare in the years after childbirth. However, their chances of steady work in regular jobs did not increase. Instead, they are more likely to be on unsteady pathways, combining childcare with regular or non-regular jobs. Our results suggest that increases in females’ employment might be partly attributed to mothers’ higher probability to obtain precarious non-regular work.

Einleitung: Das Mammakarzinom ist weltweit die häufigste Krebserkrankung bei Frauen. Mammakarzinome werden unter anderem anhand der Expression unterschiedlicher Rezeptoren, insbesondere Hormonrezeptoren und HER2, in verschiedene Subgruppen eingeteilt, die unterschiedliche therapeutische Optionen beinhalten. Zur Behandlung des HER2-überexprimierenden Mammakarzinoms wurden verschiedene zielgerichtete Therapien entwickelt. Die erste von ihnen war der monoklonale Antikörper Trastuzumab. Dieser wurde sowohl für die (neo)adjuvante als auch palliative Therapie zugelassen. Die vorliegende Arbeit verglich mehrere HER2-gerichtete Behandlungsschemata in der ersten palliativen Therapielinie des HER2-überexprimierenden metastasierten Mammakarzinoms hinsichtlich des progressionsfreien Überlebens (PFS) und des Gesamtüberlebens (OS). Des Weiteren wurden die Auswirkungen einer adjuvanten Trastuzumab-Therapie auf das PFS und OS in der metastasierten Situation untersucht. Methodik: Die Daten aller Patientinnen mit HER2-überexprimierendem MBC, die in den Jahren 2000 bis 2012 in der Mammakarzinom-Ambulanz der Medizinischen Klinik m.S. Onkologie und Hämatologie am Campus Charité Mitte therapiert wurden, wurden retrospektiv ausgewertet. Patienten- und Tumorcharakteristika sowie applizierte Therapien wurden ausgewertet; als Effektivitätsparameter stellten PFS und OS die wichtigsten Endpunkte dar. Ergebnisse: Es wurden 75 Patientinnen identifiziert, die eine ausreichende Datengrundlage boten. Patientinnen, die als Erstlinientherapie eine antihormonelle Therapie mit oder ohne Anti-HER2-Therapie erhielten, wiesen das längste mediane OS auf. Eine Therapie mit Taxanen plus Anti-HER2-Therapie war einer Kombination anderer Chemotherapeutika mit Anti-HER2-Therapie in Bezug auf das mediane OS überlegen (43 vs. 24 Monate), ohne dass ein Unterschied im PFS im Kontext der ersten Therapielinie festgestellt werden konnte. 20% der Patientinnen hatten bereits adjuvant eine Trastuzumab-Therapie erhalten. Das mediane OS dieser Gruppe war kürzer als das der Patientinnen, die kein adjuvantes Trastuzumab erhalten hatten (27 vs. 43 Monate). Ein Einfluss der adjuvanten Trastuzumab-Therapie auf das PFS im Rahmen der Erstlinientherapie in der metastasierten Situation zeigte sich nicht (8 vs. 7 Monate). Patientinnen mit einem Trastuzumab-freien-Intervall (TFI) von mindestens sechs Monaten hatten ein längeres mittleres OS als Patientinnen mit einem kürzeren TFI (30 vs. 21 Monate). Schlussfolgerung: Adjuvante Vorbehandlung mit Trastuzumab ging im Falle einer späteren Metastasierung, möglicherweise durch frühzeitige Induktion von Resistenzen, mit einem kürzeren OS einher. Hierbei stellte ein TFI von mindestens 6 Monaten einen Überlebensvorteil dar. Taxane in Kombination mit Anti-HER2-Therapie bestätigten sich als effektive erste palliative Therapielinie bei HER2-überexprimierendem MBC und gingen mit einem längeren OS einher als andere Chemotherapeutika in Kombination mit Anti-HER2-Therapie., Background: Breast cancer is the most common cancer in women worldwide. The term combines heterogeneous tumors that distinguish by certain receptors. One of these subtypes, HER2-positive breast cancer, can be treated with the monoclonal antibody Trastuzumab, which is approved for the treatment of adjuvant as well as metastatic breast cancer (MBC). This study investigates the impact of different palliative anti-HER2-therapies for MBC in terms of progression free survival (PFS) and overall survival (OS). It also investigates the impact of adjuvant Trastuzumab therapy on PFS and OS in MBC. Methods: The data of all patients with HER2-positive MBC treated between 2000 and 2012 at the outpatient department of the Campus Charité Mitte were retrospectively analyzed with a focus on the tumor characteristics, treatments, PFS and OS. Results: A total of 75 patients with a sufficient data set were identified. Patients receiving first line palliative therapy with antihormonal therapy with or without anti- HER2-therapy showed the longest median OS. The shortest median PFS was seen with chemotherapy alone. Therapy with Taxanes plus anti-HER2-therapy was superior to other chemotherapiy combined with anti-HER2-therapy in terms of median OS (43 vs. 24 months) with no difference in median PFS. 15 patients (20%) had received adjuvant Trastuzumab (TA), leading to shorter median OS compared to those patients who had not been pretreated with Trastuzumab (27 vs. 43 months). No effects of adjuvant Trastuzumab on the PFS after firstline treatment could be observed (8 vs. 7 months). Patients with a Trastuzumab free intervall (TFI) of at least six months had longer OS compared to patients with shorter TFI (30 vs. 21 months). Conclusion: Adjuvant Trastuzumab is associated with shorter OS in case of later metastatic disease, possibly due to early induction of resistance. Along with other studies our findings suggest that a TFI is beneficial in terms of survival. The combination of taxanes with Trastuzumab is an effective first-line palliative treatment in HER2-positive MBC and shows longer OS than any other chemotherapy regimen combined with Trastuzumab.
