The internet of medical thing system (IoMTS) comprises the fifth-generation (5G) networking technology that collects and shares digital data from signal- or image-capturing devices through computer and wireless communication networks. This framework enables healthcare professionals to gain immediate visibility into a patient’s condition and facilitates communication with patients and family members. Recently, artificial intelligence (AI)- based methods are being increasingly applied to preprocess digital data and extract features. The key physiological parameters and feature patterns can then be incorporated into AI- based tools to help monitor, detect, and diagnose applications. However, these digital data contain patients’ privacy and may be restricted to authorized users. In a public channel, IoMTS must ensure information security for protection against hacker attacks. Hence, in this study, a symmetric encryption and decryption protocol was designed to ensure infosecurity of biosignals and medical images and assist in specific purposes in disease diagnosis. For a symmetric cryptography scheme, this study proposed a key generator combining a chaotic map and Bell inequality and generating unordered numbers and unrepeated 256 secret keys in the key space. Then, a machine learning - based model was employed to train the encryptor and decryptor for both biosignals and image infosecurity. After secure - data transmission, a case study is conducted for classifying medical images. Here, a classifier based on a convolutional neural network (CNN) is used for AI- assisted breast tumor diagnosis. In addition, for biosignal infosecurity, raw data were collected from a radar millimeter-wave (mm-Wave) sensing firmware for detecting vital signs. The experimental results are validated for heartbeat signals, respiratory signals, and mammography images, demonstrating the effectiveness and feasibility of the proposed encryption, decryption, and AI-assisted diagnosis methods.

Abstract Chest x‐ray (CXR) examination is a common first‐line, non‐invasive, and rapid screening method in clinical examinations. The posteroanterior (PA) and anteroposterior (AP) view modes can both be used to detect related cardiopulmonary diseases, such as pneumonitis, tuberculosis, pulmonary fibrosis, lung tumors, and cardiomegaly. Compared with cardiac computed tomography and cardiac magnetic resonance imaging methods, CXR examination has a short scanning duration and costs less, and is suitable for routine and follow‐up health examinations. Cardiomegaly is an asymptomatic disease in the early stage and cannot be detected through electrocardiography measurements. Thus, early cardiomegaly classes detections, such as cardiac hypertrophy and ventricular dilatation, can help make decisions regarding drug treatments and surgeries. In addition, an automatic assistive tool is required to differentiate between normal individuals and those with cardiomegaly to address the problem of manual inspection and labor shortage. Hence, PA view‐based CXR classification is used to develop a deep learning (DL)‐based high‐dimensional multiple regression analysis (MRA) model for CXR image classification in rapid cardiomegaly screening. This multilayer network model uses a two‐channel three‐layer convolution‐normalization‐pooling process with two‐dimensional (2D) multi convolution operations to enhance images and to extract feature patterns; and then a one‐dimensional feature conversion is used to estimate the four coordinate points of the maximal horizontal cardiac diameter (MHCD) and maximal horizontal thoracic diameter (MHTD), which can be used to estimate cardiothoracic ratio and detect cardiomegaly. For experimental tests, the training and testing datasets are collected from the National Institutes of Health CXR Image Database (Clinical Center, USA), and 10‐fold cross‐validation was used for model evaluation in terms of precision (%), recall (%), accuracy (%), and F1 score. These indexes are used to evaluate the feasibility of the proposed MRA estimator. In addition, the performances of the proposed model are compared with those of conventional DL‐based multilayer classifiers.

Abstract Background The Fusarium head blight caused by Fusarium graminearum results in reduced crop yields and the potential for vomitoxin contamination, which poses a risk to both human and livestock health. The primary method of control relies on the application of chemical fungicides. Results The current study found that the tebuconazole sensitivity of 165 F. graminearum isolates collected from the Huang-Huai-Hai region of China between 2019 and 2023 ranged from 0.005 to 2.029 µg/mL, with an average EC50 value of 0.33 ± 0.03 µg/mL. The frequency distribution conformed to a unimodal curve around the mean, and therefore provides a useful reference for monitoring the emergence of tebuconazole resistance in field populations of F. graminearum. No cross-resistance was detected between tebuconazole and other unrelated fungicides such as flutriafol, propiconazole and fluazinam, but there was a clear negative cross-resistance with triazole fungicides including fludioxonil, epoxiconazole, hexaconazole, and metconazole. Abstract Optical parametric oscillation (OPO) in Kerr microresonators can efficiently transfer near-infrared laser light into the visible spectrum. To date, however, chromatic dispersion has mostly limited output wavelengths to >560 nm, and robust access to the whole green light spectrum has not been demonstrated. In fact, wavelengths between 532 nm and 633 nm, commonly referred to as the “green gap”, are especially challenging to produce with conventional laser gain. Hence, there is motivation to extend the Kerr OPO wavelength range and develop reliable device designs. Here, we experimentally show how to robustly access the entire green gap with Kerr OPO in silicon nitride microrings pumped near 780 nm. Our microring geometries are optimized for green-gap emission; in particular, we introduce a dispersion engineering technique, based on partially undercutting the microring, which not only expands wavelength access but also proves robust to variations in resonator dimensions. Using just four devices, we generate >150 wavelengths evenly distributed throughout the green gap, as predicted by our dispersion simulations. Highlights A blade-coated composite paste, composed of a compressible 3D carbon skeleton, PEDOT:PSS, and CNTs, can naturally dry to form a porous electrode on paper with a micro- and nano-structured surface. The all-paper pressure sensor demonstrated an ultrahigh sensitivity of 1014 kPa−1, a wide responsive range up to 300 kPa, and an ultralow operating voltage of 0.01 V. The sensor showcased superior detection capability, ranging from subtle wrist pulses and robust finger taps to large-area spatial force.

Abstract The mechanisms underlying tissue repair in response to damage have been one of main subjects of investigation. Here we leverage the wound-induced hair neogenesis (WIHN) models in adult mice to explore the correlation between degree of damage and the healing process and outcome. The multimodal analysis, in combination with single-cell RNA sequencing help to explore the difference in wounds of gentle and heavy damage degrees, identifying the potential role of toll-like receptor 9 (TLR9) in sensing the injury and regulating the immune reaction by promoting the migration of γδT cells. The TLR9 deficient mice or wounds injected with TLR9 antagonist have greatly impaired healing and lower WIHN levels. Inhibiting the migration of γδT cells or knockout of γδT cells also suppress the wound healing and regeneration, which can’t be rescued by TLR9agonist. Finally, the amphiregulin (AREG) is shown as one of most important effectors secreted by γδT cells and keratinocytes both in silicon or in the laboratory, whose expression influences WIHN levels and the expression of stem cell markers. In total, our findings reveal a previously unrecognized role for TLR9 in sensing skin injury and influencing the tissue repair and regeneration by modulation of the migration of γδT cells, and identify the TLR9-γδT cells-areg axis as new potential targets for enhancing tissue regeneration.

Purpose: Primary surgery failure of macular holes causes poor visual acuity outcomes. Several studies indicate that small-medium idiopathic full-thickness macular holes (iFTMH) have consistent and high anatomical closure rates after vitrectomy and internal limiting membrane (ILM) peeling, regardless of iFTMH diameters. However, there is no systematic analysis examining the relationship between iFTMH diameters and anatomical closure rates. Methods: In this systematic review and meta-regression, we searched PubMed, Embase, and Web of Science databases on October 24th, 2022. We included studies regarding iFTMH, with ILM peeling/inverted flap technique, long-lasting gas tamponade, and face-down position after surgery. Univariable meta-regression with a restricted cubic spline model and component-plus-residual plot after covariables adjustment were used to explore non-linear association. Results: A total of 7257 participants from 19 randomized controlled trials and 49 observational studies were included in this meta-analysis. In ILM peeling group, every 100-μm increment in diameter was associated with a 3.8 % (95 % confidence interval [CI], 1.8%–5.7 %, P 0.9). The restricted cubic spline model and component-plus-residual plot controlling for age, sex, and symptom duration prior to surgery showed no evident non-linearity in both surgical techniques. Conclusions: The iFTMH diameter is linear and inversely associated with the anatomical closure rate after the ILM peeling technique, but not with the inverted flap technique. The present study supports the use of advanced techniques, e.g., inverted flap technique, in small-medium iFTMH to improve anatomical closure rates.

Abstract Background To estimate glucose disposal rate (eGDR) as a newly validated surrogate marker of insulin resistance. Few studies have explored the association between changes in eGDR levels and stroke incidence. This study aims to explore the effect of the level of eGDR control on stroke and events. Methods Data were obtained from the China Longitudinal Study on Health and Retirement (CHARLS). The eGDR control level was classified using K‐means cluster analysis. Logistic regression analysis was used to explore the association between different eGDR control levels and incident stroke. Restrictive cubic spline regression was used to test the potential nonlinear association between cumulative eGDR and stroke incidence. Results Of the 4790 participants, 304 (6.3%) had a stroke within 3 years. The odds ratio (OR) was 2.34 (95% confidence interval [CI], 1.42–3.86) for the poorly controlled class 4 and 2.56 (95% CI, 1.53–4.30) for the worst controlled class 5 compared with class 1 with the best controlled eGDR. The OR for well‐controlled class 2 was 1.28 (95% CI, 0.79–2.05), and the OR for moderately controlled class 3 was 1.95 (95% CI, 1.14–3.32). In restrictive cubic spline regression analysis, eGDR changes are linearly correlated with stroke occurrence. Weighted quartile and regression analysis identified waist circumference and hypertension as key variables of eGDR for predicting incident stroke. Conclusions Poorly controlled eGDR level is associated with an increased risk of stroke in middle‐aged and elderly people. Monitoring changes in eGDR may help identify individuals at high risk of stroke early.

Solanesol, an aliphatic terpene alcohol predominantly found in solanaceous plants, has gained recognition for its anti-inflammatory, antibacterial, and neuroprotective properties. This study investigates the potential efficacy of solanesol in alleviating chronic inflammatory pain induced by injection of complete Freund's adjuvant (CFA) into the left hind paw. Behavioral assessments revealed a significant reduction in mechanical and thermal hypersensitivity following solanesol administration, accompanied by a partial alleviation of concomitant anxiety-like behaviors. Mechanistically, Western blot analysis demonstrated a substantial decrease in the levels of TNF-α and IL-1β after solanesol administration. Immunohistochemical staining further revealed a notable suppression of microglial and astrocytic activation induced by CFA injection. These findings collectively suggest that solanesol holds promise as a latent therapeutic agent for the treatment of chronic inflammatory pain.

Abstract Natural cartilage exhibits superior lubricity as well as an ultra‐long service lifetime, which is related to its surface hydration, load‐bearing, and deformation recovery feature. Until now, it is of great challenge to develop reliable cartilage lubricating materials or coatings with persistent robustness. Inspired by the unique biochemical structure and mechanics of natural cartilage, the study reports a novel cartilage‐hydrogel composed of top composite lubrication layer and bottom mechanical load‐bearing layer, by covalently manufacturing thick polyelectrolyte brush phase through sub‐surface of tough hydrogel matrix with multi‐level crystallization phase. Due to multiple network dissipation mechanisms of matrix, this hydrogel can achieve a high compression modulus of 11.8 MPa, a reversible creep recovery (creep strain: ≈2%), along with excellent anti‐swelling feature in physiological medium (v/v0 < 5%). Using low‐viscosity PBS as lubricant, this hydrogel demonstrates persistent lubricity (average COF: ≈0.027) under a high contact pressure of 2.06 MPa with encountering 100k reciprocating sliding cycles, negligible wear and a deformation recovery of collapse pit in testing area. The extraordinary lubrication performance of this hydrogel is comparable to but beyond the natural animal cartilage, and can be used as compliant coating for implantable articular material of UHMWPE to present, offering more robust lubricity than current commercial system.

BackgroundMicrobial community dynamics have been involved in numerous diseases, including cancer. The diversity of intertumoral microbiota in human papillomavirus independent endocervical adenocarcinoma (HPVI ECA) is not well-characterized.ObjectiveOur objective is to delineate the intratumoral microbiota profile in HPVI ECA and investigate its potential influence on oncogenesis.MethodsWe analyzed 45 HPVI ECA cases, comprising 36 gastric-type ECA (GEA) and 9 clear cell carcinomas (CCC). We compared the microbial composition within cancerous and adjacent noncancerous tissue samples using 5R-16S ribosomal DNA sequencing. Further, we investigated the correlation between specific microbes and clinical-pathological metrics as well as patient outcomes.ResultsOur findings demonstrate notable differences in the microbial spectra between cancerous and adjacent noncancerous tissues. Amongst HPVI ECA subtypes, GEAs exhibit more microbial variations compared to CCCs. Using the Random Forest algorithm, we identified two distinct microbial signatures that could act as predictive biomarkers for HPVI ECA and differentiate between GEA and CCC. Varied microbial abundances was related to clinical characteristics of HPVI ECA patients. In addition, high levels of Micrococcus and low levels of unknown genus75 from the Comamonadaceae family were associated with poorer outcomes in HPVI ECA patients. Similarly, an abundance of Microbacterium correlated with reduced overall survival (OS), and a high presence of Streptococcaceae family microbes was linked to reduced recurrence-free survival (RFS) in GEA patients. Intriguingly, a high abundance of Micrococcus was also associated with a worse OS in GEA patients.ConclusionThe study reveals distinct microbial signatures in HPVI ECA, which have potential as biomarkers for disease prognosis. The correlation between these tumor-associated microbiota features and clinicopathological characteristics underscores the possibility of microbiome-based interventions. Our research provides a foundation for more in-depth studies into the cervical microbiome’s role in HPVI ECA.

Background: The COVID-19 pandemic had a substantial impact on cancer services. The aim of our study was to evaluate the recovery of endoscopic activity and cancer detection after the COVID-19 pandemic. Methods: Endoscopic data from January 2019 to December 2020 were retrospectively collected to assess the endoscopic activity and cancer detection during the COVID-19 peak period (February 2020) and the post-COVID-19 peak period (March to July 2020). Results: The COVID-19 pandemic almost brought endoscopic activity and cancer detection to a standstill. Diagnostic procedure and endoscopic resection showed the greatest reduction. With the decline in COVID-19 infections, endoscopic activity gradually returned to previous level in July. However, the detection rate of gastric cancer resumed in September, whereas colorectal cancer resumed in August. The monthly detection rates of gastric and colorectal cancers decreased from their initial peaks of 2.98 % and 6.45 %, respectively, and finally were even lower than the average in 2019. Similarly, the mean age of patients who received endoscopy also declined as the detection rates resumed. The increasing colonoscopies allowed the missing colorectal cancer patients to be caught up. In contrast, it was expected that 6.69 % of gastric cancer patients were missed and did not receive needed endoscopy. Conclusions: The recovery of cancer detection occurred later than that of endoscopic activity, especially for gastric cancer. Older people were vulnerable to the continuous impact of COVID-19 pandemic than young people for seeking medical services. Urgent efforts are required to recover and maintain cancer services before subsequent waves of the COVID-19 pandemic.

Despite the excellent lubricity of conventional hydrogel materials due to their wet-soft properties, they produce severe mechanical elastic deformation at higher interfacial contact stresses. Balancing the load-bearing capacity and lubricating properties of hydrogel material is the difficulty of the current research work for articular cartilage substitutes. Great progress has been made in developing bionic joint materials with high load-bearing and low-friction hydrogels based on gradient designs. However, most bionic materials are based on sliding friction greatly limiting the improvement of lubrication performance. Herein, we designed and prepared a new hydrogel material with high load-bearing capacity and stable lubrication performance, breaking through the traditional friction method and turning to “sliding” for “rolling”. The network on the hydrogel surface was dissociated by UV irradiation and the pores on the surface were filled with SiO2 nanoparticles. The dense network structure of the underlying layer endows the hydrogel material with good load-bearing properties, while the high degree of hydration of the surface layer and the rolling friction effect of SiO2 nanoparticles greatly enhance the lubrication property. With the synergistic effect of these designs, the multi-layered hydrogel with nanoparticles on the surface achieved an ultra-low average coefficient of friction (COF) of ∼0.00809 at a high load of 50 N during 30,000 cycles. This idea of hydrogel material design provides a new strategy for the replacement of biomimetic articular cartilage materials.
