4. Clinical and imaging manifestations of Erdheim-Chester disease （six cases）

Abstract

Objective: To investigate the clinical and imaging characteristics of an inflammatory myeloid neoplasm-- Erdheim-Chester disease (ECD). Methods: The clinical and imaging data from 6 patients with pathology -confirmed ECD visited Ruijin Hospital, Shanghai Jiao Tong University during March 2019 to February 2022 were retrospectively analyzed. Results: The age of 6 ECD patients ranged from 11 to 64 years, of which 4 were young, aged 11 to 33 years. The ratio of male to female was 2∶1. The main symptoms were central diabetes insipidus（2 cases）, ataxia（2 cases）, lower limb pain （3 cases）and periorbital yellow plaques （3）. The skeletal system was involved in 5 of 6 cases. All 6 cases involved extra-skeletal systems, including skin and subcutaneous soft tissue (n=5), central nervous system (n=2), cardiovascular system (n=2), chest (n=2, including 1 with lung adenocarcinoma), retroperitoneal mass (n=1) and renal fibrosis (n=1).Among 6 cases of ECD, 2 cases were diagnosed as ECD on the imaging, and 1 case as xanthogranuloma in the eye, and 3 other cases that could not tell on the imaging. For clinical diagnosis,2 patients were clinically misdiagnosed as having malignant tumors, but the disease type was not clearly identified. A case with false negative result in both lower limbs on initial X-ray imaging, and CT examination showed bone sclerosis. In patients with ECD, the characteristic X-ray and CT findings of the skeletal system were symmetric bone sclerosis of the long bones of both upper and lower limbs, while non-characteristic X-ray and CT findings were osteolytic bone destruction. Multiple extra-skeletal lesions showed abnormalities in density, signal and radioactivity concentration. Conclusions: Erdheim-Chester disease usually presents with central diabetes insipidus and ataxia, accompanied by cutaneous xanthogranuloma and other skin lesions. The main imaging manifestation of ECD is osteosclerosis of both lower extremities, with multiple system involvement.

Abstract

Objective: To analyze the diagnosis, treatment and prognosis of breast cancer patients in Ruijin Hospital for further clinical management and study. Methods Non-metastatic breast cancer patients with surgical treatment in our hospital were included from January 2009 to December 2018. Clinicopathological features of enrolled patients and chan-ging trend of different surgery including breast conserving surgery and sentinel lymph node biopsy (SLNB) in the period of this study were analyzed. Kaplan-Meier curve analysis was used to compare 5-year and 10-year disease free survival (DFS) and overall survival(OS) of patients with different treatments. Results This study had 8 210 patients. There were 959 (11.7%) patients with in situ disease and 5 379(71.3%) patients with negative lymph node in 7 549 patients without neoadjuvant therapy. There were 949(12.6%) cases and 2964 (39.3%) cases with breast cancer at stage 0 and stage I, respectively. A total of 2377(29.0%) patients underwent breast conserving surgery with increase from 16.5% in 2009 to 34.8% in 2013. The patients with SLNB were 4149 cases (52.5%) from 0.6% in 2009 to 69.3% in 2018. All patients from 2009 to 2018 achieved complete follow-up with a median follow-up of 46 months: 5-year and 10-year DFS were 87.6% and 80.5%; 5-year and 10-year OS were 94.2% and 90.7%, respectively. There were 3800 patients with surgery followed from year 2009 to 2014 with a median-follow-up of 79 months: 5-year DFS rate and OS rate were 86.9% and 93.1%; 10-year DFS rate and OS rate were 79.1% and 88.4%, respectively. Conclusions Both rates of breast conserving surgery and SLNB in Ruijin Hospital increased in last 10 years. Patients had more than 93% of 5-year OS with modern treatment modality of breast cancer, which could indicate clinical management and research in future.

Abstract

Pulmonary arterial hypertension (PAH) is commonly associated with chronic hypoxemia in disorders such as chronic obstructive pulmonary disease (COPD). Prostacyclin analogs are widely used in the management of PAH patients; however, clinical efficacy and long-term tolerability of some prostacyclin analogs may be compromised by concomitant activation of the E-prostanoid 3 (EP3) receptor. Here, we found that EP3 expression is upregulated in pulmonary arterial smooth muscle cells (PASMCs) and human distal pulmonary arteries (PAs) in response to hypoxia. Either pharmacological inhibition of EP3 or Ep3 deletion attenuated both hypoxia and monocrotaline-induced pulmonary hypertension and restrained extracellular matrix accumulation in PAs in rodent models. In a murine PAH model, Ep3 deletion in SMCs, but not endothelial cells, retarded PA medial thickness. Knockdown of EP3α and EP3β, but not EP3γ, isoforms diminished hypoxia-induced TGF-β1 activation. Expression of either EP3α or EP3β in EP3-deficient PASMCs restored TGF-β1 activation in response to hypoxia. EP3α/β activation in PASMCs increased RhoA-dependent membrane type 1 extracellular matrix metalloproteinase (MMP) translocation to the cell surface, subsequently activating pro-MMP-2 and promoting TGF-β1 signaling. Activation or disruption of EP3 did not influence PASMC proliferation. Abstract

Distribution of SARS-CoV-2 virus and pathological features of multiple organs in COVID-19 patients remains unclear, which interferes with the improvement of COVID-19 diagnosis and treatment. In this article, we summarize the pathological findings obtained from systematic autopsy (37 cases) and percutaneous multiple organ biopsy (“minimally invasive autopsy”, 54 cases). These findings should shed light on better understanding of the progression of COVID-19 infection and the means of more effective intervention.
