1. Epidemiology and molecular detection of human adenovirus and non-polio enterovirus in fecal samples of children with acute gastroenteritis: A five-year surveillance in northern Brazil.

Abstract: Acute gastroenteritis (AGE) is a common pediatric infection that remains a significant cause of childhood morbidity and mortality worldwide, especially in low-income regions. Thus, the objective of this study was to detect human adenovirus (HAdV) and non-polio enterovirus (NPEV) in fecal samples from the Gastroenteritis Surveillance Network, and to identify circulating strains by nucleotide sequencing. A total of 801 fecal samples were tested using qPCR/RT-qPCR, and 657 (82.0%) were inoculated into HEp-2C and RD cell lines. The HAdV and NPEV positivity rates obtained using qPCR/RT-qPCR were 31.7% (254/801) and 10.5% (84/801), respectively, with 5.4% (43/801) co-detection. Cytopathic effect was observed in 9.6% (63/657) of patients, 2.7% (18/657) associated with HAdV, and 6.2% (41/657) associated with NPEV after testing by ICC-PCR. A comparison of the two methodologies demonstrated an agreement of 93.5% for EVNP and 64.4% for HAdV. These two viruses were detected throughout the study period, with HAdV positivity rates ranging from 41% in Amapá to 18% in Pará. The NEPV varied from 18% in Pará/Rondônia to 3% in Acre. The most affected age group was over 60 months for both HAdV and NPEV. Samples previously positive for rotavirus and norovirus, which did not show a major difference in the presence or absence of diarrhea, fever, and vomiting, were excluded from the clinical analyses of these two viruses. These viruses circulated over five years, with a few months of absence, mainly during the months corresponding to the waves of SARS-CoV-2 infection in Brazil. Five HAdV species were identified (A, B, C, D, and F), with a greater predominance of HAdV-F41 (56.5%) followed by HAdV-C (15.2%). Three NPEV species (A, B, and C) were detected, with serotypes E14 (19.3%) and CVA-24 (16.1%) being the most prevalent. The present study revealed a high diversity of NPEV and HAdV types circulating in children with AGE symptoms in the northern region of Brazil.

Abstract: Introdução/Objetivos: As infecções respiratórias agudas (IRA) constituem um importante problema em saúde pública, especialmente aquelas desencadeadas pelo Vírus Sincicial Respiratório (VSR), um dos principais agentes virais associados à doença grave na população pediátrica em todo o mundo. O VSR pertence à família Pneumoviridae, gênero Orthopneumovirus e baseado em suas características antigênicas é classificado em dois subgrupos, VSRA e VSRB. Deste modo, este estudo objetivou identificar o subgrupo mais frequente e caracterizar geneticamente cepas de VSR em amostras oriundas de um surto de IRA, em pacientes de zero a dois anos de idade, ocorrido no Estado do Amapá, no período de março a maio de 2023. Métodos: Para tal, o Laboratório Central de Saúde Pública do Estado do Amapá enviou 188 amostras de secreção respiratória para a pesquisa de etiologia viral ao Laboratório de Vírus Respiratórios do Instituto Evandro Chagas (LVR-IEC), Laboratório de Referência junto a Rede Nacional de Vigilância de Influenza e outros vírus respiratórios do Ministério da Saúde. A análise das amostras envolveu a extração do ácido nucleico viral utilizando kit comercial, detecção e definição do subgrupo de VSR por Reação em Cadeia mediada pela Polimerase em tempo real precedida de Transcrição Reversa (RT-qPCR) e sequenciamento do genoma completo pela abordagem de metagenômica shotgun, na plataforma NextSeq 500 Illumina. Resultados: Das 188 amostras analisadas, 96 (51,06%) foram positivas para VSR, destas 61 (63,54%) pertencem ao subgrupo A e 16 (16,66%) ao B, em 19 amostras não foi possível identificar o subgrupo. O sequenciamento genômico foi realizado em duas cepas do subgrupo A e quatro do B. A análise genômica demonstrou que as cepas de VSRA agruparam no clado GA2.3.5 e as do subgrupo VSRB no clado GB5.0.5a. Conclusão: Nossos dados corroboram o importante papel do VSR na indução de IRA em pacientes pediátricos, com o VSRA sendo predominante na população investigada. A análise genética não evidenciou mudanças que possam conferir maior virulência, patogenicidade e/ou transmissibilidade das cepas detectadas no Amapá. Acredita-se que as medidas de proteção adotadas durante a pandemia de COVID-19 afetaram a circulação de outros vírus respiratórios, como o VSR, criando, desta forma, grupos suscetíveis à infecção. Nossos dados reforçam a importância do monitoramento constante do VSR, para auxiliar no melhor manejo clínico e controle de infecções por esse patógeno em crianças.

Abstract: Introdução/objetivo: Popularmente conhecida como varíola dos macacos a monkeypox (MPOX) é uma zoonose ocasionada por um Orthopoxyvirus (OPXV) que teve seu primeiro caso descrito em humanos na República Democrática do Congo em 1970, tornando-se posteriormente endêmico em países da África Central e Ocidental. Em maio de 2022 foi relatada a detecção em vários países não endêmicos onde não se tinham ligações epidemiológicas conhecidas e o número de casos continuou a aumentar com a contínua transmissão em todo o mundo, no Brasil o primeiro caso foi confirmado em junho de 2022, logo após foi instituída a vigilância de rotina dos casos suspeitos. Diante disto, este estudo objetivou descrever a prevalência, as características epidemiológicas e clínicas dos casos confirmados de MPOX no Estado do Pará. Métodos: Para isso, foram analisadas amostras de swab de lesão coletadas de casos suspeitos e que deram entrada no Laboratório de Enterovírus (LEV) do Instituto Evandro Chagas (IEC) que atua como referência laboratorial para o Monkeypox no Estado do Pará no período de julho de 2022 a junho de 2023. Foi realizado o levantamento e tabulação das informações epidemiológicas e clínicas dos casos, após isso as amostras foram extraídas e testadas por Reação em Cadeia mediada pela Polimerase em tempo real (qPCR) utilizando oligonucleotídeos e sondas específicas para detecção de MPOX. Resultado: Das 284 amostras recebidas 45,8% (130) foram confirmadas, onde 95% (124) pertenciam a pacientes do sexo masculino tendo o grupo etário de 20 a 29 anos com 86% (61) dos casos. Os sintomas mais comuns relatados foram febre (78,4%), lesão cutânea (62,3%), cefaléia (60%) e adenomegalia (38,4%). Dos pacientes positivos 86% (112) se enquadram no grupo de homens que fazem sexo com homens, 52% (68) são pessoas que vivem com HIV e 18% (24) possuem alguma infecção sexualmente transmissível ativa, sendo a sífilis a mais prevalente com 91,6% (22) dos casos. Foi registrado a hospitalização e posterior óbito de um paciente que vivia com HIV e possuía sífilis ativa. Conclusão: Diante disto, pode-se avaliar que o perfil clínico e epidemiológico dos pacientes no Estado do Pará se assemelha com os descritos em literatura em todo o mundo, logo, torna-se fundamental a vigilância epidemiológica deste patógeno uma vez que atual e rápida disseminação e evolução do MPOX não têm precedentes e representa uma ameaça contínua à saúde pública.

4. INVESTIGAÇÃO DE PARECHOVIRUS, AICHIVIRUS E COSAVIRUS EM AMOSTRAS DE GASTROENTERITE AGUDA NA REGIÃO NORTE E NORDESTE DO BRASIL

Abstract: Introdução/objetivos: As gastroenterites agudas (GEA) são doenças que se constituem como a segunda maior causa de morte em crianças de todo mundo, especialmente em crianças menores de cinco anos, ocorrendo principalmente em países de baixa e média renda. Os agentes virais como rotavírus e norovírus têm sido demonstrados como as causas mais frequentes, no entanto alguns membros da família Picornaviridae também foram associados à diarreia em humanos. Diante disto, objetivou-se a realização da vigilância expandida dos picornavírus em amostras fecais de crianças com quadro de gastroenterite aguda em estados da região Norte e Nordeste do Brasil entre os anos de 2020 e 2021. Métodos: Para isso, foram utilizadas suspensões fecais provenientes de crianças com quadro de gastroenterite aguda e encaminhadas ao Laboratório de Enterovírus do Instituto Evandro Chagas através das redes de vigilância de rotavírus e póliovirus nos anos de 2020 e 2021. Para detecção foi realizada a extração do RNA viral utilizando o QIAmp Viral RNA Mini Kit (QIAGEN), posteriormente foi realizada a reação de RT-qPCR utilizando o kit comercial GoTaq Probe 1-Step RT-qPCR System (Promega) e oligonucleotídeos e sondas específicos para detecção de Parechovirus, Aichivirus e Cosavirus. Foram analisadas 419 amostras de suspensão fecal, nas quais 31,7% (133) foram positivas para parechovirus (HpeV), aichivírus (AiV) e cosavírus (CosV). Resultados: Com isso, foi observado à prevalência dos CosV, os quais estavam presentes em 57,1% (76/133) das amostras, seguido pelo HpeV com 37% (49/133) e por fim os AiV com 6% (8/133). Foram identificadas 18 codetecções entre os vírus investigados, sendo possível observar de HpeV com CosV em 13 amostras, duas codetecções entre AiV e CosV e três codetecções entre os três vírus investigados. Conclusão: Diante do exposto, conclui-se que é importante a vigilância desses vírus para fomentar estudos que visem comprovar sua atuação e comportamento como agentes causadores de gastroenterite, visto que sua prevalência em amostras vem se tornando cada vez mais prevalente e a escassez de estudos no mundo e no Brasil com esse objetivo acabam postergando esses resultados, por isso torna-se de extrema importância à continuidade dessa pesquisa nas regiões norte e nordeste, por conta de sua alta prevalência, para saber sua atuação e comportamento como agentes gastroentéricos.

Abstract: Introdução/objetivos: Dentre as doenças infecciosas de grande importância para a saúde pública, a gripe ou influenza desempenha um papel significativo nos índices de morbidade e mortalidade em todo o mundo. Tradicionalmente, os vírus influenza A são os mais descritos devido ao seu potencial de causar pandemias, no entanto os vírus influenza B apresentam grande impacto sendo associados a epidemias sazonais e ocasionando doenças graves, principalmente em crianças. Diante disto, este estudo objetivou investigar as propriedades moleculares dos vírus influenza B que circularam durante a epidemia sazonal em estados das Regiões Norte e Nordeste do Brasil no ano de 2023. Metodologia: Foram analisadas 387 amostras com resultado de RT-qPCR positivo para influenza B recebidas no Laboratório de Vírus Respiratório (LVR) do Instituto Evandro Chagas de janeiro a maio de 2023. Destas, 184 foram selecionadas para o sequenciamento genômico, adotando-se os seguintes critérios, amostras com Ct≤ 29, distribuídas por semanas epidemiológicas e unidades federativas, visando garantir que houvesse representatividade temporal e espacial. As bibliotecas foram preparadas utilizando Nextera XT DNA Library Preparation Kit (Ilumina) e submetidas ao sequenciamento de nova geração por amplicon, na plataforma MiSeq Illumina. As sequências obtidas foram montadas e alinhadas com cepas vacinais e outras obtidas de diferentes regiões do Brasil e do Mundo disponibilizadas no GISAID. Resultados: Durante o período analisado foram gerados 174 genomas completos de influenza B. A análise genética demonstrou que todas as amostras pertenciam a linhagem Victoria, clado V1A.3ª.2, que apresentam como marcadores genéticos a deleção de três aminoácidos (resíduos HÁ1 162-164), classificando-as no grupo V.1ª.3. Desta forma, as cepas circulantes apresentam-se geneticamente relacionada a cepa vacinal B/Austria/1359417/2021 (V.1ª.3ª.2) preconizada para o ano de 2023 no hemisfério sul. Conclusão: A vigilância genômica dos vírus influenza nos permite entender melhor os a dinâmica evolutiva destes agentes, gerando dados que nos permitem inferir sobre a compatibilidade das cepas circulantes com as que compõe a vacina. Desta forma, favorecendo a instituição de intervenções efetivas visando melhorar a aceitação da vacina contra influenza, garantindo assim um maior impacto da vacinação, especialmente no que tange a redução do ônus trazido por esta doença para a população humana.

Abstract: Abstract Although acute gastroenteritis (AGE) has been reported as a common infectious disease in children, there is scarce information about enterovirus (EV) circulating associated with AGE cases in Brazil. The purpose of the present study was to identify and characterize the enteroviruses associated with AGE in children in Belém, Brazil. A total of 175 stool samples were obtained from children hospitalized revealing the presence of EV in 26.3% (46/175) of infections. EV type was identified in 78.3% (36/46) and EV-B species (61.1%; 22/36) was the most prevalent EV-detected followed by EV-C (25%; 9/36) and EV-A (13.9%; 5/36). This study has provided important information about the enterovirus circulation in Pará state, Northern Brazil.

Abstract: The last case of paralytic poliomyelitis caused by wild poliovirus in Brazil occurred in 1989. The interruption of the indigenous poliovirus transmission was obtained through mass immunization campaigns to eligible children and an active epidemiological and laboratorial surveillance of all cases of acute flaccid paralysis (AFP) among children under 15 y of age. This paper describes and evaluates the performance of the AFP surveillance system in different geographic areas of Brazil between 2005 and 2014, using indicators recommended by WHO. AFP surveillance indicators as well as virological investigation of polio and non-polio enteroviruses in stool samples received in the laboratory were assessed from 2005–2014. During the period, 5463 cases of AFP were investigated. Of these, 55% were males and 45% were females. Those under 5 y of age represented 48% of all cases reported and investigated. AFP notification rate was within the acceptable values with mean value of 1.3 (North), 1.4 (Northeast), 1.1 (Southern), 1.0 (Southeast) and 1.4 (Midwest) cases of AFP per 100.000 population aged 15 y as well as the adequacy of fecal specimens received in the laboratory. Sabin- related polioviruses accounted for 1.7% of the isolates while, 6.7% were non-polio enterovirus with the values ranging from 5.0% to 8.9 %. No wild-type polio was found. The AFP epidemiological and laboratorial surveillance activities have been kept at appropriate levels in Brazil. These data are a very strong indication, which supports the status of country free of polio.

Abstract: This study showed that laboratory markers of recent infection by dengue, Zika or chikungunya arboviruses were detected in the biological samples of approximately one-third of patients with encephalitis, myelitis, encephalomyelitis or Guillain-Barré syndrome, in a surveillance programme in Piauí state, Brazil, between 2015-2016. Fever and myalgia had been associated with these cases. Since in non-tropical countries most infections or parainfectious diseases associated with the nervous system are attributed to herpesviruses, enteroviruses, and Campylobacter jejuni, the present findings indicate that in tropical countries, arboviruses may now play a more important role and reinforce the need for their surveillance and systematic investigation in the tropics.

Abstract: Outbreaks caused by vaccine-derived polioviruses are challenging the final eradication of paralytic poliomyelitis. Therefore, the surveillance of the acute flaccid paralysis cases based on poliovirus isolation and characterization remains an essential activity. Due to the use of trivalent oral poliovirus vaccine (OPV), mixtures containing more than one serotype of Sabin-related polioviruses are frequently isolated from clinical samples. Because each poliovirus isolate needs to be individually analyzed, we designed polymerase chain reaction primers that can selectively distinguish and amplify a genomic segment of the three Sabin-related poliovirus serotypes present in mixtures, thus, optimizing the diagnosis and providing prompt information to support epidemiologic actions.

Abstract: As in humans, sub-clinical infection by arboviruses in domestic animals is common; however, its detection only occurs during epizootics and the silent circulation of some arboviruses may remain undetected. The objective of the present paper was to assess the current circulation of arboviruses in the Nhecolândia sub-region of South Pantanal, Brazil. Sera from a total of 135 horses, of which 75 were immunized with bivalent vaccine composed of inactive Eastern equine encephalitis virus (EEEV) and Western equine encephalitis virus(WEEV) and 60 were unvaccinated, were submitted to thorough viral isolation, reverse transcriptase polymerase chain reaction (RT-PCR) and neutralization tests for Saint Louis encephalitis virus (SLEV), EEEV, WEEV and Mayaro virus (MAYV). No virus was isolated and viral nucleic-acid detection by RT-PCR was also negative. Nevertheless, the prevalence of neutralizing antibodies in horses older than seven months was 43.7% for SLEV in equines regardless of vaccine status, and 36.4% for WEEV and 47.7% for EEEV in unvaccinated horses. There was no evidence of MAYV infections. The serologic evidence of circulation of arboviruses responsible for equine and human encephalitis, without recent official reports of clinical infections in the area, suggests that the Nhecolândia sub-region in South Pantanal is an important area for detection of silent activity of arboviruses in Brazil.

Abstract: In view of the high circulation of migratory birds and the environmental and climatic conditions which favor the proliferation of arthropods, the Brazilian Pantanal is susceptible to circulation of arboviruses. However, the amount of data concerning arbovirus vectors in this area is scarce; therefore the aim of this study was to conduct a preliminary investigation of Culicidae species in the Nhecolândia Sub-region of South Pantanal, Brazil and their potential importance in the arbovirus transmission. A total of 3684 specimens of mosquitoes were captured, 1689 of which caught in the rainy season of 2007, were divided into 78 pools and submitted to viral isolation, Semi-Nested RT-PCR and Nested RT-PCR, with a view to identifying the most important arboviruses in Brazil. Simultaneously, 70 specimens of ticks found blood-feeding on horses were also submitted to the same virological assays. No virus was isolated and viral nucleic-acid detection by RT-PCR was also negative. Nevertheless, a total of 22 Culicidae species were identified, ten of which had previously been reported as vectors of important arboviruses. The diversity of species found blood-feeding on human and horse hosts together with the arboviruses circulation previously reported suggest that the Nhecolândia Sub-region of South Pantanal is an important area for arbovirus surveillance in Brazil.Regiões como o Pantanal brasileiro, que apresentam fatores como riqueza de fauna silvestre incluindo circulação de aves migratórias e condições ambientais e climáticas favoráveis à proliferação de artrópodes estão potencialmente sujeitas à circulação de arbovírus. Entretanto, poucos trabalhos foram realizados acerca da presença de arbovírus em potenciais vetores no Pantanal. Neste sentido o principal objetivo deste trabalho foi conduzir uma investigação preliminar para presença de arbovírus em amostragens de culicídeos capturados na Sub-região da Nhecolândia no Pantanal Sul. Um total de 3684 mosquitos foi capturado, dos quais 78 grupos compondo uma amostragem de 1789 espécimes foram submetidos às técnicas de isolamento viral e RT-PCR para os mais importantes arbovírus no Brasil. Simultaneamente, 70 espécimes de carrapatos capturados durante hematofagia em cavalos também foram submetidos à pesquisa viral. Não houve isolamento viral em nenhuma amostra analisada e os resultados de detecção de ácido nucléico viral foram também negativos. Entretanto, foram identificadas 22 espécies de culicídeos, dez das quais previamente reportadas como vetores de importantes arbovírus. A competência vetorial de espécies capturadas durante hematofagia em humanos e cavalos aliada ao relato prévio de circulação de arbovírus sugerem a Sub-região da Nhecolândia como uma importante área de vigilância para arbovírus no Centro-Oeste do Brasil.

Abstract: We have determined the complete nucleotide and the deduced amino acid sequences of Brazilian dengue virus type 3 (DENV-3) from a dengue case with fatal outcome, which occurred during an epidemic in the state of Rio de Janeiro, Brazil, in 2002. This constitutes the first complete genetic characterization of a Brazilian DENV-3 strain since its introduction into the country in 2001. DENV-3 was responsible for the most severe dengue epidemic in the state, based on the highest number of reported cases and on the severity of clinical manifestations and deaths reported.

14. Molecular characterization and phylogenetic study of coxsackievirus A24v causing outbreaks of acute hemorrhagic conjunctivitis (AHC) in Brazil.

Abstract: BACKGROUND: Coxsackievirus A24 variant (CA24v) is the most prevalent viral pathogen associated with acute hemorrhagic conjunctivitis (AHC) outbreaks. Sixteen years after its first outbreak in Brazil, this agent reemerged in 2003 in Brazil, spread to nearly all states and caused outbreaks until 2005. In 2009, a new outbreak occurred in the northeast region of the country. In this study, we performed a viral isolation in cell culture and characterized clinical samples collected from patients presenting symptoms during the outbreak of 2005 in Vitória, Espírito Santo State (ES) and the outbreak of 2009 in Recife, Pernambuco State (PE). We also performed a phylogenetic analysis of worldwide strains and all meaningful Brazilian isolates since 2003. METHODS AND FINDINGS: Sterile cotton swabs were used to collect eye discharges, and all 210 clinical samples were used to inoculate cell cultures. Cytopathic effects in HEp-2 cells were seen in 58 of 180 (32%) samples from Vitória and 3 of 30 (10%) samples from Recife. Phylogenetic analysis based on a fragment of the VP1 and 3C gene revealed that the CA24v causing outbreaks in Brazil during the years 2003, 2004 and 2005 evolved from Asian isolates that had caused the South Korean outbreak of AHC during the summer of 2002. However, the 2009 outbreak of AHC in Pernambuco was originated from the reintroduction of a new CA24v strain that was circulating during 2007 in Asia, where CA24v outbreaks has been continuously reported since 1970. CONCLUSIONS: This study is the first phylogenetic analysis of AHC outbreaks caused by CA24v in Brazil. The results showed that Asian strains of CA24v were responsible for the outbreaks since 1987 and were independently introduced to Brazil in 2003 and 2009. Phylogenetic analysis of complete VP1 gene is a useful tool for studying the epidemiology of enteroviruses associated with outbreaks.

Due to the advanced stage of polio eradication, the possible role of non-polio enteroviruses (NPEVs) associated to acute flaccid paralysis (AFP) cases has been highlighted. In this study, we described epidemiological aspects of NPEVs infections associated to AFP and explore the viral genetic diversity, information still scarce in Brazil. From 2005 to 2017, 6707 stool samples were collected in the scope of the Brazilian Poliomyelitis Surveillance Program. NPEVs were isolated in 359 samples (5.3%) and 341 (94.9%) were genotyped. About 46 different NPEV types were identified with the following detection pattern EV-B?>?EV-A?>?EV-C. The major EV-types were CVA2, CV4, EV-A71, CVB3, CVB5, E6, E7, E11, CVA13 and EV-C99, which corresponds to 51.6% of the total. Uncommon types, such as CVA12, EV-90 and CVA11, were also identified. Different E6 genogroups were observed, prevailing the GenIII, despite periods of co-circulation, and replacement of genogroups along time. CVA2 sequences were classified as genotype C and data suggested its dispersion in South-American countries. CVA13 viruses belonged to cluster B and Venezuelan viruses composed a new putative cluster. This study provides extensive information on enterovirus diversity associated with AFP, reinforcing the need of tailoring current surveillance strategies to timely monitor emergence/re-emergence of NPEVs.

Abstract: The COVID-19 pandemic in Brazil has demonstrated an important public health impact, as has been observed in the world. In Brazil, the Amazon Region contributed with a large number of cases of COVID-19, especially in the beginning of the circulation of SARS-CoV-2 in the country. Thus, we describe the epidemiological profile of COVID-19 and the genetic diversity of SARS-CoV-2 strains circulating in the Amazon Region. We observe an extensive spread of virus in this Brazilian site. The data on sex, age and symptoms presented by the investigated individuals were similar to what has been observed worldwide. The genomic analysis of the viruses revealed important amino acid changes, including the D614G and the I33T in Spike and ORF6 proteins, respectively. The latter found in strains originating in Brazil. The phylogenetic analyzes demonstrated the circulation of the lineages B.1 and B.1.1, whose circulation in Brazil has already been previous reported. Our data reveals molecular epidemiology of SARS-CoV-2 in the Amazon Region. These findings also reinforce the importance of continuous genomic surveillance this virus with the aim of providing accurate and updated data to understand and map the transmission network of this agent in order to subsidize operational decisions in public health.

Ananindeua, PA, Brasil. In the current investigation, fecal material was obtained during a community?based longitudinal study conducted from 1983 to 1986. This study consisted of 71 children aged newborn to 3 years. A total of 216 samples from three of these children were screened by real?time quantitative polymerase chain reaction (RT?qPCR) for the presence of enteroviruses, and positive samples were serotyped by VP1 and VP3 sequencing of the viral genome. Of these, 12 (5.6%) came from symptomatic cases, and the remaining asymptomatic cases were collected fortnightly during the 3 years of study. A positivity of 63.4% (137/216) was obtained by RT?qPCR, with 58.3% (7/12) in relation to the symptomatic group and 63.7% (130/204) in relation to the asymptomatic group. The 137 positive samples were inoculated into the RD, HEp2C, and L20B cell lines, and the cytopathic effect was observed in 37.2% (51/137) samples. It was also possible to identify 40.9% (56/137), between isolated (n?=?46) and nonisolated (n?=?10). Enterovirus serotype diversity (n?=?25) was identified in this study, with the predominant species being B (80.3%), followed by C (16.1%) and A (3.6%). Cases of reinfection by different serotypes were also observed in the three children studied. Analyses involving different age groups of these minors confirmed that the most affected age was between 12 to 24 months, with a prevalence of 77.6% (52/67). The enterovirus (EV) circulated in the 3 years of research, showed peaks in some months, without defined seasonality. This study demonstrated a high circulation and serotype diversity of EV in fecal samples, collected over 30 years ago. This endorsed the evaluation of important points of the epidemiology of these viruses, such as the presence of coinfection and reinfection of the same individual by different circulating serotypes. Understanding the frequency and duration of EV infections is important in determining their association with persistent diarrhea.

In the past 5 years, however, CV-A6 has emerged as a new important pathogen worldwide, and more severe and extensive dermatologic presentations has been reported. OBJECTIVES: Identify the clinical spectrum for atypical HFMD and enterovirus serotypes in Bel?m, Par?, Amazon region of northern Brazil. STUDY DESIGN: A prospective ambulatory clinic-based surveillance conducted from January to June 2019, involving patients under 15 years with symptoms of HFMD. Stool, serum, oropharyngeal, and skin swab samples were analyzed. Real-time RT-PCR was performed to detect the viral genome of enteroviruses. Positive specimens were submitted to semi-nested PCR. Physical examinations and demographic data were recorded on a standardized form. RESULTS: 48 patients with symptoms of HFMD were included in the study and collected all samples according to protocol. Enteroviruses were detected in 83 % of patients. An atypical form of HFMD with vesiculobullous exanthema was present in 70 % (28/40); desquamation of the palms and soles detected in 90 % (36/40) and onychomadesis in 30 % (12/40) of patients. The serotype was identified in 22 patients, CV- A6 occurred in 81.8 % of them. CONCLUSION: This is the first ambulatory surveillance and virologic investigation involving HFMD performed in outpatients from Amazon region, Brazil. The detection of CV-A6 was related to atypical forms HFMD. Desquamation of the palms and soles and nail changes occurred with frequency, such as a late sequel in the HFMD disease.

A large outbreak (over 200,000 cases) of acute hemorrhagic conjunctivitis (AHC) took place in Brazil during the summer of 2017/2018, seven years after a nationwide epidemic, which occurred in 2011. To identify the etiological agent, 80 conjunctival swabs from patients with a clinical presentation suggestive of AHC were analyzed at the national enterovirus laboratory. Real-time RT-PCR for human enteroviruses was performed, and enterovirus RNA was detected in 91.25% (73/80) of the specimens. Twenty-nine swab fluids were used to inoculate cell cultures (RD and Hep2C), and 72.4% (21/29) yielded a cytopathic effect. Genotype IV coxsackievirus A24v (CV-A24v) was identified as the causative agent of the outbreak. Phylogenetic analysis based on the VP1 gene revealed that Brazilian isolates were genetically related to strains that caused an outbreak in French Guiana in 2017. Our results show the re-emergence of CV-A24v causing AHC outbreaks in Brazil between the end of 2017 and the beginning of 2018.

Genetic analysis of outbreak sequences will be essential for characterizing the diversity of circulating strains, identifying hotspots of virus transmission and guiding public health control. We report a case of encephalitis associated with Zika virus infection and reactivation of varicella-zoster virus in the central nervous system of a Brazilian child. This case raises the possibility that reactivation of the latent varicella-zoster virus may be a mechanism of neurological impairment induced by acquired Zika virus infection.

This study showed that laboratory markers of recent infection by dengue, Zika or chikungunya arboviruses were detected in the biological samples of approximately one-third of patients with encephalitis, myelitis, encephalomyelitis or Guillain-Barr? syndrome, in a surveillance programme in Piau? state, Brazil, between 2015-2016. Fever and myalgia had been associated with these cases. Since in non-tropical countries most infections or parainfectious diseases associated with the nervous system are attributed to herpesviruses, enteroviruses, and Campylobacter jejuni, the present findings indicate that in tropical countries, arboviruses may now play a more important role and reinforce the need for their surveillance and systematic investigation in the tropics.

The last case of paralytic poliomyelitis caused by wild poliovirus in Brazil occurred in 1989. The interruption of the indigenous poliovirus transmission was obtained through mass immunization campaigns to eligible children and an active epidemiological and laboratorial surveillance of all cases of acute flaccid paralysis (AFP) among children under 15 y of age. This paper describes and evaluates the performance of the AFP surveillance system in different geographic areas of Brazil between 2005 and 2014, using indicators recommended by WHO. AFP surveillance indicators as well as virological investigation of polio and non-polio enteroviruses in stool samples received in the laboratory were assessed from 2005-2014. During the period, 5463 cases of AFP were investigated. Of these, 55% were males and 45% were females. Those under 5 y of age represented 48% of all cases reported and investigated. AFP notification rate was within the acceptable values with mean value of 1.3 (North), 1.4 (Northeast), 1.1 (Southern), 1.0 (Southeast) and 1.4 (Midwest) cases of AFP per 100.000 population aged 15 y as well as the adequacy of fecal specimens received in the laboratory. Sabin- related polioviruses accounted for 1.7% of the isolates while, 6.7% were non-polio enterovirus with the values ranging from 5.0% to 8.9 %. No wild-type polio was found. The AFP epidemiological and laboratorial surveillance activities have been kept at appropriate levels in Brazil. These data are a very strong indication, which supports the status of country free of polio.

Este estudo apresenta uma elevada circula??o de enterov?rus em crian?as com gastroenterite aguda na Cidade de Bel?m, Estado do Par?, Brasil. Enteroviruses are transmitted by fecal-oral and respiratory routes and can be associated with sporadic cases and outbreaks of gastroenteritis. A causative agent in approximately 40 per cent of diarrheal cases still remains undiagnosed. Non-polio enteroviruses were detected in 46 (26 per cent) of 176 diarrhea stool specimens by both real-time RT-PCR (rRT-PCR) and semi-nested RT-PCR assays. This study presents a high circulation of enteroviruses in children with acute gastroenteritis in the City of Bel?m, Par? State, Brazil.

Abstract: Aseptic meningitis is one of the most common neurological disorders caused by enteroviruses. Among them, Echovirus 30 (E30) is described as the main etiological agent of many outbreaks and sporadic cases. This study investigated the genomic variability of E30 isolated from the cerebrospinal fluid (CSF) of aseptic meningitis cases that occurred from 1998 to 2008 in Brazil. Over a 10-year period (1998-2008), 302 non-polio enteroviruses were isolated, of which 177 were identified as E30 (58.6%). Phylogenetic analysis of the complete VP1 gene (876 nt) of 48 E30 isolates was performed and compared with additional Brazilian and foreign strains. E30 VP1 sequences segregated into three distinct major groups and seven subgroups, which were linked to the isolation year. In general, sequence divergence among E30 strains ranged from 0.2% to 13.8%. A common direct ancestor for this set of E30 strains was not defined. Brazilian isolates from Group I were related genetically to a 1997 USA isolate and both may have a common origin. Group III representatives showed close relationship to the 2007 Argentinean isolates. The present results complement existing data on the molecular characterization and genetic variability of E30 and may contribute to the understanding of the epidemiology of aseptic meningitis in the region.

Abstract: In view of the high circulation of migratory birds and the environmental and climatic conditions which favor the proliferation of arthropods, the Brazilian Pantanal is susceptible to circulation of arboviruses. However, the amount of data concerning arbovirus vectors in this area is scarce; therefore the aim of this study was to conduct a preliminary investigation of Culicidae species in the Nhecolândia Sub-region of South Pantanal, Brazil and their potential importance in the arbovirus transmission. A total of 3684 specimens of mosquitoes were captured, 1689 of which caught in the rainy season of 2007, were divided into 78 pools and submitted to viral isolation, Semi-Nested RT-PCR and Nested RT-PCR, with a view to identifying the most important arboviruses in Brazil. Simultaneously, 70 specimens of ticks found blood-feeding on horses were also submitted to the same virological assays. No virus was isolated and viral nucleic-acid detection by RT-PCR was also negative. Nevertheless, a total of 22 Culicidae species were identified, ten of which had previously been reported as vectors of important arboviruses. The diversity of species found blood-feeding on human and horse hosts together with the arboviruses circulation previously reported suggest that the Nhecolândia Sub-region of South Pantanal is an important area for arbovirus surveillance in Brazil. Regiões como o Pantanal brasileiro, que apresentam fatores como riqueza de fauna silvestre incluindo circulação de aves migratórias e condições ambientais e climáticas favoráveis à proliferação de artrópodes estão potencialmente sujeitas à circulação de arbovírus. Entretanto, poucos trabalhos foram realizados acerca da presença de arbovírus em potenciais vetores no Pantanal. Neste sentido o principal objetivo deste trabalho foi conduzir uma investigação preliminar para presença de arbovírus em amostragens de culicídeos capturados na Sub-região da Nhecolândia no Pantanal Sul. Um total de 3684 mosquitos foi capturado, dos quais 78 grupos compondo uma amostragem de 1789 espécimes foram submetidos às técnicas de isolamento viral e RT-PCR para os mais importantes arbovírus no Brasil. Simultaneamente, 70 espécimes de carrapatos capturados durante hematofagia em cavalos também foram submetidos à pesquisa viral. Não houve isolamento viral em nenhuma amostra analisada e os resultados de detecção de ácido nucléico viral foram também negativos. Entretanto, foram identificadas 22 espécies de culicídeos, dez das quais previamente reportadas como vetores de importantes arbovírus. A competência vetorial de espécies capturadas durante hematofagia em humanos e cavalos aliada ao relato prévio de circulação de arbovírus sugerem a Sub-região da Nhecolândia como uma importante área de vigilância para arbovírus no Centro-Oeste do Brasil.

Abstract: Background: The virological surveillance of acute flaccid paralysis (AFP) is a critical component of the initiative of the World Health Organization (WHO) to eradicate poliomyelitis worldwide. Furthermore rapid methods are needed either to detect or rule out the presence of polioviruses during the late stages of eradication, especially in polio-free areas. Objectives: The aim of this study was to evaluate a fast protocol combining one passage (5 days) in cell culture followed by RT-PCR and molecular typing in order to detect and type poliovirus (PV) and other enteroviruses associated with AFP cases. Study design: A total of 216 fecal suspensions from AFP suspected cases were tested by using this approach and compared with the WHO gold standard. Results: Using the WHO protocol enterovirus was detected in 12 out of the 216 AFP samples (5.55%) while with the proposed protocol enterovirus was detected in 15 out of the 216 AFP samples (6.94%). The additional positive samples detected by the proposed method were classified as non-polio enteroviruses (NPEV). Conclusions: The proposed protocol showed higher sensitivity than the WHO gold standard, reducing the entire process of identification and typing of the isolates from the typically 14–21 days to only ∼6–8 days.

Abstract: We have determined the complete nucleotide and the deduced amino acid sequences of Brazilian dengue virus type 3 (DENV-3) from a dengue case with fatal outcome, which occurred during an epidemic in the state of Rio de Janeiro, Brazil, in 2002. This constitutes the first complete genetic characterization of a Brazilian DENV-3 strain since its introduction into the country in 2001. DENV-3 was responsible for the most severe dengue epidemic in the state, based on the highest number of reported cases and on the severity of clinical manifestations and deaths reported.

O presente estudo teve como objetivo identificar e caracterizar HAdV em amostras de fezes provenientes de pacientes oriundos da Regi?o Norte do Brasil, no per?odo de 1998 a 2012, com quadro de paralisia fl?cida aguda, negativas para poliov?rus e enterov?rus n?o p?lio. No estudo, 19 amostras apresentaram efeito citop?tico caracter?stico de HAdV quando inoculadas nas linhagens HEp2-C e L20B. Essas amostras foram submetidas ? rea??o em cadeia mediada pela polimerase (PCR) em tempo real (qPCR) e sequenciamento nucleot?dico parcial do gene que codifica a prote?na h?xon. Do total de amostras, 13 foram confirmadas por qPCR, PCR e sequenciamento nucleot?dico. A an?lise gen?tica demonstrou a seguinte caracteriza??o viral: uma amostra como HAdV-F41; tr?s como HAdV-A31 e nove da esp?cie C, sendo tr?s HAdV-C5 e seis HAdV-C6. As amostras positivas foram provenientes dos Estados do Par? (6/19 ? 31,6%), Amazonas (6/19 ? 31,6%) e Acre (1/19 ? 5,2%). The human adenovirus (HAdV) cause a variety of infections such as acute respiratory, gastrointestinal, ocular, urinary tract and also serious neurological syndromes. The present study aimed to identify and characterize HAdV in stool samples of patients from the Northern region of Brazil in the period of 1998 to 2012, with acute flaccid paralysis, negative for poliovirus and enterovirus nonpolio. In this study, 19 samples showed a characteristic cytopathic effect of HAdV when inoculated in HEp2-C and L20B lineages. These samples were subjected by the polymerase chain reaction (PCR) in real-time (qPCR) and partial nucleotide sequencing of the hexon gene. Of the total samples, 13 were confirmed by qPCR, PCR and nucleotide sequencing. Genetic analysis showed the following viral characterization: one sample of HAdV-F41; three samples of HAdV-A31 and nine species C, three of HAdV-C5-C6 and six of HAdV. The positive samples were from the States of Par? (6/19 ? 31.6%), Amazonas (6/19 ?31.6%) and Acre (1/19 ? 5.2%). In relation to the age group, from 13 positive cases, 12 were children under 5 years old, representing 92.3% of cases. The results suggest a possible HAdV participation in the etiology of acute flaccid paralysis occurred in Northern Brazil.

Coxsackievirus A24v (CA24v) was identified as the etiologic agent, and partial sequences from the VP1 gene show that the isolates are closely related to CA24v viruses that previously caused AHC epidemics in South Korea and French Guiana.

Abstract: Outbreaks caused by vaccine-derived polioviruses are challenging the final eradication of paralytic poliomyelitis. Therefore, the surveillance of the acute flaccid paralysis cases based on poliovirus isolation and characterization remains an essential activity. Due to the use of trivalent oral poliovirus vaccine (OPV), mixtures containing more than one serotype of Sabin-related polioviruses are frequently isolated from clinical samples. Because each poliovirus isolate needs to be individually analyzed, we designed polymerase chain reaction primers that can selectively distinguish and amplify a genomic segment of the three Sabin-related poliovirus serotypes present in mixtures, thus, optimizing the diagnosis and providing prompt information to support epidemiologic actions.

Abstract: As in humans, sub-clinical infection by arboviruses in domestic animals is common; however, its detection only occurs during epizootics and the silent circulation of some arboviruses may remain undetected. The objective of the present paper was to assess the current circulation of arboviruses in the Nhecolândia sub-region of South Pantanal, Brazil. Sera from a total of 135 horses, of which 75 were immunized with bivalent vaccine composed of inactive Eastern equine encephalitis virus (EEEV) and Western equine encephalitis virus(WEEV) and 60 were unvaccinated, were submitted to thorough viral isolation, reverse transcriptase polymerase chain reaction (RT-PCR) and neutralization tests for Saint Louis encephalitis virus (SLEV), EEEV, WEEV and Mayaro virus (MAYV). No virus was isolated and viral nucleic-acid detection by RT-PCR was also negative. Nevertheless, the prevalence of neutralizing antibodies in horses older than seven months was 43.7% for SLEV in equines regardless of vaccine status, and 36.4% for WEEV and 47.7% for EEEV in unvaccinated horses. There was no evidence of MAYV infections. The serologic evidence of circulation of arboviruses responsible for equine and human encephalitis, without recent official reports of clinical infections in the area, suggests that the Nhecolândia sub-region in South Pantanal is an important area for detection of silent activity of arboviruses in Brazil.

Abstract: BACKGROUND: Coxsackievirus A24 variant (CA24v) is the most prevalent viral pathogen associated with acute hemorrhagic conjunctivitis (AHC) outbreaks. Sixteen years after its first outbreak in Brazil, this agent reemerged in 2003 in Brazil, spread to nearly all states and caused outbreaks until 2005. In 2009, a new outbreak occurred in the northeast region of the country. In this study, we performed a viral isolation in cell culture and characterized clinical samples collected from patients presenting symptoms during the outbreak of 2005 in Vitória, Espírito Santo State (ES) and the outbreak of 2009 in Recife, Pernambuco State (PE). We also performed a phylogenetic analysis of worldwide strains and all meaningful Brazilian isolates since 2003. METHODS AND FINDINGS: Sterile cotton swabs were used to collect eye discharges, and all 210 clinical samples were used to inoculate cell cultures. Cytopathic effects in HEp-2 cells were seen in 58 of 180 (32%) samples from Vitória and 3 of 30 (10%) samples from Recife. Phylogenetic analysis based on a fragment of the VP1 and 3C gene revealed that the CA24v causing outbreaks in Brazil during the years 2003, 2004 and 2005 evolved from Asian isolates that had caused the South Korean outbreak of AHC during the summer of 2002. However, the 2009 outbreak of AHC in Pernambuco was originated from the reintroduction of a new CA24v strain that was circulating during 2007 in Asia, where CA24v outbreaks has been continuously reported since 1970. CONCLUSIONS: This study is the first phylogenetic analysis of AHC outbreaks caused by CA24v in Brazil. The results showed that Asian strains of CA24v were responsible for the outbreaks since 1987 and were independently introduced to Brazil in 2003 and 2009. Phylogenetic analysis of complete VP1 gene is a useful tool for studying the epidemiology of enteroviruses associated with outbreaks.
