4. University Social Responsibility: A Paradox or a Vast Field of Tensions?

- Abstract
University social responsibility (USR) is a fashionable concept that is often presented as a paradox, with the implication that it can help universities meet the social dimension of higher education, without questioning the hegemonic meanings of academic excellence and the university mission. We draw on data collected through a focus group of experts on USR to suggest that this concept has the potential to contribute to the transformation of higher education, particularly if its tensions and contradictions are addressed. Three tensions emerged from the data: real versus unreal change, institutional cooperation versus competition, and the right to privacy versus excessive transparency. We conclude that USR is neither a neutral nor a consensual concept; rather, it is eminently political, and HEIs and their leaders, teachers, staff, and students should confront, discuss, and take a stand on its tensions and contradictions.

Background: Socioeconomic conditions are strongly associated with breast and cervical cancer incidence and mortality patterns; therefore, social protection programmes (SPPs) might impact these cancers. This study aimed to evaluate the effect of SPPs on breast and cervical cancer outcomes and their risk/protective factors. Methods: Five databases were searched for articles that assessed participation in PPS and the incidence, survival, mortality (primary outcomes), screening, staging at diagnosis and risk/protective factors (secondary outcomes) for these cancers. Only peer-reviewed quantitative studies of women receiving SPPs compared to eligible women not receiving benefits were included. Independent reviewers selected articles, assessed eligibility, extracted data, and assessed the risk of bias. A harvest plot represents the included studies and shows the direction of effect, sample size and risk of bias. Findings: Of 17,080 documents retrieved, 43 studies were included in the review. No studies evaluated the primary outcomes. They all examined the relationship between SPPs and screening, as well as risk and protective factors. The harvest plot showed that in lower risk of bias studies, participants of SPPs had lower weight and fertility, were older at sexual debut, and breastfed their infants for longer. Interpretation: No studies have yet assessed the effect of SPPs on breast and cervical cancer incidence, survival, or mortality; nevertheless, the existing evidence suggests positive impacts on risk and protective factors.

Bats are important reservoirs and spreaders of pathogens. Giardia duodenalis is a globally important protozoan that infects humans and other mammals with considerable public health burden, particularly on the child development. Based on genetic variation and host specificity, G. duodenalis is categorized into eight genotypes/assemblages A-H. Assemblages A and B are widespread globally and are associated with human and animal disease. There is evidence of Giardia in the bat feces from diverse geographic regions, but the G. duodenalis assemblages are unknown, which is a key point for the One Health view. Here, we successfully amplified the BG/GDH/DIS3/HCMP2/HCMP3 targets of G. duodenalis from five bat species captured in the Brazilian Amazon biome revealing the presence of zoonotic G. duodenalis assemblages A and B in the feces of these flying mammals. Our study reveals that bats may play a role in transmission of zoonotic G. duodenalis, at least in this biome.

Introdução Pacientes com insuficiência cardíaca, doenças do sistema de condução cardíaca ou fibrilação atrial necessitam de dispositivos para auxiliar no controle dos batimentos cardíacos, como marca-passo permanente, ressincronizador cardíaco ou cardioversor desfibrilador implantável. A estimulação ventricular direita (RVP), na forma unicameral ou atrioventricular, tem sido o tratamento convencional, no entanto, estudos demonstram que pode resultar em dessincronia ventricular, redução da função cardíaca, recorrência de fibrilação atrial e aumento da mortalidade. A reversão desses eventos tem sido relatada por meio da Terapia de Ressincronização Cardíaca, tratamento para pacientes com insuficiência cardíaca sintomática, bloqueio de ramo esquerdo, fração de ejeção reduzida e indicação de dispositivos cardíacos, melhorando os sintomas, causando remodelamento e reduzindo a mortalidade. Entretanto, a terapia convencional e ressincronização cardíaca possuem limitações, podendo não alcançar benefícios clínicos em alguns pacientes. O conhecimento dos efeitos deletérios das técnicas tradicionais estimulou pesquisas, visando uma estimulação mais fisiológica. A estimulação Hissiana ou His-Purkinje (HBP) é uma técnica que tem por objetivo produzir uma ativação mais fisiológica, quando comparada com a estimulação RVP. A despolarização dos ventrículos pela estimulação hissinana induz ativação ventricular mais sincronizada, evitando a dessincronia. Este parecer teve como objetivo avaliar a eficácia e segurança da estimulação hissiana, em comparação com o tratamento convencional. Métodos Elaborada pergunta PICO: P-Pacientes com indicação para dispositivos cardíacos eletrônicos implantáveis; I-Estimulação Hissiana; C-Estimulação ventricular direita; O-Duração complexo QRS, fração de ejeção, diâmetro ventricular e classe funcional NYHA. Foram pesquisadas as plataformas de busca MEDLINE via Pubmed, Cochrane Database of Systematic Reviews (COCHRANE) e Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS). As buscas foram realizadas em março de 2023, por três revisores independentes. Resultados Os sete ensaios clínicos incluídos compararam as técnicas Estimulação Hissiana (HPB) versus Estimulação ventricular direita (RVP) em relação ao parâmetro eletrocardiográfico, duração do complexo QRS e três estudos encontraram um resultado significativo melhor no grupo intervenção (HBP). Para os resultados ecocardiográficos, avaliando a fração de ejeção, dois estudos tiveram resultados significativamente melhores no grupo HBP e um estudo o resultado foi semelhante entre os grupos. Em relação ao parâmetro funcional mensurado, dois estudos tiveram resultados significativamente melhores no grupo HBP e um estudo apresentou resultados semelhantes entre os grupos. Dos sete estudos incluídos, em relação ao risco de viés, apenas dois estudos apresentaram baixo e médio risco, quatro apresentaram alto risco em 1, 2 ou 3 itens avaliados. Discussão e conclusões A técnica HBP demostrou ser superior à técnica convencional, no entanto, são necessários ensaios clínicos randomizados de maior qualidade metodológica e Revisões Sistemáticas para verificar a eficácia clínica da técnica, envolvendo maior número de pacientes e tempo de acompanhamento mais longo.

A raiva é uma zoonose que surge na interface da saúde única, pois animais infectados podem transmitir o vírus para outros animais e para humanos, o que causa um grande problema para a saúde pública, por se tratar de uma zoonose com letalidade de quase 100%. Tendo em vista a gravidade e a alta letalidade da raiva e a importância das ações extensionistas de educação em saúde para a prevenção da doença, este trabalho tem como objetivo apresentar as ações de educação em saúde para prevenção da raiva e promoção da saúde única, realizadas pelo projeto: Por que a raiva é uma questão de saúde única? Foi realizado um estudo descritivo do tipo relato de experiência sobre as ações realizadas. As abordagens das ações foram descritas em seis etapas: 1. Raiva: não há prevenção, sem vacinação!; 2.Imersão em Raiva; 3. Conversando sobre a raiva na Festa Literária de Glória (FLIG); 4. Promovendo a saúde única na campanha de vacinação antirrábica de cães e gatos; 5. Saúde única no contexto da vigilância da raiva no estado do Mato Grosso do Sul; 6. Orientando produtores e criadores de animais de produção sobre a raiva. As ações do projeto beneficiaram discentes dos cursos das ciências agrárias do Sertão, Agentes de Combate às Endemias e pessoas da comunidade, fornecendo os conhecimentos básicos sobre a doença necessários para a prevenção. A experiência relatada foi exitosa por proporcionar a sensibilização de um tema de grande relevância e uma doença preocupante para a saúde pública e saúde única. Abstract: Rabies is a zoonosis that appears in the Unified Health System interface, as infected animals can transmit the virus to other animals and humans, which causes a major problem for public health because it is a zoonosis with a lethality of almost 100 %. Bearing in mind the severity and high lethality of rabies and the importance of outreach actions of health education for disease prevention, this work aims to present health education actions for rabies prevention and Unified Health System promotion, carried out for the project: Why is rabies a unique health issue? A descriptive study of the actions was conducted based on an experience report. Action approaches were described in six steps: 1) Rabies: There is no prevention without vaccination!; 2) Immersion in rabies; 3) Talking about rabies at the Literary Party of Glória (Festa Literária de Glória – FLIG, in Portuguese); 4) Promoting Unified Health System with the anti-rabies vaccination campaign for dogs and cats; 5) Unified Health System in the context of rabies surveillance in the state of Mato Grosso do Sul; 6) Guiding producers and breeders of animals about rabies. The project's actions benefited students of agricultural science courses in the Sertão region, Endemic Disease Control Agent, and people from the community, providing basic disease knowledge necessary for prevention. The reported experience successfully raised awareness of a topic of great relevance and a worrying disease for public health and Unified Health System. No período em que se discute o novo Plano Nacional de Educação, e entendendo que investigações sobre a implementação dos planos municipais de educação podem lançar luz nesse processo, apresentamos os resultados de pesquisa documental que investigou como a gestão pública de Castro Alves, Bahia, tem implementado a Meta 1 de seu Plano Municipal de Educação. Em abordagem top-down, investigaram-se (categorização temática) a base legal e os registros sobre educação infantil de 2014 a 2021. Como nos demais entes, o município baiano não atingiu as metas estabelecidas e não há evidências de forte atuação intersetorial ou de participação social, mas houve avanços quanto ao currículo e à estruturação da oferta da educação infantil. Por fim, são listadas oito recomendações para a formulação do próximo plano.

Introdução: Legionella pneumophila, bactéria aeróbia Gram-negativa, considerada importante agente de pneumonia grave em pacientes imunossuprimidos, com alta taxa de letalidade e pode se tornar um problema quando se prolifera em sistemas de água de hospitais ocasionando surtos. Objetivo: Caracterização clínica, epidemiológica e dos fatores de risco relacionados aos casos de IRAS em surto por L. pneumophila em unidade de transplante de medula óssea (TMO). Método: Estudo caso-controle em unidade de TMO de um hospital de ensino da cidade de São Paulo, de jan/2020-dez/2023. Considerados Casos pacientes com diagnóstico confirmado de IRAS por L. pneumophila, critérios CDC, e Controle pacientes internados por no mínimo 48h no mesmo período considerado como de possível relação com a transmissão da Legionella para cada caso (14 dias antes do início dos sintomas), em fase de condicionamento ou até D+30, sem diagnóstico de legionelose. Selecionados 2 controles para cada caso. Significância estatística considerada p < 0,05. Identificação do agente no ambiente por pesquisa molecular com metagenômica, técnica de detecção de amplicon. Resultados: Foram identificados 8 casos de IRAS por L. pneumophila, sendo um caso em 2020 e 2021, quatro em 2022 e dois em 2023, todos através de teste de antígeno urinário. 75% eram do sexo feminino, idade média de 52 anos. Todos foram transplantados por neoplasias hematológicas (62,5% com leucemia mieloide aguda), sendo 75% alogênicos aparentados. O intervalo entre o transplante de células tronco hematopoiéticas e a legionelose foi em média 9 dias, os principais sintomas apresentados: febre (100%), dispneia (87,5%), tosse (62,5%), dor torácica (62,5%) e diarreia (62,5%). Todos apresentaram pneumonia, sendo a imagem de consolidação (100%), derrame pleural (62,5%) e vidro fosco (62,5%) as alterações mais frequentes nas imagens. Todos receberam terapia combinada com levofloxacina e macrolídeo. Um paciente foi transferido para unidade de terapia intensiva e necessitou de ventilação mecânica. Não ocorreu óbito. Como fator de risco, identificado apenas a internação em quarto C (p = 0,001), local onde foi encontrada L. pneumophila em chuveirinho do banheiro por técnica de metagenômica. Neutropenia grave mostrou-se como um possível fator de risco (p = 0,054). Conclusão: O ambiente mostrou-se importante fator associado aos casos de legionelose. Contudo, devido ao número reduzido de casos não identificamos outros fatores, sendo apenas a neutropenia grave um fator de risco possivelmente relacionado ao paciente.

Os Clubes de Ciências são contextos de Educação Não Formal, investigados por diferentes perspectivas, entretanto, pouco se discute sobre o desenvolvimento profissional dos educadores que atuam nesse contexto. Partindo desse pressuposto, elaboramos uma pesquisa de revisão integrativa com o objetivo de caracterizar a formação docente em Clubes de Ciências comunicados na produção científica brasileira, principalmente se tratando dos percursos de formação continuada. Dos 32 trabalhos identificados, 27 estão relacionados à formação inicial e apenas 5 investigam a formação continuada de educadores que atuam em Clubes de Ciências. Contudo, os percursos formativos ficam pouco evidentes ou não são relatados. Constatamos assim, uma lacuna de conhecimentos acerca da formação continuada docente nesse contexto de educação não formal. Com relação à contribuição que esses contextos podem oferecer aos educadores, verificamos que não há incentivo para que suas práticas sejam divulgadas na sociedade e o percurso de desenvolvimento profissional não encoraja os educadores a participarem de espaços públicos. Sugerimos uma ampliação e aprofundamento das discussões acerca do desenvolvimento profissional nesses contextos, uma vez que os Clubes de Ciências possuem suas especificidades em educação científica e, por conseguinte, à profissionalização de seus educadores.

IntroductionLeishmaniasis, a neglected tropical parasitic disease, is regarded as a major public health problem worldwide. The first-line drugs for leishmaniasis suffer from limitations related to toxicity and the development of resistance in certain parasitic strains. Therefore, the discovery of alternative treatments for leishmaniasis is imperative, and natural products represent a valuable source of potential therapeutic agents.MethodsThe present study aimed at finding new potential antileishmanial agents from the aerial parts of the medicinal plant Momordica charantia. This study was based on bioassay-guided fractionation of the M. charantia extract against promastigotes and amastigotes of Leishmania (Leishmania) amazonensis. The cytotoxicity of the extract, fractions, and isolated compounds were evaluated against peritoneal murine macrophages by employing the MTT assay for assessing cell metabolic activity.ResultsAntileishmanial assay-guided fractionation of the M. charantia extract led to the bioactive cucurbitacin-enriched fraction and the isolation of four bioactive cucurbitacin-type triterpenoids, which exhibited significant antileishmanial activity, with IC50 values between 2.11 and 3.25 μg.mL−1 against promastigote and amastigote forms, low toxicity and selectivity indexes ranging from 8.5 to 17.2.ConclusionOur findings demonstrate that the fractions and cucurbitacin-type triterpenoids obtained from the aerial parts of M. charantia are promising natural leishmanicidal candidates.

ABSTRACT Background: The intensity of dengue virus (DV) replication and circulating non-structural protein 1 (NS1) levels may promote changes in the human immune response and favor severe forms of infection. We investigated the correlations between NS1 with CXCL-8, CXCL-10, IFN-γ, and IL-12p40 serum levels, and IFN-γ receptor α chain (CD119) expression, and CXCL10 production by peripheral blood mononuclear cells (PBMCs) stimulated with recombinant IFN-γ in DV-infected patients with different clinical forms. Methods: Dengue virus NS1, CXCL-8, CXCL-10, IFN-γ, and IL-12p40 serum levels were measured in 152 DV-infected patients with different clinical forms and 20 non-infected individuals (NI) using enzyme-linked immunosorbent assay (ELISA). In addition, we investigated the CXCL-10 production after in vitro IFN-γ stimulation of PBMCs from 48 DV-infected individuals (with different clinical forms of dengue fever) and 20 NI individuals using ELISA, and CD119 expression on CD14+ cells with flow cytometry. Results: Patients with dengue hemorrhagic fever (DHF) had significantly higher NS1, CXCL-8, and CXCL-10 serum levels than those with classic dengue fever (DF). The response of PBMCs to IFN-γ stimulation was lower in patients with DHF than in those with DF or dengue with complications (DWC), with lower CD119 expression and reduced CXCL-10 synthesis. In addition, these alterations are associated with high NS1 serum levels. Conclusions: Patients with DHF reported high NS1 levels, low CD119 expression, and low CXCL-10 synthesis in PBMCs, which may be associated with infection progression and severity.

A tuberculose, doença infecciosa crônica causada pelo Mycobacterium tuberculosis, continua sendo um problema de saúde pública de elevada magnitude. Esta pesquisa teve por objetivo traçar o perfil epidemiológico dos casos de tuberculose notificados no Brasil, entre os anos de 2010 e 2022. Realizou-se um estudo transversal, descritivo, retrospectivo e quantitativo que utilizou como unidades de análise os estados brasileiros. A amostra foi delimitada ao período de 2010 a 2022, e as informações foram coletadas do Departamento de HIV/Aids, Tuberculose, Hepatites Virais e Infecções Sexualmente Transmissíveis – DVIAHV, e do SINAN (Sistema de Informação de Agravos de Notificação), sendo considerados: ano e mês da notificação, município de notificação, município e zona de residência, idade, sexo, raça, escolaridade, município provável da infecção, classificação final, critério de confirmação e evolução do caso. As informações obtidas foram tabuladas no software R versão 4.2.2 e submetidas a análise estatística descritiva. Foram notificados, no período, 1.163.946 casos de tuberculose em todo o Brasil. Os principais acometidos foram indivíduos do sexo masculino com faixa etária de 20 a 49 anos. Relataram fazer uso de tabaco 15,6%, álcool 17,6% e drogas ilícitas 10,8%. Destacou-se o estado do Amazonas e Rio de Janeiro com maior prevalência da doença. A taxa de mortalidade foi de 3,7%. Conclui-se que o perfil dos acometidos por tuberculose no Brasil é caracterizado por indivíduos do sexo masculino, de 20 a 39 anos de idade e da raça parda. Além disso, destacou-se indivíduos que fazem uso de tabaco, álcool e drogas ilícitas. Por isso, medidas de controle e prevenção da tuberculose devem ser intensificadas para esta população mais vulnerável.

ABSTRACT Objective: To analyze the evidence on the influence of Lean and/or Six Sigma for process optimization in the perioperative period. Methods: Integrative review carried out in the MEDLINE (PubMed), Web of Science, EMBASE, CINAHL, Scopus and LILACS databases on the use of Lean and/or Six Sigma to optimize perioperative processes. The studies included were analyzed in three thematic categories: flow of surgical patients, work process and length of stay. Results: The final sample consisted of ten studies, which covered all operative periods. Lean and/or Six Sigma make a significant contribution to optimizing perioperative processes. Final considerations: Lean and/or Six Sigma optimize perioperative processes to maximize the achievement of system stability indicators, making it possible to identify potential problems in order to recognize them and propose solutions that can enable the institution of patient-centered care.

Abstract This study aimed to evaluate the influence of the Er,Cr:YSGG irradiation and 980-nm diode lasers on the surface roughness (SR) and volume loss (VL) of dentin subjected to cariogenic challenge. Subsequently, 130 specimens of bovine dentin were divided into the following 13 groups: NT: no treatment; FG: fluoride gel; FV: fluoride varnish; Di: 980-nm diode; Di + FG; Di + FV; FG + D; FV + Di; Er: Er,Cr:YSGG; Er + FG; Er + FV; FG + Er and FV + Er. Er,Cr:YSGG laser parameters were as follows: 0.25 W; 5.0 Hz; 4.46 J/cm2 without water and 55% air. Furthermore, the 980-nm diode laser parameters were 2.0 W; 2.0 Hz; 21.41 J/cm2. The samples from each group were subjected to pH cycling. A confocal laser scanning microscope was used to evaluate SR and VL. Difference between the volume of the reference and treated areas + DES/RE was used to determine SR and VL. The mean values of the different groups were subjected to analysis of variance and Tukey’s post-hoc test. The VL values were analyzed using the Kruskal–Wallis and Dunn post-hoc test (p < 0.05). The SR of the reference area did not show a statistically significant 1807-3107-bor-38-e025treatment and cariogenic challenge (p > 0.05). Moreover, VL in the FV + Di and FV + Er groups showed a statistically significant difference compared with areas submitted to different types of treatment and cariogenic challenge (p > 0.05). Er,Cr:YSGG and 980-nm diode lasers associated with fluoride varnishes decreased dentin VL in bovine teeth submitted to cariogenic challenge.

Objetivo: relatar a experiência do reconhecimento de uma área de saúde identificando as suas potências e vulnerabilidades com o auxílio dos processos de territorialização. Materiais e Métodos: durante o mês de março, a equipe de acadêmicos do terceiro período de Enfermagem. Primeiramente avaliou o território, as microáreas e especificidades de uma Estratégia de Saúde da Família de um município norte-mineiro. Em decorrência disso, o grupo escolheu mapear e atualizar uma área, com a avaliação integral dos recursos socioeconômicos. A atualização foi viabilizada com a procura, no âmbito das ruas da microárea, por cada domicílio e pelas características sociais e econômicas do território. Com o auxílio dos agentes comunitários de saúde e do aplicativo digital integralizado do sistema de saúde, os discentes, também, tiveram acesso às demandas da população e puderam registrar em um croqui. Além disso, a equipe entrevistou um informante chave, morador antigo do bairro, a fim de obter dados acerca da história e das subjetividades da construção do bairro. Resultados: a avaliação do território permitiu reconhecer as características socioeconômicas, culturais, geográficas e estruturais do espaço geográfico avaliado não somente como espaço físico estático, mas, também, dotado de processos sociais. Por fim, a compreensão de todo o cenário de saúde resultou na atualização dessa área específica de saúde do município. Conclusão: com o êxito da renovação territorial da microárea, os acadêmicos, munidos pela experiência positiva com o trabalho do enfermeiro e da equipe multiprofissional em saúde, aproximaram-se das demandas futuras, enquanto profissionais da enfermagem, de forma integral.

Abstract The terracing is important for erosion processes control in agricultural areas. However, agricultural terraces are being removed from the production areas under no-tillage systems. There are still uncertainties about the terracing effects on the soil saturated hydraulic conductivity (Ks) and macroporosity (Macro), which represent the functionality of the system. We aimed to evaluate the magnitude and spatial distribution of Ks and Macro and the correlation between these soil properties in two paired megaplots, of 1.923 ha, one with terraces (T) and another without terraces (WT). Ks and Macro were determined at 0.00-0.10 m, 0.10-0.20 m, 0.20-0.30 m, and 0.30-0.40 m soil layers and submitted to descriptive statistical, geostatistics, and Spearman correlation analysis. Ks was highest in the 0.00-0.20 m and lowest in the 0.20-0.40 m layers in T. In WT, Ks was highest in the 0.00-0.10 m and lowest in the 0.10-0.40 m layers. However, the Macro was highest in the 0.00-0.10 m and lowest in the 0.10-0.40 m layers in both megaplots. Ks and Macro had a positive correlation in both megaplots. The spatial distribution of Ks and Macro had a positive correlation, with regions with higher Ks coinciding with regions with higher Macro in both megaplots. The spatial distribution of Ks and Macro in WT did not have a clear trend, while in T there was a slight stratification in strips interspersed with higher and lower Ks and Macro. These initial trends are not conclusive considering the short term between terraces removal and Ks and Macro evaluation.

Introdução: As meningites são um problema global e de emergência médica, que requer diagnóstico e tratamento imediato, devido a sua elevada taxa de mortalidade e morbidade. Objetivo: Retratar a epidemiologia das meningites infecciosas no estado de Mato Grosso do Sul, de 2010 a 2022. Método: Estudo transversal, descritivo, retrospectivo e quantitativo que utilizou como unidades de análise o estado de Mato Grosso do Sul. A amostra foi delimitada ao período de 2010 a 2022, as informações foram codificadas e coletadas do Sistema de Informação de Agravos de Notificação (Sinan), sendo considerados: ano e mês da notificação, município de notificação, idade, sexo, raça/cor, escolaridade, etiologia, sorogrupo, critério de confirmação e evolução do caso. Resultados: No período analisado foram notificados 2.826 casos de meningite com 71,3% de confirmação. Destacou-se como mais acometidos os indivíduos do sexo masculino, de raça/cor parda, e as crianças de zero a nove anos de idade. A prevalência no estado foi de 7,7 casos por 10.000 habitantes. A maioria dos casos de meningite (39,7%) foi classificada como meningite não especificada e 23,1%, como meningite asséptica. A taxa de letalidade no período foi de 11,3%, com concentração dos óbitos nos extremos de idade. Conclusões: Embora a meningite seja uma doença conhecida há muito tempo, sua taxa de letalidade continua elevada. Sugere-se a intensificação das ações de controle e a prevenção junto aos municípios aliada à demonstração da importância da notificação do agravo junto à vigilância epidemiológica, o que auxiliará a organização de estratégias de ação mais pontuais e direcionadas ao combate dos casos.

Background Healthcare professionals (HCPs) play an important role in vaccination; those with low confidence in vaccines are less likely to recommend them to their patients and to be vaccinated themselves. The study’s purpose was to adapt and validate long- and short-form versions of the International Professionals’ Vaccine Confidence and Behaviors (I-Pro-VC-Be) questionnaire to measure psychosocial determinants of HCPs’ vaccine confidence and their associations with vaccination behaviors in European countries. Research design and methods After the original French-language Pro-VC-Be was culturally adapted and translated, HCPs involved in vaccination (mainly GPs and pediatricians) across Germany, Finland, France, and Portugal completed a cross-sectional online survey in 2022. A 10-factor multigroup confirmatory factor analysis (MG-CFA) of the long-form (10 factors comprising 34 items) tested for measurement invariance across countries. Modified multiple Poisson regressions tested the criterion validity of both versions. Results 2,748 HCPs participated. The 10-factor structure fit was acceptable to good everywhere. The final MG-CFA model confirmed strong factorial invariance and showed very good fit. The long- and short-form I-Pro-VC-Be had good criterion validity with vaccination behaviors. Conclusion This study validates the I-Pro-VC-Be among HCPs in four European countries; including long- and short-form tools for use in research and public health.

34. Performance of severity indices for admission and mortality of trauma patients in the intensive care unit: a retrospective cohort study

Abstract Background Little is known about the performance of severity indices for indicating intensive care and predicting mortality in the Intensive Care Unit (ICU) of trauma patients. This study aimed to compare the performance of severity indices to predict trauma patients’ ICU admission and mortality. Methods A retrospective cohort study which analyzed the electronic medical records of trauma patients aged ≥ 18 years, treated at a hospital in Brazil, between 2014 and 2017. Physiological [Revised Trauma Score (RTS), New Trauma Score (NTS) and modified Rapid Emergency Medicine Score (mREMS)], anatomical [Injury Severity Score (ISS) and New Injury Severity Score (NISS)] and mixed indices [Trauma and Injury Severity Score (TRISS), New Trauma and Injury Severity Score (NTRISS), Base-deficit Injury Severity Score (BISS) and Base-deficit and New Injury Severity Score (BNISS)] were compared in analyzing the outcomes (ICU admission and mortality) using the Area Under the Receiver Operating Characteristics Curves (AUC–ROC). Results From the 747 trauma patients analyzed (52.5% female; mean age 51.5 years; 36.1% falls), 106 (14.2%) were admitted to the ICU and 6 (0.8%) died in the unit. The ISS (AUC 0.919) and NISS (AUC 0.916) had better predictive capacity for ICU admission of trauma patients. The NISS (AUC 0.949), TRISS (AUC 0.909), NTRISS (AUC 0.967), BISS (AUC 0.902) and BNISS (AUC 0.976) showed excellent performance in predicting ICU mortality. Conclusions Anatomical indices showed excellent predictive ability for admission of trauma patients to the ICU. The NISS and the mixed indices had the best performances regarding mortality in the ICU.

Red rot disease reduces sugarcane yield and impacts the sugar quality, posing an important threat to the sugarcane industry in Florida. Although Colletotrichum falcatum, the causal agent of red rot in Florida, was first reported in 1984 based on morphology, molecular and pathological data have remained limited, highlighting the critical need for comprehensive characterization. Thirteen isolates were obtained from three local sugarcane varieties in Belle Glade, Florida. Phylogenetic analyses of five genetic markers (ITS, ACT, TUB2, GAPDH, and CHS-1) confirmed all the strains as C. falcatum. In addition, the study documented the disease progression at the cellular level and assessed the pathogenicity of representative strains using the leaf sheath and whole-seedling inoculation methods. The varieties CP96-1252 and CP89-2143 showed greater host resistance. These findings represent the first report of C. falcatum causing red rot in southern Florida, offer valuable insights for/into red rot management, and provide a basis for future breeding programs to enhance sugarcane resistance to red rot disease.

36. Afro-Latin American Pharmacogenetics of CYP2D6, CYP2C9, and CYP2C19 in Dominicans: A Study from the RIBEF-CEIBA Consortium

Background/Objectives: Research on pharmacogenetic variability in response to prescribed drugs and across ethnic groups is essential for personalized medicine, particularly in admixed and unstudied populations. For the first time, this study examines CYP2D6, CYP2C9, and CYP2C19 alleles and genotypes in 197 healthy volunteers from the Dominican Republic, as part of the RIBEF-CEIBA collaborative network. Methods: The analysis focuses on the participants’ tri-hybrid genomic ancestry, with CYP alleles determined by real-time PCR and molecular ancestry inferred using 90 AIMs. Linear regression was used to associate ancestry components with CYP frequencies. Results: The average ancestry was 23.8% European, 42.6% Native American, and 33.6% African, the latter being higher than in most Latin American populations. Native American ancestry was also higher than expected. Predicted phenotype frequencies based on genotypes were 4.2% poor metabolizers (gPMs) and 3.6% ultrarapid metabolizers (gUMs) for CYP2D6, as well as 3% gPMs, 22.8% rapid metabolizers (gRMs), and 1.5% gUMs for CYP2C19. No gPM individuals were observed for CYP2C9. Certain alleles associated with decreased CYP2D6 activity (*17 and *29) and increased CYP2C19 activity (*17 and gUMs) were positively linked with African ancestry and negatively with Native American ancestry. Rare CYP2C9 alleles (*5 and *6) with clinical relevance were additionally found. Conclusions: These findings build on previous results from the RIBEF-CEIBA collaborative network, demonstrating differences in allele frequencies of CYP2D6, CYP2C9, and CYP2C19 in relation to genomic ancestry. In summary, ethnicity must be considered in the development of pharmacogenetic guidelines for clinical application, research, and regulation to avoid widening the biotechnology gap and to allow Personalized Medicine to reach the entire world population.

Background/Objectives: Cytokine storm in severe COVID-19 is responsible for irreversible tissue damage and death. Soluble mediators from the TNF superfamily, their correlation with clinical outcome, and the use of TNF receptors as a potent predictor for clinical outcome were evaluated. Methods: Severe COVID-19 patients had the levels of soluble mediators from the TNF superfamily quantified and categorized according to the clinical outcome (death versus survival). Statistical modeling was performed to predict clinical outcomes. Results: COVID-19 patients have elevated serum levels from the TNF superfamily. Regardless of sex and age, the sTNFRI levels were observed to be significantly higher in deceased patients from the first weeks following the onset of symptoms. We analyzed hematological parameters and inflammatory markers, and there was a difference between the groups for the following factors: erythrocytes, hemoglobin, hematocrit, leukocytes, neutrophils, band cells, lymphocytes, monocytes, CRP, IL-8, IFN-γ, IL-10, IL-6, IL-4, IL-2, leptin MIF sCD40L, and sTNFRI (p < 0.05). A post hoc analysis showed an inferential capacity over 70% for some hematological markers, CRP, and inflammatory mediators in deceased patients. sTNFRI was strongly associated with death, and the sTNFRI/sTNFRII ratio differed between outcomes (p < 0.001; power above 90%), highlighting the impact of these proteins on clinical results. The final logistic model, including sTNFRI/sTNFRII and CRP, indicated high sensitivity, specificity, accuracy, and an eight-fold higher odds ratio for an unfavorable outcome. Conclusions: The joint use of the sTNFRI/sTNFRII ratio with CRP proves to be a promising tool to assist in the clinical management of patients hospitalized for COVID-19.

Abstract Background Given the rising occurrence of antibiotic resistance due to the existence and ongoing development of resistant bacteria and phenotypes, the identification of new treatments and sources of antimicrobial agents is of utmost urgency. An important strategy for tackling bacterial resistance involves the utilization of drug combinations, and natural products derived from plants hold significant potential as a rich source of bioactive compounds that can act as effective adjuvants. This study, therefore, aimed to assess the antibacterial potential and the chemical composition of Miconia albicans, a Brazilian medicinal plant used to treat various diseases. Methods Ethanolic extracts from leaves and stems of M. albicans were obtained and subsequently partitioned to give the corresponding hexane, chloroform, ethyl acetate, and hydromethanolic phases. All extracts and phases had their chemical constitution investigated by HPLC–DAD-MS/MS and GC–MS and were assessed for their antibiofilm and antimicrobial efficacy against Staphylococcus aureus. Furthermore, their individual effects and synergistic potential in combination with antibiotics were examined against clinical strains of both S. aureus and Acinetobacter baumannii. In addition, 10 isolated compounds were obtained from the leaves phases and used for confirmation of the chemical profiles and for antibacterial assays. Results Based on the chemical profile analysis, 32 compounds were successfully or tentatively identified, including gallic and ellagic acid derivatives, flavonol glycosides, triterpenes and pheophorbides. Extracts and phases obtained from the medicinal plant M. albicans demonstrated synergistic effects when combined with the commercial antibiotics ampicillin and ciprofloxacin, against multi-drug resistant bacteria S. aureus and A. baumannii, restoring their antibacterial efficacy. Extracts and phases also exhibited antibiofilm property against S. aureus. Three key compounds commonly found in the samples, namely gallic acid, quercitrin, and corosolic acid, did not exhibit significant antibacterial activity when assessed individually or in combination with antibiotics against clinical bacterial strains. Conclusions Our findings reveal that M. albicans exhibits remarkable adjuvant potential for enhancing the effectiveness of antimicrobial drugs against resistant bacteria.

Objetivou-se caracterizar os casos de COVID-19 em crianças residentes no estado de Mato Grosso do Sul e analisar seus respectivos desfechos. Para esta pesquisa foi realizado um estudo transversal, descritivo, retrospectivo e qualiquantitativo delimitado de março de 2020 a dezembro de 2021, e os dados foram coletados a partir das informações disponíveis no Monitor de Apoio a Informações em Saúde (Painel Mais Saúde), Sivep-Gripe e no Instituto Brasileiro de Geografia e Estatística (IBGE), sendo: ano, mês, município, número de casos, faixa etária, sexo, taxa de letalidade e óbitos. Durante o período de estudo foram notificados 14.616 casos de COVID-19 em criança de 0 a 9 anos. Vale destacar que o pico de ocorrência foi no mês de maio de 2021, seguido por queda acentuada até dezembro do mesmo ano. Dentre os acometidos, os principais sintomas relatados foram febre (44,4%), tosse (43%), dor de garganta (25,2%) e dificuldade respiratória (4,1%). Evoluíram para SRAG 1,9% dos casos de síndrome gripal por COVID-19, destacando-se as menores de 1 ano. Dentre as crianças que evoluíram para quadro de SRAG por COVID-19, 43,3% apresentavam algum tipo de comorbidade, e a taxa de letalidade foi de 5%. Esta pesquisa permitiu concluir que a ocorrência de COVID-19 em crianças no estado de Mato Grosso do Sul apresentou média prevalência e que a evolução para SRAG ocorreu em uma quantidade baixa de casos, porém a taxa de letalidade neste público alcançou 5%, o que demonstra a importância da doença também para esta faixa etária

Introduction Physicians play a critical role in encouraging their patients to get vaccinated, in part by responding to patients’ concerns about vaccines. It is, therefore, important to understand what difficulties physicians have in dealing with different concerns they may encounter. The aim of this article was to determine physicians’ perceptions of difficulties in rebutting different antivaccination arguments from patients using data collected as part of a cross-sectional, cross-national questionnaire on physicians’ vaccine attitudes and behaviours.Methods Physicians in 4 European countries (Finland, Germany, France and Portugal, total n=2718) rated 33 different arguments, chosen to represent 11 different psychological motivations underlying vaccine hesitancy, in terms of their perceptions of how difficult each argument would be to rebut.Results Across all countries, physicians perceived arguments based on religious concerns and ‘reactance’ (ie, resistance to perceived curbs of freedom) to be the most difficult to rebut, whereas arguments based on patients’ distorted perception of the risks of disease and vaccines were perceived to be the easiest. There were also between-country differences in the level of perceived difficulty of argument rebuttal. Physicians’ perceived difficulty with rebutting arguments was significantly negatively correlated with their vaccine recommendation behaviours and their preparedness for vaccination discussions.Conclusions Physicians may feel better equipped to counter arguments that can be rebutted with facts and evidence but may struggle to respond when arguments are motivated by psychological dispositions or values.

The occurrence of mycotoxigenic species in peanuts is a major concern, and has been investigated in depth for many years. However, most studies focus on the occurrence of aflatoxigenic fungi, such as Aspergillus section Flavi. The present study aimed to clarify the occurrence of Aspergillus section Nigri, a group that harbors species capable of producing ochratoxin A (OTA), which has scarcely been investigated in peanuts. A total of 52 peanut samples, collected in the field and from storage, were analyzed. Aspergillus section Nigri was isolated from 64% and 100% of field and storage samples, respectively, and 218 strains were obtained. Based on the multiloci phylogeny of the CaM and BenA loci, six species of Aspergillus section Nigri were identified: A. brasiliensis, A. niger, A. neoniger, A. welwitschiae, A. costaricaensis, and A. japonicus. The incidence of ochratoxigenic strains was 5.0% (11/218), and only A. niger and A. welwitschiae were able to produce OTA. The presence of OTA in peanuts was found in 6 field and 8 storage samples, with levels ranging from 0.106 to 0.387 and 0.090 to 0.160 µg/kg, respectively.
