Abstract

Abstract Background We set out to assess the appropriateness of current placement of mentally disordered offenders allocated by the courts in Italy to REMS or to forensic community residences. We hypothesised that as in other countries, the match between a standardised assessment and the decision of the court would be imperfect. Methods The DUNDRUM Toolkit was translated into Italian. The translation had good psychometric properties. In order to compare the current level of therapeutic security with a calculated safest current placement, we compared the DUNDRUM-1 triage security assessment of need for therapeutic security prior to treatment, with evidence for progress made in treatment (DUNDRUM-3) and forensic recovery (DUNDRUM-4). The more conservative of these two would be taken as the safe current level of need for therapeutic security. Results The Italian translation of the DUNDRUM Toolkit had good internal consistency and mean scores had a Reliable Change Index less than one unit. 3.7% of those in REMS (medium security) were assessed as needing high security and 38% were ready to move to a less secure place. In low secure places, 56% were assessed as needing a higher level of therapeutic security and 6% could have moved to open non-secure places. Conclusions The Italian translation of the DUNDRUM Toolkit allows an assessment of the current working of the model of care for forensic psychiatry following the reforms of 2015. Most patients are safely placed. A small but important proportion needed high secure places that are not currently available. (3.7% of 604 nationally, 95% Confidence Interval 1.2% to 8.4%, 7 to 50). Abstract

Gaia Data Release 2 (Gaia DR2) contains results for 1693 million sources in the magnitude range 3 to 21 based on observations collected by the European Space Agency Gaia satellite during the first 22 months of its operational phase. We describe the input data, models, and processing used for the astrometric content of Gaia DR2, and the validation of these results performed within the astrometry task. Some 320 billion centroid positions from the pre-processed astrometric CCD observations were used to estimate the five astrometric parameters (positions, parallaxes, and proper motions) for 1332 million sources, and approximate positions at the reference epoch J2015.5 for an additional 361 million mostly faint sources. Special validation solutions were used to characterise the random and systematic errors in parallax and proper motion. For the sources with five-parameter astrometric solutions, the median uncertainty in parallax and position at the reference epoch J2015.5 is about 0.04 mas for bright (G<14 mag) sources, 0.1 mas at G=17 mag, and 0.7 mas at G=20 mag. In the proper motion components the corresponding uncertainties are 0.05, 0.2, and 1.2 mas/yr, respectively. The optical reference frame defined by Gaia DR2 is aligned with ICRS and is non-rotating with respect to the quasars to within 0.15 mas/yr. From the quasars and validation solutions we estimate that systematics in the parallaxes depending on position, magnitude, and colour are generally below 0.1 mas, but the parallaxes are on the whole too small by about 0.03 mas. Significant spatial correlations of up to 0.04 mas in parallax and 0.07 mas/yr in proper motion are seen on small (<1 deg) and intermediate (20 deg) angular scales. Abstract

Parallaxes for 331 classical Cepheids, 31 Type II Cepheids and 364 RR Lyrae stars in common between Gaia and the Hipparcos and Tycho-2 catalogues are published in Gaia Data Release 1 (DR1) as part of the Tycho-Gaia Astrometric Solution (TGAS). In order to test these first parallax measurements of the primary standard candles of the cosmological distance ladder, that involve astrometry collected by Gaia during the initial 14 months of science operation, we compared them with literature estimates and derived new period-luminosity ($PL$), period-Wesenheit ($PW$) relations for classical and Type II Cepheids and infrared $PL$, $PL$-metallicity ($PLZ$) and optical luminosity-metallicity ($M_V$-[Fe/H]) relations for the RR Lyrae stars, with zero points based on TGAS. The new relations were computed using multi-band ($V,I,J,K_{\mathrm{s}},W_{1}$) photometry and spectroscopic metal abundances available in the literature, and applying three alternative approaches: (i) by linear least squares fitting the absolute magnitudes inferred from direct transformation of the TGAS parallaxes, (ii) by adopting astrometric-based luminosities, and (iii) using a Bayesian fitting approach. TGAS parallaxes bring a significant added value to the previous Hipparcos estimates. The relations presented in this paper represent first Gaia-calibrated relations and form a "work-in-progress" milestone report in the wait for Gaia-only parallaxes of which a first solution will become available with Gaia's Data Release 2 (DR2) in 2018., Comment: 29 pages, 25 figures. Abstract

Context. The first Gaia Data Release contains the Tycho-Gaia Astrometric Solution (TGAS). This is a subset of about 2 million stars for which, besides the position and photometry, the proper motion and parallax are calculated using Hipparcos and Tycho-2 positions in 1991.25 as prior information. Aims. We investigate the scientific potential and limitations of the TGAS component by means of the astrometric data for open clusters. Methods. Mean cluster parallax and proper motion values are derived taking into account the error correlations within the astrometric solutions for individual stars, an estimate of the internal velocity dispersion in the cluster, and, where relevant, the effects of the depth of the cluster along the line of sight. Internal consistency of the TGAS data is assessed. Results. Values given for standard uncertainties are still inaccurate and may lead to unrealistic unit-weight standard deviations of least squares solutions for cluster parameters. Reconstructed mean cluster parallax and proper motion values are generally in very good agreement with earlier Hipparcos-based determination, although the Gaia mean parallax for the Pleiades is a significant exception. We have no current explanation for that discrepancy. Most clusters are observed to extend to nearly 15 pc from the cluster centre, and it will be up to future Gaia releases to establish whether those potential cluster-member stars are still dynamically bound to the clusters. Conclusions. The Gaia DR1 provides the means to examine open clusters far beyond their more easily visible cores, and can provide membership assessments based on proper motions and parallaxes. Abstract

Gaia Data Release 1 (Gaia DR1) contains astrometric results for more than 1 billion stars brighter than magnitude 20.7 based on observations collected by the Gaia satellite during the first 14 months of its operational phase. We give a brief overview of the astrometric content of the data release and of the model assumptions, data processing, and validation of the results. For stars in common with the Hipparcos and Tycho-2 catalogues, complete astrometric single-star solutions are obtained by incorporating positional information from the earlier catalogues. For other stars only their positions are obtained by neglecting their proper motions and parallaxes. The results are validated by an analysis of the residuals, through special validation runs, and by comparison with external data. Results. For about two million of the brighter stars (down to magnitude ~11.5) we obtain positions, parallaxes, and proper motions to Hipparcos-type precision or better. For these stars, systematic errors depending e.g. on position and colour are at a level of 0.3 milliarcsecond (mas). For the remaining stars we obtain positions at epoch J2015.0 accurate to ~10 mas. Positions and proper motions are given in a reference frame that is aligned with the International Celestial Reference Frame (ICRF) to better than 0.1 mas at epoch J2015.0, and non-rotating with respect to ICRF to within 0.03 mas/yr. The Hipparcos reference frame is found to rotate with respect to the Gaia DR1 frame at a rate of 0.24 mas/yr. Based on less than a quarter of the nominal mission length and on very provisional and incomplete calibrations, the quality and completeness of the astrometric data in Gaia DR1 are far from what is expected for the final mission products. The results nevertheless represent a huge improvement in the available fundamental stellar data and practical definition of the optical reference frame., Comment: Accepted for publication in Astronomy & Astrophysics

Abstract

We develop here a new approach for the relativistic modeling of the photons moving into a quasi-Minkowskian space-time, where the metric is generated by an arbitrary n-body distribution within an isolated Solar System. Our model is built on the prescriptions of the theory of General Relativity and leaves the choices of the metric, as well as that of the motion of the observer, arbitrary. Adopting a quasi-Minkowskian expression of the metric accurate to order (v/c)^2, a thorough numerical test campaign is conducted to verify correctness and reliability of the model equations. The test results show that the model behaves according to predictions. Specifically, comparisons to true (simulated) data demonstrate that stellar distances are reconstructed up to the specified level of accuracy. Although the (v/c)^2 approximation is not always sufficient for its application to future astrometric experiments, which require modeling to (v/c)^3, this work serves also as a natural test-ground for the higher order model, whose formulation is now close to completion, and will be presented in a forthcoming paper., Comment: 60 pages, 15 figures. We propose a simple and unambiguous way to deduce the parameters of black holes which may reside in AGNs and some types of X-ray binaries. The black-hole mass and angular momentum are determined in physical units. The method is applicable to the sources with periodic components of variability, provided one can assume the following: (i) Variability is due to a star or a stellar-mass compact object orbiting the central black hole and passing periodically through an equatorial accretion disk (variability time-scale is given by the orbital period). (ii) The star orbits almost freely, deviation of its trajectory due to passages through the disk being very weak (secular); the effect of the star on the disk, on the other hand, is strong enough to yield observable photometric and spectroscopic features. It has truly become a field that is intensely linked to molecular genetic alterations, especially DNA-repair defects. Germline breast cancer 1 gene (BRCA1) and breast cancer 2 gene (BRCA2) mutations are implicated in the highest risk of prostate cancer (PC) predisposition and aggressiveness. Poly adenosine diphosphate ribose polymerase (PARP) proteins play a key role in DNA repair mechanisms and represent a valid target for new therapies. Olaparib is an oral PARP inhibitor that blocks DNA repair pathway and coupled with BRCA mutated-disease results in tumor cell death. In phase II clinical trials, including patients with advanced castration-resistant PC, olaparib seems to be efficacious and well tolerated. Waiting for randomized phase III trials, olaparib should be considered as a promising treatment option for PC. Abstract

Objective To assess the accuracy of pathological diagnosis by transvaginal ultrasound-guided biopsy versus surgery in patients with suspicious primary advanced tubo-ovarian carcinoma. The Feasibility, adequacy, and safety of the procedure were also evaluated. Methods Consecutive women with pre-operative suspicious primary advanced tubo-ovarian carcinoma presenting between July 2019 and September 2021 were enrolled. Accuracy was calculated including only cases who underwent surgery. Feasibility was defined as the number of cases in which ultrasound-guided biopsy was possible according to tumor characteristics (morphology and site). Adequacy was defined as the number of conclusive diagnoses out of the samples collected. Safety was defined by the number of major complications which were defined as hospitalization, surgery, and/or blood transfusion. Results A total of 278 patients were eligible for the study; 158 were enrolled, while 120 were excluded for logistic reasons or patient refusal. Ultrasound-guided biopsy was not feasible in 30 (19%) patients. The samples obtained in the remaining 128 patients were all adequate (100%), and no major complications were noted. A total of 26 (20%) patients started neoadjuvant chemotherapy on the basis of the diagnosis obtained by ultrasound, whereas 102 (80%) patients underwent surgery. Accuracy of ultrasound-guided biopsy versus surgery was 94% (96/102), with six false negative cases at ultrasound (6%). Site (prevesical peritoneum) and size (<8 mm) of the nodules resulted as major predictive factors for ultrasound-guided biopsy failure (false negative). Ultrasound-guided biopsy correctly identified 86 primary invasive tubo-ovarian carcinomas and 10 metastatic tumors. Conclusion Ultrasound-guided biopsy is a feasible, safe, and accurate method to provide histological diagnosis in suspicious advanced tubo-ovarian cancer patients.
