19. Diagnosis and management of suspected cases of bioterrorism: a pediatric perspective. (Special Article)

Abstract

Since October 3, 2001, the Centers for Disease Control and Prevention and other organizations have been investigating potential bioterrorist-related anthrax cases. The pediatrician may be faced with complex issues related to diagnosis and treatment of illnesses caused by intentionally released biological agents. The agents that pose a major potential bioterrorist threat are reviewed by the clinical syndromes they produce: acute respiratory distress with fever, influenza-like illnesses, acute rash with fever, neurologic syndromes, and blistering syndromes. Specific and detailed diagnostic, treatment, and prophylaxis information is provided for anthrax, plague, tularemia, smallpox, botulism, viral hemorrhagic fevers, and other diseases. In cases of suspected bioterrorism, the pediatrician must be able to obtain diagnostic and treatment information efficiently and expeditiously. The system controlling the interaction between public and nonpublic health laboratories in suspected cases of bioterrorism is described. Finally, information regarding emergency contacts and links to educational resources is provided. Abstract

Pediatric residents should be trained in intensive care procedures as well as general pediatric practices. Researchers found that, among 597 questionnaire respondents, pediatricians serving smaller communities tended to perform more of 29 selected procedures and were more likely to provide newborn and childhood intensive care than those serving larger communities. More than 65% of all respondents reported providing intermediate intensive care. Most individual pediatricians do not, however, perform these selected procedures any more often than once every 10 days., Objective. This study sought to examine variations in the frequency of procedures performed and patterns of care of sick infants and older children by general pediatricians in different sized communities. The results of the study will be considered in developing relevant educational experiences for postgraduate trainees. Methodology. Questionnaires were sent to 1412 Texas pediatricians requesting frequency-information for 29 procedures and whether they provided various levels of care to sick infants and older children. Responses were tabulated by the size of the community in which each pediatrician practiced. Results. Fifty-four percent of the questionnaires were returned. The proportion of pediatricians performing each procedure was significantly different for all but 8 of the 29 procedures between communities of less than 100 000 and more than 100 000 population. For all procedures with significant differences, the proportion of physicians performing the procedures was significantly greater for pediatricians practicing in communities of less than 100 000 population. No significant difference was found between the proportion of pediatricians providing newborn level II and III care; however, more than 65% of both groups provided level II care. Physicians in communities of less than 100 000 population were more likely to provide intermediate and intensive care beyond the newborn period. Conclusion. The general practice rotation in the community setting will not provide adequate training experiences for many of the procedures currently being performed by general pediatricians. 27. Seizures and other neurologic sequelae of bacterial meningitis in children

Abstract

There has been a dramatic decrease in mortality due to bacterial meningitis in recent decades. Unfortunately, permanent neurological abnormalities, including mental retardation, are common among children who survive this infection. However, little is known of the factors that contribute to the likelihood of permanent damage, and it is unclear what features help establish the prognosis for an individual patient. For this reason, a prospective study was organized for the long-term follow-up and evaluation of 185 infants and children with bacterial meningitis. The average follow-up was 8.9 years. Hemophilus influenzae meningitis was responsible for the majority of the cases, affecting 64 percent of the subjects. Meningitis due to Streptococcus pneumoniae caused 16 percent of the cases, and Neisseria meningitidis caused 11 percent. Other organisms were involved in 8 percent, while in 5 percent the causative organism could not be isolated or identified. A total of 58 patients had seizures during the acute phase of their illness, and 69 patients (37 percent) had some neurologic abnormality on examination one month after meningitis. However, it was found that abnormalities persisting one month after infection had a good chance of resolving, and only 26 children (14 percent) had persistent neurologic abnormalities. Hearing loss was the most common; in 18 patients it was the only deficit. Eight patients had severe neurologic deficits such as mental retardation, paralysis, or blindness. Thirteen children had one or more late seizures. The occurrence of late seizures was statistically correlated with the occurrence of seizures during the acute infection, but more striking was the association of the late seizures with other neurological deficits. Three patients had only one late seizure. Of the remaining 10 children, 7 had serious neurological deficits other than sensorineural hearing loss. The results indicate that children with a normal neurological examination after recovering from bacterial meningitis have an excellent chance of escaping serious complications, including seizures. 28. Subdural effusion and its relationship with neurologic sequelae of bacterial meningitis in infancy: a prospective study

Abstract

Effusion, or escape of fluid, into the subdural area of the brain commonly occurs in bacterial meningitis. How the effusion occurs is not understood. The clinical symptoms and laboratory findings that might predict the development of effusion and the complications that may occur were studied in 113 infants between 1 and 18 months, who had bacterial meningitis caused by Haemophilus influenzae, Streptococcus pneumoniae, or Neisseria meningitidis. Transillumination of the skull with a high intensity white light was used to screen the infants for subdural effusion. Effusion developed in 39 percent (44 out of 113) of the patients. Symptoms that were associated with the possible development of effusion included: young age (less than five months old), rapid onset of disease (within one day), low numbers of white blood cells, and high levels of protein and bacterial antigens in the cerebrospinal fluid. Neurologic abnormalities, such as seizures, hearing loss, neurologic deficits or developmental delays, were more likely to occur in patients with effusion at the start of and during antibiotic treatment for the meningitis. However, there were no differences in occurrence of long-term neurologic abnormalities between patients with subdural effusion and patients who did not have effusion as seen in a five-and-a-half-year follow-up after the meningitis occurred. Therefore, invasive therapy is not needed for infants with meningitis who have subdural effusion if they are improving with antibiotic treatment.
